Abstract

The aim of this thesis by prospective publication was to evaluate the comparative accuracy of screening technologies for the detection of COAG and other sight-threatening eye diseases. It incorporates 4 peer-reviewed publications: 1). Methodology and reporting of diagnostic accuracy studies of automated perimetry in glaucoma: evaluation using a standardised approach (Ophthalmic Physiol Opt. 2015; 35(3):315-23). This publication reported on an evaluation of the methodological and reporting quality of diagnostic accuracy studies of perimetry in glaucoma and determined whether there had been any improvement since the publication of the Standards for Reporting of Diagnostic. Accuracy (STARD) guidelines in 2003. The main findings were that methodological and reporting quality was sub-optimal and appeared not to have improved substantially following the development of the STARD guidance. 2). Development and validation of a new glaucoma screening test using temporally modulated flicker (Ophthalmic Physiol Opt. 2018; 38(6):617-628). This publication described the psychometric characteristics and diagnostic accuracy of the Accelerator 4-Alternative Forced-Choice Flicker Test prototype (A4FTp) for detecting Chronic open angle glaucoma (COAG). The performance of the A4FTp was also compared with standard screening tests of ocular structure and function (Frequency Doubling Technology (FDT) perimetry and iVue Spectral Domain Optical Coherence Tomography (SD-OCT). The time taken to complete the A4FTp was relatively short and initial results are promising. The diagnostic accuracy for the A4FTp was comparable to those of the FDT and SD-OCT for detecting COAG and we concluded that with further refinement the A4FTp could have role in glaucoma detection in the future. 3). Diagnostic accuracy of technologies for glaucoma case finding in a community setting (Ophthalmology. 2015;122(12):2407-15). This publication described a cross sectional, observational, community-based study to evaluate the diagnostic performance of the FDT, Moorfields Motion Displacement Test (MMDT), iVue SD-OCT and ocular response analyzer (ORA) used alone or in combination, for the detection of COAG. Diagnostic performance of individual tests gave acceptable accuracy for COAG detection. The best performing parameter was inferior RNFL thickness recorded using the SD-OCT. Although the low specificity of visual-function tests precluded their use in isolation, an acceptable performance was achieved by combining RNFL thickness analysis with visual function tests 4). Role of advanced technology in the detection of sight-threatening eye disease in a UK community setting (BMJ Open Ophthalmology 2019; 4:e000347). This publication extended the analysis described above to determine the performance of same screening tests for detecting sight-threatening eye disease in a cohort of elderly subjects recruited from primary care. The main finding was that a subset of screening tests (FDT, SD-OCT, together with a recorded visual acuity <6/12) was the most effective in detecting significant eye disease in this elderly population. The study provided useful preliminary data to inform the development of further larger, multi-center screening studies to validate this screening panel. The work described in this thesis makes a useful contribution to the evidence base on the use of imaging and visual function technologies to identify COAG and other sight-threatening eye diseases in at-risk populations and provides clear directions for future research this area.

Abstract

We provide the first ab-initio calculation of the Euclidean long-distance window of the isospin symmetric light-quark connected contribution to the hadronic vacuum polarization for the muon $g-2$ and find $a_\mu^{\rm LD,iso,conn,ud} = 411.4(4.3)(2.4) \times 10^{-10}$. We also provide the currently most precise calculation of the total isospin symmetric light-quark connected contribution, $a_\mu^{\rm iso,conn,ud} = 666.2(4.3)(2.5) \times 10^{-10}$, which is more than 4$\sigma$ larger compared to the data-driven estimates of Boito et al. 2022 and 1.7$\sigma$ larger compared to the lattice QCD result of BMW20., Comment: 12 pages, 9 figures

3. Discovering heavy neutrino-antineutrino oscillations at the $Z$-pole

Abstract

Collider-testable type I seesaw extensions of the Standard Model are generally protected by an approximate lepton number (LN) symmetry. Consequently, they predict pseudo-Dirac heavy neutral leptons (HNLs) composed of two nearly degenerate Majorana fields. The interference between the two mass eigenstates can induce heavy neutrino-antineutrino oscillations (NNOs) leading to observable lepton number violation (LNV), even though the LN symmetry is approximately conserved. These NNOs could be resolved in long-lived HNL searches at collider experiments, such as the proposed Future Circular $e^+e^-$ Collider (FCC-$ee$) or Circular Electron Positron Collider (CEPC). However, during their $Z$-pole runs, the LN carried away by the light (anti)neutrinos produced alongside the HNLs prevents LNV from being observed directly. Nevertheless, NNOs materialise as oscillating signatures in final state distributions. We discuss and compare a selection of such oscillating observables, and perform a Monte Carlo simulation to assess the parameter space in which NNOs could be resolved., Comment: 29 pages, 15 figures

5. Impact of duodenoscope reprocessing factors on duodenoscope contamination:a retrospective observational study

Abstract

Background: Despite adherence to reprocessing protocols, duodenoscopes frequently remain contaminated, highlighting significant knowledge gaps in reprocessing efficiency. Aim: To identify risk factors in duodenoscope reprocessing procedures affecting contamination rates. Methods: Cultures from Pentax ED34-i10T2 duodenoscopes collected between February 2022 and December 2023 were included. Contamination was determined by the presence of micro-organisms of gut or oral origin (MGO). Data on duodenoscope use, reprocessing lead times and personnel were retrieved from electronic medical records. Risk factors were derived from reprocessing guidelines and literature. These included a delay >30 min in initiating manual cleaning, manual cleaning duration of ≤5 min, drying time <90 min, personnel reprocessing frequency, and storage exceeding seven days. A logistic mixed-effects model evaluated these factors' impact on duodenoscope contamination. Findings: Out of 307 duodenoscope cultures, 58 (18.9%) were contaminated with MGO. Throughout the study period, the duodenoscopes underwent 1296 reprocessing cycles. Manual cleaning times of ≤5 min significantly increased contamination odds (adjusted odds ratio (aOR): 1.61; 95% confidence interval (CI): 1.10–2.34; P = 0.01). Increased usage of a duodenoscope was associated with reduced odds of contamination (aOR: 0.80; 95% CI: 0.64–0.995; P = 0.045). Other studied risks showed no clear association with contamination rates. Conclusion: Manual cleaning times of ≤5 min increased the odds of contamination with MGO. Delays in reprocessing initiation and incomplete drying, traditionally considered as risk factors, were not associated with an increased risk of contamination in this study. Future research should explore whether enhanced surveillance of reprocessing times can mitigate duodenoscope contamination.

6. Overview of a national endoscopy database:The Trans.IT database and its impact on data registration quality

Abstract

Background: The Trans.IT database is a national gastrointestinal (GI) endoscopy database developed in 2012. It automatically collects anonymous data from GI endoscopy procedures in a centralized database. All endoscopists use a structured reporting tool for uniform data collection. In this study, we aim to provide an overview of the database and to evaluate its impact on data registration quality. Methods: We used all ERCPs, colonoscopies and colorectal cancer (CRC)-screening colonoscopies performed between 2016 and 2020. We excluded centers joining after 2016 and patients below age 18. Data registration quality for ERCPs included completeness of data for: intention of ERCP, Schutz score, ASA classification, papillary status (virgin or previous sphincterotomy), cannulation (success or failure to cannulate the desired duct) and procedural success. For colonoscopies: indication, ASA-classification, Boston Bowel Preparation Score (BBPS), cecal intubation, polyp detection rate (PDR). For CRC-screening colonoscopies, ASA-classification, BBPS, cecal intubation, PDR and adenoma detection rate (ADR). Results: A total of 14,156 ERCPs, 150,962 colonoscopies and 37,199 colorectal cancer screening colonoscopies were included in our analysis. For ERCPs, registration of procedural intention, Schutz score, ASA classification, papillary status, cannulation and procedural success improved from 34.9%, 32.7%, 72.6%, 36.5%, 34.6%, 27.2% in 2016, to 86.4%, 84.6%, 97.4%, 86.4%, 82.1%, 84.0%, respectively, in 2020. For non-screening colonoscopies, registration of indication, ASA classification, BBPS, cecal intubation and PDR improved from 40.4%, 60.5%, 47.6%, 69.8% and 32.3% in 2016 to 90.3%, 88.9%, 59.8%, 79.1% and 39.1%, respectively, in 2020. For CRC-cancer screening colonoscopy registration equaled outcome, PDR and ADR changed from 74.7% to 63.6% in 2016 to 66.3% and 53.8% in 2020, respectively.Conclusions:

7. A climatic suitability indicator to support Leishmania infantum surveillance in Europe : a modelling study

Abstract

Background: Leishmaniases are neglected diseases transmitted by sand flies. They disproportionately affect vulnerable groups globally. Understanding the relationship between climate and disease transmission allows the development of relevant decision-support tools for public health policy and surveillance. The aim of this modelling study was to develop an indicator that tracks climatic suitability for Leishmania infantum transmission in Europe at the subnational level. Methods: Historical records of sand fly vectors, human leishmaniasis, bioclimatic indicators, and environmental variables were integrated in a machine learning framework (XGBoost) to predict suitability in two past periods (2001–2010 and 2011–2020). We further assessed if predictions were associated with human and animal disease data from selected countries (France, Greece, Italy, Portugal, and Spain). Findings: An increase in the number of climatically suitable regions for leishmaniasis was detected, especially in southern and eastern countries, coupled with a northward expansion towards central Europe. The final model had excellent predictive ability (AUC = 0.970 [0.947–0.993]), and the suitability predictions were positively associated with human leishmaniasis incidence and canine seroprevalence for Leishmania. Interpretation: This study demonstrates how key epidemiological data can be combined with open-source climatic and environmental information to develop an indicator that effectively tracks spatiotemporal changes in climatic suitability and disease risk. The positive association between the model predictions and human disease incidence demonstrates that this indicator could help target leishmaniasis surveillance to transmission hotspots. Abstract

Seismic noise and local disturbances are dominant noise sources for ground-based gravitational waves detectors in the low frequency region (0.1-10 Hz) limiting their sensitivity and duty cycle. With the introduction of high-performance seismic isolation systems based on mechanical pendula, the 2nd generation laser interferometric detectors have reached the scientific goal of the first direct observation of GW signals thanks to the extension of the detection bandwidth down to 10 Hz. Now, the 3rd generation instrument era is approaching, and the Einstein telescope giant interferometer is becoming a reality with the possibility to install the detector in an underground site where seismic noise is 100 times smaller than on surface. Moreover, new available technologies as well as the experience acquired in operating advanced detectors are key points to further extend the detection bandwidth down to 2 Hz with the possibility to suspend cryogenic payload and then mitigating thermal noise too. Here, we present a preliminary study devoted to improving seismic attenuation performance of the advanced VIRGO superattenuator in the low frequency region of about five orders of magnitude. Particular care has been carried on in analyzing the possibility to improve the vertical attenuation performance with a multi-stage pendulum chain equipped with magnetic anti-springs that is hung to a double inverted pendulum in nested configuration. The feedback control requirements and possible strategies to be adopted for this last element will be presented.

Abstract

Record-breaking temperatures were recorded across the globe in 2023. Without climate action, adverse climate-related health impacts are expected to worsen worldwide, affecting billions of people. Temperatures in Europe are warming at twice the rate of the global average, threatening the health of populations across the continent and leading to unnecessary loss of life. The Lancet Countdown in Europe was established in 2021, to assess the health profile of climate change aiming to stimulate European social and political will to implement rapid health-responsive climate mitigation and adaptation actions. In 2022, the collaboration published its indicator report, tracking progress on health and climate change via 33 indicators and across five domains. This new report tracks 42 indicators highlighting the negative impacts of climate change on human health, the delayed climate action of European countries, and the missed opportunities to protect or improve health with health-responsive climate action. The methods behind indicators presented in the 2022 report have been improved, and nine new indicators have been added, covering leishmaniasis, ticks, food security, health-care emissions, production and consumption-based emissions, clean energy investment, and scientific, political, and media engagement with climate and health. Considering that negative climate-related health impacts and the responsibility for climate change are not equal at the regional and global levels, this report also endeavours to reflect on aspects of inequality and justice by highlighting at-risk groups within Europe and Europe's responsibility for the climate crisis., This online publication has been corrected.Errata: Correction to Lancet Public Health 2024; 9: e495–522. Abstract

PurposeRecently introduced hybrid 2-[18 F]-fluoro-2-deoxy-D-glucose (18 F-FDG) Positron Emission Tomography (PET) combined with Magnetic Resonance Imaging (MRI) may aid in proper diagnosis and staging of perihilar cholangiocarcinoma (pCCA). The aim of this study is to assess the effect of 18 F-FDG PET/MRI on diagnosis and clinical decision making in the pre-operative work up of pCCA.MethodsIn this single-centre pilot study patients with presumed resectable pCCA underwent state-of-the-art 18 F-FDG hybrid PET/MRI using digital silicone photomultiplier detectors integrated within a 3-Tesla bore. Data were collected on several baseline and imaging characteristics. The primary outcome measure was the added diagnostic information and the effect on clinical decision making. Secondary aim was to correlate quantitative PET signal intensity to patient- and tumour characteristics. High and low SUVmax subgroups related to the mean value were made. Significance of lesion- and patient characteristics with the high and low SUVmax subgroups, as well as TLR and TBR, was evaluated with Fisher's exact test or Mann-Whitney-U test.ResultsIn total 14 patients were included (mean age 62.4 years, 64% male). Final diagnosis was pCCA in 10 patients (71.4%), follicular lymphoma in one patient (7.1%) and benign disease in the remaining three patients. FDG-PET/MRI added valuable diagnostic information in six (43%) patients and affected clinical decision making in two of these patients (14%) by increasing confidence for malignancy which lead to the decision for surgery on short term. High SUVmax values were seen in half of cases with pCCA and half of cases with non-cancerous lesions. In addition, high SUVmax values were directly associated with primary sclerosing cholangitis when present (p = 0.03).ConclusionSimultaneous 18 F-FDG-PET/MRI added diagnostic information in six of fourteen patients and influenced clinical decision making in two patients (14%) with presumed resectable pCCA.

Background and Aim: More insight into the incidence of and factors associated with progression following a first episode of acute pancreatitis (AP) would offer opportunities for improvements in disease management and patient counseling. Methods: A long-term post hoc analysis of a prospective cohort of patients with AP (2008–2015) was performed. Primary endpoints were recurrent acute pancreatitis (RAP), chronic pancreatitis (CP), and pancreatic cancer. Cumulative incidence calculations and risk analyses were performed. Results: Overall, 1184 patients with a median follow-up of 9 years (IQR: 7–11) were included. RAP and CP occurred in 301 patients (25%) and 72 patients (6%), with the highest incidences observed for alcoholic pancreatitis (40% and 22%). Pancreatic cancer was diagnosed in 14 patients (1%). Predictive factors for RAP were alcoholic and idiopathic pancreatitis (OR 2.70, 95% CI 1.51–4.82 and OR 2.06, 95% CI 1.40–3.02), and no pancreatic interventions (OR 1.82, 95% CI 1.10–3.01). Non-biliary etiology (alcohol: OR 5.24, 95% CI 1.94–14.16, idiopathic: OR 4.57, 95% CI 2.05–10.16, and other: OR 2.97, 95% CI 1.11–7.94), RAP (OR 4.93, 95% CI 2.84–8.58), prior pancreatic interventions (OR 3.10, 95% CI 1.20–8.02), smoking (OR 2.33, 95% CI 1.14–4.78), and male sex (OR 2.06, 95% CI 1.05–4.05) were independently associated with CP. Conclusion: Disease progression was observed in a quarter of pancreatitis patients. We identified several risk factors that may be helpful to devise personalized strategies with the intention to reduce the impact of disease progression in patients with AP.

Contains fulltext : 305445.pdf (Publisher’s version ) (Open Access), Endoscopic ultrasound (EUS) with fine needle aspiration or fine needle biopsy is the gold standard for sampling tissue to diagnose pancreatic cancer and autoimmune pancreatitis or to analyze cyst fluid. The most common reported adverse event of fine needle aspiration and/or fine needle biopsy is acute pancreatitis, which is likely induced by the same pathophysiological mechanisms as after endoscopic retrograde cholangiopancreatography (ERCP). According to the current European Society of Gastrointestinal Endoscopy guideline, nonsteroidal anti-inflammatory drugs are administered prior to ERCP as a scientifically proven treatment to reduce post-ERCP pancreatitis incidence rate. A single suppository of diclofenac or indomethacin prior to EUS guided tissue acquisition (TA) is harmless in healthy adults. Since it is associated with low costs and, most important, may prevent a dreadsome complication, we strongly recommend the administration of 100 mg diclofenac rectally prior to EUS-TA. We will explain this recommendation in more detail in this review as well as the risk and pathophysiology of post-EUS TA pancreatitis.

Abstract

Hadronic spectral densities play a pivotal role in particle physics, a primer example being the R-ratio defined from electron-positron scattering into hadrons. To predict them from first principles using Lattice QCD, we face a numerically ill-posed inverse problem, due to the Euclidean signature adopted in practical simulations. Here we present a recent numerical analysis of the vector isovector spectral density extracted using the multi-level algorithm (recently extended also to the case of dynamical fermions) and discuss its implications.

We addressed the assumption that AIV prevalence is low or nil among land birds, a hypothesis that was not supported by the results—rather, we found AIV infections at relatively high prevalences in birds of the orders Apodiformes (hummingbirds) and Passeriformes (songbirds). Abstract

Record-breaking temperatures were recorded across the globe in 2023. Without climate action, adverse climate-related health impacts are expected to worsen worldwide, affecting billions of people. Temperatures in Europe are warming at twice the rate of the global average, threatening the health of populations across the continent and leading to unnecessary loss of life. The Lancet Countdown in Europe was established in 2021, to assess the health profile of climate change aiming to stimulate European social and political will to implement rapid health-responsive climate mitigation and adaptation actions. In 2022, the collaboration published its indicator report, tracking progress on health and climate change via 33 indicators and across five domains. This new report tracks 42 indicators highlighting the negative impacts of climate change on human health, the delayed climate action of European countries, and the missed opportunities to protect or improve health with health-responsive climate action. The methods behind indicators presented in the 2022 report have been improved, and nine new indicators have been added, covering leishmaniasis, ticks, food security, health-care emissions, production and consumption-based emissions, clean energy investment, and scientific, political, and media engagement with climate and health. Considering that negative climate-related health impacts and the responsibility for climate change are not equal at the regional and global levels, this report also endeavours to reflect on aspects of inequality and justice by highlighting at-risk groups within Europe and Europe's responsibility for the climate crisis., We thank the global Lancet Countdown and the Wellcome Trust (grant number 209734/Z/17/Z) for their financial and technical support. 16. The use of Brassica species for the management of potato cyst nematode infestations of potatoes

Abstract

The potato cyst nematodes (PCN), Globodera pallida and G. rostochiensis are economically the most important nematode problems of potatoes within the EU, costing the UK potato industry over £36 million based on yield loss and nematicides application. Historically, farmers within the UK have relied heavily on soil fumigant and granular nematicides for PCN control. However, increasing political pressure on pesticide usage means that alternative and sustainable strategies are required. Biofumigation, the suppression of soil borne pests and pathogens by biocidal compounds released when brassicaceous tissues are hydrolysed, offers a potential sustainable alternative in managing PCN. In this study, summer cultivated B. juncea and R. sativus reduced PCN population post-potato harvest. Eruca sativa proved to be cold tolerant following overwinter cultivation, but did not reduce PCN multiplication . Glucosinolate concentrations in the brassicas varied significantly between different plant regions and cultivation seasons. B. juncea leaves produced high 2-propanyl levels while R. sativus produced predominantly 4-methylsulfinylbutyl and 2-phenylethyl GSL in foliage and root respectively. Eruca sativa produced a blend of GSL in both above and below grown tissues, most of which were either low or non-ITC producing GSL. Metconazole application enhanced 2-propanyl GSL production in B. juncea tissues. Glucosinolate concentrations correlated positively with PCN mortality in the summer-cultivated experiments. In glasshouse experiments, sinigrin was significantly degraded in brassica cultivated soil pre- and post- incorporation of the brassicas. Positive relationships were observed between PCN mortality and microbial activity, while GSL concentration was found to be inversely related to microbial activity. Finally, the LD50 for B. juncea and R. sativus against PCN were determined in-vitro as 0.027/0.032 and 0.546/0.035 mg 1 ml- for leaf/root extracts respectively. This study has demonstrated that using B. juncea and R. sativus can play an important role in PCN management, particularly if included in an integrated pest management scheme in ware potato production.

Abstract

Background and aim: Elevated triglyceride (TG) levels seem to identify subjects at increased cardiovascular risk, independent of LDL-C levels. We sought to evaluate the predictive role of hypertriglyceridemia, defined as TG levels ≥150 mg/dl, in very high risk (VHR) patients with chronic coronary syndromes (CCS) treated with statins. Methods and results: Using the data from the STable Coronary Artery Diseases RegisTry (START) study, an Italian nationwide registry, we assessed the association between the TG levels and baseline clinical characteristics, pharmacological treatment and major adverse cardio-cerebrovascular events (MACCE) at 1 year in a large cohort of statin-treated patients at VHR. Of the 4751 consecutive patients with CCS enrolled in the registry and classified as VHR, 2652 (55.8%) had TG values available (mean 120.6 ± 54.9) and were treated with at least a statin at baseline: 2019 (76.1%) with TG < 150 and 633 (23.9%) with TG ≥ 150 mg/dl. At 1 year from enrolment, MACCE occurred in 168 (6.3%) patients, without differences between the two groups of TG (5.9 vs 7.6%; p = 0.14). At multivariable analysis, hypertriglyceridemia did not result as independent predictor of the MACCE (hazard ratio: 1.16; 95% confidence intervals: 0.82–1.64; p = 0.42). Conclusions: In the present large, nationwide cohort of consecutive CCS patients at VHR with statin-controlled LDL-C levels, hypertriglyceridemia was present in around 24% of cases and did not result as predictor of MACCE at 1 year. Further studies with a longer follow-up and larger sample size are needed to better define the prognostic role of TG levels when intensive LDL lowering therapies are used.

Abstract

Phase transitions in a non-perturbative regime can be studied by ab initio Lattice Field Theory methods. The status and future research directions for LFT investigations of Quantum Chromo-Dynamics under extreme conditions are reviewed, including properties of hadrons and of the hypothesized QCD axion as inferred from QCD topology in different phases. We discuss phase transitions in strong interactions in an extended parameter space, and the possibility of model building for Dark Matter and Electro-Weak Symmetry Breaking. Methodological challenges are addressed as well, including new developments in Artificial Intelligence geared towards the identification of different phases and transitions.

23. Translating topological benefits in very cold lattice simulations

Abstract

Master-field simulations offer an approach to lattice QCD in which calculations are performed on a small number of large-volume gauge-field configurations. The latter is advantageous for simulations in which the global topological charge is frozen due to a very fine lattice spacing, as the effect of this on observables is suppressed by the spacetime volume. Here we make use of the recently developed Stabilised Wilson Fermions to investigate a variation of this approach in which only the temporal direction (T) is taken larger than in traditional calculations. As compared to a hyper-cubic lattice geometry, this has the advantage that finite-L effects can be useful, e.g. for multi-hadron observables, while compared to open boundary conditions, time-translation invariance is not lost. In this proof-of-concept contribution, we study the idea of using very cold (i.e. long-T) lattices to topologically "defrost" observables at fine lattice spacing. We identify the scalar-scalar meson two-point correlation function as a useful probe and present first results from Nf = 3 ensembles with time extents up to T = 2304 and a lattice spacing of a = 0.055 fm.

24. Hybrid galaxy evolution modelling : Monte Carlo Markov Chain parameter estimation in semi-analytic models of galaxy formation

Abstract

We introduce a statistical exploration of the parameter space of the Munich semi-analytic model built upon the Millennium dark matter simulation. This is achieved by applying a Monte Carlo Markov Chain (MCMC) method to constrain the 6 free parameters that define the stellar mass function at redshift zero. The model is tested against three different observational data sets, including the galaxy K-band luminosity function, B −V colours, and the black hole-bulge mass relation, to obtain mean values, confidence limits and likelihood contours for the best fit model. We discuss how the model parameters affect each galaxy property and find that there are strong correlations between them. We analyze to what extent these are simply reflections of the observational constraints, or whether they can lead to improved understanding of the physics of galaxy formation. When all the observations are combined, the need to suppress dwarf galaxies requires the strength of the supernova feedback to be significantly higher in our best-fit solution than in previous work. We interpret this fact as an indication of the need to improve the treatment of low mass objects. As a possible solution, we introduce the process of satellite disruption, caused by tidal forces exerted by central galaxies on their merging companions. We apply similar MCMC sampling techniques to the new model, which allows us to discuss the impact of disruption on the basic physics of the model. The new best fit model has a likelihood four times better than before, reproducing reasonably all the observational constraints, as well as the metallicity of galaxies and predicting intra-cluster light. We interpret this as an indication of the need to include the new recipe. We point out the remaining limitations of the semi-analytic model and discuss possible improvements that might increase its predictive power in the future.

Abstract

Room modes are a recognised problem in small critical listening rooms and are known to cause colouration of sound reproduced within them. Investigations on the causes and solutions for this problem have been carried out for some time. Interest in the topic has extended to loudspeaker manufacturers who have mainly concentrated in developing methods for controlling the loudspeaker-room interaction in order to ameliorate low frequency reproduction. Compared to objective work on passive and active control methods, the study of the subjective perception of room resonances has been somewhat neglected. Available publications mostly concern the effects of single resonances, which are perhaps not fully representative of conditions as experienced in real rooms. A study into the subjective perception of room modes is presented. The experimental methodology employs psychoacoustic techniques to study the perception of factors such as modal distribution, and effects of resonances on single tones. Results show that the subjective perception of room modes is strongly affected by temporal issues, and that changes exerted merely on magnitude frequency response are detectable but not likely to remove the effects of resonances for all listeners. Furthermore, it is shown that a reduction of the modal Q-factor, associated with a reduction of decay rates, has a significant effect in decreasing the detection of resonances. Q-factor difference limen were evaluated for three reference decay characteristics corresponding to reference Q-factors of 30, 10 and 1. The limen were 6±2.8, 10±4.1 and 16±5.4 respectively, meaning that detection of changes to modal decay decreases with decreasing decay time. These results may be used to define more perceptually relevant design guidelines for critical listening environments, and indicate target criteria for control techniques used in room correction. The outcomes of this investigation will have repercussions on the design of better rooms for critical listening.

Abstract

The master-field approach to lattice QCD envisions performing calculations on a small number of large-volume gauge-field configurations. Substantial progress has been made recently in the generation of such fields, and this must be joined with measurement strategies that take advantage of the large volume. In these proceedings, we describe how to compute simple hadronic quantities efficiently and estimate their errors in the master-field approach, i.e. by studying cross-correlations of observables on a single configuration. We discuss the scaling of the uncertainty with the volume and compare extractions based on momentum-projected and position-space two-point functions. The latter show promising results, already at intermediate volumes, but come with additional technical complexities such as a more complicated manifestation of boundary effects, which we also address.

Abstract

Permanent ponds are valuable freshwater systems and biodiversity hotspots. They provide diverse ecosystem services (ESs), including water quality improvement and supply, food provisioning, and biodiversity support, despite significant pressure from multiple anthropogenic stressors and the impacts of ongoing global change. However, ponds are largely overlooked in management plans and legislation, and ecological research has focused on large freshwater ecosystems, such as rivers or lakes. Protection of ponds is often insufficient or indirectly provided via associated habitats such as wetlands. This situation is likely exacerbated by the lack of a full-scale understanding of the importance of ponds. In this review, we provide a detailed overview of permanent ponds across Europe, including their usages and the biodiversity they support. By discussing the concepts of pondscape and metacommunity theory, we highlight the importance of connectivity among and between ponds and identified fluxes of emerging insects as another ES of ponds. Those insects are rich in essential nutrients such as polyunsaturated fatty acids (PUFAs), delivered through them to the terrestrial environment; however, the extent and impact of this ES remains largely unexplored. Several potential stressors, especially related to ongoing global change, that influence pond diversity and integrity are discussed. We provide our insights on future pond management. Adaptive measures, taking into account the pond system per se within the pondscape, are the most promising to mitigate the loss of natural ponds and restore and conserve natural small waterbodies as refuges and diversity hotspots in increasingly urbanized landscapes.

Abstract

Background The open abdomen is an innovation that greatly improved surgical understanding of damage control, temporary abdominal closure, staged abdominal reconstruction, viscera and enteric fistula care, and abdominal wall reconstruction. This article provides an evidence-informed, expert, comprehensive narrative review of the open abdomen in trauma, acute care, and vascular and endovascular surgery. Methods A group of 12 international trauma, acute care, and vascular and endovascular surgery experts were invited to review current literature and important concepts surrounding the open abdomen. Results The open abdomen may be classified using validated systems developed by a working group in 2009 and modified by the World Society of the Abdominal Compartment Syndrome-The Abdominal Compartment Society in 2013. It may be indicated in major trauma, intra-abdominal sepsis, vascular surgical emergencies, and severe acute pancreatitis; to facilitate second look laparotomy or avoid or treat abdominal compartment syndrome; and when the abdominal wall cannot be safely closed. Temporary abdominal closure and staged abdominal reconstruction methods include a mesh/sheet, transabdominal wall dynamic fascial traction, negative pressure wound therapy, and hybrid negative pressure wound therapy and dynamic fascial traction. This last method likely has the highest primary fascial closure rates. Direct peritoneal resuscitation is currently an experimental strategy developed to improve primary fascial closure rates and reduce complications in those with an open abdomen. Primary fascial closure rates may be improved by early return to the operating room; limiting use of crystalloid fluids during the surgical interval; and preventing and/or treating intra-abdominal hypertension, enteric fistulae, and intra-abdominal collections after surgery. Abstract

We compute the standard Euclidean window of the hadronic vacuum polarization using multiple independent blinded analyses. We improve the continuum and infinite-volume extrapolations of the dominant quark-connected light-quark isospin-symmetric contribution and address additional sub-leading systematic effects from sea-charm quarks and residual chiral-symmetry breaking from first principles. We find $a_\mu^{\rm W} = 235.56(65)(50) \times 10^{-10}$, which is in $3.8\sigma$ tension with the recently published dispersive result of Colangelo et al., $a_\mu^{\rm W} = 229.4(1.4) \times 10^{-10}$, and in agreement with other recent lattice determinations. We also provide a result for the standard short-distance window. 31. Heavy neutrino-antineutrino oscillations at the FCC-ee

Abstract

We discuss the impact of heavy neutrino-antineutrino oscillations (NNOs) on heavy neutral lepton (HNL) searches at proposed electron-positron colliders such as the future circular $e^+e^-$ collider (FCC-ee). During the $Z$ pole run, HNLs can be produced alongside a light neutrino or antineutrino that escapes detection and can decay into a charged lepton or antilepton together with an off-shell $W$ boson. In this case, signals of lepton number violation only show up in the final state distributions. We discuss how NNOs, a typical feature of collider-testable low-scale seesaw models where the heavy neutrinos form pseudo-Dirac pairs, modify such final state distributions. For example, the forward-backward asymmetry (FBA) of the reconstructed heavy (anti)neutrinos develops an oscillatory dependence on the HNL lifetime. We show that these oscillations can be resolvable for long-lived HNLs. We also discuss that when the NNOs are not resolvable, they can nevertheless significantly modify the theory predictions for FBAs and observables such as the ratio of the total number of HNL decays into $\ell^-$ over ones into $\ell^+$, in an interval of the angle~$\theta$ between the HNL and the beam axis. Item does not contain fulltext, Trained immunity is a de facto memory for innate immune responses, leading to long-term functional reprogramming of innate immune cells. In physiological conditions, trained immunity leads to adaptive states that enhance resistance against pathogens and contributes to immunosurveillance. Dysregulated trained immunity can however lead either to defective innate immune responses in severe infections or cancer or to inflammatory and autoimmune diseases if trained immunity is inappropriately activated. Here, we review the immunological and molecular mechanisms that mediate trained immunity induction and propose that trained immunity represents an important target for prophylactic and therapeutic approaches in human diseases. On the one hand, we argue that novel approaches that induce trained immunity may enhance vaccine efficacy. On the other hand, induction of trained immunity in cancer, and inhibition of exaggerated induction of trained immunity in inflammatory disorders, are viable targets amenable for new therapeutic approaches.

Abstract

Internal waves are commonly observed in oceans and lakes where high stratification exists. In the present study in the Strait of Gibraltar, we analyse internal soliton signals recorded in different locations in their eastward propagation from their release point (Camarinal Sill) to the continental slope of the northwestern Alboran Sea. Moreover, the effect of subinertial forcing on the release of solitons is also explored. The internal soliton activity was assessed from different approaches: (i) in-situ data (i.e., current and temperature measurements or High-Frequency Radar), (ii) numerical modelling, and (iii) an analytical approach. The arrival of solitons over the continental slope of the north-western Alboran Sea showed fortnightly variability in both number (occurrence) and amplitude during spring tides when compared with during neap tides. The observed arrival times of the solitons oscillated between 14 (spring tides) and 20 h (neap tides). Nevertheless, to provide a comprehensive explanation for the fluctuations in travel times, it is necessary to consider the subinertial variability driven by atmospheric forcing, which impacts both the flow in the Strait of Gibraltar and the mesoscale patterns in the Alboran Sea (e.g., the Coastal Cyclonic Gyre).

Abstract

Because of the mass gap, lattice QCD simulations exhibit stochastic locality: distant regions of the lattice fluctuate independently. There is a long history of exploiting this to increase statistics by obtaining multiple spatially-separated samples from each gauge field; in the extreme case, we arrive at the master-field approach in which a single gauge field is used. Here we develop techniques for studying hadronic observables using position-space correlators, which are more localized, and compare with the standard time-momentum representation. We also adapt methods for estimating the variance of an observable from autocorrelated Monte Carlo samples to the case of correlated spatially-separated samples.

37. Impact of ERCP simulator training on early ERCP learning curves of novice trainees:a cohort study

Abstract

Background and study aim Simulator-based training has been extensively studied in training gastroduodenoscopy and colonoscopy and shown to significantly improve learning curves of novices. Data on simulator-based training in endoscopic retrograde cholangiopancreatography (ERCP) are scarce. We aimed to determine the impact of 2-day intensive hands-on simulator training on the course of the learning curve of novice trainees. Methods We conducted a prospective cohort study using a validated mechanical ERCP simulator (Boškoski-Costamagna ERCP Trainer). Six trainees were allocated to the simulation course program (SG). Each of these trainees were paired with an endoscopy trainee starting regular ERCP training at the same center who had no exposure to a simulation course program (control group; CG). The course included lectures, live ERCP demonstrations, and hands-on ERCP training to educate trainees in basic techniques related to cannulation, stent placement, stone extraction and stricture management. After the course, both the SG and CG started formal ERCP training in their respective centers. The Rotterdam Assessment Form for ERCP was used to register each performed ERCP. Simple moving average was applied to create learning curves based on successful common bile duct (CBD) cannulation. Outcomes were plotted against a historical cohort (HC). Results Thirteen trainees were included, six trainees in the SG and seven trainees in the CG, with a total of 717 ERCPs. Mean successful ERCP cannulation rate was higher for the simulator group at baseline compared to both CG and HC, 64% versus 43% and 42%, respectively. Differences became less explicit after 40 ERCPs, but persisted until a median of 75 ERCPs. Abstract

Background and Aims: Meckel's diverticulum (MD) may remain silent or be associated with adverse events such as GI bleeding. The main aim of this study was to evaluate indicative small-bowel capsule endoscopy (SBCE) findings, and the secondary aim was to describe clinical presentation in patients with MD. Methods: This retrospective European multicenter study included patients with MD undergoing SBCE from 2001 until July 2021. Results: Sixty-nine patients with a confirmed MD were included. Median age was 32 years with a male-to-female ratio of approximately 3:1. GI bleeding or iron-deficiency anemia was present in nearly all patients. Mean hemoglobin was 7.63 ± 1.8 g/dL with a transfusion requirement of 52.2%. Typical capsule endoscopy (CE) findings were double lumen (n = 49 [71%]), visible entrance into the MD (n = 49 [71%]), mucosal webs (n = 30 [43.5%]), and bulges (n = 19 [27.5%]). Two or more of these findings were seen in 48 patients (69.6%). Ulcers were detected in 52.2% of patients (n = 36). In 63.8% of patients (n = 44), a combination of double lumen and visible entrance into the MD was evident, additionally revealing ulcers in 39.1% (n = 27). Mean percent SB (small bowel) transit time for the first indicative image of MD was 57% of the total SB transit time. Conclusions: Diagnosis of MD is rare and sometimes challenging, and a preoperative criterion standard does not exist. In SBCE, the most frequent findings were double-lumen sign and visible diverticular entrance, sometimes together with ulcers.

39. Higher yield in duodenoscope cultures collected with addition of neutralizing agent

Abstract

Aim: Microbiological cultures are the gold standard in the monitoring of duodenoscope reprocessing. However, many different sampling and culturing techniques are used, making it difficult to compare results. The latest Centers for Disease Control and Prevention protocol advises the use of a neutralizer to deactivate any remaining disinfectants in the samples. This study compared culturing results of duodenoscope samples collected with and without addition of a neutralizer. Methods: Six duodenoscopes were soiled with gut bacteria in a non-clinical experimental setting and reprocessed afterwards. Samples of the tip and working channel were collected immediately after decontamination or after drying. Dey-Engley (DE) broth was added as a neutralizer to the samples of four duodenoscopes; samples for the other two duodenoscopes were collected without the addition of DE broth. Results: Post-decontamination cultures were significantly more likely to be positive for growth of the applied micro-organisms in the group of samples with DE broth (88.1% vs 20.2%; P<0.0001). Post-drying samples were significantly more likely to be positive in the group of samples without DE broth (75.7% vs 33.4%; P<0.001). Conclusion: The addition of DE broth to samples collected from wet duodenoscopes increases the yield of those cultures. Remaining disinfectants in wet duodenoscopes can lead to false-negative results. This can be overcome by adding a neutralizer, such as DE broth, to the samples. The higher yield after drying in the group without neutralizer could be due to biofilm formation in these two duodenoscopes, but this was not investigated. Standardization of the sampling method can help to compare both clinical and study results regarding duodenoscope contamination.

Abstract

Permanent ponds are valuable freshwater systems and biodiversity hotspots. They provide diverse ecosystem services (ESs), including water quality improvement and supply, food provisioning, and biodiversity support, despite significant pressure from multiple anthropogenic stressors and the impacts of ongoing global change. However, ponds are largely overlooked in management plans and legislation, and ecological research has focused on large freshwater ecosystems, such as rivers or lakes. Protection of ponds is often insufficient or indirectly provided via associated habitats such as wetlands. This situation is likely exacerbated by the lack of a full-scale understanding of the importance of ponds. In this review, we provide a detailed overview of permanent ponds across Europe, including their usages and the biodiversity they support. By discussing the concepts of pondscape and metacommunity theory, we highlight the importance of connectivity among and between ponds and identified fluxes of emerging insects as another ES of ponds. Those insects are rich in essential nutrients such as polyunsaturated fatty acids (PUFAs), delivered through them to the terrestrial environment; however, the extent and impact of this ES remains largely unexplored. Several potential stressors, especially related to ongoing global change, that influence pond diversity and integrity are discussed. We provide our insights on future pond management. Adaptive measures, taking into account the pond system per se within the pondscape, are the most promising to mitigate the loss of natural ponds and restore and conserve natural small waterbodies as refuges and diversity hotspots in increasingly urbanized landscapes.

Abstract

Aims Recently, the cardiovascular outcomes for people using anticoagulation strategies (COMPASS) trial demonstrated that dual therapy reduced cardiovascular outcomes compared with aspirin alone in patients with stable atherosclerotic disease. Methods and We sought to assess the proportion of patients eligible for the COMPASS trial and to compare the epidemiology results and outcome of these patients with those without COMPASS inclusion or with any exclusion criteria in a contemporary, nationwide cohort of patients with stable coronary artery disease. Among the 4068 patients with detailed information allowing evaluation of eligibility, 1416 (34.8%) did not fulfil the inclusion criteria (COMPASS-Not-Included), 841 (20.7%) had exclusion criteria (COMPASS-Excluded), and the remaining 1811 (44.5%) were classified as COMPASS-Like. At 1 year, the incidence of major adverse cardiovascular event (MACE), a composite of cardiovascular death, myocardial infarction, and stroke, was 0.9% in the COMPASS-Not-Included and 2.0% in the COMPASS-Like (P = 0.01), and 5.0% in the COMPASS-Excluded group (P < 0.0001 for all comparisons). Among the COMPASS-Like population, patients with multiple COMPASS enrichment criteria presented a significant increase in the risk of MACE (from 1.0% to 3.3% in those with 1 and >_3 criteria, respectively; P = 0.012), and a modest absolute increase in major bleeding risk (from 0.2% to 0.4%, respectively; P = 0.46). Conclusion In a contemporary real-world cohort registry of stable coronary artery disease, most patients resulted as eligible for the COMPASS. These patients presented a considerable annual risk of MACE that consistently increases in the presence of multiple risk factors.

Abstract

(1) Background: Villous atrophy is an indication for small bowel capsule endoscopy (SBCE). However, SBCE findings are not described uniformly and atrophic features are sometimes not recognized; (2) Methods: The Delphi technique was employed to reach agreement among a panel of SBCE experts. The nomenclature and definitions of SBCE lesions suggesting the presence of atrophy were decided in a core group of 10 experts. Four images of each lesion were chosen from a large SBCE database and agreement on the correspondence between the picture and the definition was evaluated using the Delphi method in a broadened group of 36 experts. All images corresponded to histologically proven mucosal atrophy; (3) Results: Four types of atrophic lesions were identified: mosaicism, scalloping, folds reduction, and granular mucosa. The core group succeeded in reaching agreement on the nomenclature and the descriptions of these items. Consensus in matching the agreed definitions for the proposed set of images was met for mosaicism (88.9% in the first round), scalloping (97.2% in the first round), and folds reduction (94.4% in the first round), but granular mucosa failed to achieve consensus (75.0% in the third round); (4) Conclusions: Consensus among SBCE experts on atrophic lesions was met for the first time. Mosaicism, scalloping, and folds reduction are the most reliable signs, while the description of granular mucosa remains uncertain. © 2022 by the authors.

Abstract

Background and Aims: Pancreatoscopy-guided electrohydraulic lithotripsy (EHL) has shown potential in the treatment of patients with obstructive chronic calcifying pancreatitis (CCP). We aimed to prospectively investigate the efficacy and safety of EHL as first-line therapy in patients with CCP of the pancreatic duct (PD). Methods: A prospective single-center consecutive case series was performed including symptomatic CCP patients with obstructing stones >5 mm in the head or neck of the pancreas. Stone fragmentation was performed using EHL. Primary study outcome was technical success. Secondary outcomes were clinical success, adverse events, and number of interventions. Results: Thirty-four consecutive patients were included. Complete or partial stone clearance after EHL was achieved in 24 patients (70.6%). Pancreatoscopy was not performed because of failure to cannulate the PD (n = 5) or resolution of stones after stent placement at the index endoscopic retrograde pancreaticography (ERP) procedure (n = 3). After successful PD cannulation, pancreatoscopy was technically successful in 24 of 26 patients (92.3%). In 1 patient, the stone could not be visualized because of a resilient stricture. Complete stone clearance was achieved in 20 patients (80%) and partial clearance in 5 patients (20%), after a median of 2 ERP procedures (interquartile range, 2) and 1 EHL procedure (interquartile range, 1). In patients who underwent pancreatoscopy with EHL, mean Izbicki pain score at baseline was 62.3 ± 23.1 (25/25) and dropped significantly to 27.5 ± 35.0 (22/25) at the 6-month follow-up (P < .001). The most common adverse event was acute pancreatitis, all mild and treated conservatively (n = 7). Conclusions: Pancreatoscopy-guided EHL is a promising treatment for symptomatic CCP patients with obstructive PD stones. (Clinical trial registration number: NTR6853.)
