El objetivo de este estudio fue el de investigar el efecto que podía tener la aplicación de cargas laterales controladas de forma temprana, en la osteointegración de implantes dentales. Se analizaron los resultados obtenidos mediante análisis de frecuencia de resonancia (RFA), utilizando unos nuevos prototipos de pilares de compresión (patentados por los autores como modelo de utilidad*) y se demostró que dichas cargas controladas eran beneficiosas para mantener e incluso mejorar, la estabilidad de los implantes colocados durante los estadios iniciales de la cicatrización, acelerando el proceso de osteointegración final. *Modelo de Utilidad: “Pilares compresores perimplantarios”. Background: This study aims to show the clinical outcomes of implants supporting mandibular overdentures in edentulous patients. Methods: Mandibular edentulous patients were diagnosed with an oral examination, panoramic radiograph, and diagnostic casts for intermaxillary relations and treated with overdentures over two implants. After two-stage surgery, implants were early loaded with an overdenture at 6 weeks. Results: Fifty-four patients (28 females and 24 males) were treated with 108 implants. Thirty-two patients (59.2%) had a previous history of periodontitis. Twenty-three patients (46%) were smokers. Forty patients (74.1%) suffered from systemic diseases (i.e., diabetes, cardiovascular diseases). The clinical follow-up of the study was 147.8 ± 10.4 months. The clinical outcomes showed a global success of 94.5% of implants. Fifty-four overdentures were placed in the patients over the implants. The mean marginal bone loss was 1.12 ± 0.34 mm. Nineteen patients (35.2%) showed some kind of mechanical prosthodontic complication. Sixteen implants (14.8%) were associated with peri-implantitis. Conclusions: Based on the clinical results obtained, we can determine that the treatment of elderly edentulous patients with mandibular overdentures through the early loading of two placed implants is a successful implant protocol.

El estudio a largo plazo muestra la evaluación de los pacientes tratados mediante carga precoz de implantes con conexión interna y superficie arenada y grabada. Métodos. 40 pacientes con pérdidas dentales fueron tratados con implantes Frontier GMI ® con conexión interna y superficie arenada y grabada. Los implantes fueron cargados tras un periodo de tiempo de 6 semanas en la mandíbula y 8 semanas en el maxilar superior. Los hallazgos clínicos (implantológicos y prostodóncicos) se han seguido durante al menos 6 años. Resultados. 103 implantes fueron insertados en ambos maxilares, 46 implantes (44,7%) en el maxilar superior y 57 implantes (55,3%) en la mandíbula, para su rehabilitación prostodóncica. 37 implantes (35,9%) fueron insertados en el sector anterior y 66 implantes (64,1%) en el sector posterior. 70 implantes (68%) de forma sumergida (2 cirugías) y 33 implantes (32%) de forma no sumergida (una cirugía). Results. 103 implants were inserted (46 maxillary (44.7%), and 57 mandibular (55.3%)) for prosthodontic rehabilitation. 37 implants (35.9%) were inserted in anterior sites and 66 implant (64.1%) in posterior sites. Métodos. 40 pacientes con pérdidas dentales fueron tratados con implantes Frontier GMI ® con conexión interna y superficie arenada y grabada. Los implantes fueron cargados tras un periodo de tiempo de 6 semanas en la mandíbula y 8 semanas en el maxilar superior. Los hallazgos clínicos (implantológicos y prostodóncicos) se han seguido durante al menos 6 años. Resultados. 103 implantes fueron insertados en ambos maxilares, 46 implantes (44,7%) en el maxilar superior y 57 implantes (55,3%) en la mandíbula, para su rehabilitación prostodóncica. 37 implantes (35,9%) fueron insertados en el sector anterior y 66 implantes (64,1%) en el sector posterior. 70 implantes (68%) de forma sumergida (2 cirugías) y 33 implantes (32%) de forma no sumergida (una cirugía). Des pués de un seguimiento clínico de 92,2 meses, los resultados indican una supervivencia de los implantes del 96,2%; ya que se perdieron 4 implantes. La pérdida ósea marginal fué de 0,99 ± 0,84 mm. Las restau raciones prostodóncicas incluyeron 31 coronas unitarias, 15 puentes fijos, 5 sobredentaduras con bolas, 2 rehabilitaciones totales fijas y una rehabilitación completa híbrida. Las complicaciones tardías fueron 11 implantes (10,7%) con periimplantitis y 6 prótesis (11,1%) con complicaciones protésicas. Conclusiones. This long-term study reports the evaluation of patients treated by early loading of implants with internal connection and sandblasted-acidetched surface. El estudio muestra la evaluación de los pacientes tratados con coronas unitarias mediante la carga de los implantes unitarios. Los implantes fueron cargados funcionalmente tras un periodo de tiempo de 6 semanas en la mandíbula y 8 semanas en el maxilar superior. Resultado: Fueron insertados 216 implantes en ambos maxilares (168 en el maxilar superior y 48 en la mandíbula) para su rehabilitación prostodóncica con coronas implantosoportadas. 81 implantes fueron insertados en el sector anterior y 135 implantes en el sector posterior. Después de 5 años de seguimiento clínico, los resultados indican una supervivencia y éxito de los implantes del 95,8%. Durante el periodo de cicatrización, se perdieron 4 implantes por movilidad, mientras que 5 implantes se perdieron por periimplantitis. Se presentaron complicaciones prostodóncicas en 8 coronas con fractura de cerámica. Two implants were treatment planned. Digital data were sent to a software manufacturer for surgical template. According surgical template implants were inserted for a flapless surgical technique with an accuraced drilling. After a 6 weeks-period of time an overdenture were realized and finally, occlusion is evaluated. The follow-up were 5 years. Conclusions: This paper indicated that guided implant dentistry may constitute a successful treatment of edentulous mandible patients treated with overdentures

IIntroducción: La implantología oral representa en la actualidad, una modalidad terapéutica odontológica en los pacientes con pérdida dental total y parcial. El estudio muestra la evaluación de los pacientes tratados con coronas unitarias mediante la carga de los implantes unitarios. Métodos: Fueron tratados146 pacientes con pérdidas dentales unitarias con implantes con superficie arenada y grabada Galimplant®. Los implantes fueron cargados funcionalmente tras un periodo de tiempo de 6 semanas en la mandíbula y 8 semanas en el maxilar superior. Los hallazgos clínicos (implantológicos y prostodóncicos) se han seguido durante 5 años. Resultado: Fueron insertados 216 implantes en ambos maxilares (168 en el maxilar superior y 48 en la mandíbula) para su rehabilitación prostodóncica con coronas implantosoportadas. 81 implantes fueron insertados en el sector anterior y 135 implantes en el sector posterior. Después de 5 años de seguimiento clínico, los resultados indican una supervivencia y éxito de los implantes del 95,8%. Durante el periodo de cicatrización, se perdieron 4 implantes por movilidad, mientras que 5 implantes se perdieron por periimplantitis. Se presentaron complicaciones prostodóncicas en 8 coronas con fractura de cerámica. Introducción: La cirugía guiada y la carga precoz de los implantes constituye un enfoque integral de la rehabilitación de los pacientes edéntulos. El objetivo del presente trabajo era presentar el protocolo diagnóstico, quirúrgico y protésico del tratamiento con implantes mediante la técnica de cirugía guiada y carga precoz en el tratamiento con sobredentadura con implantes. Caso clínico: Paciente varón de 68 años que acude a consulta para tratamiento con implantes de la mandíbula edéntula. La tomografía computarizada de haz cónico (CBCT) muestra que el paciente presenta un adecuado volumen óseo para la inserción de implantes. La información obtenida por la CBCT es utilizada para la evaluación y el plan de tratamiento. Se planificaron dos implantes interforaminales. Los datos digitales de imagen tomográfica fueron enviados a un centro especializado para la realización de la férula quirúrgica. De acuerdo a la férula guiada se insertaron los implantes sin colgajo con un fresado preciso. A las 6 semanas de la cirugía, una sobredentadura fue realizada y ajustada sobre los ataches, evaluando su oclusión. El periodo de seguimiento ha sido de 5 años. Conclusiones: Este caso clínico indica que la cirugía guiada puede constituir una terapéutica exitosa en el tratamiento del paciente edéntulo mandibular con sobredentaduras., Introduction: Guided surgery and early functional loading of dental implants constitute a comprehensive approach for rehabilitation of edentulous patients. The aim of this paper was to present the diagnosis, surgical and prosthetic protocol of implants by guided technique and early loading in the treatment of edentulous mandible. Case report: A 68-year-old-man, requested an evaluation regarding implant treatment for the edentulous mandible. The cone beam computarized tomography (CBCT) showed that the patient have an adequate bone volume for mandibular implant placement. The data from CBCT were used for evaluation and treatment planning. Two implants were treatment planned. Digital data were sent to a software manufacturer for surgical template. According surgical template implants were inserted for a flapless surgical technique with an accuraced drilling. After a 6 weeks-period of time an overdenture were realized and finally, occlusion is evaluated. The follow-up were 5 years. Conclusions: This paper indicated that guided implant dentistry may constitute a successful treatment of edentulous mandible patients treated with overdentures

Secondary objectives included stability of these implants and peri-implant bone level. Materials and MethodsBone level (R) implants (Institut Straumann AG) inserted into premolar and/or molar maxillary sites were loaded after 6 weeks of healing. The implants were monitored for 1 year using the following outcome measures: implant success, primary and secondary stability, and peri-implant bone level. ResultsOut of 37 implants placed in 13 patients, 36 reached sufficient stability and were early loaded, whereas one underwent a delayed loading protocol. One-year success rate of early-loaded implants was 100%. Implant stability at baseline was 71.75. 6 to be steadily increased thereafter up to 1 year (80.3 +/- 3.3), except at 2 weeks when a nonsignificant decrease was noticed (71.9 +/- 3.9). Continuous and significant bone loss was observed, reaching 0.4 +/- 0.1mm in the first postoperative year. ConclusionBone level implants with the SLActive surface placed into low-density bone and loaded after 6 weeks of healing can predictably achieve and maintain a successful tissue integration.

Material and methodsTwelve patients with bilateral edentulous posterior mandibular were randomly assigned to treatment either with immediate (test) or early loaded implants (control). Seventy-two self-tapping implants with SLA surface (O 4, 1/4, 8mm; length 8 and 10mm) were analyzed in this study. Test implants (36) were loaded on the day of surgery and control implants 6weeks later. The measuring of implant stability quotient (ISQ) was performed on 0, 6th, 12th, and 52nd week after implant insertion. The bone resorption, modified plaque, and bleeding index were notified at 1 and 5years later. ResultsAfter 5years, survival in the both groups was 100%. The mean value of primary implant stability was 76.920.79 ISQ. In the first 6weeks, ISQ values significantly increased in the test group (77.92 +/- 1.16 vs. 79.61 +/- 0.90) as well as in the control group (7.92 +/- 1.05 vs. 77.55 +/- 0.99). A significant longitudinal increase in ISQ value was recorded in test and control group. The differences between immediate and early loaded implants were statistically insignificant (P>0.05). At the 5years, no statistically significant differences were found between immediate and early loaded implants with respect to mean crestal bone loss measurements (0.4 +/- 0.24 vs. 0.8 +/- 0.15mm), mean bleeding index (0.22 +/- 0.11 vs. 0.25 +/- 0.11), and mean plaque index (0.17 +/- 0.15 vs. 0.19 +/- 0.20). ConclusionBased on these results, the self-tapping implants inserted in posterior mandible can provide adequate primary stability value as the main factor for immediate and EL protocol.

Métodos: 39 pacientes con pérdidas dentales fueron tratados con implantes con conexión interna y superficie arenada y grabada Frontier GMI®. Los implantes fueron cargados funcionalmente tras un periodo de tiempo de 6 semanas en la mandíbula y 8 semanas en el maxilar superior. Los hallazgos clínicos (implantológicos y prostodóncicos) se han seguido durante 1 año. Resultados: 111 implantes fueron insertados en ambos maxilares (50 en el maxilar superior y 61 en la mandíbula) para su rehabilitación prostodóncica. 44 implantes fueron insertados en el sector anterior y 67 implantes en el sector posterior. Después de 1 año de seguimiento clínico, los resultados indican una supervivencia y éxito de los implantes del 99,1%. Durante el periodo de cicatrización libre de carga funcional, se perdió 1 implante por movilidad. Como complicaciones tardías hubo un caso de periimplantitis. Las restauraciones prostodóncicas incluyeron 30 coronas unitarias, 15 puentes fijos, 5 sobredentaduras con bolas y 2 rehabilitaciones totales fijas. Conclusiones: Los hallazgos clínicos del presente estudio indican que la rehabilitación prostodóncica mediante la inserción de implantes de titanio con conexión interna y superficie arenada + grabada, y cargados precozmente, representa una terapéutica odontológica con éxito elevado., Introduction: Today, oral implantology constitute a therapeutic modality in the prosthodontic treatment of patients with partial and total tooth loss. Methods: 39 patients with tooth loss were treated with Frontier GMI® sandblasted and acid-etched surface implants. Implants were loaded after a healing free-loading period of 6 weeks (mandible) and 8 weeks (maxilla). Clinical findings (implants and prosthodontics) were followed during 1 year. Results: 111 implants were inserted (50 maxillary, and 61 mandibular) for prosthodontic rehabilitation. 44 implants were inserted in anterior sites and 67 in posterior sites. After 1-year follow-up, clinical results indicate a survival and success rate of implants of 99.1%. One implant were lost during the healing period by mobility. A case of periimplantitis were reported as late complication after functional loading. Prosthodontic restorations included 30 single crowns, 15 fixed bridges, 5 ball-overdentures and 2 fixed totally rehabilitation. Conclusions: Clinical results of this study indicate that prosthodontic rehabilitation supported by internal connection and sandblasted and etched-titanium implants and loaded early can be a successful dental treatment.

Purpose: This study was intended to investigate, through resonance frequency analysis (RFA), the stability of implants placed in an early loading protocol via the osteotome sinus floor elevation technique in the posterior maxilla. All implants featured a sandblasted/acid-etched active surface. Materials and Methods: An early loading protocol was considered for patients in whom implants with a sandblasted, large-grit/acid-etched active surface (SLActive) were placed in the posterior maxilla by the osteotome sinus floor elevation technique. Implant stability, which was measured by RFA at surgery and at weekly intervals for the following 6 weeks, was used as the most significant inclusion criterion. At the end of the stability observation period, only implants with a stability quotient of 65 or higher were loaded. Implants were followed for 2 years. Results: Twenty-seven patients received 42 implants, 40 of which were subjected to an early loading protocol 6 weeks after placement. Two years after loading, all 40 implants were surviving, without clinically or radiographically detectible complications. Conclusion: An early loading protocol can be used for SLActive implants placed in the posterior maxilla via the osteotome sinus floor elevation technique if their stability is confirmed by RFA. The zygomatic implant is an alternative to bone grafting in extremely resorbed maxilla. This study evaluates the results of a consecutive cohort of 20 patients (mean age 56 years) with extremely resorbed maxillas provided with four zygomatic implants. The first 10 patients had a two-stage procedure, the next 10 next patients benefited from a one-stage surgical procedure and one of them had flapless guided surgery with Nobelguide® in development and immediate function. The same surgical drilling protocol, according to Branemark's procedure, was applied to all the patients. Except for one patient who lost three implants, 18 patients received a fixed Procera® implant bridge and another an overdenture retained by a screwed bar fixed on the four zygomatic implants. The cumulative survival rate after 40 months is 96%. Although bone augmenting procedures such as onlay grafts and sinus grafts are popular and well-documented, the four zygomatic implants procedure results in less morbidity, shorter delays between anatomical reconstruction and functional rehabilitation and can provide immediate or early loading with immediate function. This study evaluates the results of a consecutive cohort of 20 patients (mean age 56 years) with extremely resorbed maxillas provided with four zygomatic implants. The first 10 patients had a two-stage procedure, the next 10 next patients benefited from a one-stage surgical procedure and one of them had flapless guided surgery with Nobelguide® in development and immediate function. The same surgical drilling protocol, according to Branemark's procedure, was applied to all the patients. Except for one patient who lost three implants, 18 patients received a fixed Procera® implant bridge and another an overdenture retained by a screwed bar fixed on the four zygomatic implants. The cumulative survival rate after 40 months is 96%. Although bone augmenting procedures such as onlay grafts and sinus grafts are popular and well-documented, the four zygomatic implants procedure results in less morbidity, shorter delays between anatomical reconstruction and functional rehabilitation and can provide immediate or early loading with immediate function. Cordioli et al. and Krennmair et al. reported that rehabilitation with mandibular overdentures anchored to a single implant could be a therapeutic alternative for elderly patients experiencing discomfort and functional difficulties with conventional mandibular dentures. But the problem that a treatment period lasted for a long time was not solved. The present article is a case report of 86-year-old woman who treated with mandibular overdenture anchored to a single implant and a so-called early load was done and it could get good progress. She consulted our clinic because of the pain due to a mandibular denture in March 2003. Her mandible was edentulous with severe resorption and mandibular denture was instable. In the maxilla only right canine and right first premolar remained, and partial denture was attached. In April she received a new mandibular complete denture but she complained that she could not eat stiff food. Informed consent about a single-implant-supported overdenture, considering her age and economic condition, was obtained and then in July a single surface-modified implant of 13mm in length was placed at the mandibular midline. Since initial stability was good, a healing abutment was attached immediately. Healing process was good and one month later a healing abutment was replaced with a ball abutment and then a function as implant supported overdenture was given. After that retention and stability of the mandibular denture improved remarkably. She could become to eat many kinds of food and got a great satisfaction. At 1 year after loading peri-implant soft tissue was healthy. The radiographic examination revealed a stable situation., Cordioliら、およびKrennmairらは、下顎正中部に設置した1本のインプラントに支持、維持を求めたオーバーデンチャーを高齢者に適用し、口腔機能の改善、患者満足度の増大等その有用性を報告しているが、治療期間が長期にわたるという問題は解決されていない｡今回、86歳の女性に対し、シングルインプラント支持の下顎オーバーデンチャーを適用し、さらに早期負荷を行い、良好な経過が得られたのでその概要を報告した｡患者は2003年3月下顎義歯による疼痛を主訴に当科を受診した｡下顎は無歯顎で顎堤は全体的に著しく吸収し、下顎総義歯は動揺著明｡上顎は43のみが残存し、部分床義歯を装着｡同年4月、下顎総義歯を新たに製作したが、旧義歯同株硬いものが咬めないと訴えた｡年齢および経済的条件を考慮し、シングルインプラントをアンカーとする義歯の件を話したところ同意が得られ、同年7月インプラント埋入術を施行した｡下顎正中部に径4.3mm、長さ13mmのフィクスチャーを1本埋入した｡埋入トルクは32Ncmと初期固定は良好であり、ヒーリングアバットメントを装着し、1回法として手術を終了した｡手術後の治癒経過は良好で、約1か月後に同アバットメントを撤去し、ボールアバットメントに交換した｡使用中の下顎義歯にボールアタッチメントを接着し、同日より荷重を開始した｡以後、下顎義歯の維持、安定は格段に向上し、さらに摂取可能となった食物の種類も増加し、多大な満足度を得た｡荷重開始後1年経過するが、インプラント周囲粘膜は健常であり、X線写真でも骨吸収はほとんど認められない｡

0-2116, Researchers conducted an extensive literature search, survey of TxDOT districts, and survey of other state Departments of Transportation (DOTs) to determine the state of the art for bridge deck curing, early loading, early age behavior, and durability prediction models. Several concrete mixtures commonly used in regional districts throughout the state of Texas were tested. Each concrete mixture was tested under field conditions within its respective climatic region. Field testing and testing of field collected samples provided information on concrete strength, concrete maturity and permeability. Supplementary laboratory experiments were performed to determine permeability using the salt ponding method, and the rapid chloride penetration test, RCPT. Researchers used the laboratory tests to correlate results with field collected samples and concrete strengths. Both field and laboratory test data were compared with existing models for predicting concrete maturity for various mix designs and curing methods. Core samples from field sites were examined using petrographic techniques to determine air void contents, calcium hydroxide formation, aggregate/paste interface features, cracks, unhydrated cement grains and other petrographic features. Researchers related the variations of petrographic features with wet mat curing duration. Finally, researchers investigated the fracture properties of several medium-strength concrete mixes cured for various durations in order to characterize the extent of cracking that might allow the ingress of chlorides from de-icing salts to the reinforcing bars. The purpose of this report is to present results from the literature review.
