Background and Objectives: People living with dementia have historically been excluded from qualitative research and their voices ignored due to the perception that a person with dementia is not able to express their opinions, preferences and feelings. Research institutions and organizations have contributed by adopting a paternalistic posture of overprotection. Furthermore, traditional research methods have proven to be exclusionary towards this group. The objective of this paper is to address the issue of inclusion of people with dementia in research and provide an evidence-based framework for dementia researchers based on the five principles of human rights: Participation, Accountability, Non-discrimination and equality, Empowerment and Legality (PANEL). Design: This paper adapts the PANEL principles to the research context, and uses evidence from the literature to create a framework for qualitative research in people with dementia. This new framework aims to guide dementia researchers in designing studies around the needs of people with dementia, to improve involvement and participation, facilitate research development and maximize research outcomes. Results: A checklist is presented with questions related to the five PANEL principles. These questions cover ethical, methodological and legal issues that researchers may need to consider while developing qualitative research for people with dementia. Conclusions: The proposed checklist offers a series of questions and considerations to facilitate the development of qualitative research in patients with dementia. It is inspired by current human rights work of recognized dementia researchers and organizations who have been directly involved in policy development. This paper considers the potential for the Mental Capacity Act (MCA) of England and Wales to incentivise the exclusion of people with dementia from research. The MCA is intended to standardise and safeguard the inclusion of people with cognitive impairments in research. This entails various procedural requirements, which in pressurised research contexts can lead researchers to exclude people with dementia as a means of simplifying bureaucratic constraints. I consider the risks of an 'unethical ethics', wherein procedural ethics indirectly causes the exclusion of people with dementia from research, undermining historic successes toward increased inclusivity. I suggest several solutions, including enhanced sensitivity to impairments and shifting the burden of proof from justifying inclusion to justifying exclusion. The paper responds to the 'ethics creep' tradition in procedural ethics, and critical appraisals of capacity legislation in dementia research. This approach recognises that institutional research ethics is itself a major ethical concern and can unwittingly beget unethical practices. This paper studies whether risky borrowing will induce a supplier to expand capacity and thus mitigate a capacity underinvestment problem in supply chains. It demonstrates that the relationship between capacity and capital structure is not as simple as the existing literature suggests. If the supplier is very powerful, it will use a mixed capital structure and build less capacity than if it were equity financed. If the buyer is very powerful, the supplier will use all debt financing and overbuild capacity. In this case, the supplier would not agree to build capacity ex ante if it were prohibited from borrowing. Hence, borrowing is Pareto optimal. Under a more balanced power structure, the supplier may choose all debt financing and overbuild capacity or all equity financing and underbuild capacity, depending on the trade‐off of the bargaining benefits and costs of debt financing. This paper considers a supply chain setting where several capacitated suppliers compete for orders from a single retailer in a multiperiod environment. At each period, the retailer places orders to the suppliers in response to the prices they announce. Each supplier has a fixed capacity. We consider a make-to-stock setting where the retailer can carry inventory. The retailer faces exogenous, price-dependent demand. We study the problem using ideas from fluid models. In particular, we (i) analyze when there are pure equilibrium policies in this setting and characterize the structure of these policies; (ii) consider coordination mechanisms; and (iii) present some preliminary computational results. Objective: To critically examine a recent decision of the Victorian Supreme Court that found that the Mental Health Tribunal and the Victorian Civil and Administrative Tribunal erred in the application of the capacity test in the Mental Health Act 2014 (Vic) and that compulsory electroconvulsive therapy would infringe upon the human rights of two patients who had no insight into their chronic schizophrenia.Conclusions: After considering the concepts of insight and capacity to consent to treatment, the paper concludes that the decision in NJE and PBU v Mental Health Tribunal [2018] VSC 564 is problematic clinically. This paper studies how to design service outsourcing contracts to ensure fast, quality services from an independent service provider. The outsourcer does not have perfect information about the service provider's capacity cost (i.e., cost of providing fast service) and quality cost (i.e., cost of achieving a high quality level). Moreover, the two unknown costs may be positively, or negatively, correlated with each other. We solve for the outsourcer's optimal outsourcing contract, and show that the structure of the optimal contract depends on the relationship between the costs. Specifically, we highlight the following observations when the two costs are negatively correlated: First, under certain conditions, the outsourcer may be able to squeeze the supplier's profit (information rent) to zero for an intermediate range of cost realizations; second, it is possible that the service supply chain is coordinated by using the outsourcer's optimal contract. We then examine the performance of two classes of commonly observed contracts that are relatively simple to implement. It has been found that these simple contracts generally perform well when the costs are positively correlated, but they could perform much worse when the costs are negatively correlated. Our results therefore caution outsourcing companies that the potential trade‐off between capacity cost and quality cost may require a careful design of outsourcing contracts. [ABSTRACT FROM AUTHOR]

Because autonomy is regarded as central to modern bioethics; there is a considerable focus on the criteria by which autonomy may be judged. The most significant criterion used in day-to-day practice is decision-making capacity. A person who is decision-making capacitous is regarded as autonomous ipso facto. This rarely considered background analysis places a great deal of weight on the notion of decision-making capacity and how this is assessed. A person who is deemed to lack autonomy will be treated differently to a person who is deemed to have autonomy, at least when it comes to some medical decisions. In this paper we consider how extending the standard approach to decision-making capacity may deliver a more authentic reflection of autonomy. We refer to this as the 'authentic approach', to highlight its difference. Objective: Discussions of capacity to consent in mental health care usually revolve around capacity to consent to treatment. This paper instead explores the issue of capacity to consent to sexual activity in a mental health inpatient setting as a way of exploring capacity from a different perspective. This is not a purely theoretical exercise, with both consensual sexual activity and sexual assault commonplace in mental health inpatient units, current policy and practice approaches are clearly not working and require re-examination. Methods: Four key frameworks are explored: human rights law, mental health law, the criminal law and the law of tort governing the duty of care. These frameworks are explored by highlighting relevant case law and statutes and considering their potential application in practice. This is undertaken using the state of Victoria, Australia, as a case study. Results: The four frameworks are shown to be consistent with each other but inconsistent with contemporary policy. All four legal frameworks explored require clinicians to take a case-by-case assessment to ensure that a person's right to make their own decisions is preserved ' unless the contrary is demonstrably justified ' or where it is ' legally demanded '. While Victorian inpatient units attempt to enforce a blanket ban on consensual sexual activity in inpatient settings, this ban may be without legal basis and may be in breach of both human rights and mental health law. Conclusion: In policing the lawful bodily interactions of their patients and pushing sexual activity out of sight, clinicians may be breaching their duty of care to provide sexual health support and risk creating an environment in which the therapeutic relationship will be sacrificed to the enforcement of institutional policy. This study focuses on the elusive concept of community based tourism (CBT) with particular attention to Namibia’s Community Based Natural Resources Management (CBNRM). In this paper, CBT is viewed as a novel development strategy, but this strategy is fraught with lots of challenges. Considering that many African destinations have not cultivated their tourism to drive domestic tourists, the strategy tends to suffer from overdependence on the arrival of international tourists. Notwithstanding the benefits of tourism in developing countries, it appears as though tourism has not contributed to the much needed interventions in poverty alleviation, particularly at the household level. Various limitations related to the economic, socio-cultural, political and environmental challenges within the context of CBNRM in Namibia are discussed. With varying scales of tourism development in the country, the paper concludes that there are a number of challenges that CBNRM in Namibia is facing, particularly those related to the issues of governance, capacity and complete devolution of rights to local communities. Therefore, a concerted effort has to be made for ensuring total empowerment of local communities related to managing and sustaining operations with minimum intervention from donor agencies, and thereby, developing linkages with international tour operators and other multinational companies for maintaining competitiveness. This paper is an attempt to study the cyclic performances of recycled aggregate concrete (RAC)-filled steel tubular columns. Thirteen specimens with circular section, including 10 RAC-filled steel tubular (RACFST) columns and 3 reference normal concrete-filled steel tubular (CFST) columns, were tested under constant axial load and cyclically increasing flexural loadings. The parameters in the test are: (1) concrete type: normal concrete and recycled aggregate concrete; and (2) axial load ratio (n), from 0.05 to 0.52. It was found that, generally, the performances of RACFST columns under constant axial load and cyclically increasing flexural loadings are similar to the corresponding normal CFST columns, and RACFST columns can also exhibit high ductility levels and energy dissipation abilities. Objective: This paper provides a conceptual, historical and legislative overview of fitness issues. These include fitness to be interrogated or charged, to plead, to be tried and to be sentenced in the context of Australia and New Zealand. Conclusions: Legislation regarding fitness issues needs to strike a balance between protecting the rights of mentally impaired defendants to a fair trial process and recognizing that mental illness in itself does not absolutely preclude participation in a trial process or necessarily diminish criminal responsibility. Although the key determinants of fitness seem to be the ability to cope with court processes and communicate with counsel, the application of clinical judgement to legal criteria is not always straightforward. Renewable energies are considered as an essential element of any strategy for sustainable energy development. The poor in the developing world without access to modern energies are regarded as a major market for renewable energies. Renewable energies are expected to play a major role in the global future energy provision. There has been interest in electricity from renewable sources named green electricity or green pool as a special market. The term green energy is also used for green energy produced from cogeneration, energy from municipal waste, natural gas, and even conventional energy sources. Green power refers to electricity supplied from more readily renewable energy sources than traditional electrical power sources. The environmental advantages of the production and use of green electricity seem to be clear. Renewable energy sources are the third largest contributor to global electricity generation. Background: Although recommendations exist for making agriculture and food systems nutrition sensitive, guidance about how to successfully operationalize these recommendations within national institutions is limited. Over the 13 years from 2010 to 2023, there were several undertakings to strengthen the enabling environment for sustainable nutrition-sensitive agriculture (NSA) and food systems in Nigeria. Some studies were also conducted over the period to foster better understanding of the national enabling environment and facilitate effective actions. Objective: This article documents successes and failures from Nigeria's experiences by reflecting on critical developments, events, policies, and programmes on Nigeria's journey toward advancing nutrition through agriculture and food systems; as well as findings from conducted studies. Discussion: Successes include the existence of a Nutrition and Food Safety Division in the Ministry of Agriculture and an approval for a Nutrition Department, existence of an agriculture sector nutrition strategy, increased private sector engagement and commitment to nutrition-sensitive food systems, and increased funding for nutrition in the agriculture sector. Challenges especially remain for scaling up strategic, operational, and delivery capacity of both organizations and individuals involved in advancing NSA and food systems. Lessons learned include—institutionalizing NSA and food systems takes time; knowledge brokering is indispensable and requires several collaborating entities and stakeholders; and approaches used should be suited to available government capacity. Conclusion: More than a decade of efforts targeting different enabling environment factors has resulted in greater political commitment for nutrition within the agriculture sector and an improved enabling environment for NSA and food systems. The goal of the article is to provide lessons to other low- and middle-income countries that would like to develop agricultural and food systems that improve nutrition so that these countries are able to avoid or manage predictable risks. Successes include the establishment of an Office in the Ministry of Agriculture that oversees the incorporation of nutrition considerations into agricultural activities, the development of a strategy to help agricultural stakeholders know what to do to improve nutrition, and increased financing of activities related to the target recommendations. Challenges include difficulties in increasing the knowledge, skills, and motivation of relevant organizations and stakeholders to add new ways of action to their normal routine. There is a need to keep on doing the things that facilitated success and avoid the causes of the challenges. The commitment of the government to implementing the recommendations for agricultural and food systems that improve nutrition remains strong and the government will continue to try different methods to increase required knowledge, skills, and motivation. Background: Inadequate coordination mechanisms and capacity to coordinate are limiting factors for maximizing the ability of agriculture to improve nutrition in Sub-Saharan Africa. Effective coordination requires the availability of a platform for stakeholder convening, planning, operationalization of ideas, communication, and accountability. Nigeria's Federal Ministry of Agriculture and Rural Development established one such platform to support the institutionalization of nutrition-sensitive agriculture. Platform members include several departments within the Ministry, other ministries, and development partners. While the platform achieved key milestones and fostered collaboration, some gaps remained. Objective: This study reports an assessment to understand the perspectives of members of the coordination platform and identify ways of increasing effectiveness. Methods: Desk reviews of relevant documents and 18 key informant interviews were conducted. Documents and interview notes were coded and analysed to identify recurring themes. Themes were appraised using a nutrition coordination framework. Results: Sufficiently understanding the nutrition role of representatives' own organization/department and the purpose of the coordination platform and its activities was important for success. The profile and seniority of representing officers also mattered. While the leadership of the Ministry was committed to advancing nutrition through agriculture, the coordination platform could improve its functionality through consistent leadership, increased seniority of member representatives, and appropriate communication. Conclusions: Multisectoral coordination platforms are necessary but do not alone achieve nutrition coordination. Effective leadership and investments in time, strategic orientation, and training are critical to achieving a shared purpose, individual sector fulfilment of nutrition roles, and additional coordination success factors. The research found that there was strong interest to participate among all stakeholders, and the platform was useful for the Ministry to increase awareness and support for nutrition-sensitive agriculture. Still, the common vision around the platform's objectives, among the stakeholders involved was insufficiently articulated and communicated. Actions meeting attendees were required to take often differed from their departmental/organizational mandate and/or were not in their technical area of competence. Participating in activities related to the platform, therefore, increased the workload of its attendees. Furthermore, communication about the roles and expectations of the stakeholders involved in the platform was inadequate. The stakeholders additionally had inadequate clarity about the leadership of the platform. Guidance from a framework for nutrition coordination developed by other authors suggests that improving leadership; creating a shared understanding of the objectives of the platform among all stakeholders; increasing nutrition capacity among all organizations involved; and ensuring that involved organizations find the collaboration to be beneficial for their own mandate; will be important next steps for achieving the purpose of collaboration. Background: Renewed efforts by the Nigerian government to address malnutrition have led to nutrition actions by several sectors, including the agriculture sector. However, the success of these actions depends on the characteristics of the stakeholders involved, including their relationships and coordination. Objective: This article reports a 2015 study of nutrition-sensitive agricultural stakeholders in Nigeria that assessed what the stakeholders do, where they work and how they are organized to improve nutrition. The study provides a baseline for assessing progress and measuring stakeholder and coordination changes in the Nigerian nutrition-sensitive agriculture landscape. Methods: Semi-structured interviews (n = 17) and focus group discussions (n = 2) were held with federal, state, and local government level stakeholders; reviews of stakeholder program documents were also conducted. Results: The study identified 7 groups of nutrition-sensitive agriculture stakeholders and several coordination challenges. Political leadership, advocacy and provision of material and human resource support by nongovernmental organizations, and donor interest and funding have been vital for mobilizing nutrition-sensitive agriculture. Still, although stakeholders frequently highlighted that nutrition was an important consideration in their interventions, nutrition goals and activities and/or indicators to measure outcomes were not regularly communicated. Also, while coordination mechanisms existed, there appeared to be minimal actual cross-sectoral partnerships because of inadequate trust, competition, and conflicts over institutional turf and mandates. Conclusions: Needed enablers for improving nutrition-sensitive agriculture in Nigeria included improved stakeholder nutrition literacy, as well as enhanced stakeholder engagement facilitated by role definition, clarification, and consensus. 'Social investment' as an idea to justify social policy reforms has become more and more accepted in recent years and has decisively shaped agenda setting and policy formulation in European welfare states. The effectiveness of this new welfare state model, however, depends highly on the capacity to provide social services. Social services – job training, counselling and support for care work – are a key component of the social investment model. Drawing on the policy capacity approach, the article provides an analytical framework to study the 'operational core' of the social investment state. This implementation perspective allows us to assess whether governance actors actually have the resources to fulfil the social investment idea of enhancing citizens' freedom to act. Empirically, the article concentrates on two selected European welfare states, Germany and France, countries with similar welfare systems but very different politico-administrative systems, and on two fields of social service provision that are addressed differently in the social investment debate: early childhood education and care (ECEC) and elderly care. Empirically, we use systematic content analysis to intensively study policy documents and secondary analyses. What are the geometric and information processing characteristics of elementary figures composed of simple physical dimensions? There have been a number of investigations of perception of rectangles, including debate about configurality (e.g., integrality and gestalt properties) as well as the prime perceptual dimensions. Yet, because of ambiguity even in the "right" definition of configurality and an absence of penetrating methodologies, there is still little known concerning the information processing of these patterns. To this end, the present study brings together two separate theory-driven methodologies, general recognition theory (GRT) and systems factorial technology (SFT). The first attacks the problem of dimensional interactions while the latter seeks to uncover process characteristics such as architecture, decisional stopping rules, and workload capacity. The same observers and as much as possible, the same stimuli were used in both approaches. Through our GRT analyses, we found strong evidence for dependencies between the percepts of height and width on both within-stimulus and cross-stimulus bases. Height perception was better with narrow widths and width perception was superior with short heights. In addition, a significant positive within-trial correlation of dimensions was evidenced within squares but not with rectangles. Our SFT initiative uncovered consistent signatures of parallelism paired with super capacity, the latter appearing both through the traditional conditioning on being correct and still present when modest speed accuracy trade-off was accounted for. Thus, the SFT and GRT inferences were quite compatible with a plausible cause of the positive correlations being across-channel facilitatory interactions which led to super capacity processing. Contemporary research mainly relates police ineffectiveness in sectarian crimes prevention-investigation and enforcement of anti-terror laws to disorganized police infrastructure, a shortage of manpower, a lack of professionalism and limited access to modern equipment. This article, by applying capacity (i.e. manpower, resources, professional and technical training, etc.) and autonomy (effective and neutral exercise of powers within mandates delegated to them) concepts, aims to examine police effectiveness in the enforcement of anti-sectarian laws in the context of interference in their functions by the political executive and the State's Army. It addresses the question: How do police capacity and autonomy issues affect police effectiveness in prevention-investigation of sectarian crimes and enforcement of anti-terror laws? This article applies a mixed method research approach by using data from books, journal articles, newspapers, TV talk shows, online sources and interviews. It relates the police's effectiveness in prevention-investigation of sectarian crimes and enforcement of anti-terror laws to their capacity and autonomy issues in the Punjab Province of Pakistan. It argues that the police due to capacity and autonomy issues cannot exercise delegated powers impartially, which affects their effectiveness in prevention-investigation of sectarian crimes and enforcement of anti-terror laws. Objective: The development of new injectable antiretroviral therapy facilitates treatment for Human immunodeficiency virus (HIV) positive individuals who lack capacity to consent, posing a safety risk to both themselves and the wider community. We consider pathways to enforce treatment and propose an algorithm to determine the most appropriate legal instrument for application. Conclusion: Legislative safeguards in mental health and guardianship legislation provide oversight and protection for those who suffer from illness and require coercive treatment. Objectives: We compared the quality of the written informed consent forms for electroconvulsive therapy (ECT) in Australian jurisdictions. Method: For this comparative audit-type study, a checklist was developed to compare informed consent forms from different jurisdictions. The main information sources for consent forms were government health department websites and Google. The directors of clinical services were contacted if a consent form was not available through a web source. Results: Majority of the informed consent forms covered information about ECT, general anaesthesia and alternative treatments, supports available for decision making, and a reference to the right to withdraw consent. Missing information affected information areas such as likely outcome if no ECT, lack of guaranteed response and cultural and linguistic supports. We study the optimal design of reservations for a firm with limited capacity. The firm faces a random number of customers, each of whom has a random valuation for service. The reservation policy has two components: pricing and overbooking. For the former, the firm charges a reservation fee (at the time of reservation) and a service price (at the time of service). For the latter, the firm imposes a booking limit that caps the number of reservations it sells. Given the firm's reservation policy, customers make reservations in advance and later decide whether to show up. Denying service to reservation holders is costly. We obtain the following equilibrium results. First, when demand is small relative to capacity, the firm's pricing structure relies on reservation fees prepaid in advance, but when demand is large relative to capacity, it relies on payment received upon service. Second, when demand is low and/or predictable, the firm accepts all reservation requests, but when demand is high and/or variable, the firm uses a booking limit. [ABSTRACT FROM AUTHOR]

Evidence-based decision-making is less common in low- and middle-income countries where the research capacity remains low. Nepal, a lower-middle-income country in Asia, is not an exception. We conducted a rapid review to identify the trend of health research in Nepal and found more than seven-fold increase in the number of published health-related articles between 2000 and 2018. Recent research indicates the need to understand the role financial resources play in election administration. A key question is, when considering "financial resources," how much does economic hardship affect the differences between the budgeted amount and the actual amount spent?. The limited research that has examined this question comes from the United Kingdom; it shows that there are significant differences between the two measures and they vary systematically based upon fiscal environmental conditions. This research examines whether the fiscal environment influences election administration budgets, spending, and the resulting budget variance in local US jurisdictions. Using county election administration spending data from four states, this research indicates election administration budgets, spending, and variances are related to the fiscal environment. Not only does this work have implications for measurement of election cost, but this work is key to understanding the financial situation election administration faces given pandemic-related economic woes. With a growing ageing population and increased life expectancy in the UK, oral healthcare professionals will be exposed to a greater number of patients with health conditions which may affect cognitive function, communication and capacity to consent to treatment. Assessing patient capacity is encountered routinely in dental practice and so oral healthcare professionals should be well informed of their responsibilities in this context. Capacity-building initiatives are popular among nongovernmental organizations (NGOs) worldwide. In response to a lack of valid and reliable capacity measures for NGOs working on various social issues, Shumate and colleagues developed an 8-dimension, 45-item NGO capacities instrument, based on data from U.S. NGOs. However, the proliferation of international research on NGO capacity raises questions about the degree to which such an instrument equally applies in other countries. To allow meaningful comparisons of NGO capacities across countries, we examine the measurement equivalence of this NGO capacity instrument across a matched sample of Chinese (N = 119) and U.S. (N = 150) NGOs. Findings suggest a new NGO capacity instrument, which comprises seven factors and 28 items and better accounts for the capacity of both Chinese and U.S. NGOs. There is a dearth of knowledge concerning statutes and practices designed to protect younger children from formal juvenile court petitions. This case study examines minimum age laws and related statutes in the six largest U.S. states and explores implementation of these policies and practices in major metropolitan areas within these states. The primary study methods included legal analysis and stakeholder interviews. Legal analysis identified diverse minimum age laws across states and complex statutes related to children's capacity to stand trial and competency. Stakeholders across the six cases agreed that the state should protect young children from formal delinquency petitions when possible. Several U.S. states are considering setting or raising a minimum age of juvenile court jurisdiction. However, there is scant evidence to suggest if a state minimum age law would protect children from developmentally inappropriate proceedings beyond existing capacity and competency statutes. To address this central question, this case study focuses on the state of California and considers (a) existing state laws, (b) state juvenile crime data, and (c) opinions of diverse juvenile justice stakeholders. Triangulated analysis found that a low number of California children below the age of 12 years are petitioned in juvenile court and most are referred for misdemeanor or status offenses. In recent years, there have been national and international policy advances around capacity and decision-making and an apparent burgeoning rights-based approach to the issue, all of which have the potential to impact on the experience for people with dementia in Ireland. There is little evidence however on whether these policies and principles are being translated into practice and whether traditional paternalistic approaches to decision-making are being challenged. To gain insight into current practice, research was undertaken with social workers working with older people in Ireland; reporting on the involvement of people living with dementia in care-planning processes. Data collection included a mixed method approach; an on-line survey of social workers from across the country who reported on their open caseload during the month of June 2015 (N = 38 social workers reporting on the experiences of 788 older people, of which 39% of older people had a formal diagnosis of dementia). In addition, semi-structured telephone interviews were conducted with social workers working in the nine Community Health Organisation areas (N = 21). Findings show that people with dementia were high users of social work services, accounting for 44.5% of the client group. Social workers reported that there were no standardised approaches to how Health and Social Care Professionals involved people with dementia in care planning and decision-making. Overall, people with dementia were more likely to be excluded from decision-making processes due to (i) assumptions that they lacked capacity, (ii) family members preferences that the person was not involved, (iii) communication difficulties, (iv) time constraints, (v) little or no opportunity given or (vi) the person delegated decision-making to others. Good practices were identified through multidisciplinary team approaches and formal care planning meetings. This research highlights variability in how people with dementia participate in decision-making around their care. It sheds light on existing barriers which challenge the full implementation of the Irish Assisted Decision-Making legislation; highlighting the need for appropriate guidance and education for Health and Social Care Professionals. The findings also show that family dynamics and existing relationships can play a role in how people with dementia participate and are involved. To ensure consistent opportunities for participation, effective practices and approaches to supporting the participation of people living with dementia in care planning needs to be developed and rolled out in all care settings through increased training and adoption of standardised approaches. Patients with Alzheimer's disease and other types of dementia with acute medical problems, who have lost capacity and are without advance directives, are at risk of being over treated inhospitals. To deal with this growing demographic and ethical crisis, patients with dementia need to plan for their future medical care while they have capacity to do so. This article will examine the role of each member of the dementia care triad and how to empower the patient to participate in planning future medical care. A case will be made that physicians have the same professional disclosure obligations to dementia patients as they do to all other capable patients with terminal illnesses. Because there is little consensus about what facts should be included in a diagnostic disclosure, this article will offer a proposal to empower newly diagnosed patients with dementia with capacity to plan for their future medical care. 'Forced care' describes the provision of personal care to an individual who does not have the capacity to make a decision about that care and resists receiving that care. This study explored the views of clinical psychologists on supporting staff involved with forced care and considered the following question: Do clinical psychologists feel that they have a role in guiding decisions around forced care, and if so, what? Interview data were gathered from five clinical psychologists experienced in the field of Older Adult psychology in the UK. This study concludes that the environment in which psychologists work influences the psychologist's beliefs about whether psychologists should be involved with making recommendations about forced care, and how they should be involved. Objective: It is increasingly recognised that persons with mental illness experience physical health issues at greater rates than the general population and that there are significant barriers to accessing appropriate treatment. One less obvious barrier to appropriate care may be the law. This review examines the legal regimes within Australia and New Zealand that regulate consent for medical and surgical treatment for persons detained under mental health legislation. The review begins with a brief overview of concepts of consent and capacity then examines the law with regards to consent for non-psychiatric treatment for persons detained in psychiatric facilities. The complexity and cross-jurisdictional consistency is considered and potential future directions and possibilities for reform are discussed.Conclusion: Examination of the different laws regarding consent for medical or surgical treatment for persons admitted to psychiatric facilities are complex and demonstrate lack of consistency across jurisdictions. Cloud services have grown rapidly in recent years as a new form of service, but there exists scant research on the competition and service offering strategies of cloud service providers. We study these strategic choices by considering a setting where an incumbent cloud service provider may offer two service classes (premium and standard) and an entrant provider may offer the standard service. We analyze two competitive strategies the incumbent provider: (a) deterring competitor entry, and (b) not deterring competitor entry. We also evaluate two service offering strategies of the incumbent provider: (a) offering both the premium and the standard service, and (b) offering the premium service only. Based on the results of our model, we present several useful managerial insights. First of all, we find that, under monopoly setting, it may sometimes be beneficial for the service provider to encourage higher usage despite the extra cost. Further, in the duopoly setting, higher customer usage impacts the Deter/Allow strategy choice and is neither always beneficial nor always detrimental to the incumbent provider, and such impact depends on profit margin and customer usage distribution. We also find that a larger market encourages the incumbent provider to add the standard service and switch between Allow and Deter strategies, since a larger market size may reduce the profit of an incumbent provider sticking to the Deter strategy. The depletion of non-renewable energy resources, has simultaneously increased the demand for energy storage system like battery. Many efforts have been made to improve the performance of battery such as high concentration of electrolyte, high power density of electrodes and high ion selective separator or membrane. The application of a biomass-derived as a separator have emerged as tantalising alternatives to liquid and synthetic polymer electrolyte for battery. The motivation is clear because of the corrosive, hazardous and leakage of liquid electrolyte usage; non-biodegradable and insolubility of synthetic polymer electrolyte in certain solvents. Due to environmental concern and sustainability, red seaweed-pulp is a good candidate to be used as a separator for battery. The SEM image revealed that morphology of red seaweed consists of 20-30% solid material (endofibre), which consists of small, regular and hollow fibre. These characteristics enable the pulp to absorb and retain the wettability of electrolyte and contribute high ion conductivity of battery. In this study, novel battery, a separator made from red seaweed-pulp, is comprised of aluminium plate (anode), graphite plate (cathode), and sodium chloride (electrolytes) that have been developed. This red seaweed pulp-based battery has shown a very good electrochemical potential from 2.5 to 3.0 V, maximum capacity up to 6 mAh, and the charge-discharge can withstand up to 10 cycles. Despite many attempts to reduce health disparities, health professionals face obstacles in improving poor health outcomes within the African American (AA) community. To promote change for improved health measures, it is important to implement culturally tailored programming through a trusted institution, such as the AA church. While churches have the potential to play an important role in positively impacting health among AAs, it is unclear what attributes are necessary to predict success or failure for health promotion within these institutions. The purpose of this study was to create a model, the Brand's PREACH [Predicting Readiness to Engage African American Churches in Health) Model, to predict the readiness of AA churches to engage in health promotion programming. Thirty-six semistructured key informant interviews were conducted with 12 pastors, 12 health leaders, and 12 congregants to gain information on the relationship between church infrastructure (physical structure, personnel, funding, and social/cultural support), readiness, and health promotion programming. The findings revealed that church infrastructure has an association with and will predict the readiness of a church to engage in health promotion programming. The ability to identify readiness early on will be useful for developing, implementing, and evaluating faith-based interventions, in partnership with churches, which is a key factor for sustainable and effective programs. The extent to which dementia affects a person's testamentary capacity has been the subject of much litigation with some countries introducing legal tests to assess capacity. In light of substantial societal change in Albania in the last two decades and an epidemic of property litigation, Albanian legal practice is witnessing an increasing number of attempts to posthumously nullify wills. Plaintiffs are mainly relatives of the deceased testator who are unhappy with the quantity or quality of the property they have inherited. Based on plaintiffs' claims, solicitors may request expert neuropsychiatric reviews postmortem, often basing their position on prescription drug use by the testator during his/her last years of life. Improving low-achieving schools is a critical challenge facing urban education. Recent national policy shifts have pressed states to take an expanded role in school improvement efforts. In 2009, a federal grant competition called Race to the Top (RttT) compelled states to improve their capacity to implement ambitious education reform agendas. Drawing on the theory of organizational capacity, the study sampled five RttT winning states' plans to support improving low-achieving schools. In this article, we study the electricity time-of-use (TOU) tariff for an electricity company with stochastic demand. The electricity company offers the flat rate (FR) and TOU tariffs to customers. Under the FR tariff, the customer pays a flat price for electricity consumption in both the peak and non-peak periods. Under the TOU tariff, the customer pays a high price for electricity consumption in the peak period and a low price for electricity consumption in the non-peak period. The electricity company uses two technologies, namely the base-load and peak-load technologies, to generate electricity. We derive the optimal capacity investment and pricing decisions for the electricity company. Furthermore, we use real data from a case study to validate the results and derive insights for implementing the TOU tariff. We show that in almost all the cases, the electricity company needs less capacity for both technologies under the TOU tariff than under the FR tariff, even though the expected demand in the non-peak period increases. In addition, except for some extreme cases, there is essentially no signicant reduction in the total demand of the two periods, although the TOU tariff can reduce the demand in the peak period. Under the price-cap regulation, the customer may pay a lower price on average under the TOU tariff than under the FR tariff. We conduct an extensive numerical study to assess the impacts of the model parameters on the optimal solutions and the robustness of the analytical results, and generate managerial implications of the research findings. We analyze the role of pricing and branding in an incumbent firm's decision when facing competition from an entrant firm with limited capacity. We do so by studying two price competition models (Stackelberg and Nash), where we consider the incumbent's entry-deterrence pricing strategy based on a potential entrant's capacity size. In an extension, we also study a branding model, where the incumbent firm, in addition to pricing, can also invest in influencing market preference for its product. With these models, we study conditions under which the incumbent firm may block the entrant (i.e., prevent entry without any market actions), deter the entrant (i.e., stop entry with suitable market actions) or accommodate the entrant (i.e., allow entry and compete), and how the entrant will allocate its limited capacity across its own and the new market, if entry occurs. We also study the timing difference between the two different dynamics of the price competition models and find that the incumbent's first-mover advantage benefits both the incumbent and the entrant. Interestingly, the entrant firm's profits are not monotonically increasing in its capacity even when it is costless to build capacity. In the branding model, we show that in some cases, the incumbent may even increase its price and successfully deter entry by investing in consumer's preference for its product. Finally, we incorporate demand uncertainty into our model and show that the incumbent benefits from demand uncertainty while the entrant may be worse off depending on the magnitude of demand uncertainty and its capacity. Effective planning for community health partnerships requires understanding how initial readiness—that is, contextual factors and capacity—influences implementation of activities and programs. This study compares the context and capacity of drug and violence prevention coalitions in Mexico to those in the United States. Measures of coalition context include community problems, community leadership style, and sense of community. Measures of coalition capacity include the existence of collaborative partnerships and coalition champions. The assessment was completed by 195 members of 9 coalitions in Mexico and 139 members of 7 coalitions in the United States. Psychometric analyses indicate the measures have moderate to strong internal consistency, along with good convergent and discriminant validity in both settings. Results indicate that members of Mexican coalitions perceive substantially more serious community problems, especially with respect to education, law enforcement, and access to alcohol and drugs. Compared to respondents in the United States, Mexican respondents perceive sense of community to be weaker and that prevention efforts are not as valued by the population where the coalitions are located. The Mexican coalitions appear to be operating in a substantially more challenging environment for the prevention of violence and substance use. Their ability to manage these challenges will likely play a large role in determining whether they are successful in their prevention efforts. While adults are assumed to have the capacity to consent to medical research, and young children to have no capacity, adolescents’ capacity to consent is not well described. Adapting the MacArthur Competence Assessment Tool for Clinical Research (MacCAT-CR), we describe adolescents’ capacity to consent to medical research and factors influencing that capacity. Our pilot study included a community-based sample of 30 adolescents, 14 to 21 years of age, who completed the MacCAT-CR after undergoing a simulated informed consent process. We found that adolescents’ capacity to consent to research was associated with age, health literacy, and family affluence. These findings suggest that investigators and institutional review boards should be aware that factors other than age may influence capacity to consent, and, for modifiable factors, such as health literacy, consent processes for medical research with adolescents can be modified. Firms mitigate uncertainty in demand and supply by carrying safety stock, planning for excess capacity and diversifying supply sources. In this study, we provide a framework to jointly optimize these three levers in a periodic review infinite horizon setting, and in particular we examine how one can reduce inventory and capacity investments through proper diversification strategies. Observing that a modified base-stock inventory policy is optimal, we find that the capacity-diversification problem is well behaved and characterize the optimal mix of safety stock, excess capacity and extra number of supply sources. We find that higher supply uncertainty results in higher safety stock, more excess capacity, and higher diversification. But safety stock and diversification are non-monotonic in demand uncertainty. Background: There is growing recognition that "nutrition-sensitive" development is necessary to ensure nutrition security and reduce malnutrition. While agriculture has the potential to be a strong driver of malnutrition reduction and serves as the main source of livelihood for approximately two-thirds of East Africa's population, its potential to reduce malnutrition is currently not being realized.Objective: Leveraging Agriculture for Nutrition in East Africa is a research study based in Ethiopia, Kenya, and Uganda that seeks to understand the enabling environment necessary for optimizing the contribution of the food and agriculture sector to nutrition outcomes. Its objectives were to explore stakeholder perceptions of nutrition-agriculture linkages, of political and institutional challenges and opportunities, of evidence that is available and influential for policy making, and of key issues with regard to capacity.Methods: Open-ended and semistructured interviews were conducted with 53 stakeholders from government, civil society, donors, United Nations organizations, private sector, and research/academic institutions in Ethiopia, Kenya, and Uganda in 2014.Results: Although policy opportunities and contexts are different between the 3 countries, stakeholders identified similar barriers to greater action, including a narrow focus on solely market-oriented and staple crop production, a lack of clarity and incentives within the agriculture sector about improving nutrition and how to translate policy into action, and lack of capacity in human and financial resources. Many actions to improve the nutrition sensitivity of agriculture were mentioned, including crop diversification, value chain activities and improved market access, nutrition education, and reduction in time and labor costs to women.Conclusion: Many opportunities exist to strengthen the impact of agriculture on nutrition in East Africa, but stronger formulation and implementation of policies will require adequate human resources, funds, timely data on the context, sector alignment on priority actions, and alignment on a framework or indicators for accountability. Allocentric spatial memory, the memory for locations coded in relation to objects comprising our environment, is a fundamental component of episodic memory and is dependent on the integrity of the hippocampal formation in adulthood. Previous research from different laboratories reported that basic allocentric spatial memory abilities are reliably observed in children after 2 years of age. Based on work performed in monkeys and rats, we had proposed that the functional maturation of direct entorhinal cortex projections to the CA1 field of the hippocampus might underlie the emergence of basic allocentric spatial memory. We also proposed that the protracted development of the dentate gyrus and its projections to the CA3 field of the hippocampus might underlie the development of high-resolution allocentric spatial memory capacities, based on the essential contribution of these structures to the process known as pattern separation. Here, we present an experiment designed to assess the development of spatial pattern separation capacities and its impact on allocentric spatial memory performance in children from 18 to 48 months of age. We found that: (1) allocentric spatial memory performance improved with age, (2) as compared to younger children, a greater number of children older than 36 months advanced to the final stage requiring the highest degree of spatial resolution, and (3) children that failed at different stages exhibited difficulties in discriminating locations that required higher spatial resolution abilities. Background: Almost half of all children in South Asia are stunted. Although agriculture has the potential to be a strong driver of undernutrition reduction and serves as the main source of livelihood for over half of South Asia's population, its potential to reduce undernutrition is currently not being realized. Objective: The Leveraging Agriculture for Nutrition in South Asia (LANSA) research consortiumseeks to understand how agriculture and agrifood systems can be better designed to improve nutrition in South Asia. In 2013 and 2014, LANSA carried out interviews with stakeholders influential in, and/or knowledgeable of, agriculture-nutrition policy in India, Pakistan, and Bangladesh, togain a better understanding of the institutional and political factors surrounding the nutrition sensitivity of agriculture in the region. Methods: Semistructured interviews were carried out in India, Bangladesh, and Pakistan with a total of 56 stakeholders representing international organizations, research, government, civil society, donors, and the private sector. Results: The findings point to mixed perspectives on countries' policy sensitivity toward nutrition. There was consensus among stakeholders on the importance of political commitment to nutrition, improving nutrition literacy, strengthening capacities, and improving the use of financial resources. Conclusions: Although there are different ways in which South Asian agriculture can improve its impact on nutrition, sensitizing key influencers to the importance of nutrition for the health of a country's population appears as a critical issue. This should in turn serve as the premise for political commitment, intersectoral coordination to implement nutrition-relevant policies, adequately resourced nutrition-specific and nutrition-sensitive programs, and sufficient capacities at all levels. This article examines the implementation of new special education legislation in Finland among students with significant disabilities. The data consist of a nationwide survey and field observations. In the analysis, a policy implementation framework is utilized as a theoretical lens. The findings suggest that schools and municipalities that had participated in long-term development projects had more capacity and willingness to reform special education practices substantially, suggesting that learning in one policy area can transfer to other domains across multiple levels of national and local governments. How should a firm with limited capacity introduce a new product? Should it introduce the product as soon as possible or delay introduction to build up inventory? How do the product and market characteristics affect the firm's decisions? To answer such questions, we analyze new product introductions under capacity restrictions using a two-period model with diffusion-type demand. Combining marketing and operations management decisions in a stylized model, we optimize the production and sales plans of the firm for a single product. We identify four different introduction policies and show that when the holding cost is low and the capacity is low to moderate, a (partial) build-up policy is indeed optimal if consumers are sensitive to delay. Under such a policy, the firm (partially) delays the introduction of its product and incurs short-term backlog costs to manage its future demand and total costs more effectively. However, as either the holding cost or the capacity increases, or consumer sensitivity to delay decreases, the build-up policy starts to lose its appeal, and instead, the firm prefers an immediate product introduction.
