1. The Gender Gap in Educational Outcomes in Norway. OECD Education Working Papers, No. 183

Abstract

As is the case in most OECD countries, boys in Norway are more likely to have lower levels of academic achievement and attainment than girls. While this phenomenon is not recent, it has become increasingly pronounced in recent years and, as a result, is attracting considerable attention from policy-makers in many countries. This paper develops evidence of gender gaps in educational outcomes in Norway and selected OECD countries and identifies examples of policies and practices that could help close existing gender gaps in Norway. The first part of the paper describes gender gaps in school achievement, attainment, attitudes, beliefs and behaviours using an international comparative analysis. Evidence from PIRLS, TIMSS, PISA and the OECD Survey of Adult Skills (PIAAC) is used to identify gender gaps during primary and secondary schooling as well as young adulthood. The second part of the paper summarises evidence on policies and practices that were implemented in other countries and that could support efforts in Norway to mitigate, prevent and reduce gender gaps in achievement and attainment. Most of the evidence on policies and practices reviewed in the report comes from the peer countries Finland, the Netherlands and the United States that were identified of particular relevance for Norway, given the policy challenge Norway faces. 3. Revisiting the upper Visean mud mounds from Derbyshire (UK): the role of brachiopods in their growth

Abstract

Several brachiopod-rich mud mounds occur in the upper Visean (Brigantian) of the Derbyshire Carbonate Platform succession in UK. The re-evaluation of the lithofacies architecture of a Derbyshire mud mound complex, developed in an intraplatform middle-ramp environment, led to the recognition of three lithofacies associations: (a) a 10 m thick basal unit of automicrite boundstone with siliceous sponge spicules and brachiopod–bryozoan packstone to wackestone beds; (b) a 10 m thick, 250 m wide, lens-shaped, convex-up massive core of clotted peloidal micrite and fenestellid bryozoan boundstone with sponge spicules; (c) inclined brachiopod–bryozoan–crinoid packstone flank beds. In the mud mound complex core, most of the carbonate mud with clotted peloidal and structureless micrite fabric is the result of biologically induced and influenced in-situ precipitation processes (automicrite). Brachiopods are not, as previously thought, limited to storm-scoured “pockets” in the mud mound complex core but are abundant and diverse in all lithofacies and lived on the irregular mud mound complex surface concentrating in depressions sustained by automicrite boundstone and the growth of bryozoans and sponges. The upper Visean Derbyshire mud mounds are, thus, representatives of a newly defined fenestellid bryozoan–brachiopod–siliceous sponge mud mound category, occurring in various middle–upper Visean Western European sites, a sub-type of the fenestellid bryozoan–crinoid–brachiopod Type 3 buildups of Bridges et al. (1995). These mud mounds, and other types of brachiopod-rich buildups, developed in carbonate platform settings between fair-weather and storm wave base, in dysphotic environments with dispersed food resources during the Visean. Brachiopod mud mound colonisation was favoured by moderate water depth, availability of food resources, and diverse substrates.

Abstract

Background: Recent guidelines suggested a wider use of long-acting injectable antipsychotics (LAI) than previously, but naturalistic data on the consequences of LAI use in terms of discontinuation rates and associated factors are still sparse, making it hard for clinicians to be informed on plausible treatment courses. Objective: Our objective was to assess, under real-world clinical circumstances, LAI discontinuation rates over a period of 12 months after a first prescription, reasons for discontinuation, and associated factors. Methods: The STAR Network ‘Depot Study’ was a naturalistic, multicentre, observational prospective study that enrolled subjects initiating a LAI without restrictions on diagnosis, clinical severity or setting. Participants from 32 Italian centres were assessed at baseline and at 6 and 12 months of follow-up. Psychopathology, drug attitude and treatment adherence were measured using the Brief Psychiatric Rating Scale, the Drug Attitude Inventory and the Kemp scale, respectively. Results: The study followed 394 participants for 12 months. The overall discontinuation rate at 12 months was 39.3% (95% confidence interval [CI] 34.4–44.3), with paliperidone LAI being the least discontinued LAI (33.9%; 95% CI 25.3–43.5) and olanzapine LAI the most discontinued (62.5%; 95% CI 35.4–84.8). The most frequent reason for discontinuation was onset of adverse events (32.9%; 95% CI 25.6–40.9) followed by participant refusal of the medication (20.6%; 95% CI 14.6–27.9). Medication adherence at baseline was negatively associated with discontinuation risk (hazard ratio [HR] 0.853; 95% CI 0.742–0.981; p = 0.026), whereas being prescribed olanzapine LAI was associated with increased discontinuation risk compared with being prescribed paliperidone LAI (HR 2.156; 95% CI 1.003–4.634; p = 0.049). 6. Journey-Deuce bicompartmental knee arthroplasty with the addition of computer navigation achieves good clinical outcomes and implant survival at 10 years

Abstract

Purpose To report 10-year outcomes and survivorship in patients undergoing bicompartmental knee arthroplasty (BCKA) using the Journey-Deuce prosthesis in a consecutive prospective case series. Methods Between November 2006 and November 2009, 41 patients with a mean age of 69.6 years (range 51–86) underwent 51 bicompartmental knee arthroplasties with the Journey-Deuce knee prosthesis. All patients presented with symptomatic medial and patellofemoral compartment osteoarthritis, with intact cruciate ligaments and a preserved lateral compartment on plain radiographs and Magnetic Resonance Imaging. Clinical assessment was undertaken pre-surgery and at 1, 2, 5 and 10 years post-surgery using the Oxford Knee Score (OKS), EuroQol Group 5-Dimension self-reported questionnaire (EQ-5D) and maximal active range of motion (ROM). Results 30 patients (37 knees) were followed-up at a mean time of 11.4 years (SD 1.1; range 10.5–14.0). Eight patients (ten knees) were deceased and three could not be contacted at final review. No major component revision was performed. Pre-operative OKS 25.4 (SD 5.2; range 15–40), knee flexion 116.4° (SD 10.3°; range 100°–140°) and EQ-5D 70.5 (SD 19.9; range 25–95). 10-year OKS 43.5 (SD 4.1; range 32–48), knee flexion 127.3° (SD 11.1°; range 105°–144°) and EQ-5D 77.4 (SD 9.3; range 60–100). The OKS (p < 0.0001), EQ-5D (p = 0.024) and active knee flexion ROM (p < 0.0001) all significantly improved from pre-surgery to 1-year post-surgery, with no further significant changes in these scores between any post-operative time period up until 10 years. 32% (7/22) of tibial and 45% (10/22) of femoral components showed progressive radiolucencies between 2 and 5-year and 10-year follow-up. Conclusions This is the largest cohort of patients having undergone BCKA (with the Journey-Deuce prosthesis) with longest follow-up described in the literature. 7. Phylogeny and biogeography of the pantropical whip spider family Charinidae (Arachnida: Amblypygi)

Abstract

The present contribution addresses the phylogeny and biogeography of the pantropical whip spider family Charinidae Quintero, 1986, the most species-rich in the arachnid order Amblypygi Thorell, 1883, based on morphology and multilocus DNA sequences, analysed simultaneously using parsimony, maximum likelihood and Bayesian inference. The morphological matrix comprises 138 characters, scored for four outgroup taxa and 103 ingroup terminals representing all genera and 64% of the species of Charinidae. The multilocus dataset comprises sequences from two nuclear and three mitochondrial gene loci for four outgroup taxa and 48 ingroup representing 30 (23%) taxa of Charinidae. Charinidae are monophyletic, with Weygoldtia Miranda et al., 2018 sister to a monophyletic group comprising Charinus Simon, 1892 and Sarax Simon, 1892, neither of which are reciprocally monophyletic. Charinidae diverged from other amblypygid families in the Late Carboniferous, c. 318 Mya, on the supercontinent Pangaea. Weygoldtia diverged from the common ancestor of Charinus and Sarax during the Late Permian, c. 257 Mya, when changes in climate reduced tropical forests. The divergence of Charinus and Sarax coincides with the fragmentation of Pangaea, c. 216 Mya. Sarax colonized South-East Asia via Australia. The charinid fauna of New Caledonia originated before the Oligocene, when the island separated from Australia, c. 80 Mya.

9. Systematic revision of the pantropical whip spider family Charinidae Quintero, 1986 (Arachnida, Amblypygi)

Abstract

The whip spider family Charinidae Quintero, 1986 is the most speciose and widely distributed in the arachnid order Amblypygi Thorell, 1883. It comprises three genera and 95 species distributed across all tropical continents and the eastern Mediterranean. Despite recent advances in the taxonomy of the family, a global revision of all its species, necessary to advance understanding of its systematics, biogeography and evolution, has never been conducted. In the present contribution, the family is revised in its entirety for the first time, including all previous names and 33 new species, 24 in the genus Charinus Simon, 1892: C. alagoanus sp. nov., C. apiaca sp. nov., C. carinae sp. nov., C. carioca sp. nov., C. carvalhoi sp. nov., C. cearensis sp. nov., C. diamantinus sp. nov., C. euclidesi sp. nov., C. goitaca sp. nov., C. guayaquil sp. nov., C. imperialis sp. nov., C. loko sp. nov., C. magalhaesi sp. nov., C. miskito sp. nov., C. mocoa sp. nov., C. monasticus sp. nov., C. palikur sp. nov., C. perquerens sp. nov., C. puri sp. nov., C. renneri sp. nov., C. sooretama sp. nov., C. souzai sp. nov., C. susuwa sp. nov., C. una sp. nov.; eight in the genus Sarax Simon, 1892: S. bilua sp. nov., S. dunni sp. nov., S. gravelyi sp. nov., S. indochinensis sp. nov., S. lembeh sp. nov., S. palau sp. nov., S. rahmadii sp. nov., S. tiomanensis sp. nov.; and one in the genus Weygoldtia Miranda et al., 2018: W. consonensis sp. nov. Taxonomic keys to the 132 species (excluding four nomina dubia) are presented and several taxonomic rearrangements implemented. Four subspecies are elevated to species level: Charinus cavernicolus Weygoldt, 2006, C. elegans Weygoldt, 2006, C. longipes Weygoldt, 2006, and Sarax bispinosus (Nair, 1934). Sarax batuensis Roewer, 1962 is removed from synonymy with Sarax buxtoni (Gravely, 1915) and S. buxtoni newly synonymized with Sarax rimosus (Simon, 1901). Abstract

OBJECTIVES: Healthcare systems worldwide have been overburdened by the coronavirus disease 2019 (COVID-19) outbreak. Accordingly, hospitals had to implement strategies to profoundly reshape both non-COVID-19 medical care and surgical activities. Knowledge about the impact of the COVID-19 pandemic on cardiac surgery practice is pivotal. The goal of the present study was to describe the changes in cardiac surgery practices during the health emergency at the national level. METHODS: A 26-question web-enabled survey including all adult cardiac surgery units in Italy was conducted to assess how their clinical practice changed during the national lockdown. Data were compared to data from the corresponding period in 2019. RESULTS: All but 2 centres (94.9%) adopted specific protocols to screen patients and personnel. A significant reduction in the number of dedicated cardiac intensive care unit beds (-35.4%) and operating rooms (-29.2%), along with healthcare personnel reallocation to COVID departments (nurses-15.4%, anaesthesiologists-7.7%), was noted. Overall adult cardiac surgery volumes were dramatically reduced (1734 procedures vs 3447; P 0.001), with a significant drop in elective procedures [580 (33.4%) vs 2420 (70.2%)]. CONCLUSIONS: This national survey found major changes in cardiac surgery practice as a response to the COVID-19 pandemic. This experience should lead to the development of permanent systems-based plans to face possible future pandemics. These data may effectively help policy decision-making in prioritizing healthcare resource reallocation during the ongoing pandemic and once the healthcare emergency is over. © 2021 The Author(s). Abstract

The whip spider family Charinidae Quintero, 1986 is the most speciose and widely distributed in the arachnid order Amblypygi Thorell, 1883. It comprises three genera and 95 species distributed across all tropical continents and the eastern Mediterranean. Despite recent advances in the taxonomy of the family, a global revision of all its species, necessary to advance understanding of its systematics, biogeography and evolution, has never been conducted. In the present contribution, the family is revised in its entirety for the first time, including all previous names and 33 new species, 24 in the genus Charinus Simon, 1892: C. alagoanus sp. nov., C. apiaca sp. nov., C. carinae sp. nov., C. carioca sp. nov., C. carvalhoi sp. nov., C. cearensis sp. nov., C. diamantinus sp. nov., C. euclidesi sp. nov., C. goitaca sp. nov., C. guayaquil sp. nov., C. imperialis sp. nov., C. loko sp. nov., C. magalhaesi sp. nov., C. miskito sp. nov., C. mocoa sp. nov., C. monasticus sp. nov., C. palikur sp. nov., C. perquerens sp. nov., C. puri sp. nov., C. renneri sp. nov., C. sooretama sp. nov., C. souzai sp. nov., C. susuwa sp. nov., C. una sp. nov.; eight in the genus Sarax Simon, 1892: S. bilua sp. nov., S. dunni sp. nov., S. gravelyi sp. nov., S. indochinensis sp. nov., S. lembeh sp. nov., S. palau sp. nov., S. rahmadii sp. nov., S. tiomanensis sp. nov.; and one in the genus Weygoldtia Miranda et al., 2018: W. consonensis sp. nov. Taxonomic keys to the 132 species (excluding four nomina dubia) are presented and several taxonomic rearrangements implemented. Four subspecies are elevated to species level: Charinus cavernicolus Weygoldt, 2006, C. elegans Weygoldt, 2006, C. longipes Weygoldt, 2006, and Sarax bispinosus (Nair, 1934). Sarax batuensis Roewer, 1962 is removed from synonymy with Sarax buxtoni (Gravely, 1915) and S. buxtoni newly synonymized with Sarax rimosus (Simon, 1901). Abstract

Background For many years, long-acting intramuscular (LAI) antipsychotics have been prescribed predominantly to chronic and severe patients, as a last resort when other treatments failed. Recently, a broader and earlier use of LAIs, particularly second-generation LAIs, has been emphasized. To date, few studies attempted to frame how this change in prescribing took place in real-world practice. Therefore, this study aimed to describe the clinical features of patients prescribed with LAIs, and to explore possible prescribing differences between first- and second-generations LAIs under ordinary clinical practice in Italy. Methods The STAR Network “Depot” Study is an observational, longitudinal, multicenter study involving 35 centers in Italy. In the cross-sectional phase, patients prescribed with LAIs were consecutively recruited and assessed over a period of 12 months. Descriptive statistics and multivariable logistic regression analyses were employed. Results Of the 451 recruited patients, 61% were males. The level of social and working functioning was heterogeneous, as was the severity of disease. Seventy-two per cent of the patients had a diagnosis of the schizophrenia spectrum. Seventy per cent were prescribed with second-generation antipsychotic (SGA) LAIs (mostly paliperidone, aripiprazole and risperidone). Compared to first-generation antipsychotic (FGA) LAIs, patients prescribed with SGA LAIs were more often younger; employed; with a diagnosis of the schizophrenia spectrum or bipolar disorder; with higher levels of affective symptoms; with fewer LAI prescriptions in the past. Discussion LAIs' prescribing practices appear to be more flexible as compared to the past, although this change is mostly restricted to SGA LAIs.

The aim of the present study was to determine if stem cells derived from primitive fetal cells present in human amniotic fluid (hAFSCs) and cultured in the presence of a mixture of hyaluronic (HA), butyric (BU), and retinoic (RA) acids show a higher yield of differentiation toward the cardiovascular phenotype as compared with untreated cells. During the differentiation process elicited by exposure to HA + BU + RA, genes controlling pluripotency and plasticity of stem cells, such as Sox2, Nanog, and Oct4, were significantly downregulated at the transcriptional level. At this point, a significant increase in expression of genes controlling the appearance of cardiogenic and vascular lineages in HA + BU + RA-treated cells was observed. Abstract

The interruption of the manufacturing process of agalsidase beta has led to a worldwide shortage of this drug. In the EU, nearly all patients initially reduced their agalsidase beta dose, and many of these switched to agalsidase alfa (Replagal Shire HGT). The clinical consequences of this period of drug shortage need to be further evaluated. A gradual increase of agalsidase beta supply is now expected. This implies that patients could resume or even commence agalsidase beta treatment. Guidance for prioritization of patients is needed to support equitable distribution of agalsidase beta to EU member states. To achieve this, in absence of level I clinical evidence, a draft consensus proposal was initiated and distributed. No full consensus was achieved, as there is disagreement regarding the indications for switching patients from agalsidase alfa to agalsidase beta. Some physicians support the concept that the 1.0 mg/kg EOW dose of agalsidase beta is more effective than agalsidase alfa at 0.2 mg/kg EOW, while others believe that at recommended dose, the preparations are equivalent. In light of these difficulties and the uncertainties with respect to supply of agalsidase beta, recommendations were agreed upon by a subgroup of physicians. These current recommendations focus on prioritization of criteria indicative of disease progression.

23. Signatures of small-world and scale-free properties in large computer programs

Abstract

A large computer program is typically divided into many hundreds or even thousands of smaller units, whose logical connections define a network in a natural way. This network reflects the internal structure of the program, and defines the ``information flow'' within the program. We show that, (1) due to its growth in time this network displays a scale-free feature in that the probability of the number of links at a node obeys a power-law distribution, and (2) as a result of performance optimization of the program the network has a small-world structure. We believe that these features are generic for large computer programs. Abstract

Summary: The antiepileptic effect of allopurinol was assessed in a double‐blind, randomized, placebo‐controlled, cross‐over trial in 84 patients with epileptic seizures refractory to standard antiepileptic drugs (AEDs). During a retrospective baseline period, patients experienced at least four seizures of any type per month. The effects of allopurinol and matching placebo were examined for 4‐month periods. Allopurinol dosage was 150 mg daily for children weighing <20 kg and 300 mg daily for other patients. Efficacy analysis based on the Wilcoxon rank‐sum test was conducted for the 80 patients who completed the study. No significant period effect or treatment‐period interaction was noted. Allopurinol significantly reduced total seizures (p = 0.00S), and secondarily generalized seizures (p = 0.0015). Median seizure reduction for total seizures was 10.5 and 27.9% for secondarily generalized seizures. Subjective preferences by clinicians evaluated blindly significantly favored allopurinol. No significant change occurred in the plasma concentration of concomitant AEDs between treatment periods, but serum urate decreased by 32% during allopurinol treatment.
