Abstract Circulating genetically abnormal cells (CACs) have emerged as a promising biomarker for the early diagnosis of lung cancer, particularly in patients with pulmonary nodules. However, their performance may be suboptimal in certain patient populations. This study aimed to refine patient selection to improve the detection of CACs in pulmonary nodules. A retrospective analysis was conducted on 241 patients with pulmonary nodules who had undergone pathological diagnosis through surgical tissue specimens. Utilizing consensus clustering analysis, the patients were categorized into three distinct clusters. Cluster 1 was characterized by older age, larger nodule size, and a higher prevalence of hypertension and diabetes. Notably, the diagnostic efficacy of CACs in Cluster 1 surpassed that of the overall patient population (AUC: 0.855 vs. 0.689, P = 0.044). Moreover, for Cluster 1, an integrated diagnostic model was developed, incorporating CACs, sex, maximum nodule type, and maximum nodule size, resulting in a further improved AUC of 0.925 (95% CI 0.846–1.000). In conclusion, our study demonstrates that CACs detection shows better diagnostic performance in aiding the differentiation between benign and malignant nodules in older patients with larger pulmonary nodules and comorbidities such as diabetes and hypertension. Further research and validation are needed to explore how to better integrate CACs detection into clinical practice.

Abstract Chiral recognition of amino acids is very important in both chemical and life sciences. Although chiral recognition with luminescence has many advantages such as being inexpensive, it is usually slow and lacks generality as the recognition module relies on structural complementarity. Here, we show that one single molecular-solid sensor, L-phenylalanine derived benzamide, can manifest the structural difference between the natural, left-handed amino acid and its right-handed counterpart via the difference of room-temperature phosphorescence (RTP) irrespective of the specific chemical structure. To realize rapid and reliable sensing, the doped samples are obtained as nanocrystals from evaporation of the tetrahydrofuran solutions, which allows for efficient triplet-triplet energy transfer to the chiral analytes generated in situ from chiral amino acids. The results show that L-analytes induce strong RTP, whereas the unnatural D-analytes produce barely any afterglow. The method expands the scope of luminescence chiral sensing by lessening the requirement for specific molecular structures.

Abstract Aims Frequent and extensive within-day glycemic variability (GV) in blood glucose levels may increase the risk of hypoglycemia and long-term mortality in hospitalized patients with diabetes. We aimed to assess the amplitude and frequency of within-day GV in inpatients with type 2 diabetes and to explore the factors influencing within-day GV. Methods We conducted a single-center, retrospective observational study by analyzing hospital records and 10-day real-time continuous glucose monitoring data. Within-day GV was assessed using the coefficient of variation (%CV). The primary outcome was the amplitude and frequency of within-day GV. The frequency of within-day GV was assessed by the consecutive days (CD) of maintaining within the target %CV range after first reaching it (CD after first reaching the target) and the maximum consecutive days of maintaining within the target %CV range (Max-CD). The target %CV range was less than 24.4%. We evaluated the factors influencing within-day GV using COX regression and Poisson regression models. Results A total of 1050 cases were analyzed, of whom 86.57% reduced the amplitude of within-day GV before the sixth day of hospitalization. Of the 1050 hospitalized patients, 66.57% stayed within the target %CV range for less than two days after first reaching the target and 69.71% experienced a Max-CD of fewer than four days. Reducing the average postprandial glucose excursion (hazard ratio [HR]: 0.81, 95% confidence interval [CI]: 0.77–0.85; incidence rate ratios [IRR]: 0.72, 95% CI: 0.69–0.74) and the use of α-glucosidase inhibitors (IRR: 1.1, 95% CI: 1.01–1.18) and glucagon-like peptide-1 agonist (IRR: 1.30, 95% CI: 1.02–1.65) contributed to reducing the amplitude and decreasing the frequency of within-day GV. However, the use of insulin (HR: 0.64, 95% CI: 0.55–0.75; IRR: 0.86, 95% CI: 0.79–0.93) and glinide (HR: 0.47, 95% CI: 0.31–0.73; IRR: 0.84, 95% CI: 0.73–0.97) may lead to an increased frequency of within-day GV. Conclusions An increasing frequency of within-day GV was observed during the hospitalization in patients with type 2 diabetes, despite the effective reduction in the amplitude of within-day GV. Using medications designed to lower postprandial blood glucose could contribute to minimize the risk of frequent within-day GV.

Background and objective The SMARCA4 mutation has been shown to account for at least 10% of non-small cell lung cancer (NSCLC). In the present, conventional radiotherapy and targeted therapy are difficult to improve outcomes due to the highly aggressive and refractory nature of SMARCA4-deficient NSCLC (SMARCA4-DNSCLC) and the absence of sensitive site mutations for targeted drug therapy, and chemotherapy combined with or without immunotherapy is the main treatment. Effective SMARCA4-DNSCLC therapeutic options, however, are still debatable. Our study aimed to investigate the efficacy and prognosis of programmed cell death 1 (PD-1) immune checkpoint inhibitors (ICIs) in combination with chemotherapy and chemotherapy in patients with stage III-IV SMARCA4-DNSCLC. Methods 46 patients with stage III-IV SMARCA4-DNSCLC were divided into two groups based on their treatment regimen: the chemotherapy group and the PD-1 ICIs plus chemotherapy group, and their clinical data were retrospectively analyzed. Efficacy assessment and survival analysis were performed in both groups, and the influencing factors for prognosis were explored for patients with SMARCA4-DNSCLC. Results Male smokers are more likely to develop SMARCA4-DNSCLC. There was no significant difference in the objective response rate (76.5% vs 69.0%, P=0.836) between chemotherapy and the PD-1 ICIs plus chemotherapy or the disease control rate (100.0% vs 89.7%, P=0.286). The one-year overall survival rate in the group with PD-1 ICIs plus chemotherapy was 62.7%, and that of the chemotherapy group was 46.0%. The difference in median progression-free survival (PFS) between the PD-1 ICIs plus chemotherapy group and the chemotherapy group was statistically significant (9.3 mon vs 6.1 mon, P=0.048). The results of Cox regression analysis showed that treatment regimen and smoking history were independent influencing factors of PFS in patients with stage III-IV SMARCA4-DNSCLC, and family history was an individual influencing factor of overall survival in patients with stage III-IV SMARCA4-DNSCLC. Conclusion Treatment regimen may be a prognostic factor for patients with SMARCA4-DNSCLC, and patients with PD-1 ICIs plus chemotherapy may have a better prognosis.

Electric field measurement techniques based on Rydberg atoms have proven to have unique advantages in high sensitivity and have drawn wide attention in microwave electric field metrology and communication. The Rydberg states employed for microwave sensing are usually excited by two- or three-step laser irradiation. It leads to the Rydberg atoms being populated by some specific magnetic quantum number states determined by the laser polarization configurations, whose further coupling to another Rydberg state is also constrained by the microwave polarization. Based on a three-laser optical excitation, we demonstrate this effect on the microwave spectra of the 85Rb transitions 41F7/2 → 42D5/2 and 41F7/2 → 41G7/2 at various combining polarization configurations. It shows a strong polarization dependence on the microwave spectrum and then affects the sensitivity of Rydberg atom-based microwave field sensing. Advisable polarization schemes for higher sensitivity are suggested by the two types of transitions.

ObjectiveThe aim of this study is to report an isolated pleural cryptococcosis with pleural effusion as the only manifestation, confirmed by pleural biopsy in a patient with thymoma combined with myasthenia gravis, who developed pleural effusion of unknown origin after long-term glucocorticoids and tacrolimus therapy.MethodsPathological examination of the right pleural biopsy tissue from a patient with unexplained recurrent pleural effusion was implemented. Morphological analysis of the fungal component and metagenomic next-generation sequencing (mNGS) on the pleural tissue were performed.ResultsA biopsy specimen of the right pleura revealed numerous yeast-like organisms surrounded by mucous capsules and Cryptococcus neoformans was detected by mNGS with a species-specific read number (SSRN) of 4, confirming the diagnosis of pleural cryptococcosis. Pleural effusion was eliminated with amphotericin B and fluconazole, and healthy status was maintained at the time of review 1 year later.ConclusionCryptococcosis, manifested by simple pleural effusion, is extremely rare, but when repeated pleural effusion occurs in immunocompromised patients or in patients with malignant tumors, the possibility of cryptococcosis should be treated with high vigilance and pleural biopsy is recommended if necessary in order to confirm the diagnosis.

Massive sandstone without sedimentary structure is developed in the Ch6-Ch7 Section of Yanchang Formation in the Binchang Block, Southern Ordos Basin, which has a good oil and gas show, good productivities and good exploration and development prospect. However, the formation mechanism of the sandstone is still in dispute. It is of great significance to determine the sedimentary facies and sedimentary model of the sandstone in Ch6-Ch7 Section in order to analyze the formation mechanism of sweet spot in the low permeability sandstone reservoirs, to predict the sweet spot distribution model and guide the subsequent exploration and development. In this paper, 15 lithofacies and 3 main types of sedimentary microfacies, namely, sandy debris flow, turbidite and seismite slump microfacies, have been identified by using a large amount of core sedimentary description data, grain size analysis data and geological mapping analysis, and these three kinds of sedimentary assemblies in space. The deep water gravity flow deposition model in the Ch6-Ch7 Section can be summarized as the sublacustrine fan model and can be divided into three subfacies or assemblies: the upper fan subfacies dominated by the assembly of seismite-slump and sandy debris flow, the mid-fan subfacies dominated by the assembly of the sand debris flow-turbidite microfacies and the lower-fan subfacies dominated by turbidite flow-basin plain microfacies assembly.

Distributed fiber-optic seismic sensing is a next-generation seismic acquisition technology. It records dynamic strain based on the phase change of back-scattered Rayleigh light caused by incident seismic waves. This method generates a dense record of the seismic wavefield using optic fibers which are durable in extreme environments and have low maintenance costs. Consequently, it has been widely used for surveys of urban areas, the ocean bottom, boreholes, and glaciers. In this study, we focus on its applications in seismic imaging and the monitoring of seismic velocity change. Previous studies have employed both passive and active sources to explore structures ranging from near-surface depths to the Moho. Although impressive achievements have been made, the research community still faces challenges such as data quality, signal fidelity, storage, processing, and information mining of large data volumes. Therefore, active and passive high-resolution 4D imaging, instrument transfer function calculation, and effective big data mining should be conducted as part of future studies to achieve high-density and high-precision seismic imaging with this fiber-optic sensing technology.

ObjectiveThis study aimed to investigate the relationship between anti-MDA5 titer and type I IFN signature in patients with MDA5+ DM.MethodsWe explored the transcriptome profiling of PBMCs in MDA5+ DM patients with high-titer of antibody at disease onset or relapse and normal low-titer after treatment and healthy donors. Subsequently, we revealed the dynamic relationship between serum type I IFN scores and antibody titers.ResultDifferentially expressed genes in MDA5+ DM patients were enriched for related pathways and biological functions linked to viruses and cytokines compared to healthy donors. Similar differences remained pooled between the high-titer and low-titer group, and type I-specific interferon response genes showed upregulation in high-titer group. Significant correlations were found between anti-MDA5 titers and type I IFN scores (r = 0.50, P< 0.001). Contemporaneous anti-MDA5 titers revealed to be significantly higher in the group with ultra-high type I IFN scores (vs. high group, P = 0.027; vs. low group, P< 0.001). Longitudinal assessment of type I IFN scores and anti-MDA5 titers, including pre- and post-treatment changes at initial diagnosis and dynamic changes during treatment, presented an asynchrony between the two parameters in response to treatment.ConclusionAnti-MDA5 antibody titers correlated with type I IFN signature in patients with MDA5+ DM and they both changed dynamically but not synchronously over the course of treatment.

ObjectiveTo assess the diagnostic efficacy of metagenomic next generation sequencing (mNGS) for proven invasive pulmonary aspergillosis (IPA).MethodsA total of 190 patients including 53 patients who had been diagnosed with proven IPA were retrospectively analyzed. Using the pathological results of tissue biopsy specimens as gold standard, we ploted the receiver operating characteristic (ROC) curve to determine the optimal cut-off value of mNGS species-specific read number (SSRN) of Aspergillus in bronchoalveolar lavage fluid (BALF)for IPA. Furthermore, we evaluated optimal cut-off value of mNGS SSRN in different populations.ResultsThe optimal cut-off value of Aspergillus mNGS SSRN in BALF for IPA diagnosis was 2.5 for the whole suspected IPA population, and 1 and 4.5 for immunocompromised and diabetic patients, respectively. The accuracy of mNGS was 80.5%, 73.7% and 85.3% for the whole population, immunocompromised and diabetic patients, respectively.ConclusionsThe mNGS in BALF has a high diagnostic efficacy for proven IPA, superioring to Aspergillus culture in sputum and BALF and GM test in blood and BALF. However, the cut-off value of SSRN should be adjusted when in different population.

Background: Platinum-based chemotherapy is the main treatment for advanced lung squamous cell carcinoma (LUSC). Eventually, patients with LUSC develop resistance to cisplatin, which affects the prognosis. Hence, the researchers sought to find a lncRNA in LUSC that affects resistance to cisplatin. Methods: The lncRNA microarray assay was used to screen the differential expression of lncRNA. qPCR was used to detect lncRNA DSCAS (DSCAS) expression in tissues and cell lines. Lentiviral transfection was used to regulate the expression of DSCAS. CCK-8, colony formation, wound healing, transwell, and flow cytometry assays were used to assess the biological behaviors and sensitivity to cisplatin of LUSC cell. RNA-RNA interaction was tested using the dual luciferase reporting assay, RNA-IP, and RNA-RNA pull-down assay. The downstream pathway of DSCAS was verified by qPCR and Western blotting assays. Results: DSCAS was highly expressed in LUSC tissues and cells, and its expression levels were higher in cisplatin-insensitive tissues than in cisplatin-sensitive tissues. Elevation of DSCAS promoted cell proliferation, migration and invasion as well as increased cisplatin resistance of lung cancer cells, while demotion of DSCAS inhibited cell proliferation, migration and invasion as well as decreased the cisplatin resistance of lung cancer cells. DSCAS bound to miR-646-3p to regulate the expression of Bcl-2 and Survivin, which affected the cell apoptosis and sensitivity to cisplatin in LUSC cells. Conclusions: DSCAS regulates biological behavior and cisplatin sensitivity in LUSC cells by competitively binding to miR-646-3p to mediate the expression of Survivin and Bcl-2, known as apoptosis-related proteins.

Lonicera japonica (L. japonica) is utilized as foods and healthy drink around the world. Until now, the immunomodulatory activity of polysaccharides from L. japonica (LJP) is little studied. In our previous work, LJP was fractionated to a neutral component (LJP-N) and an acidic component (LJP-A) by a DEAE-Cellulose column, and LJP-N was a starch-like polysaccharide and LJP-A was a pectic polysaccharide. In this study, LJP-N and LJP-A were investigated for their immunomodulatory effect, and the protective effect of these two polysaccharide fractions against immune injury by cyclophosphamide (CTX) in BALB/c mice was evaluated. Results showed that when compared with CTX group, LJP fractions, especially for LJP-A (200 mg kg−1) can enhance the immune function, improve atrophy of the lymphoid organs thymus and spleen (the increased approximately 1.8- and 1.6-fold), increase the phagocytic function of macrophages (increased approximately 1.7-fold), increase the secretion of cytokines (the levels of IL-6, IL-2, and TNF-α increased approximately 2.5-, 2.0-, and 1.4-fold) and immunoglobulins levels (the levels of IgM and IgG increased approximately 1.2- and 1.7-fold) in serum, and enhance the cytotoxic activity of NK cells (increased approximately 3.5-fold). Taken together, the present results suggest that LJP-A, rich in uronic acid, with a molecular weight of 400 kDa, may be a potential candidate as the functional foods.

Width-thickness ratio was an important parameter for designing Concrete Filled Rectangular Steel Tube (CFRST). Welding longitudinal stiffener on the internal wall of steel pipe could delay the local buckling, which increased the limit of width-thickness ratio. If there was not enough stiffener and its sectional dimension was too small, the local buckling of steel pipe would occur, inducing its bearing capacity seriously. If the stiffener sectional dimension was too large, concrete filled in steel tube would be broken up, which reduces its bearing capacity. To solve that problem, this paper studied local buckling of CFRST with longitudinal stiffener under axial compression and design of longitudinal stiffener. It established buckling analysis model, simplified local buckling analysis as calculating buckling load of thin plate clamped on loading side and unloading side under axial force. It deduced buckling load and buckling coefficient based on the principle of energy. The results showed that buckling mode depended on stiffening rigidity. Therefore, it put forward minimum stiffening rigidity ratio that controlled the stiffener design. This paper also came up with a formula to calculate minimum stiffening rigidity ratio. It provided guidance on designing number, sectional dimension and material performance.

We have demonstrated a Rydberg atom-based two-band communication with the optically excited Rydberg state coupled to another pair of Rydberg states by two microwave fields, respectively. The initial Rydberg state is excited by a three-color electromagnetically-induced absorption in rubidium vapor cell via cascading transitions, with all of them located in infrared bands: a 780 nm laser servers as a probe to monitor the optical transmittancy via transition 5S1/2→5P3/2, 776 nm and 1260 nm lasers are used to couple the states 5P3/2 and 5D5/2 and states 5D5/2 and 44F7/2. Experimentally, we show that two channel communications carried on the two microwave transitions influence each other irreconcilably, so that they cannot work at their most sensitive microwave-optical conversion points simultaneously. For a remarkable communication quality for both channels, the two microwave fields both have to make concessions to reach a common microwave-optical gain. The optimized balance for the two microwave intensities locates at EMW1=6.5 mV/cm and EMW2=5.5 mV/cm in our case. This mutual exclusive influence is theoretically well-explained by an optical Bloch equation considering all optical and microwave field interactions with atoms.

We present a theoretical and experimental investigation of the improvement and determination of the sensitivity of Rydberg atom-based microwave RF sensor. An optical Bloch equation has been set up based on the configuration that two-color cascading lasers exciting atom to highly Rydberg state and a microwave RF coupling this Rydberg state to its adjacent neighbor. The numerical simulation shows that the sensitivity of the atomic RF sensor is correlated with the amplitude strengths of the applied two lasers and the RF itself. It also depends on the frequency detuning of the coupling laser, which induces an asymmetrically optical splitting. The coupling laser frequency fixing at the shoulder of the stronger one is more favorable for a higher sensitivity. Accordingly, we perform an experimental demonstration for the optimization of all these parameters and the sensitivity is improved to 12.50(04) nVcm−1·Hz−1/2.

Photoionization dynamics of bounded electrons in the ground state, the first and second excited states of a hydrogen atom, triggered by ultrashort near-infrared laser pulses, have been investigated in a transition regime (γ∼1) that offers both multiphoton and tunneling features. Significant differences in spectral characteristics are found between the three low-energy states. The H(2s) ionization probability is larger than the H(2p) value with a special oscillating structure, but both are much greater than the ground state H(1s) in a wide range of laser intensities. By comparing the momentum spectrum and angular distributions of low-energy photoelectrons released from these degenerate states, we find the H(2p) state shows a stronger long-range Coulomb attraction force than the H(2s) state on account of the difference in the initial electron wave packet. Furthermore, analysis of the photoelectron momentum distributions sheds light on both the first and second excited states with a symmetrical intercycle interference structure in a multicycle field but an intracycle interference of an asymmetric left-handed or right-handed rotating spectrum in a few-cycle field. By analyzing photoelectron spectroscopy, we identify the parity characteristics of photoelectrons in different energy intervals and their corresponding above-threshold single-photon ionization (ATSI) or above-threshold double-photon ionization (ATDI) processes. We finally present the momentum distributions of the electrons ionized by laser pulses with different profiles and find the carrier-envelope phase (CEP) is a strong factor in deciding the rotating structure of the emission spectrum, which provides a new method to distinguish the CEP of few-cycle pulses.

Background: As a result of their low incidence, most of the studies on intracranial aneurysms associated with middle cerebral artery (MCA) anomalies were presented as case reports or small case series. No systematic review on this specific entity has been conducted.Methods: A PubMed search of the published studies was performed on April 6th, 2019 for patients who had intracranial aneurysms associated with MCA anomalies. The languages included in this study were English, Chinese, and Japanese.Results: Finally, 58 articles reporting of 67 patients including 1 case in our center were included. The identified patients (37 females, 55.2%) aged from 4 to 81 (49.85 ± 15.22) years old. 50 (50/67, 74.6%) patients presented with hemorrhagic stroke either from the MCA anomalies associated aneurysms or other sources. 63 aneurysms (63/67, 94.0%) were saccular, 3 (4.5%) were dissecting or fusiform, and 1 (1.5%) was pseudoaneurysm. 32 (32/65, 49.2%) patients had other concurrent cerebrovascular anomalies. 56 (83.6%) patients underwent open surgeries, 8 (11.9%) patients underwent endovascular treatment, and 3 (4.5%) patients were conservatively managed. 56 (56/61, 91.8%) patients achieved a good recovery.Conclusions: The pathophysiological genesis of intracranial aneurysms associated with MCA anomalies is still obscure. The inflicted patients tend to have other concurrent cerebrovascular anomalies, which denotes that congenital defect in cerebrovascular development might play a role in this process. Most of the affected patients could experience a good recovery after treatment.

Background: In rare circumstances, patients with intracranial (dural arteriovenous fistulas) DAVFs could be complicated with brainstem engorgement, which might lead to delayed or false diagnosis and subsequent improper management.Methods: On July 2th, 2019, a systematic search was conducted in the PubMed database for patients with intracranial DAVFs complicated with brainstem engorgement.Results: Sixty-eight articles reporting of 86 patients were included for final analysis. The patients were aged from 20 to 76 years (57.10 ± 12.90, n = 82). The female to male ratio was 0.68 (35:51). Thirty-three (40.2%, 33/82) patients were initially misdiagnosed as other diseases. The specific location distributions were cranio-cervical junction, cavernous sinus, superior petrosal sinus, transverse and/or sigmoid sinus, tentorium, and other sites in 27 (32.5%), 11 (13.2%), 9 (10.8%), 10 (12.0%), 21 (25.3%), and 5 (6.0%) patients, respectively. The Cognard classification of DAVFs were II, III, IV, and V in 9 (10.7%, 9/84), 1 (1.2%, 1/84), 1 (1.2%, 1/84), and 73 (86.9%, 73/84) patients. Eighteen (22%, 18/82) patients were demonstrated to have stenosis or occlusion of the draining system distal to the fistula points. The mean follow-up period was 7.86 (n = 74, range 0–60 months) months. Fifty-four (70.1%, 54/77) patients experienced a good recovery according to the mRS score.Conclusions: Intracranial DAVFs complicated with brainstem engorgement are rare entities. Initial misdiagnosis and delayed definite diagnosis are common in the past three decades. The treatment outcome is still unsatisfactory at present. Early awareness of this rare entity and efficiently utilizing the up to date investigations are of utmost importance.

In recent years, the Ch6–Ch7 deep-water gravity flow deposits of the Yanchang Formation in the Southern Ordos Basin have been characterised as sandy debris flow sediments, a modification from previous sedimentary facies characterisations of braided river delta fronts, deep lacustrine turbidite fans and seismite deposits. This leads to the necessity of a detailed interpretation of the origin and reservoir properties of the deep-water deposits of the Jinghe Oil Field in the Ordos Basin. A large number of core images were analysed, identifying 15 lithofacies and 3 main sedimentary facies, including the sandy debris flow microfacies, turbidite microfacies and seismite-slump microfacies. Sedimentary facies determination was proved by particle size analysis and vertical and horizontal microfacies distributions. The sedimentary process can be described by earthquake and gravity deformation inducing a slide of the large deposit of delta front sediments on the slope break down the slope. Simultaneously, ambient lake water penetrated the sediments, forming seismite-slump microfacies with load structures, liquefied structures and slump deformation structures. With continuous sediment liquefaction, sandy debris flow microfacies, which were massive bedding sandstones, were formed during transportation. Leading the sediments, turbidite flows resulted from flow transformation which were possibly remolded by weak bottom currents in intermittent periods. Studies of reservoir properties and oil shows indicate that sandy debris flow sandstones have the best reservoir properties and oil shows, followed by turbidite sandstones, with seismite-slump sandstones being the poorest. The sandy debris flow and part of the turbidite sandstones have good oil production potential.

IgA nephropathy (IgAN) is now widely recognized as the most common primary glomerulonephritis worldwide, especially in China. The immunosuppressive treatment option for IgAN is still controversial. Previously, we proved that mycophenolate mofetil (MMF; Shanghai Roche, China) combined with low-dose prednisone was an effective and safe option for biopsy-proven mild to moderate IgAN patients in a short term of follow-up. This article we first reported the safety and efficacy of this regimen in a 42-year-old male biopsy-proven advanced 10-year follow-up IgAN case (Lee’s Class V; the patient was biopsied 10 years ago, so the Oxford Mesangial hypercellularity Endocapillary hypercellularity Segmental glomerulosclerosis Tubular atrophy/interstitial fibrosis (MEST) classification was not used). The mycophenolate and prednisone were only given for a limited time. The other main medications included calcium channel blockers and antiplatelet agents. Clinical and laboratory indexes were aperiodic assessed during the 10-year follow-up. The serum creatinine decreased from 356 to around 210 μmol/L and urine excretion protein reduced from 3.4 g/d to about 0.5 g/d after 6 months of the initiation of this regimen, respectively. These perfect treatment effects could maintain well during the whole follow-up period. No obvious complications were observed.

The stability and robustness of quadrotors are always influenced by unknown or immeasurable system parameters. This paper proposes a novel adaptive parameter estimation technology to obtain high-accuracy parameter estimation for quadrotors. A typical mathematical model of quadrotors is first obtained, which can be used for parameter estimation. Then, an expression of the parameter estimation error is derived by introducing a set of auxiliary filtered variables. Moreover, an augmented matrix is constructed based on the obtained auxiliary filtered variables, which is then used to design new adaptive laws to achieve exponential convergence under the standard persistent excitation (PE) condition. Finally, a simulation and an experimental verification for a typical quadrotor system are shown to illustrate the effectiveness of the proposed method.

This paper focuses on the relationship between L-posets and complete L-lattices from the categorical view. By considering a special class of fuzzy closure operators, we prove that the category of complete L-lattices is a reflective full subcategory of the category of L-posets with appropriate morphisms. Moreover, we characterize the Dedekind-MacNeille completions of L-posets and provide an equivalent description for them.

To discover the effect of environmental factors on pollinator visitation to flowering Medicago sativa, several field experiments were designed to examine the diurnal movement patterns of wild bee species in the Hexi Corridor of northwestern China. Our study results showed that Megachile abluta, M. spissula, and Xylocopa oalga showed unimodal diurnal foraging behavior, whereas Andrena parvula and Anthophora melanognatha showed bimodal diurnal foraging behavior, Correlation analysis indicated that diurnal foraging activities of pollinators were significantly correlated with environmental factors. Correlations of foraging activities versus environmental factors for M. abluta, M. spi.ssula, and X. valga best fit a linear model, whereas those of A. parvula and A. melanognatha best fit a parallel quadratic model. Results of this study indicated that solitary wild bees such as M. abluta, M. spiseula, X. valga, A. parvula, and A. melanognatha are potential alfalfa pollinators in the Hexi Corridor. An understanding of the environmental factors that affect the behaviors of different wild bees foraging in alfalfa are basic to the utilization of solitary wild bees in a practical way for increased, or more consistent, pollination of alfalfa for seed production. A fuzzy logic and neuro-fuzzy system for classification of hydrometeor type based on polarimetric radar measurements is described in this paper. The hydrometeor classification system is implemented by using fuzzy logic and a neural network, where the fuzzy logic is used to infer hydrometeor type, and the neural network learning algorithm is used for automatic adjustment of the parameters of the fuzzy sets in the fuzzy logic system according to prior knowledge. Five radar measurements, namely, horizontal reflectivity (ZH ), differential reflectivity (ZDR ), differential propagation phase shift (KDP ), correlation coefficient [ rHV (0)], and linear depolarization ratio (LDR ), and corresponding altitude, have been used as input variables to the neuro-fuzzy network. The output of the neuro-fuzzy system is one of the many possible hydrometeor types: 1) drizzle, 2) rain, 3) dry and low density snow, 4) dry and high-density crystals, 5) wet and melting snow, 6) dry graupel, 7) wet graupel, 8) small hail, 9) large hail, and 10) a mixture of rain and hail. The neuro-fuzzy classifier is more advantageous than a simple neural network or a fuzzy logic classifier because it is more transparent (instead of a 'black box') and can learn the parameter of the system from the past data (unlike a fuzzy logic system). We have investigated the Rydberg state population of an argon atom by an intense laser pulse and its wavepacket dynamics monitored by another successive laser pulse in the tunneling regime. A wavepacket comprising a superposition of close high-lying Rydberg states is irradiated by a multicycle laser pulse, where the sub-wave components in the wavepacket have fixed relative phases. A time-delayed second laser pulse is employed to apply on the excited Rydberg atom. If the time is properly chosen, one of the sub-wave components will be guided towards the ionization area while the rest remains intact. By means of this pump–probe technique, we could control and monitor the Rydberg wavepacket dynamics and reveal some interesting phenomenon such as the survival rate of individual Rydberg states related to the classical orbital period of electron. We propose a method for studying the tunneling process by analyzing the instantaneous ionization rate of a circularly polarized laser. A numerical calculation shows that, for an atom exposed to a long laser pulse, if its initial electronic state wave function is non-spherical symmetric, the delayed phase shift of the ionization rate vs the laser cycle period in real time in the region close to the peak intensity of the laser pulse can be used to probe the tunneling time. In this region, an obvious time delay phase shift of more than 190 attoseconds is observed. Further study shows that the atom has a longer tunneling time in the ionization under a shorter wavelength laser pulse. In our method, a Wigner rotation technique is employed to numerically solve the time-dependent Schrödinger equation of a single-active electron in a three-dimensional spherical coordinate system. Objective To investigate the association of inflammatory and nutritional indicators with the efficacy of neoadjuvant chemotherapy (NACT) in patients with upper gastric cancer before and after NACT, and to establish a clinical predictive model. Methods A total of 117 patients with upper gastric cancer who underwent NACT in Department of Gastroenterology in our hospital from April 2013 to January 2022 were enrolled, and according to the efficacy of chemotherapy, they were divided into effective group and ineffective group. Related clinical data were collected, such as age, sex, smoking history, drinking history, initial symptoms, tumor location, degree of tumor differentiation, tumor stage, tumor pathological type, and routine blood test results within one week before NACT and within one week after NACT. Univariate and multivariate analyses were performed to identify the influencing factors for the efficacy of NACT, and then a nomogram model was established and validated. Introduction. Previous studies have shown that TGF-ß1/Smad3 signaling promotes renal fibrosis by inhibiting miR-29. To date, only few studies have reportedon circulating microRNAs in IgA nephropathy (IgAN). However, the plasma expression of miR-29a and its role in patients with IgAN remains unclear. In this study, we attempted to elucidate whether plasma miR-29a expression can be used as a biomarker for monitoring disease states. Methods. For this study, 15 healthy subjects, 36 patients with untreated renal biopsy-proven IgAN, and 79 patients with IgAN, who were under treatment for a period of 1 year on an average, all of whom had similar age and gender distributions, were included. The plasma expression of miR-29a in each group was explored by real-time PCR, and the relationship between miR-29a expression and clinical, pathological, and prognostic indicators of IgAN was further evaluated. Results. Relative plasma expression of miR-29a in patients with IgAN was significantly lower than that in healthy controls (P < .001), and these changes in plasma miR-29a could be suppressed by treatment (P < .05). Plasma miR-29a was positively correlated with eGFR and negatively correlated with proteinuria and serum creatinine, irrespective of whether or not the patients with IgAN accepted treatment (P < .05). Plasma miR-29a level was negatively correlated with primary pathological parameters such as crescent formation, Lee's and Oxford classification (P < .05). Kaplan-Meier analysis revealed that patients with high plasma expression of miR-29a had better renal function and better response to treatment compared to those with low expression (P < .05). Objective: To observe the therapeutic effects of electrical acupuncture and auricular-plaster therapy for chronic fatigue syndrome (CFS)., Method: 64 CFS patients were randomly divided into two groups. 32 cases in the treatment group were treated by the electrical acupuncture and auricular-plaster therapy, and 32 cases in the control group with oral hydrocortisone., Results: The total effective rates were respectively 93.75% in the treatment group and 75.00% in the control group, with a statistically significant difference between the two groups (P < 0.05)., Conclusion: Electrical acupuncture and auricular-plaster therapy may show a better anti-fatigue effect than that of routine Western drugs.
