Anticancer peptides (ACPs) are a group of peptides that exhibit antineoplastic properties. The utilization of ACPs in cancer prevention can present a viable substitute for conventional cancer therapeutics, as they possess a higher degree of selectivity and safety. Recent scientific advancements generate an interest in peptide-based therapies which offer the advantage of efficiently treating intended cells without negatively impacting normal cells. However, as the number of peptide sequences continues to increase rapidly, developing a reliable and precise prediction model becomes a challenging task. In this work, our motivation is to advance an efficient model for categorizing anticancer peptides employing the consolidation of word embedding and deep learning models. First, Word2Vec, GloVe, FastText, One-Hot-Encoding approaches are evaluated as embedding techniques for the purpose of extracting peptide sequences. Then, the output of embedding models are fed into deep learning approaches CNN, LSTM, BiLSTM. To demonstrate the contribution of proposed framework, extensive experiments are carried on widely-used datasets in the literature, ACPs250 and independent. Experiment results show the usage of proposed model enhances classification accuracy when compared to the state-of-the-art studies. The proposed combination, FastText+BiLSTM, exhibits 92.50% of accuracy for ACPs250 dataset, and 96.15% of accuracy for the Independent dataset, thence determining new state-of-the-art.

The objective is to review relevant literature on emergency services patient management, focusing on Koenig’s 1996 Simple Triage and Rapid Treatment (START) and Secondary Assessment of Victim Endpoint (SAVE) frameworks. Establishing a comprehensive seismic and mass casualty incident (MCI) protocol chain is the goal. The prehospital phase of seismic MCIs treats hypovolemia and gets patients to the nearest hospital. START-A plans to expedite emergency patient triage and pain management. The SAVE algorithm is crucial for the emergency patient secondary assessment. It advises using Glasgow Coma Scale, Mangled Extremity Severity Score, Burn Triage Score, and Safe Quake Score for admission, surgery, transfer, discharge, and outcomes. This compilation emphasizes the importance of using diagnostic tools like bedside blood gas analyzers and ultrasound devices during the assessment process, drawing from 6 February earthquake research. The findings create a solid framework for improving emergency medical response strategies, making them applicable in similar situations.

Introduction: Suppression of tumorigenicity 2 (ST2) plays a key role in the serious complications that may occur during Coronavirus disease-2019 (COVID-19), including systemic inflammatory condition, sepsis, Acute respiratory distress syndrome, and fibrosis. In this study, we evaluated the relationship between the serum ST2 concentrations and the mortality and need for mechanical ventilation. Materials and Methods: A prospective observational study was conducted in patients diagnosed with COVID-19 who presented to the emergency department (ED) between October 2021 and April 2022. Patients with Severe acute respiratory syndrome-Coronavirus-2 infection were consecutively enrolled according to the clinical spectrum defined in the guidelines. Admission to the intensive care unit (ICU), requirement of intubation, and 90-day mortality were the primary study outcomes. Clinical, imaging and laboratory data were assessed to determine the clinical spectrum and severity scores. ST2 was assessed using the micro ELISA method and Sandwich-ELISA principle. Results: Of the 64 (48.5%) patients admitted to the ICU, 43 (32.6%) required mechanical ventilation. During the 90-day follow-up period, 44 (33.3%) patients died due to clinical deterioration. Objective: Upper gastrointestinal system (UGIS) bleeding is a life-threatening abdominal emergency. Numerous scoring systems have been developed to identify patients who may develop mortality due to UGIS bleeding. We aimed to determine the effectiveness of the Glasgow Blatchford Score (GBS), Rockall score (RS), and AIMS 65 score in predicting the length of hospital stay, re-bleeding, and transfusion need. Material and Methods: It was carried out retrospectively by recording the parameters and clinical scoring systems collected from the archive files and epicrisis information of the patients with the pre-diagnosis of UGIS hemorrhage. Results: Sixty-three (67.7%) of 93 patients were male. Four patients (4.3%) needed intensive care, and in-hospital mortality occurred in 4 (4.3%) patients. BACKGROUND: Selective lung intubation is a life-saving procedure in emergency departments. While various equipment can be used in selective lung intubation, most of this equipment is not readily available; therefore, single-lumen endotracheal intubations are performed for rapid interventions.MATERIALS AND METHODS: This study was designed as a randomized, prospective, cross-over study using the 90° rotation technique for selective intubation on a manikin model with and without endotracheal tube introducer (ETI) in difficult airway settings. Forty-six emergency physicians were included in the study. The primary outcome was evaluating time to selective intubations, and secondary outcomes were first and second attempt success rates and the self-perceived difficulty level of each method according to the participants.RESULTS: The mean time to the first successful endotracheal intubation was significantly longer for both right selective and left selective intubations with ETI utilization than without ([39.71 ± 9.83 vs. 21.86 ± 5.94 s], [P < 0.001]), ([42.2 ± 10.81 vs. 26.23 ± 7.97 s], [P < 0.001], respectively). The first-pass success rate did not differ for right selective intubation with or without an ETI (45/46 [97.8%] and 45/46 [97.8%], respectively). However, the first-pass success rate for left selective intubation was significantly higher with ETI as compared to without an ETI (30/46 [65.2%] and 13/46 [28.3%], respectively) (P < 0.001).CONCLUSIONS: While the success rates of right selective intubation were the same, the left selective intubation success rates with ETI are higher than the styletted endotracheal tube, which can be strong evidence for this method's applicability in practice. Expanding the use of ETI and increasing the experience of the practitioners can contribute to further success.

A 27-year-old female patient, G2P1, presented to the emergency department (ED) with acute onset nausea, vomiting, and mild chronic abdominopelvic pain. Physical examination revealed bilateral lower quadrant tenderness without rebound, guarding, or rigidity, and vital signs were within normal limits. Electrolytes, complete blood count, and liver and kidney function tests were normal. A pregnancy test was negative, and urinalysis did not reveal any abnormalities. No free fluid was observed in the abdominal ultrasound, and the ovaries and other intra-abdominal structures were found to be normal. The patient underwent intravenous contrast-enhanced abdomiopelvic computed tomography (CT) scan.

Amaç: Çalışmamızda dermatolojik şikayetlerle acil kliniğine başvuran hastaların demografik verilerinin tanımlanması, gerçek dermatolojik acil vakaların oranının belirlenmesi ve beck anksiete ve depresyon ölçekleri ile başvuru nedenlerinin psiko-sosyal zemininin ve acil servise acil olmayan hastaların başvuru gerekçelerinin tanımlanması amaçlanmıştır. Materyal ve Metot: Prospektif gözlemsel ve anket çalışma araştırma tek merkez olarak 3 aylık dönemde acil tıp kliniğine dermatolojik aciller nedeniyle başvuran 18 yaşından büyük hastalar dahil edilerek yapıldı. Çalışmada acil servise başvuran bu hastalar ilk tanı ve tedavileri acil tıp hekimleri tarafından yapıldıktan sonra dermatoloji uzmanına konsulte edildi. Hastaların demografik verileri, dermatolojik yaşam indeksi, beck anksiete ve depresyon ölçeği kayıt altına alındı. Bulgular: 200 hastanın ortalama yaşı 40,5 (16,39), 116’ sı (%58) kadındı. En sık başvuru grubunun şikayetleri 1 ay ve üzerinde (n=58, %29) idi. Gerçek dermatolojik hasta grubu tüm hastaların %29,5’ i idi. En sık başvuru nedeni hastaların durumlarının acil olduğunu düşünmeleri (%47,50) ve klinik tanıları en sık ürtiker (%24) idi. Hastalar dermatolojik yaşam kalite indeksine göre; 79’ unun (%39,5) orta derecede etkilendiği gözlendi. Beck anksiete puan ölçeğine göre tüm hastaların ortalama değeri 15,22 12,56’ idi. Beck depresyon sınıflamasına göre hastaların ortalaması 12,8 +8,9’ idi. Hastalar beck depresyon-anksiete ölçeği ve dermatolojik yaşam indeksine göre hafif ve orta-şiddetli olarak 2 gruba ayrıldığında her iki grup arasında cinsiyet, yaş, medeni durum, eğitim durumu, gelir düzeyi, şikayet başvuru süresi, başvuru sebebi arasında istatistiksel olarak anlamlı fark tespit edilmedi, sadece beck anksiete depresyon ölçeğine göre cinsiyet farklılıklarına göre istatistiksel olarak anlamlı fark tespit edildi (p=0,029). Sonuç: Çalışmamız acilden gelen hastaların kısa sürede dermatoloji hekimi tarafından değerlendirilmesi ile daha doğru tanı, hızlı tedavi imkanı sağlaması yanısıra gereksiz tıbbi yaklaşımların ve masrafların önlenmesinde fayda sağlamıştır. Hastaların acile başvurularında anksiyete düzeylerinin yüksek olmasının önemli bir faktör olduğu, bu nedenle bilinçlendirme çalışmalarının gerek sosyal medya gerek hastane temelli çalışmalarla bu noktaya değinilerek artırılması sık başvurunun önlenmesinde fayda sağlayacaktır kanaatindeyiz.

Background:Although water-pipe smoking is a great public health problem, data regarding the acute and chronic effects and the degree of toxin exposure are limited. While water pipe-related malignancy, pulmonary, infectious, cardiac effects, infertility, and biological effects have been described in a meta-analysis, there are no studies in the literature about its neurologic effects.Aims:To evaluate water pipe-related acute neurological effects and cerebral blood flow through transcranial Doppler ultrasonography and serum S100 calcium binding protein calcium binding protein level measurements.Study Design:Prospective observational study.Methods:Vital signs and baseline carboxyhemoglobin and S100 calcium binding protein levels, cerebral flood changes with transcranial Doppler ultrasound were evaluated and recorded before and after water-pipe smoking.Results:The mean age of the 31 volunteers was 30.61 (±5.67) years, and 24 of them (77.42%) were male. Amaç: Bu klinik çalışmada amaç pulmoner tromboemboli tanısı alan hastalarda ortalama trombosit hacmi ile emboli yükünün ilişkisini ortaya koymaktır.Gereç ve Yöntem: Bu retrospektif çalışmaya pulmoner emboli tanısı alan 120 hasta dahil edildi. Hastaların ortalama trombosit hacmi, Troponin-I, D-dimer değerleri, arter kan gazları, göğüs radyografisi, elektrokardiyografi, ekokardiyografi ve venöz doppler ultrason bulguları kaydedildi. Ayrıca bilgisayarlı tomografi anjiyografi kullanılarak pulmoner arteriyel obstrüksiyon indeksi hesaplandı.Bulgular: Ortalama trombosit hacmi ile pulmoner arter basıncı arasında ve pulmoner arteriyel obstrüksiyon indeksi arasında pozitif korelasyon tespit edildi. Ortalama trombosit hacmi < 8.5fl olan ve > 8.5fl olan gruplar arasında pulmoner arter obstrüksiyon indeksi açısından istatistiksel anlamlı bir fark tespit edildi (25.0 and 45.0). Ortalama trombosit hacmi > 8.5fl olan olan grupta pulmoner arter basıncı < 8.5fl olan gruptan istatistiksel anlamlı olarak yüksek bulundu.Sonuç: Pulmoner arteriyel bilgisayarlı tomografi obstrüksiyon indeksi ile ortalama trombosit hacmi arasındaki korelasyonun pulmoner embolizm şiddetini belirlemedeki değeri net değildir.

SUMMARY: Objectives: The aim of this study was to determine the antibiotic resistance of infectious and non-infectious E. coli species in order to increase the success of empirical antibiotic treatment in urinary system infections. Methods: The antibiotic susceptibility of 464 E. coli strains that were isolated from urine samples of patients who visited Derince Training and Research Hospital Emergency Department between January 1 and December 31, 2012 were retrospectively evaluated from records. The antibiogram results were classified as susceptible, moderately susceptible or resistant. Moderately susceptible strains were assumed to be resistant. Results: Bacterial proliferation was seen in 563 (28.1%) of the 1998 urine cultures tested. One hundred and twelve cultures could not be evaluated due to contamination, and there was no proliferation in 1323 cultures. E. coli strains were isolated in 464 (82.4%) of the cultures in which proliferation was seen. Three hundred and sixty seven (79%) of the patients were female, 97 (21%) were male, and the mean age of all of the patients was 41.1±24.1 years (min: 1, max: 90). The antibiograms of the E. coli strains revealed that meropenem had the lowest resistance (0%), while ampicillin-sulbactam had the highest resistance (36.8%). Conclusions: In this study, we investigated the antibiotic resistance of E. coli strains isolated from urine cultures in our region. Future studies, perhaps similar to this one, can be performed in the future to increase the success of treatments. ÖZET: Amaç: Bu çalışmada üriner sistem enfeksiyonlarında ampirik antibiyotik tedavi başarısını artırmak için enfeksiyon etkeni olan veya olmayan E.coli suşlarının çeşitli antibiyotik türlerine direnci araştırıldı. Gereç ve Yöntem: 1 Ocak-31 Aralık 2012 tarihleri arasında Derince Eğitim ve Araştırma Hastanesi acil servisine başvuran hastaların mikrobiyoloji laboratuvarına gönderilmiş idrar örneklerinden izole edilen 464 E.coli suşunun antibiyotik duyarlılıkları bilgisayar kayıtları üzerinden retrospektif olarak değerlendirildi. Antibiyogram sonuçları duyarlı, orta duyarlı ve dirençli olarak sınıflandırıldı. Orta duyarlı suşlar dirençli kabul edildi. Bulgular: 1998 idrar kültüründen 563'ünde (%28.1) üreme oldu. Kültürlerin 112'si kontaminasyon nedeniyle değerlendirilemedi, 1323 kültürde ise üreme olmadı. Üreme olan kültürlerden 464'ünde (%82.4) E.coli suşları izole edildi. Hastaların 367'si (%79) kadın 97'si (%21) erkek, tüm hastaların yaş ortalaması 41.1±24.1 (min: 1, maks: 90) idi. E.coli suşlarına karşı antibiyogramlar incelendiğinde, direncin en düşük görüldüğü antibiyotik meropenem (%0), en yüksek görüldüğü antibiyotik ise ampisilinsulbaktam olarak saptandı (%36.8). Sonuç: Bölgemizde idrar kültürlerinden izole edilen E.coli suşlarına karşı antibiyotik dirençlerini incelediğimiz çalışmamızın benzerlerinin ilerleyen dönemde yapılmasının tedavi başarısına yardımcı olacağını düşünmekteyiz.
