Thin polypropylene fibers have been used for internal experiments in storage rings as an option for hydrogen targets. The change of the hydrogen content due to the radiation dose applied by the circulating proton beam has been investigated in the range 1 × 106–2 × 108 Gy at beam momenta of 1.5–3 GeV/c by comparing the elastic pp-scattering yield to that from inelastic p-carbon reactions. It is found that the loss of hydrogen as a function of applied dose receives contributions from a fast and a slow component.

The EDDA-detector at the cooler-synchrotron COSY/Jülich has been operated with an internal CH2 fiber target to measure proton–proton elastic scattering differential cross-sections. For data analysis knowledge of beam parameters, like position, width and angle, are indispensable. We have developed a method to obtain these values with high precision from the azimuthal and polar angles of the ejectiles only, by exploiting the coplanarity of the two final-state protons with the beam and the kinematic correlation. The formalism is described and results for beam parameters obtained during beam acceleration are given.

Die prospektive Studie untersuchte den Verlauf von Hochrisikoschwangerschaften mit zwischen 24/0 und 34/0 Schwangerschaftswochen erstmals diagnostiziertem ARED-Flow in der Arteria umbilicalis hinsichtlich ihres fetal outcome im Vergleich zu einer nach Gestationsalter gematchten Kontrollgruppe. Während der Studiendauer von 1995-1999 wurden 60 Feten aus Einlingsschwangerschaften mit ARED-Flow in der Umbilikalarterie in die Beobachtung einbezogen. Feten mit Malformationen und Chromosomenanomalien wurden ausgeschlossen. Die Beobachtung erfolgte mit wiederholten Doppler-Messungen der arteriellen und venösen Gefäße, mit CTG und mütterlichen Parametern. Eine Entbindung wurde eingeleitet, wenn sich entweder die fetalen Parameter verschlechterten (68%), wie ein Reverse Flow im Ductus venosus oder späte Dezelerationen im CTG oder eine mütterliche Indikation gegeben war (28%), wie das Auftreten einer Präeklampsie. Gemäß Protokoll wurden 50% der Feten mit ARED-Flow 6 Tage nach Erstdiagnose entbunden. Bei mütterlicher Indikation war dies bereits nach 4 Tagen, bei fetaler Indikation nach 7 Tagen der Fall. Im Ergebnis zeigte sich eine Mortalität von 38% mit 23 Verstorbenen (16 intrauteriner Fruchttod und 7 postnataler Tod), die meisten intrauterinen Fruchttode (14) traten bei einem Schwangerschaftsalter unter 29/0 SSW auf. 44 Feten wurden lebend geboren. Die lebendgeborenen Feten wurden in drei Gruppen A, B und C nach dem bei der Entbindung erreichten Schwangerschaftsalter eingeteilt. Zwischen 24/0 und 28/6 SSW (Gruppe A) betrug die Mortalität 36%, zwischen 29/0 und 31/6 SSW (Gruppe B) 10% und ab 32/0 SSW (Gruppe C) 8%. Auf der Grundlage pränatal-sonographischer Befunde werden bei schweren, nichttherapierbaren Fehlbildungen vorzeitige Schwangerschaftsbeendigungen durchgeführt. Vor diesem Hintergrund ist es unbedingt erforderlich, Qualitätskontrollen der pränatal-sonographischen Diagnostik und der Fetalpathologie vorzunehmen. Dabei liegt der Schwerpunkt in der Aufdeckung möglicher Fehlerursachen differenter Befunde zwischen der pränatal-sonographischen Diagnostik und der Fetalpathologie und deren Beseitigung. Mögliche Fehlerursachen für Fehldiagnosen werden dargestellt und kategorisiert sowie die Fehldiagnosen einer Fallgruppe nach ihren Ursachen analysiert. Anschließend werden Vorschläge zur Reduktion vermeidbarer Fehler gemacht. In der vorliegenden Arbeit wurden 119 Autopsiefälle, davon 77 % induzierte Aborte, 9 % Spontanaborte und 11 % Totgeborene an der Frauenklinik der Charité Berlin aus dem Jahr 2000 ausgewertet. Es wurden alle pränatalen sonographischen und postmortalen pathologischen Befunde der betrachteten Fälle miteinander verglichen und auf Übereinstimmung überprüft. Insgesamt wurden 801 Einzeldiagnosen untersucht, deren Auswertung in 38,75 % identische Befunde ergab. Bei 8 % dieser Befunde mit bestätigten klinisch-pränatalen Diagnosen wurden durch die Fetalautopsie wesentliche Zusatzbefunde mit möglichen Auswirkungen auf das Schwangerschaftsmanagement oder das Wiederholungsrisiko erhoben. Der Anteil an falsch-negativen Diagnosen betrug 24 % aller Befunde. After the termination of such a pregnancy an autopsy of the fetus is performed to verify the prenatal diagnosis. Sometimes remain differences between the prenatal and the postmortal examination. The sources for different diagnoses are analysed in the view to make suggestions for the reduction of avoidable mistakes. Material and methods: The here presented study offers a systematic analysis of the differences between prenatal and postmortal examination. We divided the sources of the differences into fetal, maternal, temporal, arteficial, technical and subjective factors. We examined all fetal autopsies at the institute of pathology of the Charité Berlin of the year 2000 to test the feasibility of the method. Within the retrospective study we analysed 119 autopsies of fetuses. The gestational age varied between the 13th and 36th week. Der Nutzen von Telemedizin und Telecare/Homecare ist heute nicht mehr umstritten. Es existiert eine große Anzahl von Lösungen und Konzepten. Zum Teil werden sie bereits in der Routine eingesetzt. Die Systeme sind in der Regel vom Typ "stand-alone" bzw. "Insellösung". Ansätze einer Plattformtechnologie sind bei einigen Systemen erkennbar. Gemeinsame Probleme der vorhandenen Lösungen liegen im Bereich fehlender Standardisierung von Dateninhalten und Verfahren sowie einer soliden gesetzlichen und abrechnungstechnischen Grundlage. Von diesem Zustand bis zum flächendeckenden Einsatz von Telemedizin/Telecare ist noch ein weiter Weg. Die vorliegende Arbeit beschäftigt sich vorwiegend mit einem Teilaspekt dieses breiten Themas. Es ist die Frage, wie Telemedizin und Telecare sich optimal in den Arbeitsablauf eines Allgemeinmediziners integrieren lassen. Die Analyse zeigte, dass die Probleme in folgenden Bereichen zu finden sind: * Fehlende allgemeine und internationale Telemedizin-Plattform * Fehlende zentrale elektronische Patientenakte * Unzureichende Offenheit der Praxissoftware * Unzureichende Ko-Existenz von herkömmlichen und telemedizinischen Lösungen * Unzureichende gesetzliche und abrechnungstechnische Grundlage Folgerichtig muss ein Anforderungskatalog aufgestellt werden, der hilft, diese Mängel zu beseitigen. Auf der Basis dieses Anforderungskatalogs wurde ein Konzept entwickelt, in dessen Zentrum die allgemein-medizinische Praxis steht. Im Hintergrund der Praxis ist eine auf vorhandenen Standards fußenden Plattform, die eine flächendeckende Kommunikation aller Teilnehmer ermöglicht, vom Patienten über den Arzt bis zur Fachklinik, Apotheke und Kassenärztlichen Vereinigung sowie Versicherungen. Der Anschluss der Praxissoftware an diese Plattform ist eine wesentliche Voraussetzung zur Implementierung. Ein wichtiger Bestandteil des Konzeptes ist der Vorschlag einer international anerkannten Ontologie (UMLS), die neben der Vereinheitlichung der Dateninhalte auch bei de, Today there is no question that telemedicine and telecare/homecare can provide benefit to their users. A large number of such systems are available. Partially they are used for daily works. Normally they are stand-alone, without an underlying platform. In the last few years one can recognize the beginning of a platform technology. The most common problems of existing solutions are caused by the lack of standardisation of data items and procedures, the lack of a reliable legal basis and billing possibilities. It is a long way to go to get the telemedicine/telecare running. This document describes a sub-aspect of the large topic telemedicine/telecare. It includes the question how one can integrate telemedicine/telecare into the daily workflow of a private doctor office. The system analysis shows that problems occurring during the integration can be subdivided into following topics: * Lack of a general and international telemedicine platform * Lack of a generally accepted electronic patient record * Lack of a interoperability of the medical information systems * Lack of a harmonized co-existence of telemedicine and medical works * Lack of a legal basis and billing possibilities As a consequence of the analysis` results a catalogue of requirements must be established which provides the basis for a system concept. The central point of the concept must be the doctor office. Its background should be a set of standards which establishes the platform for the communication between every participant to the telemedicine: patients, doctors, clinics, pharmacy, medical invoice organisations and insurance companies. The most essential requirement to implement this platform is the connection of the IT equipment in private doctor offices to the applications and databases of the platform. Aufgrund der rasanten technischen Entwicklung im Bereich der pränatalen Diagnostik in den letzten Jahren werden mehr angeborene Fehlbildungen zu einem früheren Zeitpunkt der Schwangerschaft und detaillierter erkannt. Das führt dazu, dass die zur Autopsie gelangenden Feten zunehmend kleiner sind und komplexere Befunde aufweisen. Gleichzeitig werden die genetischen Ursachen von mehr Erkrankungen, Fehlbildungen und Syndromen aufgedeckt, so dass die DNA-Diagnostik, Chromosomenanlyse u.a. molekulargenetische und biochemische Erkenntnisse die klinisch-genetische Diagnostik zunehmend ergänzen. Durch Festlegung einer gezielten Autopsiestrategie und Nutzung moderner Kommunikationsmittel kann der Pathologe die in den letzten Jahren gestiegenen klinischen Anforderungen erfüllen. Ein Problem ist es jedoch, dass nicht genügend Pathologen auf dem Gebiet der Fetalpathologie spezialisiert sind. Das trifft nicht nur für Deutschland zu, sondern das ist mehr oder weniger eine weltweite Situation. Ein möglicher Lösungsansatz dieses Problems wäre die Nutzung von Softwareprogrammen, die dazu dienen, unerfahrene Pathologen moderne Anleitungen während der Autopsie von Feten zu geben. Das kann mit einer telepathologischen Betreung durch einen Experten auf diesem Gebiet kombiniert werden. Darüberhinaus erlaubt die derzeitige digitalisierte Befunddokumentation den Vergleich aktueller mit gespeicherten Fällen sowie Recherchen und stellt eine gute Basis zur Erstellung von Lehrmaterial dar., Due to the enormous technical developments in recent years in the area of prenatal ultrasound more malformations are detected in greater detail and at an earlier point in time during pregnancy than in the past. As a result, fetuses of increasingly smaller size and with complex diagnoses are presented for autopsy. At the same time, the genetic causes of ever more diseases, malformations and syndromes are being discovered, so that DNA diagnostics, chromosome analysis and, among others, molecular-genetic and biochemical investigations have come increasingly to complement clinical-genetic diagnostics. Through establishment of a clinically oriented autopsy strategy and the use of modern communication media, pathologists can satisfy the increased clinical demands of recent years. The remaining problem is that we do not have enough pathologists experienced in fetal pathology. This is not only true for Germany, it is more less the situation worldwide. One means of dealing with this problem may be the use of software programs to guide inexperienced pathologists through the modern autopsy procedure. This can be combined with the use of telepathological advise given by an expert in the field. Over and above this, digitized documentation of findings can be used in research, which allows the comparison of current with previously saved cases and presents a good basis for the development of education material.

Several available codes for hadronic event generation and shower simulation are discussed and their predictions are compared to experimental data in order to obtain a satisfactory description of hadronic processes in Monte Carlo studies of detector systems for medium energy experiments. The most reasonable description is found for the intranuclear-cascade (INC) model of Bertini which employs microscopic description of the INC, taking into account elastic and inelastic pion–nucleon and nucleon–nucleon scattering. The isobar model of Sternheimer and Lindenbaum is used to simulate the inelastic elementary collisions inside the nucleus via formation and decay of the D33-resonance which, however, limits the model at higher energies. To overcome this limitation, the INC model has been extended by using the resonance model of the HADRIN code, considering all resonances in elementary collisions contributing more than 2% to the total cross-section up to kinetic energies of 5 GeV. In addition, angular distributions based on phase shift analysis are used for elastic nucleon–nucleon as well as elastic and charge exchange pion–nucleon scattering. Also kaons and antinucleons can be treated as projectiles. Good agreement with experimental data is found predominantly for lower projectile energies, i.e. in the regime of the Bertini code.

Die Systeme sind in der Regel vom Typ "stand-alone" bzw. "Insellösung". Ansätze einer Plattformtechnologie sind bei einigen Systemen erkennbar. Gemeinsame Probleme der vorhandenen Lösungen liegen im Bereich fehlender Standardisierung von Dateninhalten und Verfahren sowie einer soliden gesetzlichen und abrechnungstechnischen Grundlage. Von diesem Zustand bis zum flächendeckenden Einsatz von Telemedizin/Telecare ist noch ein weiter Weg. Die vorliegende Arbeit beschäftigt sich vorwiegend mit einem Teilaspekt dieses breiten Themas. Es ist die Frage, wie Telemedizin und Telecare sich optimal in den Arbeitsablauf eines Allgemeinmediziners integrieren lassen. Die Analyse zeigte, dass die Probleme in folgenden Bereichen zu finden sind: * Fehlende allgemeine und internationale Telemedizin-Plattform * Fehlende zentrale elektronische Patientenakte * Unzureichende Offenheit der Praxissoftware * Unzureichende Ko-Existenz von herkömmlichen und telemedizinischen Lösungen * Unzureichende gesetzliche und abrechnungstechnische Grundlage Folgerichtig muss ein Anforderungskatalog aufgestellt werden, der hilft, diese Mängel zu beseitigen. Auf der Basis dieses Anforderungskatalogs wurde ein Konzept entwickelt, in dessen Zentrum die allgemein-medizinische Praxis steht. Im Hintergrund der Praxis ist eine auf vorhandenen Standards fußenden Plattform, die eine flächendeckende Kommunikation aller Teilnehmer ermöglicht, vom Patienten über den Arzt bis zur Fachklinik, Apotheke und Kassenärztlichen Vereinigung sowie Versicherungen. Der Anschluss der Praxissoftware an diese Plattform ist eine wesentliche Voraussetzung zur Implementierung. Ein wichtiger Bestandteil des Konzeptes ist der Vorschlag einer international anerkannten Ontologie (UMLS), die neben der Vereinheitlichung der Dateninhalte auch bei de, Today there is no question that telemedicine and telecare/homecare can provide benefit to their users. A large number of such systems are available. Partially they are used for daily works. Normally they are stand-alone, without an underlying platform. In the last few years one can recognize the beginning of a platform technology. The most common problems of existing solutions are caused by the lack of standardisation of data items and procedures, the lack of a reliable legal basis and billing possibilities. It is a long way to go to get the telemedicine/telecare running. This document describes a sub-aspect of the large topic telemedicine/telecare. It includes the question how one can integrate telemedicine/telecare into the daily workflow of a private doctor office. The system analysis shows that problems occurring during the integration can be subdivided into following topics: * Lack of a general and international telemedicine platform * Lack of a generally accepted electronic patient record * Lack of a interoperability of the medical information systems * Lack of a harmonized co-existence of telemedicine and medical works * Lack of a legal basis and billing possibilities As a consequence of the analysis` results a catalogue of requirements must be established which provides the basis for a system concept. The central point of the concept must be the doctor office. Its background should be a set of standards which establishes the platform for the communication between every participant to the telemedicine: patients, doctors, clinics, pharmacy, medical invoice organisations and insurance companies. The most essential requirement to implement this platform is the connection of the IT equipment in private doctor offices to the applications and databases of the platform. Aufgrund der rasanten technischen Entwicklung im Bereich der pränatalen Diagnostik in den letzten Jahren werden mehr angeborene Fehlbildungen zu einem früheren Zeitpunkt der Schwangerschaft und detaillierter erkannt. Das führt dazu, dass die zur Autopsie gelangenden Feten zunehmend kleiner sind und komplexere Befunde aufweisen. Gleichzeitig werden die genetischen Ursachen von mehr Erkrankungen, Fehlbildungen und Syndromen aufgedeckt, so dass die DNA-Diagnostik, Chromosomenanlyse u.a. molekulargenetische und biochemische Erkenntnisse die klinisch-genetische Diagnostik zunehmend ergänzen. Durch Festlegung einer gezielten Autopsiestrategie und Nutzung moderner Kommunikationsmittel kann der Pathologe die in den letzten Jahren gestiegenen klinischen Anforderungen erfüllen. Ein Problem ist es jedoch, dass nicht genügend Pathologen auf dem Gebiet der Fetalpathologie spezialisiert sind. Das trifft nicht nur für Deutschland zu, sondern das ist mehr oder weniger eine weltweite Situation. Ein möglicher Lösungsansatz dieses Problems wäre die Nutzung von Softwareprogrammen, die dazu dienen, unerfahrene Pathologen moderne Anleitungen während der Autopsie von Feten zu geben. Das kann mit einer telepathologischen Betreung durch einen Experten auf diesem Gebiet kombiniert werden. Darüberhinaus erlaubt die derzeitige digitalisierte Befunddokumentation den Vergleich aktueller mit gespeicherten Fällen sowie Recherchen und stellt eine gute Basis zur Erstellung von Lehrmaterial dar., Due to the enormous technical developments in recent years in the area of prenatal ultrasound more malformations are detected in greater detail and at an earlier point in time during pregnancy than in the past. As a result, fetuses of increasingly smaller size and with complex diagnoses are presented for autopsy. At the same time, the genetic causes of ever more diseases, malformations and syndromes are being discovered, so that DNA diagnostics, chromosome analysis and, among others, molecular-genetic and biochemical investigations have come increasingly to complement clinical-genetic diagnostics. Through establishment of a clinically oriented autopsy strategy and the use of modern communication media, pathologists can satisfy the increased clinical demands of recent years. The remaining problem is that we do not have enough pathologists experienced in fetal pathology. This is not only true for Germany, it is more less the situation worldwide. One means of dealing with this problem may be the use of software programs to guide inexperienced pathologists through the modern autopsy procedure. This can be combined with the use of telepathological advise given by an expert in the field. Over and above this, digitized documentation of findings can be used in research, which allows the comparison of current with previously saved cases and presents a good basis for the development of education material.

Nierenfehlbildungen, vorzeitiger Blasensprung, Hydrops fetalis und Skelettfehlbildungen sind häufige Ursachen eines Minderwuchses der Lunge, der immer mit einer schlechten Prognose einhergeht. Bis heute gibt es jedoch kein zuverlässiges Verfahren zur pränatalen Diagnose einer Lungenhypoplasie, die das geburtshilfliche Management erheblich erleichtern könnte. Bei Totgeburten wird eine Lungenhypoplasie pathologisch-anatomisch durch einen verminderten Lungen / Körpergewichts-Index (L/KG- Index) und einen verringerten Radial Alveolar Count (RAC) belegt. Ziel dieser Studie war es, ein sonographisches Verfahren zu validieren, mit dem man eine fetale Lungenhypoplasie schon während der Schwangerschaft diagnostizieren könnte. Die Untersuchung basierte auf gestationsaltersabhängigen Referenzwerten für verschiedene Biometrieparameter, die das fetale Lungenwachstum gut erfassen. Die aufgestellten Normkurven wurden hinsichtlich ihres prädiktiven Werts zur Vorhersage einer Lungenhypoplasie geprüft. Dazu wurden Messungen bei 39 Feten mit einem hohem Risiko zur Entwicklung einer Lungenhypoplasie zwischen der 17. und 30. SSW vorgenommen. The patients were splitted into four groups: preterm rupture of membranes, hydrothorax, urinary tract anomalies and skeletal dysplasia. In each fetus seven different parameters were measured: the anterior-posterior and the transverse diameter at the level of the diaphragm, the four chamber view and the clavicula and additionally the lung length. The best results were found at the level of the four chamber view (sensitivity 61 % and specifity 75 %).
