The aim of this study is to evaluate the efficacy and toxicity of trastuzumab emtansine (T-DM1) in cases with metastatic breast cancer (mBC) in different lines of treatment. Retrospective analysis of T-DM1 results of human epidermal growth factor receptor 2 (Her2) positive 414 cases with mBC from 31 centers in Turkey. Except 2, all of the cases were female with a median age of 47. T-DM1 had been used as second-line therapy in 37.7% of the cases and the median number of T-DM1 cycles was 9. Progression-free survival (PFS) and overall survival (OS) times were different according to the line of treatment. The median OS was found as 43, 41, 46, 23 and 17 months for 1st, 2nd, 3rd, 4th and 5th line, respectively (p = 0.032) while the median PFS was found as 37, 12, 8, 8 and 8 months, respectively (p = 0.0001). Treatment was well tolerated by the patients. The most common grade 3–4 adverse effects were thrombocytopenia (2.7%) and increased serum gamma-glutamyl transferase (2%). The best of our knowledge this is the largest real-life experience about the safety and efficacy of T-DM1 use in cases with mBC after progression of Her2 targeted treatment. This study suggests and supports that T-DM1 is more effective in earlier lines of treatment and is a reliable option for mBC. [ABSTRACT FROM AUTHOR]

Nivolumab is an immunotherapy drug developed to increase the ability of the immune system to target and kill cancer cells and is a human immunoglobulin G4 monoclonal antibody blocking programmed cell death-1. Nivolumab is a checkpoint inhibitor that stops a signal that hinders stimulation of the tumor cell-attacking T-cells. It is a relatively new drug used in melanoma treatment. A 67-year-old male patient was operated on for acral lentiginous melanoma and admitted to our outpatient clinic with white spots on his face, head, and back of both hands while receiving nivolumab treatment. New-onset vitiligo during melanoma treatment is associated with more favorable clinical outcomes. Improved survival was demonstrated in this type of patient. Therefore, nivolumab treatment was continued because of new-onset vitiligo in our patient. This patient was living for 4 years without any clinical progression since receiving nivolumab treatment. Background: To retrospectively evaluate the survival rates effectiveness and complications of transarterial chemoembolization on colorectal cancer with liver metastases. Material and Methods: Fiftyseven patients (mean age 63) and 112 procedures with colorectal cancer liver metastases retrospectively underwent. Survival times and periprocedural complications were assessed. Results: The treatment was tolerated well by all patients. Survival rate for 1 and 2 years from first transarterial chemoembolization was 58.4% and 26.5%, respectively. Median survival time was 18.5 months. There were 62 times postembolization syndrome in 32 patients after transarterial chemoembolization. One patient had abscess in liver after chemoembolization. Conclusions: Transarterial chemoembolization is a minimally invasive treatment for liver metastases of colorectal carcinomas and has little major complications. Due to poor prognosis in advanced non-small cell lung cancer (NSCLC), new effective markers are required in the monitoring of the disease. The present study aimed to investigate the association between the serum IL-1 receptor antagonist (IL-1Ra) level, overall survival (OS), and treatment response in NSCLC, and to evaluate the usefulness of the serum IL-1Ra level as a prognostic marker for NSCLC. Eighty patients (72 men and 8 women) and 40 healthy volunteers (13 men and 27 women) were included in the present study. The median progression-free survival was 16 weeks for patients with high serum IL-1Ra levels, and 35 weeks for patients with low serum IL-1Ra levels (P=0.027). The median OS was 38 weeks in patients with a high serum IL-1Ra level, and 62 weeks in patients with a low serum IL-1Ra level (P=0.065). The results of the present study have demonstrated that there was a significant correlation between IL-1Ra levels and NSCLC progression and survival, although the correlation between IL-1Ra levels and the response to treatment was not statistically significant. Bleomycin is an antibiotic agent that has anti-tumor efficiency. Dose limiting toxicity depends on pulmonary complications which can be diagnosed by clinical and radiological findings. Positron emission tomography / computed tomography screening is also useful for this purpose. We report an interstitial eosinophilic pneumonia case, diagnosed with Positron emission tomography / computed tomography and surgery, developing after bleomycin combination chemotherapy. Metastatic carcinoma of the palatine tonsil is extremely rare in the literature. We report herein a case with unilateral tonsillar metastasis of small cell lung carcinoma which may mimic tonsillolithiasis, with a review of the literature about this rare entity. [ABSTRACT FROM AUTHOR]

Desmoid tumors are rarely observed tumors that develop from deep muscle and aponeurotic tissues. They are histologically intermediate, and exhibit a local aggressive course. These tumors have been observed in the abdominal region, and are rarely encountered in the thoracic wall. The most effective treatment appears to be surgery. A 59-year-old female patient was admitted with the complaint of right breast pain and a palpable mass. The patient had undergone a modified radical mastectomy surgical procedure of the right breast due to invasive breast carcinoma 10 years before. A mass with a soft tissue density and a size of 44x22 mm was detected in the thoracic computed tomography of the patient, which subsequently resulted in biopsy followed by surgery. As a result of the pathological examination of the mass, the diagnosis was determined as desmoid tumor. Due to the closeness of the tumor to the surgical margin, the patient was administered radiotherapy and maintenance imatinib treatment was started. [ABSTRACT FROM AUTHOR]

Aim. The current standard treatment of locally advanced rectal carcinoma is total mesorectal excision and postoperative adjuvant chemotherapy after neoadjuvant concurrent chemoradiotherapy (NCRT). Many studies have shown that pathological complete response (pCR) is an important prognostic factor for patients receiving NCRT. Many studies have therefore been conducted to increase pCR rates by changing the perioperative treatment strategies. Prolonging the chemotherapy time may be a reasonable way to increase the effectiveness of NCRT, pCR, and survival rates. We investigated whether neoadjuvant consolidation chemotherapy had an effect on tumor response and survival. Methods. The data of 163 patients diagnosed with locally advanced rectal carcinoma were evaluated. The data of 107 patients (Group 1) who were radiologically T3–T4 and/or N+ and received chemotherapy after NCRT until their operations were compared with the data of 56 patients (Group 2) who were operated after NCRT. Results. Group 1 patients had tumor and node downstaging. Their pCR was found significantly higher than in Group 2 (p = 0.005). In Group 1 patients with T3, pCR was significantly higher than for those with T4. The elapsed time between NCRT and surgery was significantly longer in patients with pCR (respectively, p = 0.012 and p = 0.008). Conclusion. Neoadjuvant consolidation chemotherapy after NCRT is a safe approach that can lead to higher pathological complete response rates. The time until surgery with neoadjuvant consolidation chemotherapy may provide the chance to follow the patient without surgery in addition to increasing pCR. [ABSTRACT FROM AUTHOR]

Objective: Glioblastoma Multiforme (GBM) is the most common primary malignant brain tumor in adults. Optimal treatment is adjuvant concomitant chemoradiotherapy following the widest possible surgical resection (maximum safe resection) followed by adjuvant chemotherapy. However, the geriatric group is not treated optimally. Recent studies have shown that optimal treatment benefits in the older group, similar to those in younger patients. We aimed to investigate the effect of optimal treatment on survival in patients 65 years of age and older with GBM. Methods: In Pamukkale University Medical Oncology Clinic, the files of 71 patients with a pathological diagnosis of GBM between 2010 and 2018 were retrospectively evaluated. Demographic and clinicopathological characteristics of the patients were recorded from the file. Results: Of the 71 patients evaluated, 25 (35.2%) were 65 and over, 46 (64.8%) were under 65 years old. Of the twenty-five patients, 11 (44%) were male and 14 (56%) were female. Median follow-up time was 16.2 months (12.4-19.9) and median overall survival time was 12.5 months (10.4-14.7) in all patient groups. The median overall survival of patients under the age of 65 years was 15.1 months (12.6-17.6 months), and patients aged 65 and over were 8.6 months (7.9-9.3 months) (p <0.001). The median overall survival was 8.9 months (8.4-9.4 months) in patients 65 years and older who received optimal treatment, while the median overall survival was 7.9 months (3.4-12.4 months) in patients who did not (p:0.048). Conclusion: The median overall survival time of patients 65 years and older who received optimal therapy was significantly longer than those who did not. Introduction: Cancer cachexia is one of the most frequent effects of malignancy, is often associated with poor prognosis, and may account for up to 20% of cancer deaths. The aim of our study was to evaluate the relationship of cancer cachexia and serum levels of resistin and leptin in patients with advanced non-small cell lung cancer., Methods: A total of 67 chemotherapy-naïve patients with advanced-stage non-small cell cancer and a control group containing 20 healthy individuals without a known chronic disease were enrolled in this study. All individuals in the control group were age and sex matched. Demographic, anthropometric, laboratory data and serum levels of adipokines were measured for 2 groups. Progression-free survival and overall survival were estimated using the Kaplan-Meier method. Survival among various factors was calculated using the log-rank test., Results: Patients presented significantly higher serum resistin ( P = .0001) and lower serum leptin levels ( P = .025) than the control group. Primary mesenchymal tumors of the colon are extremely rare tumors among soft tissue sarcomas. These tumors are more aggressive and have poorer prognosis than adenocarcinoma of the colon. Here, we presented 3 cases of primary mesenchymal tumors of the colon. Their histopathological diagnoses are leiomyosarcoma, pleomorphic liposarcoma, and desmoplastic small round cell tumor, respectively. The rarity of primary mesenchymal tumors of the colon makes it difficult to approach the treatment and predict the prognosis of these rare tumors.

Pulmonary sarcomas constitute only 0.1-0.5% of all primary lung malignancies. These tumors may derive from the lung parenchyma, bronchial tree or pulmonary arteries. The most important entity in the differential diagnosis is metastatic synovial sarcoma. A 76-years-old woman was admitted for investigation of a fever, productive cough, dyspnea, weight loss and left-sided chest pain which had been present for one month. A chest computerised tomography showed enlarged mediastinal lymph nodes were observed, as well as a left-sided pleural effusion. Thoracentesis revealed hemorrhagic pleural effusion which was exudate and lymphocyte predominant, closed pleural biopsy showed chronic inflammation. Left sided thoracoscopy was performed under local anesthesia, total collapse of left lung and multiple pleural nodules were observed on the visceral pleura multiple biopsies were obtained from those nodules. Pathologic examinations revealed "synovial sarcoma". As skeleton single photon emission tomography was unremarkable, primary pleuropulmonary synovial sarcoma was decided as diagnosis and chemotherapy was planned for the patient. Primary pleuropulmonary synovial sarcoma is a rare neoplasm of lung and pleura but it is rare entity.

Aim: To evaluate the relationship between gallbladder (GB) motor function and H pylori infection in the stomach., Methods: All cases (86) underwent the 14C urea breath test (UBT). 14C-UBT was found as positive in 58 and negative in 28 dyspeptic patients. 14C-UBT was accepted as a gold standard test. Clo test and histopathologic examination were compared with the results of 14C-UBT in cases who tolerated upper gastrointestinal endoscopy procedure. Cholescintigraphy with 99mTc-mebrofenin was used to determine the parameters of GB motor function (GB filling and emptying time, half of the emptying time, ejection fraction at 30th and 60th min) in all patients., Results: We found the sensitivity and specificity as 88% and 86% for Clo test and as 89% and 80% for histologic evaluation, respectively. The parameters of GB function were not significantly different in H pylori positive and negative patients. The GB emptying was normal in both groups. Minimum GB filling time was 30 min in 34 of 86 cases (39.5%), filling was not observed in 2 cases. The GB ultrasonography (USG) results were normal for all cases and bile composition abnormality was not determined., Conclusion: Our study showed that 14C-UBT is highly reliable method to detect the presence of H pylori. The presence of H pylori infection does ot directly affect the GB function.
