Abstract Background We report an unusual case of metastatic colon adenocarcinoma to the maxilla as an initial clinical sign of the disease, this being the second case reported in the palate. In addition, we show an extensive review of the literature, with clinical cases of adenocarcinoma with metastasis to the mouth. Case presentation An 80-year-old man complained of “swelling on the palate” with a 3-week evolution time. He reported suffering from constipation and high blood pressure. The intraoral examination revealed a pedunculated, red, and painless nodule on the maxillary gingiva. Under the diagnostic hypotheses of squamous cell carcinoma and malignant neoplasm of the salivary gland, an incisional biopsy was performed. Microscopically, the columnar epithelium was observed forming papillary areas, neoplastic cells with prominent nucleoli, hyperchromatic nuclei, atypical mitotic figures, and mucous cells, being positive for CK 20, suggesting the provisional diagnosis of metastatic adenocarcinoma, probably of gastrointestinal origin. The patient was submitted to endoscopy and colonoscopy exams, and a lesion in the sigmoid region of the colon was observed. After a colon biopsy, a moderately differentiated adenocarcinoma was confirmed, establishing the final diagnosis of metastatic neoplasia of colon adenocarcinoma to the oral lesion. The literature review revealed 45 clinical cases of colon adenocarcinoma with metastasis to the oral cavity. To the best of our knowledge, it is the second case on the palate. Conclusions Colon adenocarcinoma with metastasis to the oral cavity is rare but should be included in the differential diagnosis of neoplasms of the oral cavity, even when there are no known primary tumors in some cases, and this may be the first indication of the presence of a tumor.

Studies have been directed towards the production of new titanium alloys, aiming for the replacement of Ti-6 Aluminium-4 Vanadium (TiAlV) alloy in the future. Many mechanisms related to biocompatibility and chemical characteristics have been studied in the field of implantology, but enzymatic defenses against oxidative stress remain underexplored. Bone marrow stromal cells have been explored as source of cells, which have the potential to differentiate into osteoblasts and therefore could be used as cells-based therapy. The objective of this study was to evaluate the activity of the antioxidant enzymes superoxide dismutase (SOD) and catalase (CAT) in porous scaffolds of Ti-6 Aluminium-4 Vanadium (TiAlV), Ti-35 Niobium (TiNb), and Ti-35 Niobium-7 Zirconium-5 Tantalum (TiNbZrTa) on mouse bone marrow stromal cells. Porous titanium alloy scaffolds were prepared by powder metallurgy. After 24 hours, cells plated on the scaffolds were analyzed by scanning electron microscopy (SEM). The antioxidant enzyme activity was measured 72 hours after cell plating. Quantitative real time PCR (qRT-PCR) was performed after 3, 7, and 14 days, and Runx2 (Runt-related transcription factor2) expression was evaluated. The SEM images showed the presence of interconnected pores and growth, adhesion, and cell spreading in the 3 scaffolds. Although differences were noted for SOD and CAT activity for all scaffolds analyzed, no statistical differences were observed (p>0.05). The osteogenic gene Runx2 presented high expression levels for TiNbZrTa at day 7, compared to the control group (TiAlV day 3). At day 14, all scaffolds had more than 2-fold induction for Runx2 mRNA levels, with statistically significant differences compared to the control group. Even though we were not able to confirm statistically significant differences to justify the replacement of TiAlV regarding antioxidant enzymes, TiNbZrTa was able to induce faster bone formation at early time points, making it a good choice for biomedical and tissue bioengineering applications.

Titanium (Ti) and Ti-6 Aluminium-4 Vanadium alloys are the most common materials in implants composition but β type alloys are promising biomaterials because they present better mechanical properties. Besides the composition of biomaterial, many factors influence the performance of the biomaterial. For example, porous surface may modify the functional cellular response and accelerate osseointegration. This paper presents in vitro and in vivo evaluations of powder metallurgy-processed porous samples composed by different titanium alloys and pure Ti, aiming to show their potential for biomedical applications. The porous surfaces samples were produced with different designs to in vitro and in vivo tests. Samples were characterized with scanning electron microscopy (SEM), energy dispersive spectroscopy (EDS) and elastic modulus analyses. Osteogenic cells from newborn rat calvaria were plated on discs of different materials: G1-commercially pure Ti group (CpTi); G2-Ti-6Al-4V alloy; G3-Ti-13 Niobium-13 Zirconium alloy; G4-Ti-35 Niobium alloy; G5-Ti-35 Niobium-7 Zirconium-5 Tantalum alloy. Cell adhesion and viability, total protein content, alkaline phosphatase activity, mineralization nodules and gene expression (alkaline phosphatase, Runx-2, osteocalcin and osteopontin) were assessed. After 2 and 4 weeks of implantation in rabbit tibia, bone ingrowth was analyzed using micro-computed tomography (μCT). EDS analysis confirmed the material production of each group. Metallographic and SEM analysis revealed interconnected pores, with mean pore size of 99,5μm and mean porosity of 42%, without significant difference among the groups (p>0.05). The elastic modulus values did not exhibit difference among the groups (p>0.05). Experimental alloys demonstrated better results than CpTi and Ti-6Al-4V, in gene expression and cytokines analysis, especially in early experimental periods. In conclusion, our data suggests that the experimental alloys can be used for biomedical application since they contributed to excellent cellular behavior and osseointegration besides presenting lower elastic modulus.

This in vitro and in vivo study compared different topographies of Ti samples (dense, porosity of 30% and 40%) with or not CaP coating, prepared by powder metallurgy. Osteogenic cells from newborn rat calvaria were plated onto the samples and cell adhesion (24 hours), alkaline phosphatase activity (7 and 10 days) and mineralization nodules (14 days) were assessed. Sixteen rabbits were used for in vivo study. Each animal received three non-treated and three treated implants in the right or left tibia, respectively. Histometric evaluation of bone-implant contact (BIC) were assessed at 1, 2, 4 and 8 weeks. Metallographic analysis revealed porosities of 30% and 40%, with pore size ranging from 250 to 350 µm. Cell adhesion test and ALP revealed similar cell behavior, independently of topography and CaP coating (P > 0.05%). However, CaP coating combined with porosity of 40% influenced positively the mineralized matrix formation (P < 0.05%). CaP-coated implants showed higher BIC than non-CaP implants and BIC was different between the short (1 and 2 weeks) and long (4 and 8 weeks) healing periods (P < 0.05%). The results suggest that CaP coating combined with 40% porosity implants allowed greater osteogenesis in vitro and increased BIC in vivo.

The purpose of this study was to evaluate bone regeneration in rabbits, comparing two types of physical barriers used to treat bone defects in guided tissue regeneration (GTR). Two osseous defects (8 mm in diameter) were performed in each hind-foot of four adult rabbits, using surgical burs with constant sterile saline solution irrigation. Defects obtained on the right hind-foots were protected with polytetrafluoroethylene (PTFE) barriers while Gengiflex membranes were used over wounds created in the left hind-foots. Rabbits were sacrificed after three months for histological evaluation of treatments performed. Defects covered with PTFE barriers were completely repaired with bone tissue. Incomplete lamellar bone formation was detected in defects treated with Gengiflex membrane, resulting in voids and lack of continuity of bone deposition. Results withdrawn from this study demonstrated that the non-porous PTFE barrier fulfilled all requirements to induce natural bone tissue regeneration, thus becoming a more effective alternative to treat osseous defects than Gengiflex membrane.

A porous material for bone ingrowth with adequate pore structure and appropriate mechanical properties has long been sought as the ideal bone-implant interface. This study aimed to assess in vivo the influence of three types of porous titanium implant on the new bone ingrowth. The implants were produced by means of a powder metallurgy technique with different porosities and pore sizes: Group 1 = 30 % and 180 µm; Group 2 = 30% and 300 µm; and Group 3 = 40% and 180 µm;. Six rabbits received one implant of each type in the right and left tibiae and were sacrificed 8 weeks after surgery for histological and histomorphometric analyses. Histological analysis confirmed new bone in contact with the implant, formed in direction of pores. Histomorphometric evaluation demonstrated that the new bone formation was statistically significantly lower in the group G1 than in group G3, (P = 0.023). Based on these results, increased porosity and pore size were concluded to have a positive effect on the amount of bone ingrowth.

Castor oil bean cement (COB) is a new material that has been used as an endodontic sealer, and is a candidate material for direct pulp capping. OBJECTIVE: The purpose of this study was to evaluate the biocompatibility of a new formulation of COB compared to calcium hydroxide cement (CH) and a control group without any material, in the subcutaneous tissue of rats. MATERIAL AND METHODS: The materials were prepared, packed into polyethylene tubes, and implanted in the rat dorsal subcutaneous tissue. Animals were sacrificed at the 7th and 50th days after implantation. A quantitative analysis of inflammatory cells was performed and data were subjected to ANOVA and Tukey's tests at 5% significance level. RESULTS: Comparing the mean number of inflammatory cells between the two experimental groups (COB and CH) and the control group, statistically significant difference (p=0.0001) was observed at 7 and 50 days. There were no significant differences (p=0.111) between tissue reaction to CH (382 inflammatory cells) and COB (330 inflammatory cells) after 7 days. After 50 days, significantly more inflammatory cells (p=0.02) were observed in the CH group (404 inflammatory cells) than in the COB group (177 inflammatory cells). CONCLUSIONS: These results demonstrate that the COB cement induces less inflammatory response within long periods.

Porous titanium scaffolds are promising materials for biomedical applications such as prosthetic anchors, fillers and bone reconstruction. This study evaluated the bone/titanium interface of scaffolds with interconnected pores prepared by powder metallurgy, using scanning electron microscopy (SEM) and energy dispersive spectroscopy (EDS). Porous scaffolds and dense samples were implanted in the tibia of rabbits, which were subsequently killed 1, 4, and 8 weeks after surgery. Initial bone neoformation was observed one week after implantation. Bone ingrowth in pores and the Ca/P ratio at the interface were remarkably enhanced at 4 and 8 weeks. The results showed that the interconnected pores of the titanium scaffolds promoted bone ingrowth, which increased over time. The powder metallurgy technique thus proved effective in producing porous scaffolds and dense titanium for biomedical applications, allowing for adequate control of pore size and porosity and promoting bone ingrowth.

INTRODUÇÃO: Os leiomiomas são neoplasias benignas do músculo liso que ocorrem mais comumente nos tratos geniturinário e gastrintestinal, entretanto são raros na cavidade bucal, na qual provavelmente a maioria desses tumores tem sua origem a partir de músculo liso vascular. OBJETIVOS: Relatar dois casos com história clínica semelhante, confrontando seus aspectos com aqueles encontrados na literatura. RELATO DOS CASOS: Dois casos de mulheres na quinta e sexta décadas de vida apresentando nódulo submucoso em ventre lingual. Após exame microscópico que revelou numerosos vasos sangüíneos entremeados a células fusiformes, com núcleos ovalados ou alongados, e exame imuno-histoquímico com positividade para actina de músculo liso nos dois casos, o diagnóstico final foi leiomioma vascular. CONCLUSÃO: O estudo do leiomioma vascular bucal é de extrema importância devido à raridade e semelhança desse com outras lesões da cavidade bucal. A análise imuno-histoquímica é importante para o diagnóstico final do leiomioma.INTRODUCTION: The leiomyomas are benign tumors of smooth muscle origin, which are more frequently found in genitourinary and gastrointestinal tracts. However, leiomyomas rarely arise in the oral cavity, and when it happens, the majority of them is originated by smooth muscle of blood vessels. AIMS: Report two cases of oral vascular leiomyoma with similar clinical characteristics, and discuss their aspects with the literature. CASE REPORT: Two cases of vascular leiomyoma in women that presented a nodular mass in the tongue ventral surface were described. The microscopic analysis revealed numerous blood vessels surrounded by spindle cells with ovoid or elongated nuclei. The two cases presented immunopositivity for smooth muscle actin, confirming the diagnosis. CONCLUSION: The study of vascular leiomyoma is required due to its rarity and similar aspects with other oral common lesions. The immunohistochemical analysis is a fundamental tool for the final diagnosis of leiomyoma.

OBJETIVO: O objetivo deste estudo foi avaliar a reparação óssea em ratos espontaneamente hipertensos (SHR) e compará-la com a de ratos normotensos, avaliando machos e fêmeas. MÉTODOS: Um defeito ósseo foi criado no fêmur esquerdo de 24 SHR (12 machos e 12 fêmeas) e 24 ratos normotensos (12 machos e 12 fêmeas). Os animais foram divididos em dois grupos com diferentes períodos de sacrifício: sete e 21 dias após o procedimento cirúrgico. Após processamento laboratorial de rotina, as análises histológica e histométrica foram realizadas e os dados submetidos à análise de variância (ANOVA) e ao teste de Tukey (5%). RESULTADOS: Machos e fêmeas do mesmo grupo apresentaram características histológicas similares. Após sete dias, todos os animais apresentaram trabéculas ósseas irregulares, entretanto, os osteoblastos periosteais eram achatados nos SHR, enquanto nos normotensos, essas células apresentavam formato cúbico. Após 21 dias, todos os espécimes mostraram fechamento linear em toda a extensão superficial do defeito ósseo e os SHR apresentaram osteoblastos achatados, enquanto os normotensos apresentaram células cúbicas circundando as trabéculas ósseas. A análise estatística dos dados histométricos indicou médias similares entre machos e fêmeas, exceto para ratos normotensos, aos sete dias. Além disso, aos 21 dias, foi observada maior neoformação óssea nos ratos hipertensos quando comparados aos normotensos, tanto machos quanto fêmeas. CONCLUSÃO: Concluiu-se que os animais SHR apresentaram maior neoformação e maturidade óssea nos defeitos que animais normotensos, no período de 21 dias após a cirurgia.OBJECTIVE: The aim of this study was to evaluate the bone healing in spontaneously hypertensive rats (SHR) and compare the results with normotensive rats, evaluating male and female animals. METHODS: A bone drill defect was created in the left femur of 24 SHR (12 males and 12 females) and 24 normotensive rats (12 males and 12 females). The animals were divided into two groups and sacrificed 7 and 21 days after the surgical procedure. After the routine laboratory processing, histological and histometric analysis were carried out and data were submitted to ANOVA and Tukey’s test (5%). RESULTS: Males and females from the same group had similar histological characteristics. After seven days, all animals presented irregular bone trabeculae. The periosteal osteoblasts were flattened in SHR, and presented a cuboid shape in normotensive animals. After 21 days, the bone defects of all specimens showed a linear closure in all the superficial extension. In addition, SHR presented flattened osteoblasts surrounding the bone trabeculae, while normotensive ones showed cuboidal cells. Statistical analysis of the histometric data indicated similar means between the male and female groups, except for normotensive rats on day 7. In addition, a larger amount of new bone formation was observed in hypertensive when compared to normotensive rats on day 21, in males as well as females. CONCLUSION: We conclude that bone healing in SHR was more significant than in normotensive ones, as shown by the histological and histometric evaluation 21 days after surgery.

Aim: To evaluate the influence of ovariectomy combined with lack of masticatory force in the evolution of periodontal disease induced in rats. Methods: Forty rats were bilaterally ovariectomized and 40 were submitted to sham ovariectomy. Periodontal disease was induced in the mandibular left first molar and the maxillary left first molar was extracted from half of the rats. The rats were randomly euthanized at 3, 7, 14 and 30 days post periodontal disease induction. Serial sections were obtained from the furcation area and stained for histological and histomorphometric analysis. The results of the histomorphometric analysis were statistically analyzed by ANOVA and Tukey tests. Results: The results demonstrated statistically significant differences in the percentage of bone tissue when the variables presence or absence of estrogen (p=0.020) and time of euthanasia (p=0.002) were evaluated. However, the extraction procedure did not significantly affect the percentage of bone tissue (p=0.598). Conclusions: The bone loss resulting from periodontal disease is increased by estrogen deficiency and varies according to the time course of periodontitis. In contrast, masticatory force does not seem to interfere in bone loss derived from periodontal disease.

Background. Calcified thrombi are a common finding, especially in the pelvic veins. There are generally multiple thrombi, and they are generally associated with vascular malformations. Design. Herein we report a rare case of a single labial phlebolith, not associated with any other vascular lesion. We aim to alert clinicians to the possibility of the occurrence of vascular thrombi in the mouth and to describe the clinical and histological characteristics of such lesions in order to simplify the diagnosis and treatment. Furthermore, we have reviewed the English-language literature published since 1970 reporting oral (including masticatory muscles) phleboliths. Results. Twenty-nine cases of phleboliths have been reported in the literature since 1970. Only three of the reported phleboliths were solitary and not associated with other vascular lesions, as in the case presented here. Conclusion. Although phleboliths not associated with other vascular lesions are not common, clinicians should be aware of the existence of this pathology and include it as differential diagnosis of oral lesions.

Spindle cell lipoma is a benign lipomatous neoplasm, which rarely occurs in the oral cavity. The aims of this paper are to report a case of spindle cell lipoma located in buccal mucosa and discuss the main clinical, histological, and immunohistochemical findings of this entity. Thus, we report a 4-year history of an asymptomatic smooth surface nodule in an elderly Caucasian man with clinical hypothesis of fibroma. The histopathological examination showed spindle cells, mature adipose tissue, and many mast cells in a stroma of connective tissue presenting ropey collagen fibers bundles. After immunohistochemical analysis, the final diagnosis was spindle cell lipoma.

The purpose of this study was to analyze the bone repair around commercially pure titanium implants with rough and porous surface, fabricated using powder metallurgy technique, after their insertion in tibiae of rabbits. Seven male rabbits were used. Each animal received 3 porous-surface implants in the left tibia and 3 rough-surface implants in the right tibia. The rabbits were sacrificed 4 weeks after surgery and fragments of the tibiae containing the implants were submitted to histological and histomorphometric analyses to evaluate new bone formation at the implant-bone interface. Means (%) of bone neoformation obtained in the histomorphometric analysis were compared by Student's t-test for paired samples at 5% significance level.. The results of the histological analysis showed that osseointegration occurred for both types of implants with similar quality of bone tissue. The histomorphometric analysis revealed means of new bone formation at implant-bone interface of 79.69 ± 1.00% and 65.05 ± 1.23% for the porous- and rough-surface implants, respectively. Statistically significant difference was observed between the two types of implants with respect to the amount new bone formation (pO propósito deste estudo foi avaliar a reparação óssea ao redor de implantes de superfície porosa comparados com implantes de superfície rugosa, ambos confeccionados de titânio puro grau 2 por meio da técnica de metalurgia do pó. Os implantes foram inseridos em tíbias de coelhos. Foram utilizados neste estudo 7 coelhos machos, sendo que cada um recebeu 3 implantes de superfície porosa na tíbia esquerda e 3 implantes de superfície rugosa na tíbia direita. Os animais foram sacrificados 4 semanas após a cirurgia e os fragmentos das tíbias contendo os implantes foram submetidos à análise histológica e histomorfométrica, visando analisar a neoformação óssea na interface osso-implante. As médias (%) obtidas na análise histomorfométrica foram avaliadas por meio do teste estatístico t-student de amostras pareadas com nível de significância de 5%. Os resultados da análise histológica mostraram que a osseointegração foi obtida nos dois tipos de implantes com similar qualidade de tecido ósseo. Na análise histomorfométrica, verificaram-se médias de neoformação óssea na interface osso-implante de 79,69% ± 1,00 e 65,05 ± 1,23 para os implantes de superfície porosa e rugosa, respectivamente, e foi observada diferença estatisticamente significante entre os dois tipos de implantes com relação à quantidade de neoformação óssea. Concluiu-se que os implantes de superfície porosa contribuíram para a osseointegração devido à sua maior superfície de contato na interface osso-implante.

INTRODUÇÃO: Nestes últimos anos, descobriu-se a complexidade dos mecanismos que influenciam a atividade óssea, e grande parte das pesquisas direcionou-se para o estudo de fatores capazes de modular as funções ósseas. Essa expansão da pesquisa deve-se, em parte, ao reconhecimento da osteoporose como importante problema na velhice. A osteoporose constitui uma das osteopatias mais comuns, caracterizando-se pela redução da massa óssea, determinada, por sua vez, pelo desequilíbrio entre reabsorção e neoformação. OBJETIVO: Apresentar uma revisão da literatura sobre os principais aspectos da remodelação e da reparação associados à deficiência estrogênica. Remodelação óssea: O osso apresenta processo contínuo de remodelação, entretanto anormalidades nesse processo ocorrem em algumas doenças, entre elas a osteoporose, sendo que a deficiência estrogênica parece ter o papel principal na sua gênese. Reparação óssea: Tal processo envolve uma cascata complexa de respostas biológicase, assim como a remodelação, é afetado por fatores locais e externos e regulado pela interação de diferentes mecanismos. Portanto, o aumento ou o decréscimo da capacidade de reparação óssea têm sido relacionados a alterações ocorridas na remodelação. Deficiência estrogênica e metabolismo ósseo: A maioria dos autores sugere uma redução na capacidade de remodelação e de reparação do tecido. DISCUSSÃO: Ainda não está determinado qual estágio da reparação é mais alterado, se a fase inicial de formação do calo ósseo, se a de mineralização ou se a fase tardia da reparação, a remodelação óssea. CONCLUSÃO: Como os mecanismos fisiológicos e a patogênese das alterações ósseas causadas pela deficiência estrogênica não estão completamente estabelecidos, novas pesquisas ainda são necessárias.INTRODUCTION: In the past few years, it was recognized the great complexity of the mechanisms that influence bone cellular activity and several studies focus on the factors able to modulate the bone functions. The increase of bone research is, in part, due to the establishment of osteoporosis as a healthy problem common in elderly. Osteoporosis is one of the most important osteopathy, characterized by the bone mass reduction, resulted from disequilibrium between bone resorption and bone formation. OBJECTIVE: Based on the relationship between estrogen and bone metabolism, the aim of this study is present a review of literature about the principal aspects of bone turnover and bone repair associated to estrogen deficiency. Bone turnover: Bone tissue is in continuous turnover, however, changes in this process can result in some disorders, such as osteoporosis. Bone repair: Involves a sequence of biological events. It is affected by local and external factors and regulated by interaction of several mechanisms, like bone turnover. Estrogen deficiency and bone metabolism: The capacity to repair has been associated to changes in bone turnover and repair. DISCUSSION: It is not known which bone repair stage is modified: the bone formation, the mineralization or the resorption stage. CONCLUSION: The pathophysiology of bone changes caused by estrogen deficiency are not completely clear, so, new studies are still necessary.

The main goal of this study was to histologically evaluate the healing of surgically created defects on the tibiae of adult male rats after implantation of two types of bioactive glass. Sixteen adult Wistar rats (body weight of 300g) were divided into two groups: PerioGlas (PG) (n=8) and BioGran (BG) (n=8). Unicortical bone defects with 3-mm diameter were performed in both tibiae of the animals and filled with two types of glass particles. The rats were then sacrificed at 7, 14, 30 and 60 days, and the tissues were prepared for histological processing, sectioning, and staining with hematoxylin and eosin, as well as Mallory trichrome, and analyzed under light microscope. Within 7-14 days, both groups presented connective tissue septa with new bone formation, more intense in the PG group. In the subsequent periods (30 and 60 days), both groups presented more mature bone tissue around the glass particles. Bone trabeculae formed in all experimental periods were juxtaposed to the glass particles. It can be concluded that both materials promoted comparable bone formation over the entire extension of the defect, independently of the size of the granules, thus confirming their biological osteoconductive property.O objetivo deste estudo foi avaliar a resposta histológica após a implantação de dois tipos de cerâmicas bioativas em defeitos criados na tíbia de ratos. Dezesseis ratos da raça Wistar, pesando 300g, foram divididos em dois grupos: PerioGlass (PG) (n=8) e Biogran (BG) (n=8). Defeitos ósseos unicorticais de 3 mm de diâmetro foram realizados na tíbia dos animais e preenchidos com os dois tipos de partículas cerâmicas. Os animais foram sacrificados aos 7, 14, 30 e 60 dias no pós-operatório. O material preparado para processamento e exame histológico em microscopia óptica foi corado com hematoxilina-eosina e tricrômio de Mallory. Aos 7 dias ambos os grupos apresentaram septos ósseos neoformados, mais intensos no grupo PG. Isto também foi observado aos 14 dias. Nos períodos posteriores (30 e 60 dias), ambos os grupos apresentaram tecido ósseo mais maduro ao redor das partículas cerâmicas. As trabéculas ósseas formadas em todos os períodos experimentais foram justapostas com as partículas. Podemos concluir que ambos os materiais promoveram o preenchimento ósseo em toda a extensão do defeito independente de suas granulações comprovando suas propriedades osteocondutivas.
