Increased thyroid stimulating hormone (TSH) levels are detected quite often especially in older patients. Increased TSH level doesn't usually indicate the formation of persistent hypothyroidism and may be due to concomitant somatic disorders. Determining the threshold values of TSH associated with cardiovascular diseases (CVD) is an important clinical task.Aim. To study the detectability of subclinical hypothyroidism, the range of thyroid-stimulating hormone values associated with CVD in patients who seek medical care at the multidisciplinary hospital of the North-West region.Material and methods. 1340 patients of different sex and age with normal TSH value (0.4-2.5 mlU/l) and subclinical hypothyroidism were included into the study. Subclinical hypothyroidism was considered an increase in TSH level >4.0 mlU/L, provided that the level of free T4 is normal. The aim of our study is to assess the local cytokine levels as prognostic factors for early relapse in NMIBC patients. 75 patients with NMIBC were enrolled in the study: 51 with primary NMIBC and 24 with initially recurrent NMIBC, LG and HG tumors were diagnosed in each group. Patients with primary NMIBC were monitored during 9 months after treatment: TURB and chemotherapy (No. 6). During TURB samples of tumors were taken, supernatants were obtained and tissue cytokine levels were measured (IL-1β, IL-6, IL-10, IL-18, TNFα, IFNγ, IL-8) by ELISA test. The results showed that in patients with primary NMIBC early relapses were diagnosed in 15 (46.8%) of LG tumors and in 11 (45%) of HG tumors matching that there was no difference depending upon tumor grade. In initially recurrent tumors of both LG and HG NMIBC the amounts of cytokines were maximal: in LG tumors they exceeded the primary ones from 7.1 (IFNγ) to 300 (IL-6) while in HG - from 2.0 (IL-10) to 9.7 (IL-6). The amounts of IL-1β, IL-6, IL-10, IFNγ, IL-8 were higher in those LG primary tumors which relapsed in 6-9 months compared to the ones which didn't, though their levels were much lower than in initially manifested relapse (from 2.6 times for IFNy to 150 times for IL-6). A similar trend, though not for all the same cytokines, was observed in HG tumors: tissue levels of IL-6, IL-10, IL-18 and TNFα were higher in tumors which relapsed in 6-9 months after treatment. The increase of 2 cytokines' levels were common for both LG and HG tumors (IL-6 and IL-10). This finding might be considered as a new prognostic factor of the early relapse. We conclude that relapse of LG and HG NMIBC is related to some immune mechanisms, namely to local hyperproduction of cytokines, especially IL-6 and IL-10, though IL-1β, IL-8, IFNγ could have an impact on LG and IL-18, TNFα — on HG tumors. Taking into account common signaling pathways of IL-6 and IL-10 like JAK/STAT, these transcription factors might be potential targets for new effective approaches to treatment.

The article examines the processes of internationalization, provides information on the number of foreign students in various countries and in Russia, discusses the arguments for choosing the country for study by international students, including socio-cultural and linguistic preferences. The authors address the issues relating to the status of the Russian language in countries of the Central Asia region – the former republics of the Soviet Union. The article provides an information on the expansion of Western and Eastern countries into the educational system of these countries. The conclusion is made about the need for Russian universities to more actively enter the international market of educational services, especially in those areas where the Russian Federation has undeniable achievements (natural science fields of training, etc.).

The attractiveness of the University in the world market of educational services is an indicator of its success, which is estimated by a set of criteria including the number of foreign students studying there. For Russian universities, the number of international students is one of the key indicators of internationalization. The authors pay special attention to the criterion of internationalization of higher education in the world ranking of universities. The article provides data related to measures to attract young foreigners to study in higher education institutions of the Russian Federation, including the Volgograd State Technical University. Today, an interest in engineering professions among foreign applicants has increased markedly. The effectiveness of the learning process among foreign students, their achievement levels strongly depend on the success of their adaptation to the new socio-cultural environment. The article addresses the problems of language adaptation of foreign students who came from the former Soviet republics, mainly on the example of students from Turkmenistan.

Purpose of the study. To study the features of the local distribution of populations of immune system cells in patients with non-invasive muscular bladder cancer of various degrees of malignancy.Materials and methods. The study included 51 patients with newly diagnosed non-muscle-invasive bladder cancer (papillary urothelial carcinoma) who received complex treatment and follow-up after 9 months at the oncourological department of the National Medical Research Center of Oncology. Patients were divided into two groups: group 1 – with a tumor of low malignant potential (Low grade – LG), n = 31; group 2 – with a tumor of high malignant potential (High grade – HG), n = 20. After 6–9 months, 24 patients were diagnosed with a relapse of the disease – in 48,4 % in patients of group 1 (n = 15) and in 45 % – in group 2 (n = 9). In cell suspensions obtained from the primary and recurrent tumors, as well as the perifocal zone, the relative number of populations of immunocompetent cells was estimated using flow cytometry. A comparison was made of the content of individual populations of lymphocytes in the tumor tissue, the perifocal zone of primary and recurrent lesions of various degrees of malignancy. Statistical processing was performed using Statistica 13.0.Results. The development of a recurrent tumor of low malignant potential is accompanied by the involvement of cells of innate immunity (NK- and NKT-lymphocytes) into its microenvironment, which is associated with an imbalance in the number of main cells of adaptive immunity – a fairly pronounced decrease in the tumor of T-lymphocytes of the helper-inductor type was noted with a constant content cytotoxic T-lymphocytes, as well as the multidirectional nature of changes in DP- (decrease) and DN-lymphocytes (increase). A feature of the development of a recurrent tumor of high malignant potential is that it is accompanied by the involvement of innate immunity cells (NK- and NKT-lymphocytes) into its microenvironment, as well as multidirectional changes in DP- (decrease) and DN-lymphocytes (increase).Conclusion. Studies of the population composition of tumors and their perifocal tissues of NMIBC revealed a number of features that are reflected in the redistribution of cytolytic cells, the formation of immunosuppressive conditions, which are reflected both in the manifestation of the biological properties of tumor cells and in changes in the cellular composition of bladder tissues involved in the process. development and progression of cancer.

Purpose of the study. Determine the content of cancer stem cells (CSCs) in the tumor tissue (TT) and perifocal tissues (PT) in muscle-non-invasive bladder cancer.Materials and methods. We’ve examined fragments of TT and PT of 7 muscle-non-invasive bladder cancer (NMIBC) after surgical intervention – transurethral resection of the urinary bladder (TUR). In tissue samples that were used to obtain cell suspension of TT and PT using the BD Medimachine apparatus (BD, USA) was treated with monoclonal antibodies CD45-APCCy7, CD44-FITC, CD133-РЕ, CD24-PE (BD, USA) and were assessed on flow cytometer FacsCantoII (BD, USA). The percentage of cells with CSC phenotypic markers was determined in the analysis sample: CD45-CD44+CD24+, CD45-CD44+, CD45-CD24+, CD45-CD133+, CD45-CD44+CD133+. The presence of significant differences in the groups was evaluated using the STATISTICA 13 software package and the differences between the samples were considered significant at p < 0.05. The percentage of cells of the corresponding phenotype was calculated relative to the total number of cells. The relative numbers of cells with CSC phenotypic markers, such as CD24, CD44, were 77 % and 58 % higher in TT than in PT: 18.3 ± 3.5 vs. 4.3 ± 2.1, p ≤ 0.044 and 15.5 ± 5.3 vs. 6.5 ± 0.8, p ≤ 0.043, respectively. The number of CD133+ cells was 83 % higher in PT compared to TT – 41.6 ± 12.1 vs. 22.7 ± 7.6, p ≤ 0.047.Conclusion. The study of CSCs is a promising direction for the study of oncogenesis and can be used to assess the nature of the further development of relapse and / or progression of the disease, as well as various therapeutic approaches that are aimed at eliminating with CSC phenotypic markers and blocking the pathways leading to the emergence and maintenance of this cell population in patients with NMIBC.
