Background: Esophageal cancer is an invasive tumor with rapid growth and a poor prognosis. The annual incidence of esophageal cancer in the United States is 6 per 100 000 men and 1.6 per 100 000 women. This is different in the case of Iran, and the number of patients in the country is higher than in other countries. In this evaluation, the researchers present the experience with open Transhiatal Esophagectomy (Orringer) technique outcomes. Objectives: This study aimed at evaluating the Orringer technique and its complications in patients with esophageal cancer for 25 years at the Cancer Research Center of Shahid Beheshti University of Medical Sciences. Methods: This is a descriptive study conducted from the beginning of 1988 to 2016. In this study, 114 patients with a mean age of 55 years and a gender classification of 60% male and 40% female were evaluated. Results: After resection, more than 98% of patients were in the T3 N1-N3 stage. There were 4 mortalities: 2 were due to descending aorta tearing, 1 was due to tracheobronchial tearing, and 1 was pneumonia From 40 cases of pleural effusion, only 5 cases (12.5%) needed a thoracotomy tube. Only 2 patients needed intervention from 30 patients with dysphagia; 1 case was relieved by balloon dilatation and the other needed a free jejunal flap. Five-year survivals were 30% and 20% versus 48% and 23% in the neoadjuvant and non-neoadjuvant groups, respectively. conclusions: The present study and other experiences showed that non-radical surgery such as the Orringer technique is acceptable and compassable compared to radical surgery in 3 fields. Aim: This study aimed to investigate the effect of circadian rhythm stability and amplitude on musculoskeletal disorder prevalence and work–family conflict among nurses. Design: This cross‐sectional study was conducted on 386 nurses working in three hospitals, in Shahroud, Iran. Method: The data were collected using the Circadian Rhythm Inventory, Nordic Musculoskeletal Disorders, and Carlson Work‐Family Conflict questionnaires. Results: The highest prevalence of musculoskeletal disorders was reported in the lower back (68.4%), upper back (65.5%) and shoulders (53.6%), respectively. No significant relation was found between circadian rhythm stability and amplitude, and musculoskeletal disorders prevalence. The nurses who could not overcome drowsiness reported more Work Interference with Family (WIF), Family Interference with Work (FIW) (time‐based) and FIW (strain‐based) compared to the nurses who overcame drowsiness. In addition, the appropriate nurses for shift work experienced less WIF, FIW (behavioural‐based) and WIF (strain‐based) compared to the nurses who could not work overtime or at night. There has been a growing interest in assessing the condition and quality of life in older adult population, and factors contributing to it. In this context, fear of falling syndrome is introduced as one of the main factors that can affect the quality of life in the general adult population. However, little is known about the relationship between fear of falling and other disorders in older adults, particularly among the Middle East population. Here we investigated the associations between fear of falling and anxiety and depression disorders. A standardized questionnaire was used to assess sociodemographic variables and physical health condition. Subsequently, fear of falling and anxiety and depression scales were assessed using the Falls Efficacy Scale International and the Hospital Anxiety Depression Scale, respectively. Our results highlighted the high risk of having anxiety and depression disorders among older adults. The findings also indicate that physical and mental health disorders play a significant role in incident fear of falling. Therefore, improving mental health might lend controlling fear of falling syndrome, which then preventing falls incident and subsequent injury in older adult population. [ABSTRACT FROM AUTHOR]

Aim: To investigate the relation between the circadian rhythm amplitude and stability, and occupational stress with burnout syndrome and job dissatisfaction among shift working nurses. Background: Irregular shift working nurses are prone to burnout syndrome (BS) and job dissatisfaction (JD). circadian rhythm difference and occupational stress might be effective in causing burnout syndrome and dissatisfaction. Design: In total, 684 nurses who worked in four teaching hospitals were selected by stratified random sampling in a cross‐sectional design based on STROBE guidelines. Methods: The data were collected by the Circadian Type Inventory, Job Content Questionnaire, Minnesota Satisfaction Questionnaire and the Maslach Burnout Inventory. Multiple linear regression, one‐way ANOVA and independent t test were used for statistical analysis by SPSS v24. Results: About 15% of the nurses suffered from high levels of burnout syndrome. Psychological demand (β = 0.022, CI 95% = 0.003‒0.047 and β = 0.016, CI 95% = 0.001‒0.032) and workplace support (β = −0.043, CI 95% = −0.097–0.003 and β = −0.025, CI 95% = −0.046–0.006) were significant predictors of BS and JD. Male nurses reported lower BS and higher JD compared to female nurses. Irregular shift working schedule was also related to a significantly higher odd of BS (p = 0.009) and JD (p = 0.011). Nurses classified as languid experienced significantly more BS and JD than vigorous nurses. Conclusion: This study shows that BS and JD were strongly associated with psychological demand and workplace support; and vigorous nurses were less prone to BS and JD, and were more suitable for irregular shift work. Relevance to clinical practice: In order to reduce burnout syndrome and job dissatisfaction among irregular shift working nurses, we need to identify the effect of risk factors such as individual differences on the circadian rhythm and job demand which can affect nurses who work in irregular schedules. This study was conducted to investigate ocular complications and their relationship with sleep problems between IT students at Shahroud University of Technology as professional computer users and public health students at Shahroud University of Medical Sciences as non-professional computer users. This descriptive, cross-sectional study was conducted in 2014-2015 on 290 students, including 145 IT and 145 public health students. The students were selected by simple random sampling. Data were collected using five questionnaires, including a demographic questionnaire, the Ocular Surface Disease Index (OSDI), the Visual Fatigue Questionnaire, the Epworth Sleepiness Scale (ESS) and the STOP-BANG, and an examination for visual acuity. Data were analyzed in SPSS-22 using descriptive and inferential statistics and the level of statistical significance was set at P < 0.05. The comparisons showed a significant difference in the mean scores of the OSDI, VFQ and ESS between the male and female students, as all these scores were higher in the female students; however, no significant differences were observed in these scores between the professional and non-professional users. Given the significant direct relationships between the scores of the four questionnaires, it can be argued that sleep disorders and ocular disorders have reciprocal effects on each other. Sleepiness, dry eye and eye strain were not different between the groups of professional and non-professional computer users, but long hours of computer and cellphone use were observed so all students need to receive further education and correct ergonomics regardless of their field of study. Background and purpose: Population aging and advancing longevity is one of the achievements of the present century. Life satisfaction is considered as a typical sign of a successful adaptation to changes in various life situations. It seems that self-efficacy and social support may be associated with life satisfaction. There are limited publications in this field, so this study was done to investigate the relationship between life satisfactions, self-efficacy and social support in communitydwelling elderly in Sari, north of Iran. Materials and methods: In this descriptive-analytic study 129 elderly who agreed to participate were randomly selected from 10 health centers in Sari. The samples who aged more than 60 years of old were included and exclusion criteria were having incurables disease such as cancer, dementia, and severe psychological diseases. The data was collected using demographic questionnaires, Ed Diener life satisfaction, social support and self-efficacy questionnaires. To analyze the data SPSS V. 16 was applied. Results: The mean age of the elderly and life satisfaction were 67.86 ± 7.49 and 20.64 ± 8.36, respectively. We observed a significant correlation between life satisfaction and demographic variables such as economic status (P=0.002) and home status (P=0.01). Total average of social support and selfefficacy were 25.08 ± 5.98, 24.35 ± 7.55, respectively. This study found a significant association between life satisfaction with social support (r=0.44, P=0.001) and self-efficacy (r=0.49, P=0.001). Conclusion: According to the results of this study, there is a direct relationship between life satisfaction, social support, and self-efficacy. Life satisfaction in elderly can be increased by strengthening the support, independence and dignity of older people, thereby improving their social support and self-efficacy conditions.
