In the search for promising pathways for equitable and transformative climate adaptation in low-income rural areas, the present study focuses on resilience to more variable and less predictable availability of precipitation and water resources. Equitable water governance is conceptualized as formal and informal polycentric decision-making that narrows infrastructure inequities and ensures equitable water resources allocation. Focusing on recognitional and procedural equity, vulnerable women's and men's community-based water tenure is starting point. Partnering with government in Zambia and with the Water Integrity Network in Kenya, field research in three communities in rural Zambia and in rural communities and a small town sharing several schemes in Kenya is conducted. These studies recognized horizontal polycentricity of community-based water tenure by identifying common features: communities' age-old drought resilience coping strategies to meet daily domestic water needs and daily or seasonal small-scale productive water needs, by tapping water from surface- and groundwater sources through multi-purpose infrastructure, both self-financed and publicly supported, and by ‘sharing’ water ‘in’ within the community and ‘sharing out’ with neighbouring communities and powerful third parties. Procedural equity implies inclusive, locally-led planning, design and implementation in polycentric vertical governance with governments and other support agencies from local to national level and vice versa. The four interventions studied were the installation of a a solar-powered borehole for multiple uses and local government's institutional framework for potential replication in Zambia, and post-construction support of small water systems and a planned megadam in Kenya. Further research on similar transformative approaches elsewhere is recommended.

Millions of small-scale farmers in sub-Saharan Africa who are driving farmer-led irrigation development (FLID) have been turned into criminal offenders or, at least, categorically marginalised under widespread water permit systems. Under these systems, small-scale water users are obliged to apply for a permit, but very few have done so, largely because states lack the administrative capacity to inform such large numbers of people scattered across widespread rural areas of this obligation, to process large numbers of applications and to enforce conditions tied to permits. Those who use water below a usually very low threshold are exempted from this obligation, but small-scale farmers are generally above this category. This viewpoint, based on research and policy dialogues in a range of African countries, elaborates an alternative that addresses these injustices: a hybrid approach to water use authorisation. The proposed hybrid approach provides a suite of tools to legalise the water use of smallholder farmers and to overcome the colonial legacy of the side-lining of customary water law. These tools which can be combined and adjusted to suit specific contexts include: permits, targeted at, and enforced for, the relatively few high-impact users; collective permits; non-permit tools, in particular, first, general authorisations with equal or priority legal standing relative to permits and, second, the recognition of customary water law; and prioritisation.

Objectives: Bone and soft-tissue calcifications are often coincidentally diagnosed on digital panoramic radiographs (DPRs). As the use of three-dimensional (3D) images has increased in the past decade for diagnostics in the mandibular region, we evaluated 3D volume images derived from 2D panoramic images to determine if this method is suitable for early detection of calcifications in this region. Methods: In this study, three investigators retrospectively and independently evaluated 822 DPRs. If one or more calcifications were present, the 3D volume image from that patient was retrospectively evaluated to confirm the incidental findings. A radiographic system with a low-dose mode and a high-resolution 3D-image function was used. The investigators focussed on the most common calcifications, including tonsilloliths (TL), idiopathic osteosclerosis (IO) of the mandible, carotid artery calcifications (CAC), calcified submandibular lymph nodes (hereafter, CSL), and sialoliths of the submandibular salivary gland (SSG). Results: One or more calcifications were identified in 415 (50.5%) DPRs. In total, 718 calcifications were detected, 30.2% of which were TL, 16.3% IO, 11.3% CAC, 8.8% CSL, and 1.7% SSG. Only 287 (39.97 %) of the calcifications were confirmed on 3D volume images; of these, 29.2% were TL, 58.5% IO, 0.2% CAC, and 1.4% SSG. No CSLs were detected. Conclusions: Not all areas shown on the DPRs were visible in the retrospectively obtained 3D volume images. Whereas DPRs are used to diagnose calcifications such as IO, TL, SSG, CAC, and CSL, the 3D volume images were only useful for confirming the existence of IO, TL, and SSG calcifications.

Background: Competition for freshwater between cities and agriculture is projected to grow due to rapid urbanization, particularly in the Global South. Water reallocation from rural to urban regions has become a common strategy to meet freshwater needs in growing cities. Conceptual issues and associated measurement problems have impeded efforts to compare and learn from global experiences. This review examines the status and trends of water reallocation from rural to urban regions based on academic literature and policy documents. Methods: We conduct a systematic literature review to establish the global reallocation database (GRaD). This process yielded 97 published studies (academic and policy) on rural-to-urban reallocation. We introduce the concept of reallocation ‘dyads’ as the unit of analysis to describe the pair of a recipient (urban) and donor (rural) region. A coding framework was developed iteratively to classify the drivers, processes and outcomes of water reallocation from a political economy perspective. Results: The systematic review identified 69 urban agglomerations receiving water through 103 reallocation projects (dyads). Together these reallocation dyads involve approximately 16 billion m ^3 of water per year moving almost 13 000 kilometres to urban recipient regions with an estimated 2015 population of 383 million. Documented water reallocation dyads are concentrated in North America and Asia with the latter constituting the majority of dyads implemented since 2000. Synthesis: The analysis illustrates how supply and demand interact to drive water reallocation projects, which can take many forms, although collective negotiation and administrative decisions are most prevalent. Yet it also reveals potential biases and gaps in coverage for parts of the Global South (particularly in South America and Africa), where reallocation (a) can involve informal processes that are difficult to track and (b) receives limited coverage by the English-language literature covered by the review. Data regarding the impacts on the donor region and compensation are also limited, constraining evidence to assess whether a water reallocation project is truly effective, equitable and sustainable. We identify frameworks and metrics for assessing reallocation projects and navigating the associated trade-offs by drawing on the concept of benefit sharing.

The South African National Water Act (Act 36 of 1998) was hailed by the international water community as one of the most progressive pieces of water legislation in the world, and a major step forward in the translation of the concept of integrated water resources management (IWRM) into legislation. It has been widely quoted and referred to, and a number of countries ranging from China to Zambia have used it as an example in the revision of their own water legislation. And yet, 15 years down the line, implementation of the act has been only partially successful. In a number of critical aspects, implementation has, in fact, been weak. This paper sets out some personal reflections on the challenges facing the implementation of this remarkable piece of legislation and on the failure to achieve the initial high ambitions within the South African water sector. Through this process, it may be that there are lessons for other countries and for South Africa itself as it continues to face the challenge of implementation of the National Water Act (NWA).
