1. XAFS Analysis of Sulfuric Acid Solution Related to the Effect of Te and Sb in Suppressing the Disproportionation Reaction of Mn3+ in Mn/Ti Redox Flow Cells

Abstract

In sulfuric acid (H2SO4) aqueous solutions containing manganese (Mn) and titanium (Ti) ions, which were developed to be used for the positive electrolyte in redox flow (RF) batteries, the addition of tellurium (Te) or antimony (Sb) has been found very effective in suppressing the MnO2 precipitation in the charging process from Mn2+ to Mn3+. To investigate its mechanism, x-ray absorption near edge structure (XANES) measurements were carried out for cations in the solution samples at various state of charges (SOCs). In the case of the Te-added solutions, the change rate of Mn ion valence over the SOC was found to very low at the SOCs lower than 30 %. It was also indicated that the valence change of Te ions from Te4+ to Te6+ proceeded at the SOCs below 30 %. On the other hand, no significant change was observed in the valence of Ti at any SOCs. Similar trends were confirmed in the Sb-added solution samples. In conclusion, the supplied charges were considered to be used to increase not only the valence of Mn ions but also that of Te or Sb ions, which resulted in the suppression of the MnO2 precipitation.

Abstract

Polydimethylsiloxane (PDMS)-coated macroporous silica (MPSi) particles were developed for the extraction of volatile organic compounds (VOCs) from air samples in thermal desorption-gas chromatographic analysis. The gas sample was collected by a gas sampling pump at 100 mL/min. Different amounts of PDMS coating were prepared, and the fundamental extraction and desorption performances were investigated using even numbered n-alkanes of C8–C28. The extraction efficiency increased by increasing PDMS coating, and the desorption efficiency showed more than 99% at a desorption temperature of 310°C for all the investigated adsorbents. This remarkable desorption performance showed a higher peak area of the analytes than that of the Tenax TA-packed thermal desorption tube, especially for higher molecular weight compounds. Furthermore, an activated carbon particle of Carbopack X was introduced on the latter side of the PDMS-MPSi to extract low-molecular weight compounds. This double-bed-type extraction tube showed a successful determination of a wide range of VOCs. The double-bed-type extraction tube was then applied to the extraction of VOCs related to sick building syndrome (SBS). The results for the extraction and desorption performances of the SBS-related VOCs were well agreed with that of alkanes, and the applicability of the extraction tube to the determination of other types of conventional VOCs were confirmed.

Abstract

Abstract Aims/Introduction This study aimed to investigate the neurodevelopment of infants born to women with gestational diabetes mellitus (GDM). Materials and Methods Data from the National Birth Cohort in the Japan Environment and Children's Study from 2011 to 2014 (n = 81,705) were used. Japan uses the GDM guidelines of the International Association of Diabetes and Pregnancy Study Groups. The Japanese translation of the Ages and Stages Questionnaires, third Edition, was used to assess neurodevelopment in the following domains: communication skills, gross motor skills, fine motor skills, problem‐solving ability, and personal and social skills. The survey was carried out every 6 months from the age of 6 months to 4 years (total of eight times). Generalized estimating equations were used to evaluate the association between maternal GDM and neurodevelopmental delay based on odds ratios (ORs) and 95% confidence intervals (95% CIs). Results Neurodevelopmental delays, particularly in problem‐solving ability, fine motor skills, and personal and social skills, were significantly higher in infants born to women with GDM than in those born to women without GDM (adjusted OR 1.24, 95% CI 1.12–1.36; adjusted OR 1.15, 95% CI 1.03–1.27; and adjusted OR 1.18, 95% CI 1.04–1.33). Furthermore, stratification showed no significant increase in the adjusted ORs (95% CIs) of girls. Conclusions Neurodevelopment was significantly delayed up to 4 years‐of‐age among boys born to women with GDM.

Abstract

Abstract Aims/Introduction We investigated the association between gestational diabetes mellitus (GDM) and perinatal outcomes stratified by pre‐pregnancy body mass index (BMI) and/or gestational weight gain (GWG). Materials and Methods Data from the national birth cohort in the Japan Environment and Children's Study from 2011 to 2014 (n = 85,228) were used. Japan uses the GDM guidelines of the International Association of Diabetes and Pregnancy Study Groups. The odds ratios (ORs) of perinatal outcomes were compared between women with and those without GDM. Results The OR (95% confidence interval) of having a small for gestational age infant in the GDM group with a pre‐pregnancy BMI of ≥25.0 kg/m2 and insufficient GWG (7.25 kg) was 2.04 (1.56–2.67). The OR of hypertensive disorders of pregnancy was higher in women with a BMI ≥18.5 kg/m2 in the GDM group than in the non‐GDM group. Conclusions Large for gestational age and hypertensive disorders of pregnancy were associated with pre‐pregnancy BMI and GWG in either normal weight or overweight/obese women, and the relationship was strengthened when GDM was present. Women with GDM and a BMI of ≥25.0 kg/m2 are at risk of having small for gestational age and large for gestational age infants depending on GWG.

Abstract

In sulfuric acid (H2SO4) aqueous solutions containing manganese (Mn) and titanium (Ti) ions, which are used for the positive electrolyte in redox flow (RF) batteries, the addition of a small quantity of bismuth (Bi) has been found to be very effective in suppressing the MnO2 precipitation in the charging process from Mn2+ to Mn3+. The effect of the addition of Bi on the structural change in these solutions has been analyzed using x-ray absorption fine structure (XAFS) techniques. X-ray absorption near edge structure (XANES) spectra indicated that some Mn4+ ions were generated only when Bi was not added. It was also confirmed that the Mn-O octahedral coordination was maintained at a high state of charge (SOC). On the other hand, no change was observed in the valence of Ti and Bi ions even at high SOCs. Extended x-ray absorption fine structure (EXAFS) measurements combined with fitting analysis clarified that the addition of Bi resulted in the generation of Mn-O-Bi coordination at the SOCs of 70 and 90 %. Bi-O-Mn coordination may suppress the MnO2 precipitation at a high SOC.

Abstract

Summary: The gut microbiome has emerged as a key regulator of obesity; however, its role in brown adipose tissue (BAT) metabolism and association with obesity remain to be elucidated. We found that the levels of circulating branched-chain amino acids (BCAA) and their cognate α-ketoacids (BCKA) were significantly correlated with the body weight in humans and mice and that BCAA catabolic defects in BAT were associated with obesity in diet-induced obesity (DIO) mice. Pharmacological systemic enhancement of BCAA catabolic activity reduced plasma BCAA and BCKA levels and protected against obesity; these effects were reduced in BATectomized mice. DIO mice gavaged with Bacteroides dorei and Bacteroides vulgatus exhibited improved BAT BCAA catabolism and attenuated body weight gain, which were not observed in BATectomized DIO mice. Our data have highlighted a possible link between the gut microbiota and BAT BCAA catabolism and suggest that Bacteroides probiotics could be used for treating obesity.

Abstract

Objective: This study was performed to evaluate the application of fetal middle cerebral artery peak systolic velocity (MCA-PSV) for prediction of newborn anemia with umbilical cord blood hemoglobin concentration at birth (UCB-Hb) 1.5 was defined as a positive MCA-PSV test result. Results: The MCA-PSV test was applied 2309 times (313 and 1996 times during second and third trimesters, respectively) in 699 women. The results were positive in 4.4% (102/2309) of tests and at least once in 9.9% (69/699) of women. Anemic infants were born to one (1.4%) and six (1.0%) of 69 and 630 women with and without at least one positive test result, respectively. MoM determined 4, 3, and 2 weeks before birth showed significant weak negative correlations with UCB-Hb at birth (correlation coefficient: 0.298–0.325). Conclusions: Among unselected women giving birth at or near term, the MCA-PSV test was unsatisfactory for prediction of newborn anemia in this retrospective observational study. Abstract

A first-principles molecular dynamics simulation study was carried out to investigate the structural features of Ti4+ in aqueous sulfuric acid. Ti4+ forms several stoichiometric compounds in aqueous sulfuric acid, the identity and structural information details of which have been the subject of numerous studies. Three stoichiometric compounds of Ti4+ in aqueous sulfuric acid were found in the simulations, namely, [Ti=O(OH)2(H2O)3], [Ti(OH)2(H2O)2SO4], and [Ti2O2(H2O)5(OH)2SO4]. The results indicated that HSO4− formed a complex with the Ti ion by releasing its proton, which implies that such a coordination increases the proton concentration in the solution. The multinuclear complex was formed in our simulation conditions (concentration: 1M Ti4+ and temperature: 353 K); the result supports the interpretation of a previous experimental study.

13. Glycemic control and fetal growth of women with diabetes mellitus and subsequent hypertensive disorders of pregnancy.

Abstract

Pregnant women with diabetes mellitus (DM) are at high risk for hypertensive disorder of pregnancy (HDP). Women with poor control DM sometimes have heavy-for-dates infants. However, women with HDP sometimes have light-for-dates infants. We aim to clarify the relationship between glycemic control and fetal growth in women with DM and/or subsequent HDP. Of 7893 women gave singleton birth at or after 22 gestational weeks, we enrolled 154 women with type 1 DM (T1DM) or type 2 DM (T2DM) whose infants did not have fetal abnormalities. Among women with T1DM or T2DM, characteristics of the three groups (with HDP, without HDP, and with chronic hypertension [CH]) were compared. No women with T1DM had CH, but 19 (17.4%) of 109 with T2DM did. HDP incidence was similar between women with T1DM (22.2%) and T2DM without CH (16.7%). Among women with T1DM, the incidences of fetal growth restriction (FGR) with and without HDP were similar. However, among women with T2DM without CH, this incidence was significantly higher among those with HDP (33.3%) than among those without HDP (5.3%), was significantly more common with HbA1c levels at first trimester ≥ 7.2% (33.3%) than with those < 7.2% (5.6%), and significantly more numerous without pre-pregnancy therapies for DM (23.3%) than with them (3.3%). Among women with T2DM and HDP, those with FGR had smaller placenta SDs and higher insulin dosages at delivery than those without light-for-dates. In multivariate analysis, the presence of diabetic nephropathy was a predictor of T1DM and HDP (P = 0.0105), whereas HbA1c levels ≥ 7.2% before pregnancy was a predictor of T2DM and HDP (P = 0.0009). Insulin dosage ≥ 50U/day at delivery (P = 0.0297) and the presence of HDP (P = 0.0116) independently predicted T2DM, HDP, and FGR development. Insufficient pre-pregnancy treatment of DM increased the risk of HDP.

Abstract

Cardiovascular diseases (CVDs) have become a major health problem because of the associated high morbidity and mortality rates observed in affected patients. Gut microbiota has recently been implicated as a novel endocrine organ that plays critical roles in the regulation of cardiometabolic and renal functions of the host via the production of bioactive metabolites. This review investigated the evidence from several clinical and experimental studies that indicated an association between the gut microbiota-derived toxins and CVDs. We mainly focused on the pro-inflammatory gut microbiota-derived toxins, namely lipopolysaccharides, derived from Gram-negative bacteria, and trimethylamine N-oxide and described the present status of research in association with these toxins, including our previous research findings. Several clinical studies aimed at exploring the effectiveness of reducing the levels of these toxins to inhibit cardiovascular events are currently under investigation or in the planning stages. We believe that some of the methods discussed in this review to eliminate or reduce the levels of such toxins in the body could be clinically applied to prevent CVDs in the near future.

Abstract

Background: Although the concept of induction therapy followed by surgical resection for locally advanced non-small cell lung cancer (LA-NSCLC) has found general acceptance, the appropriate indications and the strategy for this treatment are still controversial. Methods: From 2000 through 2008, 36 patients received concurrent chemoradiotherapy followed by surgery. We retrospectively reviewed these cases, analyzed the outcomes and examined the prognosis. Results: The median radiation dose given was 60 Gy. Chemotherapy included a platinum agent in all cases; cisplatin-based chemotherapy was administered to 9 cases, and a carboplatin-based chemotherapy regimen was administered to 27. A complete resection was performed in 94% of the patients. Seventeen (47.2%) patients exhibited a complete pathological response, and downstaging was induced in 26 (72%) cases. The morbidity and 30-day mortality rates were 11.1 and 0%, respectively. The 5-year overall survival rate in the patients with complete resection (n = 33) was 83.3%. Conclusions: Induction chemoradiotherapy followed by surgery for LA-NSCLC provided a favorable prognosis for selected patients. A complete pathological response was found in about half of cases. This strategy is feasible and was associated with low morbidity and high resectability rates, suggesting that it contributed to improving the treatment results.

Abstract

Glasses with the composition xBaO–(99.9 − x)SiO2–0.1ErO3/2 (0 ≤x ≤ 34.9) were fabricated by a levitation technique. The glasses in the immiscibility region were opaque due to chemical inhomogeneity, while the other glasses were colorless and transparent. The scanning electron microscope observations and electron probe microanalysis scan profiles revealed that more Er3+ ions were preferentially distributed in the regions where more Ba2+ ions existed in the chemically inhomogeneous glasses. The synchronicity of the spatial distributions of the two ions initially increased with increasing x and then decreased when the Ba2+ concentration exceeded a certain value. The peak shape and lifetime of the fluorescence at 1.55 μm depended on x as well as the spatial distribution of both ions. These results indicate that although ErOn polyhedra are preferentially coordinated with Ba2+ ions and their local structure is affected by the coordination of Ba2+, there is a maximum in the amount of Ba2+ ions that can coordinate ErOn polyhedra since the available space for Ba2+ ions is limited. These findings provide us with efficient ways to design the chemical composition of glasses with superior Er3+ fluorescence properties for optical communication network systems.

Novel chitosan-based adsorbent materials were synthesized using a trifunctional reagent as a crosslinker and the adsorption capability of the resulting polymers towards two acid dyes and several metal ions was evaluated. The crosslinked chitosan with the lowest degree of substitution showed a very high adsorption capability towards CI Acid Orange 7. Decreasing the degree of substitution and the pH of the dye solution led to an increase in the adsorption capability. The crosslinked chitosan with the lowest degree of substitution exhibited quite remarkable adsorption powers towards the Cu 2+ ion. Such adsorption towards divalent metal ions decreased in the following sequence: Cu 2+ » Ni 2+ > Cd 2+ , Pb 2+ , Ca 2+ .

Water-soluble chitin derivatives have been synthesized by the diethylaminoethylation (DEAE) reaction and the adsorption characteristics of the resulting polymeric adsorbents towards two acid dyes (CI Acid Orange 7 and CI Acid Red 88) were studied. The adsorption behaviours towards these dyes were also compared with those exhibited by two water-insoluble commercially available derivatives. The conventional water-insoluble DEAE-chitins showed a high adsorptive capacity towards CI Acid Orange 7 compared to that on untreated chitin and alkali-treated chitin. A similar trend was also found for CI Acid Red 88 dye, where the increase in adsorption was characterized more clearly. However, the adsorption capacities towards both dyes exhibited by the water-soluble DEAE-chitins were increased significantly at a particular free dye concentration, with the adsorptive power also being increased both with the degree of DEAE substitution and the hydrophobicity of the dyes.

Abstract

Background The AT(N) classification was proposed for categorising individuals according to biomarkers. However, AT(N) profiles may vary depending on the markers chosen and the target population.Methods We stratified 177 individuals who participated in the Japanese Alzheimer’s Disease Neuroimaging Initiative by AT(N) classification according to cerebrospinal fluid (CSF) biomarkers. We compared the frequency of AT(N) profiles between the classification using total tau and neurofilament light chain (NfL) as N markers (AT(N)tau and AT(N)NfL). Baseline characteristics, and longitudinal biological and clinical changes were examined between AT(N) profiles.Results We found that 9% of cognitively unimpaired subjects, 49% of subjects with mild cognitive impairment, and 61% of patients with Alzheimer’s disease (AD) dementia had the biological AD profile (ie, A+T+) in the cohort. The frequency of AT(N) profiles substantially differed between the AT(N)tau and AT(N)NfL classifications. When we used t-tau as the N marker (AT(N)tau), those who had T− were more frequently assigned to (N)−, whereas those who had T+were more frequently assigned to (N)+ than when we used NfL as the N marker (AT(N)NfL). During a follow-up, the AD continuum group progressed clinically and biologically compared with the normal biomarker group in both the AT(N)tau and AT(N)NfL classifications. More frequent conversion to dementia was observed in the non-AD pathological change group in the AT(N)tau classification, but not in the AT(N)NfL classification.Conclusions AT(N)tau and AT(N)NfL in CSF may capture different aspects of neurodegeneration and provide a different prognostic value. The AT(N) classification aids in understanding the AD continuum biology in various populations.

Abstract

Glasses with the composition xBaO-(99.9-x)SiO2-0.1ErO3/2 (0 ≤ x ≤ 34.9) were fabricated by a levitation technique. The glasses in the immiscibility region were opaque due to chemical inhomogeneity, while the other glasses were colorless and transparent. The scanning electron microscope observations and electron probe microanalysis scan profiles revealed that more Er3+ ions were preferentially distributed in the regions where more Ba2+ ions existed in the chemically inhomogeneous glasses. The synchronicity of the spatial distributions of the two ions initially increased with increasing x and then decreased when the Ba2+ concentration exceeded a certain value. The peak shape and lifetime of the fluorescence at 1.55 µm depended on x as well as the spatial distribution of both ions. These results indicate that although ErOn polyhedra are preferentially coordinated with Ba2+ ions and their local structure is affected by the coordination of Ba2+, there is a maximum in the amount of Ba2+ ions that can coordinate ErOn polyhedra since the available space for Ba2+ ions is limited. These findings provide us with efficient ways to design the chemical composition of glasses with superior Er3+ fluorescence properties for optical communication network systems. Abstract

Abstract  We report a novel method for forming reverse worm-like micelles in nonpolar organic solvents. This method requires the addition of trace amounts of a sucrose fatty acid ester (SFE), in addition to lecithin and a nonpolar organic solvent. The region in which these micelles formed increased with lecithin concentration and hydrophobicity of SFE. In addition, zero-shear viscosity (η 0) of the reverse worm-like micelles increased rapidly upon addition of SFE, reaching 1–3 million times the viscosity of n-decane. Furthermore, the change in η 0 was examined in detail by performing dynamic viscoelasticity measurements. Results showed that the η 0 of solution increased upon addition of SFE because both the length and number of reverse worm-like micelles increased along with SFE concentration. [ABSTRACT FROM AUTHOR]

Abstract

We describe the preparation of inclusion complexes composed of amylose and strongly hydrophobic polyesters in a parallel enzymatic polymerization system. This was achieved by carrying out phosphorylase-catalyzed enzymatic polymerization of α- d-glucose 1-phosphate from maltoheptaose, giving rise to amylose as a host, and lipase-catalyzed enzymatic polymerization of dicarboxylic acids and diols, leading to the guest polyesters, simultaneously. When the numbers of methylene units in a dicarboxylic acid and a diol as the monomers for the guest polyester were 8, the corresponding amylose−polyester inclusion complex was obtained. Abstract

Abstract Purpose  To evaluate visual outcomes after a removal of an epiretinal membrane (ERM) secondary to rhegmatogenous retinal detachment (RRD), proliferative vitreoretinopathy (PVR), or proliferative diabetic retinopathy (PDR). Methods  The medical charts of 51 consecutive patients who underwent PPV to remove an ERM were reviewed in an institutional setting. The preoperative best-corrected visual acuity (BCVA) was evaluated to determine if it was a possible predictor of the postoperative BCVA. The visual outcomes in the RRD, PVR, and PDR groups were compared. Follow-up periods ranged from 13 to 90 months (mean, 40.9 months). Results  After the ERM was removed, the postoperative BCVA improved significantly by 0.471 logarithm of the minimum angle of resolution units for all groups (t = 8.99; P P = 0.0082 and P = 0.035, respectively). Conclusion  The BCVA improves after removal of an ERM following PPV for eyes with RRD, PVR, or PDR. Patient age and preoperative BCVA correlated with the visual improvement after the ERM removal. Abstract

Introducing an additional functionality to chitosan, novel chitosan‐based materials were synthesized. The reactions of tosylchitosan with ammonia, ethylenediamine, and hexamethylenediamine have been studied, and the chemisorption behavior of a typical acid dye (CI Acid Orange 7) onto the resulting polymers has been evaluated. Thiol groups were introduced to the above chitosan derivatives having additional amino group at the C6 position, and the chemisorption behavior of Hg2+ ion onto the resulting polymers was measured and compared with that of N‐thioacetylchitosan. The successful synthetic reaction was demonstrated by the infrared spectroscopic measurements. It has been confirmed that the abilities of the chitosan derivatives with additional amino groups to the chemisorption of an acid dye were significantly high, and that the saturation value of the chemisorption amount of CI Acid Orange 7 toward one of 2′‐aminoethylaminodeoxychitosans was significantly high as 6.16 mol/kg. The results also showed that the saturation amounts of Hg2+ ion chemisorption were larger with the increase in the thiol group content of the chitosan derivatives with the thiol groups. © 2007 Wiley Periodicals, Inc. Abstract

Novel chitin‐based materials were synthesized and the adsorption behavior of typical acid dyes to the resulting polymers has been evaluated. The successful synthetic reaction was confirmed by the infrared spectroscopic measurements. By the reaction of alkalichitin with tosyl chloride, the corresponding tosylchitin was synthesized as a soluble and reactive precursor. Next, aminodeoxychitin was synthesized by the reaction of the tosylchitin with ammonia. Similarly, 2′‐aminoethylamino‐ and 6′‐aminohexylaminodeoxychitins were synthesized by reacting the tosylchitin with ethylenediamine and hexamethylenediamine, respectively. It has been confirmed that the adsorption abilities of these polymers for acid dyes were significantly high, and also that one of the aminodeoxychitins showed a high adsorption ability for Cu2 ion. On the basis of the adsorption data of acid dye on the aminodeoxychitin, the tosylchitin and the original chitin, the amino group contents at C6 and C2 positions were estimated. The aminodeoxychitin and aminoalkylaminodeoxychitins could be applicable to the adsorbent for anionic dyes and Cu2 ion along with a precursor for further novel derivatives. © 2007 Wiley Periodicals, Inc. Abstract

The high-temperature gas chromatographic (GC) separation of several semivolatile compounds is studied with a short metal capillary column packed with fibrous material, having a polydimethylsiloxane coating thereon. Taking advantage of the excellent heat-resistance of the fiber and also the combination of the surface-deactivated metal capillary, a temperature-programmed separation up to 450°C is successfully demonstrated for the separation of polymer standard samples. The average molecular weight of the commercially-available polymer standard samples for size exclusion chromatography (SEC) is estimated by high-temperature GC analysis and compared with the nominal value determined by a conventional SEC method. Although a slight deviation for the number-average molecular weight is observed between the GC and SEC analysis, the data for the weight-average molecular weight shows a good agreement in these methods. The results also suggest the future possibility of the fiber-packed metal capillary as a miniaturized GC column with an increased sample loading capacity. [ABSTRACT FROM AUTHOR]

Abstract Modified static head-space gas chromatography method (M-SHSGC method) was used to determine the stability constants of benzene and alkylbenzenes with 2-hydroxypropyl-β-cyclodextrin inclusion complexes. The 1:1 stability constants were in agreement with those obtained by conventional SHSGC method. The M-SHSGC method precludes the necessity of the calibration curve by the use of Henry’s law constant of guest. Abstract

BackgroundTo evaluate the efficacy of long-term gas tamponade after pars plana vitrectomy (PPV) for retinal detachment (RD) due to a macular hole (MH) in highly myopic eyes.MethodsA retrospective study was conducted at two institutions. The study included 25 highly myopic eyes with RD resulting from a MH. C 3F 8 gas tamponade was used in 13 eyes (C 3F 8 gas group), and SF 6 gas tamponade was used in 12 eyes (SF 6 gas group) following PPV. C 3F 8 gas has a longer half-life time in the vitreous cavity than SF 6 gas. The main outcome measures were the anatomic reattachment rate and the visual acuity (VA). Follow-up periods were at least 24 months.ResultsThe anatomic reattachment rate and MH closure following the initial surgery were significantly higher in the C 3F 8 gas group (100%) than in the SF 6 gas group (66.7%; P=0.039). The differences in the postoperative VAs were not significant both for all eyes and only for eyes with initial anatomic success. ( P=0.42 and P=0.54, respectively). In addition, no significant difference was found in the improvement of VA both for all eyes and for the initial success eyes ( P=0.66 and P=0.56, respectively).ConclusionWe conclude that the longer duration of tamponade with C 3F 8 gas results in a higher rate of successful closure and that C 3F 8 gas tamponade is an effective treatment for MH with RD in highly myopic eyes. Abstract

Purpose To elucidate the management and outcomes of patients with extranodal natural killer/T-cell lymphoma, nasal type (ENKL), who were diagnosed between 2000 and 2013 in Japan. Patients and Methods Data from 358 patients with ENKL diagnosed between 2000 and 2013 from 31 institutes were retrospectively analyzed. Results Patients' median age was 58 years, and 257 (72%) had localized disease. The most common first-line treatment was radiotherapy with dexamethasone, etoposide, ifosfamide, and carboplatin (RT-DeVIC) (66%) for localized ENKL and L-asparaginase-containing chemotherapy (30%) for advanced ENKL. With a median follow-up of 5.8 years, overall survival (OS) rates at 5 years for localized and advanced ENKL were 68% and 24%, respectively. The prognostic index of natural killer lymphoma was validated in our study, although only 4% of patients with localized ENKL were classified as high risk. With a median follow-up of 5.6 years, OS and progression-free survival at 5 years in the 150 patients who received RT-DeVIC in clinical practice were 72% (95% CI, 63% to 78%) and 61% (95% CI, 52% to 69%), respectively. Toxicities of RT-DeVIC were comparable to those in a previous trial. Multivariate analysis in patients with localized ENKL who received RT-DeVIC identified elevated soluble interleukin-2 receptor as an independent predictive factor for worse OS and progression-free survival (adjusted hazard ratios, 2.28 and 2.46; 95% CI, 1.24 to 4.23 and 1.42 to 4.28; P = .008 and .0014, respectively). Conclusion Favorable OS in response to new treatments was demonstrated in a large number of patients. Improved treatment approaches are needed for localized ENKL exhibiting elevated pretreatment soluble interleukin-2 receptor.
