Anti-IL5/IL5Rs have been studied in patients with chronic obstructive pulmonary disease (COPD) without convincing benefits. However, these therapies have been used in clinical practice in COPD with apparently good results.Purpose: To describe the clinical characteristics and therapeutic response of COPD patients treated with anti-IL5/IL5R in a real-world setting.Patients and Methods: This is a retrospective case series of patients followed at the Quebec Heart and Lung Institute COPD clinic. Men or women, with an established diagnosis of COPD, and treated either with Mepolizumab or Benralizumab were included. Demographics, disease and exacerbation-related data, airway comorbidities, lung function, and inflammatory profile were extracted from patients’ hospital files at baseline visit and 12 months post-treatment. Therapeutic response to biologics was assessed by measuring change in annual exacerbation rate and/or OCS daily dose.Results: Seven COPD patients treated with biologics were identified (5M:2F). All were found to be OCSdependent at baseline. Radiological evidence of emphysema was found in all patients. One case was diagnosed with asthma before age 40. Residual eosinophilic inflammation was found in 5/6 patients (blood eosinophils count 237 ± 225× 106 cells/L) despite chronic OCS use. After 12 months of anti-IL5 treatment, mean OCS dose dropped from 12.0 ± 7.6 to 2.6 ± 4.3 mg/day, representing a 78% decrease. Annual exacerbations rate was reduced by 88%, from 8.2 ± 3.3 to 1.0 ± 1.2 per year.Conclusion: Chronic OCS use is a common characteristic of patients treated with anti-IL5/IL5R biological therapies in this real-world setting. Fractional exhaled nitric oxide (FeNO), a surrogate marker of eosinophilic airway inflammation, is expected to decrease after their initiation. Our center noticed increased FeNO levels in a few patients in whom anti-IL5/IL5R therapy was initiated. Limited data are available on the kinetics of T2 inflammation biomarkers after initiation of a biologic in that population. This study aims to identify if a subgroup of severe asthma patients experiences increased FeNO levels after initiation of anti-IL5/IL5R therapy and to describe their clinical characteristics.Patients and Methods: This is a retrospective case series of 5 patients on Benralizumab (4M:1F) and 8 on Mepolizumab (5M:3F) who showed a significant increase in FeNO (> 20% AND > 25 ppb) following initiation of an anti-IL5/IL5R treatment. Clinical data, expiratory flows, and inflammation were extracted from the patients’ chart at initiation of treatment (T0), 3 months (T1) and 12 months (T2) post-treatment. Descriptive statistics were used.Results: In patients treated with Benralizumab, the increase in FeNO was observed between T0 and T1 (mean delta = 82 ± 72 ppb) with a subsequent decrease (N = 3). In most patients taking Mepolizumab (N = 6), the FeNO increase was observed between T1 and T2 (mean delta = 57 ± 35 ppb). Under treatment, no Benralizumab patient experienced asthma exacerbation while two on Mepolizumab did. All patients had a significant decrease in blood eosinophils.Conclusion: Although initiation of anti-IL5/IL5R may cause a transient rise in FeNO levels in a subgroup of patients, it does not appear to affect clinical outcomes. A compensatory mechanism involving other inflammatory pathways such as IL13 or IL4, both involved in FeNO production, could theoretically explain these findings. 3. Adaptative water quality management in water distribution systems by optimizing control valves and chlorination booster stations.

Abstract

Good water quality in a water distribution system (WDS) can be reached by using flow control valves (FCV) to reduce water residence time (WRT), and chlorine booster stations to maintain acceptable free residual chlorine concentrations (FRCC). The research developed a cost-effective, adaptative approach for managing FCV and chlorine booster stations, while maintaining acceptable pressure and FRCC ranges. The methodology is applied to a real full-scale case study considering three optimization formulations: 1) FCV operation, 2) chlorine booster station operation, and 3) combining them in an adaptive management strategy. Results revealed that chlorine booster stations individually or combined with FCV enhanced water quality significantly at an acceptable cost, while FCV optimization alone or combined with chlorine booster stations reduced WRT by, at most, 3.8%. Chlorine booster stations were thus more effective than FCV in this case study. This formulation can be applied to other WDS to improve water quality cost-effectively. [ABSTRACT FROM AUTHOR]

5. Exploring the Potential Impact of GLP-1 Receptor Agonists on Substance Use, Compulsive Behavior, and Libido: Insights from Social Media Using a Mixed-Methods Approach.

Abstract

Glucagon-like peptide-1 (GLP-1) is involved in a range of central and peripheral pathways related to appetitive behavior. Hence, this study explored the effects of glucagon-like peptide-1 receptor agonists (GLP-1 RAs) on substance and behavioral addictions, including alcohol, caffeine, nicotine, cannabis, psychostimulants, compulsive shopping, and sex drive/libido. Data were collected from various social platforms. Keywords related to GLP-1 RAs and substance/behavioral addiction were used to extract relevant comments. The study employed a mixed-methods approach to analyze online discussions posted from December 2019 to June 2023 and collected using a specialized web application. Reddit entries were the focus here due to limited data from other platforms, such as TikTok and YouTube. A total of 5859 threads and related comments were extracted from six subreddits, which included threads about GLP-1 RAs drugs and associated brand names. To obtain relevant posts, keywords related to potential substance use and compulsive behavior were selected. Further analysis involved two main steps: (1) manually coding posts based on users' references to the potential impact of GLP-1 RAs on substance use and non-substance habits, excluding irrelevant or unclear comments; (2) performing a thematic analysis on the dataset of keywords, using AI-assisted techniques followed by the manual revision of the generated themes. Second, a thematic analysis was performed on the keyword-related dataset, using AI-assisted techniques followed by the manual revision of the generated themes. In total, 29.75% of alcohol-related; 22.22% of caffeine-related; and 23.08% of nicotine-related comments clearly stated a cessation of the intake of these substances following the start of GLP-1 RAs prescription. Conversely, mixed results were found for cannabis intake, and only limited, anecdotal data were made available for cocaine, entactogens, and dissociative drugs' misuse. Regarding behavioral addictions, 21.35% of comments reported a compulsive shopping interruption, whilst the sexual drive/libido elements reportedly increased in several users. The current mixed-methods approach appeared to be a useful tool in gaining insight into complex topics such as the effects of GLP-1 RAs on substance and non-substance addiction-related disorders; some GLP-1 RA-related mental health benefits could also be inferred from here. Overall, it appeared that GLP-1 RAs may show the potential to target both substance craving and maladaptive/addictive behaviors, although further empirical research is needed. [ABSTRACT FROM AUTHOR]

Abstract

Recent media reports commented about a possible issue of the misuse of antidiabetics related to molecules promoted as a weight-loss treatment in non-obese people. We evaluated here available pharmacovigilance misuse/abuse signals related to semaglutide, a glucagon-like peptide-1 (GLP-1) analogue, in comparison to other GLP-1 receptor agonists (albiglutide, dulaglutide, exenatide, liraglutide, lixisenatide, and tirzepatide) and the phentermine–topiramate combination. To acheieve that aim, we analyzed the Food and Drug Administration's FDA Adverse Events Reporting System (FAERS) dataset, performing a descriptive analysis of adverse event reports (AERs) and calculating related pharmacovigilance measures, including the reporting odds ratio (ROR) and the proportional reporting ratio (PRR). During January 2018–December 2022, a total of 31,542 AERs involving the selected molecules were submitted to FAERS; most involved dulaglutide (n = 11,858; 37.6%) and semaglutide (n = 8249; 26.1%). In comparing semaglutide vs. the remaining molecules, the respective PRR values of the AERs 'drug abuse', 'drug withdrawal syndrome', 'prescription drug used without a prescription', and 'intentional product use issue' were 4.05, 4.05, 3.60, and 1.80 (all < 0.01). The same comparisons of semaglutide vs. the phentermine–topiramate combination were not associated with any significant differences. To the best of our knowledge, this is the first study documenting the misuse/abuse potential of semaglutide in comparison with other GLP1 analogues and the phentermine–topiramate combination. The current findings will need to be confirmed by further empirical investigations to fully understand the safety profile of those molecules. [ABSTRACT FROM AUTHOR]

Abstract

In the Saint-Charles River watershed, it is necessary to better manage water from both a qualitative and quantitative point of view. Hydrological modelling can help managers in their decision making regarding water. This work concerns the implementation of the semi-distributed water quality model soil and water assessment tool (SWAT) for the simulation of stream flows in this urbanized and relatively mountainous watershed. This first approach is compared with two others, one relying on the use of the GR4J global hydrological model, and the other relying on the use of an artificial neural network. The amount of water resulting from the melting of snow, here calculated by SWAT's snowmelt algorithm, has been provided to the models used in these other two approaches. The watershed offers an important challenge to hydrologists because of the scarcity of its database, namely the quasi absence of spatial information on precipitation and the absence of detailed pedological studies. Results show that the hydrological performance resulting from all three approaches is limited by the availability of data, but the performance of the approach using SWAT is more so than the other two. The lack of information hinders exploiting the full modelling capability of SWAT. Moreover, SWAT's complexity turns out to be disadvantageous when compared with the simpler model structures of GR4J and of the neural network. It thus appears essential, considering the Saint-Charles River watershed's strategic importance for water supply and touristic and recreational activities, that more effort should be spent in gaining climatological and pedological observations within the watershed. Sur le bassin versant de la riviere Saint-Charles, il est necessaire de mieux gerer l'eau, d'un point de vue quantitatif et qualitatif. Or la prise de decisions peut etre aidee par la modelisation hydrologique. Ce travail porte sur la mise en place du modele semi-distribue de qualite de l'eau SWAT (<< soil and water assessment tool >>) sur ce bassin versant urbain et relativement montagneux, et sur revaluation de sa capacite a simuler les debits. Cette premiere approche est comparee a deux autres, reposant pour l'une sur l'utilisation du modele hydrologique global GR4J, et pour l'autre sur l'emploi d'un reseau de neurones. La quantite d'eau issue de la fonte de la neige, calculee ici par le module de fonte de SWAT, a ete renseigneeea ces deux autres modeles. Ce bassin presente par ailleurs de reelles difficuMs pour ce type de modelisation, liees essentiellement au manque de donneees : quasi-absence d'information spatiale sur les precipitations et absence d'eetude pedologique detaillee. Les resultats, globalement satisfaisants, montrent toutefois que la performance hydrologique resultant des trois approches est limiteee par ces manques : celle utilisant SWAT davantage que les deux autres. Dans ce cas, les lacunes ne permettent pas d'utiliser le plein potentiel du modele SWAT, dont la structure plus lourde devient un inconvenient des que les donneees sont disponibles en quantite et en qualite insuffisantes pour en justifier l'exploitation, en opposition a GR4J et au reseau de neurones. Il semble donc incontournable, consideerant l'importance strategique du bassin versant de la riviere Saint-Charles pour l'approvisionnement en eau potable et le support aux activitesrecreatives et touristiques, que l'effort d'observation de ce territoire soit accru, afin d'y augmenter notamment les donneees climatologiques et pedologiques disponibles. Abstract

We examined the ontogeny of mRNA levels of IGF-I and -II, IGF type 1 (IGFI-R) and type II receptors (IGFII-R), IGF binding protein-1 and -3 (IGFBP-1 and -3), GH receptor (GHR), and tissue concentrations of IGF and IGFBP in the pancreas of pigs. Tissues were collected from fetuses at 90 and 110 d of gestation and from pigs at 1, 21, 90 and 180 d of age. Northern blots were performed using total RNA hybridized with 32p-labeled cDNA probes (human IGF-I and human IGFI-R) and cRNA probes (rat IGFII, human IGFII-R, human IGFBP-1, pig IGFBP-3, and pig GHR). There were two accelerated growth stages of the pancreas: the first one at 90 d of fetal life, which is characterized by cell hyperplasia (high ratio of DNA to body weight), and the second one at postnatal 90 d, which is attributed to cell hypertrophy (high ratios of pancreatic weight, RNA, and protein to DNA). The level of IGF-II mRNA and its tissue concentration were predominant during fetal life and low thereafter. The IGF-I mRNA level was high during fetal and early postnatal life and decreased thereafter. Messenger RNA levels of IGFI-R, IGFBP-3, and GHR and concentrations of IGFBP-1 and -2 were abundant during fetal and early postnatal life. In conclusion, IGF may be involved in various physiological periods of pancreatic development in pigs. Key Words: Pigs, Pancreas, Age, Messenger RNA, Insulin-Like growth factor
