42. Valvular Heart Disease in Women.

Abstract

Valvular heart disease is an important clinical problem in women as a number of congenital and acquired diseases affecting the cardiac valves occur with greater frequency in women. Women also have differences in physiognomy and physiology that influence the clinical presentation, diagnostic approach, therapeutic options, and results of therapy. Women, on average, tend to be smaller than men. This not only applies to their external dimensions, but also to the dimensions of the cardiovascular system. For example, the heart of a normal woman weighs on average approximately 100 g less than men1 (Robbins, Pathology, 3rd ed. W.B. Saunders Company, Philadelphia, 1967, p. 512). They experience differences in cardiovascular remodeling in response to the hemodynamic alterations that result from valvular dysfunction, physiologic changes that accompany a normal pregnancy and the pathologic effects of pregnancy-related complications can deleteriously accelerate valve-related hemodynamic alterations and symptoms of valve dysfunction. In this chapter, we will define those areas of valvular heart disease than directly applies to women, and identify those gender-related issues in the diagnosis, evaluation, and treatment of valve disease. 44. A New Method to Find Developmental Descriptions for Digital Circuits.

Abstract

In this paper we present a new method to find developmental descriptions for gate-level feed forward combinatorial circuits. In contrast to the traditional description of FPGA circuits in which an external bit stream explicitly describes the internal architecture and the connections of the circuit, developmental descriptions form the circuit by synchronously running an identical developmental program in each building block of the circuit. Unlike some previous works, the connections are all local here. Evolution is used to find the developmental code for the given problem. We use an innovative fitness function to increase the performance of evolution in search for the solutions, and also relax the position and order of the inputs and output(s) of the circuit to increase the density of the solutions in the search space. The results show that the chance of finding a solution can be increased up to 375% compared to the use of traditional fitness function. The preliminary studies show that this method is capable of describing basic circuits and is easily scalable for modular circuits. [ABSTRACT FROM AUTHOR]

Abstract

Since its introduction into clinical practice in 1963 by Starzl et al., the technique of orthotopic liver transplantation has been refined progressively. The original technique included resection of the recipient inferior vena cava (IVC) and the use of extracorporeal venovenous bypass. More confidence with the technique and the more frequent use of technical variants in pediatric liver transplantation have led to the development of recipient cava-preserving hepatectomy techniques without use of venovenous bypass and with or without use of a temporary portocaval shunt, independently of anatomical and general status of the recipient. Regardless of the exact technique, orthotopic liver transplantation is characterized by three stages: the pre-anhepatic, the anhepatic and the post-anhepatic stage.
