Castleman disease (CD) is a relatively rare lymphoproliferative disorder. Lesions predominantly originate on the chest and neck and rarely occur on the abdomen. A 34-year-old female presented to our hospital with an unexplained 10-year history of anemia. A pathological diagnosis of plasma cell-type CD was established. One cycle of chemotherapy (thalidomide, cyclophosphamide, and prednisolone) improved her anemia significantly. Prompt etiological diagnosis and early intervention are essential to address systemic manifestations in patients with CD, and it is crucial to consider CD as a differential diagnosis when intra-abdominal masses are detected.

In the human immunodeficiency virus (HIV)-infected population, especially HIV with concomitant tuberculosis (TB) or Hodgkin’s lymphoma (HL), numerous risk factors have been reported in recent years. Among them, the decreased CD4+ T cell count was recognized as the common risk factor. We report a case of a patient with HIV and TB and HL co-occurrence, in which patient’s CD4+ T cell count was inconsistent with disease. A 58-year-old male presented with fever and shortness of breath that persisted for 2 months. The patient had a 4-year history of HIV infection and underwent antiretroviral therapy (ART) effectively. After blood test, computed tomography, bone biopsy, and lymphoma biopsy, the patient was diagnosed with skeletal TB and HL, underwent TB treatment and received ART, and underwent four cycles of chemotherapy. CD4+ T cell count was not decreased before diagnosed with TB/HL and increased in this case after the fourth cycle of chemotherapy. We collected and analyzed CD4+ T cell counts in our case and reviewed relevant literature. It is suggested that CD4+ T cell count may be insufficient to predict the risk of HIV-related disease, especially lymphoproliferative disorders.

This study focuses on the automatic generation of architectural floor plans for standard nursing units in general hospitals based on Stable Diffusion. It aims at assisting architects in efficiently generating a variety of preliminary plan preview schemes and enhancing the efficiency of the pre-planning stage of medical buildings. It includes dataset processing, model training, model testing and generation. It enables the generation of well-organized, clear, and readable functional block floor plans with strong generalization capabilities by inputting the boundaries of the nursing unit's floor plan. Quantitative analysis demonstrated that 82% of the generated samples met the evaluation criteria for standard nursing units. Additionally, a comparative experiment was conducted using the same dataset to train a deep learning model based on Generative Adversarial Networks (GANs). Objectives: Renal cell carcinoma (RCC) is the most common cancer of the kidney. This study aims to evaluate the potential predictive value of E-cadherin, a marker of the epithelial mesenchymal transit (EMT) process that has been associated with tumor metastasis. Methods: We searched PubMed, Embase, and Cochrane Library to identify prospective studies. Hazard ratios (HRs), odds ratios (ORs), and 95% confidence intervals (CIs) were summarized to validate the relationship between E-cadherin and survival and clinical characteristics. The quality of the included studies was assessed using the NOS table. Then, we analyzed genetic data and clinical characteristics from The Cancer Genome Atlas Program (TCGA) database using R language with the dplyr package for validation. Results: Including 21 articles. The analysis revealed a strong link between high E-cadherin expression and favorable prognosis (for OS, HR = 0.35, 95% CI: 0.19–0.62; for PFS, HR = 0.19, 95% CI: 0.03–0.53; for DSS, HR = 0.25, 95% CI: 0.08–0.76; for RFS, HR = 0.71, 95% CI: 0.44–1.16; for DFS, HR = 0.28, 95% CI: 0.13-0.61; for T stage, OR = 0.21, 95% CI: 0.11–0.41; for N stage, OR = 0.07, 95%CI: 0.02-0.25; for M stage, OR = 0.12, 95% CI: 0.02–0.60; for clinical stage, OR = 0.29, 95% CI: 0.18–0.47; for nuclear grade, OR = 0.23, 95% CI: 0.13–0.41; for tumor size, OR = 0.49, 95% CI: 0.26–0.92). The findings were supported by bioinformatic analysis which used TCGA RCC patient's cohort (P < 0.01). In addition to stabilizing blood pressure (BP), ephedrine and phenylephrine have distinct effects on regional cerebral oxygen saturation (rSO2). However, whether its effect on rSO2 affects the occurrence of postoperative delirium (POD) remains unclear. Therefore, the aim of this study is to compare the effects of ephedrine and phenylephrine for BP maintenance on the incidence of POD in olderly adults who underwent knee arthroplasty under general anesthesia. One hundred twenty patients who were between 60 and 90 years old and underwent knee arthroplasty were included in this study. The patients were randomly divided into two groups: the ephedrine group and the phenylephrine group. After anesthesia induction, ephedrine and phenylephrine were continuously infused to maintain the intraoperative mean arterial pressure within the normal range (baseline mean arterial pressure ± 20%). The primary outcome measures included the incidence of POD within 1–3 days after surgery. The incidence of POD on the first day after surgery was lower in the ephedrine group than in the phenylephrine group (33% vs. 7%, P < 0.001). However, there was no significant difference in the incidence of POD between the two groups on the second and third postoperative days. Compared with the phenylephrine group, the ephedrine group experienced significantly greater cardiac output (CO) and rSO2 (P < 0.05). With the increasing demand for medical checkups, problems such as overcrowding and confusion in the public spaces of physical examination centers are becoming prominent. This seriously affects the efficiency of medical checkups and the satisfaction of patients. Because of the lack of reference norms and standards, spatial resources are allocated blindly in physical examination centers. Therefore, the reasonable prediction of demands for space and the allocation of areas for public space are problems we need to explore. This study aims to establish a foundation for spatial resource allocation by analyzing the relationship between area allocation and volume of medical checkups and spatial comfort. The objective is to balance service quality and construction costs in physical examination centers. Utilizing AnyLogic, this research simulates pedestrian flow characteristics during peak hours based on empirical data. Additionally, it provides statistics on and analysis of such areas in 30 physical examination centers. The findings reveal that the average per capita allocation of public space ranges from 6.90 to 7.10 m2. The functional relationship between the area of public space (y) and the total area (x) is y = 0.3649x − 57.096. This paper also proposes a relationship between the waiting space and the daily volume of medical checkups according to two comfort levels. The percentages of waiting space in public space ranges from 10% to 12.5% (comfort level) and 7% to 10% (general comfort level). This research provides architects with a specific and operable foundation for space planning, enhancing the standardization and scientification of physical examination center construction. Furthermore, the methodology used in this study proves valuable for future analyses of the interrelationships among functional units in medical buildings.
