Introdução: Nos últimos anos, temos observado aumento do número de pacientes internados na UTI do Instituto de Infectologia Emílio Ribas (IIER) com septicemia por Staphylococcus aureus resistente à meticilina da comunidade (CA-MRSA). Como primeira linha, iniciamos o tratamento com Vancomicina. No entanto, muitos pacientes apresentam piora da função renal após a introdução deste antibiótico. Nesse relato, trazemos 2 casos em que, após piora da função renal, suspendemos Vancomicina e introduzimos Ceftaroline. Ambos pacientes evoluíram com melhora clínica e melhora da função renal. Caso 1: Paciente do sexo feminino, 32 anos, previamente hígida. Deu entrada no Pronto-Socorro do IIER com história de tosse secretiva, queda do estado-geral e febre há 3 dias da admissão. Evolui com Síndrome Respiratória Aguda Grave (SRAG) e choque séptico de foco pulmonar, necessitando de intubação orotraqueal (IOT) e droga vasoativa (DVA). Na hemocultura coletada durante a abertura do protocolo de sepse, houve crescimento de S. aureus MRSA. Introduzido Vancomicina. Paciente começou a apresentar melhora dos parâmetros infecciosos, mas também importante piora da função renal. Foi optado então em suspender a Vancomicina e introduzir Ceftaroline. Paciente apresentou melhora da função renal, assim como do quadro clínico e dos parâmetros infecciosos. Paciente recebeu alta após 44 dias de internação hospitalar. Caso 2: Paciente do sexo masculino, 41 anos, morador de área livre, HIV em abandono de tratamento, usuário de crack há 26 anos. Deu entrada no Pronto-Socorro do IIER com história de prostração, astenia, tosse secretiva, dispneia e dor ventilatório-dependente há 1 semana. Referia também presença recorrente de abcessos pelo corpo, principalmente em membros, e com acometimento de região sacral na admissão. Evolui com Síndrome Respiratória Aguda Grave (SRAG) e choque séptico de foco pulmonar, necessitando de intubação orotraqueal (IOT) e droga vasoativa (DVA). Na hemocultura coletada durante a abertura do protocolo de sepse, houve crescimento de S. aureus MRSA. Realizo também TRM-Tb e pesquisa de BAAR em escarro e LBA, negativos. Realizada também pesquisa para Pneumocystis jirovecii, negativa. Introduzido portando Vancomicina. No entanto, paciente apresentou piora da função renal importante. Realizada substituição de Vancomicina por Ceftaroline. Paciente apresentou melhora da função renal, assim como do quadro clínico e dos parâmetros infecciosos.

Introdução: Hiperferritinemia à admissão é reconhecida como determinante de desfecho desfavorável em COVID-19. Mas a ferritina plasmática é também um sensível biomarcador de grande utilidade na monitorização da atividade inflamatória nestes pacientes. Relato de caso: Apresentamos dois casos nos quais a monitorização contínua deste biomarcador permitiu a detecção de Sd. Hiperferritinêmica que responderam favoravelmente à pulsoterapia com metilprednisolona. Caso 1 - Paciente do sexo masculino, 28 anos, previamente hígido. Negava comorbidades. Internado em nosso serviço no sexto dia de sintomas da COVID-19, evoluindo para SRAG. Cursou ao longo dos dias com piora expressiva do padrão respiratório, sendo necessária a realização de intubação orotraqueal. Concomitantemente à deterioração respiratória, exames laboratoriais evidenciaram aumento abrupto de proteína C reativa e de ferritina. Suscitada a hipótese de síndrome hiperferritinêmica. Assim, optou-se pela instituição de pulsoterapia com Metilprednisolona 1g/dia EV por 05 dias. Paciente evoluiu com melhora do padrão respiratório e queda da ferritina, sendo extubado dez dias depois. Caso 2 - Paciente do sexo feminino, 49 anos. Apresentava sobrepeso e hipertensão arterial sistêmica. Internada no nosso serviço no sétimo dia de sintomas da COVID-19. Evoluiu para SRAG, realizada intubação orotraqueal de urgência. Ao longo da internação, cursou com múltiplas complicações como infecção de corrente sanguínea, candidemia, pneumonia associada à ventilação, insuficiência renal aguda, infecção intestinal por Schistosoma mansoni e Giardia lamblia e TRALI - lesão pulmonar aguda associada à transfusão. Além da deterioração clínica progressiva, apresentou aumento significativo dos níveis de ferritina, caracterizando uma síndrome hiperferritinêmica, o que motivou a instituição de pulsoterapia com Metilprednisolona 1g EV ao dia por três dias. Após quarenta e quatro dias em ventilação mecânica, foi possível a transição para nebulização em traqueostomia. Recebeu alta após cinquenta e sete dias de internação hospitalar, confortável em ar ambiente, com sequela de tremor de extremidades e marcha atáxica. Discussão: A partir desses dois relatos de casos, propomos uma discussão sobre a associação entre COVID-19 grave e síndrome hiperferritinêmica, com suas possíveis abordagens terapêuticas.

Progressive multifocal leukoencephalopathy (PML) is a feared entity that occurs most frequently in conditions of extreme immunodeficiency. The diagnosis is often made long after the onset of symptoms due to the physicians’ unfamiliarity, and the unavailability of diagnostic tests in some medical centers. Although the incidence of PML is decreasing among HIV patients with the advent of highly active antiretroviral therapy (HAART), in Brazil this entity is the fourth highest neurological complication among these patients. The authors present the case of a middle-aged man who tested positive for HIV concomitantly with the presentation of hyposensitivity in the face and the right side of the body, accompanied by mild weakness in the left upper limb. The clinical features worsened rapidly within a couple of weeks. The diagnostic work-up pointed to the working diagnosis of PML after brain magnetic resonance imaging; however, the detection of the John Cunningham virus (JCV) in the cerebral spinal fluid was negative. HAART was started but the patient died after 7 weeks of hospitalization. The autopsy revealed extensive multifocal patchy areas of demyelination in the white matter where the microscopy depicted demyelination, oligodendrocytes alterations, bizarre atypical astrocytes, and perivascular lymphocytic infiltration. The immunohistochemistry was positive for anti-SV40, and the polymerase chain reaction of the brain paraffin-embedded tissue was positive for JCV. The authors highlight the challenges for diagnosing PML, as well as the devastating outcome of PML among HIV patients.

Abstract Background Virtual slides are viewed using interactive software that enables the user to simulate the behaviour of a conventional optical microscope, like adjusting magnifications and navigating to any portion of the image. Nowadays, information about the performance and features of web-based solutions for reading slides in real environments is still scarce. The objective of this study is analyzing the subjective experience of pathologists with virtual slides, comparing the time needed to read slides using different web viewers and different network connections. Methods Eight slides were randomly selected (4 biopsies and 2 cytologies) from Hospital General de Ciudad Real (HGCR) archives. Three different virtual slide web-viewing solutions were analyzed: Aperio web server, Olympus NetImage Server, and Aurora mScope. Five pathologists studied to time needed to access images of each virtual slide, selecting a panoramic view, 10 low magnification fields, and 20 high magnification fields. Results Aperio viewer is very efficient in overview images. Aurora viewer is especially efficient in lower magnifications (10×). For larger magnifications (20× and 40×) no significant differences were found between different vendors. Olympus was found to be the most user-friendly interface. When comparing Internet with intranet connections, despite being slower, users also felt comfortable using virtual slides through Internet connection. Conclusion Available web solutions for virtual slides have different advantages, mainly in functionalities and optimization for different magnifications. Pathologist should select the solutions adapted to their needs.

Objective: To characterize the pediatric patients presenting at the two pediatric centers in Bogotá, with first isolate urine culture of community-acquired extended-spectrum b-lactamase (ESBL)-producing enterobacteriaceae., Methods: Review of microbiological data of children between January, 2012 and December, 2018, obtained using the WHONET software., Results: A total of 2657 Escherichia coli, Klebsiella spp and Proteus mirabilis - positive urine cultures were obtained within a 6-year period; data of 132 patients were finally selected. Frequency of ESBL-producing bacteria infections in community-acquired urinary tract infections (UTI) was 5%: 123 E. coli (93.2%), 7 K. pneumoniae (5.2%), 1 K. oxytoca (0.8%), and 1 P. mirabilis (0.8%)., Conclusion: A predominance of female sex, preschool children, and lower tract urinary infections were found, as well as a low frequency of comorbidities. Adequate sensitivity to amikacin and nitrofurantoin was found in this study.
