Introduction/Aims: Conventional F wave analysis involves a relatively uniform physiological environment induced by supramaximal stimulations. The F wave characteristics in a dynamic physiological condition, however, are rarely investigated. This study aimed to improve understanding of F wave properties in the more dynamic process by introducing a novel method to analyze F waves based on the compound muscle action potential (CMAP) scan technique. Methods: Twenty four healthy subjects participated in the study. The CMAP scan was applied to record muscle responses in the abductor pollicis brevis (APB) and abductor digiti minimi (ADM) muscles, respectively. F wave characteristics including mean F wave amplitude and latency (F‐M latency), persistence and activating threshold were quantified. Results: An average of 200 F waves per muscle were obtained from the CMAP scan recording. Weak to moderate correlations between F wave amplitude and stimulating intensity were observed in most of the APB (19 muscles; r = 0.33 ± 0.14, all p <.05) and ADM (23 muscles, r = 0.46 ± 0.16, all p <.05) muscles. Significantly longer mean F latency and lower activating F‐threshold were found in the ADM muscles (F‐M latency: APB: 25.43 ± 2.39 ms, ADM: 26.15 ± 2.32 ms, p <.05; F‐threshold: APB: 7.68 ± 8.96% CMAP, ADM: 2.35 ± 2.42% CMAP, p <.05). Discussion: This study introduces new features of F waves using the CMAP scan technique and identifies differences of F wave characteristics between the hand muscles. Aims: Motor evoked potentials (MEP) elicited by transcranial magnetic stimulation (TMS) over the primary motor cortex are used as a neurophysiological marker of cortical excitability in clinical and scientific practice. Though, the reliability of this outcome parameter has not been clarified. Using a systematic approach, this work reviews and critically appraises studies on the reliability of MEP outcome parameters derived from hand muscles of healthy subjects and gives a proposal for most reliable TMS practice. Methods: A systematic literature research was performed in PubMed, according to the PRISMA guidelines. Articles published up to March 2023 that were written in English, conducted repeated measurements from hand muscles of healthy subjects and reliability analysis were included. The risk of publication bias was determined. Two authors conducted the literature search and rated the articles in terms of eligibility and methodological criteria with standardized instruments. Frequencies of the checklist criteria were calculated and inter-rater reliability of the rating procedure was determined. Reliability and stimulation parameters were extracted and summarized in a structured way to conclude best-practice recommendation for reliable measurements. Results: A total of 28 articles were included in the systematic review. Critical appraisal of the studies revealed methodological heterogeneity and partly contradictory results regarding the reliability of outcome parameters. Inter-rater reliability of the rating procedure was almost perfect nor was there indication of publication bias. Identified studies were grouped based on the parameter investigated: number of applied stimuli, stimulation intensity, reliability of input-output curve parameters, target muscle or hemisphere, inter-trial interval, coil type or navigation and waveform. Conclusion: The methodology of studies on TMS is still subject to heterogeneity, which could contribute to the partly contradictory results. According to the current knowledge, reliability of the outcome parameters can be increased by adjusting the experimental setup. Reliability of single pulse MEP measurement could be optimized by using (1) at least five stimuli per session, (2) a minimum of 110% resting motor threshold as stimulation intensity, (3) a minimum of 4 s intertrial interval and increasing the interval up to 20 s, (4) a figure-of-eight coil and (5) a monophasic waveform. The "split-hand syndrome" is a common clinical sign in amyotrophic lateral sclerosis (ALS), being characterized by more severe atrophy of the hand muscles on the radial side of the hand compared to the ulnar side. We aimed to investigate possible physiological differences between relevant hand muscles using low-intensity F-wave stimulation to assess spinal motoneuron excitability. We recruited 36 healthy volunteers. F-waves were recorded from the abductor pollicis brevis (APB), first dorsal interosseous (FDI) and abductor digiti minimi (ADM), using 20 supramaximal stimuli followed by 20 stimuli at a low-intensity required to obtain M-waves with 10 % amplitude of maximal CMAP. We evaluated the following F-wave parameters: F-M latency, chronodispersion, persistence, amplitude, F/CMAP amplitude ratio and number of F-wave repeaters (with low-intensity). In 10 subjects, low-intensity stimulation F-waves were compared after 20 and 50 stimuli in each muscle. Low-intensity stimulation resulted in lower F-wave amplitude and persistence and higher F/CMAP amplitude ratios. There were no significant differences in F-wave latencies and chronodispersion. When comparing the three muscles, we found higher F-wave persistence and F/CMAP amplitude ratios when recording over the ADM and APB compared to the FDI. We also found a higher number of F-wave repeaters in the ADM with low-intensity stimulation. Results from 20 to 50 low-intensity stimuli were similar. A small number of low-intensity stimuli is appropriate to study F-wave latencies and chronodispersion. • Changes in hand and face M1 are specific to the type of instrument played. • Woodwind musicians had increased interhemispheric inhibition in face but not hand M1. • String musicians had heightened plasticity in hand M1 but not face M1. The process of learning and playing a musical instrument modulates the structural and functional organization of cortical motor networks. In the present study the excitability and short-term functional plasticity of face and hand areas of primary motor cortex (M1) were compared in woodwind musicians (WM), string musicians (SM) and non-musicians (NM) to test the hypothesis that neurophysiological adaptations to the long-term experience of playing a musical instrument are site-specific and related to the particular physiological properties of the representation area in M1. Twenty-two musicians (11 SM, 11 WM) and 11 NM participated in the study. Transcranial magnetic stimulation (TMS) was used to probe rest and active short-latency intracortical inhibition (SICI), interhemispheric inhibition (IHI) and response to paired associative stimulation (PAS). TMS-induced motor evoked potentials (MEP) were recorded from the depressor anguli oris (DAO) and the first dorsal interosseous (FDI) muscles, respectively. Rest and active SICI were the same in all groups (all p > 0.05). WM exhibited significant IHI in the DAO (p = 0.031), in contrast to its absence in SM and NM. Compared with NM and WM, the PAS-induced increase in MEP amplitude in SM was significantly larger in hand M1 (p = 0.008) but not in face M1. In conclusion, neurophysiological adaptations differ between WM, in whom control of the embouchure is highly important, and SM who perform a large range of sequential finger movements and are site-specific in M1. Objective: To study compound muscle action potential (CMAP) decrement by low-frequency repetitive nerve stimulation (RNS) in different hand muscles of amyotrophic lateral sclerosis (ALS) patients and the relationship with split hand phenomenon and clinical manifestation.Methods: Clinical and decrement data of 51 ALS patients who had done RNS in different hand muscles were retrospectively reviewed from November 2016 to July 2017. Decrement data of 24 myasthenia gravis (MG) and 20 Lambert Eaton myasthenia syndrome (LEMS) patients was also reviewed to compare decrement pattern with hand muscles of ALS patients.Results: There was statistical significance between the decrement ratio of abductor digiti minimi (ADM) and abductor pollicis brevis (APB) as well as ADM and first dorsal interosseous (FDI). The decrements of the APB, ADM, and FDI were negatively correlated with their amplitude of CMAPs respectively. The difference between the decrement ratio of the APB and ADM was negatively correlated with the division ratio (CMAPAPB/CMAPADM). The decrement ratio of APB and FDI was negatively correlated with their muscle strength. There was a mild correlation between decrement ratio of APB and disease course. There was no statistical significance in the decrement pattern of the three-hand muscles of ALS patients. There was statistical significance in decrement pattern between APB of ALS and LEMS patients.Conclusion: Dysfunction of neuromuscular transmission was found in hand muscles of ALS patients, APB was involved most significantly. BACKGROUND AND OBJECTIVE: This study determined the hand muscle strength parameters and reference values in healthy subjects using the Nicholas Manual Muscle Tester (NMMT) and Manual Muscle Test (MMT) and compared these methods. METHODS: The study was carried out with 200 (102 males, 98 females) healthy adults aged between 18–25 years. Muscle strength evaluations of the hand were performed to determine hand motor ability using both NMMT and MMT. RESULTS: The mean values of age, weight, height and body mass index (BMI) were 19.48 ± 0.95 years, 60.66 ± 8.82 kg, 165.30 ± 7.95 cm and 22.14 ± 2.25 kg/m 2 , respectively, in females, whereas the same values were found as 20.40 ± 1.34 years, 72.53 ± 9.45 kg, 176.20 ± 6.29 cm and 23.25 ± 2.90 kg/m 2 , respectively, in males. There was a statistically significant difference in all demographic data between genders (p < 0.05). Furthermore, correlation coefficient in NMMT was found to be between 0.503 and 0.954. However, there was no correlation between the MMT and NMMT results. CONCLUSION: The observations presented need to be taken into consideration for evaluate musculoskeletal problems and also can be used as reference values for evaluating treatment outcomes. Introduction The thoracic limbs of cats facilitate jumping and represent one of their main ways for pursuing and capturing prey. The main muscles and nerves involved in these activities are present in the region of the forearm and of the hand. The scant anatomical reference availabe on South American cats species justifies the present comparative study. Materials and Methods The forelimbs of wildcat, ocelot and jaguar wild felines were fixed. Images of the dissected limbs were captured using a digital camera. Measurements were made using a caliper. Results The long and short heads of the extensor carpi radialis muscle of the ocelot and of the jaguar showed a great development in comparison with those of the wildcat. The flexor digitorum profundus muscle in the three felines is formed by five heads. In the jaguar, the radial or deep head presented two sesamoid bones. The brachioradialis muscle of the jaguar and of the ocelot is insertedmedially at the distal end of the radius and at the proximal row of the carpus by a thick and flattened tendon. The pronator teres muscle of the jaguar extended to the carpal region. In the wildcat and in the ocelot, this muscle was less developed Conclusions The main variations observed between the forearms and the hands of these South American cats were found between the supinator and the pronator muscles, presenting a variation in the size of their bellies and tendons. How changes in anatomy affect joint biomechanics can be studied using musculoskeletal modelling, making it a valuable tool to explore joint function in healthy and pathological joints. However, gathering the anatomical, geometrical and physiological data necessary to create a model can be challenging. Very few integrated datasets exist and even less raw data is openly available to create new models. Therefore, the goal of the present study is to create an integrated digital forearm and make the raw data available via an open‐access database. An un‐embalmed cadaveric arm was digitized using 7T MRI and CT scans. 3D geometrical models of bones, cartilage, muscle and muscle pathways were created. After MRI and CT scanning, physiological muscle parameters (e.g. muscle volume, mass, length, pennation angle, physiological cross‐sectional area, tendon length) were obtained via detailed dissection. After dissection, muscle biopsies were fixated and confocal microscopy was used to visualize and measure sarcomere lengths. This study provides an integrated anatomical dataset on which complete and accurate musculoskeletal models of the hand can be based. By creating a 3D digital human forearm, including all relevant anatomical parameters, a more realistic musculoskeletal model can be created. Objective This study was to apply the newly developed electrical impedance myography (EIM) technique to examine hand muscles in patients with an incomplete cervical spinal cord injury (SCI). Methods EIM was performed on the thenar, hypothenar, and first dorsal interosseous (FDI) muscles of SCI ( n = 16) and age-matched healthy control ( n = 18) subjects. By sending low intensity and high frequency current through the skin and measuring the consequent voltage, EIM estimates the major impedance parameters, which include resistance ( R ), reactance ( X ) and phase angle ( θ ). Results The SCI group had lower reactance and phase angle ( p < 0.0001) as compared to the control group in all three muscles, and lower resistance in the thenar muscle ( p < 0.05). The SCI group also demonstrated a smaller anisotropy in resistance ( p < 0.0001) and larger anisotropy in phase angle ( p < 0.05) compared to those from healthy controls. Conclusion The reduced reactance and phase angle of paralyzed muscles could be due to changes of membrane integrity and fat infiltration, whereas the change in the anisotropy may reflect the rearrangement of muscle fiber geometry. Variations in muscle attachments are valuable in interpreting rare clinical conditions. This case report discusses about the abnormal extensor indicis muscle attachment in left hand of male cadaver and its clinical relevance. The extensor indicis muscle was arising from lunate carpal bone and posterior radiocarpal ligament which further continued as short belly and had its distal attachment on the proximal phalanx of the index finger. This muscular variation has been described in earlier studies with 1.0% incidence. Abnormal origin of extensor indicis may be associated with pain and swelling at the back of the hand. This pain may be confused with other clinical conditions like synovial cyst and ganglion. Knowledge of such rare variations are useful to clinicians, surgeons, occupational and physical therapists for planning better surgical decisions.

The objective of this study was to apply a novel multidimensional motor unit number index (MD-MUNIX) technique to examine hand muscles in patients with incomplete cervical spinal cord injury (SCI). The MD-MUNIX was estimated from the compound muscle action potential (CMAP) and different levels of surface interference pattern electromyogram (EMG) at multiple directions of voluntary isometric muscle contraction. The MD-MUNIX was applied in the first dorsal interosseous (FDI), thenar and hypothenar muscles of SCI (n=12) and healthy control (n=12) subjects. The results showed that the SCI subjects had significantly smaller CMAP and MD-MUNIX in all the three examined muscles, compared to those derived from the healthy control subjects. The multidimensional motor unit size index (MD-MUSIX) demonstrated significantly larger values for the FDI and hypothenar muscles in SCI subjects than those from healthy control subjects, whereas the MD-MUSIX enlargement was marginally significant for the thenar muscles. The findings from the MD-MUNIX analyses provide an evidence of motor unit loss in hand muscles of cervical SCI patients, contributing to hand function deterioration.

Corticospinal excitability in the hand muscles decreases during smooth pursuit eye movement. The present study tested a hypothesis that the decrease in corticospinal excitability in the hand muscles at rest during eye movement is not caused by visual feedback but caused by motor commands to the eye muscles. Healthy men (M age=28.4 yr., SD=5.2) moved their eyes to the right with visual occlusion (dark goggles) while their arms and hands remained at rest. The motor-evoked potential in the hand muscles was suppressed by 19% in the third quarter of the eye-movement period, supporting a view that motor commands to the eye muscles are the cause of the decrease in corticospinal excitability in the hand muscles. The amount of the suppression was not significantly different among the muscles, indicating that modulation of corticospinal excitability in one muscle induced by eye movement is not dependent on whether eye movement direction and the direction of finger movement when the muscle contracts are identical. Thus, the finding failed to support a hypothetical view that motor commands to the eye muscles concomittantly produce motor commands to the hand muscles. Moreover, the amount of the suppression was not significantly different between the forearm positions, indicating that the suppression was not affected by proprioception of the forearm muscles when visual feedback is absent. Objective To investigate the effects of voluntary deep breathing on the excitability of the hand area in the primary motor cortex (M1). Methods We applied near-threshold transcranial magnetic stimulation (TMS) over M1 during the early phase of inspiration or expiration in both normal automatic and voluntary deep, but not “forced”, breathing in eight healthy participants at rest. We monitored exhaled CO 2 levels continuously, and recorded motor-evoked potentials (MEPs) simultaneously from the abductor pollicis brevis, first dorsal interosseous, abductor digiti minimi, flexor digitorum superficialis, and extensor incidis muscles. Results We observed that, during voluntary deep breathing, MEP amplitude increased by up to 50% for all recorded muscles and the latency of MEPs decreased by approximately 1 ms, compared with normal automatic breathing. We found no difference in the amplitude or latency of MEPs between inspiratory and expiratory phases in either normal automatic or voluntary deep breathing. Conclusions Voluntary deep breathing at rest facilitates MEPs following TMS over the hand area of M1, and MEP enhancement occurs throughout the full respiratory cycle. Abstract: Intermuscular coupling has been investigated to understand neural inputs to coordinate muscles in a motor performance. However, little is known on the role of nerve innervation on intermuscular coupling. The purpose of this study was to investigate how the anatomy of nerve distribution affected intermuscular coupling in the hand during static grip. Electromyographic (EMG) signals were recorded from intrinsic and extrinsic muscles while subjects performed a static grip. Coherence was computed for muscle pairs innervated by either the same or different nerves. The results did not support the hypothesis that muscles sharing the same nerve exhibit greater coupling than muscles innervated by different nerves. In general, extrinsic muscle pairs displayed higher coherence than intrinsic pairs. The results suggest that intermuscular coupling in a voluntary motor task is likely modulated in a functional manner and that different nerves might transport common neural inputs to functionally coupled muscles. Abstract: Electrical stimulation (ES) of injured peripheral nerves accelerates axonal regeneration in laboratory animals. However, clinical applicability of this intervention has never been investigated in human subjects. The aim of this pilot study was to determine the effect of ES on axonal regeneration after surgery in patients with median nerve compression in the carpal tunnel causing marked motor axonal loss. A randomized control trial was conducted to provide proof of principle for ES-induced acceleration of axon regeneration in human patients. Carpel tunnel release surgery (CTRS) was performed and in the stimulation group of patients, stainless steel electrode wires placed alongside the median nerve proximal to the surgical decompression site for immediate 1 h 20 Hz bipolar ES. Subjects were followed for a year at regular intervals. Axonal regeneration was quantified using motor unit number estimation (MUNE) and sensory and motor nerve conduction studies. Purdue Pegboard Test, Semmes Weinstein Monofilaments, and Levine''s Self-Assessment Questionnaire were used to assess functional recovery. The stimulation group had significant axonal regeneration 6–8 months after the CTRS when the MUNE increased to 290±140 (mean±SD) motor units (MU) from 150±62 MU at baseline (p <0.05). In comparison, MUNE did not significantly improve in the control group (p >0.2). Terminal motor latency significantly accelerated in the stimulation group but not the control group (p >0.1). Sensory nerve conduction values significantly improved in the stimulation group earlier than the controls. Other outcome measures showed a significant improvement in both patient groups. Molecular evidence indicates that the last common ancestor of the genus Pan and the hominin clade existed between 8 and 4 million years ago (Ma). The current fossil record indicates the Pan-Homo last common ancestor existed at least 5 Ma and most likely between 6 and 7 Ma. Together, the molecular and fossil evidence has important consequences for interpreting the evolutionary history of the hand within the tribe Hominini (hominins). Firstly, parsimony supports the hypothesis that the hand of the last common ancestor most likely resembled that of an extant great ape overall ( Pan, Gorilla, and Pongo), and that of an African ape in particular. Second, it provides a context for interpreting the derived changes to the hand that have evolved in various hominins. For example, the Australopithecus afarensis hand is likely derived in comparison with that of the Pan– Homo last common ancestor in having shorter fingers relative to thumb length and more proximo-distally oriented joints between its capitate, second metacarpal, and trapezium. This evidence suggests that these derived features evolved prior to the intensification of stone tool-related hominin behaviors beginning around 2.5 Ma. However, a majority of primitive features most likely present in the Pan- Homo last common ancestor are retained in the hands of Australopithecus, Paranthropus/early Homo, and Homo floresiensis. This evidence suggests that further derived changes to the hands of other hominins such as modern humans and Neandertals did not evolve until after 2.5 Ma and possibly even later than 1.5 Ma, which is currently the earliest evidence of Acheulian technology. The derived hands of modern humans and Neandertals may indicate a morphological commitment to tool-related manipulative behaviors beyond that observed in other hominins, including those (e.g. H. floresiensis) which may be descended from earlier tool-making species. The aim of this study was to compare the extent of central fatigue in the first dorsal interosseous (FDI) muscle of healthy adults in low, moderate and high-force submaximal contractions. Nine healthy adults completed four experimental sessions where index finger abduction force was recorded during voluntary contractions and in response to brief trains (five pulses at 100 Hz) of electrical stimulation. The ability to maximally activate FDI under volition, or voluntary activation, and its change with sustained activity (central fatigue) was assessed using the twitch interpolation technique. The fatigue tasks consisted of continuous isometric index finger abduction contractions held until exhaustion at four target force levels: 30%, 45%, 60% and 75% of the maximal voluntary contraction. The main finding was the presence of central fatigue for the 30% task, but not for the three other fatigue tasks. The extent of central fatigue was also associated with changes in a measure reflecting the status of peripheral structures/mechanisms. It appears that central fatigue contributed to task failure for the lowest force fatigue task (30%), but not for the other (higher) contraction intensities. [ABSTRACT FROM AUTHOR]

Electrical stimulation of the digital nerves can cause short- and long-latency increases in electromyographic activity (EMG) of the hand muscles, but mechanical stimulation of primarily tactile afferents in the digits generally evokes only a long-latency increase in EMG. To examine whether such stimuli can elicit short-latency reflex responses, we recorded EMG over the first dorsal interosseous muscle when subjects (n=13) used the tip of the right index finger to restrain a horizontally oriented plate from moving when very brisk tangential forces were applied in the distal direction. The plate was subjected to ramp-and-hold pulling loads at two intensities (a 1-N load applied at 32 N/s or a 2-N load applied at 64 N/s) at times unpredictable to the subjects (mean interval 2 s; trial duration 500 ms). The contact surface of the manipulandum was covered with rayon—a slippery material. For each load, EMG was averaged for 128 consecutive trials with reference to the ramp onset. In all subjects, an automatic increase in grip force was triggered by the loads applied at 32 N/s; the mean onset latency of the EMG response was 59.8±0.9 (mean ± SE) ms. In seven subjects (54%) this long-latency response was preceded by a weak short-latency excitation at 34.6±2.9 ms. With the loads applied at 64 N/s, the long-latency response occurred slightly earlier (58.9±1.7 ms) and, with one exception, all subjects generated a short-latency EMG response (34.9±1.3 ms). Despite the higher background grip force that subjects adopted during the stronger loads (4.9±0.3 N vs 2.5±0.2 N), the incidence of slips was higher—the manipulandum escaped from the grasp in 37±5% of trials with the 64 N/s ramps, but in only 18±4% with the 32-N/s ramps. The deformation of the fingertip caused by the tangential load, rather than incipient or overt slips, triggered the short-latency responses because such responses occurred even when the finger pad was fixed to the manipulandum with double-sided adhesive tape so that no slips occurred. Transcranial magnetic stimuli at different stimulus intensities were applied in six healthy subjects to test the hypothesis that, in different intrinsic hand muscles, the duration of the resultant cortically evoked silent periods (C-SPs) from each stimulus would be positively correlated between muscles, indicating a common inhibitory mechanism. A figure-of-eight coil discharging through a Magstim 200 stimulator delivered 25 stimuli at each stimulus intensity at a minimum of five intensities ranging from 55% to 160% of the individual resting motor threshold. In each subject, simultaneous surface recordings from pairs of muscles were made from the first dorsal interosseous (FDI), opponents pollicis (OP), abductor pollicis brevis (APB) and abductor digiti minimi (ADM). The C-SP durations within all three muscle pairs tested were highly correlated ( P<0.001). The amplitude of the preceding compound-muscle action potentials (CMAPs) was positively correlated between FDI and OP, but not between APB and ADM or FDI and ADM. C-SP duration was linearly related to stimulus intensity, but did not correlate with the latency or amplitude of the preceding CMAP. SPs elicited by peripheral nerve stimuli in pairs of hand muscles did not co-vary significantly. The results provide evidence that inhibitory influences of cortical origin are distributed widely to intrinsic hand muscles. The reflex responses to brisk, ramp stretch perturbations of the human flexor pollicis longus muscle (FPL) were recorded during isometric and slow concentric or eccentric contractions at similar levels of muscle excitation. The subjects flexed their thumb to push down against a thumb-rest, whose position was controlled by a servo-controlled motor. In different runs, the stretch perturbations were imposed when the thumb-rest was stationary (isometric) or was flexing or extending the interphalangeal joint of the thumb at a constant velocity, i.e. during concentric or eccentric contractions of FPL. The latency of the most prominent component of the electromyographic reflex in the isometrically contracting muscle was about 60 ms, measured from the command signal. The amplitude of this response was sharply reduced during the non-isometric contractions. While not dependent on the direction, this modulation of the reflex response increased with the speed of active movement of the interphalangeal joint (flexion or extension). The response was greatly reduced during concentric or eccentric movements as slow as 1.6 mm·s–1 (approximately 5°·s–1 at the joint). When the force rather than the position of the thumb-rest was servo-controlled, the stretch response to perturbation again diminished with speed in a self-paced flexion task, compared with an isometric “hold” condition. A new technique is described for recording the activity of single motor units in human or monkey hand muscles. A pair of microwire electrodes is introduced into the muscle using a fine needle. After insertion, the needle can be completely removed, leaving the recording microwires in situ. The method allows stable recording of a motor unit during natural movement of the hand and fingers. The identity of a given single motor unit was reflected in the form and amplitude of the motor unit-triggered average (MU-TA), derived by averaging the unrectified surface EMG recorded from the muscle with discharges of the motor unit. The MU-TA of a given unit remained constant despite variations in the form and size of its action potential. Inspection of successive MU-TAs increased confidence that records were taken from one and the same unit over long recording periods. Control experiments in human first dorsal interosseous showed that the peak-to-peak amplitude of the MU-TA was highly correlated with both the twitch force ( r= 0.65-0.92, mean 0.82, six subjects) and force threshold ( r=0.62-0.93, mean 0.83) of a given unit. Similar findings were obtained for human abductor pollicis brevis (AbPB) motor units. In the monkey, AbPB motor units which were recruited early in a precision grip task and which discharged steadily during the grip had smaller MU-TAs than laterecruited, phasic units. The combination of methods described in this paper enable a single motor unit to be identified and recognised. The relative size of the unit, which is an important parameter in most motor unit studies, can be reliably estimated from the amplitude of the MU-TA. Variations in muscle attachments are valuable in interpreting rare clinical conditions. This case report discusses about the abnormal extensor indicis muscle attachment in left hand of male cadaver and its clinical relevance. The extensor indicis muscle was arising from lunate carpal bone and posterior radiocarpal ligament which further continued as short belly and had its distal attachment on the proximal phalanx of the index finger. This muscular variation has been described in earlier studies with 1.0% incidence. Abnormal origin of extensor indicis may be associated with pain and swelling at the back of the hand. This pain may be confused with other clinical conditions like synovial cyst and ganglion. Knowledge of such rare variations are useful to clinicians, surgeons, occupational and physical therapists for planning better surgical decisions. [ABSTRACT FROM AUTHOR]

Corticospinal excitability in the hand muscles decreases during smooth pursuit eye movement. The present study tested a hypothesis that the decrease in corticospinal excitability in the hand muscles at rest during eye movement is not caused by visual feedback but caused by motor commands to the eye muscles. Healthy men (M age = 28.4 yr., SD = 5.2) moved their eyes to the right with visual occlusion (dark goggles) while their arms and hands remained at rest. The motor-evoked potential in the hand muscles was suppressed by 19% in the third quarter of the eye-movement period, supporting a view that motor commands to the eye muscles are the cause of the decrease in corticospinal excitability in the hand muscles. The amount of the suppression was not significantly different among the muscles, indicating that modulation of corticospinal excitability in one muscle induced by eye movement is not dependent on whether eye movement direction and the direction of finger movement when the muscle contracts are identical. Thus, the finding failed to support a hypothetical view that motor commands to the eye muscles concomittantly produce motor commands to the hand muscles. The objective of this study was to apply a novel multidimensional motor unit number index (MD-MUNIX) technique to examine hand muscles in patients with incomplete cervical spinal cord injury (SCI). The MD-MUNIX was estimated from the compound muscle action potential (CMAP) and different levels of surface interference pattern electromyogram (EMG) at multiple directions of voluntary isometric muscle contraction. The MD-MUNIX was applied in the first dorsal interosseous (FDI), thenar and hypothenar muscles of SCI (n = 12) and healthy control (n = 12) subjects. The results showed that the SCI subjects had significantly smaller CMAP and MD-MUNIX in all the three examined muscles, compared to those derived from the healthy control subjects. The multidimensional motor unit size index (MD-MUSIX) demonstrated significantly larger values for the FDI and hypothenar muscles in SCI subjects than those from healthy control subjects, whereas the MD-MUSIX enlargement was marginally significant for the thenar muscles. The findings from the MD-MUNIX analyses provide an evidence of motor unit loss in hand muscles of cervical SCI patients, contributing to hand function deterioration.
