AIMS: To undertake a comprehensive investigation into both the process of information acquisition and the clinical decision-making process utilized by primary care nurses in the course of treating chronic wounds. DESIGN: Scenario-based think-aloud method, enriched by the integration of information processing theory. The study was conducted within the framework of home care nursing organizations situated in [placeholder]. A cohort of primary care nurses (n = 10), each possessing a minimum of one year of nursing experience, was recruited through the collaboration of three home care nursing organizations. METHODS: Two real-life clinical practice scenarios were employed for the interviews, with the researcher adopting the roles of either the patient or another clinician to enhance the realism of the think-aloud process. Each think-aloud session was promptly succeeded by a subsequent follow-up interview. The Consolidated criteria for Reporting Qualitative research checklist was followed to guarantee a consistent and complete report of the study. RESULTS: Amidst noticeable variations, a discernible pattern surfaced, delineating three sequential concepts: 1. gathering overarching information, 2. collecting and documenting wound-specific data, and 3. interpreting information to formulate wound treatment strategies. These concepts encompassed collaborative discussions with stakeholders, while the refinement of wound treatment strategies was interwoven within both concepts 2 and 3. CONCLUSIONS: Evident variations were identified in chronic wound care clinical decision-making, regardless of educational background or experience. These insights hold the potential to inform the development of clinical decision support systems for chronic wound management and provide guidance to clinicians in their decision-making endeavours.

INTRODUCTION: The incidence of pressure ulcers remains high in patients with moderate to severe acute respiratory distress syndrome, ventilated in the prone position. A digital platform, dedicated to prone positioning and skin/tissue damage education was developed. OBJECTIVE: To evaluate the impact of the PRONEtect Education Hub versus a traditional lecture on final-year nursing students' confidence levels and knowledge in a non-inferiority study. DESIGN: A multicenter, non-blinded, parallel-group, non-inferiority study with equal randomization (1:1 allocation) was conducted at two nursing schools in Belgium. CLINICALTRIALS: gov (NCT05575869). METHODS: Following baseline assessments, the control group received a 1-h classroom lecture, and the experimental group gained access to the PRONEtect website. Three weeks later, participants completed the knowledge, confidence, and visual knowledge assessment. RESULTS: At baseline, 67 of the 80 participants completed the assessments and post-intervention, 28 and 27 participants respectively completed the confidence, knowledge, and visual knowledge assessments (dropout rate of 66.25%). Confidence levels: a mean ratio of relative change from baseline = 0.96 (Control (C)/Experimental (E)); 97.5% confidence interval (CI): 0.74 to 1.26; p = 0.74. Knowledge assessment: a mean difference in change from baseline = 1.58 (C-E); 97.5% CI: -0.58 to 3.75; p = 0.1. Although confidence and knowledge scores increased in both groups, the study cannot conclude non-inferiority. CONCLUSIONS: The trade-off between the inability to conclude efficacy of the impact of the website and the benefit of having an accessible educational platform on prone positioning and skin damage prevention makes the PRONEtect Education Hub an acceptable adjunct to traditional lecturing.

Medium-energy hadronic scattering and reactions at zero degrees are very selective to excitations with low angular momentum transfer. Only a few facilities exist worldwide where high energy-resolution measurements of this nature can be performed. The K600 Zero-Degree Facility at iThemba LABS, South Africa, was recently successfully developed. Measurements were performed for inelastic proton scattering at an incident energy of 200 MeV for targets ranging from ²⁷Al to ²⁰⁸Pb. Excitation energy-resolution of 50 keV (FWHM) was achieved. A reasonable background subtraction procedure allows for the extraction of excitation energy spectra with low background. Measurements of the (p,t) reaction at 100 MeV benefit from a large difference in magnetic rigidity between the reaction products and primary particles, resulting in almost background-free spectra with excitation energy-resolution of 32 keV (FWHM).

Household studies provide an efficient means to study transmission of infectious diseases, enabling estimation of susceptibility and infectivity by person-type. A main inclusion criterion in such studies is usually the presence of an infected person. This precludes estimation of the hazards of pathogen introduction into the household. Here we estimate age- and time-dependent household introduction hazards together with within household transmission rates using data from a prospective household-based study in the Netherlands. A total of 307 households containing 1, 209 persons were included from August 2020 until March 2021. Follow-up of households took place between August 2020 and August 2021 with maximal follow-up per household mostly limited to 161 days. Almost 1 out of 5 households (59/307) had evidence of an introduction of SARS-CoV-2. We estimate introduction hazards and within-household transmission rates in our study population with penalized splines and stochastic epidemic models, respectively. The estimated hazard of introduction of SARS-CoV-2 in the households was lower for children (0-12 years) than for adults (relative hazard: 0.62; 95%CrI: 0.34-1.0). Estimated introduction hazards peaked in mid October 2020, mid December 2020, and mid April 2021, preceding peaks in hospital admissions by 1-2 weeks. Best fitting transmission models included increased infectivity of children relative to adults and adolescents, such that the estimated child-to-child transmission probability (0.62; 95%CrI: 0.40-0.81) was considerably higher than the adult-to-adult transmission probability (0.12; 95%CrI: 0.057-0.19). Scenario analyses indicate that vaccination of adults can strongly reduce household infection attack rates and that adding adolescent vaccination offers limited added benefit.

PURPOSE: Early recognition and effective treatment of sepsis improves outcomes in critically ill patients. However, antibiotic exposures are frequently suboptimal in the intensive care unit (ICU) setting. We describe the feasibility of the Bayesian dosing software Individually Designed Optimum Dosing Strategies (ID-ODS™), to reduce time to effective antibiotic exposure in children and adults with sepsis in ICU. METHODS: A multi-centre prospective, non-randomised interventional trial in three adult ICUs and one paediatric ICU. In a pre-intervention Phase 1, we measured the time to target antibiotic exposure in participants. In Phase 2, antibiotic dosing recommendations were made using ID-ODS™, and time to target antibiotic concentrations were compared to patients in Phase 1 (a pre-post-design). RESULTS: 175 antibiotic courses (Phase 1 = 123, Phase 2 = 52) were analysed from 156 participants. Across all patients, there was no difference in the time to achieve target exposures (8.7 h vs 14.3 h in Phase 1 and Phase 2, respectively, p = 0.45). Sixty-one courses in 54 participants failed to achieve target exposures within 24 h of antibiotic commencement (n = 36 in Phase 1, n = 18 in Phase 2). In these participants, ID-ODS™ was associated with a reduction in time to target antibiotic exposure (96 vs 36.4 h in Phase 1 and Phase 2, respectively, p < 0.01). These patients were less likely to exhibit subtherapeutic antibiotic exposures at 96 h (hazard ratio (HR) 0.02, 95% confidence interval (CI) 0.01-0.05, p < 0.01). There was no difference observed in in-hospital mortality. CONCLUSIONS: Dosing software may reduce the time to achieve target antibiotic exposures. It should be evaluated further in trials to establish its impact on clinical outcomes.

Algorithmic Foundations of Robotics XIV. WAFR 2020, Perception systems for autonomy are most useful if they can operate within limited/predictable computing resources. Existing algorithms in robot navigation—e.g. simultaneous localization and mapping—employ concepts from filtering, fixed-lag, or incremental smoothing to find feasible inference solutions. Using factor graphs as a probabilistic modeling language, we emphasize the importance of marginalization operations on the equivalent Bayes (junction) tree. The objective is to elucidate the connection between simple tree-based message passing rules with the aforementioned state estimation approaches, and their frequently overlooked relation to direct marginalization on the Bayes tree. We characterize the inherent marginalization operation as part of the fundamental Chapman-Kolmogorov transit integrals which unifies many state-of-the-art approaches. The belief propagation model is then used to define five major tree inference strategies, with regard to computation recycling and resource constrained operation. A series of illustrative examples and results show the versatility of the method.

This paper presents normalizing flows for incremental smoothing and mapping (NF-iSAM), a novel algorithm for inferring the full posterior distribution in SLAM problems with nonlinear measurement models and non-Gaussian factors. NF-iSAM exploits the expressive power of neural networks, and trains normalizing flows to model and sample the full posterior. By leveraging the Bayes tree, NF-iSAM enables efficient incremental updates similar to iSAM2, albeit in the more challenging non-Gaussian setting. We demonstrate the advantages of NF-iSAM over state-of-the-art point and distribution estimation algorithms using range-only SLAM problems with data association ambiguity. NF-iSAM presents superior accuracy in describing the posterior beliefs of continuous variables (e.g., position) and discrete variables (e.g., data association).

Successful entrainment during collaboration positively affects trust, willingness to collaborate, and likeability towards collaborators. In this paper, we present a mixed-method study to investigate characteristics of successful entrainment leading to pair and group-based synchronisation. Drawing inspiration from industrial settings, we designed a fast-paced, short-cycle repetitive task. Using motion tracking, we investigated entrainment in both dyadic and triadic task completion. Furthermore, we utilise audio-video recordings and semi-structured interviews to contextualise participants' experiences. This paper contributes to the Human-Computer/Robot Interaction (HCI/HRI) literature using a human-centred approach to identify characteristics of entrainment during pair- and group-based collaboration. We present five characteristics related to successful entrainment. These are related to the occurrence of entrainment, leader-follower patterns, interpersonal communication, the importance of the point-of-assembly, and the value of acoustic feedback. BACKGROUND Among patients with resectable early-stage non-small-cell lung cancer (NSCLC), a perioperative approach that includes both neoadjuvant and adjuvant immune checkpoint inhibition may provide benefit beyond either approach alone. METHODS We conducted a randomized, double-blind, phase 3 trial to evaluate perioperative pembrolizumab in patients with early-stage NSCLC. Participants with resectable stage II, IIIA, or IIIB (N2 stage) NSCLC were assigned in a 1:1 ratio to receive neoadjuvant pembrolizumab (200 mg) or placebo once every 3 weeks, each of which was given with cisplatin-based chemotherapy for 4 cycles, followed by surgery and adjuvant pembrolizumab (200 mg) or placebo once every 3 weeks for up to 13 cycles. The dual primary end points were event-free survival (the time from randomization to the first occurrence of local progression that precluded the planned surgery, unresectable tumor, progression or recurrence, or death) and overall survival. Secondary end points included major pathological response, pathological complete response, and safety. RESULTS A total of 397 participants were assigned to the pembrolizumab group, and 400 to the placebo group. At the prespecified first interim analysis, the median follow-up was 25.2 months. Event-free survival at 24 months was 62.4% in the pembrolizumab group and 40.6% in the placebo group (hazard ratio for progression, recurrence, or death, 0.58; 95% confidence interval [CI], 0.46 to 0.72; P<0.001). The estimated 24-month overall survival was 80.9% in the pembrolizumab group and 77.6% in the placebo group (P = 0.02, which did not meet the significance criterion). A major pathological response occurred in 30.2% of the participants in the pembrolizumab group and in 11.0% of those in the placebo group (difference, 19.2 percentage points; 95% CI, 13.9 to 24.7; P<0.0001; threshold, P = 0.0001), and a pathological complete response occurred in 18.1% and 4.0%, respectively (difference, 14.2 percentage points

Organizational dehumanization (OD) is a pervasive phenomenon that can be defined as the employee's perception of being dehumanized, treated as an instrument, and denied personal subjectivity by their organization. Studies examining dehumanization in the workplace have proliferated in recent years and increasingly underscore the insidious effects of OD for employees, their social relations, and the organization itself. Here, we review research over the past 5 years on OD that has contributed significantly to our understanding of its (organizational, interpersonal, situational, and environmental) triggers and its consequences at individual and organizational levels. We conclude by discussing future research directions that may equip us to better understand and protect humanness at work.

This study was conducted to determine the frequency and type of mutations that are responsible for resistance to rifampicin (RIF), isoniazid (INH), fluoroquinolones (FLQs) and second-line injectable drugs (SLIDs) in Mycobacterium tuberculosis (MTB) isolates obtained from culture-positive pulmonary tuberculosis (TB) patients in the central, southeastern and eastern Ethiopia.Patients and Methods: In total, 224 stored culture-positive MTB isolates from pulmonary TB patients referred to Adama and Harar regional TB laboratories between August 2018 and January 2019 were assessed for mutations conferring RIF, INH, FLQs and SLIDs resistance using GenoType®MTBDRplus (MTBDRplus) and GenoType®MTBDRsl (MTBDRsl).Results: RIF, INH, FLQs and SLIDs resistance-conferring mutations were identified in 88/224 (39.3%), 85/224 (38.0%), 7/77 (9.1%), and 3/77% (3.9%) of MTB isolates, respectively. BACKGROUND: Vulvar Crohn's disease is a rare cutaneous manifestation of inflammatory bowel disease and to date, studies have reported on under 300 cases worldwide. The condition has an increased risk of malignancy, and diagnosis is often difficult. Treatment protocols are yet to be developed. This paper aimed to provide the first account of patients' experience of living with vulvar Crohn's. METHODS: A previous qualitative study exploring experiences of sexual well-being in inflammatory bowel disease and experiences of discussing sexual well-being with healthcare professionals found 3 participants who self-reported vulvar Crohn's disease. Data from the whole cohort (n = 43) were previously reported. Telephone semi-structured interviews were used for data collection. van Manen's phenomenology of practice framework informed analysis. RESULTS: Due to significant differences in experiences, this subgroup of 3 women with vulvar Crohn's warranted separate attention. The common theme of the group was A decade of waiting, describing the major delays experienced in being diagnosed. The symptoms reported appeared to be very severe, and sexual well-being was very negatively affected. CONCLUSIONS: Women with vulvar Crohn's trust in healthcare professionals was eroded as a result of a decade delay in diagnosis, while the quality of life and relationships suffered.

Introduction: During the coronavirus pandemic (COVID -19), the use of prone positioning in critically ill patients with acute respiratory distress syndrome (ARDS) increased substantially. As a result, clinicians had to (re)learn how to treat the patient in the prone position while preventing adverse events such as pressure ulcers, skin tears and moisture-associated skin damage. Aim: The purpose of the study was to determine participants' learning needs related to patients in the prone position and the prevention of skin damage, such as pressure ulcers, and what they perceived as a positive or negative learning experience. Design: This study used a qualitative methodological framework and employed an exploratory design. Participants: A purposive sample of clinicians (n = 20) with direct or indirect work experience with prone ventilated patients was recruited in Belgium and Sweden. Methods: Individual semi-structured interviews were conducted in Belgium and Sweden between February and August 2022. Data were analysed thematically using an inductive approach. The COREQ guideline was utilised to comprehensively report on the study. Findings: Two themes were identified: 'Adapting to a crisis' and 'How to learn', with the latter having two subthemes: 'balancing theory and practice' and 'co-creating knowledge'. Unexpected circumstances necessitated a personal adaption, a change in learning methods and a pragmatic adaptation of protocols, equipment and working procedures. Participants recognised a multifaceted educational approach which would contribute to a positive learning experience regarding prone positioning and skin damage prevention. The importance of poising theoretical teaching with practical hands-on training was highlighted with an emphasis on interaction, discussion, and networking between peers. OBJECTIVE: The incidence of skin/tissue damage, such as pressure ulcers, remains high in mechanically ventilated patients in the prone position. According to guidelines, critically ill patients with acute respiratory distress syndrome (ARDS) should be prone for at least 12-16 hours to improve oxygenation and decrease mortality. Therefore, educating clinicians on how to reposition and manage the patient safely in a prone position plays a vital role in preventing adverse events. This project aimed to develop accessible online educational content to assist clinicians in safely executing the prone manoeuvre and minimise skin/tissue damage. METHOD: The development of the educational content was based on: a gap analysis and comprehensive review of available educational resources; evidence-based scientific literature; advice from international experts; and a qualitative study exploring the learning needs of 20 clinicians in Belgium and Sweden between February-August 2022. RESULTS: Volunteer clinicians assisted with the creation of eight simulation videos which were professionally filmed and edited. The interactive videos included the supine-to-prone and prone-to-supine manoeuvres, endotracheal and nasogastric tube securement, eye care, stoma care, protecting high-risk areas from pressure damage, and incontinence-associated dermatitis prevention. A prone positioning protocol, a checklist summarising the key aspects of the protocol, and teaching aids (slide deck for didactic lecturing) were developed and validated by a review of the relevant evidence-based literature and the international expert panel. A website was designed to host the content, with free user access, at www.pronetection.com. CONCLUSION: Education is one strategy towards prevention of complications of prone positioning. Accessible education could assist clinicians unfamiliar with prone positioning or current clinicians requiring refresher training to safely manage patients in this position.

We present the SARAO MeerKAT Galactic Plane Survey (SMGPS), a 1.3 GHz continuum survey of almost half of the Galactic Plane (251\deg $\le l \le$ 358\deg and 2\deg $\le l \le$ 61\deg at $|b| \le 1.5\deg $). SMGPS is the largest, most sensitive and highest angular resolution 1 GHz survey of the Plane yet carried out, with an angular resolution of 8" and a broadband RMS sensitivity of $\sim$10--20 $\mu$ Jy/beam. Here we describe the first publicly available data release from SMGPS which comprises data cubes of frequency-resolved images over 908--1656 MHz, power law fits to the images, and broadband zeroth moment integrated intensity images. A thorough assessment of the data quality and guidance for future usage of the data products are given. Finally, we discuss the tremendous potential of SMGPS by showcasing highlights of the Galactic and extragalactic science that it permits. These highlights include the discovery of a new population of non-thermal radio filaments; identification of new candidate supernova remnants, pulsar wind nebulae and planetary nebulae; improved radio/mid-IR classification of rare Luminous Blue Variables and discovery of associated extended radio nebulae; new radio stars identified by Bayesian cross-matching techniques; the realisation that many of the largest radio-quiet WISE HII region candidates are not true HII regions; and a large sample of previously undiscovered background HI galaxies in the Zone of Avoidance., Comment: Submitted to MNRAS. Individuals with autism spectrum development (ASD) often demonstrate atypical perception and production of biological motion (BM), a term for the movement patterns of animate beings. Given the importance of visual perceptual and motor skills for social interaction and communication, a fundamental concern for those with ASD, further investigation into these domains, their potential association and their underlying role in ASD is critical. This project aimed to examine performance in both domains using a common metric, the two-thirds power law (2/3 PL), a law of motion that characterizes many forms of human movement. Using a preferential looking paradigm, this study examined attentional patterns toward motion adhering to this law, as well as more traditional BM stimuli with temporal or spatial manipulations. The study also employed a novel task to assess adherence to the law and other kinematics during drawing movements on an iPad tablet. Measures of cognitive, motor and social functioning were collected to explore whether task-specific performance related to atypical functioning in these areas. While the study did not detect a significant relationship between domains with respect to the law, the tasks did reveal a number of atypicalities in perceptual and motor performance in autism, as well as some associations with broader social functioning. Limitations and future directions of this research are also discussed.

The health and functioning of soil ecosystems are the foundation of sustainable food production and land management. Of key importance in achieving sustainability, is the frequent measurement of soil health, and indices based on the community structure of nematodes are amongst the most widely used toolsets by soil ecologists. Thirty years after the development of the Maturity Index, we aimed to evaluate the application, utility, and future directions of nematode-based indices (NBIs). This review focused on NBIs that are calculated using the coloniser-persister classification of nematodes. Data from 672 empirical studies in terrestrial environments revealed that the NBIs presented a dissimilar usage trend. The Channel Index and Metabolic Footprints showed the strongest increase in application rates over time, thus indicating a greater interest in studying decomposition pathways and ecosystem functioning, respectively. Furthermore, nematode-based indices were mostly applied in agricultural systems associated with herbaceous crops and in studies investigating, for example, soil nutrient enrichment following manure and/or inorganic fertilizer application. We further provide a framework for selecting a focus-orientated subset of NBIs for testing hypotheses based on the underlying ecological mechanisms. Also, we highlight important considerations, including the unexpected behaviour of some nematode taxa, in the interpretation of NBIs. The improvement of NBIs relies on advancing our understanding of the autecology of nematodes. Finally, we deliver insight into the further development of NBIs considering recent methodological advancements. We highlight that NBIs have been and might become increasingly important in providing valuable information on soil ecosystem health and functioning, especially considering the urgent need for more sustainable land use.

The responsibility for building community resilience cannot and should not rest with the public sector alone. It requires all sectors to collaborate for the benefit of the entire community. Specifically, it is important for private sector organisations to participate in building community resilience because they have vested interest in the area because of their physical assets, suppliers, customers and corporate value of social responsibility. This article explores collaboration between private companies and community of Diepsloot, Johannesburg, South Africa, to build disaster resilience in the community. The study applied qualitative research methods. Data were collected through focus group interviews with the community of Diepsloot and semi-structured individual interviews with representatives of private companies operating in the vicinity of Diepsloot. A total of 55 respondents participated in the study. Respondents included five corporate social responsibility (CSR) managers from private companies and 50 community members. The findings of the study showed that private companies are involved in addressing socio-economic challenges in Diepsloot. Addressing such challenges contributes a great deal to reducing exposure to hazards and the vulnerability factors to disasters, thereby contributing to building resilience. Whereas some respondents preferred the private companies to work with the communities directly, the study recommends the use of community structures such as Community Based Organisations (CBOs) when private companies engage in community initiatives. The article contributes to better understanding of the private sector’s contribution to build community resilience.

The inner $\sim$200 pc region of the Galaxy contains a 4 million M$_{\odot}$ supermassive black hole (SMBH), significant quantities of molecular gas, and star formation and cosmic ray energy densities that are roughly two orders of magnitude higher than the corresponding levels in the Galactic disk. At a distance of only 8.2 kpc, the region presents astronomers with a unique opportunity to study a diverse range of energetic astrophysical phenomena, from stellar objects in extreme environments, to the SMBH and star-formation driven feedback processes that are known to influence the evolution of galaxies as a whole. We present a new survey of the Galactic center conducted with the South African MeerKAT radio telescope. Radio imaging offers a view that is unaffected by the large quantities of dust that obscure the region at other wavelengths, and a scene of striking complexity is revealed. We produce total intensity and spectral index mosaics of the region from 20 pointings (144 hours on-target in total), covering 6.5 square degrees with an angular resolution of 4$"$,at a central frequency of 1.28 GHz. Many new features are revealed for the first time due to a combination of MeerKAT's high sensitivity, exceptional $u,v$-plane coverage, and geographical vantage point. We highlight some initial survey results, including new supernova remnant candidates, many new non-thermal filament complexes, and enhanced views of the Radio Arc Bubble, Sgr A and Sgr B regions. Extensive research has shown that dispositional optimism has a positive impact on physical and psychological health and well-being. The present study looked at the relationship between dispositional optimism and one health-related behaviour in particular, namely exercise behaviour. Enough exercise is important for several health and well-being reasons, leaving people with good intentions to exercise, but the majority do not manage to follow through. People face daily distractors and temptations, which act as obstacles in their pursuit of a healthier lifestyle. A trait that helps combat this dilemma is self-control. The first hypothesis was that dispositional optimism would be positively associated with exercise behaviour; and the second was that trait self-control would moderate this relationship. Previous research on the topic has been somewhat mixed, hence the addition of a moderating variable to potentially help explain inconsistent findings. Data was collected using a survey study and participants were recruited via the network of the researcher. The final sample consisted of 283 participants. Results revealed no association between dispositional optimism and exercise behaviour in this study, and no moderating effect of trait self-control, which left both hypotheses unsupported. However, a significant positive association was found between trait self-control and exercise. These results raise questions regarding the effects of optimism on specific health-related behaviours, implying that more research is required in this field. Though somewhat indirectly, the findings also add to the large pool of research on the positive impact of trait self-control on behaviour.

Background Arthropod-borne pathogens and their vectors are present throughout Africa. They have been well studied in livestock of sub-Saharan Africa, but poorly studied in companion animals. Given their socioeconomic importance, the African Small Companion Animal Network (AFSCAN), as part of the WSAVA Foundation, initiated a standardized multi-country surveillance study. Methods In six countries (Ghana, Kenya, Nigeria, Tanzania, Uganda, and Namibia) in both rural and urban settings, 160 infested cats were sampled to assess their ectoparasite community (ticks and fleas), as well as the micro-parasite prevalence within those ectoparasites (60 and 118 pools of ticks and fleas, respectively) and blood (276 cats, including 116 non-infested). Results Almost two thirds of all infested cats originated from Tanzania and Kenya. Despite the large macro-geographical variation, no consistent difference was found in ectoparasite diversity and numbers between East and West Africa. Far more flea-infested than tick-infested cats were found. The most dominant ectoparasite was Ctenocephalides felis. Among the ticks, the exophilic Haemaphysalis spp. were the commonest, including species that are not typically linked with companion animals (Haemaphysalis spinulosa and Haemaphysalis elliptica). The most prevalent pathogens found in the blood and fleas were Bartonella henselae and Mycoplasma haemofelis. In the ticks, the dog-associated Hepatozoon canis was most commonly found. A high degree of co-parasitism was found in all countries and habitats. Conclusions Our continent-wide standardized field study highlights the cat’s potential to serve as a reservoir of pathogens that can be transmitted to humans or livestock, especially when cats are expected to become more commonly kept in African villages and towns.

BackgroundPharmacokinetic data for bedaquiline in children are limited. We described the pharmacokinetics and safety of bedaquiline in South African children and adolescents receiving treatment for multidrug/rifampicin-resistant tuberculosis (MDR/RR-TB) in routine care.MethodsIn this observational cohort study, children aged 6-17 years receiving bedaquiline at recommended doses as part of MDR/RR-TB treatment underwent semi-intensive pharmacokinetic sampling. Bedaquiline and the M2 metabolite plasma concentrations were quantified, and nonlinear mixed-effects modeling performed. Pediatric data were described using a pre-established model of bedaquiline pharmacokinetics in adults. The exposure reference was 187 µg ⋅ h/mL, the median weekly area under the curve (AUC) of adults at week 24 of treatment with bedaquiline. Safety was assessed through monthly clinical, blood and electrocardiogram monitoring, and treatment outcomes described.ResultsFifteen children (3 human immunodeficiency virus [HIV]-positive) with median age 13.3 years (range 6.5-16.3) were included. A bedaquiline pharmacokinetic model was adapted to be allometrically scaled in clearance and volume, centered in the median child population weight. Bedaquiline bioavailability was 57% of that in adults. Overall bedaquiline exposures were below target, and AUC reference attainment was achieved in only 3 (20%) children. Ten children experienced 27 adverse events at least possibly related to bedaquiline; no adverse events led to bedaquiline withdrawal. Two adverse events (arthritis and arthralgia) were considered severe, and 2 children had mild QT interval corrected for heart rate using Fridericia's formula (QT) prolongation.ConclusionsThe evaluated doses of bedaquiline in children ≥ 6 years of age were safe but achieved slightly lower plasma concentrations compared to adults receiving the recommended dose, possibly due to delayed food intake relative to bedaquiline administration.

BackgroundMoxifloxacin is a recommended drug for rifampin-resistant tuberculosis (RR-TB) treatment, but there is limited pediatric pharmacokinetic and safety data, especially in young children. We characterize moxifloxacin population pharmacokinetics and QT interval prolongation and evaluate optimal dosing in children with RR-TB.MethodsPharmacokinetic data were pooled from 2 observational studies in South African children with RR-TB routinely treated with oral moxifloxacin once daily. The population pharmacokinetics and Fridericia-corrected QT (QTcF)-interval prolongation were characterized in NONMEM. Pharmacokinetic simulations were performed to predict expected exposure and optimal weight-banded dosing.ResultsEighty-five children contributed pharmacokinetic data (median [range] age of 4.6 [0.8-15] years); 16 (19%) were aged <2 years, and 8 (9%) were living with human immunodeficiency virus (HIV). The median (range) moxifloxacin dose on pharmacokinetic sampling days was 11 mg/kg (6.1 to 17). Apparent clearance was 6.95 L/h for a typical 16-kg child. Stunting and HIV increased apparent clearance. Crushed or suspended tablets had faster absorption. The median (range) maximum change in QTcF after moxifloxacin administration was 16.3 (-27.7 to 61.3) ms. No child had QTcF ≥500 ms. The concentration-QTcF relationship was nonlinear, with a maximum drug effect (Emax) of 8.80 ms (interindividual variability = 9.75 ms). Clofazimine use increased Emax by 3.3-fold. Model-based simulations of moxifloxacin pharmacokinetics predicted that current dosing recommendations are too low in children.ConclusionsMoxifloxacin doses above 10-15 mg/kg are likely required in young children to match adult exposures but require further safety assessment, especially when coadministered with other QT-prolonging agents.

This thesis presents a sum-product inference algorithm for platform navigation called Multi-modal iSAM (incremental smoothing and mapping). Common Gaussian only likelihoods are restrictive and require a complex front-end processes to deal with non-Gaussian measurements. Instead, our approach allows the front-end to defer ambiguities with non-Gaussian measurement models. We retain the acyclic Bayes tree (and incremental update strategy) from the predecessor iSAM2 max-product algorithm [Kaess et al., IJRR 2012]. The approach propagates continuous beliefs on the Bayes (Junction) tree, which is an efficient symbolic refactorization of the nonparametric factor graph, and asymptotically approximates the underlying Chapman-Kolmogorov equations. Our method tracks dominant modes in the marginal posteriors of all variables with minimal approximation error, while suppressing almost all low likelihood modes (in a non-permanent manner). Keeping with existing inertial navigation, we present a novel, continuous-time, retroactively calibrating inertial odometry residual function, using preintegration to seamlessly incorporate pure inertial sensor measurements into a factor graph. We centralize around a factor graph (with starved graph databases) to separate elements of the navigation into an ecosystem of processes. Designers often make assumptions in the behavioral models of circuit elements when simulating circuits. For instance high-frequency effects have been neglected so far in the design of superconductor digital circuits, while much has been done in the past to model them. Indeed these effects had little influence on the accuracy of digital circuits simulations until recently. The increase in clock frequency, combined with longer paths between cells and higher yield requirements for large scale circuits fabrication, has led to the need of more accurate models, including in particular high frequency effects such as quasi-particle losses. To do so, this work uses a state-space model that describes the circuit under study with internal state variables and a set of first-order differential equations. We extract the state-space model while analytically enforcing the DC requirements of superconductors that are required to account for flux-trapping. The model accurately traps flux at DC, and given the model is fitted with enough poles, the high-frequency effects are also accurate relative to the reference model. The high-frequency effects have been investigated on a practical circuit: a long-distance Passive Transmission Line (PTL) designed for the high-density MIT Lincoln Labs SFQSee process. Results obtained in the time domain allow to observe the effects of dispersion of pulses traveling on long paths of PTLs. Ease of doing business with municipalities and within municipal areas refers to the simplicity, effortlessness, and extent to which private enterprises can establish and conduct their business. It is influenced by multiple factors such as the conduciveness of the statutory and regulatory framework and the effectiveness of municipal structures and processes, in order to reduce bureaucratic red tape for the simplification of administrative processes such as the registration of business property, the issuing of permits, and access to electricity. Municipalities thus play a significant role in attracting and retaining business investment by establishing a conducive climate for business growth and local economic development. The aim of this article was to analyze the findings of a survey that was conducted to pinpoint business investment pull and push factors in selected municipalities in South Africa. By means of semi-structured interviews, a survey was undertaken with seven target groups. The obtained data were then triangulated with a desktop analysis of sampled businesses (i.e., case studies) that had relocated, mainly due to municipal service delivery failures. The survey revealed the underlying factors that lead to ease of doing business weaknesses, uncovered winning strategies to attract business investment, and proposed measures to enhance municipal business retention praxis.

Drought and water scarcity constrain the socioeconomic development of many (semi-)arid regions of Southern Africa. Moreover, due to the increase of water withdrawals upstream, the Limpopo River is no longer perennial in Mozambique. Fortunately, its river bed can store significant amounts of freshwater, because of the occurrence of thick and often coarse sand deposits formed through pronounced dryland weathering, erosion, and sedimentation in the river channel. Such so-called “sand rivers” exist in many parts of semi-arid Africa and have varying configurations and hydrological conditions. The current research aims to comparatively assess the Limpopo sand river aquifer in terms of recharge and discharge dynamics, storage potential, and interactions with the surface water flow, as a function of its specific hydrological conditions: its large size, location downstream of a dam releasing permanent ecological flow, and its relatively undeveloped state. For this purpose field investigations were carried out at two sites, involving groundwater level measurements, 2D geoelectrical surveying, water chemical and stable isotope analysis, and sediment classification. These investigations reveal the occurrence of medium to coarse sands with thicknesses that can reach 10–15 m, dropping to 2–5 m in the main river channel, underlain by less permeable clays and silts. Analysis of the river level shows that large parts of the sand river are flooded almost every year, providing optimal conditions for recurring and rapid recharge of the system (confirmed by infiltration tests) through two mechanisms: direct infiltration of surface runoff and lateral flow toward non-flooded areas of the river valley, also confirmed by the chemical and isotope study. During the dry season, groundwater provides base flow to the river and the average water level drop in the sand river system is about 1.8 m. RATIONALE: Diaper dermatitis (DD) is a significant problem in the care of newborns, infants, and young children and good recognition of signs and symptoms optimises timely treatment. There is also a need for a standardised set of related descriptors to enable communication between health care providers about diagnosis and treatment. OBJECTIVE: The study aimed to review the literature for descriptive words to define the clinical signs, symptoms, and characteristics, including anatomical locations and assessment scales or tools, in the newborn, infants and young children population who have been diagnosed with DD. METHODS: A systematic review process utilising PRISMA guidelines [1] was conducted using advanced search techniques to analyse the terms from the database thesauri and keywords. Data bases including Ovid MEDLINE® and Epub Ahead of Print, in-Process & Other Non-Indexed Citations, Daily and Versions® were searched from 1946 to March 18, 2021. Cochrane Handbook Searching for and selecting studies were used as a guide and as best practice to conduct the search. RESULTS: Of the 551 publications originally identified, 55 full text publications were examined and 21 met the inclusion criteria. The wide range of descriptors used in the literature for DD supports the need for a clearer approach to report the signs and symptoms, and the severity, of DD. CONCLUSIONS: These findings point to gaps in the literature, both in recording signs and symptoms of DD and features such as wetness, infection, anatomical location, severity and pain in newborns, infants, and young children. © 2019 IEEE. The recent success of object detection systems motivates object-based representations for robot navigation; i.e. semantic simultaneous localization and mapping (SLAM). The semantic SLAM problem can be decomposed into a discrete inference problem: determining object class labels and measurement-landmark correspondences (the data association problem), and a continuous inference problem: obtaining the set of robot poses and object locations in the environment. A solution to the semantic SLAM problem necessarily addresses this joint inference, but under ambiguous data associations this is in general a non-Gaussian inference problem, while the majority of previous work focuses on Gaussian inference. Previous solutions to data association either produce solutions between potential hypotheses or maintain multiple explicit hypotheses for each association. We propose a solution that represents hypotheses as multiple modes of an equivalent non-Gaussian sensor model. We then solve the resulting non-Gaussian inference problem using nonparametric belief propagation. © 2020 IEEE. This paper presents a method for processing sparse, non-Gaussian multimodal data in a simultaneous localization and mapping (SLAM) framework using factor graphs. Our approach demonstrates the feasibility of using a sum-product inference strategy to recover functional belief marginals from highly non-Gaussian situations, relaxing the prolific unimodal Gaussian assumption. The method is more focused than conventional multi-hypothesis approaches, but still captures dominant modes via multi-modality. The proposed algorithm exists in a trade space that spans the anticipated uncertainty of measurement data, task-specific performance, sensor quality, and computational cost. This work leverages several major algorithm design constructs, including clique recycling, to put an upper bound on the allowable computational expense - a major challenge in non-parametric methods. To better demonstrate robustness, experimental results show the feasibility of the method on at least two of four major sources of non-Gaussian behavior: i) the first introduces a canonical range-only problem which is always underdetermined although composed exclusively from Gaussian measurements; ii) a real-world AUV dataset, demonstrating how ambiguous acoustic correlator measurements are directly incorporated into a non-Gaussian SLAM solution, while using dead reckon tethering to overcome short term computational requirements.

Importance: In the early COVID-19 pandemic, SARS-CoV-2 testing was only accessible and recommended for symptomatic persons or adults. This restriction hampered assessment of the true incidence of SARS-CoV-2 infection in children as well as detailed characterization of the SARS-CoV-2 disease spectrum and how this spectrum compared with that of other common respiratory illnesses. Objective: To estimate the community incidence of SARS-CoV-2 infection in children and parents and to assess the symptoms and symptom severity of respiratory illness episodes involving SARS-CoV-2-positive test results relative to those with SARS-CoV-2-negative test results. Design, Setting, and Participants: This cohort study randomly selected Dutch households with at least 1 child younger than 18 years. A total of 1209 children and adults from 307 households were prospectively followed up between August 25, 2020, and July 29, 2021, covering the second and third waves of the COVID-19 pandemic. Participation included SARS-CoV-2 screening at 4- to 6-week intervals during the first 23 weeks of participation (core study period; August 25, 2020, to July 29, 2021). Participants in all households finishing the core study before July 1, 2021, were invited to participate in the extended follow-up and to actively report respiratory symptoms using an interactive app until July 1, 2021. At new onset of respiratory symptoms or a SARS-CoV-2 positive test result, a household outbreak study was initiated, which included daily symptom recording, repeated polymerase chain reaction testing (nose-throat swabs and saliva and fecal samples), and SARS-CoV-2 antibody measurement (paired dried blood spots) in all household members. Outbreaks, households, and episodes of respiratory illness were described as positive or negative depending on SARS-CoV-2 test results. The health and functioning of soil ecosystems are the foundation of sustainable food production and land management. Of key importance in achieving sustainability, is the frequent measurement of soil health, and indices based on the community structure of nematodes are amongst the most widely used toolsets by soil ecologists. Thirty years after the development of the Maturity Index, we aimed to evaluate the application, utility, and future directions of nematode-based indices (NBIs). This review focused on NBIs that are calculated using the coloniser-persister classification of nematodes. Data from 672 empirical studies in terrestrial environments revealed that the NBIs presented a dissimilar usage trend. The Channel Index and Metabolic Footprints showed the strongest increase in application rates over time, thus indicating a greater interest in studying decomposition pathways and ecosystem functioning, respectively. Furthermore, nematode-based indices were mostly applied in agricultural systems associated with herbaceous crops and in studies investigating, for example, soil nutrient enrichment following manure and/or inorganic fertilizer application. We further provide a framework for selecting a focus-orientated subset of NBIs for testing hypotheses based on the underlying ecological mechanisms. Also, we highlight important considerations, including the unexpected behaviour of some nematode taxa, in the interpretation of NBIs. The improvement of NBIs relies on advancing our understanding of the autecology of nematodes. Finally, we deliver insight into the further development of NBIs considering recent methodological advancements. We highlight that NBIs have been and might become increasingly important in providing valuable information on soil ecosystem health and functioning, especially considering the urgent need for more sustainable land use.
