A case of a mixed infection due to Candida albicans and the zygomycete Absidia corymbifera in a 38-year-old, previously healthy, Caucasian male is presented. The infection developed following serial rib fractures, and ruptures of kidney, liver and biliary tract as well as a pancreatic contusion resulting from a traffic accident. During intensive care treatment the patient he underwent several surgical procedures but subsequently experienced multi-organ failure and sepsis. Some weeks later, fungal growth was observed macroscopically on the patient's skin and wounds. From wound swabs C. albicans and A. corymbifera were grown. Histopathology of abdominal tissue yielded pseudohyphae and coenocytic hyphae. Although surgical debridement and antifungal treatment with amphotericin B and 5-flucytosine were started immediately, the patient died in therapy-refractory septic multi-organ failure. Die Aktivierung der Blutgerinnung tritt in unterschiedlichem Maß bei invasiven einheimischen Mykosen auf. Während es bei Candidosen und Aspergillosen nach Einbruch der Pilze in die Blutbahn gelegentlich zur Bildung von Thromben kommt, die zu septischen Embolien Anlass geben können, werden bei Mucormykosen (Syn. Zygomykosen) regelmäßig ausgedehnte thrombotische Verschlüsse der befallenen Gefäße und folglich Nekrosen des abhängigen Gewebes beobachtet. Für die Auslösung der Gerinnung durch Candida und Aspergillen können sekretorische Aspartat-Proteinasen der Pilze verantwortlich sein, die den Gerinnungsfaktor X aktivieren, wie bereits früher gezeigt wurde [1, 2]. Für die gravierenden Gerinnungsereignisse bei Mucormykosen ist hingegen offenbar eine extrazelluläre subtilisin-ähnliche Serinproteinase (Arp) verantwortlich, die bereits in geringen Konzentrationen bei Raumtemperatur eine Parakoagulation von Fibrinogen auslöst, und die zugleich den wichtigsten Inhibitor der Gerinnung, das Antithrombin III, zerstört. Neue Befunde zur Wirkung von Arp werden diskutiert. Activation of blood coagulation to a varying extent affect the course of domestic invasive mycoses. Upon invasion of blood vessels by Candida or aspergilli, occasionally thrombi are formed, which may cause septic embolism. In the course of mucormycosis (syn. zygomycosis) thrombotic occlusion of afflicted blood vessels and subsequent necrosis of dependent tissue regularly occurs. Coagulation during candidosis or aspergillosis may be triggered by secreted aspartic proteinases which are able to activate factor X as has been shown previously [1, 2]. During mucormycosis, severe blood coagulation apparently is due to paracoagulation of fibrinogen which is triggered by low concentrations of extracellular fungal subtilisin-like proteinase (Arp). The enzyme is also able to inactivate the major inhibitor of blood coagulation (antithrombin III). Recent findings on the action of Arp are discussed. [ABSTRACT FROM AUTHOR]

Using a reproducible model of Candida albicans keratitis in rabbits we studied the effect of topical fluconazole, a new triazole. Candida albicans DSM 70010 (2.5 X 10(5) cells) was injected into the corneal stroma of both eyes of 21 rabbits. All eyes developed a corneal ulcer. Forty-eight hours after inoculation the animals were divided into three groups: (1) 14 eyes, received fluconazole (2 mg/ml) and the epithelium subsequently removed; (2) 14 eyes, received only fluconazole drops; (3) 14 eyes, received 0.9% NaCl: half of this group was also debrided. We applied one drop of either substance 10 times a day for 24 days. A further six rabbits were used to judge if the drug penetrated into the cornea and aqueous humour. There was a highly significant difference between the fluconazole groups (1,2) and the control group (3) as to hypopyon and complications (descemetocele, corneal perforation) as well as recultivation of C. albicans from corneal tissue. The difference between the fluconazole groups with and without debridement was not significant. The drug penetrated into the cornea and aqueous humour of both uninflamed and inflamed eyes.
