Ladera, a postwar interracial housing cooperative, provides a compelling look at 1940s housing policies, exposing cultural norms of race in state lending structures. In contrast with most midcentury suburban developments, Ladera was architecturally progressive in design and egalitarian in scope, open to all regardless of race, class, or creed. The research examines the innovative planning and fiscal features of the community, followed by an explication of the 1940s lending landscape and state-sponsored financing process that ultimately reshaped Ladera's development. As a necessary corrective to established narratives of California's housing policies, this study reveals the influential intersection of racial and class dynamics prior to Palo Alto's eventual transformation into the nexus of Silicon Valley, exposing the critical preconditions that produced the sprawling, segregated technopolis of today. The cooperative's history is analyzed through archival materials, including the cooperative's records, personal journals, architectural drawings, and the FHA's internal memos and correspondence. A suite of diagnostics used to assess impurity content and dynamics has been updated, upgraded, and installed on the Pegasus-III Experiment. Typical plasma parameters during local helicity injection start-up are τshot ∼ 10 ms, ne ∼ 1 × 1019 m−3, and Te ∼ 50 eV. The deployed diagnostics are compatible with this modest temperature and density regime and provide species identification, source localization, and estimation of radiation losses. Impurity species are determined by recording time-evolving, single line-of-sight spectra at 1.25 kfps using a SPRED (Survey, Poor Resolution, Extended Domain) vacuum ultraviolet spectrometer. SPRED is equipped with 450 g/mm grating, giving a spectral resolution of 0.33 nm and a spectral range from ∼10 to 110 nm, useful to identify light impurity species in this temperature and density range. An absolutely calibrated spectrometer that collects light from the plasma at Rtan = 15.9 cm and Δt ≥ 2 ms is used as a visible survey spectrometer and for continuum measurements. The radiated power from the plasma is estimated with a photodiode-based diagnostic. Two 16-channel absolute extreme ultraviolet diode arrays are placed behind pinhole apertures, resulting in 32 lines of sight at Z = 0, with a spatial resolution of 2–3 cm and a time response of 60 kHz. A photometrically calibrated collinear Dα/near infrared filtered photodiode-based system measures the Dα emission and around 1040 nm. Abstract Background/Objectives Ataxia–telangiectasia (A‐T) is a complex inherited disease associated with an increased risk of malignancy. Surveillance guidelines have demonstrated significant health benefits in other cancer predisposition syndromes. However, evidence‐based guidelines for cancer screening are not currently used in the United Kingdom for people affected by A‐T. This study aims to understand how people with A‐T and their parents feel about cancer surveillance using whole‐body magnetic resonance imaging (MRI) to inform the future development of cancer surveillance guidelines. Design/Methods We conducted semistructured interviews with people affected by A‐T. Data were analysed inductively using thematic analysis. Results Nine parents of children with A‐T and four adults with A‐T were interviewed. Five main themes emerged from the data, including (1) cancer screening was considered invaluable with the perceived value of early detection highlighted; (2) the cancer fear can increase anxiety; (3) the perceived limitations around current practice, with the responsibility for monitoring falling too strongly on parents and patients; (4) the need for effective preparation for cancer screening, including clear communication and (5) the challenges associated with MRI screening, where specific recommendations were made for improving the child's experience. Conclusion This study suggests that stakeholders are positive about the perceived advantages of a cancer screening programme. Ongoing support and preparation techniques should be adopted to maximise adherence and minimise adverse psychosocial outcomes. Abstract Background Diagnosing multiple sclerosis (MS) can be a lengthy process, which can negatively affect psychological well‐being, condition management, and future engagement with health services. Therefore, providing timely and appropriate emotional support may improve adjustment and health outcomes. Purpose To develop a patient care pathway for providing emotional support around the point of diagnosing MS, and to explore potential barriers and facilitators to delivery and implementation. Methods Focus groups were conducted with 26 stakeholders, including 16 people living with MS, 5 carers/family members and 5 professionals working with people living with MS (3 MS nurses, 1 psychiatrist, and 1 charity staff member). Discussions were audio‐recorded, transcribed verbatim and analyzed using framework analysis. Results Participants suggested that a patient care pathway should include comprehensive information provision as a part of emotional support at diagnosis, and follow‐up sessions with a healthcare professional. Barriers including increasing staff workloads and financial costs to health services were acknowledged, thus participants suggested including peer support workers to deliver additional emotional support. All participants agreed that elements of a care pathway and embedded interventions should be individually tailored, yet provided within a standardized system to ensure accessibility. Conclusions A patient care pathway was developed with stakeholders, which included an embedded MS Nurse support intervention supplemented with peer support sessions. Participants suggested that the pathway should be delivered within a standardized system to ensure equity of service provision across the country. Patient or Public Contribution This research was conceptualized and designed collaboratively with Nottingham Multiple Sclerosis Patient and Public Involvement and Engagement (PPIE) group members. One member is a co‐author and was actively involved in every key stage of the research process, including co‐design of the pathway and research protocol, data collection (including presenting to participants and moderating group discussions), analysis and write‐up. Authors consulted with PPIE members at two meetings (9 and 11 PPIE attendees per meeting) where they gave feedback on the research design, findings and the resulting pathway. People living with MS and carers of people with MS were included in the focus groups as participants.

Anthropogenic-source habitat fragmentation leads to increased habitat edge in the environment, with potential negative consequences for wildlife. We examine the influence of increased edge on a ground nesting bird of conservation concern, the hen harrier Circus cyaneus. Using eight years of data collected in the Slieve Bloom Mountains Special Protection Area, central Ireland, and an average breeding population of ten pairs, we investigate how habitat fragmentation and edge influences hen harrier nest site selection, breeding success and productivity. We also used a deterministic population matrix model to assess population breeding trends and to simulate population growth rate responses to increased habitat edge by varying demographic parameters such as productivity and juvenile survival. Our results show that habitat edge had a significant effect on nest site selection, breeding success and productivity. Hen harriers were more likely to nest in areas of high edge/area ratio, but this was associated with lower breeding success and productivity, suggesting a possible ecological trap. This mismatch between nest site selection and breeding output may be linked to this species' high reproductive site fidelity. Our population matrix indicated a population increase, whereas population monitoring indicates population stability. Our simulations suggest that increased edge will have a negative effect on population growth rate, providing a greater understanding of the relationship between hen harrier population trends and changing habitat configuration. These results highlight the importance of contiguous habitats and the need for appropriate land use management in protected upland areas for breeding raptors. Minimising habitat fragmentation and forest edge to create larger blocks of uniform peatland habitat should be an integral part of the conservation management of hen harrier breeding areas both in Ireland and in similar breeding habitats such as those in Britain. We have characterized kilo-electron-volt (keV) emission from Al plasmas for various laser illuminations at the OMEGA laser with the goal of optimizing the ability to backlight low-atomic-number materials such as beryllium for fusion ignition studies. The plasma is diagnosed by spectral-measurement comparisons to detailed theoretical atomic physics models. It is found that a significant fraction of the radiation is due to the x-ray continuum, that the electron temperature Te depends weakly on laser energy and power, and that the conversion efficiency to Lyman α (Lα, the N=2→1 transition in H-like Al) at 1.73 keV is reduced as laser energy is increased. As the number of beams is increased, the extra laser energy goes into a larger, higher-density plasma in which He-like and H-like ions are more effectively ionized. [ABSTRACT FROM AUTHOR]

Plastic pollution and climate change are serious and interconnected threats to public and planetary health, as well as major drivers of global social injustice. Prolific use of plastics in the construction industry is likely a key contributor, resulting in burgeoning efforts to promote the recycling or downcycling of used plastics. Businesses, materials scientists, institutions, and other interested stakeholders are currently exploring the incorporation of plastic waste into building materials and infrastructure at an accelerated rate. Examples include composite asphalt-plastic roads, plastic adhesives, plastic-concrete, plastic/crumb rubber turf, plastic lumber, plastic acoustic/thermal insulation, plastic-fiber rammed earth, and plastic soil reinforcement/stabilizers. While some believe this to be a reasonable end-of-life scenario for plastic waste, research shows such efforts may cause further problems. These uses of plastic waste represent an ongoing effort at “greenwashing,” which both delays and distracts from finding real solutions to the plastic pollution crisis. Hypothesized effects of incorporating plastic waste in construction materials, including economic, environmental, human health, performance, and social impacts, are evaluated in this mini review. We compare known impacts of these treatments for plastic waste and provide recommendations for future research. Evidence shows that such practices exacerbate the negative ecological, health, and social impacts of plastic waste and increase demand for continued production of new (virgin) plastics by creating new markets for plastic wastes. We urge caution—and more research—before widely adopting these practices.

In August 2015, Google reorganized its various interests as a conglomerate called Alphabet Inc. Under the new umbrella, Google's search, data aggregation, and advertising subsidiaries, were joined by Sidewalk Lab and its suite of urban products: high-speed broadband services, Android Pixel2 phone, mobile mapping, autonomous cars, artificial intelligence, smart homes, and all the data captured therein. The City of Toronto's recent award to Alphabet's Sidewalk Lab for design services has sparked a heated controversy among urban planners and citizens alike. Toronto's decision not only signals a different model of professional practice, but it also represents a conceptual shift away from citizen to urban consumer. By engaging a private technology company, one that passively captures data on its customers and then re-sales that data to third parties, Toronto's smart city points to a significant change in the understanding and practice of contemporary urban planning and design. Acknowledging the city as a site of disciplinary disruption, this paper introduces Bratton's stack theory as a way to understand networked urbanism more generally, and Waterfront Toronto specifically. We build on Bratton's position by closely examining twenty-first century histories and anthropologies related to the Internet, privacy, and the dominance of big data. Our principal concern is with the transformation of personal and environmental data into an economic resource. Seen through that particular lens, we argue that Toronto's smart city has internalized relations of colonization, whereby the economic objectives of a multinational technology company take on new configurations at a local level of human (and non-human) information extraction - thereby restructuring not only public land, but also everyday life into a zone of unmitigated consumption. This article has been written for the 40th year of the publication of Medical Teacher. While we celebrate the contribution of simulated participants (SPs) to health professions education through values and value-based learning, we also offer critical reflection on elements of our practice, commencing with language. We argue for the use of the term simulated rather than standardized and acknowledge the dominant role of the SP as patient and the origins of the methodology. These shifts in terms and their implications in practice reflect changes in the conceptualization of SP-based methodology. Recently published standards for those who work with SPs (SP practitioners) are noted as an important milestone in our community’s development. We consider contemporary practices addressing the complex notions of values and value in SP-based learning. We simultaneously refer to the work of SPs and SP practitioners. Phases of educational design including identifying learning objectives, scenario design, implementation, feedback and debriefing are used to illustrate methodological shifts. Within each of these phases, there are relational issues that have to date often gone unchecked and are under reported in literature. Finally, using the metaphor of a murmuration, we celebrate contemporary practices of the global SP practitioner community. [ABSTRACT FROM AUTHOR]

Experiments performed at the Sandia National Laboratories (SNL) Z-machine, located in Albuquerque, New Mexico produce hot (∼220 eV) plasmas. X-ray emission from the plasma is used to drive radiation flow experiments. Our standard plasma diagnostic suite consists of x-ray diodes (XRDs), silicon photodiodes, and nickel thin film bolometers. Small diagnostic holes allow us to view the hot plasma from the side, top axial anode side, and bottom axial cathode side. Computer software has been written to process the raw data to calculate data quality, fold in detector spectral response and experiment geometry for emitted flux, calculate a multidetector spectral unfold, and yield an equivalent time-dependent Planckian temperature profile. Spectral unfolds of our XRD data generally yield a Planckian-like spectrum. In our presentation we will compare our diagnostic techniques, analysis, and results to more accurately characterize spectral unfolds in order to establish better drive conditions for our experiments. A multiple-laboratory campaign is underway to qualify beryllium as a fusion capsule ablator for the National Ignition Facility [Moses and Wuest, Fusion Sci. Technol. 43, 420 (2003)]. Although beryllium has many advantages over other ablator materials, individual crystals of beryllium have anisotropic properties, e.g., sound speed, elastic constants, and thermal expansion coefficients, which may seed hydrodynamic instabilities during the implosion phase of ignition experiments. Experiments based on modeling have begun at the OMEGA laser [Boehly, McCrory, Verdon et al., Fusion Eng. Design 44, 35 (1999)] to create a test bed for measuring instability growth rates with face-on radiography of perturbed beryllium samples with the goal of establishing a specification for microstructure in beryllium used as an ablator. The specification would include the size and distribution of sizes of grains and voids and the impurity content. The experimental platform is a 4 kJ laser-heated (for ∼6 ns) hohlraum that is well modeled for radiation temperature and for shock pressure and breakout timing through the driven beryllium sample. A 1 atm methane gas fill has been used to maintain a clear line of sight through the hohlraum for radiography with acceptable plasma backscatter losses. The peak radiation temperature is 145 eV; the pressure early in the laser pulse is 1 Mbar for over 1 ns. Radiographs of sinusoidally perturbed copper-doped (0.9% by atom) beryllium samples have been obtained more than 10 ns after drive initiation. With the current laser drive, a growth factor approaching ten has been measured for initial 2.5 μm perturbations with on-axis radiography. [ABSTRACT FROM AUTHOR]

The Dynamic Hohlraum (DH) radiation on the Z facility at Sandia National Laboratories [R. B. Spielman, W. A. Stygar, J. F. Seamen et al., Proceeding of the 11th International Pulsed Power Conference, Baltimore, 1997, edited by G. Cooperstein and I. Vitkovitsky (IEEE, Piscataway, NJ, 1997), Vol. 1, p. 709] is a bright source of radiant energy that has proven useful for high energy density physics experiments. But the radiation output from a DH on Z needs to be well known. In this paper, a new method is presented for measuring the radiation fluence deposited in an experiment, specifically, an experiment driven by a Z DH. This technique uses a blast wave produced in a SiO2 foam, which starts as supersonic but transitions to subsonic, producing a shock at the transition point that is observable via radiography. The position of this shock is a sensitive measure of the radiation drive energy from the Z DH. Computer simulations have been used to design and analyze a Z foam blast wave experiment. Collective Thomson scattering is widely used to measure bulk plasma parameters in high density, laser-produced plasmas, and is used to detect plasma waves from instabilities. However, inhomogeneity in these small plasmas often leads to a spectrum with insufficient resolution to discern phenomena such as wave damping and nonlinear wave effects. Two techniques are discussed for laser-produced plasmas to overcome these limitations, and provide details of wave damping and nonlinear behavior. First, imaging Thomson scattering is used to obtain spatially resolved plasma wave profiles in a 100–200 eV plasma, and allows us to infer ion-ion collisional damping rates. Second, a diffraction-limited laser beam is used to drive stimulated Raman scattering in a hot plasma, generating large amplitude Langmuir waves. The comparatively small interaction volume permits sufficient spectral resolution to observe nonlinear wave behavior previously unresolved in other experiments. [ABSTRACT FROM AUTHOR]

The photoemissive cathode type of x-ray diode (XRD) is popular for measuring time and spectrally resolved output of pulsed power experiments. Vitreous carbon XRDs currently used on the Sandia National Laboratories Z-machine were designed in the early 1980s and use materials and processes no longer available. Additionally cathodes used in the high x-ray flux and dirty vacuum environment of a machine such as Z suffer from response changes requiring recalibration. In searching for a suitable replacement cathode, we discovered very high purity vitreous-carbon planchets are commercially available for use as biological substrates in scanning electron microscope (SEM) work. After simplifying the photocathode mounting to use commercially available components, we constructed a set of 20 XRDs using SEM planchets that were then calibrated at the National Synchrotron Light Source at Brookhaven National Laboratory. We present comparisons of the reproducibility and absolute calibrations between the current vitreous-carbon XRDs and our new design. [ABSTRACT FROM AUTHOR]

Abstract Background The literature on simulated or standardized patient (SP) methodology is expanding. However, at the level of the program, there are several gaps in the literature. We seek to fill this gap through documenting experiences from four programs in Australia, Canada, Switzerland and the United Kingdom. We focused on challenges in SP methodology, faculty, organisational structure and quality assurance. Methods We used a multiple case study method with cross-case synthesis. Over eighteen months during a series of informal and formal interactions (focused meetings and conference presentations) we documented key characteristics of programs and drew on secondary document sources. Results Although programs shared challenges in SP methodology they also experienced differences. Key challenges common to programs included systematic quality assurance and the opportunity for research. There were differences in the terminology used to describe SPs, in their recruitment and training. Other differences reflected local conditions and demands in organisational structure, funding relationships with the host institution and national trends, especially in assessments. Conclusion This international case study reveals similarities and differences in SP methodology. Programs were highly contextualised and have emerged in response to local, institutional, profession/discipline and national conditions. Broader trends in healthcare education have also influenced development. Each of the programs experienced challenges in the same themes but the nature of the challenges often varied widely.

Abstract Background Role-play is widely used as an educational method for learning about communication in medical education. Although educational theory provides a sound rationale for using this form of simulation, there is little published evidence for its effectiveness. Students' prior experiences of role-play may influence the way in which they engage in this method. This paper explores students' experiences with the aim of producing guidelines for maximising the benefits of role-play within this learning context. Methods First-year undergraduate medical students participated in a role-play session as part of their communication programme. Before and after the session, students completed questionnaires. In the pre-session questionnaire, students were asked about their experiences of role-play and asked to identify helpful and unhelpful elements. Immediately after the session, students answered similar questions in relation to the role-play activity they had just completed. Descriptive statistics were used to analyse quantitative data and qualitative data was thematically analysed. Results 284 students completed evaluation forms. Although 63 (22.2%) had prior unhelpful experiences, most students (n = 274; 96.5%) found this experience helpful. Summary findings were that students reported the key aspects of helpful role-play were opportunities for observation, rehearsal and discussion, realistic roles and alignment of roles with other aspects of the curriculum. Unhelpful aspects were those that evoked strong negative emotional responses and factors that contributed to a lack of realism. Conclusion Role-play was valued by students in the acquisition of communication skills even though some had prior unhelpful experiences. Guidelines for effective role-play include adequate preparation, alignment of roles and tasks with level of practice, structured feedback guidelines and acknowledgment of the importance of social interactions for learning.

Communities of practice (CoPs) are social systems consisting of individuals who come together to share knowledge and solve problems around a common interest. For educators, membership in a CoP can facilitate access to expertise and professional development activities and generate new collaborations. This teaching tip focuses on online CoPs and provides tips for setting up and running such communities. The initial planning phase involves establishing the purpose of the CoP, recruiting an administrative team, designing the structure of the online environment, and choosing a platform. Once the online platform is launched, running the CoP involves building the membership, encouraging engagement (primarily in discussion forums), finding ways to create and share useful resources, and sustaining the community as an active and effective CoP. We also describe a specific example of an online CoP for veterinary educators involved in clinical skills teaching. The membership has grown to represent an international community who engage in a range of activities including sharing knowledge, tips and ideas, asking questions, discussing challenges, and promoting collaborative activities.

Combined therapies with Heat Shock Protein 90 (HSP90) inhibitors and Heat Shock Protein 70 (HSP70) inducers are gaining significant interest in cancer and cardiovascular research. Here, we tested the hypothesis that HSP90 inhibitors and HSP70 inducers, together, can block the development of pulmonary fibrosis. We exposed New Zealand White Rabbits to hydrochloric acid (HCl, 0.1 N, 1.5 mL/kg), one of the top five chemicals most commonly involved in accidental exposures and inhalation injuries worldwide, and treated animals with either the orally available HSP90 inhibitor TAS-116 (1.7 mg/kg 5x/week) or TAS-116 combined with the HSP70 inducer, geranylgeranyl acetone (GGA, 50 mg/kg, 3x/week). At 60 days post-HCl instillation, TAS and GGA treatment markedly reduced the degree of pulmonary fibrosis, lung dysfunction, and activation of profibrotic pathways. The use of HSP70 inducers may be a helpful tool to improve the profile of HSP90 inhibitors and reduce their minimal effective dose and side effects. Further investigation is required to explore the exact synergistic mechanism behind the antifibrotic profile of HSP90 inhibitors and HSP70 inducers.

Patients with chronic lymphocytic leukemia (CLL) respond well to initial treatment with the B-cell lymphoma 2 (BCL2) inhibitor venetoclax. Upon relapse, they often retain sensitivity to BCL2 targeting, but durability of response remains a concern. We hypothesize that targeting both BCL2 and B-cell lymphoma-extra large (BCLXL) will be a successful strategy to treat CLL, including for patients who relapse on venetoclax. To test this hypothesis, we conducted a pre-clinical investigation of LP-118, a highly potent inhibitor of BCL2 with moderate BCLXL inhibition to minimize platelet toxicity. This study demonstrated that LP-118 induces efficient BAK activation, cytochrome C release, and apoptosis in both venetoclax-naïve and -resistant CLL cells. Significantly, LP-118 is effective in cell lines expressing the BCL2 G101V mutation and in cells expressing BCLXL but lacking BCL2 dependence. Using an immunocompetent mouse model, Eμ-TCL1, LP-118 demonstrates low platelet toxicity, which hampered earlier BCLXL inhibitors. Finally, LP-118 in the RS4;11 and OSU-CLL xenograft models results in decreases in tumor burden and survival advantage, respectively. These results provide a mechanistic rationale for the evaluation of LP-118 for the treatment of venetoclax-responsive and -relapsed CLL.

Objective: The purpose of this randomized controlled trial was to test the effects of an online, coached mindfulness intervention on momentary negative affect (mNA) for youth with high levels of trait negative affectivity., Method: Participants were 111 youth ages 12 to 17 years old ( M = 14.17, SD = 1.60). Youth self-identified as 68% female, 29% male, and 4.5% gender diverse; 54.55% identified as White; 31.82 reported being Hispanic/Latinx. Participants were selected for having high levels of trait negative affect and were randomized to receive either the mindfulness program or no intervention. We used ecological momentary assessment to measure stress and emotions and to derive measures of mNA comprised of stressor-independent and stressor-reactive negative affect. The ecological momentary assessment protocol involved participants completing a short survey/diary entry on Qualtrics four times per day for 5 days. Internalizing symptoms were assessed with the Patient Health Questionnaire-8, Generalized Anxiety Disorder-7, and the Screen for Child Anxiety Related Disorders., Results: The mindfulness intervention resulted in a significant reduction in stressor-reactive negative affect ( t = 2.001, df = 96, p = .048; Cohen's d = .40), but not stressor-independent mNA or overall mNA. Additionally, reductions in stressor-reactive negative affect significantly correlated with changes in internalizing symptomatology (standardized β = .26, p = .032)., Conclusions: These results indicate that among youth with high levels of trait negative affectivity, a relatively affordable and accessible digital mindfulness program significantly reduced stressor-reactive negative affect. The absence of an effect on stressor-independent or overall average mNA suggests some specificity of the effects of mindfulness to stressor-reactive negative affect in an at-risk sample of adolescents. Background: Behavioral medicine has made key contributions toward improving health outcomes. Engaging community partners in research is critical to addressing persistent health inequities. The aim of this scoping review was to explore how researchers engaged community partners within the field of behavioral medicine research from 2005 to 2023., Method: Publication databases and gray literature were searched for research that engaged community partners to address questions relevant to behavioral medicine. Articles were screened by title and abstract, and then by full text. Articles meeting the inclusion criteria were coded using the framework provided by the Engagement Navigator to identify engagement approaches, methods, and tools and when they were used during the research., Results: Of 1486 articles initially identified, 58 met the inclusion criteria. Most articles used well-known approaches (e.g., community-based participatory research; 67%), methods (e.g., advisory committees; 59%), and tools (e.g., interviews; 41%), and engaged with healthcare service providers (62%) and/or patients (53%). Community partners were most often included in research planning and design (79%), and less often in dissemination (45%)., Conclusion: Community engagement has considerable potential to address health inequities. Our assessment of the approaches, methods, and tools used by behavioral medicine researchers to engage with a diverse range of community partners points toward promising strategies for enhancing the impact of community engagement. Adolescents and young adults (AYA) with increased risk for cancer due to hereditary predisposition, previous cancer treatment, or both are eligible for increased surveillance, chemoprevention, and prophylactic surgery that can improve early detection and prevention of cancers. One way to ensure continuity of cancer prevention care is to support adolescents through the transition from pediatric to adult health care. Yet, there are limited data on the impl ementation of health care transition (HCT) programs for AYA with increased risk for cancer. We conducted a scoping review of the literature on transition programs for AYA at increased risk of cancer due to known germline risk or prior cancer diagnosis, with a focus on implementation factors relevant to designing, implementing, and sustaining a new program. Data from 54 articles were extracted and analyzed using the RE-AIM implementation science framework. Few HCT programs have been implemented for AYA with hereditary cancer syndromes. Several groups have done preimplementation work for future hereditary cancer programs, but programs for cancer survivors are farther along the translational spectrum. Acute respiratory distress syndrome (ARDS), often preceded by acute lung injury (ALI), is characterized by the accumulation of inflammatory fluid in the lung alveoli, leaky alveolar epithelium and endothelium, and overexpression of proinflammatory cytokines. This progression from ALI to ARDS is a major contributor to the high mortality observed in patients with COVID-19. The spike protein of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) binds to lung angiotensin-converting enzyme 2 (ACE2), and in addition to facilitating viral cell entry, it plays an important role in the development of ALI and ARDS, especially in the later phases of COVID-19 as well as long-COVID. Protein tyrosine phosphatase (PTP) 4A3 is a key mediator of ARDS pathology. This study tested the hypothesis that targeting PTP4A3 would prevent COVID-19-associated ALI. Intratracheal administration of SARS-CoV-2 spike protein subunit 1 to K18-hACE2 transgenic mice expressing human ACE2 elicited pulmonary and systemic inflammation, leaky alveoli, overexpression of cytokines, structural lung injury, and lung dysfunction; all these symptoms were ameliorated by the selective, allosteric inhibitor of PTP4A3, KVX-053. These findings provide the first evidence supporting a role for PTP4A3 in the development of SARS-CoV-2-mediated ALI. SIGNIFICANCE STATEMENT: This study tested the hypothesis that targeting PTP4A3 would prevent COVID-19-associated ALI/ARDS. Social insects offer powerful models to investigate the mechanistic foundation of elaborate individual behaviors comprising a cooperative community. Workers of the leafcutter ant genus Atta provide an extreme example of behavioral segregation among many phenotypically distinct worker types. We utilize the complex worker system of Atta cephalotes to test the molecular underpinnings of behavioral programming and, in particular, the extent of plasticity to reprogramming. We identify specific neuropeptides as mediators of worker division of labor in A. cephalotes , finding two neuropeptides associated with characteristic behaviors of leafcutting and of brood care. Manipulation via genetic knockdown or by injection of these neuropeptides led to stark loss or gain of each behavior and to transcriptomic shifts in the predicted direction, that is, towards gene pathways expressed in the natural caste. We also compare specific A. cephalotes worker transcriptomes with those of orthologous workers of the eusocial mammal, the naked mole rat H. gaber , revealing global similarities between caste-biased expression and link to specific roles of our studied neuropeptides in ants. This work underscores the essential function of neuropeptides in establishing complex social behavior and a remarkable plasticity among individual behavioral types.

Objectives: To evaluate the feasibility and acceptability of an emotional support programme for newly diagnosed people with multiple sclerosis., Design: Three-arm, mixed methods, randomised controlled trial comparing usual care, versus usual care plus nurse-specialist support, versus usual care plus nurse-specialist support plus peer support., Participants: Community-dwelling adults within two years of diagnosis or undergoing diagnosis., Interventions: PrEliMS involves information provision, emotional support, and strategies and techniques based on psychoeducation, Acceptance and Commitment Therapy principles, supportive listening. One version of the intervention was provided by nurse-specialists alone and the other was provided by nurse-specialists plus peer support., Main Measures: The main outcome of interest was the feasibility of proceeding to a definitive trial, exploring recruitment rate, acceptability, completion of outcome measures (perceived stress, mood, self-efficacy, psychological impact, and service use), and signal of efficacy., Results: Of 40 participants randomised (mean age 36.2 years (SD = 14.8); 54% women; 85% with relapsing-remitting MS), 36 and 38 returned 3- and 6-month questionnaires, respectively. Participant interviews suggested the trial was largely feasible, and the intervention acceptable, with some amendments to trial procedures and intervention delivery noted. There were, however, no statistically significant differences between groups at followup for any measures, and effect-size estimates were small., Conclusion: A definitive trial combining nurse-specialist and peer support adjustment to diagnosis intervention is warranted, but more work exploring the delivery and fidelity of the intervention is needed before this is pursued.

Hydrochloric acid is one of the most prevalent and hazardous chemicals. Accidental spills occur in industrial plants or during transportation. Exposure to HCl can induce severe health impairment, including acute and chronic pulmonary diseases. We have previously described the molecular, structural, and functional aspects of the development of chronic lung injury and pulmonary fibrosis caused by intratracheal instillation of HCl in mice. Although mouse models of human disease have many advantages, rodents are evolutionary far from human and exhibit significant anatomical and physiological differences. Genetic and anatomic similarities between rabbits and humans are significantly higher. Rabbit models of HCl-induced lung injury have been used sparsely to evaluate acute lung injury. In this study, for the first time, we utilized rabbits as a model of HCl-induced pulmonary fibrosis and chronic lung injury. Neutropenia is a relatively uncommon but notable secondary effect of HIV infection. While the various hematopoietic effects of HIV and AIDS are well-described in the literature, high-quality evidence directly linking neutropenia with mortality in HIV-infected patients remains limited. The multifactorial etiology of neutropenia complicates its diagnosis, particularly when it occurs secondary to HIV. We present the case of a 35-year-old African American male with congenital HIV, who presented with severe neutropenia accompanied by a fever in the context of untreated HIV. The initial differential diagnosis was broad, including benign ethnic neutropenia (given the patient's African American ethnicity), tuberculosis (given the potential for anti-tuberculosis therapy to cause neutropenia and its commonality as a co-infection in HIV patients), sepsis-related neutropenia, and AIDS-related bone marrow suppression. However, through further workup, it became apparent that HIV-related bone marrow suppression ultimately led to pancytopenia. This case highlights how HIV patient non-adherence to antiretroviral therapy (ART) and hematologic abnormalities complicate the diagnosis of hematopoietic abnormalities from HIV. It also discusses how vertical transmission and abrupt ART discontinuation create a new phenotype of HIV patients with delayed presentations of AIDS-related complications. This patient's presentation also provides insight into the consequences of untreated HIV following the self-discontinuation of long-term HIV management therapy due to low healthcare literacy and loss of follow-up. Experiencing trauma increases risk for posttraumatic stress disorder (PTSD) and depression, and individuals who experience psychopathology after a traumatic event often experience symptoms from both disorders. Because a tendency to view events in a more negative light and a propensity toward threat appraisals are risk factors for both PTSD and depression, negative valence bias-a tendency to appraise emotional ambiguity as having a more negative (less positive) meaning-may be a transdiagnostic risk factor. In other words, we expect individuals with a negative valence bias experience greater PTSD and depression symptoms. We measured valence bias and self-reported PTSD and depression symptoms in a sample of college students in 2021 ( n = 287; 72.5% reported experiencing trauma). Although valence bias was not associated with PTSD symptoms as a whole, we found in our exploratory model that more negative bias was associated with greater dysphoria-related PTSD symptoms and greater depression symptoms (indirect effect p = .03). Thus, we propose a model whereby a more negative valence bias contributes to increased susceptibility for maladaptive stress responses, which may be associated with greater likelihood of symptoms of dysphoria-related PTSD and depression. These findings suggest that valence bias represents a transdiagnostic affective risk factor, warranting future research examining the impacts of bias-altering interventions (e.g., mindfulness-based treatments) as a means for managing symptoms in individuals with heightened dysphoria-related PTSD and/or depression symptoms. Additionally, in post hoc analyses it emerged that Latinx participants displayed a more negative valence bias, indicating the need for more research in diverse samples. Objectives: Hospitals in the US operate under various value-based payment programs, but little is known regarding the strategies they use in this context to improve quality and reduce costs, overall or in voluntary programs including Bundled Payments for Care Improvement Advanced (BPCI-A)., Study Design: A survey was administered to hospital leaders at 588 randomly selected acute care hospitals, with oversampling of BPCI-A participants, from November 2020 to June 2021. Twenty strategies and 20 barriers were queried in 4 domains: inpatient, postacute, outpatient, and community resources for vulnerable patients., Methods: Summary statistics were tabulated, and responses were adjusted for sampling strategy and nonresponse., Results: There were 203 respondents (35%), of which 159 (78%) were BPCI-A participants and 44 (22%) were nonparticipants. On average, respondents reported implementing 89% of queried strategies in the inpatient domain, such as care pathways or predictive analytics; 65% of postacute strategies, such as forming partnerships with skilled nursing facilities; 84% of outpatient strategies, such as scheduling close follow-up to prevent emergency department visits/hospitalizations; and 82% of strategies aimed at high-risk populations, such as building connections with community resources. There were no differences between BPCI-A and non-BPCI-A hospitals in 19 of 20 care redesign strategies queried. However, 78.3% of BPCI-A-participating hospitals reported programs aimed at reducing utilization of skilled nursing and inpatient rehabilitation facilities compared with 37.6% of non-BPCI-A hospitals (P < .0001)., Conclusions: Hospitals pursue a broad range of efforts to improve quality. BPCI-A hospitals have attempted to reduce use of postacute care, but otherwise the strategies they pursue are similar to other hospitals.

Background: Pre-exposure prophylaxis (PrEP) and postexposure prophylaxis (PEP) prevent HIV among individuals at high risk for acquisition. Pre-existing structural barriers to PrEP/PEP access among rural patients may be exacerbated further if pharmacies do not keep PrEP/PEP in stock, constituting a significant barrier to mitigating the HIV epidemic., Objectives: To compare PrEP/PEP availability for same-day pickup in rural vs urban Georgia and Pennsylvania pharmacies., Methods: We conducted a cross-sectional simulated patient caller study, calling pharmacists in Georgia and Pennsylvania to see whether PrEP/PEP was available for same-day pickup. We identified retail pharmacies through state pharmacy boards and categorized rurality using state-based definitions. We used multivariable logistic regression to assess PrEP availability by rurality and Ending the HIV Epidemic (EHE) designation, accounting for chain pharmacy status and county-level racial composition., Results: Among 481 pharmacies contacted (304 in Pennsylvania and 177 in Georgia), only 30.77% had PrEP for same-day pickup and only 10.55% had PEP for same-day pickup. PrEP availability did not differ significantly by state. Urban pharmacies had 2.02 (95% CI: 1.32-3.09) greater odds of PrEP same-day availability compared to rural pharmacies. Pharmacies in EHE counties had 3.45 (95% CI: 1.9-6.23) times higher odds of carrying PrEP compared to non-EHE counties., Conclusions: Pharmacies were unlikely to carry PrEP or PEP. Pharmacies in rural compared to urban, and non-EHE compared to EHE locations were less likely to carry PrEP.
