Abstract Parkinson’s disease (PD) is associated with aggregation of misfolded α-synuclein and other proteins, including tau. We designed a cross-sectional study to quantify the brain binding of [11C]PBB3 (a ligand known to bind to misfolded tau and possibly α-synuclein) as a proxy of misfolded protein aggregation in Parkinson’s disease (PD) subjects with and without cognitive impairment and healthy controls (HC). In this cross-sectional study, nineteen cognitively normal PD subjects (CN-PD), thirteen cognitively impaired PD subjects (CI-PD) and ten HC underwent [11C]PBB3 PET. A subset of the PD subjects also underwent PET imaging with [11C](+)DTBZ to assess dopaminergic denervation and [11C]PBR28 to assess neuroinflammation. Compared to HC, PD subjects showed higher [11C]PBB3 binding in the posterior putamen but not the substantia nigra. There was no relationship across subjects between [11C]PBB3 and [11C]PBR28 binding in nigrostriatal regions. [11C]PBB3 binding was increased in the anterior cingulate in CI-PD compared to CN-PD and HC, and there was an inverse correlation between cognitive scores and [11C]PBB3 binding in this region across all PD subjects. Our results support a primary role of abnormal protein deposition localized to the posterior putamen in PD. This suggests that striatal axonal terminals are preferentially involved in the pathophysiology of PD. Furthermore, our findings suggest that anterior cingulate pathology might represent a significant in vivo marker of cognitive impairment in PD, in agreement with previous neuropathological studies.

Abstract Prompt ElastoGravity Signals are light-speed gravity-induced signals recorded before the arrival of seismic waves. They have raised interest for early warning applications but their weak amplitudes close to the background seismic noise have questioned their actual potential for operational use. A deep-learning model has recently demonstrated its ability to mitigate this noise limitation and to provide in near real-time the earthquake magnitude (M w ). However, this approach was efficient only for large earthquakes (M w ≥ 8.3) of known focal mechanism. Here we show unprecedented performance in full earthquake characterization using the dense broadband seismic network deployed in Alaska and Western Canada. Our deep-learning model provides accurate magnitude and focal mechanism estimates of M w ≥ 7.8 earthquakes, 2 minutes after origin time (hence the tsunamigenic potential). Our results represent a major step towards the routine use of prompt elastogravity signals in operational warning systems, and demonstrate its potential for tsunami warning in densely-instrumented areas.

AIM: To analyze the evidence available on foot reflexology with application of caring consciousness to reduce pain in older adults METHODS: Integrative review carried out in Medline/PubMed, CINAHL, EBSCO Host, SCOPUS, Web of Science, and EMBASE databases using the MESH terms “Aged,” “Pain,” “Musculoskeletal manipulations,” “Nursing,” and “Nursing theory.” in English, Spanish, Portuguese, and French published between 2012 and 2022. The selected studies were exported to Rayyan software and evaluated by title and abstract; after reading the full text, were analyzed according to the PRISMA guide checklist. RESULTS: In this study, 497 articles were found, 4 were selected. 75% of the studies were based on the Ingham method of reflexology. The elements that demonstrate caring consciousness and enhance the healing effect of the therapy were foot grooming, use of essential oils, implementation of relaxation techniques, description of pressure points and combining other therapies. In 75% of the cases, foot reflexology was effective to reduce pain. CONCLUSION: Those authors who apply caring consciousness in the technique of reflexology obtained more significant results in the reduction of pain. The incorporation of external elements shows the intention of nursing to enhance the effects of the therapy, as well as the search for connection with the elderly.

Purpose: To report the clinical outcomes and reoperation rates of arthroscopic and endoscopic iliopsoas release at short-term follow-up after ipsilateral total hip arthroplasty (THA) at 2 separate medical institutions and to evaluate whether demographic and radiographic parameters are associated with postoperative patient-reported outcomes (PROs). Methods: Patients with iliopsoas tendinitis in the setting of prior THA who underwent arthroscopic iliopsoas fractional lengthening from 1988 to 2023 at 2 academic institutions were reviewed. Patients were included if they had 12 months of follow-up and underwent evaluation of preoperative anterior acetabular component overhang, surgery satisfaction, postoperative subjective hip flexion strength and anterior groin pain improvement, modified Harris Hip Score, Single Assessment Numeric Evaluation score, Tegner activity scale score, visual analog scale (VAS) score, and revision hip arthroplasty. Results: Sixty hips in 58 patients (19 male and 39 female patients) were followed up for a mean of 39.3 months (range, 12.0-105.9 months) postoperatively. Of the patients, 77% reported feeling “much better” or “slightly better,” 75% reported improved anterior groin pain, and 60% reported improved subjective hip flexion strength. The surgery satisfaction rating was 7.2 ± 3.3 (scale of 0 to 10). The mean postoperative modified Harris Hip Score, VAS score for pain at rest, VAS score for pain with use, and Single Assessment Numeric Evaluation score were 73.9 ± 19.4, 1.3 ± 2.4, 3.8 ± 2.9, and 71.9 ± 21.9, respectively. Preoperative anterior acetabular component overhang was 3.3 ± 6.5 mm and did not significantly correlate with postoperative PROs (P ≥ .45). The Tegner score improved from 2.5 ± 1.7 preoperatively to 2.9 ± 1.4 postoperatively (P = .0253). Three patients underwent revision arthroplasty at a mean of 25.3 months (range, 11.6-40.4 months) postoperatively, with an acetabular component revision rate of 3.3%. Conclusions: Satisfactory outcomes and low revision arthroplasty rates were observed in patients undergoing arthroscopic iliopsoas lengthening after THA. There was no statistically significant relation between anterior acetabular component overhang and final PROs. Level of Evidence: Level IV, therapeutic case series.

The olfactory system enables humans to smell different odors, which are closely related to emotions. The high temporal resolution and non-invasiveness of Electroencephalogram (EEG) make it suitable to objectively study human preferences for odors. Effectively learning the temporal dynamics and spatial information from EEG is crucial for detecting odor-induced emotional valence. In this paper, we propose a deep learning architecture called Temporal Attention with Spatial Autoencoder Network (TASA) for predicting odor-induced emotions using EEG. TASA consists of a filter-bank layer, a spatial encoder, a time segmentation layer, a Long Short-Term Memory (LSTM) module, a multi-head self-attention (MSA) layer, and a fully connected layer. We improve upon the previous work by utilizing a two-phase learning framework, using the autoencoder module to learn the spatial information among electrodes by reconstructing the given input with a latent representation in the spatial dimension, which aims to minimize information loss compared to spatial filtering with CNN. The second improvement is inspired by the continuous nature of the olfactory process; we propose to use LSTM-MSA in TASA to capture its temporal dynamics by learning the intercorrelation among the time segments of the EEG. TASA is evaluated on an existing olfactory EEG dataset and compared with several existing deep learning architectures to demonstrate its effectiveness in predicting olfactory-triggered emotional responses. Interpretability analyses with DeepLIFT also suggest that TASA learns spatial-spectral features that are relevant to olfactory-induced emotion recognition.

Background: Injury to the medial ulnar collateral ligament (MUCL) is a common setback experienced by many throwing athletes often requiring reconstruction with 12 to 18 months of rehabilitation. Current reconstructive techniques fail to anatomically restore the MUCL. This presentation demonstrates the “anatomic technique” for reconstruction of the MUCL supplemented by internal brace augmentation. Indications: Initial injuries of the MUCL can often be managed conservatively; however, many complete tears and those failing nonoperative management are frequently treated with MUCL reconstruction. Technique Description: After graft harvest, a 6-cm incision is created over the medial epicondyle to expose the sublime tubercle and medial epicondyle. Sutures are placed through the torn ligament which are used to formally repair the native ligament. A 4-mm socket is drilled in the MUCL origin of the medial epicondyle, and two 2-mm penetrating sockets are drilled toward the base of the 4-mm socket. The sutures in the native ligament are passed through the sockets and later tied over the bone bridge to complete the native ligament repair. A tight rope is shuttled through the medial epicondyle socket and assembled. The palmaris longus autograft and a suture tape are loaded onto the tightrope and reduced into the humeral socket. On the ulna, soft suture anchors are placed on either side of the sublime tubercle. The distal end of the graft is then sutured utilizing a FiberWire. The graft is secured to the sublime tubercle by tying down the previously placed suture anchors. The suture tapes and internal brace are loaded into a SwiveLock suture anchor, which is fixed distally along the sublime tubercle ridge creating an anatomic reconstruction. Then, the native ligament sutures and tight rope are re-tensioned and tied across the humeral bone bridge for final fixation. Results: In our cohort of 26 professional and amateur throwers, >90% returned to play at the same level at a mean of 9.9 months postoperatively. Discussion/Conclusion: Anatomic reconstruction of the MUCL with internal bracing is a viable option for MUCL injuries and may allow expedited return to sport for most athletes. Election is an important moment for the democratic process in Indonesia, Indonesia has held elections regularly for a long time. Election in Indonesia still carried out conventionally by coming to Tempat Pemungutan Suara (TPS) or Voting Place to vote. The purpose of this research is to design a mobile based election e-voting system as means to conduct elections for head of state or region and make it easier for the people to vote without having to travel. The research method used in this research is descriptive analysis with a qualitative approach. The result of this research is to show that the E-PIL application can make it easier for Komisi Pemilihan Umum (KPU) to carry out election (pemilu) also make it easier for the people to vote only by accessing E-PIL application that can be accessed via each smartphone. The main concept of this application is e-voting feature for election that can be used in every election agenda. Also makes it easier for the KPU in voting process because E-PIL application can only be used by 1 ID on 1 device that integrated with the government. In the end, E-PIL application is present as a medium for elections as well as to adapt in today’s digital world

Juvenile nasopharyngeal angiofibroma (JNA) is a histopathologically benign and highly vascular neoplasm of the nasopharynx. Surgery is the treatment of choice for this type of tumor, but its highly vascular nature could result in profuse and uncontrollable bleeding. Preoperative transcatheter arterial embolization offers an effective and minimally invasive modality for reducing intraoperative blood loss. We report 2 cases of JNA in a 17-year-old male and 14-year-old male who underwent preoperative embolization of external carotid artery branch using gelatin sponge slury and polyvinyl alcohol. Even without internal carotid artery branch embolization, both of our reported cases had intraoperative blood loss of 1100 mL and 1300 mL which are less than the previously reported mean blood loss of 1428 mL in patients who underwent both internal and external carotid artery branch embolization.

Análisis de vulnerabilidades, se encargó de seleccionar las herramientas para explotar las vulnerabilidades identificadas. Explotación, consistió en realizar acciones para comprometer el sistema auditado utilizando herramientas automatizadas simulando un Rubber Ducky con el Arduino Raspberry Pi Pico. Post explotación, se enfocó en realizar pruebas para comprobar la información que se puede obtener al explotar las debilidades identificadas. Los resultados que se obtuvieron en base de las cookies, Facebook es la red social más vulnerable y expone la mayor cantidad de datos a los atacantes. Twitter detecta y bloquea la actividad sospechosa, mientras que LinkedIn y Reddit son las más seguras, no se pudo acceder a las cuentas utilizando las cookies extraídas. Por otro lado, YouTube es la plataforma de Streaming que brinda más información a los atacantes, mientras que Netflix es una de las más vulnerables debido a la gran cantidad de cookies encontradas.

Background: Medial-sided elbow injuries are becoming increasingly common among throwing athletes due to overuse and increased specialization at early ages. High valgus stress and repetitive elbow flexion/extension during throwing not only affects the ligaments and dynamic support of the elbow, but commonly affects the ulnar nerve. Indications: Management of cubital tunnel syndrome is initially rest, therapy, and functional training; however, if conservative measures do not appropriately address the ulnar neuropathy, surgical decompression with subcutaneous transposition is a reliable treatment option. Technique Description: An incision is created over the medial epicondyle. The medial antebrachial cutaneous nerve is identified and protected. The ulnar nerve is identified and tagged with a vessel loop to allow for appropriate handling of the nerve. Decompression of the ulnar nerve begins proximally by spreading the tissues superficial to the ulnar nerve and splitting the fascia overlying it. Then, dissection deep to the nerve is performed. A small strip of the medial intermuscular septum will be used as a sling to hold the nerve securely in the transposed position. This is released proximally and the distal attachment to the medial epicondyle is left in place. Decompression is then continued distally by releasing the superficial fascia over the flexor carpi ulnaris (FCU) and a portion of the deep FCU muscle belly and fascia. The nerve is decompressed circumferentially, while preserving penetrating branches to the FCU as able. Ultimately, the ulnar nerve is decompressed 10 cm proximal and 10 cm distal to the medial epicondyle. The intermuscular septum is then pulled over the nerve, checked for appropriate length, and sutured in place both posterior and anterior to the ulnar nerve. Results: Symptom improvement after decompression and transposition is high (>90%); however, performance outcomes in overhead and throwing athletes is variable, and return to previous or higher level of play ranges from 60% to 90%. Discussion/Conclusion: Ulnar nerve decompression and transposition can reliably address underlying symptoms, but results are mixed for return to sport outcomes in overhead and throwing athletes. Ventriculoperitoneal (VP) shunts represent a surgical option for patients affected by increased intracranial hypertension when medical management fails or is contraindicated. Complications following implantation include shunt obstruction, infection, over and under drainage, migration or disconnection of the tube, formation of a pseudocyst, and allergy to the silicone tube. We report the case of a 31-year-old woman who presented to the emergency room with nausea and generalized malaise, found to have the distal segment of the VP catheter perforating her gastric wall into the stomach lumen which required surgical intervention. In this report, we describe a rare complication associated with the implantation of ventriculoperitoneal shunt (VPS) catheters and the subsequent management plan.

Abstract Objectives Vocal fold (VF) scarring, manifested by increased collagen, decreased glycosaminoglycans (GAGs), and disrupted elastic fibers, remains a negative consequence of VF injury or resection. The objective of this study is to compare four reconstructive options after Vf mucosal resection in rabbits. A Cell‐Based Outer Vocal fold Replacement (COVR) using human adipose‐derived mesenchymal stromal cells (hASCs) in fibrin scaffold is directly compared with a decellularized scaffold implant, hASC injection, and resection alone without reconstruction. The primary hypothesis is that the cells‐in‐scaffold construct better reconstitutes the VF structure than either cells or scaffold alone, or than healing by secondary intention. Methods A total of49 rabbits received bilateral VF cordectomy, followed by either COVR implant, decellularized scaffold implant, hASC injection, or no reconstruction (injured control group). Larynges were harvested after 6 weeks. Results Histology demonstrated greater lamina propria thickness, less collagen deposition, and more GAGs in COVR animals versus all other treatment groups. Evidence of persistent human cells was found in about half of the cell‐treated animals. RNA levels of fibrosis pathway and macrophage phenotype markers were statistically unchanged among treatment groups at 6 weeks. Conclusion These data support the efficacy of COVR implantation in restoring VF microstructure in rabbits. The intact COVR was required; isolated components of decellularized scaffold or injected hASC still produced histologic scarring. We propose that the unique bilayered cell structure within fibrin enables controlled matrix remodeling to minimize wound contraction and fibrosis, and to promote GAG deposition. IntroductionL-carnitine (LC) has been associated with inflammatory mediator reduction and with downregulating the angiotensin-converting enzyme-2 (ACE2) receptor, which is the target of SARS-CoV-2 attachment.MethodsThis pilot phase 2 randomized, double-blind placebo-controlled trial contained two cohorts. Cohort 1 comprised 101 individuals with negative RT-PCR SARS-CoV-2 test results who cohabitated with an individual diagnosed with SARS-CoV-2 infection. Cohort 2 comprised 122 individuals with positive SARS-CoV-2 RT-PCR test results who were asymptomatic or had mild COVID-19 pneumonia symptoms. Participants in each cohort were randomized 1:1 to receive either 2 g elemental oral LC supplementation or placebo daily for 21 days. Primary endpoints included adverse events, SARS-CoV-2 infection incidence in Cohort 1, and disease progressions in Cohort 2. Secondary endpoints included between-group laboratory profile comparisons and Cohort 2 ACE1/ACE2 plasma levels. Disease progression was compared between the Cohort 2 groups using chest computed tomography.ResultsIn Cohort 1, two SARS-CoV-2 infections occurred in each group. The common adverse events included headache, dyspnea, and tiredness. In Cohort 2, platelet counts were elevated, and fibrinogen levels reduced in the LC group compared with those of the placebo group.ConclusionOur study showed that LC was well-tolerated and suggests it modulates coagulation pathways. Furthermore, chest computed tomography images of the Cohort 2 LC group showed significant lung lesion improvement, suggesting that LC may slow COVID-19 progression.

Background:. Our understanding of the risk factors for and effectiveness of prophylactic measures against shoulder periprosthetic joint infections (PJIs) continues to evolve. This study’s objective was to study patient characteristics, procedural characteristics, and various infection prophylactic measures and their effects on the risk of shoulder PJI after primary arthroplasty. Methods:. Nine hundred and ninety-eight patients in a longitudinally maintained, single-institution shoulder arthroplasty database who had at least 2 years of clinical follow-up were retrospectively reviewed. Patient and procedural characteristics were collected. Perioperative variables, including the use of intraoperative antibiotics, topical antibiotics, antibiotic-containing irrigation solution, and a postoperative drain, were collected. Patients who developed shoulder PJI were compared with those without shoulder PJI to identify any association with patient or procedural characteristics. Results:. Of the 998 patients, 20 (2.0%) met the criteria for shoulder PJI. Cutibacterium was identified as the causative organism in 19 (95%) of 20 culture-positive reoperations. On univariate analysis, patients in the PJI group were more likely to be younger (p < 0.001), to be male (p = 0.014), to have commercial insurance (p = 0.003), to use alcohol (p = 0.048), and to have had a ream-and-run or hemiarthroplasty procedure (p = 0.005). On multivariable analysis, older age was independently associated with a lower risk of PJI (odds ratio [OR] per year = 0.95; 95% confidence interval [CI], 0.91 to 0.99; p = 0.014). Reverse total shoulder arthroplasty (OR, 10.32; 95% CI, 0.92 to 116.33; p = 0.059) and hemiarthroplasty (OR, 8.59; 95% CI, 0.86.30 to 85.50; p = 0.067) trended toward a higher risk of PJI. Conclusions:. Younger patients and patients undergoing procedures other than anatomic total shoulder arthroplasty are at higher risk for shoulder PJI. The majority of culture-positive reoperations were a result of Cutibacterium species. Level of Evidence:. Therapeutic Level III. Background:. Ream-and-run arthroplasty can improve pain and function in patients with glenohumeral arthritis while avoiding the complications and activity restrictions associated with a prosthetic glenoid component. However, stiffness is a known complication after ream-and-run arthroplasty and can lead to repeat procedures such as a manipulation under anesthesia (MUA) or open surgical revision. The objective of this study was to determine risk factors associated with repeat procedures indicated for postoperative stiffness after ream-and-run arthroplasty. Methods:. We conducted a retrospective review of our shoulder arthroplasty database to identify patients who underwent ream-and-run arthroplasty and determined which patients underwent subsequent repeat procedures (MUA and/or open revision) indicated for postoperative stiffness. The minimum follow-up was 2 years. We collected baseline demographic information and preoperative and 2-year patient-reported outcome scores and analyzed preoperative radiographs. Univariate and multivariate analyses determined the factors significantly associated with repeat procedures to treat postoperative stiffness. Results:. There were 340 patients who underwent ream-and-run arthroplasty. The mean Simple Shoulder Test (SST) scores for all patients improved from 5.0 ± 2.4 preoperatively to 10.2 ± 2.6 postoperatively (p < 0.001). Twenty-six patients (7.6%) underwent open revision for stiffness. An additional 35 patients (10.3%) underwent MUA. Univariate analysis found younger age (p = 0.001), female sex (p = 0.034), lower American Society of Anesthesiologists (ASA) class (p = 0.045), posterior decentering on preoperative radiographs (p = 0.010), and less passive forward elevation at the time of discharge after ream-and-run arthroplasty (p < 0.001) to be significant risk factors for repeat procedures. Multivariate analysis found younger age (p = 0.040), ASA class 1 compared with class 3 (p = 0.020), and less passive forward elevation at discharge (p < 0.001) to be independent risk factors for repeat procedures. Of the patients who underwent open revision for stiffness, 69.2% had multiple positive cultures for Cutibacterium. Conclusions:. Younger age, ASA class 1 compared with class 3, and less passive forward elevation in the immediate postoperative period were independent risk factors for repeat procedures to treat postoperative stiffness after ream-and-run arthroplasty. Abstract Hypercholesterolemia is becoming a problem with increasing significance. Dietary vegetable oils may help to improve this condition due to presence of phytonutrients with potentially synergistic cholesterol-lowering effects. The objective of this 8-week double-blinded randomized clinical trial was to investigate the effects of consuming 30 g of two different blended cooking oils, rich in omega-3 alpha-linolenic acid and phytonutrients, or refined olive oil on the intestinal microbiota in 126 volunteers with borderline hypercholesterolemia. Multi-factor analysis of relationships between the gut microbiota composition at various taxonomic ranks and the clinical trial parameters revealed the association between beneficial effects of the dietary intervention on the blood lipid profile with abundance of Clostridia class of the gut microbiota. This microbiota feature was upregulated in the course of the dietary intervention and associated with various plasma markers of metabolic health status, such as Triglycerides, Apolipoprotein B and Total Cholesterol to HDL ratio in a beneficial way. The relative abundance of a single species—Clostridium leptum—highly increased during the dietary intervention in all the three study groups. The oil blend with the highest concentration of omega-3 PUFA is associated with faster and more robust responses of the intestinal microbiota, including elevation of alpha-diversity. Butyrate production is being discussed as a plausible process mediating the observed beneficial influence on the plasma lipid profile. Causal mediation analysis suggested that Clostridium genus rather than the higher rank of the phylogeny—Clostridia class—may be involved in the diet-induced improvements of the blood lipid profile.

Background: Distal biceps tendon ruptures commonly present in the middle-aged population resulting in deficits of strength and endurance. Operative management has shown improved functional outcome; however, there is not a clear consensus on an ideal surgical method. Indications: Distal biceps tendon repair is indicated for patients with clinical evidence and supporting magnetic resonance imaging confirmation of complete or partial rupture of the biceps tendon. Ideally, surgery is performed within 1 to 2 weeks of the injury to minimize the amount of scar tissue that is formed and the severity of the tendon retraction. Technique Description: A single incision is created on the volar surface of the forearm approximately 15 mm distal to the main flexor crease. After dissection to and retrieval of the biceps tendon, a #2 FiberTag stitch is placed distally and secured in a standard looped, locking fashion. An anchor is then placed in the most proximal aspect of the radial tuberosity and preliminarily placed around the tendon. An all-suture intramedullary cortical button is then placed in the distal aspect of the radial tuberosity. The FiberTag sutures are then shuttled through the button and tightened to anatomically reduce the distal biceps. The sutures on the proximal anchor are then used as supplemental fixation. Results: An anterior, single incision technique provides exposure necessary for dual anchor fixation and anatomic restoration of the distal biceps tendon. Utilizing this approach, patients are found to have improved flexion and pronation at 1 year, lower rates of heterotopic ossification, and lower rates of reoperation. Conclusion: Distal biceps tendon repair through an anterior, single incision provides excellent exposure for surgical repair. Utilizing a dual anchor technique, the distal button allows for anatomic fixation while the proximal suture anchor provides secondary fixation and increases the bone-tendon interface. Método: Revisión de tipo integrativa en las bases de datos de PubMed, Scopus, Web of Science, biblioteca virtual en salud (BVS), SpringerLinks y ScienceDirect utilizando los descriptores de ciencias de la salud (DECS): Salud familiar (Family Health); Anciano (Aged); Alzheimer (Alzheimer), Enfermería familiar (Family Nursing) y estudios controlados antes y después (Controlled Before-After Studies); se utilizó el operador booleano AND para unir los descriptores. Resultados principales: Se encontraron 5 artículos dentro de los cuales se abordan Intervenciones de enfermería enfocadas al cuidador principal e intervenciones de enfermería enfocadas a la familia. Conclusión principal: Las intervenciones en salud familiar pueden ir orientadas hacia el cuidador primario o toda la familia e igualmente tendrá algún tipo de efecto indirecto sobre el anciano con Alzheimer.

IntroductionStandard-of-care treatment for patients with newly diagnosed glioblastoma (GBM) after surgery or biopsy includes concurrent chemoradiation followed by maintenance temozolomide (TMZ) with tumor treating fields (TTFields). Preclinical studies suggest TTFields and radiotherapy work synergistically. We report the results of our trial evaluating the safety of TTFields used concurrently with chemoradiation.MethodsThis is a single-arm pilot study (clinicaltrials.gov Identifier: NCT03477110). Adult patients (age ≥ 18 years) with newly diagnosed glioblastoma and a Karnofsky performance score (KPS) of ≥ 60 were eligible. All patients received concurrent scalp-sparing radiation (60 Gy in 30 fractions) with TMZ (75 mg/m2 daily) and TTFields (200 kHz). Maintenance therapy included TMZ and continuation of TTFields. Scalp-sparing radiation treatment was used to reduce radiation dermatitis. Radiation treatment was delivered through the TTFields arrays. The primary endpoint was safety and toxicity of tri-modality treatment within 30 days of completion of chemoradiation treatment.ResultsThere were 30 patients enrolled, including 20 (66.7%) men and 10 (33.3%) women, with a median age of 58 years (range 19 to 77 years). Median KPS was 90 (range 70 to 100). A total of 12 (40%) patients received a gross total resection and 18 (60%) patients had a subtotal resection. A total of 12 (40%) patients had multifocal disease at presentation. There were 20 (66.7%) patients who had unmethylated O(6)-methylguanine-DNA-methyltransferase (MGMT) promotor status and 10 (33.3%) patients who had methylated MGMT promoter status. Median follow-up was 15.2 months (range 1.7 to 23.6 months). Skin adverse events were noted in 83.3% of patients, however, these were limited to Grade 1 or 2 events, which resolved spontaneously or with topical medications. The primary end point was met; no TTFields discontinuation occurred during the evaluation period due to high grade scalp toxicity. A total of 27 (90%) patients had progression, with a median progression-free survival (PFS) of 9.3 months (95% confidence interval (CI): 8.5-11.6 months). The 1-year progression-free survival was 23% (95% CI: 12%-45%). The median overall survival (OS) was 15.8 months (95% CI: 12.5 months-infinity). The 1-year overall survival was 66% (95% CI: 51%-86%).ConclusionsConcurrent TTFields with scalp-sparing chemoradiation is a feasible and well-tolerated treatment option with limited toxicity. A phase 3, randomized clinical trial (EF-32, clinicaltrials.gov Identifier: NCT04471844) investigating the clinical benefit of concurrent TTFields with chemoradiation treatment is currently enrolling.Clinical Trial RegistrationClinicaltrials.gov, identifier NCT03477110.

Introducción: La interacción de los estudiantes de enfermería con los enfermeros clínicos en su lugar de práctica influye en su proceso de aprendizaje, generando posibles modelos mentales de atención que muy probablemente serán replicados cuando los estudiantes egresen. Objetivo: Comprender la experiencia del profesional de enfermería en su relación con el estudiante durante su práctica clínica. Metodología: Estudio cualitativo fenomenológico, con muestreo por bola de nieve, hasta llegar a saturación de datos. Colecta de datos mediante entrevista no estructurada a profundidad, las entrevistas fueron grabadas y transcritas literalmente para el análisis. Resultados: Participaron 19 enfermeros (4 hombres y 15 mujeres) y se identificaron dos categorías: estar-con y coexistiendo con los estudiantes. Conclusiones: El profesional de enfermería considera que los estudiantes deben comprender y trabajar de manera colaborativa para aprovechar su experiencia y conocimientos, evitar las criticas destructivas, así como respetar la individualidad. Una buena relación entre estudiantes y enfermeros clínicos podría llevar a un mejor aprovechamiento de la práctica.

The comprehensive protection that the State, Society and the Family must guarantee to all children, living in Ecuador, must achieve the full comprehensive development of their rights, within a framework of freedom, dignity and equity, as established by the constitution. However, there are few disabilities, including underserved language. On the basis of this provision, this article was proposed to evaluate the rights and difficulties of oral language in children who attend to assert their right of care to the "My Community" Foundation. To this end, a methodology was used with a mixed-type approach, which involved qualitative and quantitative tools. The population consisted of 15 subjects, including school-age children. Among the results it could be noted, that the largest finding corresponded to 61% of the disturbances in pronunciation, presented among its conclusions that there is little fluency of oral language in terms of normality during the production of sounds and quality voice required for speech.

Abstract A recent work by Kimura et al. (Earth Planets Space 71:27, 2019. https://doi.org/10.1186/s40623-019-1006-x) (hereafter referred to as K19) claims to provide the first observational constraints on the prompt elastogravity signals (PEGS) induced by an earthquake. To make their claim, the authors argue that the observations shown in Vallée et al. (Science 358:1164–1168, 2017. https://doi.org/10.1126/science.aao0746) (hereafter referred to as V17) are spurious and their modeling inaccurate. Here we show that K19’s claim is invalid because it is based on flawed data processing. In fact, K19’s analysis involves an incomplete correction of the instrument response of broadband seismic sensors, which essentially dismisses low-frequency components of the data that are critical for the detection of intrinsically low-frequency signals such as PEGS. As a direct consequence, signals are much more difficult to observe than in V17, where the low part of the signal spectrum is carefully taken into account. This deficient data processing also explains why the signal amplitude reported by K19 after stacking data from multiple stations is lower than the individual signals reported by V17. Moreover, failing to take appropriate measures of data quality control, K19 used signals from low-quality sensors to call into question the signals detected by high-quality sensors. Finally, K19 use an inadequate simulation approach to model PEGS, in which the important effect of the ground acceleration induced by gravity changes is ignored. In summary, K19 do not show any viable arguments to question the observations and modeling of PEGS presented in V17.

Todo ser humano, donde se encuentre, siempre está inmerso y en relación dinámica con la cultura que lo rodea, sean personas diversas, o universitarios, entonces la misión de la docencia es formar personas conscientes de su mundo y de lo que son capaces de hacer a favor de este. Este ensayo permitió dilucidar sobre la cultura de vida vinculada a la docencia de enfermería, concluyendo que la desestructuración del proceso pedagógico de enfermería sin cultura de vida, resta el espíritu de superación de estudiantes con expectativas de progreso, porque se considera a veces a la ignorancia como la única e inequívoca expresión de dominio. En la docencia de enfermería, considerando la característica humana que identifica su labor, asociando cultura de la vida y los enlaces figurativos que no se evidencian en la formación, evoca sensaciones extrañas, que difícilmente se pueden explicar con palabras, ante la contemplación de tantas desigualdades.

Objetivo: Describir y analizar el Desempeño del Docente Universitario en la Práctica Áulica. Método: revisión sistémica, integrativa de tipo exploratoria que consistió en la síntesis de datos e información relacionada al desempeño del docente universitario en la práctica áulica en libros, artículos académicos, científicos, revistas indexadas y otros trabajos similares revisados en plataformas digitales como: Scielo y Google académico, 24 artículos constituyeron la muestra. Resultados principales: la educación de los profesionales de enfermería evoluciona constantemente, la apropiada preparación de educadores de enfermería es importante para el desarrollo de conocimientos, habilidades y actitudes de los enfermeros. La práctica reflexiva es un concepto frecuentemente utilizado en enfermería, de hecho, se sugiere que la práctica reflexiva a menudo se confunde, pero existe cuando esta no se da por sentada, razón por la cual el resultado es más aprendizaje de la experiencia. Los avances técnicos y científicos con los que se enfrentan los profesionales de enfermería, conjuntamente con la mayor información que tiene el usuario, exigen una mayor calidad de atención. Conclusión general: El docente en la práctica áulica dista del rol vinculado al cuidado con la teoría, existiendo preferencia por lo técnico y teórico, por lo cual se hace necesario repensar la epistemología conveniente para la práctica y los planes de estudio con el fin de formar profesionales reflexivos.

Immune modulation is a hallmark of cancer. Cancer–immune interaction shapes the course of disease progression at every step of tumorigenesis, including metastasis, of which circulating tumor cells (CTCs) are regarded as an indicator. These CTCs are a heterogeneous population of tumor cells that have disseminated from the tumor into circulation. They have been increasingly studied in recent years due to their importance in diagnosis, prognosis, and monitoring of treatment response. Ample evidence demonstrates that CTCs interact with immune cells in circulation, where they must evade immune surveillance or modulate immune response. The interaction between CTCs and the immune system is emerging as a critical point by which CTCs facilitate metastatic progression. Understanding the complex crosstalk between the two may provide a basis for devising new diagnostic and treatment strategies. In this review, we will discuss the current understanding of CTCs and the complex immune-CTC interactions. We also present novel options in clinical interventions, targeting the immune-CTC interfaces, and provide some suggestions on future research directions.

This study is about the String Ensemble Learning in Extracurricular at SMK Methodist Charles Wesley Medan. The purpose of learning is to know the learning process of string ensemble extracurricular, knowing in teaching methods and materials string ensemble, knowing the learning ability of ensembles extracurricular participant, and to identify constraints string ensemble extracurricular teaching at SMK Methodist Charles Wesley Medan. The study is based on theoretical foundation to explain the theory of learning, the learning component, understanding ability, sense of ensemble, string ensemble understanding, and extracurricular activities. This study will take a place in Jalan Padang Golf (In), Central Business District Block CC-DD Polonia, Medan Polonia. This study uses descriptive qualitative research, observation, interviews, documentation and literature review. The samples are part of the population that is about to be investigated, which in this study were teachers and all students who take the string ensemble extracurricular activities are 15 students and 1 teacher. Once the entire data has been collected, then analyzed to answer all the questions of the study. The learning process is carried out for 60 minutes, the time used to explain the theory is a 15-minute and 45-minute practice. The implementation stage string ensemble learning students who take these activities are all students in both the junior high school students Methodist Charles Wesley, the high school students of Charles Wesley Methodist School, and SMK Methodist Charles Wesley were merged into a single string ensemble group. Abstract Background Quantifying the strength of the intrinsic foot muscles has been a challenge for clinicians and researchers. The reliable measurement of this strength is important in order to assess weakness, which may contribute to a variety of functional issues in the foot and lower leg, including plantar fasciitis and hallux valgus. This study reports 3 novel methods for measuring foot strength – doming (previously unmeasured), hallux flexion, and flexion of the lesser toes. Methods Twenty-one healthy volunteers performed the strength tests during two testing sessions which occurred one to five days apart. Each participant performed each series of strength tests (doming, hallux flexion, and lesser toe flexion) four times during the first testing session (twice with each of two raters) and two times during the second testing session (once with each rater). Intra-class correlation coefficients were calculated to test for reliability for the following comparisons: between raters during the same testing session on the same day (inter-rater, intra-day, intra-session), between raters on different days (inter-rater, inter-day, inter-session), between days for the same rater (intra-rater, inter-day, inter-session), and between sessions on the same day by the same rater (intra-rater, intra-day, inter-session). Results ICCs showed good to excellent reliability for all tests between days, raters, and sessions. Average doming strength was 99.96 ± 47.04 N. Average hallux flexion strength was 65.66 ± 24.5 N. Average lateral toe flexion was 50.96 ± 22.54 N. Conclusions These simple tests using relatively low cost equipment can be used for research or clinical purposes. If repeated testing will be conducted on the same participant, it is suggested that the same researcher or clinician perform the testing each time for optimal reliability.

Aspergillosis is difficult to treat and carries a high mortality rate in immunocompromised patients. Neutrophils play a critical role in control of infection but may be diminished in number and function during immunosuppressive therapies. Here, we measure the effect of three bifunctional small molecules that target Aspergillus fumigatus and prime neutrophils to generate a more effective response against the pathogen. The molecules combine two moieties joined by a chemical linker: a targeting moiety (TM) that binds to the surface of the microbial target, and an effector moiety (EM) that interacts with chemoattractant receptors on human neutrophils. We report that the bifunctional compounds enhance the interactions between primary human neutrophils and A. fumigatus in vitro, using three microfluidic assay platforms. The bifunctional compounds significantly enhance the recruitment of neutrophils, increase hyphae killing by neutrophils in a uniform concentration of drug, and decrease hyphal tip growth velocity in the presence of neutrophils compared to the antifungal targeting moiety alone. We validated that the bifunctional compounds are also effective in vivo, using a zebrafish infection model with neutrophils expressing the appropriate EM receptor. We measured significantly increased phagocytosis of A. fumigatus conidia by neutrophils expressing the EM receptor in the presence of the compounds compared to receptor-negative cells. Finally, we demonstrate that treatment with our lead compound significantly improved the antifungal activity of neutrophils from immunosuppressed patients ex vivo. This type of bifunctional compounds strategy may be utilized to redirect the immune system to destroy fungal, bacterial, and viral pathogens.

Earthquakes have been theorised to produce gravity signals that may arrive before seismic waves, but until now they had not been detected. Montagneret al. have detected prompt gravity signals from the 2011 Tohoku-Oki earthquake thus allowing an early warning of earthquakes before seismic wave arrival.

Clinical flow cytometry is a reliable methodology for whole blood cell phenotyping for different applications. The BD FACSLyric™ system comprises a flow cytometer available in different optical configurations, BD FACSuite™ Clinical software, and optional BD FACS™ Universal Loader. BD FACSuite Clinical software used with BD™ FC Beads and BD CS&T Beads enable universal setup for performance QC, instrument control, data acquisition/storage, online/offline data analysis, and instrument standardization. BD Biosciences sponsored the clinical evaluation of the BD FACSLyric 10-color configuration at seven clinical sites using delinked and de-identified blood specimens from HIV-infected and uninfected subjects to enumerate T-, B-, and NK-lymphocytes with the BD Multitest™ reagents (BD Multitest IMK kit and BD Multitest 6-color TBNK). Samples were analyzed on the BD FACSLyric system with BD FACSuite Clinical software, and on the BD FACSCanto™ II system with BD FACSCanto clinical software and BD FACS 7-Color Setup beads. Category: Other Introduction/Purpose: The intrinsic muscles of the arch have been classified as the core of the foot. Intrinsic foot muscles (IFM) work as local stabilizers and extrinsic foot muscles (EFM) work as global movers to help control the pronation of the foot. These muscles prevent excessive strain of the MLA. Previous research has shown that strengthening IFM may reduce pain and improve function in people with plantar fasciitis. Additionally, habitual use of minimal shoes that lack support of the arch require greater demand on the intrinsic muscles. The purpose of this study was to determine the effectiveness of two different strengthening interventions on IFM and EFM size and strength. Methods: 57 healthy runners (avg 15-30 mpw) were randomly assigned to a foot strengthening (FS, n=19), a minimal shoe walking (MSW, n=19), or a control group (C, n=19). Baseline cross-sectional areas of abductor hallucis, quadratus plantae, flexor digitorum brevis, and flexor digitorum longus, and thickness of the flexor hallucis brevis, tibialis posterior, and tibialis anterior were obtained with ultrasound using a 10 MHz linear probe. Doming, great toe flexion and lateral toes flexion strength were also measured using a custom-built dynamometer. The FS group then underwent an 8 wk foot and ankle strengthening program. The MSW group progressively walked in minimal footwear from 2,500-7,000 over the course of 8 wks. All groups, including C, maintained normal running mileage in conventional footwear throughout the 8 wks. Size and strength measures were then repeated. A repeated measures ANCOVA with a post-hoc test was used to determine differences in size and strength between groups. Results: All muscles measured in the FS and MSW groups increased significantly in size from weeks 0-8. There were no changes in muscle size in the C group. The FS group experienced significantly greater size changes for the flexor hallucis brevis than the MSW group. All other changes in muscle size were not significant between the FS and MSW groups. The FS and MSW groups showed significant increases in strength in all 3 tests from week 0-8. Strength of the control group did not change at any time. The increases in strength in the FS and MSW groups were not significantly different than each other. Conclusion: Exercises that target the intrinsic and extrinsic foot muscles are effective in increased strength and size of the muscles. However, similar increases in strength and size can be obtained by progressive walking in minimal footwear. Combining both these approaches may have an even greater effect. Stronger feet may offer greater protection against overuse injuries such as plantar fasciitis.

The arch’s structure, movement, and integrity during running gait largely depends on the function of intrinsic and extrinsic foot muscles. There are many injuries that can be associated with a dysfunctional medial longitudinal arch. Improving the strength of the foot muscles could help improve arch function during running. The purpose of this study was to compare changes in arch deformation after eight weeks in a foot strengthening exercise group (FS), a group walking in minimalist shoes (MSW), and a control group (C). Methods: 24 healthy runners (average of 15-30 miles/week, no experience with minimalist footwear) were randomly assigned to FS (n=9), MSW (n=7) or C (n=8). FS subjects followed a series of progressive exercises designed to strengthen the intrinsic and extrinsic foot muscles. MSW subjects walked in minimal footwear, increasing from 2,500 to 7,000 steps/day over an 8 wk period. All runners maintained their pre-study mileage in conventional running shoes throughout the 8-week study Three-dimensional motion analysis data was collected at the beginning of the study and after week 8. Passive-reflective markers were placed according to the Oxford Foot Model. Subjects ran at a self-selected pace on a treadmill while data were collected for at least 10 strides. Peak midfoot angles were extracted and averaged within each trial. Group means at week 0 and week 8 were compared using paired t-tests with alpha set at 0.05. Results: While all groups experienced a decrease in midfoot angle, only the FS group experienced a significant change (see table). These results suggest that the foot exercise intervention resulted in more arch control during running than the progressive walking in minimal shoes. Greater changes may have been found if the subjects progressively ran in the minimal shoes which would have placed greater demands on the arch musculature. Conclusion: Based on these data, it seems possible for patients to reduce the amount of arch deformation during running by using targeted foot exercises. This could help with patients who have overuse injuries associated with increased arch deformation. These results also suggest that foot strengthening may be an effective way to prepare for minimal footwear running.

Cerebral amyloidomas are rare cerebral mass lesions often associated with significant morbidity. Cerebral amyloid accumulation can be the result of a number of disease states and it is crucial for proper patient care to identify the pathogenic process leading to amyloidoma formation. Low grade clonal B-cell processes are one cause of cerebral amyloidomas. We report a case of an 87-year-old woman who presented with a lymphoplasmacytic lymphoma associated cerebral amyloidoma complicated by cerebral hemorrhage, discuss the proper workup of this disease entity, and present a review of the literature on this topic.

Veterinary diagnostic laboratories identify and characterize influenza A viruses primarily through passive surveillance. However, additional surveillance programs are needed. To meet this need, an active surveillance program was conducted at pig farms throughout the midwestern United States. From June 2009 through December 2011, nasal swab samples were collected monthly from among 540 groups of growing pigs and tested for influenza A virus by real-time reverse transcription PCR. Of 16,170 samples, 746 were positive for influenza A virus; of these, 18.0% were subtype H1N1, 16.0% H1N2, 7.6% H3N2, and 14.5% (H1N1)pdm09. An influenza (H3N2) and (H1N1)pdm09 virus were identified simultaneously in 8 groups. This active influenza A virus surveillance program provided quality data and increased the understanding of the current situation of circulating viruses in the midwestern US pig population.

As a result of human-to-pig transmission, pandemic influenza A (H1N1) 2009 virus was detected in pigs soon after it emerged in humans. In the United States, this transmission was quickly followed by multiple reassortment between the pandemic virus and endemic swine viruses. Nine reassortant viruses representing 7 genotypes were detected in commercial pig farms in the United States. Field observations suggested that the newly described reassortant viruses did not differ substantially from pandemic (H1N1) 2009 or endemic strains in their ability to cause disease. Comparable growth properties of reassortant and endemic viruses in vitro supported these observations; similarly, a representative reassortant virus replicated in ferrets to the same extent as did pandemic (H1N1) 2009 and endemic swine virus. These novel reassortant viruses highlight the increasing complexity of influenza viruses within pig populations and the frequency at which viral diversification occurs in this ecologically important viral reservoir.

The impact of chronic cadmium exposure and slow accumulation on the occurrence and development of diabetes is controversial for human populations. Islets of Langerhans play a prominent role in the etiology of the disease, including by their ability to secrete insulin. Conversion of glucose increase into insulin secretion involves mitochondria. A rat model of pancreatic β-cells was exposed to largely sub-lethal levels of cadmium cations applied for the longest possible time. Cadmium entered cells at concentrations far below those inducing cell death and accumulated by factors reaching several hundred folds the basal level. The mitochondria reorganized in response to the challenge by favoring fission as measured by increased circularity at cadmium levels already ten-fold below the median lethal dose. However, the energy charge and respiratory flux devoted to adenosine triphosphate synthesis were only affected at the onset of cellular death. The present data indicate that mitochondria participate in the adaptation of β-cells to even a moderate cadmium burden without losing functionality, but their impairment in the long run may contribute to cellular dysfunction, when viability and β-cells mass are affected as observed in diabetes.

The BD FACSPresto™ Near-Patient CD4 Counter was developed to expand HIV/AIDS management in resource-limited settings. It measures absolute CD4 counts (AbsCD4), percent CD4 (%CD4), and hemoglobin (Hb) from a single drop of capillary or venous blood in approximately 23 minutes, with throughput of 10 samples per hour. We assessed the performance of the BD FACSPresto system, evaluating accuracy, stability, linearity, precision, and reference intervals using capillary and venous blood at KEMRI/CDC HIV-research laboratory, Kisumu, Kenya, and precision and linearity at BD Biosciences, California, USA.For accuracy, venous samples were tested using the BD FACSCalibur™ instrument with BD Tritest™ CD3/CD4/CD45 reagent, BD Trucount™ tubes, and BD Multiset™ software for AbsCD4 and %CD4, and the Sysmex™ KX-21N for Hb. Background Intraoperative antiplatelet therapy is recommended for emergent stenting during mechanical thrombectomy (MT). Most patients undergoing MT are also given thrombolysis. Antiplatelet agents are contraindicated within 24 hours of thrombolysis. We evaluated outcomes and complications of patients stented with and without intravenous aspirin during MT.Methods All patients who underwent emergent extracranial stenting during MT at the Royal Stoke University Hospital, UK between 2010 and 2020, were included. Patients were thrombolysed before MT, unless contraindicated. Aspirin 500 mg intravenously was given intraoperatively at the discretion of the operator. Symptomatic intracranial haemorrhage (sICH) and the National Institutes for Health Stroke Scale score (NIHSS) were recorded at 7 days, and mortality and functional recovery (modified Rankin Scale: mRS ≤2) at 90 days.Results Out of 565 patients treated by MT 102 patients (median age 67 IQR 57–72 years, baseline median NIHSS 18 IQR 13–23, 76 (75%) thrombolysed) had a stent placed. Of these 49 (48%) were given aspirin and 53 (52%) were not. Patients treated with aspirin had greater NIHSS improvement (median 8 IQR 1–16 vs median 3 IQR −9–8 points, p=0.003), but there were no significant differences in sICH (2/49 (4%) vs 9/53 (17%)), mRS ≤2 (25/49 (51%) vs 19/53 (36%)) and mortality (10/49 (20%) vs 12/53 (23%)) with and without aspirin. NIHSS improvement (median 12 IQR 4–18 vs median 7 IQR −7–10, p=0.01) was greater, and mortality was lower (4/33 (12%) vs 6/15 (40%), p=0.05) when aspirin was combined with thrombolysis, than for aspirin alone, with no increase in bleeding.Conclusion Our findings based on registry data derived from routine clinical care suggest that intraprocedural intravenous aspirin in patients undergoing emergent stenting during MT does not increase sICH and is associated with good clinical outcomes, even when combined with intravenous thrombolysis.
