Health communication has relevance for virtually every aspect of health and well-being, including disease prevention. This review explored the effectiveness of communications in enhancing the adoption of or adherence to behavioural interventions (non-pharmaceutical interventions (NPIs)) related to COVID-19. The review takes the UK as a case study and focuses on self-reported behaviours (e.g. social distancing). It also reviews the psychosocial determinants of adherence. Searches were conducted using PubMed, Scopus, CINAL, ASSIA and iCite databases. Eleven thousand five hundred records were identified and 13 were included in the final sample. Included studies suggest that NPI adoption or adherence was generally high, and communication had significant impacts, with key themes including clarity and consistency, trust and control. Based on the evidence in this review, features of effective communication in the context of NPI adoption or adherence are (i) information should be conveyed clearly and conflicting (mixed) messages should be avoided; (ii) information should be conveyed by trusted sources (e.g. health authorities) and (iii) communication should strike a balance between being authoritative but avoiding language seen as controlling (e.g. 'you must'). Future research should prioritize quantitative, experimental and longitudinal study designs, that focus specifically on communication as an intervention, and which measure behaviour. Abstract Background If UK healthcare services are to respond effectively to pandemic influenza, levels of absenteeism amongst healthcare workers (HCWs) must be minimised. Current estimates of the likelihood that HCWs will continue to attend work during a pandemic are subject to scientific and predictive uncertainty, yet an informed evidence base is needed if contingency plans addressing the issues of HCW absenteeism are to be prepared. Methods This paper reports the findings of a self-completed survey of randomly selected HCWs across three purposively sampled healthcare trusts in the West Midlands. The survey aimed to identify the factors positively or negatively associated with willingness to work during an influenza pandemic, and to evaluate the acceptability of potential interventions or changes to working practice to promote the continued presence at work of those otherwise unwilling or unable to attend. 'Likelihood' and 'persuadability' scores were calculated for each respondent according to indications of whether or not they were likely to work under different circumstances. Binary logistic regression was used to compute bivariate and multivariate odds ratios to evaluate the association of demographic variables and other respondent characteristics with the self-described likelihood of reporting to work. Results The survey response rate was 34.4% (n = 1032). Results suggest absenteeism may be as high as 85% at any point during a pandemic, with potential absence particularly concentrated amongst nursing and ancillary workers (OR 0.3; 95% CI 0.1 to 0.7 and 0.5; 95% CI 0.2 to 0.9 respectively). Conclusion Levels of absenteeism amongst HCWs may be considerably higher than official estimates, with potential absence concentrated amongst certain groups of employees. Although interventions designed to minimise absenteeism should target HCWs with a low stated likelihood of working, members of these groups may also be the least receptive to such interventions. Changes to working conditions which reduce barriers to the ability to work may not address barriers linked to willingness to work, and may fail to overcome HCWs' reluctance to work in the face of what may still be deemed unacceptable risk to self and/or family.

Abstract Background Healthcare workers (HCWs) will play a key role in any response to pandemic influenza, and the UK healthcare system's ability to cope during an influenza pandemic will depend, to a large extent, on the number of HCWs who are able and willing to work through the crisis. UK emergency planning will be improved if planners have a better understanding of the reasons UK HCWs may have for their absenteeism, and what might motivate them to work during an influenza pandemic. This paper reports the results of a qualitative study that explored UK HCWs' views (n = 64) about working during an influenza pandemic, in order to identify factors that might influence their willingness and ability to work and to identify potential sources of any perceived duty on HCWs to work. Methods A qualitative study, using focus groups (n = 9) and interviews (n = 5). Results HCWs across a range of roles and grades tended to feel motivated by a sense of obligation to work through an influenza pandemic. A number of significant barriers that may prevent them from doing so were also identified. Perceived barriers to the ability to work included being ill oneself, transport difficulties, and childcare responsibilities. Perceived barriers to the willingness to work included: prioritising the wellbeing of family members; a lack of trust in, and goodwill towards, the NHS; a lack of information about the risks and what is expected of them during the crisis; fear of litigation; and the feeling that employers do not take the needs of staff seriously. Barriers to ability and barriers to willingness, however, are difficult to separate out. Conclusion Although our participants tended to feel a general obligation to work during an influenza pandemic, there are barriers to working, which, if generalisable, may significantly reduce the NHS workforce during a pandemic. The barriers identified are both barriers to willingness and to ability. This suggests that pandemic planning needs to take into account the possibility that staff may be absent for reasons beyond those currently anticipated in UK planning documents. In particular, staff who are physically able to attend work may nonetheless be unwilling to do so. Although there are some barriers that cannot be mitigated by employers (such as illness, transport infrastructure etc.), there are a number of remedial steps that can be taken to lesson the impact of others (providing accommodation, building reciprocity, provision of information and guidance etc). We suggest that barriers to working lie along an ability/willingness continuum, and that absenteeism may be reduced by taking steps to prevent barriers to willingness becoming perceived barriers to ability.

Abstract Background Healthcare workers (HCWs) will be key players in any response to pandemic influenza, and will be in the front line of exposure to infection. Responding effectively to a pandemic relies on the majority of medical, nursing, laboratory and hotel services staff continuing to work normally. Planning assumes that during a pandemic normal healthcare service levels will be provided, although it anticipates that as caseloads increase only essential care will be provided. The ability of the NHS to provide expected service levels is entirely dependent upon HCWs continuing to work as normal. Methods/design This study is designed as a two-phase multi-method study, incorporating focus groups and a questionnaire survey. In phase one, qualitative methods will be used to collect the views of a purposive sample of HCWs, to determine the range of factors associated with their responses to the prospect of working through pandemic influenza. In phase two, the findings from the focus groups, combined with the available literature, will be used to inform the design of a survey to determine the generalisability of these factors, enabling the estimation of the likely proportion of HCWs affected by each factor, and how likely it is that they would be willing and/or able to continue to work during an influenza pandemic. Discussion There are potentially greater than normal health risks for some healthcare workers working during a pandemic, and these workers may be concerned about infecting family members/friends. HCWs will be as liable as other workers to care for sick family members and friends. It is vital to have information about how motivated HCWs will be to continue to work during such a crisis, and what factors might influence their decision to work/not to work. Through the identification and subsequent management of these factors it may be possible to implement strategies that will alleviate the concerns and fears of HCWs and remove potential barriers to working.

The influence of public awareness and knowledge in maintaining environmental regulations in urban areas is evaluated by focusing on expert views and surveys.

An empirical study of public conceptions of responsibility and choice in relation to health protection, and the influence of these upon responses to official information is presented using two contrasting studies of air pollution and child immunization. Conclusions are developed relating to information provision in an age promoting individualization and retraction of government, specially the need for explicit engagement with the benefits of health protection measures.

Communicating the rationale for allocating resources to manage policy priorities and their risks is challenging. Here, we demonstrate that environmental risks have diverse attributes and locales in their effects that may drive disproportionate responses among citizens. When 2,065 survey participants deployed summary information and their own understanding to assess 12 policy-level environmental risks singularly, their assessment differed from a prior expert assessment. However, participants provided rankings similar to those of experts when these same 12 risks were considered as a group, allowing comparison between the different risks. Following this, when individuals were shown the prior expert assessment of this portfolio, they expressed a moderate level of confidence with the combined expert analysis. These are important findings for the comprehension of policy risks that may be subject to augmentation by climate change, their representation alongside other threats within national risk assessments, and interpretations of agency for public risk management by citizens and others. Starting from a general understanding that experience of hazards is important in motivating protective response, this paper reports a novel study to understand the relationship between householder experience, understanding and response to two natural hazards - flooding and sea-level rise - in three contrasting high-risk areas of England. It presents a generic Individual Understanding and Response Framework (IURF) as a simple but potentially valuable means of comparing hazards and expressing the dynamic processes that appear to heighten or attenuate understanding and drive or constrain responses to specific natural hazards. The IURFs confirm the complexity of factors underlying householder understanding and response. Even in high-risk areas a lack of recent direct personal experience of flood events serves to attenuate understanding and to constrain motivation to take personal action. For sea-level rise, as yet a largely 'unknown' hazard in the local context, perceived responsibility to act is transferred to others. Social networks are confirmed as important local sources of information often more important than the official. People evaluate potential protection or mitigation measures in terms of their efficacy, cost and implementation barriers. This paper takes a new look at the importance of context - institutional and political - in effective public engagement processes. It does so through a rare comparative opportunity to examine the effectiveness of processes of public engagement in two UK waste authorities, where the same waste company was involved as both the primary contractor for the delivery of the waste management service (including new energy-from-waste facilities) and, furthermore, the same staff delivered the public engagement. Interrogating these cases affords the opportunity to place flesh on the bones of the sometimes 'abstract' skeleton of context. While engagement processes support effective local governance in an era of partnerships and deliberative democracy, the paper identifies that the methods adopted cannot be played out devoid of detailed understanding and response to local context, including the strength of partnership working between the public and private sector, the degree of political support for engagement, and the extent to which a traditional institutional paternalism still dominates. Learning that transcends participation processes is critical if public engagement is to translate into a legacy of enhanced environmental citizenship. However, a lack of empirical evidence has limited discussion to date to largely 'aspirational' claims. This paper offers the first rigorous examination of whether public participation does generate beyond-process social learning. Initially we review the literature on public participation and environmental citizenship to identify the key dimensions of social learning. We then re-visit a well-worked case study of an innovative public engagement process on the Hampshire waste strategy from the 1990s. Approximately one third of the original participants have been interviewed to identify whether and how the experience had a lasting effect on them. Key methodological difficulties are discussed, not least the analytical difficulties of attributing learning to a process that happened ten years previously. However, we argue that there is evidence that both instrumental and communicative learning have taken place, and conclude by identifying key areas that require further research. Public engagement through deliberative processes is promoted in both academic and policy circles as a potential means to build public trust in risk decisions and decision-makers. Governments in particular seem to optimistically take a positive relationship between public engagement and trust almost for granted. This paper provides a new and critical analysis of this hoped-for relationship, questioning whether such a direct and positive link between engagement and trust is a false hope. The paper draws upon personal experience of deliberative processes to discuss key components of an engagement process that have the potential to impact positively on trust. Specifically, who is engaged and which interests are represented; an open and collaborative framing of the discussion, and a direct and clear relationship between engagement and the risk decision. But the paper argues that given the complexities of optimising these process elements and in the light of the known underlying dimensions of trust, expectations are misplaced and that enduring trust is unlikely to spring from engagement itself. This is not to negate the other benefits of engagement, rather it is to focus on those key elements that will need to be in place, both process and beyond, if trust is to be enhanced. This paper explores the relations between different forms of knowledge and urban environmental policy arenas. It briefly considers the evolution of the urban environmental agenda and growing interest in questions of knowledge transfer. It then explores reasons for an apparent knowledge-policy ‘gap’, including familiar explanations, such as the problems of communicating research findings, as well as those based on more subtle and complex interpretations of both knowledge and policy processes. It concludes with some proposals for thinking about the boundary between knowledge and policy and constructive ways to enhance the sustainable urban environment agenda. The paper introduces the other contributions in this theme issue—concerned with diverse aspects of knowledge transfer in the context of urban environments—and draws upon insights from a seminar series at which these papers were first presented. The authors draw upon survey evidence of expert conceptualisations of the value of public knowledge in environmental decisionmaking. In the context of local air quality management in particular, they consider how experts understand the potential benefits of technological citizenship, and what status they accord to lay knowledge relative to their own roles. Evidence suggests a continuing expert-deficit model of lay knowledge, with suspicions that the public misunderstand environmental issues. Although the need for public ‘buy-in’ to the solutions to problems such as air pollution is supported, this does not translate to a more proactive engagement of lay knowledge in the assessment of such issues. Experts seem to be personally challenged by such notions. The authors discuss the need for a cultural shift in expert understanding of the value of lay knowledge, supported by a move away from an oversimplification of the need for, and value of, public participation. Assessing the social risks associated with climate change requires an understanding of how humans will respond because it affects how well societies will adapt. In the case of rapid or dangerous climate change, of particular interest is the potential for these responses to cross thresholds beyond which they become maladaptive. To explore the possibility of such thresholds, a series of climate change scenarios were presented to U.K. participants whose subjective responses were recorded via interviews and surveyed using Q methodology. The results indicate an initially adaptive response to climate warming followed by a shift to maladaptation as the magnitude of change increases. Beyond this threshold, trust in collective action and institutions was diminished, negatively impacting adaptive capacity. Climate cooling invoked a qualitatively different response, although this may be a product of individuals being primed for warming because it has dominated public discourse. The climate change scenarios used in this research are severe by climatological standards. In reality, the observed responses might occur at a lower rate of change. Whatever the case, analysis of subjectivity has revealed potential for maladaptive human responses, constituting a dangerous or rapid climate threshold within the social sphere. Environmental justice arguments focused on improving the quality of life of the poor contend that environmental ‘bads’ are more often located in areas of social disadvantage. In relation to waste facilities such claims of distributional outcomes have been enhanced by epidemiological analyses of apparent disease clusters in the vicinity. While politically engaging these arguments have been supported rarely by robust evidence. However, if repositioned in more structural issues related to the unequal societal distribution of power, resources and environmental burdens, they do prompt questions about the processes by which equitable decisions are made in a sustainable waste management context. The paper discusses the scientific and institutional barriers affecting the effective balancing of equity issues, arguing that while deliberative processes potentially challenge ‘expert black-boxing’ they also challenge current political and regulatory structures for waste management. Immunisation is the cornerstone of childhood disease prevention. In this context the combined measles, mumps and rubella vaccination (MMR) has proved a world-wide success, although in the UK it has been at the centre of public controversy since 1998. Through the media, the public domain has witnessed contestation among expert views about the relative risks associated with the diseases vs. the potential side-effects of the vaccination. Attainment of health protection targets has been compromised. The UK Department of Health sought to redress this through a major communication exercise. This paper reports the findings of a study of information strategies that parents use to make sense of health risk issues, particularly MMR. The findings identify the importance of social networks in reinforcing parental understanding and beliefs. While the media are identified as important sources of information, there is no evidence to suggest that parents passively receive and act upon such risk messages. Official information has been able to capitalise on the strong social normalisation of vaccination, but has not responded fully to the evolving social interpretation of risks. The study reveals a preference for personal and face-to-face engagement with health professionals, stressing the importance of user-centred health risk communication. [ABSTRACT FROM AUTHOR]

Despite increased support for extended public engagement in risk decision-making, significant questions remain over the best means to integrated deliberative processes with conventional 'scientific' or technical elements. This paper analyses the barriers to analytic-deliberative processes as a means by which the public can influence risk decisions, including the generation of data and the derivation of acceptable policy options. Using evidence from waste management decision processes in Britain, the discussion identifies technical, institutional and cultural barriers to effective process. The barriers are seen to limit systematic analysis appropriate to the problems as framed by the public. The principle that the nature of the risks and the assessment required needs to be determined through discussion with the public not in advance of discussion with them is challenged by proceduralization cultures within decision authorities and ingrained technical cultural perspectives. It is evident also that fundamental barriers lie in fragmentary decision processes and weak regulation. Public participation in decision making through the use of deliberative processes is now widely promoted as the means of enhancing institutional legitimacy, citizen influence and social responsibility and learning. Different methods are being tried but key questions remain about what works best and how practice could be improved. This paper discusses four examples of the application of community advisory committees and citizens' juries to waste strategy development by English local authorities. There is recognition that trust is a significant basis for effective risk communication. However, there is still a need to understand not only the relative importance of different trust dimensions but also their basis and the components which are affecting the persuasiveness of messages in different decision contexts. This paper includes the results of a small survey of people in interest groups in the United Kingdom (UK) in terms of their views of different waste management information sources. It identifies that the extent to which expectations of the performance of different institutions are supported by observation appears to have a significant impact on trust. This paper reports some of the findings of an ESRC Global Environmental Change Programme project which considered the attitudes of individuals (management and non-management) in small and medium-sized enterprises (SMEs) to the environment and environmental compliance. The focus on individuals and an understanding of the relationship between attitudes and behaviour within businesses is essential to implementation of effective sustainable development and self-regulation policies. The research revealed that the environment is important to individuals and that environmental compliance is regarded as ‘the right thing to do’. However, the influence of the regulatory domain on businesses is revealed as considerably more complex than suggested by some other surveys. In particular the positive culture amongst individuals in businesses to the importance of compliance appears to differ from the operational climate of many SMEs; i.e. their capacity and feasibility to act. Decision-making strategies which favour the top-down model do not recognize expertise as a communication and learning process, and have been seen to fail in many risk management contexts, in particular in local waste management decision-making. Examination of a novel public involvement programme in the development of a local waste strategy provides an opportunity to understand expertise as a process: in particular, (i) how expert knowledge is selected at the technical-democratic interface, (ii) how information is shaped and balanced, and (iii) whether knowledge shifts during processes of exposure to expertise. It provides evidence that counters expert views that the public are irrational, lack interest, and are concerned only about zero-risk options. Most importantly, it provides evidence that expertise is inextricably linked to its source, and that perceptions that expertise is not independent have a significant impact on public responses. Searches were conducted using PubMed, Scopus, CINAL, ASSIA and iCite databases. Eleven thousand five hundred records were identified and 13 were included in the final sample. Included studies suggest that NPI adoption or adherence was generally high, and communication had significant impacts, with key themes including clarity and consistency, trust and control. Based on the evidence in this review, features of effective communication in the context of NPI adoption or adherence are (i) information should be conveyed clearly and conflicting (mixed) messages should be avoided; (ii) information should be conveyed by trusted sources (e.g. health authorities) and (iii) communication should strike a balance between being authoritative but avoiding language seen as controlling (e.g. 'you must'). Future research should prioritize quantitative, experimental and longitudinal study designs, that focus specifically on communication as an intervention, and which measure behaviour. Communicating the rationale for allocating resources to manage policy priorities and their risks is challenging. Here, we demonstrate that environmental risks have diverse attributes and locales in their effects that may drive disproportionate responses among citizens. When 2,065 survey participants deployed summary information and their own understanding to assess 12 policy-level environmental risks singularly, their assessment differed from a prior expert assessment. However, participants provided rankings similar to those of experts when these same 12 risks were considered as a group, allowing comparison between the different risks. Following this, when individuals were shown the prior expert assessment of this portfolio, they expressed a moderate level of confidence with the combined expert analysis. Public policy requires public support, which in turn implies a need to enable the public not just to understand policy but also to be engaged in its development. Where complex science and technology issues are involved in policy making, this takes time, so it is important to identify emerging issues of this type and prepare engagement plans. In our horizon scanning exercise, we used a modified Delphi technique. A wide group of people with interests in the science and policy interface (drawn from policy makers, policy adviser, practitioners, the private sector and academics) elicited a long list of emergent policy issues in which science and technology would feature strongly and which would also necessitate public engagement as policies are developed. This was then refined to a short list of top priorities for policy makers. Thirty issues were identified within broad areas of business and technology; energy and environment; government, politics and education; health, healthcare, population and aging; information, communication, infrastructure and transport; and public safety and national security.

The need for policy makers to understand science and for scientists to understand policy processes is widely recognised. However, the science-policy relationship is sometimes difficult and occasionally dysfunctional; it is also increasingly visible, because it must deal with contentious issues, or itself becomes a matter of public controversy, or both. We suggest that identifying key unanswered questions on the relationship between science and policy will catalyse and focus research in this field. To identify these questions, a collaborative procedure was employed with 52 participants selected to cover a wide range of experience in both science and policy, including people from government, non-governmental organisations, academia and industry. These participants consulted with colleagues and submitted 239 questions. An initial round of voting was followed by a workshop in which 40 of the most important questions were identified by further discussion and voting. The resulting list includes questions about the effectiveness of science-based decision-making structures; the nature and legitimacy of expertise; the consequences of changes such as increasing transparency; choices among different sources of evidence; the implications of new means of characterising and representing uncertainties; and ways in which policy and political processes affect what counts as authoritative evidence. We expect this exercise to identify important theoretical questions and to help improve the mutual understanding and effectiveness of those working at the interface of science and policy.
