Probiotics are widely recognized for their health benefits, but their viability during food processing and digestion poses significant challenges. The present study evaluated the impact of incorporating apple fibers into yogurt on the viability of the probiotic strain Lacticaseibacillus casei ATCC 393 during production, storage, and simulated gastrointestinal digestion. Apple fibers, a by-product of apple processing, were used as a prebiotic ingredient due to their functional and technological benefits. The incorporation of apple fibers increased probiotic viability during 28 days of refrigerated storage, improving it from 90.4% in the control yogurt to 93.9%. Under simulated gastrointestinal conditions, yogurt alone acted as a protective matrix, preserving probiotic viability, during gastric (71.0% at pH 2 after 3 h) and intestinal digestion (73.3% at 0.3% bile salts after 6 h). The inclusion of apple fibers further enhanced this protection, reducing probiotic viability loss in both gastric (81.9% at pH 2 after 3 h) and intestinal (79.0% at 0.3% bile salts after 6 h) environments. Similar results were obtained using the INFOGEST 2.0 static protocol. After the completion of the protocol (oral, gastric and intestinal phase) a viability of 71.1% (6.61 logCFU/g) was observed in the yogurt with apple fibers compared to 64.5% (6.10 logCFU/g) in the control yogurt. This enhanced protection could be attributed to the potential prebiotic properties of apple fibers, including their pectin and cellulose content, which may shield probiotics from acidic and enzymatic degradation. These findings highlight the potential of apple fiber-enriched yogurt as a functional food that supports probiotic viability during storage and throughout gastrointestinal transit. Background: Bile acid reflux stands out as a notable risk factor for dyspepsia. Among patients with bile acid reflux, prevalent endoscopic observations encompass mucosal erythema, the detection of bile acid on the mucosa, erosion, hyperugosity, and gastric mucosal atrophy. This study aimed to elucidate the association between bile acid levels in gastric fluid and the degree of gastric mucosal damage. Methods: A cross-sectional investigation involved dyspeptic patients who underwent endoscopy at Prof. Dr. I.G.N.G. Ngoerah Denpasar Hospital. Bile acid analysis was conducted through the enzymatic calorimetric method, while assessment of mucosal damage relied on the Lanza score, evaluated independently by two observers. Mucosal damage severity was categorized as either mild (score 0-2) or severe (score 3-5). Cohen’s kappa analysis, bivariate, and logistic regression analysis was empoyed. Results: Among the 99 subjects involved, 58.6% were male. H. pylori antibodies were detected in 21% of the participants, while 48% exhibited a pH below 2.77.Additionally, 48% reported a history of NSAID consumption. The mean bile acid level in gastric fluid was 156.07 μmol/L, with a median of 170.09 μmol/L (categorized as high if ≥170.09 μmol/L, and low if <170.09 μmol/L). During endoscopic examination, mild mucosal damage was observed in 61%, and severe damage in 39% (kappa 1, P<0.001). A statistically significant relationship between gastric fluid bile acid levels and mucosal damage was evident (P<0.05). Respiratory distress syndrome (RDS) is a major morbidity of prematurity. In this case–control study, we prospectively evaluated whether untargeted metabolomic analysis (gas chromatography–mass spectrometry) of the gastric fluid could predict the need for surfactant in very preterm neonates. 43 infants with RDS necessitating surfactant (cases) were compared with 30 infants who were not treated with surfactant (controls). Perinatal–neonatal characteristics were recorded. Significant differences in gastric fluid metabolites (L-proline, L-glycine, L-threonine, acetyl-L-serine) were observed between groups, but none could solely predict surfactant administration with high accuracy. Univariate analysis revealed significant predictors of surfactant administration involving gastric fluid metabolites (L-glycine, acetyl-L-serine) and clinical parameters (gestational age, Apgar scores, intubation in the delivery room). Multivariable models were constructed for significant clinical variables as well as for the combination of clinical variables and gastric fluid metabolites. The AUC value of the first model was 0.69 (95% CI 0.57–0.81) and of the second, 0.76 (95% CI 0.64–0.86), in which acetyl-L-serine and intubation in the delivery room were found to be significant predictors of surfactant therapy. This investigation adds to the current knowledge of biomarkers in preterm neonates with RDS, but further research is required to assess the predictive value of gastric fluid metabolomics in this field. Background: Ureaplasma spp. is an endemic microorganism that causes placental chorioamnionitis or preterm delivery in pregnant women, and the occurrence of bronchopulmonary dysplasia or intraventricular hemorrhaging in preterm infants after birth, although the pathogenicity of Ureaplasma remains controversial. The association between Ureaplasma exposure and the symptoms or outcomes of infected mothers or their infants born at term remains poorly understood. We investigated the clinical characteristics of preterm and term infants with or without Ureaplasma in their gastric fluid. Methods: Gastric fluid samples were collected from 47 newborns in the neonatal intensive-care unit immediately after birth and tested using multiplex polymerase chain reaction (PCR) assays targeting Ureaplasma spp., Ureaplasma parvum, and Ureaplasma urealyticum. The clinical findings and outcomes of the neonates and their mothers were retrospectively evaluated. Results: Ureaplasma spp. were detected in 9/47 samples (19%) by multiplex PCR assays. In all cases, the subspecies was U. parvum. The Ureaplasma-positive group had a significantly higher incidence of chorioamnionitis in utero than the Ureaplasma-negative group. Regarding preterm infants, the IgM levels in the Ureaplasma-positive group were significantly higher than in the Ureaplasma-negative group. In contrast, in term infants, the rates of a non-reassuring fetal status, a maternal fever, and maternal leukocyte counts and maternal C-reactive protein levels within five days before delivery in the Ureaplasma-positive group were significantly higher than those in the Ureaplasma-negative group. All three extremely-low-birth-weight infants with Ureaplasma developed bronchopulmonary dysplasia. The length of hospitalization in the Ureaplasma-positive group was almost same as that in the Ureaplasma-negative group for term infants. Conclusion: Mothers or their fetuses with exposure to Ureaplasma expressed characteristic clinical features during pregnancy and after birth.

Identifying the concentration of alcohol compounds in postmortem analysis of biological fluids can help the investigation, just as postmortem analysis of gastric juices can reveal the concentrations of alcohol consumed. However, an efficient and effective combination of extraction and measurement methods is required when looking at complex postmortem samples. Therefore, a headspace-solid phase microextraction (HS-SPME) method was developed using gas chromatography with a flame ionization detector (GC-FID) to identify postmortem alcohol concentrations. This present study optimizes and validates an effective and efficient method for postmortem alcohol extraction and quantification in gastric fluid. The optimal conditions for HS-SPME extraction using 65μm Polydimethy lsiloxane Divinyl benzene (PDMS/DVB) fiber for analyte isolation were 15 minutes. at 60°C and an exposure time of 1 minute. The validation investigation shows that the suggested approach satisfies the criteria for linearity, precision, accuracy, LOD, and LOQ for postmortem measurement of alcohol in gastric fluid.

To compare the frequency of Ureaplasma-positive gastric fluid (GF) cultures based on the cause and mode of delivery in preterm newborns. This retrospective cohort study included women with a singleton pregnancy who delivered prematurely (between 23+0 and 32+0 weeks of gestation, n=464) at a single university hospital in South Korea. The newborns' GF was obtained on the day of birth via nasogastric intubation. The frequency of Ureaplasma spp. in GF cultures was measured and compared according to the cause and mode of delivery. Ureaplasma spp. was detected in 20.3 % of the GF samples. The presence of Ureaplasma spp. was significantly higher in the spontaneous preterm birth group than in the indicated preterm birth group (30.2 vs. 3.0 %; p<0.001). Additionally, Ureaplasma spp. was more frequently found in the vaginal delivery group than in the cesarean delivery group, irrespective of the cause of preterm delivery [indicated preterm birth group (22.2 vs. 1.9 %, p=0.023); spontaneous preterm birth group (37.7 vs. 24.2 %, p=0.015)]. Ureaplasma spp. were found in 20.3 % of the GFs. However, only 1.9 % of newborns in the indicated preterm birth group with cesarean delivery had a Ureaplasma-positive GF culture. Predicting survival in very preterm infants is critical in clinical medicine and parent counseling. In this prospective cohort study involving 96 very preterm infants, we evaluated whether the metabolomic analysis of gastric fluid and urine samples obtained shortly after birth could predict survival in the first 3 and 15 days of life (DOL), as well as overall survival up to hospital discharge. Gas chromatography–mass spectrometry (GC-MS) profiling was used. Uni- and multivariate statistical analyses were conducted to evaluate significant metabolites and their prognostic value. Differences in several metabolites were identified between survivors and non-survivors at the time points of the study. Binary logistic regression showed that certain metabolites in gastric fluid, including arabitol, and succinic, erythronic and threonic acids, were associated with 15 DOL and overall survival. Gastric glyceric acid was also associated with 15 DOL survival. Urine glyceric acid could predict survival in the first 3 DOL and overall survival. In conclusion, non-surviving preterm infants exhibited a different metabolic profile compared with survivors, demonstrating significant discrimination with the use of GC-MS-based gastric fluid and urine analyses. Cannabidiol (CBD) products have ascribed an uprising trend for their health-promoting effects worldwide. In contrast to Δ9-tetrahydrocannabinol (THC), CBD exhibits no state of euphoria. Since conversion of CBD into THC in an acidic environment has been reported, it has not been proved whether this degradation will also occur in human gastric fluid. A total of 9 subjects ingested 400 mg CBD as a water-soluble liquid together with lecithin as an emulsifier and ethanol as a solubilizer. Blood samples were taken up to 4 h, and urine samples were submitted up to 48 h. THC, 11-hydroxy-Δ9-THC (THC-OH), 11-nor-9-carboxy-Δ9-THC (THC-COOH), CBD, 7-hydroxy cannabidiol (7-OH-CBD), and 7-carboxy cannabidiol (7-CBD-COOH) were determined in blood and THC-COOH and 7-CBD-COOH in urine by LC–MS/MS. Neither THC, THC-OH, nor THC-COOH were detectable in any serum specimen. Only two urine samples revealed THC-COOH values slightly above the threshold of 10 ng/ml, which could also be caused by trace amounts of THC being present in the CBD liquid. It can be concluded that negative consequences for participants of a drug testing program due to a conversion of CBD into THC in human gastric fluid appear unlikely, especially considering a single intake of dosages of less than 400 mg. Nevertheless, there is a reasonable risk for consumers of CBD products being tested positive for THC or THC metabolites. However, this is probably not caused by CBD cyclization into THC in human gastric fluid but is most likely due to THC being present as an impurity of CBD products. [ABSTRACT FROM AUTHOR]

Respiratory distress is common in neonates admitted to neonatal intensive care units. Additionally, infectious diseases such as intrauterine infections or vertical transmission are important underlying causes of respiratory failure. However, pathogens often cannot be identified in neonates, and there are many cases in which antibacterial drugs are empirically administered. Next-generation sequencing (NGS) is advantageous in that it can detect trace amounts of bacteria that cannot be detected by culturing or bacteria that are difficult to cultivate. However, there are few reports on the diagnosis of infectious diseases using NGS in the neonatal field, especially those targeting respiratory distress. The purpose of our study was to investigate the microorganisms associated with neonatal respiratory distress and to determine whether less invasive collection specimens such as plasma and gastric fluid are useful. Neonates were prospectively recruited between January and August 2020 from Nagoya University Hospital. The inclusion criteria were as follows: 1) admission to the neonatal intensive care unit; 2) respiratory distress presentation within 48 h of birth; and 3) suspected infection, collection of blood culture, and administration of antibiotics. Plasma samples and blood cultures were simultaneously collected. Gastric fluid samples were also collected if the patient was not started on enteral nutrition. Information on the patients and their mothers were collected from the medical records. DNA was extracted from 140 µL of plasma and gastric fluid samples. DNA sequencing libraries were prepared, and their quality was analyzed. DNA libraries were sequenced using high-throughput NGS. The NGS data of plasma and gastric fluid samples were analyzed using the metagenomic pipeline PATHDET, which calculated the number of reads assigned to microorganisms and their relative abundance. Putative pathogens were listed. Overall, 30 plasma samples and 25 gastric fluid samples from 30 neonates were analyzed. Microorganism-derived reads of gastric fluid samples were significantly higher than those of plasma samples. Transient tachypnea of the newborn was the most common cause of respiratory distress with 13 cases (43%), followed by respiratory distress syndrome with 7 cases (23%). There were 8 cases (29%) of chorioamnionitis and 7 cases (25%) of funisitis pathologically diagnosed. All blood cultures were negative, and only two gastric fluid cultures were positive for group B Streptococcus (Patient 15) and Candida albicans (Patient 24). Putative pathogens that met the positive criteria for PATHET were detected in four gastric fluid samples, one of which was group B Streptococcus from Patient 15. In the gastric fluid sample of Patient 24, Candida albicans were detected by NGS but did not meet the positive criteria for PATHDET. Cluster analysis of the plasma samples divided them into two study groups, and the indicator genera of each cluster (Phormidium or Toxoplasma) are shown in Figure 1. Clinical findings did not show any significant differences between the two groups. The incidence rate of chorioamnionitis was significantly higher in Ureaplasma group than in the other two groups. To the best of our knowledge, this was the first study to reveal that the presence of Ureaplasma in the gastric fluid of neonates with respiratory distress was associated with chorioamnionitis. The early diagnosis of intra-amniotic infections using gastric fluid and its treatment may change the treatment strategy for neonatal respiratory distress. Screening for Ureaplasma in neonates with respiratory distress may reduce the need for empirical antibiotic administration. Further research is required to confirm these findings. Genotypic methods for detecting antibiotic resistance in Helicobacter pylori infection offer a rapid, convenient, and accurate approach for tailored therapy. However, existing studies predominantly examine single sample types and lack comparative analyses across different samples. This study comprehensively detects and compares genotypic resistance to clarithromycin and levofloxacin in gastric mucosa, gastric fluid, and fecal samples from the same patients. The study enrolled 183 participants, comprising 124 H . pylori -positive and 59 H . pylori -negative patients. All participants provided fecal samples and underwent gastroscopy for the collection of gastric mucosa and gastric fluid. Real-time PCR was employed to detect genotypic resistance to clarithromycin and levofloxacin in conjunction with bacterial culture and antibiotic susceptibility testing. Genotypic resistance detection rates for clarithromycin were 100% in gastric mucosa, 99.2% in gastric fluid, and 79.8% in fecal samples. For levofloxacin, detection rates were 97.6%, 96.8%, and 72.6%, respectively. The results showed that PCR detection for clarithromycin exhibited high sensitivity (0.94-0.95) and specificity (0.88-0.89) across all sample types. However, PCR detection for levofloxacin demonstrated slightly lower sensitivity (0.79-0.89) and specificity (0.79-0.83). The comparison of genotypic resistance results by PCR among the three sample types showed that gastric mucosa and gastric juice exhibited higher consistency, while the consistency between feces and both gastric mucosa and gastric juice was lower. This study confirmed good consistency between genotypic and phenotypic resistance in clarithromycin and levofloxacin. While both gastric mucosa and gastric fluid samples demonstrated high detection performance, the efficiency of detecting fecal samples was constrained by challenges in DNA extraction., Importance: This study, with a large sample size, comprehensively tested both Helicobacter pylori-negative and -positive patients, including rapid urease test, histopathological evaluation and staining, bacterial culture, susceptibility testing, and resistance gene mutation analysis. By simultaneously examining gastric mucosa, gastric juice, and fecal samples from the same individuals, we minimized confounding factors arising from different sample sources, ensuring the reliability of our results. This approach effectively delineated the differences and characteristics in detection performance among different sample types, offering crucial reference data for selecting appropriate detection samples and identifying areas for improvement. The findings revealed robust concordance between genotypic and phenotypic resistance, with both gastric mucosa and gastric juice samples demonstrating excellent detection performance. Abstract Background In traditional Chinese medicine, it is believed that the “tongue coating is produced by fumigation of stomach gas”, and that tongue coating can reflect the health status of humans, especially stomach health. Therefore, studying the relationship between the microbiome of the tongue coating and the gastric fluid is of great significance for understanding the biological basis of tongue diagnosis. Methods This paper detected the microbiomes of the tongue coating and the gastric fluid in 35 gastritis patients using metagenomic sequencing technology, systematically constructed the microbial atlas of tongue coating and gastric juice, and first described the similar characteristics between the two sites. Results There was a significant correlation between tongue coating and gastric juice in terms of microbial species composition and overall diversity. In terms of species composition, it was found that the two sites were dominated by five phyla, namely, Actinobacteria, Bacteroidetes, Firmicutes, Fusobacteria and Proteobacteria, and that most of the gastric microbial species could be detected from the patient's own tongue coating. In terms of overall diversity, a significant correlation was found between the alpha diversity of the tongue coating microbiome and the gastric juice microbiome. Furthermore, in terms of abundance, 4 classes, 2 orders, 4 families, 18 genera and 46 species were found to significantly correlate between the tongue coating and the gastric fluid. Conclusions The results provide microbiome-based scientific evidence for tongue diagnosis, and offer a new perspective for understanding the biological basis of tongue diagnosis.

The progression of gastric cancer involves a complex multi-stage process, with gastroscopy and biopsy being the standard procedures for diagnosing gastric diseases. This study introduces an innovative non-invasive approach to differentiate gastric disease stage using gastric fluid samples through machine-learning-assisted surface-enhanced Raman spectroscopy (SERS). This method effectively identifies different stages of gastric lesions. The XGBoost algorithm demonstrates the highest accuracy of 96.88% and 91.67%, respectively, in distinguishing chronic non-atrophic gastritis from intestinal metaplasia and different subtypes of gastritis (mild, moderate, and severe). Through blinded testing validation, the model can achieve more than 80% accuracy. Magnetic resonance imaging (MRI) has been used in the past to quantify these fluid volumes in adults and its use is now being extended to the pediatric population. The present research pursued a retrospective, explorative analysis of existing clinical MRI data generated for pediatric patients. Images of 140 children from all pediatric subpopulations were analyzed for their resting GI fluid volumes in fasting conditions. In general, an increase in fluid volume as a function of age was observed for the stomach, duodenum, jejunum, and small intestine (SI) as a whole. No specific pattern was observed for the ileum and colon. Body mass index (BMI), body weight, body height, and SI length were evaluated as easy-to-measure clinical estimators of the gastric and SI fluid volumes. Although weight and height were identified as the best estimators, none performed ideally based on the coefficient of determination (R2). Data generated in this study can be used as physiologically relevant input for biorelevant in vitro tests and in silico models tailored to the pediatric population, thereby contributing to the efficient development of successful oral drug products for children. Microplastics and endocrine disrupting chemicals are emerging pollutants in the marine environment because of their potential hazards. The effect of gastric fluid on the adsorption and desorption of 17β-estradiol (E2) and 17α-ethynylestradiol (EE2) to microplastics was investigated. The adsorption and desorption isotherms of E2/EE2 on microplastics could be well fitted by the Freundlich model while the Gibbs free energy of these processes were negative, proving that the reaction occurred spontaneously on the heterogeneous surface of the microplastics. Desorption ratios of EE2 (55%–59%) on PVC were larger than that of E2 (49%–55%) to indicate that EE2 was less stable in gastric fluid, which could be explained by the fact that the hydrophobicity of EE2 was greater than E2. E2/EE2 were more easily desorbed from PVC in the gastric fluid and the desorption amount (5.25–12.91/7.19–17.86 µg/g) increased by 2.51 times in comparison with that in saline solution (2.22–7.81/2.87–10.80 µg/g). The decrease of pH and the increase of ionic strength in gastric fluid could further strengthen desorption of E2/EE2 from PVC. The promotion of gastric juice on desorption of PVC was achieved by reducing the hydrophobicity of the PVC surface. The desorption rate of E2/EE2 at 18°C and 38°C was respectively 44%–47%/46%–50% and 49%–55%/56%–59%, indicating that PVC loaded with E2/EE2 had a relatively greater risk of releasing pollutants in the gastric fluid of constant temperature marine organisms while higher temperatures exposed higher hazards for variable temperature animals. Abstract Objective Cannabidiol (CBD) has been reported to have anti-diabetic effects in pre-clinical and clinical studies but its inhibitory effects on α-glucosidase, a carbohydrate hydrolyzing enzyme, remain unknown. Herein, we evaluated CBD’s inhibitory effects on α-glucosidase using in vitro assays and computational studies. Methods CBD’s inhibitory effect on α-glucosidase activity was evaluated in a yeast enzymatic assay and by molecular docking. The stability of CBD in simulated gastric and intestinal fluids was evaluated by high-performance liquid chromatography analyses. Results CBD, at 10, 19, 38, 76, 152, 304, 608, and 1216 μM, inhibited α-glucosidase activity with inhibition of 17.1, 20.4, 48.1, 56.6, 59.1, 63.7, 74.1, and 95.4%, respectively. Acarbose, the positive control, showed a comparable inhibitory activity (with 85.1% inhibition at 608 μM). CBD’s inhibitory effect on α-glucosidase was supported by molecular docking showing binding energy (-6.39 kcal/mol) and interactions between CBD and the α-glucosidase protein. CBD was stable in simulated gastric and intestinal fluids for two hours (maintained ≥ 90.0%). Conclusions CBD showed moderate inhibitory effect against yeast α-glucosidase activity and was stable in gastric and intestinal fluids. However, further studies on CBD’s anti-α-glucosidase effects using cellular and in vivo models are warranted to support its potential application for the management of type II diabetes mellitus.

This study investigated the effect of natural aquatic medium and human digestive media on the dissolution of single and mixture of nanoparticles. We determined an in-vitro dissolution factor for a mixture of nanoparticles in human digestive media. The oral exposure scenario was considered in this study, with exposure of nanoparticles through surface water containing a mixture of nanoparticles. The dissolution of these nanoparticles was tracked through the human gastrointestinal tract, including saliva, gastric, and intestinal artificial fluids. The dissolution factors for ZnO and CuO nanoparticles were found to be 68% and 54% in mixture suspension at 10 mg/L nanoparticle concentration, respectively. A comparison between single and mixture of nanoparticles showed a significant difference in ion contents in various mediums, thus indicating the effect of the simultaneous presence of two nanoparticles on extents of dissolution. The presence of antibiotics in the aquatic system along with mixture of nanoparticles decreased CuO nanoparticle dissolution factor to 21% and for ZnO, dissolution factor was found to be 77%. Both particulate fraction and ionic fraction for single and mixture of nanoparticles were reported in this study. Digestive loading values associated with NPs was found to be 6.2 and 5.4 mg/L for ZnO and CuO nanoparticles, respectively. Such data is essential in assessing exposure dose and characterizing human health risk. Predicting survival in very preterm infants is critical in clinical medicine and parent counseling. In this prospective cohort study involving 96 very preterm infants, we evaluated whether the metabolomic analysis of gastric fluid and urine samples obtained shortly after birth could predict survival in the first 3 and 15 days of life (DOL), as well as overall survival up to hospital discharge. Gas chromatography–mass spectrometry (GC-MS) profiling was used. Uni- and multivariate statistical analyses were conducted to evaluate significant metabolites and their prognostic value. Differences in several metabolites were identified between survivors and non-survivors at the time points of the study. Binary logistic regression showed that certain metabolites in gastric fluid, including arabitol, and succinic, erythronic and threonic acids, were associated with 15 DOL and overall survival. Gastric glyceric acid was also associated with 15 DOL survival. Urine glyceric acid could predict survival in the first 3 DOL and overall survival. In conclusion, non-surviving preterm infants exhibited a different metabolic profile compared with survivors, demonstrating significant discrimination with the use of GC-MS-based gastric fluid and urine analyses. The results of this study support the usefulness of metabolomics in developing survival biomarkers in very preterm infants.

Introduction: In recent research, orally administered cannabidiol (CBD) showed a relatively high incidence of somnolence in a pediatric population. Previous work has suggested that when CBD is exposed to an acidic environment, it degrades to Δ9-tetrahydrocannabinol (THC) and other psychoactive cannabinoids. To gain a better understanding of quantitative exposure, we completed an in vitro study by evaluating the formation of psychoactive cannabinoids when CBD is exposed to simulated gastric fluid (SGF). Methods: Materials included synthetic CBD, Δ8-THC, and Δ9-THC. Linearity was demonstrated for each component over the concentration range used in this study. CBD was spiked into media containing 1% sodium dodecyl sulfate (SDS). Samples were analyzed using chromatography with UV and mass spectrometry detection. An assessment time of 3 h was chosen as representative of the maximal duration of exposure to gastric fluid. Results: CBD in SGF with 1% SDS was degraded about 85% after 60 min and more than 98% at 120 min. The degradation followed first-order kinetics at a rate constant of -0.031 min-1 (R2=0.9933). The major products formed were Δ9-THC and Δ8-THC with less significant levels of other related cannabinoids. CBD in physiological buffer performed as a control did not convert to THC. Confirmation of THC formation was demonstrated by comparison of mass spectral analysis, mass identification, and retention time of Δ9-THC and Δ8-THC in the SGF samples against authentic reference standards. Conclusions: SGF converts CBD into the psychoactive components Δ9-THC and Δ8-THC. The first-order kinetics observed in this study allowed estimated levels to be calculated and indicated that the acidic environment during normal gastrointestinal transit can expose orally CBD-treated patients to levels of THC and other psychoactive cannabinoids that may exceed the threshold for a physiological response. Delivery methods that decrease the potential for formation of psychoactive cannabinoids should be explored.

Background: In traditional Chinese medicine, it is believed that the "tongue coating is produced by fumigation of stomach gas", and that tongue coating can reflect the health status of humans, especially stomach health. Therefore, studying the relationship between the microbiome of the tongue coating and the gastric fluid is of great significance for understanding the biological basis of tongue diagnosis. Methods: This paper detected the microbiomes of the tongue coating and the gastric fluid in 35 gastritis patients using metagenomic sequencing technology, systematically constructed the microbial atlas of tongue coating and gastric juice, and first described the similar characteristics between the two sites. Results: There was a significant correlation between tongue coating and gastric juice in terms of microbial species composition and overall diversity. In terms of species composition, it was found that the two sites were dominated by five phyla, namely, Actinobacteria, Bacteroidetes, Firmicutes, Fusobacteria and Proteobacteria, and that most of the gastric microbial species could be detected from the patient's own tongue coating. In terms of overall diversity, a significant correlation was found between the alpha diversity of the tongue coating microbiome and the gastric juice microbiome. Furthermore, in terms of abundance, 4 classes, 2 orders, 4 families, 18 genera and 46 species were found to significantly correlate between the tongue coating and the gastric fluid. Conclusions: The results provide microbiome-based scientific evidence for tongue diagnosis, and offer a new perspective for understanding the biological basis of tongue diagnosis. Objective: The aim of the present study was to evaluate the changes caused by intravenous administration of regulatory peptides, bombesin (BBS) and neurotensin (NT), on gastric secretion, serum gastrin, and plasma levels of bombesin-1ike immunoreactivity (BLI) and neurotensin. Materials and methods: Fourteen dogs underwent an upper gastrointestinal tract operation and a Pavlov pouch for the concentration of gastric fluids was formed. The experimental animals were divided into two groups. Peptides were given one month after the second operation and after fasting for 12 hours. In group A, the effects of BBS were studied after a rapid 1 µg/kg body weight dose and a slow 30΄ 0.5 μg/kg body weight dose administration intravenously. Correspondingly to group B the effects of NT were studied in the same way. Results: The rapid intravenous infusion of BBS caused a very significant increase in gastrin levels, BLI in plasma, volume and HCl of the gastric fluids. The same results, plus a significant decrease in gastric pH, were observed following slow intravenous infusion of BBS. Concerning the NT, rapid administration caused a significant decrease in the volume of gastric fluids. Slow NT administration of caused a significant reduction in gastric fluid volume and in HCl. On the contrary, pH was significantly increased. Conclusion: Bombesin increases plasma gastrin levels and HCl secretion. • Gastric fluid IL-6 levels at birth were increased in neonates with chorioamnionitis. • Gastric fluid IL-6 correlated strongly with the presence of chorioamnionitis. • Gastric fluid IL-6 may be a promising biomarker for predicting exposure to inflammation in utero. The risk of various complications, such as neonatal death, early onset sepsis, and chronic lung disease, is increased in infants born to mothers with chorioamnionitis (CAM). However, predicting the diagnosis of histological CAM (hCAM) in the early postnatal period is challenging for clinicians due to pathological considerations. Therefore, an early diagnostic tool for hCAM is needed. Gastric fluid at birth is considered a suitable biomarker for predicting the intrauterine environment because most of its components are from amniotic fluid, and the sampling technique is less invasive. This study aimed to evaluate the clinical utility of cytokines in the gastric fluid of preterm infants at birth as predictors of hCAM. We retrieved gastric fluid and serum from 21 preterm infants with a gestational age of ≤ 32 weeks within 1 h after birth and used cytometric bead array to measure the concentrations of interleukin (IL)-2, IL-4, IL-6, IL-10, tumor necrosis factor-alpha, and interferon-gamma. We compared the cytokine concentrations in the gastric fluid and serum of the preterm infants born to mothers with or without hCAM. The gastric fluid, serum IL-6, and serum IL-10 concentrations were significantly higher in the hCAM group than that in the non-hCAM group. The best cutoff values for predicting hCAM was > 2,855 pg/mL and > 315 pg/mL for IL-6 in the gastric fluid and serum, respectively. Receiver operating characteristic curves showed that gastric fluid IL-6 concentrations correlated more strongly with the presence of hCAM than serum IL-6 concentrations. IL-6 in the gastric fluid at birth may be a more promising biomarker for predicting the presence of hCAM than that in serum. IL-6 concentration analysis in the gastric fluid at birth might help to diagnose hCAM immediately after birth and improve the prognosis of preterm infants. [ABSTRACT FROM AUTHOR]

OBJECTIVES: Our goal was to compare the hydrogen potential (pH) and residual gastric volume (RGV) of patients undergoing colonoscopy after 3 and 6 hours of colon preparation with mannitol. METHODS: We described a prospective randomized trial with a 50:50 allocation rate of two distinct times of colonoscopy after colon preparation with 10% mannitol. We included outpatients aged over 18 years, with no history of gastric surgeries and an American Society of Anesthesiologists (ASA)-rated anesthetic risk below III. Colonoscopy was performed after upper digestive endoscopy at two different times: 3 versus 6-hour after mannitol ingestion. During upper gastrointestinal endoscopy, we measured RGV and evaluated pH with a digital pH meter. Clinical trials.gov: 71123317.9.3001.0065 RESULTS: We randomized a total of 100 participants to the 3 and 6-hour groups, with the patients in the 6-hour group being younger and presenting a higher body mass index (BMI). The intervention did not result in any statistically significant differences between the two groups, neither for the RGV (p=0.98) or the pH (p=0.732). However, the subgroup of patients with diabetes mellitus showed statistically significant higher RGV values in the 3-hour group. CONCLUSION There was no difference between RGV and pH values at 3 versus 6-hour after bowel preparation with mannitol, except for RGV in diabetic patients at 3 hours. As prolonged fasting protocols may result in adverse events such as dehydration and electrolyte imbalance, we can infer that colonic preparation with mannitol in shorter fasting periods, such as 3 hours, can be adopted safely and routinely.

For this study, the transfer of plastic additives to stomach oil of northern fulmars (Fulmarus glacialis) has been investigated. Procellariiform seabirds retain oily components of their prey in theirs stomach as a means to store energy. A marine litter-derived microplastic reference mixture and separately a marine litter-derived polystyrene sample were added to stomach oils in an experiment. A total of 15 additives, including plasticizers, antioxidants, UV stabilizers, flame retardants, and preservatives, were identified in the original plastic mixtures and monitored in the leachates. These substances include those known for endocrine disruptive, carcinogenic, and/or other negative effects on organisms. Stomach oil was exposed to these plastic materials and was sampled during a long-term experiment (0, 14, and 90 days’ exposure of plastic particles in stomach oil) and a subsequent short-term detailed study (8 h and 1, 2, 4, 8, and 21 days). Five of the monitored substances were shown to strongly leach from the microplastic reference mixture into the stomach oil during the experiment. Four substances were identified in a marine litter-derived polystyrene foam, of which two leached into stomach oil. Leaching of harmful plastic additives to the stomach oil of fulmars may be of concern, as fulmars regularly ingest plastics that are retained and gradually ground in the gizzard before passage to the intestines and excretion.

Background: Norovirus (NoV) can survive in buffer at a range of pH values. However, when variations to pH are combined with the enzymatic conditions typical in gastric fluid, NoV survival is less predictable. Added to this, the material on which expelled vomitus fluid lands e.g. safety vinyl or carpet, might also affect the level of NoV survival in the environment.Aim: This study assessed survival of the NoV surrogate Feline calicivirus (FCV) after application onto four material types in the presence of synthetic gastric fluid of varying pH values.Methods: Synthetic gastric fluid of varying pH values (1.5, 2.5, 3.5 and 4.5) containing FCV (1×106 PFU/100 μL of gastric fluid) was seeded (100 μL) onto four material types: Formica, safety vinyl, cotton sheet and carpet tiles (2 cm × 2 cm) and left for 0, 30 and 60 minutes before recovering and identifying any viable viruses via plaque assay.Findings: FCV survived in synthetic gastric fluid at pH values as low as 2.5 and in some cases as low as 1.5 for at least 30 min when associated with certain materials. Greater average numbers of viable virus were recovered from the more absorbent materials of cotton (1900 PFU/sample) and carpet (1600 PFU/sample) compared with Formica (360 PFU/sample) and safety vinyl (380 PFU/sample).Conclusion: This study showed that FCV can survive in synthetic gastric fluids of low pH values for significant time periods especially on absorbent materials. This emphasises the importance of ensuring effective cleaning and decontamination procedures, particularly of soft furnishings and laundry, for infection prevention and control of NoV outbreaks. Aims: To examine the relationship between preoperative anxiety, age, gastric fluid volume (GFVw) and pH among paediatric outpatients undergoing ambulatory surgery. Methods: An observational study was conducted on patients aged 1-12 years. Preoperative anxiety was evaluated by modified Yale Preoperative Anxiety Scale on admission (mYPAS-ad) and on entry to operating rooms (mYPAS-or). Gastric content was aspirated under general anaesthesia. Results: Complete data was collected from 119 patients. Mean mYPASad was 36.36, mean mYPAS-or was higher, 48.35. Mean GFVw (ml/kg) was 0.404, pH was 1.55. Patients with higher mYPAS-or had significantly larger GFVw. mYPAS-ad and GFVw were not related. Older children had significantly larger GFVw and lower pH. Diagnostic methods for Helicobacter pylori infection include, but are not limited to, urea breath test, serum antibody test, fecal antigen test, and rapid urease test. However, these methods suffer drawbacks such as low accuracy, high false-positive rate, complex operations, invasiveness, etc. Therefore, there is a need to develop simple, rapid, and noninvasive detection methods for H. pylori diagnosis. In this study, we propose a novel technique for accurately detecting H. pylori infection through machine learning analysis of surface-enhanced Raman scattering (SERS) spectra of gastric fluid samples that were noninvasively collected from human stomachs via the string test. One hundred participants were recruited to collect gastric fluid samples noninvasively. Therefore, 12,000 SERS spectra (n = 120 spectra/participant) were generated for building machine learning models evaluated by standard metrics in model performance assessment. According to the results, the Light Gradient Boosting Machine algorithm exhibited the best prediction capacity and time efficiency (accuracy = 99.54% and time = 2.61 seconds). Moreover, the Light Gradient Boosting Machine model was blindly tested on 2,000 SERS spectra collected from 100 participants with unknown H. pylori infection status, achieving a prediction accuracy of 82.15% compared with qPCR results. Objective: This study aims toprove the presence of calprotectin in gastric fluid, and to evaluate the use of gastric fluid calprotectin in the diagnosis of gastric cancer. Materials and Methods: Included in this prospective study were 60 patients with upper gastrointestinal system complaints. Gastric fluid samples obtained during gastroscopy procedures through an ERCP cannula were stored at -20 degrees centigrade for six months, after which the calprotectin levels were measured. Standard endoscopic biopsies were also taken from the patients included in the study for tissue diagnosis. Results: A statistically significant difference was found between the groups (p=0.0001), with significant differences identified between the three groups. The p values identified in comparisons of group 1 and group 2; group 1 and group 3; and group 2 and group 3 were found to be p=0.043, p=0.001 and p=0.0001. Conclusion: Calprotectin, which is found in the gastric fluid, may be used in the differential diagnosis of benign and malignant ulcers. Objective: To explore the effect of microinjection of orexin-A into vagal complex(DVC) on the gastric function of rats. Methods: Orexin-A was microinjected into the DVC, the changes of gastric motility and gastric juice or gastric acid secretion were observed. Results: After the microinjection of orexin-A into DVC, the amplitude and frequency of gastric motility were signi覱cantly increased in a dose-dependent manner. SB334867 prevented Orexin-A-induced changes of gastric motility(P0.05). After the microinjection of orexin-A into DVC, the secretion of gastric juice and gastric acid were significantly increased in a dose-dependent manner(P0.05). Aim of the Study: The pulmonary microbiota is important for both normal homeostasis and the progression of disease, and may be affected by aspiration of gastric fluid. The aim of this study was to investigate changes in the lung microbiota induced by aspiration of gastric fluid in a laboratory rat model. Material and Methods: Using the intratracheal application method, male rats received aspiration with 0.9% normal saline (n = 11); gastric fluid (n = 24) or sterilized (gamma-irradiated) gastric fluid (n = 12) once-weekly for four weeks. On the fifth week, the animals were sacrificed, and the microbiota of the lung was assessed by 16S ribosomal RNA gene sequencing. Results: Lungs without aspiration and lungs after aspiration with normal saline had similar microbial compositions, dominated by bacteria of the genera Serratia, Ralstonia and Brucella. Evaluation of the microbiota following aspiration of gastric fluid revealed a much different profile that was dominated by bacteria from the genera Romboutsia and Turicibacter and largely independent of sterilization of the gastric fluid. Conclusion: In a laboratory rat model, aspiration with gastric fluid caused a substantial shift of the lung microbiota that could be characterized as a shift from Proteobacteria towards Firmicutes, possibly of enteric origin. Aim of the Study: The aim of this study was to investigate a new method for visualization and quantification of intrapulmonary liquid distribution after oropharyngeal gastric fluid aspiration in mice.Material and Methods: Eleven mice received oropharyngeal aspiration with a gastric fluid, India ink, and saline solution. Digital imaging and pixel calculation were used to analyze intrapulmonary fluid distribution selectively.Results: Digital pixel analysis and orophanryngeal aspiration are both safe techniques in mice and deliver reproducible/valid results. Analysis revealed an average aspirate distribution of 86.8% of the total lung area. The proportional amount of the left lung was significantly greater than that of the right lung (P = 0.023). The lobe with the lowest mean distribution was the right lower lobe (79.2% ± 4.4%).Conclusion: Digital pixel calculation is a reliable method for quantitative, macroscopic evaluation of fluid distribution in the lung. Increased use is being made of nitinol for implants that are exposed to gastric fluid. However, few corrosion studies have involved nitinol in an appropriate acidified chloride solution. In this work, the electrochemical behavior of electropolished (EP) nitinol was examined in simulated gastric fluid, the corresponding neutral solution with the same concentration (0.6%) of NaCl, and 0.9% NaCl. Cyclic potentiodynamic polarization was used to evaluate the susceptibility to pitting corrosion, while electrochemical impedance spectroscopy was used to examine the passive oxide film. The potentiodynamic tests showed that the susceptibility of EP nitinol to pitting corrosion is affected by chloride concentration and pH. Acidification, in particular, resulted in the susceptibility being markedly higher in gastric fluid compared with that in the corresponding neutral NaCl solution. The impedance data could be fitted using a parallel resistance-capacitance (as a constant phase element) circuit associated with the oxide film. The thickness of the oxide was determined from the capacitive component and found to be little affected by chloride concentration. In contrast, acidification increased the solubility of the oxide enough to decrease the thickness of the film from 5.3 nm in 0.6% NaCl to 4.2 nm in gastric fluid. The resistivity of the oxide obtained from the resistance was affected by chloride concentration (0.7 × 1011 and 1.7 × 1011 Ω m in 0.9% and 0.6% NaCl, respectively) and particularly by pH (6.3 × 1011 Ω m in gastric fluid). For a realistic evaluation of the potential hazard emanating from airborne particulate matter (APM), the determination of the total inhaled metal amounts associated with APM is insufficient in risk assessment. Additional information about metal fractions that can be mobilized by the human body is necessary, because only those soluble (also called bioaccessible) fractions can be absorbed by the human body, and thus potentially cause adverse health effects. In the present study, a dynamic flow-through approach as a front end to inductively coupled plasma optical emission spectrometry (ICP-OES) exploiting advanced flow analysis is employed for on-line handling of multiple APM samples and determination of bioaccessible trace metals under worst case extraction scenarios. The method is based on on-line continuous extraction of filter samples with synthetic gastric fluid followed by on-line ICP-OES measurement of the dissolved fraction of trace metals. The assembly permits an automated successive measurement of three sample replicates in less than 19 min. The on-line extraction procedure offers increased sample throughput and reduced risk of sample contamination and overcomes metal re-adsorption processes compared to the traditional batch-wise counterparts. Furthermore, it provides deeper information on the kinetics of the leaching process. The developed procedure was applied to the determination of bioaccessible metal fractions (Al, Ba, Cu, Fe and Mn as model analytes) in PM10 samples from Palma de Mallorca (Spain) and Vienna (Austria). Microplastics (MPs) are widely distributed throughout the environment. Upon ingesting MPs, the pollutants that they carry are then desorbed into organisms. This results in the accumulation of various chemicals within the organism. This study systematically examined the mechanism of antibiotic desorption using tire wear particles (TWP) as a carrier of antibiotics in simulated human gastrointestinal fluid and fish intestinal fluid. The findings of this study revealed the formation of cracks, pores, and depressions on the surface of photoaged TWP in an aquatic environment, as well as additional adsorption sites that are more favorable for the attachment of pollutants. Furthermore, the simulated human gastric fluid had a higher desorption rate than that of the fish intestinal fluid. The competition for TWP adsorption sites in the gastrointestinal fluid and the potential dissolution of antibiotics were the primary drivers of the increase in the desorption rate. The desorption rate in the simulated human gastrointestinal fluid was greater than that in the simulated fish intestinal fluid due to the composition of the gastrointestinal fluid. However, the carrying of pollutants by MPs poses a potential threat to human health. An in-depth understanding of the properties of gastric fluid(s) prior to an in vivo pharmacokinetic investigation can vastly improve predictions of in vivo performance. Previously, properties of animal and human gastric fluids have been characterized with varying methods. Unfortunately, characterization has often not been thorough, and some properties, such as density and viscosity, have not been reported. Here, human, porcine and canine gastric fluids were harvested and characterized for pH, viscosity, surface tension, density, and osmolarity. We found that the variability of pH and surface tension between dogs was significantly higher than the variability between pigs, and, furthermore, gastric fluids collected from the same canine species (beagles) housed in two different countries (Denmark and China) had surprisingly different pH values. Next, an in vitro dissolution study in diluted gastric fluids from each species was performed using minitablets containing ibuprofen. Human gastric fluids and porcine gastric fluids showed similar dissolution profiles and corroborated well with biorelevant human Fasted State Simulated Gastric Fluid (FaSSGF). In contrast, differences in canine gastric fluids caused highly variable dissolution results. We systematically compared our findings to those in the literature and based on this evaluation, propose obtaining aspirates from the animals used for in vivo studies to ensure knowledge on the fluid properties affecting the performance of the formulated drug in question. While the stomach is a fertile site for bacterial growth, its strong gastric acid inhibits such growth. Although Helicobacter pylori is considered a pathogenic bacterium causing peptic ulcers and gastric cancers, it is also thought to be indigenous to the stomach. The derangement of gastric motility and excessive gastric acid secretion might be induced in the stomachs of the H. pylori-free people. Which may then cause functional dyspepsia andgastro-esophageal reflux disease. A comparative analysis of the gastric microbiota by the 16S rRNA gene profiling revealed that the species richness was similar among the microbiota of gastric fluid (GF), saliva and feces, but the inter-individual diversity was much higher in the fecal microbiota than in the GF or saliva. Furthermore, a phylogenic analysis indicated that the GF and salivary microbiota were similar in the overall bacterial community structure, but both differed markedly from the community structure of the fecal microbiota. On comparing the residts between proton-pump inhibitor (PPI)-users and PPFnonusers, 1000-fold greater number of bacterial cells was found in the GF of PPFusers using culturing method, while the bacterial cell numbers and composition were nearly identical between the two groups using real time PCR and a similarity search. Evaluation of the reducing capacity of human gastric fluid from healthy individuals, under fasted and fed conditions, is critical for assessing the cancer hazard posed by ingested hexavalent chromium [Cr(VI)] and for developing quantitative physiologically-based pharmacokinetic models used in risk assessment. In the present study, the patterns of Cr(VI) reduction were evaluated in 16 paired pre- and post-meal gastric fluid samples collected from 8 healthy volunteers. Human gastric fluid was effective both in reducing Cr(VI), as measured by using the s -diphenylcarbazide colorimetric method, and in attenuating mutagenicity in the Ames test. The mean (± SE) Cr(VI)-reducing ability of post-meal samples (20.4 ± 2.6 μg Cr(VI)/mL gastric fluid) was significantly higher than that of pre-meal samples (10.2 ± 2.3 μg Cr(VI)/mL gastric fluid). When using the mutagenicity assay, the decrease of mutagenicity produced by pre-meal and post-meal samples corresponded to reduction of 13.3 ± 1.9 and 25.6 ± 2.8 μg Cr(VI)/mL gastric fluid, respectively. These data are comparable to parallel results conducted by using speciated isotope dilution mass spectrometry. Cr(VI) reduction was rapid, with > 70% of total reduction occurring within 1 min and 98% of reduction is achieved within 30 min with post-meal gastric fluid at pH 2.0. pH dependence was observed with decreasing Cr(VI) reducing capacity at higher pH. Attenuation of the mutagenic response is consistent with the lack of DNA damage observed in the gastrointestinal tract of rodents following administration of ≤ 180 ppm Cr(VI) for up to 90 days in drinking water. The fundamental understanding of the behavior of various foods in environments that simulate the human organism is of crucial importance. The study of the release of various components in solutions simulating the human gastric fluid and saliva helps in understanding their availability in the human body. In the present study, disintegration of salt and sugar particulates contained in processed agricultural products was examined. Potatoes and bananas were osmotically dehydrated in salt and sucrose solutions, respectively, and then dried using two drying methods, air drying and freeze drying. The dried products were soaked in simulated saliva and gastric fluid at 37 °C. During immersion, salt release in saliva and gastric fluid was determined using three methods; electrochemical impedance spectroscopy, for measuring the resistance of the solution, titration with silver nitrate, for measuring the amount of salt in the solution and conductivity measurements, for measuring the conductivity of the solutions. Sugar release was determined by measuring degrees brix of the solutions at regular intervals. One Brix corresponds to 1 g of sugars/100 g of solution. The results indicated that different process conditions caused different impact on release kinetics of salt and sugar particulates in saliva and gastric fluid, influencing their taste perception and the bioavailability in the human body. We performed this study to determine (1) whether the levels of inflammatory mediators, including matrix metalloproteinase-9 (MMP-9) and intercellular adhesion molecule-3 (ICAM-3), in gastric fluid (GF) in premature newborns are associated with those in amniotic fluid (AF) in their mothers and (2) whether the levels of the inflammatory mediators in newborn GF are associated with the presence of intrauterine inflammation (IUI). Methods: Sixty-two pairs of pregnant women and their premature newborns born at <35 weeks' gestation by cesarean delivery were enrolled in this study. AF and newborn GFs were obtained during cesarean section procedures. Levels of MMP-9, ICAM-3, interleukin-6 (IL-6), interleukin-8 (IL-8), and tumor necrosis factor-α (TNF- α) were measured and compared between the AF and newborn GFs in each dyad, according to the presence or absence of chorioamnionitis (CA), preterm prelabor rupture of membranes (PPROM), and preterm labor (PTL). Results: The levels of MMP-9, ICAM-3, IL-6, IL-8, and TNF-α in newborn GF were significantly correlated with those in AF in each dyad. The premature newborns and their mothers with CA had significantly higher GF MMP-9, IL-8, and TNF-α levels than those without CA. Those with PPROM or PTL showed similar findings in terms of GF MMP-9, IL-8, and TNF-α levels. Conclusion: The newborn GF immediately after birth can be a useful alternative source of information on whether a premature infant has been exposed to IUI at the time of delivery. To accurately predict thein vivoperformance of drugs from anin vitrodissolution test, the dissolution conditions used are supposed to be similar to those present in the gastrointestinal milieu. Post-prandial gastric fluid contains partially digested food mixtures consisting of fat, protein and carbohydrate. Despite this, the compendia dissolution medium recommended to simulate the gastric fluid is still composed of a simple solution of hydrochloric acid and sodium chloride with or without the addition of pepsin. Therefore, in this investigation, biorelevant dissolution media were developed to evaluate the impact of food constituents; milk with different fat contents, egg albumin, gelatin, casein, gluten, carbohydrates and amino acids on the intrinsic dissolution behavior of ketoconazole. Most of the food additives that were evaluated enhanced the apparent solubility of the drug but to different extents. The greatest enhancement in dissolution was observed in media containing either neutral amino acids or media based on milk mixtures. The formation of complexes between the drug and the additives most likely accounted for the solubilizing effect and in milk-containing media, the effect was attributed to the whole complex structure of milk rather than simply its fat content. Résumé Les infections néonatales précoces (INP) nécessitent une stratégie diagnostique et thérapeutique optimale en raison du taux élevé de mortalité, en particulier chez le prématuré et du risque de complications majeures notamment neurologiques. Le diagnostic précoce des INP est difficile du fait de signes cliniques non spécifiques chez le nouveau-né. La décision de traiter ne peut être prise que sur un faisceau d’arguments biologiques, cliniques et anamnestiques. Ainsi de nombreux nouveau-nés reçoivent une antibiothérapie probabiliste intraveineuse non sans conséquences sur leur colonisation digestive, la réponse immune du tractus gastro-intestinal et la durée de leur hospitalisation. Plus de 10 ans après les recommandations de l’ANAES, les prélèvements bactériologiques à réaliser chez le nouveau-né à la naissance restent discutés et très variables selon les centres hospitaliers. L’hémoculture est l’examen de référence pour le diagnostic de certitude de l’INP, mais est un geste invasif nécessitant un délai de culture et la faible quantité de sang prélevé la rend peu sensible. Les prélèvements non centraux (oreille, liquide gastrique…) ont une valeur limitée pour porter le diagnostic d’INP et l’intérêt de la multiplicité des sites prélevés n’est pas démontré. Cependant les prélèvements non centraux permettent de documenter l’INP et d’adapter l’antibiothérapie. L’apport des techniques de biologie moléculaire dans le diagnostic des INP est encore à définir. De nouveaux marqueurs biochimiques, comme la PCT au sang de cordon, semblent prometteurs par rapport à la CRP classiquement utilisée. En permettant de catégoriser précocement les nouveau-nés comme infectés ou non infectés, la PCT au sang de cordon serait susceptible de limiter la prescription d’antibiotiques. De nouvelles recommandations apparaissent indispensables en 2015 pour optimiser et homogénéiser la prise en charge des nouveau-nés à la naissance en France. Summary Early-onset neonatal diseases (EOND) require optimal diagnostic and therapeutic strategy to prevent severe (including neurological…) and life threatening complications notably in preterm neonates. The early diagnosis of EOND is challenging because of non-specific clinical signs by the newborn. So the decision to treat can only be taken on a range of biological, clinical and anamnestic arguments. Therefore, many newborns were treated by an empiric antibiotic ; this is not without consequences for their digestive colonization, the immune response of the gastrointestinal tract and thereby the duration of their hospitalization. Since the last guideline of ANAES in 2002, the type of bacteriological samples to conduct in the newborn at birth remains controversial and varies greatly from hospital. Blood culture is currently the gold standard for the diagnosis of sepsis. However, in addition to the fact that culture reports are available only after 48–72 hours, blood cultures are frequently false negative due to the small amount of blood that can be drawn from neonates. Non central samples (ear, gastric fluid…) have limited value to diagnose EOND and the contribution of multiple peripheral sampling remains to be demonstrated. However non central sampling may help to isolate the pathogen and to determine its antibiotics susceptibilities. Molecular diagnoses based on acid nucleic detection are still under assessment. Biochemical marker such as umbilical blood cord procalcitonin has a promising bacterial infection diagnostic value in neonates with a better diagnosis performance than CRP, limiting complementary exams and exposure to antibiotics. Background: Mast cells have been associated with obliterative bronchiolitis (OB) in human pulmonary allografts, although their role in the development of OB remains unknown. Methods: In this study, we evaluated the role of mast cells in pulmonary allograft rejection using an orthotopic rat pulmonary allograft model that utilizes chronic aspiration of gastric fluid to reliably obtain OB. Pulmonary allograft recipients (n = 35) received chronic aspiration of gastric fluid with (n = 10) and without (n = 16) treatment with a mast cell membrane stabilizer, cromolyn sodium, or chronic aspiration with normal saline (n = 9) as a control. Results: The acute graft injury associated with long ischemic time in the model (6 hours total ischemic time; typical acute graft injury rate 30%) was apparently blocked by cromolyn, because peri-operative mortality associated with the acute graft injury was not observed in any of the animals receiving cromolyn (p = 0.045). Further, the rats receiving cromolyn developed significantly fewer OB lesions than those treated with gastric fluid alone (p < 0.001), with a mean reduction of 46% of the airways affected. Conclusions: These findings provide impetus for further studies aimed at elucidating the effects of cromolyn and the role of mast cells in pulmonary allotransplantation. [ABSTRACT FROM AUTHOR]

DNA pyrosequencing, one of the advanced methods for DNA sequencing, has been employed for phylogenetic analysis of bacterial communities using the conserved 16S rRNA gene. We performed a pilot study on a mother-neonate pair utilizing the DNA pyrosequencing assays to investigate the diversity of microbial communities in maternal amniotic fluid (AF), vagina, and rectum and newborn gastric fluid (GF) and stool. Phylum level analysis revealed that bacterial community was dominated by Firmicutes (63.2%) in maternal feces, and Actinobacteria (84.9%) in maternal vaginal swab. The bacterial communities in both the AF and GF were dominated by Proteobacteria (67.8%). Interestingly, the bacterial community in the newborn's meconium was quite similar to that in the AF. However, the composition of the bacterial community in newborn's feces was different on day 14 and dominated by Firmicutes (91.1%). Genus-level analysis revealed that the bacterial community in maternal feces was dominated by Anaerococcus (19.5%) and Prevotella (18.7%), whereas that in the maternal vaginal swab was dominated by Atopobium (83.6%). The bacterial communities in both the AF and GF were dominated by Sphingomonas (38.5%). The bacterial community in the newborn's meconium was quite similar to that in the AF, which was dominated by Sphingomonas (45.2%). However, the composition of bacterial community in the newborn's feces on day 14 was relatively different. Future studies with a large number of infants are needed to determine the factors involved in the changing profile of newborn's fecal bacterial communities. [ABSTRACT FROM AUTHOR]

Microplastics entering the digestive system of living organisms can serve as a carrier of hydrophobic organic pollutants (HOPs), increasing their exposure levels and the health risks they pose to both humans and animals. The desorption kinetics of six polyhalocarbazoles (PHCZs) from 5 mm and 0.15 mm polypropylene (PP) and polyvinyl chloride (PVC) microplastic particles were assessed using a combined microplastics and food system, representing the gastric system of vertebrates and invertebrates. Results showed that the chemical transfer of PHCZs is biphasic and reversible, with rapid exchange occurring within 2–48 h, followed by a period of slow transfer, which continues for weeks to months. The desorption capacity of PHCZs loaded on 0.15 mm microplastic particles was greater than that of 5 mm particles. The bioavailability percentage of PHCZ congeners for PP (24.2%–65.3%) and PVC (43.5%–57.2%) in the vertebrate fluid system were all lower than those in the invertebrate system (34.2%–70.7% for PP and 56.3%–72.7% for PVC, respectively). These findings indicate that physiological conditions, such as polarity, ingestion fluid, and microplastic affect the desorption of PHCZs from microplastics. In addition, desorption from PP was inhibited by the presence of foodstuff loaded with PHCZs due to competition, while desorption from PVC was not significantly affected by the presence of PHCZs contaminant food. Microplastics could provide a cleaning function in gastric fluid systems containing contaminated foodstuff, especially PP, which was capable of competitive adsorption of PHCZs from food. Few investigations have focused on the adverse effects of microplastic ingestion on human health, particularly in their role as vectors for HOPs, compared to other routes of exposure and transport.
