Background: Mental illness is a public health challenge disproportionately affecting rural Australians. GPs provide most of the mental health care, and they report increasing levels of burnout and unsustainable workload in the context of increased patient complexity. This may be more salient in rural settings characterised by resource constraints. In this paper, we use evaluation data from a GP psychiatry phone line established in Western Australia's Great Southern region in 2021 to describe GPs' perspectives on the service and reflect on how it may help alleviate rural GP workload. Methods: The sample was recruited among GPs practicing in the region. Data were collected through an online survey and semistructured interviews. Descriptive statistics were used to analyse the survey data. Interview data were subjected to thematic analysis; qualitative survey data were used for triangulation. Results: A total of 45 GPs completed the survey and 14 were interviewed. Interview data yielded three themes: the criticality of timeliness; the building blocks of confidence; and trust. GPs were highly satisfied with the service, and timeliness and trust were the characteristics underpinning its effectiveness. The service built GPs' confidence in managing mental health and alcohol and other drug use issues through strengthening knowledge and providing reassurance. Conclusions: Our results suggest that a telephone line operated by trusted, local psychiatrists with knowledge of the local mental health ecosystem of support can reduce rural GP workload through building confidence and strengthening personal agency, helping GPs navigate the ethical and clinical labyrinth of managing patient complexity in rural settings. GPs provide most of the mental health care and deal with an increasing number of patients with complex problems; this is more acute in rural areas where there are fewer services. This paper shows how a rural GP psychiatry phone line operated by local psychiatrists helps GPs manage their patients' mental health problems and may, in turn, reduce the workload rural GPs experience. Context: This paper introduces the special issue of Pacific Conservation Biology devoted to the natural history of Two Peoples Bay Nature Reserve on the south coast of Western Australia. Methods: This paper provides the background to the special issue. Key results: Two Peoples Bay Nature Reserve was gazetted in 1967 for the conservation of the native biota, including two species; the Djimaalap/noisy scrub-bird (Atrichornis clamosus) and Ngilgaitch/Gilbert's potoroo (Potorous gilbertii), both believed extinct for over 100 years before being rediscovered on the Reserve. The Reserve is 4774.7 ha in area, with wetlands, heathlands, granite outcrops, sand dunes, beaches, cliffs, and islands. Since it was established, mean annual rainfall has decreased by 16.8%, mean annual maximum temperature has increased by 0.2°C, and mean annual minimum temperature has increased by 0.7°C. Conclusions: The paper poses the question: what do the changes of drier winters, hotter summers, and more extreme weather events mean for managers of conservation areas such as Two Peoples Bay? Implications: Changing climate will pose problems for the managers of Two Peoples Bay Nature Reserve in ensuring the conservation of the Djimaalap/noisy scrub-bird and Ngilgaitch/Gilbert's potoroo. Context: This paper sets out the events leading to the establishment of Two Peoples Bay Nature Reserve. Methods: This paper is based on research into literature relating to the Albany region, government files and oral accounts relating to the area. Key results: Long known to the Noongar peoples, Two Peoples Bay was first mapped by Europeans in 1791 by Captain George Vancouver. The area was rapidly established as a base for the over-exploitation of natural resources. As the region became more developed and human population increased, recreational pressure on the area was such that the Western Australian Government surveyed Two Peoples Bay to establish a townsite. Just before the final surveys for the townsite were completed, the supposedly-extinct noisy scrub-bird (Atrichornis clamosus), was rediscovered there. National and international pressure on the WA Government from the conservation movement resulted in the reversal of the decision to develop a town in the area and led to the establishment of Two Peoples Bay Nature Reserve in 1967. Conclusions: The establishment of Two Peoples Bay Nature Reserve in the face of pressure for the development of a town on the bay, provides an interesting example of the needs of nature conservation over-riding real estate development. Implications: Although nature conservation values rarely stop real estate development, the Two Peoples Bay example demonstrates that nature conservation may prevail over real estate development, in this case involving the rediscovery of a species believed extinct. We describe the events leading to establishment of Two Peoples Bay Nature Reserve. The area was long known to Noongar peoples. Mapping by Europeans in 1791 was followed by sealing and whaling in the 19th century, and after World War II plans were prepared for a townsite. Millennia of evolutionary ecology have seen Australia become one of the driest and flattest continents on Earth—and in the process, home to more than 700 species of Eucalyptus. Colonial scientists named them using a binomial system, thereby overwriting local vernaculars that had persisted for tens of thousands of years. This paper traces the man commemorated in the Albany Blackbutt, Eucalyptus staeri , a tree unique to the Great Southern region of Western Australia, traditionally the land of the Menang Noongar people. Using a biographical lens, the paper examines the intersection of Western science and commerce in plant collection and naming, and the ways in which these processes exclude or discount Indigenous knowledge. The paper argues that a more holistic and inclusive historical interpretation of herbarium specimens of E. staeri is achieved by correcting and re-analysing information about the German settler after whom it is named, John Staer, while at the same time acknowledging the Noongar people's deep knowledge (kartijin) of plants that has been passed down over many thousands of years. Background: The study explored the experiences and perceptions of GPs regarding the management of chronic non-cancer pain (CNCP). Specifically, participants were asked to identify perceived enablers and barriers to CNCP care and how the care of patients with CNCP may be improved. Methods: The study utilised a qualitative descriptive methodology. General practice in Western Australia. The sample was purposive with 12 Australian GPs from predominantly metropolitan locations and with experience in managing CNCP. Semi structured interviews were conducted. Each interview was of 45–60 min duration. All interviews were audio recorded and transcribed using a secure transcription service. Thematic analysis developed themes inductively and deductively. Results: Themes emerged regarding: the importance of a holistic and personalised approach; the important role of a coordinating GP; the need for an evidence-based approach to opioid management; concerns relating to access to multidisciplinary services; the importance of clinician and patient education regarding CNCP; and an acknowledgement of the challenges for doctors and patients in managing CNCP. Conclusions: Currently, the management of CNCP in Australia is challenging. Notable challenges include: difficulties with continuity of patient care; challenges with patient expectations of treatment, in particular opioid medications; difficulty with access to the health services required to enable holistic care; and the need for improved pain education in the community. The breadth of these challenges suggests there is a need for supportive organisational and structural considerations in the healthcare system to enable optimal care of CNCP in the community. Access to specialist services for chronic non-cancer pain is problematic ins In Australia, resulting in general practitioners bearing much of the burden of care that requires a holistic and multidimensional approach. In this paper, 12 general practitioners in Western Australia described, through qualitative interviews, the challenges of caring for patients with chronic pain including fragmentation of care, difficulty with access to recommended services, mismatched patient and practitioner expectations and inadequate GP and community knowledge about chronic pain. Context: Australia is renowned for its climate variation, featuring years with drought and years with floods, which result in significant production and profit variability. Accordingly, to maximise profitability, dryland farming systems need to be dynamically managed in response to unfolding weather conditions. Aims: The aim of this study is to identify and quantify optimal tactical livestock management for different weather-years. Methods: This study employed a whole-farm optimisation model to analyse a representative mixed enterprise farm located in the Great Southern region of Western Australia. Using this model, we investigated the economic significance of five key livestock management tactics. These included timing of sheep sales, pasture-area adjustments, rotational grazing, crop grazing and sheep nutrition adjustments. Key results: The results showed that, on the modelled dryland mixed-enterprise farm in the Great Southern region of Western Australia, short-term adjustments to the overall farm strategy in response to unfolding weather conditions increased expected profit by approximately 16%. Each tactic boosted profit by between A$7704 and A$53,171. However, we outline several complexities that farmers must consider when implementing tactics. Conclusions: The financial gains from short-term tactical management highlighted their importance and farmers' need to develop and apply those skills. The tactical skills promote business resilience and adaptability in the face of climate uncertainties. Implications: The study highlighted the economic value of dynamic livestock management in response to climate variations, offering farmers in the Great Southern region the means to underpin profitable and sustainable farm practices. The ghost bat (Macroderma gigas) is Australia's largest echolocating bat. It is restricted to several disjunct populations in the north of the continent, including a population in the Pilbara region of Western Australia. In 2016 the ghost bat was listed as Vulnerable under Australian federal legislation, owing to declining numbers across many regional populations. The most severe threat to ghost bats in the Pilbara region is the destruction and disturbance of habitat due to mining operations, but disturbance to their roosts from other infrastructure developments and changes to and loss of foraging habitat also pose significant threats. A set of research priorities for ghost bats in the Pilbara was developed during a workshop attended by mining industry representatives, environmental consultants, scientists and government regulators. Five research priorities were identified: (1) identify and characterise critical diurnal roosts and foraging habitat; (2) improve knowledge of the distribution, movement and dispersal patterns of ghost bats in the region; (3) improve knowledge of population size, persistence and long-term trends; (4) better understand the cumulative, direct and indirect impacts of mining and other development activities; and (5) better understand the threats posed by fence entanglements, cane toads and feral cats. Ghost bats are under threat across northern Australia owing to activities such as mining and infrastructure development that destroys their roosts, vegetation clearing and grazing that diminishes their food resources, and the invasion of poisonous cane toads. Objectives: To assess whether adding clinical information and written discharge documentation variables improves prediction of paediatric 30-day same-hospital unplanned readmission compared with predictions based on administrative information alone. Methods: A retrospective matched case-control study audited the medical records of patients discharged from a tertiary paediatric hospital in Western Australia (WA) between January 2010 and December 2014. A random selection of 470 patients with unplanned readmissions (out of 3330) were matched to 470 patients without readmissions based on age, sex, and principal diagnosis at the index admission. Prediction utility of three groups of variables (administrative, administrative and clinical, and administrative, clinical and written discharge documentation) were assessed using standard logistic regression and machine learning. Results: Inclusion of written discharge documentation variables significantly improved prediction of readmission compared with models that used only administrative and/or clinical variables in standard logistic regression analysis (χ217 = 29.4, P = 0.03). Highest prediction accuracy was obtained using a gradient boosted tree model (C-statistic = 0.654), followed closely by random forest and elastic net modelling approaches. Variables highlighted as important for prediction included patients' social history (legal custody or patient was under the care of the Department for Child Protection), languages spoken other than English, completeness of nursing admission and discharge planning documentation, and timing of issuing discharge summary. Conclusions: The variables of significant social history, low English language proficiency, incomplete discharge documentation, and delay in issuing the discharge summary add value to prediction models. What is known about the topic?: Despite written discharge documentation playing a critical role in the continuity of care for paediatric patients, limited research has examined its association with, and ability to predict, unplanned hospital readmissions. Machine learning approaches have been applied to various health conditions and demonstrated improved predictive accuracy. However, few published studies have used machine learning to predict paediatric readmissions. What does this paper add?: This paper presents the findings of the first known study in Australia to assess and report that written discharge documentation and clinical information improves unplanned rehospitalisation prediction accuracy in a paediatric cohort compared with administrative data alone. It is also the first known published study to use machine learning for the prediction of paediatric same-hospital unplanned readmission in Australia. The results show improved predictive performance of the machine learning approach compared with standard logistic regression. Objective: Burden of disease studies measure the impact of disease at the population level;however, methods and data sources for estimates of prevalence vary. Using a selection of cardiovascular diseases, we aimed to describe the implications of using different disease models and linked administrative data on prevalence estimation within three Australian burden of disease studies. Methods: Three different methods (A = 2011 Australian Burden of Disease Study; B = 2015 Australian Burden of Disease Study; C = 2015 Western Australian Burden of Disease Study), which used linked data, were used to compare prevalence estimates of stroke, aortic aneurysm, rheumatic valvular heart disease (VHD) and non-rheumatic VHD. We applied these methods to 2015 Western Australian data, and calculated crude overall and age-specific prevalence for each condition. Results: Overall, Method C produced estimates of cardiovascular prevalence that were lower than the other methods, excluding non-rheumatic VHD. Prevalence of acute and chronic stroke was up to one-third higher in Method A and B compared to Method C. Aortic aneurysms had the largest change in prevalence, with Method A producing an eight-fold higher estimate compared to Method C, but Method B was 10–20% lower. Estimates of VHD varied dramatically, with an up to six-fold change in prevalence in Method C due to substantial changes to disease models and the use of linked data. Conclusions: Prevalence estimates require the best available data sources, updated disease models and constant review to inform government policy and health reform. Availability of nation-wide linked data will markedly improve future burden estimates. What is known about the topic? Accurate prevalence and subsequent burden estimates that reflect population-level disease burden are an important resource and are necessary for informing health policy decisions and resource distribution. Objective: Health research priorities are commonly identified and resourced by strategic leaders. The importance of recognising the expertise of clinician-researchers is being prioritised by a national funding shift towards applied research. There is a dearth of evidence regarding research priorities for maternity care in rural and remote health in Australia. This study aimed to develop an evidence-based consensus of maternity research priorities in regional, rural, and remote areas of Australia's largest rural health service (by land area) in Western Australia. Methods: A three-phased Delphi method was selected to achieve an interdisciplinary, evidence-based consensus on maternity research priorities within Western Australian Country Health Service. Results: Across three study phases, 432 participants responded. Representation was from seven regions and all stakeholder roles within the regions. Phase 1 included 173 responses yielding 53 concepts categorised under five domains. Phase 2 involved 161 participants who prioritised concepts under domains of (i) workforce and education; (ii) health equity; (iii) Aboriginal health; (iv) logistics and health systems; and (v) clinical. Phase 3 included 96 participants revealing 15 maternity research priorities with the top four ranked concepts: 'recruitment and retention of staff'; 'care for women and families with vulnerabilities', 'models of care offering continuity' and 'systems efficiencies'. Conclusions: The novel evidence provided in this study, in conjunction with a strong consensus on research priorities and an interdisciplinary approach, strengthens the findings of this study and amplifies the mandate of action without delay. Objective: To describe patterns of use of the available Government-subsidised mental health services among people living in Australian residential aged care facilities. Methods: A retrospective population-based trend analysis was conducted, including all non-Indigenous people living in an Australian facility between 2012 and 2017. Adjusted incidence proportions and trends were estimated for four groups of mental health services. Results: The use of Medicare-subsidised mental health services was very low overall. The proportion of residents who accessed primary care mental health services increased from 1.3% in 2012/2013 to 2.4% in 2016/2017, while psychiatry service use increased from 1.9 to 2.3%. Claims for clinical psychology increased from 0.18 to 0.26%, and claims for a registered psychologist, occupational therapist or social worker rose from 0.45 to 1.2%. People with dementia were less likely than people without dementia to access all services aside from psychiatry services. Conclusions: Less than 3% of residents accessed funding subsidies for mental health services and people with dementia experienced pronounced barriers to service access. Mental health care is a pillar of the publicly-funded health system in Australia, and low use of these services among aged care residents indicates a need for organisational and policy changes to improve access. What is known about the topic? People living in residential aged care facilities report very high rates of mental health conditions, including depression and anxiety. What does this paper add? We demonstrate very low use (<3%) of Government-funded mental health services among people living in residential aged care facilities in Australia, with only small increases in use over time. Objective: This study assessed the effect of the frequency of general practitioner (GP) visitation in the 12 months before a 21% consumer copayment increase in the Pharmaceutical Benefits Scheme (PBS; January 2005) on the reduction or discontinuation of statin dispensing for tertiary prevention. Methods: The study used routinely collected, whole-population linked PBS, Medicare, mortality and hospital data from Western Australia. From 2004 to 2005, individuals were classified as having discontinued, reduced or continued their use of statins in the first six months of 2005 following the 21% consumer copayment increase on 1 January 2005. The frequency of GP visits was calculated in 2004 from Medicare data. Multivariate logistic regression models were used to determine the association between GP visits and statin use following the copayment increase. Results: In December 2004, there were 22 495 stable statin users for tertiary prevention of prior coronary heart disease, prior stroke or prior coronary artery revascularisation procedure. Following the copayment increase, patients either discontinued (3%), reduced (12%) or continued (85%) their statins. Individuals who visited a GP three or more times in 2004 were 47% less likely to discontinue their statins in 2005 than people attending only once. Subgroup analysis showed the effect was apparent in men, and long-term or new statin users. The frequency of GP visits did not affect the proportion of patients reducing their statin therapy. Conclusions: Patients who visited their GP at least three times per year had a lower risk of ceasing their statins in the year following the copayment increase. GPs can help patients maintain treatment following rises in medicines costs. Objective: This study investigated the link between socioeconomic status and unplanned dental presentations at the Perth Children's Hospital (PCH), as well as the link between the socioeconomic status of unplanned dental patients and any previous admissions to the PCH. Methods: Records of 351 unplanned visits to the PCH were collected, including reason for attendance (infection, trauma, other), the patient's residential location (suburb) and the history of any previous presentations at the PCH. The socioeconomic status of each patient was based on the Index of Relative Socio-Economic Disadvantage, divided into quintiles. Geographic information systems (GIS) were used to spatially map the residential locations of the patients with unplanned dental presentations. QGIS was used to map and geocode the data. Analysis of variance and Chi-squared tests were used to determine associations between subgroups and other variables. Results: 'Unplanned dental presentation' in this study refers to patients who present without an appointment, including by referral from the emergency department of the PCH or outside the PCH. Approximately two-thirds of unplanned dental presentation among patients from low socioeconomic groups were for dental infection, whereas the major reason for presentation among patients from higher socioeconomic groups was trauma. More than half the patients in low socioeconomic groups had at least one previous presentation at the PCH due to other medical issues. Conclusion: Children from low socioeconomic groups, or from outside of Perth, were more likely to present with dental infections, which are mostly preventable at the primary care level; these patients often presented a more significant burden to the health system. Public health interventions should aim to promote preventive oral health care, especially for children from low socioeconomic groups. Objective: To date, there has been little research that has comprehensively analysed dental treatment under general anaesthesia (DGA) at Perth Children's Hospital (PCH) for dental emergencies (dental pain and sepsis). This cross-sectional descriptive analysis of the PCH dental department analysed the demographic of children admitted and the type of treatment used, and assessed the cost. Methods: This was a retrospective descriptive study analysing the dental records of patients ranging from 2 to 16 years of age at PCH in Western Australia. Of the 310 cases randomly selected from the 2018–19 study period, 202 were admitted for DGA. Two outcome measures were derived: cumulative count of treatment mix and cost analysis. Results: The mean (±s.d.) age at the time of admission was 6.2 ± 2.6 years and the mean (±s.d.) decayed (d/D), missing, (m/M) and filled (f/F) teeth (dmft/DMFT) was 2.1 ± 0.8. Of the 429 teeth affected, 282 were molars. Treatments were grouped therapeutically; of the 856 treatments performed, 465 were extractions (54%). The total cost, consisting of direct and indirect costs, was A$313 823, and equated to an approximate mean (±s.d.) of A$1554 ± 109 per case. Conclusion: Untreated dental caries was the most common cause for hospital admission. Most cases presenting at the emergency department were young children (<7 years old) who underwent extractions under DGA. What is known about the topic?: The Child Dental Benefits Schedule (CDBS) was introduced to improve access and affordability to oral health care for children without private dental insurance. However, a significant number of children are still being admitted to hospital for emergency dental treatment. Perennial pastures are the dominant feedbase in many regions of the world, and offer several advantages when compared with an annual pasture system. In Western Australia (WA) there has been a concerted effort over seven decades to develop new perennial pasture options and expand the adoption of suitable species. The agricultural region of WA (i.e. south-western Australia) is characterised by a Mediterranean climate where the 5–7 month summer drought has proved a considerable challenge with only a small number of the many promising species being adopted commercially. Research, development, and extension have covered a wide range of herbaceous perennial legumes, leguminous and native shrubs, herbs, and temperate and warm season grasses. This paper reviews the literature to determine whether a perennial pasture must satisfy the following criteria to be successful and widely adopted in south-western Australia: (i) sourced from a similar Mediterranean environment and adapted to the target soils; (ii) have a relative advantage over the annual-based system it replaces; (iii) a robust management package; and (iv) a viable seed supply. The findings of this review highlight that perennial pastures must indeed satisfy multiple criteria to be commercially successful. Notably, the requirement for the source of the germplasm to have a good match between climate and soils is less clear because some of the commercially successful species come from diverse environments. We conclude with some key learnings for future perennial pasture development as climate change intensifies the research challenge and the drive for producers to adapt. In Western Australia, there has been a concerted effort to develop new perennial pasture options. Research, development, and extension have covered a range of herbaceous perennial legumes, leguminous and native shrubs, herbs and temperate and warm season grasses. Overall, this search has proved to be a considerable challenge in a Mediterranean environment with many promising species having little or no commercial impact, and only a small number of species have had widespread commercial adoption. Objective. Capital is an essential enabler of contemporary public hospital services funding hospital buildings, medical equipment, information technology and communications. Capital investment is best understood within the context of the services it is designed and funded to facilitate. The aim of the present study was to explore the information on capital investment in Australian public hospitals and the relationship between investment and acute care service delivery in the context of efficient pricing for hospital services. Methods. This paper examines the investment in Australian public hospitals relative to the growth in recurrent hospital costs since 2000-01 drawing from the available data, the grey literature and the reports of six major reviews of hospital services in Australia since 2004. Results. Although the average annual capital investment over the decade from 2000-01 represents 7.1% of recurrent expenditure on hospitals, the most recent estimate of the cost of capital consumed delivering services is 9% per annum. Five of six major inquiries into health care delivery required increased capital funding to bring clinical service delivery to an acceptable standard. The sixth inquiry lamented the quality of information on capital for public hospitals. In 2012-13, capital investment was equivalent to 6.2% of recurrent expenditure, 31% lower than the cost of capital consumed in that year. Conclusions. Capital is a vital enabler of hospital service delivery and innovation, but there is a poor alignment between the available information on the capital investment in public hospitals and contemporary clinical requirements. The policy to have capital included in activity-based payments for hospital services necessitates an accurate value for capital at the diagnosis-related group (DRG) level relevant to contemporary clinical care, rather than the replacement value of the asset stock. Objective: To determine the extent of medical and non-medical out-of-pocket expenses (OOPE) among regional/rural and outer metropolitan Western Australian patients diagnosed with cancer, and the factors associated with higher costs. Methods: Cross-sectional data were collected from adult patients living in four regional/rural areas and two outer metropolitan regions in Western Australia who had been diagnosed with breast, prostate, colorectal or lung cancer. Consenting participants were mailed demographic and financial questionnaires, and requested to report all OOPE related to their cancer treatment. Results: The median total OOPE reported by 308 regional/rural participants and 119 outer metropolitan participants were A$1518 (interquartile range (IQR): A$581–A$3769) and A$2855 (IQR: A$958–A$7142) respectively. Participants most likely to experience higher total OOPE were younger than 65 years of age, male, resided in the outer metropolitan area, worked prior to diagnosis, had private health insurance, were in a relationship, and underwent surgery. Multivariate analysis of regional/rural participants revealed that receiving care at a rural cancer centre was associated with significantly lower non-medical OOPE (estimated mean A$805, 95% confidence interval (CI): A$735–A$875, P = 0.038; compared with other rural participants (A$1347, 95% CI: A$743–A$1951, P < 0.001)). Conclusion: The cancer patients who participated in this study experienced variation in OOPE, with outer metropolitan participants reporting higher OOPE compared with their regional/rural counterparts. There is a need for cost transparency and access to care close to home, so that patients can make informed choices about where to receive their care. In Western Australia, a cancer centre providing comprehensive cancer care was established in the second most populous region to ameliorate the high costs for travel and accommodation that regional cancer patients are known to experience. What does this paper add?: This study is unique because it collected detailed cost information from patients and reports on the OOPE of regional/rural and outer metropolitan Western Australian patients receiving care for one of the four most common cancers; it therefore offers novel insight into the experiences of these groups. This study demonstrates that outer metropolitan cancer patients are experiencing much higher OOPE compared with regional/rural cancer patients. Additionally, regional/rural study participants who accessed a Regional Cancer Centre experienced significantly lower non-medical OOPE, compared with regional/rural study participants receiving care elsewhere. Objective: The aim of this study was to identify the hepatitis C treatment intentions of Aboriginal people living with hepatitis C virus (HCV) in Western Australia. Methods: This study used a mixed-methods design. In the cross-sectional survey, 123 Aboriginal people who inject drugs and self-report as living with hepatitis C completed a purpose-designed questionnaire. In the qualitative phase, 10 participants were interviewed about the factors influencing their future intentions to undertake hepatitis C treatment. Results: Analysis of the survey data revealed significant associations between an intention to undertake hepatitis C treatment and support, community attachment, stable housing and stigma. In addition, there was a high overall level of expressed intention to undertake HCV treatment, with 54% of participants responding positively. Analysis of the qualitative data supported quantitative findings, revealing concerns about stigma, lack of social support and unstable housing as factors affecting the intention to undertake hepatitis C treatment. Conclusion: This mixed methods study with Aboriginal people living with self-reported HCV indicates interventions focused on reducing stigma and unstable housing could positively affect hepatitis C treatment intentions. These findings have implications for developing holistic programs to promote and support people on hepatitis C treatment. Objective: The aim of this study was to explore Western Australian cancer patients' experiences of out-of-pocket expenses (OOPE) during diagnosis and cancer treatment using a phenomenological approach. Methods: Semi-structured interviews were conducted with a purposive convenience sample of 40 Western Australian cancer patients diagnosed with breast, lung, prostate or colorectal cancer. Participants were asked about the impact of their diagnosis, the associated costs and their experience within the health system. Data were analysed using thematic content analysis. Results: Three key themes influencing participant OOPE experiences were identified: (1) personal circumstances; (2) communication with health providers; and (3) coping strategies. Despite Australia's public healthcare system, several participants found the costs affected their financial security and resorted to coping strategies including medication rationing and restrictive household budgeting. The key themes had a complex and interrelated effect on patient OOPE experiences and were used to adapt Carrera et al. 's model of economic consequences of cancer treatment on the patient and patient coping to describe these relationships in a mixed healthcare system. Conclusion: Organised efforts must be implemented to mitigate maladaptive coping strategies being used by cancer patients: (1) health providers should seek informed financial consent from patients before commencing treatment; and (2) financial aid and support schemes for cancer patients should be reviewed to ensure they are delivered equitably. Carnaby's cockatoo, Calyptorhynchus latirostris , is endemic to south-western Australia. It has undergone a major decline in range and abundance as a result of extensive removal of breeding and foraging habitat. It is now classified as endangered by the State of Western Australia, the Australian government, and internationally. In order to plan effective conservation management for the species it is important to assess the success of breeding populations throughout its range. In this paper we examine the efficacy of using the relationship between growth in the length of a nestling's folded left wing and body mass to assess nestling condition, and examine known breeding failure in relation to nestling condition in two breeding populations: one at Coomallo Creek (studied from 1970–2017), and the other at Manmanning (1969–76). Results demonstrated that the lighter the nestlings, the higher the rate of breeding failure. Data from the Coomallo Creek population were used to prepare a table of nestling folded left wing length and body mass as a benchmark for assessing nestling condition in 10 other breeding populations, based on data collected from 1970 to the present. Following extensive clearing that removed foraging and breeding habitat, two of the 10 populations produced nestlings that were significantly lighter than the benchmark, and both populations subsequently declined to extinction. The commencement of egg-laying each season at Coomallo Creek was strongly influenced by total rainfall in the first half of autumn. The length of the egg-laying period between 1970–76 and 2009–17 increased by 5.2 weeks (40%). This increase was related to changes in rainfall and temperature over more than four decades. Despite the lengthening of the egg-laying period, nestling condition was unaffected, suggesting that, at least in the short term, the Carnaby's cockatoo population at Coomallo Creek is coping with the effects of climate change. Many agricultural soils are naturally acidic, and agricultural production can acidify soil through processes such as nitrogen (N) fixation by legumes and application of N fertiliser. This means that decisions about mitigation of soil acidity (e.g. through application of lime), crop rotation and N fertiliser application are interdependent. This paper presents a dynamic model to determine jointly the optimal lime application strategies and N application rates in a rainfed cropping system in Western Australia. The model accounts for two crop rotations (with and without a legume break crop), for the acid tolerance of different crop types, and for differences in the acidifying effect of different N fertilisers. Results show that liming is a profitable strategy to treat acidic soils in the study region, but that there are interactions between N and acidity management. Choice of fertiliser affects optimal lime rates substantially, with the use of a more acidifying ammonium-based fertiliser leading to higher lime rates. The optimal liming strategy is also sensitive to inclusion of a legume crop in the rotation, because its fixed N can be less acidifying than fertiliser, and it allows a reduction in fertiliser rates. Higher rainfall zones have greater N leaching, which contributes to a higher optimal rate of lime. We find that injection of lime into the subsoil increases profit. Optimal lime rates in the absence of subsoil incorporation are higher than usual current practice, although the economic gains from increasing rates are small. [ABSTRACT FROM AUTHOR]

Objective: The aim of this study was to examine the characteristics and prevalence of all-cause unplanned hospital readmissions at a tertiary paediatric hospital in Western Australia from 2010 to 2014. Methods: A retrospective cohort descriptive study was conducted. Unplanned hospital readmission was identified using both 28- and 30-day measurements from discharge date of an index hospital admission to the subsequent related unplanned admission date. This allowed international comparison. Results: In all, 73 132 patients with 134 314 discharges were identified. During the 5-year period, 4070 discharges (3.03%) and 3330 patients (4.55%) were identified as 30-day unplanned hospital readmissions. There were minimal differences in the rate of readmissions on Days 28, 29 and 30 (0.2%). More than 50% of readmissions were identified as a 5-day readmission. Nearly all readmissions for croup and epiglottitis occurred by Day 5; those for acute bronchiolitis and obstructive sleep apnoea requiring tonsillectomy and/or adenoidectomy occurred by Day 15 and those for acute appendicitis and abdominal and pelvic pain occurred by Day 30. Conclusion: This study highlights the variability in the distribution of time intervals from discharge to readmission among diagnoses, suggesting the commonly used 28- or 30-day readmission measurement requires review. It is crucial to establish an appropriate measurement for specific paediatric conditions related to readmissions for the accurate determination of the prevalence and actual costs associated with readmissions. However, the 30-day measurement is commonly used in the literature. Only five Australian studies were identified with a focus on readmissions associated with specific paediatric health conditions. The mirid subfamily Orthotylinae is hyperdiverse in Australia but poorly described; this work is part of a series of papers on the documentation of this fauna. Two new species of the Australian endemic plant bug genus Myrtlemiris Cheng, Mututantri & Cassis (Heteroptera: Miridae: Orthotylinae: Orthotylini) are described as new to science: Myrtlemiris kararensis , sp. nov., and M. lochada , sp. nov. The new Australian genus and species Neomyrtlemiris picta gen. nov. et sp. nov. are also described. Myrtlemiris is analysed phylogenetically using 32 morphology-based characters, as well as molecular alignments (COI , 16S rRNA, 18S rRNA and 28S rRNA ; including 1958 base pairs) with the genus found to be monophyletic based on molecular, morphological and combined analyses. Neomyrtlemiris is the sister to Myrtlemiris. Host plant association analysis demonstrated constraints to the myrtaceous tribe Chamelaucieae, with Malleostemon and Calytrix recovered as ancestral host plants. Ancestral state reconstructions were carried out on male genitalic structures, demonstrating the phylogenetic value of endosomal spicules. This work demonstrates that the Orthotylinae are a rich component of Australia's biodiversity, particularly in the south-west of Western Australia. This work continues documentation of the Australian mirid tribe Orthotylinae, a fauna that was virtually undescribed 20 years ago. Two new species of the genus of Myrtlemiris from Western Australia, and a new genus and new species (Neomyrtlemiris picta), all of which were collected during the Bush Blitz program, are described. This work also includes a combined molecular and morphological phylogeny confirming the monophyly of Myrtlemiris , as well as ancestral state reconstruction of male genitalic characters. Sheep stocking rate influences farm profit significantly; however determining the optimal stocking rate is a difficult task. In this paper, we address this challenge through three main steps. First, we review the definition of stocking rate; second, we examine prior research relevant to the review topic and highlight the factors that need to be considered when determining the optimal stocking rate; and third, we make recommendations for improvements in research on establishing the optimal sheep stocking rate. Inconsistency in the definition of stocking rate can lead to miscommunication among researchers, advisers and farmers. If 10 dry sheep equivalents (DSE)/ha is optimal for one flock, it may not be optimal for another flock because the DSE measure does not fully capture the nuances of different patterns of nutritional requirements among sheep classes and feed availabilities and their respective prices and costs. The optimal stocking rate occurs when the marginal economic benefit of an additional animal equals its marginal cost. Determining this point requires an understanding of the quantity and quality of feed available throughout the year, the optimal liveweight profile throughout the year, the impact of seasonal variation, the impact of labour availability, the cost of alternative feeds, prices of livestock and livestock products, the risk preferences of the farmer, and any emission policies relating to greenhouse gases. Farmers tend to use their own judgement to set their stocking rates, with the aim of maximising utility. However, the complexities listed make it a challenging task. Thus, researchers have used various simulation and programming models to aid decision-making over optimal stocking rates, but most farmers continue to rely on their own personal judgement. Moreover, often a focus of this modelling is for sheep systems in eastern Australia. Generalising this research across Australia is difficult due to differences in climatic conditions and markets across Australia. Often farmers are unaware of the profits they are foregoing when choosing either an overly conservative or excessive stocking rate. Our research has shown that foregone income of up to AUD50 per hectare can occur when a stocking rate 30% below or above the optimum is selected. Thus, despite the complexities that underpin the stocking rate decision, we believe that there are potential rewards from further research on the optimisation of stocking rates. Background: In Western Australia, the issue of bushfires (wildfires) poses a persistent health risk to both volunteer and career forestry firefighters, populations that have been historically understudied. Aims: This descriptive qualitative study aimed to examine firefighters' level of understanding concerning hazardous exposures, their Personal Protective Equipment (PPE) protocols and their decontamination practices. Methods: Guided by the socioecological model, 23 semi-structured interviews were conducted with firefighters representing both volunteer and forestry sectors. Key results: Results indicate a gap in knowledge regarding the health risks associated with bushfire-generated smoke and contaminants among volunteer firefighters. Forestry firefighters, conversely, showed a greater awareness of these risks. Social dynamics, such as peer pressure, were identified as historical deterrents to PPE use, although recent trends indicate positive change. Forestry firefighters also highlighted systemic shortcomings, including inadequate provision of respiratory protection and lack of decontamination facilities for cleaning themselves and their PPE. Conclusion: This study underscores the critical need for educational initiatives to address these gaps in understanding, along with organisational reforms to promote a culture encouraging PPE use and to provide essential resources for effective decontamination. Implications: This research emphasises that a multi-level approach is essential for safeguarding the health of firefighters in bushfire-prone environments. Volunteer firefighters in Western Australia have a significant knowledge gap about of bushfire smoke health risks and protection. Forestry firefighters are more informed but still face issues like inadequate respiratory protection and lack of decontamination facilities. Background. This study examined postgraduate work after an undergraduate clinical year spent in the Rural Clinical School of Western Australia (RCSWA), compared with 6 weeks Rural Undergraduate Support and Coordination (RUSC)-funded rural experience in a 6-year undergraduate medical course. Rural background, sex and Rural Australian Medical Undergraduate Scholarship (RAMUS)-holding were taken into account. Methods. University of Western Australia undergraduate data were linked by hand with postgraduate placements to provide a comprehensive dataset on the rural exposure history of junior medical practitioners working in Western Australia between 2004 and 2007. Results. Participation in the RCSWA program was associated with significantly more postgraduate year one rural work than RUSC placement alone (OR=1.5, CI 0.97-2.38). The RCSWA workforce effect increased at postgraduate year two (OR=3.0, CI 1.6484 to 5.5935 relative to RUSC). Rural-origin practitioners who chose the RCSWA program were more likely than other rural-origin practitioners to take rural rotations in both postgraduate years. RAMUS holders' choice in relation to the RCSWA program predicted later rural work. There were no effects of sex. Conclusions. Rural initiatives, in particular the Rural Clinical School program, are associated with postgraduate rural choices. The real impact of these data rely on the translation of early postgraduate choices into long-term work commitments. What is known about the topic? Specific training of rural graduates is strongly related to subsequent rural workforce. In addition, rural educational placements affect medical students' intentions to practise rurally. Retrospective data from rural relative to urban general practitioners link rural background, rural undergraduate experience and rural postgraduate experience with rural work. Much of the Australian prospective outcomes research has looked at workforce intentions of graduates, or the workforce outcomes of small graduate cohorts of Rural Clinical Schools. This paper followed nearly 500 graduates by name through to their PGY1 and PGY2 workforce decisions. As this cohort comprised 80% of three completely graduated MBBS years, these data validly represent the outcome of a medical school which is comparable to all Australian medical schools in its RUSC, Rural Clinical School and RAMUS recipients. The paper provides the first peer-reviewed outcomes data for the RAMUS scholarship program. What are the implications for practitioners? Rural supervision of medical students is an effective way to recruit new workforce. The longer students are supervised in rural settings, the better. But undergraduate programs need to be linked with postgraduate initiatives that take graduates of medical student initiatives into vocational programs able to deliver a trained rural workforce. The prevailing disparities in Aboriginal health in Australia are a sobering reminder of failed health reforms, compounded by inadequate attention to the social determinants shaping health and well-being. Discourse around health reform often focuses on the role of government, health professionals and health institutions. However, not-for-profit health organisations are also playing an increasing role in health policy, research and program delivery across the prevention to treatment spectrum. This paper describes the journey of the National Heart Foundation of Australia in West Australia (Heart Foundation WA hereafter) with Aboriginal employees and the Aboriginal community in taking a more proactive role in reducing Aboriginal health disparities, focusing in particular on lessons learnt that are applicable to other non-government organisations. Although the Heart Foundation WA has employed and worked with Aboriginal people and has long identified the Aboriginal community as a priority population, recent years have seen greater embedding of this within its organisational culture, governance, policies and programs. In turn, this has shaped the organisation's response to external health reforms and issues. Responses have included the development of an action plan to eliminate disparities of cardiovascular care in the hospital system, and collaboration and engagement with health professional groups involved in delivery of care to Aboriginal people. Examples of governance measures are also described in this paper. Although strategies and the lessons learnt have been in the context of cardiovascular health disparities, they are applicable to other organisations across the health sector. Moreover, the most powerful lesson learnt is universal in its relevance; individual programs, policies and reforms are more likely to succeed when they are underpinned by whole of organisation ownership and internalisation of the need to redress disparities in health. Context. Historical application of phosphorus (P) fertilisers has enabled grazing agriculture in south-west Western Australia but has led to excessive soil P levels that pose a eutrophication risk. However, adoption of critical soil-test P values for pastures determined from the Better Fertiliser Decisions for Pastures (BFDP) meta-analysis of historical Australian trials has been poor because of perceived lack of relevance to the soils and contemporary pasture species in the region. Aims. We aimed to validate critical Colwell P values from the BFDP project for soils and contemporary pasture species of south-west Western Australia. Methods. Fifty P-rate response trials were performed with contemporary pasture species, and the results compared with BFDP. Key results. Trial results were consistent with BFDP and fell within 95% prediction intervals of response calibrations of Colwell P and relative yield pairs for different P buffering index (PBI) ranges. Soils with PBI <15 required lower critical Colwell P values. Responsiveness to P application was 8% (±4%), consistent with a 5% response expected for Colwell P levels supporting 95% relative yield. Application of nitrogen, potassium and sulfur significantly increased yield while increasing basal cover of annual ryegrass (Lolium multiflorum) and reducing that of clover (Trifolium spp.) in the sward. Response calibration coefficients and critical Colwell P values before and after addition of the trial data to BFDP showed a strong correlation (r2 >0.99), although critical values were slightly lower for lower soil PBI. Conclusions. Critical Colwell P values from BFDP are relevant and applicable to soils and contemporary pasture species of south-west Western Australia. Implications. Agronomic advice and application of P should be based on interpretation of Colwell P values, with P applied only when levels are below the critical value. Pasture composition should also be considered when interpreting Colwell P values. This approach minimises risks of P loss and improves economic outcomes for growers. [ABSTRACT FROM AUTHOR]

Context. Australian grain producers may need to report their farm greenhouse emissions. Accordingly, nitrogen fertiliser application strategies will need to include consideration of their environmental as well as economic impacts. Aims. We aim to identify the nitrogen application strategies suited to dryland cropping in Western Australia that are highly profitable and that generate lower emissions. Methods. Simulation modelling is used to examine the gross margins and emissions associated with four broadly different nitrogen strategies at 14 locations in the grainbelt of Western Australia for different frequencies of cropping. Key results. Strategies that generate high gross margins and moderate emissions often focus on maximising the gross margin of crop production, and apply a decile 5 view of unfolding seasonal conditions. A similarly useful strategy applies nitrogen in a fixed ratio where a tonne of expected cereal receives 45 units of nitrogen from various sources, and a tonne of expected canola receives 70 units of nitrogen from various sources. Where a farmer prefers to apply a constant rate of nitrogen, then exceedingly high or low rates of application should mostly be avoided, either for economic or for environmental reasons, with the better option at many locations being to apply 50 or 75 kg N/crop ha. Conclusions. A few preferred nitrogen application strategies are suggested to be applicable to dryland cropping in the study region. The strategies achieve high profits and generate moderate or low emissions. Implications. Worldwide predation by feral cats has caused severe declines in many animal species. Prior to extinction, threatening processes typically alter species behaviour, habitat use, survival and reproductive rates. By monitoring these parameters, we may be able to determine if feral cats are a significant threatening process to a species prior to extinction. We aimed to test whether feral cats are negatively affecting the population size, morphometrics and movement patterns of mulgaras (Dasycercus blythi) on the Matuwa Indigenous Protected Area, in the arid rangelands of Western Australia. We hypothesised that mulgaras, which inhabit a cat-free environment, would have a larger home range and denser population, and be in better condition than those that inhabited an adjacent area with a suppressed density of feral cats. We trapped (n = 64), and radio-tracked (n = 19) mulgara inside and outside an I 100 ha cat-free fenced enclosure on five occasions in 2012. Treatment (i.e. inside or outside the cat-free enclosure) was a significant variable for explaining variation in mulgara density. There was no significant difference in morphometrics or the home ranges of mulgara between the sexes or treatments. Head length, pes length and weight, as well as tail width and length, were significantly different between sexes and over time, suggesting that seasonal variation influences mulgara populations more than low levels of cat predation. We update the list of indigenous mammals known from each of Western Australia's mainland bioregions since European settlement, assess their regional status in terms of extent of occurrence, then compare these regional status lists and derived faunal attrition values with those published in 2006. Seven bioregions show substantially more faunal attrition since 2006, including the Avon Wheatbelt, Victoria Bonaparte and Northern Kimberley. The status of 27 species has worsened in at least one bioregion, an average of 1.7 species per bioregion across the 26 bioregions. We also examine potential explanatory factors associated with faunal attrition across the bioregions using beta regression and AIC-based model selection. Faunal attrition was strongly associated with low average annual rainfall, a low proportion of species that shelter in rockpile habitat, high environmental change, a high proportion of species in the 'Critical Weight Range' and, to a lesser degree, a high proportion of omnivorous species. Background. Phos-Chek WD881A is a short-term retardant used by fire-fighters in Western Australia to suppress and control the movement of fire across the landscape. It is currently applied at a working concentration of 0.1-1%. Aim. Our objective was to assess and quantify the impact of the suppressant on seed germination and seedling emergence across eight native and two weed species commonly found in Eucalyptus wandoo woodland. Methods. Seeds were exposed to five Phos-Chek concentrations, from 0 to 10% (v/v), in a germination trial in Petri dishes, and three concentrations of 0, 0.1 and 1% (v/v) in a seedling emergence trial. Key findings. Increasing concentrations of Phos-Chek both delayed and reduced germination and emergence for all species except Acacia saligna. The sensitivity to Phos-Chek varied among the tested species. Conclusions. Phos-Chek had a significant impact on the germination and emergence of native and invasive species, with irreversible damage to seed viability in one taxa (Allocasuarina humilis). Implications. It is recommended that applications of Phos-Chek foam remain closer to the minimum recommended concentration (0.1% v/v) to reduce adverse effects on the recruitment of sensitive species, particularly during dry autumns when leaching of this chemical is likely to be limited. The stripe-faced dunnart (Sminthopsis macroura) is one of the most widespread dasyurids in Australia, occurring mostly in semiarid and arid habitats. It is not known to inhabit coastal regions of southern Australia, and no records have previously been recorded from latitudes greater than 28.5°S in Western Australia. Following the capture of an individual south of the known species range provisionally identified based on external morphology as S. macroura , we used DNA and craniodental morphology to corroborate the specimen's identification, providing a record of the species at Eucla, Western Australia. This represents a large range extension for S. macroura of 630 km from the nearest confirmed records of the species in Western Australia and South Australia. A suspected stripe-faced dunnart (Sminthopsis macroura) was captured at a coastal dune site near Eucla, Western Australia. The velvet-leaf hibiscus, Hibiscus krichauffianus F.Muell. (Malvaceae subfamily Malvoideae), is predominantly associated with sand dunes in the Australian arid zone but the name is presently also applied to plants occurring in different habitats on the southern edge of the range, and in disjunct locations in Western Australia and eastern Queensland. We investigated the morphological and environmental variation within H. krichauffianus throughout the geographic range. As a result, we have identified two new species, H. verecundus McLay & Albr. and H. calcareus McLay & Albr.; coined a phrase name, H. sp. Belele (D.W.Goodall 3417) for a putative new taxon; and recircumscribed H. krichauffianus. We also assessed material collected when the protologue of H. krichauffianus was published in the late 1850s and selected a lectotype from the original material identified. The velvet-leaf hibiscus, Hibiscus krichauffianus. Our investigation of the Australian arid zone specialist Hibiscus krichauffianus resulted in the discovery of two new species, H. verecundus and H. calcareus , both of which are described here. We also recircumscribe and lectotypify H. krichauffianus, and raise a phrase name entity. Context: Modern crop stubbles provide an important source of feed for sheep during summer but can vary widely in their feeding value due to Genetics × Environment × Management interactions during crop growth, and how chaff is managed at harvest. Stubbles are difficult to assess for grazing value, due to the heterogeneity of forage components (e.g. stem, leaf, grains), which means that livestock producers rely on their observations and experience to manage sheep grazing stubbles. Aims: To provide current information on the feeding value of stubbles, and to establish better methods for estimating stubble feeding value. Methods: In the present paper, we report the results of sheep grazing wheat-stubble paddocks and a survey of the nutritive value of standing stubbles and chaff lines and piles of grain crops (including wheat, barley, canola and lupins) from crop paddocks in the mixed-farming region of Western Australia. Key results: Without supplementation, wheat stubbles provided 50–100 dry sheep equivalent grazing days per hectare, before the estimated metabolisable energy (ME) intake of sheep fell below maintenance. This was influenced by ewe body condition score, where sheep of lower body condition gained more weight on stubbles (89 g/day higher weight gain per unit lower body condition score). Estimated ME content of edible chaff (leaf, fine stem and seed sheath or pod, excluding main stem and grain) was 5% higher in barley, 19% higher in lupin, and 5% lower in canola, than in wheat chaff, which was 5.9 MJ ME/kg DM. Implications: While the grazing value of modern stubbles is variable, they have an important role in the seasonal feedbase by providing several months of feed during summer. Chaff by itself does not meet the energy or protein requirements of livestock, so a high-protein grain supplement (e.g. lupins) is necessary to maintain non-lactating ewes fed only chaff. The increased use of chaff lines and piles at harvest, primarily for weed control, provides a new opportunity to fill feed gaps in sheep and cattle enterprises if the material can be transported and stored efficiently, or if grazed in situ , as is common practice. Crop stubbles, the residues left behind in cropping paddocks after harvest, provide a valuable source of feed for sheep in Western Australia during summer when green pasture is scarce. The feed value to sheep in stubbles is variable, and depends of the crop type, variety, location and method of harvest. Globalisation has increased the occurrence of species being introduced outside of their natural range. The African carder bee, Pseudoanthidium (Immanthidium) repetitum (Hymenoptera: Megachilidae), is one such species. P. repetititum was first recorded in Australia in 2000 in Queensland (north-east Australia), and rapidly spread down the east coast of Australia, and by 2015 was reported to be a common component of bee assemblages in urban community gardens in Victoria (southern Australia). Here, I report the first occurrences of this species in Western Australia, on the other side of the continent, representing a major expansion of the distribution of the species. Thus far there are three confirmed and one unconfirmed localities where this species has been seen, all localised to the Mandurah region in residential areas of Western Australia. Female specimens were collected from a garden in Halls Head, where it was observed to be abundant. Other bees, both native and the introduced European honeybee, were foraging alongside it. The occurrence of P. repetitum represents a major expansion in its distribution. It remains rare and localised; however, given known negative impacts of introduced species on native fauna and flora, especially in Australia, vigilance is required to monitor this species. A recent plan to extend a highway in south-west Western Australia has been abandoned, but not before a 10–200 m wide strip of vegetation along the alignment had been cleared. Attempts are now being made to rehabilitate the cleared area. The aim of this paper is 2-fold. First, it documents the changes in invertebrate abundance, richness and evenness in this abandoned landmark development. Second, it explores the changes in invertebrate species composition, concentrating on ants, in terms of known trends in biodiversity, with particular emphasis on biotic homogenisation. We report on the immediate changes in invertebrate assemblages from a spatial continuum of sites passing through six discrete woodland communities. A total of 69 species of ants were sampled by pitfall traps from the sites, with 50 being sampled in the 7 reference sites and 46 in the 13 cleared sites. As far as most of these simple indices are concerned, the previously cleared areas generally exhibited values that are not appreciably different from those of the reference sites, except that species richness was marginally higher in the reference sites and ant abundance was higher in the cleared area sites. There was considerable variation in numbers of ant species within Ant Niche Template categories in both the reference and cleared area sites. However, numbers of nest specialists and trophic specialists were significantly lower in cleared area sites; opportunistic species and one generalist were more abundant in cleared areas. The two-dimensional solution of the multi-dimensional scaling ordination grouped all cleared sites close together, with the reference sites and modified native vegetation site being widely scattered across the graph. The tight bunching of the cleared sites suggests that clearing for the proposed highway has resulted in homogenising the ant fauna, creating a characteristic assemblage that differed from all of the original reference sites. When considered at the ordinal level, significantly less Clitellata (worms), Malacostraca (slaters), Araneae (spiders), Blattodea (cockroaches), Dermaptera (earwigs) and Coleoptera (beetles) were sampled from the cleared sites, but significantly more Collembola (springtails), Homoptera and Heteroptera (both types of sucking bugs) were sampled in these sites. Collembola may be responding to the abundance of exotic grasses whereas the sucking bugs may have benefitted from the lush herb and shrub layer associated with the regrowth and rehabilitation efforts. Ants were surveyed along a corridor that had been cleared for an abandoned highway. We surveyed 7 uncleared reference sites and 13 cleared sites that had been rehabilitated 2 years earlier. Species richness was marginally higher in the reference sites and ant abundance was higher in the cleared sites but clearing resulted in homogenisation of the ant assemblages that were associated with the original vegetation associations. This case study describes the process of selecting the most appropriate state-wide hospital system to manage COVID-19 cases in a setting of low community transmission of COVID-19 infection. A rapid review of the literature was conducted of the advantages and disadvantages of having designated COVID hospitals. This led to three different options being presented for discussion. Following consultation, the option chosen was for all hospital facilities to remain prepared to care for COVID-19 patients where they present rather than having specified designated hospitals because this was considered the most practical option currently. Objective. In Australia, access to dental care has been available through several different pathways: (1) private practice; (2) public clinics; (3) Aboriginal Medical Services (AMS)-based clinics; and (4) until recently, the Chronic Disease Dental Scheme (CDDS). The aim of the present study was to compare the types of dental services most commonly delivered in the various clinical pathways based on the hypothesis that disease-driven care should lead to similar mixes of dental care provided. Methods. Data from a series of previously published sources was used to identify and compare the most commonly performed dental procedures in the different pathways. A comparison was also made with the available international data (US). Results. There was a marked difference between service mixes provided through the four pathways. Patients obtaining dental care through AMS-based and public pathways had more extractions and less restorative and preventive care compared with private and CDDS pathways. Compared with the international data, dental service mixes in Australia were found to be not as evenly distributed. Value of care provided through private and CDDS pathways were two- to threefold higher than that of AMS-based and public pathways. Conclusion. The data indicate that the original hypothesis that the disease-driven care should lead to similar mixes of provided dental care, is not supported. What is known about the topic? The pricing of dental care is a topic that gets substantial discussion in the private sector. However, its effect in the government sector is very limited in its consideration. This research is a bold first step in Australia to look at the influence of different modes of care delivery on the mix of care provided. What does this paper add? This research is a unique first effort to examine the influence of different modes of care delivery on the type of care provided. It clearly highlights that the pathway for care in the government sector do have very different care outcomes. What are the implications for practitioners? The genus Erythrophleum Afzel ex R.Br. is revised for Australia and three species are recognised, all previously included in E. chlorostachys (F.Muell.) Baill. Erythrophleum arenarium R.L.Barrett & M.D.Barrett is described as a new species to accommodate populations from desert sands of the Great Sandy Desert and Dampier Botanical Districts in north-western Australia, parapatric to the remaining species. Erythrophleum pubescens R.L.Barrett & M.D.Barrett is described as a new species spanning tropical Australia, and is sympatric with E. chlorostachys sensu stricto in the Northern Territory and adjacent areas of Western Australia and Queensland. Morphological examination has shown these three taxa to be consistently distinct across their respective ranges. Analyses of the nuclear ribosomal ITS1 region recovered three well-supported clades corresponding to the three morphologically defined species, and ITS1 has utility as a marker to separate sterile specimens. Erythrophleum pubescens is widespread in the Australian Monsoon Tropics, from the coast of Western Australia, near Derby, to Cape York Peninsula in northern Queensland. Erythrophleum chlorostachys is also widespread, from the eastern Kimberley Region, in Western Australia, to the Gulf of Carpentaria, in northern Queensland. A lectotype is chosen for Laboucheria chlorostachya F.Muell. Full descriptions, illustrations of key features and identification keys are provided for the three Australian species. Diabetes prevalence is increasing in Australia, and there are stark inequities in prevalence and clinical outcomes experienced by Indigenous people and low socioeconomic groups compared with non-Indigenous and socioeconomically advantaged groups. This paper explores the impact of Indigenous status and socioeconomic disadvantage on the experience of diabetes care in the primary health setting. Data were collected through focus groups and interviews. The sample, comprising 38 participants (Indigenous and non-Indigenous), was recruited from disadvantaged areas in Perth, Australia. Data analysis was mainly deductive and based on a conceptual framework for the relationship between socioeconomic position and diabetes health outcomes. Most participants reported accessing general practitioners regularly; however, evidence of access to dietitians and podiatrists was very limited. Perceived need, cost, lack of information on available services and previous negative experiences influenced health care-seeking behaviour. Complexity and lack of coordination characterised the model of care reported by most participants. In contrast, Indigenous participants accessing an Aboriginal community-controlled health organisation reported a more accessible and coordinated experience of care. Our analysis suggests that Indigenous and socioeconomically disadvantaged people tailor their health care-seeking behaviour to the limitations imposed by their income and disadvantaged circumstances. Objective. This study of equity and access characterises admissions for coronary, cerebrovascular and peripheral arterial disease by hospital type (rural, tertiary and non-tertiary metropolitan) in a representative Australian population. Methods. We conducted a descriptive analysis using data linkage of all residents aged 35-84 years hospitalised in Western Australia with a primary diagnosis for an atherothrombotic event in 2007. We compared sociodemographic and clinical features by atherothrombotic territory and hospital type. Results. There were 11 670 index admissions for atherothrombotic disease in 2007 of which 46% were in tertiary hospitals, 41% were in non-tertiary metropolitan hospitals and 13% were in rural hospitals. Coronary heart disease comprised 72% of admissions, followed by cerebrovascular disease (19%) and peripheral arterial disease (9%). Comparisons of socioeconomic disadvantage reveal that for those admitted to rural hospitals, more than one-third were in the most disadvantaged quintile, compared with one-fifth to any metropolitan hospital. Conclusions. Significant differences in demographic characteristics were evident between Western Australian tertiary and non-tertiary hospitals for patients hospitalised for atherothrombotic disease. Notably, the differences among tertiary, non-tertiary metropolitan and rural hospitals were related to socioeconomic disadvantage. This has implications for atherothrombotic healthcare provision and the generalisation of research findings from studies conducted exclusively in the tertiary metropolitan hospitals. What is known about the topic? Equity and access to hospital care for atherothrombotic disease in a geographically diverse population is poorly characterised. National data show that both fatal and non-fatal coronary heart disease and non-fatal stroke hospitalisations increase with remoteness. Fatal in-hospital stroke is greatest in major cities, whereas peripheral arterial disease hospitalisations are greatest in the inner and outer regional areas. What does this paper add? This study demonstrates that around 13% of atherothrombotic events were treated in rural hospitals with in-hospital case fatality higher than in tertiary and non-tertiary metropolitan hospitals. Purpose To explore health professionals' views about Australian Indigenous people's health and the delivery of healthcare to them in the Pilbara region of Western Australia. Methods An open-ended questionnaire was used to gather information from health professionals located across diverse regions in the Pilbara. The responses were analysed with the use of thematic analysis. In the first stage, codes were developed by assigning names to small sections of the interview transcripts. Next, the most salient incisive codes were identified and developed into themes that captured the most important issues. Findings Twenty-eight health professionals indicated that the most important health issues were chronic diseases, substance abuse and ear disease. These health issues were often attributed to a cycle of poor health perpetuated throughout generations. Educational initiatives were thought to be integral to intervening in this cycle. Of particular importance in improving the effectiveness of educational initiatives was facilitating the participation of Australian Indigenous peoples to determine the content of such initiatives. The other main issues the health professionals identified were lifting the standard of Australian Indigenous housing and implementing strategies to improve the continuity of healthcare. Conclusion Educational initiatives need to be prioritised to improve the health of Australian Indigenous people in the Pilbara and the initiatives should be delivered with the involvement of the local community in order to increase the likelihood of sustained behavioural change. Innovative solutions are required to improve the continuity of healthcare in the Pilbara, including increased use of mobile services. What is known about this topic? Objectives. To explore how Western Australian mental health clinicians want to evaluate their care. Methods. Using a participatory action research framework, 10 senior psychiatrists and 11 clinical nurse specialists working in an inpatient mental health setting participated in individual interviews, focus groups and meetings. All interviews were de-identified during transcription and transcripts and field notes were analysed for common themes. Results. Participants identified what they wanted to measure, how they wanted to measure it and how these changes could be implemented. Clinicians stressed the importance of measuring context (physical, clinical and service) and process as well as outcome, and of evaluating care at an individual and service level with consumer involvement. What is known about the topic? Completion rates of mandatory national outcome measures in mental health in Australia are variable and clinicians have mixed views as to their value. Several barriers have been identified as to their use including clinical, resource and ownership issues. What does this paper add? Some studies have identified areas of good practice and elicited practical suggestions for improvement but few have asked clinicians how they actually want to evaluate the care they provide. This study explored how mental health clinicians wanted to evaluate their care, using a participatory action research framework that encouraged participants to pinpoint problems and issues, account for their social context and develop actions to address them. What are the implications for practitioners? Clinicians were enthusiastic for high quality care and evaluation, but pessimistic about their ability to introduce sustainable change. Background. The risk of large, severe bushfires is increasing in Australia, and little is known about their impact on threatened species. Aims. This study aimed to investigated the abundance, occupancy and metapopulation structure of the quokka (Setonix brachyurus) for 5 years following a large, severe bushfire in Western Australia. Methods. Data collected across spatiotemporally replicated transects were used to estimate probabilities of occupancy and abundance, and to evaluate the influence of covariates. Key results. Quokka abundance was reduced to 16% of pre-fire levels, with distances up to 74 km separating subpopulations. Recolonising quokkas occupied habitat in parts of the landscape that had retained some vertical vegetation structure and contained a sedge-dominated understorey, and where damage by feral pigs was low. This coincided with topographic refugia that consisted of deep, mesic riparian systems or perched wetland systems. After 5 years, abundance had increased to 46% of pre-fire levels, with distances of 5-30 km separating subpopulations. Conclusion. Quokka recovery rate is slower than expected, and the metapopulation remains vulnerable to further fragmentation arising from disturbances that affect habitat patch suitability and movement corridors. South-western Western Australia is a global biodiversity hotspot renowned for exceptional diversity of plants and animals. The evolutionary processes that have generated this high biodiversity are not always clear, particularly for invertebrates, yet the area supports a very large number of endemic species that have diversified in situ. We use an integrative taxonomic approach based on adult and immature morphology, ecology, behaviour and molecular data to investigate the taxonomic status of a sympatric but seasonally isolated form (Neolucia agricola occidens Waterhouse & Lyell, 1914 form 'Julimar') of the polyommatine butterfly Neolucia agricola (Westwood, 1851) in south-western Western Australia. Our molecular dataset comprised 112 samples representing all Neolucia Waterhouse & Turner, 1905 species (100 COI 5′ sequences, 658 bp, plus 12 COI 3′, tRNA Leu , COII and EF1-α sequences, 3303 bp). Maximum likelihood phylogenetic analysis of the combined dataset recovered form 'Julimar' and N. agricola as reciprocally monophyletic, with a mean uncorrected ' p ' pairwise divergence of 5.77% for the 'barcode' region of COI. Based on this and other evidence we recognise form 'Julimar' as a new species, Neolucia bollami Eastwood, Braby & Graham, sp. nov. , sister to N. agricola and endemic to south-western Western Australia. As a result of these findings, we evaluated the evolutionary history of the two Neolucia species in WA and the processes that may have contributed to the diversification in sympatry or allopatry. We conclude that the multiple effect traits associated with a host shift, including host fidelity and temporal divergence, played an important role in the diversification process and in maintaining the reproductive integrity of the nascent allochronic species. Context: The legume shrub/tree leucaena (Leucaena leucocephala (Lam.) de Wit.) is highly regarded as a cattle fodder, although it is considered an environmental weed in many parts of Australia. Aims: We investigated the feasibility of developing a forage variety of triploid leucaena through interspecific hybridisation that is sterile (without seeds) as a strategy to mitigate its weed risk. Methods: A conventional breeding strategy was used to create triploid hybrids from hand-crosses between tetraploid and diploid species of leucaena. Molecular markers were developed to confirm successful crosses and flow cytometry was used to confirm the ploidy level. The plants are being evaluated for flowering behaviour, seed and herbage production across two environmentally diverse sites at Carnarvon and Kununurra in northern Western Australia. Key results: During 2018/19 and 2019/20, 2260 crosses between 45 different combinations of diploid females by tetraploid males and their reciprocals have created over 3000 putative triploid hybrid plants. This is the first time that triploids have successfully been created in Australia and 10 new parental combinations were created as a world-first. Conclusions: A cohort of sterile triploid plants has been identified and selections will be made based on their edible biomass productivity, insect tolerance, capacity for regrowth after cutting, nutritive value and plant chemistry (including anti-methanogenic responses). A sterile leucaena variety will have application not only in Australian grazing systems but also in other tropical regions of the world. The morphologically complex and charismatic genus Geleznowia (Rutaceae) is endemic to south-western Australia and faces existing and potential conservation issues associated with land clearing, climate change and commercial harvesting. Two species are currently recognised in the genus, but horticulturally recognised forms and phrase-named taxa reflect additional suspected species diversity. The genus exhibits complicated and subtle patterns of morphological variation that have historically inhibited delimitation of taxonomic entities and, as a result, precluded effective conservation assessments. Here we used ddRAD data from 25 populations across the range of Geleznowia to elucidate genomic diversity in the group in conjunction with morphological re-assessment so as to delimit species and revise the taxonomy. Our analyses consistently identified seven entities that maintain genomic distinctiveness even in sympatry with other entities, supporting the inference of reproductive barriers and lineage divergence. Morphological assessment of more than 300 specimens corroborated these seven taxa. Consequently, we recognise seven species of Geleznowia in Western Australia, retaining G. amabilis K.A.Sheph. & A.D.Crawford, recircumscribing G. verrucosa Turcz., reinstating G. calycina (J.Drumm. ex Harv.) Two new species of urodacid scorpion are described from the Pilbara region in Western Australia, where they are both patchily distributed along creek lines in the north-east of the region. Urodacus uncinus sp. nov. and Urodacus lunatus sp. nov. are indistinguishable based on external morphology: adults are medium-sized, yellow burrowing scorpions with remarkable sexual dimorphism in the telson, in which males have a uniquely swollen vesicle and an aculeus that is more strongly curved than other known species of Urodacus. The species are superficially similar to Urodacus similis L.E. Koch, 1977 and Urodacus yaschenkoi Birula, 1903 in the morphology of the first four metasomal segments, which are extremely short and not much longer than high. The two new species can only be discerned from each other based on the morphology of their hemispermatophores, which highlights the extremely conserved morphology of species in the genus and suggests that many new species await description with careful examination of their genitalia. Context: Studies of Phoma black stem and leaf spot disease (caused by Phoma medicaginis) in annual medics (Medicago spp.) normally involve a 'once-only' inoculation not reflecting multiple pathogen infection and phytoestrogen production cycles in the field. Phytoestrogen production by plants can result in lower ovulation rates in grazing animals. Aims: We aimed to determine whether sequential infections by P. medicaginis increase production of phytoestrogens in annual medics, and to measure the genetic diversity of isolates. Methods: In a greenhouse experiment, pathogenicity and virulence were investigated across 32 isolates of P. medicaginis following one, two or three rounds of inoculation of M. polymorpha var. brevispina. Production of the phytoestrogens coumestrol and 4′- O -methyl coumestrol was measured, and correlation with disease parameters assessed. DNA sequencing using ITS , β-tubulin , calmodulin and P. medicaginis -specific EFNI-1α was applied for phylogenetic analysis of isolates from Western Australia and elsewhere. Key results: Across isolates, highest leaf disease incidence was 76%, petiole disease incidence 61%, leaf disease severity 52% and petiole disease severity 53%. Stem coumestrol content range was 45–1247 mg kg−1, and 4′- O -methyl coumestrol 0–344 mg kg−1. All measures were highest after three rounds of inoculation. Overall, there was a positive correlation of leaf disease incidence with coumestrol content (P < 0.05) and of both leaf and petiole disease incidence with 4′- O -methylcoumestrol content (P < 0.01, P < 0.05, respectively). Phylogenetic analysis revealed a high degree of genetic similarity among Western Australian isolates, generally grouping into a single separate cluster across the four markers, and genetically distinct from isolates sourced outside Australia. Conclusions: Leaf disease incidence was the best discriminating disease parameter for coumestrol and 4′- O -methylcoumestrol content. Western Australian isolates of P. medicaginis were genetically similar and unique, possibly due to geographic separation. Implications: The study emphasised the importance of sequential inoculations when screening annual Medicago genotypes towards developing cultivars with superior disease resistance and enhanced animal reproductive outcomes.
