MYC rearrangements (MYCr) occur in several B-cell neoplasms and impact disease progression and overall survival. We used whole genome sequencing (WGS) and whole transcriptome sequencing (WTS) to analyze and compare MYCr in different B-cell neoplasms. The MYCr features of cases with plasma cell myeloma (PCM) (n = 88) showed distinct characteristics compared to cases with mature B-cell lymphomas (n = 62, including Burkitt lymphoma (BL), diffuse large B-cell lymphoma (DLBCL) and high grade lymphoma with MYC and BCL2 and/or BCL6 rearrangements (HGBL)): they were more complex and showed a wider variety of translocation partners and breakpoints. Additionally, unlike B-cell lymphomas, they showed no evidence of activation-induced deaminase (AID) involvement in the formation of MYCr with immunoglobolin heavy chain (IGH), indicating a different mechanism of origin. The different MYCr characteristics resulted in poor MYCr detection rates by fluorescence in situ hybridization of only 50% in PCM, compared to 94% in lymphoma. [ABSTRACT FROM AUTHOR]

BACKGROUND: The diagnostic value of Doppler ultrasonography of the portal vein for the evaluation of liver function is still contradictory. OBJECTIVE: Aim of this study was to test the relationship between clinical liver function tests based on MRI and breath testing and blood flow in the portal vein. METHODS: The portal vein velocity was measured by color coded Doppler ultrasonography (CCDS) and tested against the relative enhancement (RE), a MRI-based index of liver function. The signal intensity in the liver parenchyma was assessed before (pre) and after (post) administration of contrast agent, the RE was calculated afterwards. Further, the liver function was also assessed using a 13C-Methacetin-based breath test. The blood flow in the portal vein was tested for possible correlation applying Pearson's correlation coefficient. RESULTS: Using CCDS, all patients show a hepatopetal portal blood flow. The portal vein velocity is decreasing with progression of liver damage and there was a significant correlation of portal velocity with SI post (r = 0.411, p = 0.024). However, the portal velocity did not correlate significantly with the 13C-MBT readout (r = 0.233; p = 0.216), SI pre (r = 0.271, p = 0.147) or the relative enhancement (r = 0.303; p = 0.103). CONCLUSIONS: The results of this proof-of-principle study indicate that CCDS-based assessment of portal velocity is of only limited value for the evaluation of liver function. [ABSTRACT FROM AUTHOR]

PURPOSE: Contrast enhanced magnetic resonance imaging (MRI) is able to assess liver function by characteristic changes of signal intensity (SI). The aim was to evaluate dynamic contrast-enhanced SI-indices of the abdominal aorta, portal vein and liver. METHODS: 72 patients underwent Gd-EOB-DTPA-enhanced MRI and a 13C-methacetin-based liver breath test (13C-MBT) for evaluation of liver function. Region-of-interest measurements in the liver, abdominal aorta and portal vein during native, arterial (AP), late arterial (LAP), portal venous (PVP) and hepatobiliary phase (HBP) were applied to analyze SI-indices in T1-weighted volume-interpolated breath-hold examination (VIBE) sequences with fat-suppression and relative enhancement (RE) analysis was performed. RESULTS: The liver (p < 0.001), the portal vein (p < 0.001) and abdominal aorta (p = 0.002) showed significant decrease of REs with decreasing liver function. An increasing trend between logarithmic values of 13C-MBT and REs of hepatic parenchyma (HBP; r = 0.662, p < 0.001), portal vein (PVP; r = 0.532, p < 0.001) and abdominal aorta (PVP; r = 0.421, p < 0.001) was observed. CONCLUSIONS: RE measurements of the hepatic parenchyma proofed to be a trustable evaluation method for liver function evaluation. Objectives: To determine whether liver function as determined by intravenous administration of 13C-methacetin and continuous real-time breath analysis can be estimated quantitatively from gadoxetic acid (Gd-EOB-DTPA)-enhanced magnetic resonance (MR) relaxometry.Methods: Sixty-six patients underwent a 13C-methacetin breath test (13C-MBT) for evaluation of liver function and Gd-EOB-DTPA-enhanced T1-relaxometry at 3 T. A transverse 3D VIBE sequence with an inline T1 calculation based on variable flip angles was acquired prior to (T1 pre) and 20 min post-Gd-EOB-DTPA (T1 post) administration. The reduction rate of T1 relaxation time (rrT1) and T1 relaxation velocity index (∆R1) between pre- and post-contrast images was evaluated. 13C-MBT values were correlated with T1post, ∆R1 and rrT1, providing an MRI-based estimated 13C-MBT value. The interobserver reliability was assessed by determining the intraclass correlation coefficient (ICC).Results: Stratified by three different categories of 13C-MBT readouts, there was a constant increase of T1 post with increasing progression of diminished liver function (p ≤ 0.030) and a constant significant decrease of ∆R1 (p ≤ 0.025) and rrT1 (p < 0.018) with progression of liver damage as assessed by 13C-methacetin breath analysis. ICC for all T1 relaxation values and indices was excellent (> 0.88). Background: Gd-EOB-DTPA, a liver specific contrast agent with T1- shortening effects, is routinely used in clinical magnetic resonance imaging (MRI) for detection and characterization of focal liver lesions. Gd-EOB-DTPA-enhanced T1 relaxometry has recently received increasing attention as a tool for the quantitative analyses of liver function. However, this T1 relaxometry technique is limited due to various artifacts caused by B1 inhomogeneities and motion artifacts. This study aims to compare two different T1 relaxometry techniques for evaluating liver function as determined using a 13 C-methacetin-based breath test ( 13 C-MBT)., Methods: Ninety-six patients underwent gadoxetic acid-enhanced MRI of the liver at 3T and a 13 C-MBT. Two different prototype sequences for T1 relaxometry were used, a 3D VIBE sequence using Dixon water-fat separation and variable-flip-angles (VFA), as well as a 2D Look-Locker sequence (LL). Images were acquired before (T1 pre) and 20 minutes after (T1 post) administration of liver-specific contrast agent to evaluate the reduction rates of T1 relaxation time (rrT1) in accordance with the 13 C-MBT outcome. To analyze both MR sequences' performance, the intraclass correlation coefficient (ICC) between four ROI measurements within the same liver and the coefficient of repeatability (CR) were calculated., Results: T1 relaxometry measurements based on MR sequences, VFA, and LL show a constant change in line with impaired liver function progression. Simple regression models showed a log-linear correlation of 13 C-MBT values with all evaluated T1 relaxometry measurements (VFA T1post, VFA rrT1, LL T1post, LL rrT1, P<0.001). Both ICC (VFA T1post, LL T1post; 0.75, 0.95) and CR (VFA T1 post, LL T1 post; 179, 101) showed a better agreement between ROI measurements using the LL sequence., Conclusions: Both T1 relaxometry sequences are suitable for the evaluation of liver function based on 13 C-MBT. More than 90% of patients with myelodysplastic/myeloproliferative neoplasms (MDSs/MPNs) harbor somatic mutations in myeloid-related genes, but still, current diagnostic criteria do not include molecular data. We performed genome-wide sequencing techniques to characterize the mutational landscape of a large and clinically well-characterized cohort including 367 adults with MDS/MPN subtypes, including chronic myelomonocytic leukemia (CMML; n = 119), atypical chronic myeloid leukemia (aCML; n = 71), MDS/MPN with ring sideroblasts and thrombocytosis (MDS/MPN-RS-T; n = 71), and MDS/MPN unclassifiable (MDS/MPN-U; n = 106). A total of 30 genes were recurrently mutated in ≥3% of the cohort. Distribution of recurrently mutated genes and clonal architecture differed among MDS/MPN subtypes. Statistical analysis revealed significant correlations between recurrently mutated genes, as well as genotype-phenotype associations. We identified specific gene combinations that were associated with distinct MDS/MPN subtypes and that were mutually exclusive with most of the other MDSs/MPNs (eg, TET2-SRSF2 in CMML, ASXL1-SETBP1 in aCML, and SF3B1-JAK2 in MDS/MPN-RS-T). Patients with MDS/MPN-U were the most heterogeneous and displayed different molecular profiles that mimicked the ones observed in other MDS/MPN subtypes and that had an impact on the outcome of the patients. Specific gene mutations also had an impact on the outcome of the different MDS/MPN subtypes, which may be relevant for clinical decision-making. The application of navigational systems has the potential to improve percutaneous interventions. The accuracy of ablation probe placement can be increased and radiation doses reduced. Two different types of systems can be distinguished, tracking systems and robotic systems. This review gives an overview of navigation devices for clinical application and summarizes first findings in the implementation of navigation in percutaneous interventions using irreversible electroporation. Because of the high number of navigation systems, this review focuses on commercially available ones.

Electroporation is a well-known phenomenon that occurs at the cell membrane when cells are exposed to high-intensity electric pulses. Depending on electric pulse amplitude and number of pulses, applied electroporation can be reversible with membrane permeability recovery or irreversible. Reversible electroporation is used to introduce drugs or genetic material into the cell without affecting cell viability. Electrochemotherapy refers to a combined treatment: electroporation and drug injection to enhance its cytotoxic effect up to 1000-fold for bleomycin. Since several years, electrochemotherapy is gaining popularity as minimally invasive oncologic treatment. The adoption of electrochemotherapy procedure in interventional oncology poses several unsolved questions, since suitable tumor histology and size as well as therapeutic efficacy still needs to be deepen. Electrochemotherapy is usually applied in palliative settings for the treatment of patients with unresectable tumors to relieve pain and ameliorate quality of life. In most cases, it is used in the treatment of advanced stages of neoplasia when radical surgical treatment is not possible (eg, due to lesion location, size, and/or number). Further, electrochemotherapy allows treating tumor nodules in the proximity of important structures like vessels and nerves as the treatment does not involve tissue heating. Overall, the safety profile of electrochemotherapy is favorable. Most of the observed adverse events are local and transient, moderate local pain, erythema, edema, and muscle contractions during electroporation. The aim of this article is to review the recent published clinical experiences of electrochemotherapy use in deep-seated tumors with particular focus on liver cases. The principle of electrochemotherapy as well as the application to cutaneous metastases is briefly described. A short insight in the treatment of bone metastases, unresectable pancreas cancer, and soft tissue sarcoma will be given. Preclinical and clinical studies on treatment efficacy with electrochemotherapy of hepatic lesions and safety of the procedure adopted are discussed.
