Abstract

Research about intraoperative images for surgical analysis is currently popular. It is expected that the analysis will become more accurate by adding the usage status of surgical energy devices. This paper proposes a new method for the state recognition of an energy device. The measurement system will be integrated into the OPeLiNK system of the Smart Cyber Operating Theatre, SCOT. The developed system uses image recognition in the form of template matching to estimate the state of the device. A new approach based on predetermined locations with respect to a reference image is proposed for indicator localization. For light indicator recognition, a new method based on the luminance level with respect to template images with threshold estimation is proposed. For character recognition, an existing Normalized Cross Correlation method is applied. The system has been tested on the Conmed System 2450. 2963 videos of nine colon surgeries performed at Japan's National Cancer Center were evaluated. The results show that the system has a 98.2% accuracy in giving information on the use of energy devices during surgery and outperforms the state-of-the-art in measuring more significant data. Abstract

Background: This study was aimed to compare the efficacy of two combination tablets of dipeptidyl peptidase-4 (DPP-4) inhibitors and metformin with different dosages, alogliptin/metformin (AM) and vildagliptin/metformin (VM), on glycemic control in patients with type 2 diabetes (T2D). Methods: This was a prospective, multicenter, open-label, randomized, parallel group, comparative trial. After a run-in period of treatment with metformin alone, a total of 59 Japanese outpatients with T2D, aged 20–79 years with glycated hemoglobin (HbA1c) levels of 6.5%–10% were randomly assigned to 12-week AM treatment, alogliptin 25 mg/metformin 500 mg combination tablet orally once a day, or VM treatment, vildagliptin 50 mg/metformin 250 mg combination tablet orally twice a day. The primary endpoints were the changes in HbA1c and fasting plasma glucose (FPG) levels from baseline to week 12 between the two groups. Blinded intermittently scanned continuous glucose monitoring (isCGM) was performed between weeks 10 and 12. The incidence of adverse events during the study was also evaluated. Results: In all, 52 participants were analyzed. Significant decreases in HbA1c and FPG levels from baseline to week 12 were observed in both treatment groups. However, there were no significant differences between the AM and VM groups in the change in HbA1c level (–0.3% and –0.4%, P = 0.309) or the FPG level (–9.0 and –15.0 mg/dL, P = 0.789). The isCGM revealed that both treatments achieved the recommended glycemic target range. No adverse events, such as severe hypoglycemia, were observed in either group. Conclusions: We concluded that there were no significant differences in the efficacy of two combination tablets of DPP-4 inhibitors and metformin with different dosages on glycemic control in patients with T2D. Abstract

Featured Application: This work allows for ultrasound imaging with wearable probes by roughly attaching them to the skin, facilitating the monitoring of the body internal tissues in daily life. Skin-attachable ultrasound probes face challenges in imaging the intended cross-section due to the difficulty in precisely adjusting the position and angle of attachment. While matrix element arrays are capable of imaging any cross-section within a three-dimensional field of view, their implementation presents a challenge due to the significant number of required ultrasound elements. We propose a method for optimizing the coordinates and shapes of elements based on the focusing quality onto the imaging points under the positional and angular errors in the element array. A 128-element array was optimized through the proposed method and its imaging performance was evaluated with simulated phantoms. The optimized array demonstrated the ability to clearly visualize the simulated wires, cysts, and blood vessels even with the positional error of 3 mm and the angular error of 20°. These results indicate the feasibility of developing a skin-attachable ultrasound probe that can be easily used in daily life without requiring precise positional and angular accuracy. Abstract

Background: Predicting the origin of premature ventricular contraction (PVC) from the preoperative electrocardiogram (ECG) is important for catheter ablation therapies. We propose an explainable method that localizes PVC origin based on the semantic segmentation result of a 12‐lead ECG using a deep neural network, considering suitable diagnosis support for clinical application. Methods: The deep learning‐based semantic segmentation model was trained using 265 12‐lead ECG recordings from 84 patients with frequent PVCs. The model classified each ECG sampling time into four categories: background (BG), sinus rhythm (SR), PVC originating from the left ventricular outflow tract (PVC‐L), and PVC originating from the right ventricular outflow tract (PVC‐R). Based on the ECG segmentation results, a rule‐based algorithm classified ECG recordings into three categories: PVC‐L, PVC‐R, as well as Neutral, which is a group for the recordings requiring the physician's careful assessment before separating them into PVC‐L and PVC‐R. The proposed method was evaluated with a public dataset which was used in previous research. Results: The evaluation of the proposed method achieved neutral rate, accuracy, sensitivity, specificity, F1‐score, and area under the curve of 0.098, 0.932, 0.963, 0.882, 0.945, and 0.852 on a private dataset, and 0.284, 0.916, 0.912, 0.930, 0.943, and 0.848 on a public dataset, respectively. These quantitative results indicated that the proposed method outperformed almost all previous studies, although a significant number of recordings resulted in requiring the physician's assessment. Conclusions: The feasibility of explainable localization of premature ventricular contraction was demonstrated using deep learning‐based semantic segmentation of 12‐lead ECG.
