• No study has assessed the clinical and biological features of chikungunya virus epidemics in French Guiana. • We performed the first study suggesting the use of a clinical score to differentiate dengue virus infection (DENV) from chikungunya (CHIKV) infection. • This is a topical theme as DENV and CHIKV epidemics recently occurred in India and Brazil. External validation of this score will thus be possible. • The study sample was relatively large (162 CHIKV patients versus 452 DENV patients). Only two other studies have compared CHIKV and DENV: a study conducted in Singapore included 117 CHIKV patients and 862 DENV patients, and a study conducted in Gabon included 270 CHIKV patients and 53 DENV patients. It should be noted that both of these studies reached contradictory conclusions. Chikungunya (CHIKV) and dengue viruses (DENV) are two arboviruses with epidemic potential and similar clinical presentations. The potential life-threatening risk associated with DENV justifies an immediate biological assessment and medical follow-up which may be delayed for CHIKV. To compare the clinical variables that would help differentiate patients infected with CHIKV or DENV, and then to compute a predictive score. Retrospective case-control study comparing CHIKV-infected patients diagnosed by RT-PCR in 2014 with patients infected with DENV diagnosed by positive NS1 antigen test in 2013. Children aged < 15 years and pregnant women were excluded. Clinical and biological variables were compared, and a multivariate analysis was performed. A clinical score was developed using the β coefficients to differentiate the infections. Over the study period 168 patients infected with CHIKV were compared with 452 patients with DENV. The clinical variables independently associated with CHIKV was joint and back pain, and those associated with DENV were headache, muscle pain, nausea/vomiting, diarrhea, and hemorrhagic signs. The clinical score had 98% sensitivity for DENV and a ROC curve of 0.96. These two infections have a similar clinical presentation but the use of the proposed clinical score during the acute phase of the disease would make it possible to identify cases of DENV during a CHIKV epidemic to suggest adequate patient management. Introduction Le virus Chikungunya (CHIKV) et celui de la dengue (DENV) sont deux arboviroses à fort potentiel épidémique. Les deux épidémies survenues en Guyane, respectivement en 2014 et 2013, ont semblé toucher des populations et quartiers très différents malgré leur vecteur et mode de transmission commun. L’objectif principal de l’étude était de comparer les indicateurs démographiques, sociaux et économiques des premiers patients infectés par DENV et CHIKV au début de ces deux épidémies à Cayenne. Matériels et méthodes Étude rétrospective cas-témoins comparant les 168 premiers patients infectés par CHIKV, diagnostiqués par RT-PCR d’avril à juin 2014, aux 168 premiers patients infectés par DENV diagnostiqués par NS1 de mars à août 2013. Les enfants de moins de 15 ans et les femmes enceintes ont été exclus. Les indicateurs dont les lieux de naissance, types de prise en charge sociale et quartiers de résidence ont été comparées en analyse bivariée. Le découpage par Ilots Regroupés pour l’Information Statistique (IRIS) de l’île de Cayenne a été utilisé et un score composite élaboré par l’Institut national de la statistique et des études économiques y a été appliqué. Le score a été créé à partir de 7 indicateurs en vue de définir les zones géographiques prioritaires. Résultats Soixante-dix pour cent des patients CHIKV ( n = 117/168) étaient nés à l’étranger contre 34,0 % ( n = 58/168) pour DENV (OR 4,35 ; [2,69–7,06] ; p < 0,001) et à 45,0 % d’origine haïtienne ( n = 76/168). Près de 54,0 % ( n = 90/168) des patients CHIKV étaient en situation sociale, bénéficiant soit de l’aide médicale d’état, de la couverture maladie universelle ou étant sans couverture, contre 32,7 % des patients DENV (OR 2,37 ; [1,49–3,78] ; p < 0,001). Quatre-vingt-deux pour cent ( n = 123/168) vivaient dans des quartiers de bas niveau socio-économiques contre 44 % (5,81 ; [3,35–10,2] ; p < 0,001). Ces résultats doivent être interprétés avec prudence et ne doivent pas conduire à stigmatiser des personnes déjà en situation précaire, mais à l’inverse soulignent les réelles inégalités en matière de santé publique sur notre territoire. Conclusion Il s’agit de la première étude connue comparant CHIKV à DENV sur le plan socio-économique. Il semblerait que les patients en situations de précarité, sociale et économique, se retrouvent en premières ligne lors de l’apparition de maladies émergentes, comme le CHIKV. Plusieurs facteurs sont à prendre en considération, certains peu contrôlables tels que l’immunité globale de la population, quasi nulle pour CHIKV contrairement à DENV, et d’autres à améliorer tels que la gestion de l’environnement : assainissement, gestion des déchets, connexion à l’eau courante, afin de limiter la prolifération des vecteurs. Rares sont les études qui ont comparées les caractéristiques clinico-biologiques des patients infectés par ces 2 virus. Le risque hémorragique lié à DENV justifie d’un bilan biologique immédiat, alors que celui de CHIKV pourrait être différé, notamment dans un contexte d’épidémie de CHIKV dans une zone d’endémie de DENV. L’objectif principal de l’étude était de comparer les variables épidémiologiques, cliniques et biologiques qui aideraient à différencier un patient infecté par CHIKV ou DENV lors de sa première consultation médicale. L’objectif secondaire était de construire un score prédictif d’infection par DENV par rapport à CHIKV. Matériels et méthodes Étude rétrospective cas-témoins comparant les patients infectés par CHIKV diagnostiqués par RT-PCR d’avril à juin 2014 à des patients témoins, ambulatoires et hospitalisés, infectés par DENV diagnostiqués par NS1 de mars à août 2013. Les enfants de moins de 15 ans et les femmes enceintes ont été exclus. Les données cliniques et biologiques ont été comparées en analyse univariée puis multivariée. Un score clinique a été construit à partir des coefficients β de la multivariée, afin de différencier les deux infections. Résultats Sur la période d’étude 168 patients infectés par CHIKV ont été comparés à 452 patients DENV. La médiane d’âge était de 45,3 ans (IQR = 29,9–58,7) dans le groupe CHIKV contre 34,5 dans le groupe DENV (IQR = 29,9–58,7 ; p < 0,001) et un sex-ratio à 1/1. Ont été hospitalisés 8,3 % ( n = 14/168) des patients CHIKV contre 35,2 % ( n = 159/452) des DENV (OR 0,16 ; [0,08–0,30] ; p < 0,001). En analyse multivariée, les variables cliniques indépendamment associées à CHIKV étaient la présence d’arthralgies et de rachialgies, et celles associées à DENV les céphalées, myalgies, nausées/vomissements, diarrhées et signes hémorragiques. Un score clinique allant de 0 à + 15 comprenant ces 7 variables a été construit et son seuil choisi de façon à obtenir un score très sensible pour la suspicion d’infection par DENV par rapport à CHIKV. En effet, le risque de dengue hémorragique justifie de pouvoir identifier un cas de DENV en cas d’épidémie de CHIKV, nécessitant alors un suivi clinique, biologique et une recherche de critères de gravités particulièrement attentive. Conclusion Il s’agit de la première étude comparant CHIKV à DENV avec une telle taille d’échantillon. Ces deux infections ont une présentation clinique proche, mais l’application du score clinique proposé à la phase aiguë de la maladie permettrait d’identifier les cas de DENV en cas d’épidémie de CHIKV afin de proposer une prise en charge adéquate au patient. Introduction En Guyane, les chauves-souris sont responsables de morsures à risque de transmission du virus de la rage. L’espèce hématophage, Desmodus rotondus , est la plus redoutée, mais il existe plus de 100 espèces toutes potentiellement porteuses du virus, ce qui expose les chiroptérologues à un risque lors des captures à visée scientifique. L’objectif de cette étude était d’évaluer les connaissances de ces professionnels vis-à-vis du risque rabique et de sa prévention et d’évaluer leurs besoins en formation et informations. Matériels et méthodes Etude prospective descriptive sur la base d’un auto-questionnaire en ligne (Sondage Online) réalisé du 6 octobre au 6 novembre 2017 et adressé aux 60 membres de la liste de diffusion électronique « chiroguyane ». Les données en rapport avec leurs pratiques, leurs connaissances de la maladie, son mode de transmission, du traitement, de la vaccination antirabique et du suivi sérologique ont été analysées. Résultats Vingt personnes ont répondu au questionnaire (33 % de la liste de diffusion), dont 19 complètement. 65 % étaient des hommes, 100 % originaires d’Europe, 45 % de plus de 40 ans. 50 % étaient en contact direct avec des chauves-souris au moins une fois par mois et 20 % plusieurs fois par semaine. 37 % manipulaient à mains nues. 95 % savaient que l’agent de la rage est un virus. 95 % savaient qu’il existe un traitement post-exposition, vaccin pour 54 % d’entre eux et immunoglobulines pour 27 %. 84 % savent que la maladie est systématiquement mortelle une fois déclarée. 59 % identifiaient les chauves-souris comme réservoir de la rage en Guyane et 33 % comme vecteur principal, 28 % pour les chiens. Seules 2 personnes ignoraient qu’il y avait déjà eu un cas de rage en Guyane (11 %). 63 % savaient que la vaccination pré-exposition était recommandée chez les chiroptérologues. 79 % avaient eu un protocole antirabique complet. 56 % d’entre eux avaient fait au moins un contrôle d’anticorps. 63 % savaient qu’une consultation au CTAR était nécessaire à chaque nouvelle exposition, et 90 % pensait que cette vaccination était nécessaire à leur activité. 63 % pensaient être insuffisamment informés et 79 % souhaitait plus d’informations. Conclusion Les chiroptérologues semblent assez bien informés sur la rage et le risque inhérent à leur activité, en contraste avec l’importance des prises de risques. Les connaissances sur les possibilités thérapeutiques sont moins bonnes et la couverture vaccinale déclarée insuffisante. Les informations sur la prise en charge pré et post-exposition du risque rabique et le suivi sérologique pourront être proposés via des groupes de diffusion et Facebook, et dans un cadre associatif. Introduction Le traitement préventif des reviviscences de paludisme à Plasmodium vivax préconisé par l’OMS est la primaquine à la dose de 15 mg/j pendant 14 jours. Mais en raison d’une augmentation du nombre de reviviscences post traitement notamment en Asie du sud-est, le CDC préconise depuis 2006 l’utilisation de primaquine à la dose de 30 mg/j. En France, les cas importés de paludisme à P . vivax sont traités à cette posologie selon l’avis du Haut Conseil de la santé publique en 2008. Cette augmentation de posologie ne semble pas justifiée en Guyane et expose potentiellement à la survenue plus fréquente d’effets indésirables à la dose de 30 mg/j. L’objectif de cette étude était de comparer l’efficacité de deux posologies de primaquine (15 vs 30 mg/j pendant 14 jours) pour la prévention des reviviscences de P . vivax de l’adulte dans notre hôpital du 01/01/2007 au 01/08/2016. Matériels et méthodes Pour repérer les cas de reviviscence, nous avons exploré la base de données du laboratoire de parasitologie et le dossier médical de chacun des patients, en considérant la période de 6 mois à partir de la date du traitement par primaquine. Les cas survenant au-delà de 6 mois en zone d’endémie ont été considérés comme des nouvelles infections. Une description par courbe de Kaplan-Meier a été effectuée pour comparer la survenue d’évènements selon le dosage de primaquine utilisé. Un test du log rank a été utilisé pour comparer les deux groupes de traitement. Résultats Le nombre de patients inclus était de 464, âgés de 35 (± 12) ans, avec un sex-ratio H/F de 3,5. Ils étaient 236 à avoir reçu 15 mg/j, et 228 à avoir reçu 30 mg/j de primaquine. Les patients ont été traités à la dose de 15 mg/kg avant 2011, et 30 mg/kg après 2011. Le nombre de reviviscences était de 26 (11,0 %) et de 27 (11,8 %) à 15 et 30 mg/j, respectivement. Le test du log rank ne met pas en évidence de différence entre les deux groupes ( p = 0,76). Conclusion Ces résultats préliminaires ne montrent pas de différence d’efficacité entre les posologies de primaquine à 15 mg/j et 30 mg/j pendant 14 jours pour des cas d’infections à Plasmodium vivax survenus en Guyane. We report here an atypical case of acute sacroiliitis caused by Erysipelothrix rhusiopathiae revealing tricuspid endocarditis in a 53-year-old woman without medical history. She was admitted to Cayenne hospital because of intense right hip and thigh pain, associated with fever. A right sacroiliitis was visible on the computed tomography (CT) scan, confirmed on MRI. Transesophageal echocardiography revealed a large mobile tricuspid vegetation. Blood cultures were positive for E. rhusiopathiae . CT scan showed pulmonary alveolar opacities, consistent with septic emboli. Clinical improvement was obtained under ceftriaxone followed by ciprofloxacin for 6 weeks of treatment. We present a review of bone and joint infections caused by E. rhusiopathiae . So far, not a single case has been reported in Latin America., (Copyright © 2022 SFMTSI.)

The estimated seroprevalence in the general population after chikungunya virus (CHIKV) epidemics ranged from 38 to 63%. Despite a low case fatality, subacute and chronic rheumatic forms of CHIKV infection generate significant morbidity and have a socioeconomic impact. The objective of the study was to estimate the prevalence of chronic post-CHIKV rheumatic or musculoskeletal pain (pCHIK-RMSP) at 3 and 6 months after the initial symptoms. An observational study was conducted at Cayenne General Hospital in French Guiana between April 1 and June 30, 2014. All patients seen for CHIKV infection confirmed by RT-PCR were prospectively included. Pregnant women and children under 15 were excluded from the study. All patients were called by phone at 3 and 6 months to enquire about the presence of pCHIK-RMSP. Out of a total of 254 eligible patients, 168 were selected. The mean age was 45.3 years (SD ± 1.4 yo) and the sex ratio (M/F) was 0.75. No death was reported. At 3 months, 40.2% (95% CI 31.1-49.3) of patients (n = 45/112) had pCHIK-RMSP and 31.3% (95% CI 22.2-40.4) of patients (n = 31/99) at 6 months. The median time of end to pain was 2 weeks after the date of onset of signs. The present study provides succinct but informative data about pCHIK-RMSP, which represents the real burden of the disease. There are few studies on that subject in the Amazonian region, but our study shows a lower impact than in the Indian Ocean islands where the population is older.

Background: The preventive treatment of Plasmodium vivax relapse recommended by the World Health Organization is primaquine at a dose of 15 mg/day for 14 days, except for malaria cases from Asia and Oceania. Since 2006, CDC recommends the use of primaquine at 30 mg/day for 14 days. In France, all cases of malaria due to P. vivax are treated with 30 mg of primaquine. This systematically increased dosage needs to be evaluated according to epidemiological context. The aim of the study was to compare relapses after 14 days of primaquine at 15 or 30 mg/day., Methods: All patients treated with primaquine after a vivax malaria episode in French Guiana, between 1 January, 2007 and 1 August, 2016, were studied. Based on the compulsory hospital pharmacy forms for primaquine delivery, adult patients who received 15 or 30 mg of primaquine during 14 days for hypnozoite eradication were included. The recommended dose was initially 15 mg and was changed to 30 mg in 2011. Vivax malaria recurrences within 2 months after primaquine treatment, and vivax malaria recurrences 2-6 months after primaquine in each treatment group were analysed using survival analysis at 2, 3 and 6 months., Results: Out of 544 patients included, 283 received 15 mg/day and 261 received 30 mg/day of primaquine. At 2 and 3 months after primaquine treatment, the number of recurrences was 7 (2.5%) and 19 (7.3%), and 9 (3.4%) and 15 (5.3%), in the 15 and 30 mg groups (p = 0.51 respectively 0.35), respectively. Within 3 months, the median time to recurrence was 2.05 months in the 15 and 30 mg groups. At 6 months after primaquine treatment, the number of recurrences was 25 (8.8%) and 31 (11.9%) at 15 and 30 mg, respectively (p = 0.24). The median time to recurrence was 2.38 months at 15 mg/day and of 2.64 months at 30 mg/day., Conclusions: There were no significant differences between primaquine at 15 or 30 mg/day for 14 days in the prevention of P. vivax relapses at 2, 3 and 6 months after primaquine treatment in French Guiana.

Thrombotic thrombocytopenic purpura (TTP) is a thrombotic microangiopathy associated to severe ADAMTS13 deficiency. It has been linked to various viral infections. Among arboviruses, only Crimean-Congo haemorrhagic fever and dengue fever have been linked to this severe disease. Background: Identifying patients at risk of developing severe dengue is challenging. The objective of the present study was to determine the incidence of hypotension and its predictive factors during the Dengue 2 epidemic in 2013., Methods: In 2013, a longitudinal study was performed using data from all confirmed cases of dengue seen in Cayenne General Hospital. The analysis used Cox proportional modeling to obtain adjusted hazards ratios for hypotension., Results: A total of 806 confirmed patients were included 78 (9.6%) of whom developed hypotension. Extensive purpura, cutaneomucous hemorrhage, serous effusion and age 1-15 years were associated with subsequent hypotension whereas 'aches' and a rash were associated with a lower incidence of hypotension. The biological variables independently associated with hypotension were: increase of hematocrit, low protein concentrations, low sodium concentration and lymphocytes over 1400/ml. A risk score was computed from the scaled Cox model coefficient., Conclusions: From a clinician's perspective, extensive purpura, cutaneomucous hemorrhage, serous effusion, age 1-15 years, hematocrit increase, low protein, low sodium, lymphocytosis and the absence of aches or of a rash, may be important warning signs to predict subsequent hypotension and shock. Over half of the patients with the highest risk score subsequently developed hypotension. The prognostic score had a 48.2% sensitivity with less than 10% of false positives. Introduction: The frequency, the clinical characteristics, and the prognosis of dengue is highly variable. Dengue fever is associated with a range of neurological manifestations. The objective of the present study was to determine the incidence of neurological signs and their predictive factors using data from cases of dengue seen and followed in Cayenne Hospital during the Dengue 2 epidemic in 2013., Methods: In 2013, a longitudinal study using data from all cases of dengue seen in Cayenne hospital was collected. Medical records used a standardized form to collect demographic information, clinical signs and biological results and the date at which they were present. The analysis used Cox proportional modeling to obtain adjusted Hazard ratios., Results: A total of 1574 patients were included 221 of whom developed central nervous system signs. These signs were spontaneously resolutive. There were 9298person days of follow-up and the overall incidence rate for central nervous system signs was 2.37 per 100 person-days. The variables independently associated with central nervous system anomalies were headache, Adjusted Hazard ratio (AHR) = 1.9(95%CI = 1.4-2.6), bleeding AHR = 2 ((95%CI = 1.3-3.1), P = 0.001, abdominal pain AHR = 1.9 ((95%CI = 1.4-2.6), P<0.001, aches AHR = 2.1 ((95%CI = 1.5-2.9), P<0.001, and fatigue AHR = 1.5 ((95%CI = 1.3-1.7), P<0.001., Discussion: Overall, the present study suggests that neurological signs of dengue are not exceptional even in patients without the most severe features of dengue. These manifestations were spontaneously resolutive. Here it was not possible to distinguish between encephalitis or encephalopathy. Further studies would require more in depth exploration of the patients.

Background: The objective of the study was to determine the incidence of transaminase elevation during dengue, and its predictive factors., Methods: In 2013, a longitudinal study was performed using data from all cases of dengue seen in Cayenne Hospital. Cox proportional modeling was used. Signs of major transaminase elevation were defined as an increase in aspartate amino transferase (AST) or alanine amino transferase (ALT) concentration over 10 times the normal value (10N)., Results: There were 1574 patients and 13 249 person-days of follow-up. The incidence rate for signs of transaminase elevation (10N) was 0.55 per 100 person-days. Six patients had major transaminase elevation with AST>1000 units (0.43 per 1000 patient-days), and 73 patients (4.6%) developed transaminase elevation with AST >10N. The variables independently associated with major transaminase elevation were hyponatremia, low platelets, dehydration, hematocrit increase, food intolerance, positive nonstructural protein 1 (NS1), age over 15 years and the notion of paracetamol intake., Conclusions: Although very frequent, the incidence of major transaminase elevation was lower than reported elsewhere perhaps because of good access to care, or of the particular serotype causing this epidemic.
