Abstract

Aims: Bioabsorbable polymer drug-eluting stents (DES) may reduce the inflammation and delayed healing associated with some permanent polymer-coated DES. Whether late clinical outcomes are improved, particularly among patients with medically treated diabetes, is unknown. Therefore, we analysed outcomes from a pre-specified substudy of the EVOLVE II trial to evaluate the safety and effectiveness of the SYNERGY stent in patients with diabetes mellitus. Methods and Results: SYNERGY is a thin-strut, platinum-chromium everolimus-eluting stent with an ultra-thin bioabsorbable poly(DL-lactide-co-glycolide) abluminal polymer. The EVOLVE II randomised, controlled trial proved the non-inferiority of the SYNERGY versus the PROMUS Element Plus stent for one-year target lesion failure (TLF: ischaemia-driven target lesion revascularisation [ID-TLR], target vessel myocardial infarction [TVMI], or cardiac death). The pre-specified EVOLVE II diabetes substudy prospectively pooled randomised patients with diabetes (N=263) with a sequential single-arm diabetic cohort (n=203). The substudy primary endpoint was one-year TLF compared with a pre-specified performance goal (14.5%). The primary endpoint occurred in 7.5% of SYNERGY-treated patients with diabetes, significantly less than the performance goal (p<0.0001). The two-year rate of TLF was 11.2% (cardiac death 1.5%, TVMI 6.4%, ID-TLR 6.8%) and definite/probable stent thrombosis occurred in 1.1% of patients. Conclusion(s): The EVOLVE II diabetes substudy demonstrates the efficacy and safety of the SYNERGY stent in patients with medically treated diabetes. Abstract

Posterior chest wall defects are frequently encountered after excision of tumors as a result of trauma or in the setting of wound dehiscence after spine surgery. Various pedicled fasciocutaneous and musculocutaneous flaps have been described for the coverage of these wounds. The advent of perforator flaps has allowed the preservation of muscle function but their bulk is limited. Musculocutaneous flaps remain widely employed. The trapezius and the latissimus dorsi (LD) flaps have been used extensively for upper and middle posterior chest wounds, respectively. Their bulk allows for obliteration of the dead space in deep wounds. The average width of the LD skin paddle is limited to 10-12cm if closure of the donor site is expected without skin grafting. In 2001 a modification of the skin paddle design was introduced in order to allow large flaps to be raised without requiring grafts or flaps for donor site closure. This V-Y pattern allows coverage of large anterior chest defects after mastectomy. We have modified this flap to allow its use for posterior chest wall defects. We describe the flap design, its indications, and its limitations with three clinical cases. Level of Evidence V This journal requires that authors assign a level of evidence to each article. Abstract

BACKGROUND Permanent stenting with bare metal or drug-eluting stents in the coronary artery is associated with neoatherosclerosis and late thrombotic events. Completely bioabsorbable drug-eluting scaffolds may potentially reduce the risk of late adverse events. RENUVIATM, a balloon expandable scaffold, is made of bioabsorbable poly(L-lactide) and everolimus-eluting abluminal poly(D, L-lactideco- glycolide) polymers. Compared to currently available bioabsorbable scaffolds, RENUVIATM incorporates an optimized design with thinner struts to improve deliverability and overexpansion capabilities, and scaffold fracture resistance. This study evaluates the safety and performance of RENUVIATM in subjects with de novo coronary lesions. Data through 6 months will be reported for the first time at TCT 2017. METHODS Subjects with single de novo native coronary artery lesion (length: <=12mm; reference vessel diameter:>= 2.75mm and <=3.25mm) were enrolled in this prospective, multi-center, single arm feasibility study (N=33). A single scaffold size (3.0mmx16mm) was available for use in the study. The primary endpoints are clinical procedural success (% diameter stenosis <=30% and no in-hospital MACE [death, MI or TLR]) and 6-month angiographic in-scaffold late loss. RESULTS The mean age of patients was 62 years, 67% were male, 24% had medically-treated diabetes, and >60% were treated for hypertension or hyperlipidemia. At the time of writing, 6-month data were available in 22 patients. Clinical procedural success was 90.9% (30/33) due to 3 peri-procedural non-Q-wave MIs. No deaths, TLR, or scaffold thromboses were observed in the analyzed population. At 6 months, angiographic late loss was 0.28+/-0.25 mm in-scaffold and 0.17+/-0.26 mm in-segment; and % diameter stenosis was 19.45+/-11.14% in-scaffold and 26.11+/-9.77 in-segment. 35. Role of thermal and electrical relaxations for the long-term conduction current in polyethylene

Abstract

The characterization of the electrical conduction behavior of polymeric insulating material is a challenging task. Typical reasons are the underlying complexity of the physical processes, the smallness of the current, and the long relaxation times. Moreover, even if the external conditions like applied voltage and temperature are time-constant, often a steady-state direct current (DC) is not established during the measurement time, which may last up from hours to months. This work reports on DC current measurements on 1 mm thick press molded LDPE and XLPE samples. The results show that the conductance is slowly decaying sub-linearly with time, I∼tp, with 0 <-p < 1, p often around-0.5. In certain cases an intermediate current maximum is observed. There is strong indication that the decay of the effective conductivity is due a thermal process rather than to an electrical process., QC 20161123

Abstract

Aims: Bioabsorbable-polymer DES may reduce inflammation and delayed healing associated with some permanent polymer-coated DES. Whether late clinical outcomes are improved, particularly among patients with diabetes mellitus, is unknown. Methods and Results: We report the prespecified analysis of patients with medically-treated diabetes enrolled in the EVOLVE II trial at 2 years following SYNERGY stent deployment. SYNERGY is a novel, thin-strut, platinum chromium stent eluting everolimus from an ultrathin 4mum bioabsorbable poly(DL-lactide-co-glycolide) polymer applied to the abluminal side of the stent. The polymer resorbs shortly after everolimus elution is complete at 3 months. SYNERGY was found to be noninferior to the permanent polymer coated PROMUS Element Plus stent for the primary endpoint of 12-month target lesion failure (TLF: composite occurrence of ischaemia-driven target lesion revascularisation, target-vessel myocardial infarction, or cardiac death) in the EVOLVE II pivotal, randomised controlled trial (RCT). The EVOLVE II Diabetes substudy pooled: 1) patients with diabetes randomised to the SYNERGY arm of the EVOLVE II RCT (N=263) and, 2) patients with diabetes enrolled in the non-randomised Diabetes single-arm study (N=203) following EVOLVE II RCT completion for a total of 466 patients with diabetes treated with SYNERGY stent. Enrolled patients could have up to 3 native coronary artery lesions in up to 2 major epicardial vessels with reference vessel diameter >=2.25 mm to <=4.00 mm, and lesion length <=34 mm. Exclusion criteria included recent (<72 hours) STEMI, left main, vein graft or ostial stenosis, in stent restenosis, chronic total occlusion, or thrombus. The prespecified Diabetes substudy primary endpoint was 12-month TLF tested against a prespecified performance goal based on TLF rates derived from cohorts of patients with diabetes treated with permanent polymer everolimus-eluting stents in prior studies (PLATINUM, SPIRIT IV, COMPARE, EVOLVE).

Abstract

Background: Statin treatment and variants in the gene encoding HMG-CoA reductase are associated with reductions in both the concentration of LDL cholesterol and the risk of coronary heart disease, but also with modest hyperglycaemia, increased bodyweight, and modestly increased risk of type 2 diabetes, which in no way offsets their substantial benefits. We sought to investigate the associations of LDL cholesterol-lowering . PCSK9 variants with type 2 diabetes and related biomarkers to gauge the likely effects of PCSK9 inhibitors on diabetes risk. Methods: In this mendelian randomisation study, we used data from cohort studies, randomised controlled trials, case control studies, and genetic consortia to estimate associations of PCSK9 genetic variants with LDL cholesterol, fasting blood glucose, HbA1c, fasting insulin, bodyweight, waist-to-hip ratio, BMI, and risk of type 2 diabetes, using a standardised analysis plan, meta-analyses, and weighted gene-centric scores. Findings: Data were available for more than 550 000 individuals and 51 623 cases of type 2 diabetes. Combined analyses of four independent PCSK9 variants (rs11583680, rs11591147, rs2479409, and rs11206510) scaled to 1 mmol/L lower LDL cholesterol showed associations with increased fasting glucose (0·09 mmol/L, 95% CI 0·02 to 0·15), bodyweight (1·03 kg, 0·24 to 1·82), waist-to-hip ratio (0·006, 0·003 to 0·010), and an odds ratio for type diabetes of 1·29 (1·11 to 1·50). Based on the collected data, we did not identify associations with HbA1c (0·03%, -0·01 to 0·08), fasting insulin (0·00%, -0·06 to 0·07), and BMI (0·11 kg/m2, -0·09 to 0·30). Interpretation: . PCSK9 variants associated with lower LDL cholesterol were also associated with circulating higher fasting glucose concentration, bodyweight, and waist-to-hip ratio, and an increased risk of type 2 diabetes. Abstract

Aims: Bioabsorbable-polymer DES may reduce inflammation and delayed healing associated with some permanent polymer-coated DES. Whether late clinical outcomes are improved, particularly among patients with diabetes mellitus, is unknown. Methods and Results: We report the prespecified analysis of patients with medically-treated diabetes enrolled in the EVOLVE II trial at 2 years following SYNERGY stent deployment. SYNERGY is a novel, thin-strut, platinum chromium stent eluting everolimus from an ultrathin 4mum bioabsorbable poly(DL-lactide-co-glycolide) polymer applied to the abluminal side of the stent. The polymer resorbs shortly after everolimus elution is complete at 3 months. SYNERGY was found to be noninferior to the permanent polymer coated PROMUS Element Plus stent for the primary endpoint of 12-month target lesion failure (TLF: composite occurrence of ischaemia-driven target lesion revascularisation, target-vessel myocardial infarction, or cardiac death) in the EVOLVE II pivotal, randomised controlled trial (RCT). The EVOLVE II Diabetes substudy pooled: 1) patients with diabetes randomised to the SYNERGY arm of the EVOLVE II RCT (N=263) and, 2) patients with diabetes enrolled in the non-randomised Diabetes single-arm study (N=203) following EVOLVE II RCT completion for a total of 466 patients with diabetes treated with SYNERGY stent. Enrolled patients could have up to 3 native coronary artery lesions in up to 2 major epicardial vessels with reference vessel diameter >=2.25 mm to <=4.00 mm, and lesion length <=34 mm. Exclusion criteria included recent (<72 hours) STEMI, left main, vein graft or ostial stenosis, in stent restenosis, chronic total occlusion, or thrombus. The prespecified Diabetes substudy primary endpoint was 12-month TLF tested against a prespecified performance goal based on TLF rates derived from cohorts of patients with diabetes treated with permanent polymer everolimus-eluting stents in prior studies (PLATINUM, SPIRIT IV, COMPARE, EVOLVE).

Abstract

We study pulsed positive discharges in air in a cylindrically symmetric setup with an electrode needle close (about 1 mm) above the top of a dielectric cylindrical rod of 4 mm in diameter mounted at its bottom on a grounded plate electrode. We present ICCD (intensified charge-coupled device) pictures and evaluations of experiments as well as simulations with a fluid discharge model; the simulations use cylindrical symmetry. In the experiments, there is an initial inception cloud phase, where the cylindrical symmetry is maintained, and later a streamer phase, where it is broken spontaneously. At 75-150 mbar, discharges with cylindrical symmetry are not attracted to the dielectric rod, but move away from it. The dielectric rod plays the sole role of an obstacle that shades (in the context of photoionization) a cone-shaped part of the inception cloud; the cone size is determined by the geometry of the setup. The material properties of the dielectric rod, such as its dielectric permittivity and the efficiency of the photon induced secondary electron emission do not have a noticeable effect. This is due to the abundance of photoionization in air, which supplies a positive discharge with free electrons and allows it to propagate along the electric field lines. Using some simple field calculations, we show that field enhancement due to dielectric polarization does not play a significant role in our geometry as long as the discharge maintains its cylindrical symmetry. The field component towards the rod is insufficiently enhanced to cause the discharge to move towards the rod. Any additional electrons produced by the dielectric surface do not influence this discharge morphology. This interpretation is supported by both experiments and simulations. At higher pressures (400-600 mbar) or for larger gaps between the needle and the dielectric rod, the inception cloud reaches its maximal radius within the gap between needle and rod and destabilizes there.
