Abstract Autophagy is a conserved, catabolic process essential for maintaining cellular homeostasis. Malfunctional autophagy contributes to neurodevelopmental and neurodegenerative diseases. However, the exact role and targets of autophagy in human neurons remain elusive. Here we report a systematic investigation of neuronal autophagy targets through integrated proteomics. Deep proteomic profiling of multiple autophagy-deficient lines of human induced neurons, mouse brains, and brain LC3-interactome reveals roles of neuronal autophagy in targeting proteins of multiple cellular organelles/pathways, including endoplasmic reticulum (ER), mitochondria, endosome, Golgi apparatus, synaptic vesicle (SV) for degradation. By combining phosphoproteomics and functional analysis in human and mouse neurons, we uncovered a function of neuronal autophagy in controlling cAMP-PKA and c-FOS-mediated neuronal activity through selective degradation of the protein kinase A - cAMP-binding regulatory (R)-subunit I (PKA-RI) complex. Lack of AKAP11 causes accumulation of the PKA-RI complex in the soma and neurites, demonstrating a constant clearance of PKA-RI complex through AKAP11-mediated degradation in neurons. Our study thus reveals the landscape of autophagy degradation in human neurons and identifies a physiological function of autophagy in controlling homeostasis of PKA-RI complex and specific PKA activity in neurons.

Introduction: In recent decades, there has been a growing focus on addressing social needs in healthcare settings. California has been at the forefront of making state-level investments to improve care for patients with complex social and medical needs, including patients experiencing homelessness (PEH). Examples include Medicaid 1115 waivers such as the Whole Person Care pilot program and California Advancing and Innovating Medi-Cal (CalAIM). To date, California is also the only state to have passed a legislative mandate to address concerns related to the hospital discharge of PEH who lack sufficient resources to support self-care. To this end, California enacted Senate Bill 1152 (SB 1152), a unique legislative mandate that requires hospitals to standardize comprehensive discharge processes for PEH by providing (and documenting the provision of) social and preventive services. Understanding the implementation and impact of this law will help inform California and other states considering legislative investments in healthcare activities to improve care for PEH. Methods: To understand health system stakeholders’ perceived impact of SB 1152 on hospital discharge processes and key barriers and facilitators to SB 1152’s implementation, we conducted 32 semi-structured interviews with key informants across 16 general acute care hospitals in Humboldt and Los Angeles counties. Study data were coded and analyzed using thematic analysis informed by the Consolidated Framework for Implementation Research. Results: Participants perceived several positive impacts of SB 1152, including streamlined services, increased accountability, and more staff awareness about homelessness. In parallel, participants also underscored concerns about the law’s limited scope and highlighted multiple implementation challenges, including lack of clarity about accountability measures, scarcity of implementation supports, and gaps in community resources. Conclusion: Our findings suggest that SB 1152 was an important step toward the goal of more universal safe discharge of PEH. However, there are also several addressable concerns. Recommendations to improve future legislation include adding targeted funding for social care staff and improving implementation training. Participants’ broader concerns about the parallel need to increase community resources are more challenging to address in the immediate term, but such changes will also be necessary to improve the overall health outcomes of PEH.

Abstract Objective Protocols to evaluate for myocardial infarction (MI) using high‐sensitivity cardiac troponin (hs‐cTn) have the potential to drive costs upward due to the added sensitivity. We performed an economic evaluation of an accelerated protocol (AP) to evaluate for MI using hs‐cTn to identify changes in costs of treatment and length of stay compared with conventional testing. Methods We performed a planned secondary economic analysis of a large, cluster randomized trial across nine emergency departments (EDs) from July 2020 to April 2021. Abstract Huntington’s disease (HD) is caused by a CAG repeat expansion in the huntingtin (HTT) gene. CRISPR-Cas9 nuclease causes double-strand breaks (DSBs) in the targeted DNA that induces toxicity, whereas CRISPR interference (CRISPRi) using dead Cas9 (dCas9) suppresses the target gene expression without DSBs. Delivery of dCas9-sgRNA targeting CAG repeat region does not damage the targeted DNA in HEK293T cells containing CAG repeats. When this study investigates whether CRISPRi can suppress mutant HTT (mHTT), CRISPRi results in reduced expression of mHTT with relative preservation of the wild-type HTT in human HD fibroblasts. Although both dCas9 and Cas9 treatments reduce mHTT by sgRNA targeting the CAG repeat region, CRISPRi delays behavioral deterioration and protects striatal neurons against cell death in HD mice. Collectively, CRISPRi can delay disease progression by suppressing mHtt, suggesting DNA DSB-free CRISPRi is a potential therapy for HD that can compensate for the shortcoming of CRISPR-Cas9 nuclease.

Residential energy trading systems (RETS) enable homeowners with distributed energy resources (DERs) to participate in virtualized energy markets that have the potential to reduce the peak demand of residential communities. Blockchains are key enablers of RETS, by virtue of providing a decentralized, self-governed network that mitigates concerns regarding privacy and transparency. However, more real-world case studies are needed to evaluate the techno-economic viability of blockchain-based RETS to improve their positive uptake. Thus, this article develops a permissioned blockchain-based RETS, which enables homeowners to select bidding strategies that consider the individual preferences of their DERs, and further evaluates the impact of the bidding strategies on reducing the peak demand of the community. The proposed system is implemented on the permissioned Hyperledger Fabric platform, where a decentralized ledger is used to store all energy bids, and a smart contract is used to execute a double auction mechanism and dispatch the homeowner DERs. The proposed system is validated by conducting simulations on a 8-home community using real-world data, and also by deploying the system to a Canadian microgrid, where the smart contract execution time is benchmarked. Simulation results demonstrate the efficacy of the proposed system by achieving a peak demand reduction of up to 48 kW (62%), which leads to an average savings of $1.02 M for the distribution system operator by avoiding transformer upgrades. Also, the simulation results show that the execution time of the proposed smart contract is 17.12 seconds across 12 nodes, which is sufficient for RETS.

Islet amyloid polypeptide (IAPP) deposition is associated with islet cell loss in diabetes. Here the authors show that a small molecule autophagy enhancer reduces IAPP accumulation in vitro, and also improves glucose tolerance in hIAPP + mice fed high-fat diet, accompanied by reduced hIAPP accumulation, in vivo.

X-linked adrenoleukodystrophy (ALD) caused by the ABCD1 mutation, is the most common inherited peroxisomal disease. Previously, we generated an ALD patient-derived SCHi001-A iPSC model. In this study, we have performed the first genome editing of ALD patient-derived SCHi001-A iPSCs using homology-directed repair (HDR). The mutation site, c.1534G > A [GenBank: NM_000033.4], was corrected by introducing ssODN and the CRISPR/Cas9 system. The cell line exhibited normal iPSC plulipotency marker expression following genome editing. Mutation-corrected iPSCs from SCHi001-A iPSC line can be used in research into the pathophysiology of and therapeutics for ALD.

Evidence suggests that physical activity and alcohol use are positively related among young adults. Two studies have examined daily relations, and results have shown conflicting findings. We examined relations between physical activity and alcohol use at both within- and between-individual levels and investigated moderators of the relation at both levels. 269 college students wore accelerometers to collect physical activity data over a 2-week period. At the end of each day, they indicated whether or not they drank alcohol. Multilevel logistic regression indicated neither within- nor between-subject relations were statistically significant. Positive affect, negative affect, and drinking motives moderated these relations at the between-subject level. Contrary to previous research, we did not observe a relation between physical activity and alcohol use at the daily level. Here, we found two genomic safe harbor (GSH) candidates from chromosomes 3 and 8, based on large-scale population-based cohort data from 4,694 Koreans by CNV analysis. Furthermore, estimated genotype of these CNVRs was validated by quantitative real-time PCR, and epidemiological data examined no significant genetic association between diseases or traits and two CNVRs. After screening the GSH candidates by in silico approaches, we designed TALEN pairs to integrate EGFP expression cassette into human cell lines in order to confirm the functionality of GSH candidates in an in vitro setting. As a result, transgene insertion into one of the two loci using TALEN showed robust transgene expression comparable to that with an AAVS1 site without significantly perturbing neighboring genes. Changing the promoter or cell type did not noticeably disturb this trend. Thus, we could validate two CNVRs as a site for effective and safe transgene insertion in human cells. As technology revolutionizes the methods of both production and communication, economists have to respond to new market structures and efficiencies, and the controversial concept of decentralization emerging in recent decades should also be examined in terms of its capacity to induce structural changes in the economy. This paper takes look at one specific market of interest and models the decentralization of a hypothetical emission quota market with the inclusion of households though automated auctions, in order to provide a preliminary analysis of how the redistribution of emission quotas would impact short-run equilibrium in this market and long-run growths. Given the endogenous dynamics, we would also examine the effects of exogenous technological shocks. For the purpose and scope of this paper, a simple mathematical formulation based on the Solow growth model as well as certain strong assumptions derived from economic intuitions is established, aiming to provide a more quantitative illustration of the effects from decentralization on such a system.

Transactive energy systems (TES) are modern mechanisms in electric power systems that allow disparate control agents to utilise distributed generation units to engage in energy transactions and provide ancillary services to the grid. Although voltage regulation is a crucial ancillary grid service within active distribution networks (ADNs), previous work has not adequately explored how this service can be offered in terms of its incentivisation, contract auditability, and enforcement. Blockchain technology shows promise in being a key enabler of TES, allowing agents to engage in trustless, persistent transactions that are both enforceable and auditable. To that end, this study proposes a blockchain based TES that enables agents to receive incentives for providing voltage regulation services by (i) maintaining an auditable reputation rating for each agent that is increased proportionately with each mitigation of a voltage violation, (ii) utilising smart contracts to enforce the validity of each transaction and penalise reputation ratings in case of a mitigation failure, and (iii) automating the negotiation and bidding of agent services by implementing the contract net protocol as a smart contract. Experimental results on both simulated and real-world ADNs are executed to demonstrate the efficacy of the proposed system.

Primary Sjögren's syndrome (pSS) is an autoimmune disease with increased risk of infections. Here, we assessed whether pSS patients were at higher risk of hospitalization for community and opportunistic infections. We selected newly hospitalized pSS patients between 2011 and 2018, through a nationwide population-based retrospective study using the French Health insurance database. We compared the incidence of hospitalization for several types of infections (according to International Classification for Disease codes, ICD-10) between pSS patients and an age- and sex-matched (1:10) hospitalized control group. We calculated adjusted Hazard Ratios (aHR, 95% CI) adjusted on socio-economic status, past cardiovascular or lung diseases and blood malignancies factors. We compared 25 661 pSS patients with 252 543 matched patients. The incidence of hospitalizations for a first community infection was increased in pSS patients [aHR of 1.29 (1.22–1.31), p <.001]. The incidence of hospitalization for bronchopulmonary infections was increased in pSS patients [aHR of 1.50 (1.34–1.69), p <.001, for pneumonia]. Hospitalizations for pyelonephritis and intestinal infections were increased [aHR of 1.55 (1.29–1.87), p <.001 and 1.18 (1.08–1.29), p <.001, respectively]. Among opportunistic infections, only zoster, and mycobacteria infections (tuberculosis and non-tuberculous) were at increased risk of hospitalization [aHR of 3.32 (1.78–6.18), p <.001; 4.35 (1.41–13.5), p =.011 and 2.54 (1.27–5.06), p =.008, respectively]. pSS patients are at higher risk of hospitalization for infections. The increased risk of hospitalization for mycobacterial infections illustrates the potential bilateral relationship between the two conditions. Vaccination against respiratory pathogens and herpes zoster virus may help prevent some hospitalizations in pSS patients. Primary Sjögren's syndrome (pSS) increases hospitalization risk for community infections: bronchopulmonary, skin, dental, ear–nose–throat, intestinal infections and pyelonephritis. Hospitalizations for zoster and mycobacterial infections are also increased in this population. Lupus nephritis (LN) has been described as having worse survival and renal outcomes in African-descent patients than Caucasians. We aimed to provide long-term population-based data in an Afro-descendant cohort of LN with high income and easy and free access to specialized healthcare. Study design: We performed a retrospective population-based analysis using data from 2002–2015 of 1140 renal biopsies at the University Hospital of Martinique (French West Indies). All systemic lupus erythematosus patients with a diagnosis of LN followed for at least 12 months in Martinique or who died during this period were included. Results: A total of 89 patients were included, of whom 68 (76.4%) had proliferative (class III or IV), 17 (19.1%) had membranous (class V), and 4 (4.5%) had class I or II lupus nephritis according to the ISN/RPS classification. At a mean follow-up of 118.3 months, 51.7% of patients were still in remission. The rates of end-stage renal disease were 13.5%, 19.1%, and 21.3% at 10, 15, and 20 years of follow-up, respectively, and mortality rates were 4.5%, 5.6%, and 7.9% at 10, 15, and 20 years of follow-up, respectively. Conclusions: The good survival of our Afro-descendant LN patients, similar to that observed in Caucasians, shades the burden of ethnicity but rather emphasizes and reinforces the importance of optimizing all modifiable factors associated with poor outcome, especially socioeconomics. The results of a functional analysis of problem behavior and a paired-choice assessment were compared to determine whether the same social reinforcers were identified for problem behavior and an appropriate response (time allocation). The two assessments were conducted in classroom settings with 4 adolescents with mental retardation who engaged in severe problem behavior. Each student's classroom teacher served as the therapist for all phases of assessment. The two assessment procedures identified the same social reinforcers for problem and appropriate behavior for 3 of 4 participants. This paper illustrates two types of pitfalls when linearization methods are improperly applied. First, if we linearize the constraints before deriving the optimality conditions, the derived conditions are not correct up to first order. Second, even when we obtain the behavior of the economy that is correct to the first order, applying this behavior to welfare implications may lead to incorrect results. Introduction: Patterns of physical activity (PA) and time in bed (TIB) across the 24-hr cycle have important implications for many health outcomes; therefore, wearable accelerometers are often implemented in behavioral research to measure free-living PA and TIB. Two accelerometers, the activPAL and Actiwatch, are common accelerometers for measuring PA (activPAL) and TIB (Actiwatch), respectively. Both accelerometers have the capacity to measure TIB, but the degree to which these accelerometers agree is not clear. Therefore, this study compared estimates of TIB between activPAL and the Actiwatch accelerometers. Methods: Participants (mean ± SDage = 39.8 ± 7.6 years) with overweight or obesity (N = 83) wore an activPAL and Actiwatch continuously for 7 days, 24 hr per day. TIB was assessed using manufacturer-specific algorithms. Repeated-measures mixed-effect models and Bland–Altman plots were used to compare the activPAL and Actiwatch TIB estimates. Results: Statistical differences between TIB assessed by activPAL versus Actiwatch (p <.001) were observed. There was not a significant interaction between accelerometer and day of wear (p =.87). The difference in TIB between accelerometers ranged from −72.9 ± 15.7 min (Day 7) to −98.6 ± 14.5 min (Day 3), with the Actiwatch consistently estimating longer TIB compared with the activPAL. Conclusion: Data generated by the activPAL and Actiwatch accelerometers resulted in divergent estimates of TIB. Advances in sequencing technologies have facilitated the discovery of previously unknown genetic variants in both inherited and acquired disorders, and tools to correct these pathogenic variants are rapidly evolving. Since the first introduction of CRISPR-Cas9 in 2012, the field of CRISPR-based genome editing has progressed immensely, giving hope to many patients suffering from genetic disorders that lack effective treatment. In this review, we will examine the basic principles of CRISPR-based genome editing, explain the mechanisms of new genome editors, including base editors and prime editors, and evaluate the therapeutic possibilities of CRISPR-based genome editing by focusing on recently published clinical trials and animal studies. Background: Cutaneous Leishmaniasis (CL) is endemic in French Guiana but cases are usually sporadic. An outbreak signal was issued on May 15th 2020 with 15 suspected cases after a military training course in the rainforest. An outbreak investigation was carried out. Methodology/Principal findings: Thirty cases were confirmed. Leishmania guyanensis was the most frequent species (90%). The most frequent presentation was ulcerative (90%). Lesions on the face and hands were frequent (40% each). Eight cases (26%) presented a poor outcome after treatment with pentamidine and required a second line with amphotericin B. Three of them required further treatments with meglumine antimoniate or miltefosine. Two spots within the training area were deemed as likely sites of contamination, due to illegal logging. The isolated Leishmania strains did not form a separate cluster. Participation in Week 13 of year 2020 was associated with infection (OR = 4.59 [1.10–19.83]; p = 0.016) while undergoing only the "Fighting" exercise was protective (OR = 0.1 [0–0.74]; p = 0.021). There was no association between infection and other risk factors at the individual level. The attack rate of Regiment B (14/105 = 13.3%) was significantly higher (OR = 4.22 [1.84–9.53], p = 0.0001) compared to Regiment A (16/507 = 3.2%). The attack rate during this training course (30/858 = 3.5%) was significantly higher (OR 2.29 [1.28–4.13]; p = 0.002) than for other missions in French Guiana during the same period (22/1427 = 1.5%). Conclusions: This outbreak could be explained by a combination of factors: climatic conditions around week 13, at-risk activities including night trainings, absence of impregnation, a lesser experience of rainforest duties in Regiment B and illegal logging attracting sandflies on military training grounds. Author summary: Cutaneous Leishmaniasis is caused by parasites of the Leishmania genus and infects humans after a sandfly bite. Outbreaks are rare and hard to investigate in isolated tropical areas. In this study, the authors explored the different possible origins of an outbreak of cutaneous leishmaniasis among soldiers training in the rainforest of French Guiana. The outbreak occurred in March 2020. Concerning the symptoms, several patients presented resistant infections and multiple lines of treatment, raising the issue of resistant Leishmania strains. The different strains isolated during the outbreak were not genetically closed, as far as routine PCR techniques would indicate. The authors looked for individual behaviours exposing soldiers to sandfly bites but none was significantly associated with infection. The authors found two spots in the military training areas where illegal logging probably increased the density of sandflies and put service members at risk. The 13th week of 2020 was associated to a higher risk of infection due to climatic conditions. The aim of the study was to evaluate the tooth extracted use as autologous tooth graft after endodontic root canal therapies used for socket preservation. To this purpose, the Tooth Transformer shredding and decontamination machine has been used. The graft obtained in this way, was inserted at the time of the extraction or at a second surgery altogether with the chosen regenerative therapy. This clinical trial enrolled patients with post-estractive defects requiring the restoration bone dimension and shape in the maxillary and mandibular zone. In addition, 98 patients with 119 extraction sockets were enrolled across 10 standardized centers. An innovative preparation method, using the dedicated automated device Tooth Transformer, able to transform autologous teeth in suitable grafting material, has been used. The extracted tooth was cleaned and treated using a Tooth Transformer and made a socket preservation. Thirteen Biopsies were realized to analyze the histologic outcomes at the average time of four months to demonstrate that the autologous tooth graft made from root after endodontic therapy should be used in human bone regeneration as graft for dental implant placement. Previous research suggests that more negative or less positive couple communication can be stressful and that chronic stress can lead to less healthy patterns of physiological stress functioning. Our goal was to investigate whether couples' observed communication behaviors and reported relationship conflict were related to diurnal cortisol patterns, an important indicator of hypothalamic–pituitary–adrenal axis functioning. Sixty-two couples (n = 124 individuals) reported marital conflict and were video-recorded engaging in a coded conflict discussion. Diurnal cortisol samples were collected. Results suggested that men's greater observed communication quality predicted women's higher awakening cortisol levels as well as men's steeper decreases in cortisol across the day (i.e., slopes), men's greater reported conflict predicted women's lower awakening levels, and, in some models, women's greater reported resolution predicted women's lower awakening levels and men's steeper slopes. These findings suggest that less positive and more negative marital conflict contribute to dysfunction of the hypothalamic–pituitary–adrenal axis. [ABSTRACT FROM AUTHOR]

Transplantation of mesenchymal stem cells (MSCs) has paracrine effects; however, the effects are known to be largely limited. Here we investigated the combination effects of cell transplantation and enriched environment (EE) in a model of hypoxic-ischemic brain injury. Brain damage was induced in seven-day-old mice by unilateral carotid artery ligation and exposure to hypoxia (8% O2 for 90 min). At six weeks of age, the mice were randomly assigned to four groups: phosphate-buffered saline (PBS)-control (CON), PBS-EE, MSC-CON, and MSC-EE. Rotarod and grip strength tests were performed to evaluate neurobehavioral functions. Histologic evaluations were also performed to confirm the extent of astrocyte activation and endogenous angiogenesis. An array-based multiplex ELISA andWestern blot were used to identify growth factors in vivo and in vitro. Two weeks after treatment, levels of astrocyte density and angiogenic factors were increased in MSC-EE mice, but glial scarring was not increased. Eight weeks after treatment, angiogenesis was increased, and behavioral outcomes were synergistically improved in the MSC-EE group. Astrocytes co-cultured with MSCs expressed higher levels of angiogenic factors than astrocytes cultured alone. The mechanisms of this synergistic effect included enhanced repair processes, such as increased endogenous angiogenesis and upregulation of angiogenic factors released from activated astrocytes. The article provides information on a study which evaluated the efficacy of corticosteroid injection in the treatment of Morton's neuroma. The study also identified the duration of any symptom-free period after treatment with a single ultrasound guided injection. The results of the study are highlighted, which suggest that a single corticosteroid injection after nine months is an effective and reasonable option in the non-surgical treatment of Morton's neuroma.

During currency crises, some currencies depreciate more than the post-crisis exchange rate level, which can be described as exchange rate overshooting. Previous studies have claimed that a tight monetary policy, represented by an increase in the interest rate, stabilizes an exchange rate by causing currency appreciation, thereby explaining overshooting. This paper tests the hypothesis that overshooting simply reflects the overreaction of investors due to financial panic during currency crises, regardless of subsequent domestic policies. Empirical results suggest that: (1) the positive relationship between monetary tightening and the overshooting measure is very sensitive to sample selection; and (2) the measure of financial panic has a significant and positive relationship with the measure of overshooting in non-European countries. In particular, we study two preference formulations: a time-separable preference formulation with a fixed discount factor, and a time-nonseparable preference structure with an endogenous discount factor. We analyze the moment implications of two versions of an otherwise identical open-economy model—one with a fixed discount factor and the other with an endogenous discount factor—and study impulse responses to productivity and world real-interest-rate shocks. Our results suggest that business-cycle implications of the two models are quite similar under conventional parameter values. Older adults constitute an increasingly large share of the workforce. Older workers often contribute positively to organizational outcomes through characteristics such as deep organizational knowledge and long-standing client relationships. Thus, it is important to understand how to maintain or increase older workers' job satisfaction, a variable that has been linked to positive work outcomes. In this study, several hypotheses regarding job satisfaction and age were derived from Carstensen's socioemotional selectivity theory and were tested using longitudinal analysis of a cross-sequential sample. Supporting socioemotional selectivity theory, results showed that autonomy became increasingly important to job satisfaction as workers age. Contrary to the theory, annual income also became increasingly important to job satisfaction. We discuss the importance of our findings for theory, research, and practice. [ABSTRACT FROM AUTHOR]

In conjunction with the government of Singapore’s vision to have efficient and extensive Mass Rapid Transit (MRT) system across Singapore Island, more and more rail tunnels are planned to be constructed in close proximity to sensitive buildings and infrastructures in recent years. This being a particular challenge for major infrastructure works in the urban environment. In contract C937 of Downtown Line Stage 3 (DTL3), twin bored tunnels connect Fort Canning station to Bencoolen Station, under Fort Canning Hill, with the alignment overcrossing the existing North East Line (NEL) rail tunnel, undercrossing North South Line (NSL) and Circle Line (CCL) with clear distance of less than one bored tunnel diameter. This inevitably poses great challenges to tunnelling over and under existing live tunnels without disruption to daily operation of MRT. Further compounding the challenge is the ground condition comprising of Jurong Formation steeply variable sedimentary deposits transitioning into Fort Canning Boulder Bed a colluvial deposit of quartzite boulders in a stiff clay matrix. To successfully negotiate this challenge, requires all involved parties to make contributions in their areas of expertise and work as team together to ensure that the works progress smoothly and as planned. The designer provides an overall approach demonstrating that the work can be safely carried out and provides key parameters for the contractor who carries out the tunnelling with construction control, which in turn is supervised by the supervision engineers ensuring work is carried out safely. The client, who provides the vision in the first place, has overall responsibility for the work providing continuity and ensuring that the component parts of the job are co-ordinated and carried out successfully. In addition, different monitoring levels were also established to reduce the train speed if necessary and also bus bridging service as a contingency measure. This paper summarizes the approach to the tunnelling, key tunnelling parameters during undercrossing of existing live MRT tunnels, instrumentation monitoring as well as measured railway track and structure movements due to proximity tunnelling. It reflects upon the on crucial contributory factors that ensure success within the project and serves as a reference as to how engineering boundaries can be challenged underground.
