Background: Encephalitis is a worldwide public health issue, with a substantially high burden among children in southeast Asia. We aimed to determine the causes of encephalitis in children admitted to hospitals across the Greater Mekong region by implementing a comprehensive state-of-the-art diagnostic procedure harmonised across all centres, and identifying clinical characteristics related to patients' conditions. Methods: In this multicentre, observational, prospective study of childhood encephalitis, four referral hospitals in Cambodia, Vietnam, Laos, and Myanmar recruited children (aged 28 days to 16 years) who presented with altered mental status lasting more than 24 h and two of the following minor criteria: fever (within the 72 h before or after presentation), one or more generalised or partial seizures (excluding febrile seizures), a new-onset focal neurological deficit, cerebrospinal fluid (CSF) white blood cell count of 5 per mL or higher, or brain imaging (CT or MRI) suggestive of lesions of encephalitis. Comprehensive diagnostic procedures were harmonised across all centres, with first-line testing was done on samples taken at inclusion and results delivered within 24 h of inclusion for main treatable causes of disease and second-line testing was done thereafter for mostly non-treatable causes. An independent expert medical panel reviewed the charts and attribution of causes of all the included children. Using multivariate analyses, we assessed risk factors associated with unfavourable outcomes (ie, severe neurological sequelae and death) at discharge using data from baseline and day 2 after inclusion. This study is registered with ClinicalTrials.gov, NCT04089436, and is now complete. Findings: Between July 28, 2014, and Dec 31, 2017, 664 children with encephalitis were enrolled. Median age was 4·3 years (1·8–8·8), 295 (44%) children were female, and 369 (56%) were male. A confirmed or probable cause of encephalitis was identified in 425 (64%) patients

One chapter is devoted to analyzing a stochastic norm estimate which is useful when computing low-rank approximations of matrices. This estimate allows us to compute approximations with relative error close to machine precision, which previously was not possible.

The order Picornavirales is one of the most important viral orders in terms of virus diversity and genome organizations, ranging from a mono- or bi-cistronic expression strategies to the recently described poly-cistronic Polycipiviridae viruses. We report here the description and characterization of a novel picorna-like virus identified in rectal swabs of frugivorous bats in Cambodia that presents an unusual genome organization. Kandabadicivirus presents a unique genome architecture and distant phylogenetic relationship to the proposed Badiciviridae family. These findings highlight a high mosaicism of genome organizations among the Picornavirales.

The Japanese encephalitis virus (JEV) is one of the main causes of encephalitis in Asia, including Cambodia. An understanding of the interactions between JEV hosts and vectors (Diptera: Culicidae) remains rare in the context of expanding urbanization. The relative abundance, species diversity and population dynamics of potential JEV vectors were studied between August 2015 and July 2016 on a peri-urban and rural pig farm in Kandal province, Cambodia, where JEV is circulating. Five similar environments in the two farms were selected for mosquito trapping: pig farm, cattle house, river/canals, household/ponds and paddy fields. The main objective was to describe the distribution and the dynamics of the main JEV vector mosquito species. In total, 83,013 mosquitoes from 20 species were caught in rural and peri-urban areas, and 82.3% of the mosquitoes were potential JEV vector species. In peri-urban areas, Culex (Cx.) gelidus was the most abundant species, followed by Cx. vishnui subgroup and Cx. tritaeniorhynchus. In rural areas, the same species were dominant: Cx. vishnui subgroup, Cx. gelidus and Cx. tritaeniorhynchus. The vast majority of mosquitoes (95.9%) were collected in close proximity to pigs and cattle. In conclusion, JEV vectors were present at all study sites and throughout all months of the year, supporting a continuous circulation of JEV in Cambodia.

The Pigmented Lesion Assay (PLA, sensitivity 91-95%, specificity 69-91%, negative predictive value ?99%) is a commercially available, non-invasive gene expression test that helps dermatologists guide pigmented lesion management decisions and rule out melanoma. Earlier studies have demonstrated high clinical utility and no missed melanomas in a 3-6-month follow-up period. We undertook the current investigations to provide 12-month follow-up data on PLA(-) tests, and to further confirm utility. A 12-month chart review follow-up of 734 pigmented lesions that had negative PLA results from 5 US dermatology centers was performed. Thirteen of these lesions (1.8%) were biopsied in the follow-up period and submitted for histopathologic review. None of the lesions biopsied had a histopathologic diagnosis of melanoma. The test's utility was studied further in a registry (N=1575, 40 US dermatology offices, 62 participating providers), which demonstrated that 99.9% of PLA(-) lesions were clinically monitored, thereby avoiding a surgical procedure, and 96.5% of all PLA(+) lesions were appropriately biopsied, most commonly with a tangential shave. This long-term follow-up study confirms the PLA's high negative predictive value and high utility in helping guide the management of pigmented lesions to avoid unnecessary surgical procedures.

Viruses belonging to this family were only reported from arthropod hosts. We describe here the first full genome of a distant polycipivirus-related virus identified in frugivorous bat stools in Cambodia.

We present new algorithms for the randomized construction of hierarchically semi-separable matrices, addressing several practical issues. The HSS construction algorithms use a partially matrix-free, adaptive randomized projection scheme to determine the maximum off-diagonal block rank. We develop both relative and absolute stopping criteria to determine the minimum dimension of the random projection matrix that is sufficient for the desired accuracy. Two strategies are discussed to adaptively enlarge the random sample matrix: repeated doubling of the number of random vectors, and iteratively incrementing the number of random vectors by a fixed number. The relative and absolute stopping criteria are based on probabilistic bounds for the Frobenius norm of the random projection of the Hankel blocks of the input matrix. We discuss parallel implementation and computation and communication cost of both variants. Parallel numerical results for a range of applications, including boundary element method matrices and quantum chemistry Toeplitz matrices, show the effectiveness, scalability and numerical robustness of the proposed algorithms., Comment: 24 pages, 4 figures, 20 references

Japanese encephalitis remains the most important cause of viral encephalitis in humans in several southeast Asian countries, including Cambodia, causing at least 65 000 cases of encephalitis per year. This vector-borne viral zoonosis – caused by Japanese encephalitis virus (JEV) – is considered to be a rural disease and is transmitted by mosquitoes, with birds and pigs being the natural reservoirs, while humans are accidental hosts. In this study we report the first two JEV isolations in Cambodia from human encephalitis cases from two studies on the aetiology of central nervous system disease, conducted at the two major paediatric hospitals in the country. We also report JEV isolation from Culextritaeniorhynchus mosquitoes and from pig samples collected in two farms, located in peri-urban and rural areas. Out of 11 reverse-transcription polymerase chain reaction-positive original samples, we generated full-genome sequences from 5 JEV isolates. Five additional partial sequences of the JEV NS3 gene from viruses detected in five pigs and one complete coding sequence of the envelope gene of a strain identified in a pig were generated. Phylogenetic analyses revealed that JEV detected in Cambodia belonged to genotype I and clustered in two clades: genotype I-a, mainly comprising strains from Thailand, and genotype I-b, comprising strains from Vietnam that dispersed northwards to China. Finally, in this study, we provide proof that the sequenced JEV strains circulate between pigs, Culex tritaeniorhynchus and humans in the Phnom Penh vicinity. (Résumé d'auteur)

We report continuing results pertaining to the above referenced grant, DAAG55-98-1-0511. As detailed herein, we have successfully synthesized a number of metal cluster core-based star architectures. These architectures are designed so that their encapsulating properties can be directly compared with those of dendrimers prepared previously in our research group. We are now embarking on the measurement of the electrochemical proper- ties of these star molecules and will then publish these results as a direct comparison of star and hyperbranched architectures as regards their encapsulating properties. Encapsulation is a critical behavior in the design of nanoscale electronic elements, nanoscale sensors, etc.

The dependence of the second- and third-order polarizabilities ((beta) and (gamma) ) on ground-state polarization was measured for a series of donor-acceptor polyenes using electric field induced second harmonic generation and third harmonic generation, respectively. The changes in ground-state polarization, associated with the donor/acceptor strength or solvent polarity, were probed by x-ray crystallography, 1H-NMR, electronic absorption, and Raman spectroscopies. The observed behavior of (beta) and (gamma) as a function of ground- state polarization agrees well with theoretical predictions. In particular, positive and negative peaks, as well as sign changes, were observed for both (beta) and (gamma) . The dependences for (beta) and (gamma) are consistent with a derivative relationship between them. In addition, the third-order polarizability of a series of molecules possessing zero bond length alternation was found to be negative, in agreement with predictions based on the relationship between the polarizabilities and ground-state geometry.

High molecular weight, amorphous, partially substituted polyacetylenes (poly-RCOT) have been prepared using the ring-opening metathesis polymerization of substituted cyclooctetraenes. Spin-cast films of these polymers yielded unusually sharp, well-defined reversible electrochemistry for oxidative and reductive doping processes. As the substituent on the polymer chain was varied (R = alkyl, tert-butoxy, trimethylsilyl, and p-X-phenyl), the potentials for oxidative and reductive doping changed by 0.3 V. In addition, the separation between the oxidative and reductive doping processes varied from 1.66 V to > 2.0 V. Coulometry suggested that the reversible doping step represented a transfer of 1 electron for every 13-15 double bonds of the polymer. In contrast, the electrochemistry of predominantly- cis poly-RCOT films was irreversible, and indicated the presence of an electrochemical cis-trans isomerization on the first voltammetric sweep through either reductive or oxidative doping. Spectroelectrochemical studies indicated that the electrochemically doped poly- RCOT materials R=sec-butyl, (CH3)3Si) possessed mid-gap transitions at energies of 0.8-0.9 eV. The redox chemistry of the soluble, poly-RCOT (R=sec-butyl, (CH3) 3Si) species in CH2Cl2 solution was also investigated. Voltammetric studies showed that both the reductive and oxidative doping processes were chemically irreversible, and indicated that the dissolved polymers were electroactive over a wide range of potentials.

Using well-defined tungsten-based olefin metathesis catalysts, a family of partially substituted polyacetylenes have been synthesized via the ring-opening metathesis polymerization (ROMP) of monosubstituted cyclooctatetraenes (RCOT). These polymers are highly conjugated as evidenced by their visible absorption maxima, they are of high molecular weight as evidenced by gel permeation chromatography, and most members of the family are soluble in the as-synthesized, predominantly cis form. The polymers can be isomerized to the predominantly transform using heat or light. The rate of thermal isomerization was monitored by visible absorption spectroscopy. Polymers containing, in general, secondary or tertiary groups immediately adjacent to the main chain remain soluble in the trans form and are, in most cases, still highly conjugated., Prepared for publication in Jnl. of Am. e recently reported that there is an optimal combination of donor and acceptor strengths for a given molecular length and bridge structure that maximizes (beta) . For this combination, there is the correct degree of bond length alternation and asymmetry in the molecule. Our recent findings suggest that molecules that can be viewed as asymmetric cyanines with relatively small amounts of bond length alternation are nearly optimal. In this manner, we have identified molecules with nonlinearities many times that of conventional chromophores for a given length. In this paper, we will present a new computational analysis that allows the correlation of bond length alternation with hyperpolarizabilities and will present EFISH data on simple donor-acceptor polyene chromophores.
