Abstract Background While often unrecognized, brucellosis, a significant zoonotic disease, silently endangers the health of mothers and children worldwide. This scoping review sheds light on transmission pathways, maternal–fetal consequences, and treatment hurdles, specifically considering maternal and child health concerns. Method To comprehensively grasp brucellosis in mothers and children, we systematically scoured electronic databases (DOAJ, Google Scholar, PubMed, and Semantic Scholar) for studies published after 2005. Our search included experimental studies (both randomized controlled clinical trials and quasi-experimental), analytical observations, descriptive reports, qualitative papers, and existing systematic reviews. All retrieved data were then charted and processed following Arksey and O'Malley's established framework for scoping reviews. Result Twenty-five studies spanning varied regions and methodologies met inclusion criteria. Key findings demonstrate that zoonotic brucellosis acquisition from livestock exposures among vulnerable maternal groups accounts for up to 70% of cases. Vertical transmission from mother to child during pregnancy, delivery, or breastfeeding was reported in 15–20% of cases. Substantial risks of miscarriage (25%), preterm birth (20%), hepatosplenomegaly (10%), febrile illness (30%), and possible long-term complications were documented. Treatment success rates using combination antibiotic therapy were reported to be as high as 98%, though emerging antibiotic resistance patterns challenge effective treatment, with 25% of Brucella isolates resistant to rifampin and 51% resistant to both trimethoprim-sulfamethoxazole and streptomycin. Conclusion This review reveals the alarming yet hidden toll brucellosis takes on maternal–fetal pairs and breastfeeding. In regions battling this endemic disease, tailored education, upgraded diagnostic tools, prompt antibiotic therapy, responsible antimicrobial stewardship, and One Health collaborations offer crucial pathways to shield mothers and children from its harmful consequences. Continued research will pave the way for even better solutions to alleviate this complex zoonosis, particularly for vulnerable populations.

Our preliminary data using bone marrow-derived macrophages (BMDMs) collected from ICR mice treated with anti-sirtuin (anti-SIRT) 1 antibody showed that Brucella uptake was significantly attenuated. We then further investigated the effect of an inhibitor of SIRT1/2, cambinol, in the progression of Brucella. The in vitro results using RAW264.7 cells revealed that cambinol treatment had no effect on adhesion, uptake, intracellular survival and nitric oxide (NO) production during B. abortus infection, nor did it directly affect bacterial growth for up to 72 h. Finally, intraperitoneal treatment of 8-week-old female ICR mice infected with Brucella showed no differences in the total average weights of spleens and livers; however, the treated mice displayed higher Brucella colony-forming units (CFUs) from the spleens. Furthermore, the interleukin (IL)-10 serum level was observed to be lower in treated mice at 7 d post-infection, and none of the cytokines tested showed a change at 14 d post-infection. The overall findings showed that cambinol treatment had no effect on the proliferation of Brucella in RAW264.7 macrophages but exacerbated the splenic proliferation of the bacteria in mice and displayed reduced anti-inflammatory cytokine IL-10 at the first week of infection, suggesting that cambinol as an inhibitory of SIRT1/2 could be beneficial in the context of Brucella dissemination in animal hosts and that exploration of activating SIRTs could be an alternative treatment against Brucella infection.

Abstract Background Due to antibiotic overuse, numerous organisms have evolved multidrug resistance, a global health crisis. Klebsiella pneumoniae (K. pneumoniae) causes respiratory, biliary, and urinary infections. It initially impacts individuals with impaired immune systems. Main body of abstract The treatment of K. pneumoniae clones poses significant challenges, highlighting the need for new, more effective pathogen control methods. Our understanding of the natural or acquired antibiotic resistance mechanisms in K. pneumoniae is inadequate. Therefore, it is crucial to continue searching for novel compounds to fight against K. pneumoniae and to understand its antibiotic resistance processes fully. Short conclusion This review covers K. pneumoniae antibiotic resistance in detail. The review also identifies the molecular components of natural or acquired K. pneumoniae antibiotic resistance mechanisms. This study further digs into the novel pharmacological targets to offer therapy insights.

Protective efficiency of a combination of four recombinant Brucella abortus (B. abortus) proteins, namely outer membrane protein (Omp) 16, Omp19, Omp28, and 50S ribosomal protein L7/L12 was evaluated as a combined subunit vaccine (CSV) against B. abortus infection in RAW 264.7 cell line and murine model. The immunoreactivity of these four recombinant proteins as well as pCold-TF vector reacted with Brucella-positive serum individually, but not with Brucella-negative serum by immunoblotting assay. CSV-treated RAW 264.7 cells significantly induced production of IFN-γ and IL-12 while decreased IL-10 production at the late stage of infection compared to PBS-treated control cells. In addition, the enhancement of nitric oxide production together with cytokines secretion profile in CSV-treated cells proved that CSV notably activated bactericidal mechanisms in macrophages. Consistently, mice immunized with CSV strongly elicited production of pro-inflammatory cytokines TNF-α, IL-6 and MCP-1 compared to PBS control group. Moreover, the concentration of IFN-γ was >IL-10 and titers of IgG2a were also heightened compared to IgG1 in CSV-immunized mice which suggest that CSV induced predominantly T helper 1 T cell. These results suggest that the CSV used in the present study is a potential candidate as a preventive therapy against brucellosis.

Abstract Background Brucella causes a chronic and debilitating infection that leads to great economic losses and a public health burden. In this study, we demonstrated the brucellacidal effect of heat shock mediated by the induction of pro-inflammatory cytokines, reactive oxygen species (ROS) accumulation and apoptosis in murine macrophages and in mice. Results RAW264.7 cells were incubated at 43 °C, and BALB/c mice were subjected to whole body hyperthermia. The data showed a reduction in bacterial survival in the mice after daily heat exposure. This was accompanied by increased levels of cytokines TNF, IL-6, IL-1β and IFN-γ in the sera of the mice. Gene expression of NF-κB and inducible nitric oxide production were also induced in the mouse splenic cells. In parallel with the bacterial reduction in the mouse model, an increased bactericidal effect was observed in RAW264.7 cells after exposure to heat stress. In addition, the heat stress increased both the nuclear translocation of NF-κB and the expression of the heat shock proteins HSP70 and HSP90 in murine macrophages. Furthermore, heat exposure induced the increase of pro-inflammatory cytokines, ROS accumulation and apoptosis but did not affect the production of nitric oxide (NO) in macrophages. Conclusion This study demonstrated the induction of innate immune responses by heat stress that significantly reduced the intracellular survival of B. abortus in vitro and in vivo. Transcriptional factor NF-κB, which is a master regulator, could be termed a key activator of heat-induced immunity against Brucella. The increase in the expression and activation of NF-κB in splenic cells and macrophages was followed by enhanced antimicrobial effectors, including cytokines, ROS and NO that may contribute to the reduction of bacterial survival.

Background: We previously elucidated the protective mechanism of Korean red ginseng oil (RGO) against Brucella abortus infection, and our phytochemical analysis revealed that palmitic acid (PA) was an abundant component of RGO. Consequently, we investigated the contribution of PA against B. abortus. Objectives: We aimed to investigate the efficacy of PA against B. abortus infection using a murine cell line and a murine model. Methods: Cell viability, bactericidal, internalization, and intracellular replication, western blot, nitric oxide (NO), and superoxide (O2 -) analyses and flow cytometry were performed to determine the effects of PA on the progression of B. abortus infection in macrophages. Flow cytometry for cytokine analysis of serum samples and bacterial counts from the spleens were performed to determine the effect of PA in a mouse model. Results: PA did not affect the growth of B. abortus. PA treatment in macrophages did not change B. abortus uptake but it did attenuate the intracellular survivability of B. abortus. Incubation of cells with PA resulted in a modest increase in sirtuin 1 (SIRT1) expression. Compared to control cells, reduced nitrite accumulation, augmented O2 -, and enhanced pro-inflammatory cytokine production were observed in PA-treated B. abortus-infected cells. Mice orally treated with PA displayed a decreased serum interleukin-10 level and enhanced bacterial resistance. Conclusions: Our results suggest that PA participates in the control of B. abortus within murine macrophages, and the in vivo study results confirm its efficacy against the infection. However, further investigations are encouraged to completely characterize the mechanisms involved in the inhibition of B. abortus infection by fatty acids. [ABSTRACT FROM AUTHOR]
