Abstract

Bile duct injury (BDI) is a dangerous complication of cholecystectomy, with significant postoperative sequelae for the patient in terms of morbidity, mortality, and long-term quality of life. BDIs have an estimated incidence of 0.4–1.5%, but considering the number of cholecystectomies performed worldwide, mostly by laparoscopy, surgeons must be prepared to manage this surgical challenge. Most BDIs are recognized either during the procedure or in the immediate postoperative period. However, some BDIs may be discovered later during the postoperative period, and this may translate to delayed or inappropriate treatments. Providing a specific diagnosis and a precise description of the BDI will expedite the decision-making process and increase the chance of treatment success. Subsequently, the choice and timing of the appropriate reconstructive strategy have a critical role in long-term prognosis. Currently, a wide spectrum of multidisciplinary interventions with different degrees of invasiveness is indicated for BDI management. These World Society of Emergency Surgery (WSES) guidelines have been produced following an exhaustive review of the current literature and an international expert panel discussion with the aim of providing evidence-based recommendations to facilitate and standardize the detection and management of BDIs during cholecystectomy. In particular, the 2020 WSES guidelines cover the following key aspects: (1) strategies to minimize the risk of BDI during cholecystectomy; (2) BDI rates in general surgery units and review of surgical practice; (3) how to classify, stage, and report BDI once detected; (4) how to manage an intraoperatively detected BDI; (5) indications for antibiotic treatment; (6) indications for clinical, biochemical, and imaging investigations for suspected BDI; and (7) how to manage a postoperatively detected BDI.

Abstract

Biomaterials derived from microalgae are promising as a source of biofuels, drugs, cosmetics, and foods. Among different microalgae, Euglena gracilis has attracted attention for biomaterials production due to its high productivity of paramylon, a potential drug for HIV and colon cancer. To enhance the productivity of paramylon in E.gracilis, directed evolution is a promising approach. In general, directed evolution consists of three key steps: mutagenesis, selection, and amplification. For applying it to the enhancement of the paramylon productivity of E.gracilis, mutagenesis and selection steps are challenging because altering nuclear genome of E.gracilis is difficult due to its polyploid nature and no biomarker such as fluorescent dyes is available for quantifying paramylon. In this study, we tackle these difficulties by combining quantum-beam irradiation for mutagenesis and high-throughput Raman-activated cell screening for label-free selection.

Abstract

The spatial resolution along the pad-row direction was measured with a GEM-based TPC prototype for the future linear collider experiment in order to understand its performance for tracks with finite projected angles with respect to the pad-row normal. The degradation of the resolution due to the angular pad effect was confirmed to be consistent with the prediction of a simple calculation taking into account the cluster-size distribution and the avalanche fluctuation., Comment: 11 pages, 4 figures

Abstract

Methicillin-resistant staphylococci are disseminated all over the world and are frequent causes of health care- and community-associated infections. Methicillin-resistant strains typically carry the acquired mecA gene that encodes a low-affinity penicillin-binding protein (PBP), designated PBP2a or PBP2′. In most strains, mecA is part of a chromosomally integrated mobile genetic element called staphylococcal cassette chromosome mec (SCCmec). The mecA gene is widely disseminated among Staphylococcus aureus and other staphylococcal species, and its expression is essential for the methicillin-resistant phenotype. Recently, mecA gene homologues that are only distantly related to mecA have been identified in the genomes of staphylococci and some related bacterial species (Table 1). So far, four groups of mecA homologues have been described based on their degree of homology to the earliest identified mecA gene. We believe that this diversity warrants a new naming system based on phylogenetic principles which can also serve as a guideline for the reporting of additional novel mecA homologues that may be identified in the future.

The SCCmecs were classified into novel types, namely, IX, X, V(5C2&5) subtype c, and IVa. Type V(5C2&5) subtype c, IX, and X SCCmecs carried genes conferring resistance to metals. The structures of SCCmecs from CC398 strains were distinct from those normally found in humans, adding to the evidence that humans are not the original host for CC398.

Abstract

Argon with an admixture of CF4 is expected to be a good candidate for the gas mixture to be used for a time projection chamber (TPC) in the future linear collider experiment because of its small transverse diffusion of drift electrons especially under a strong magnetic field. In order to confirm the superiority of this gas mixture over conventional TPC gases we carried out cosmic ray tests using a GEM-based TPC operated mostly in Ar-CF4-isobutane mixtures under 0 - 1 T axial magnetic fields. The measured gas properties such as gas gain and transverse diffusion constant as well as the observed spatial resolution are presented., Comment: A supplemental material (Addendum to be published in Nucl. Abstract

A time projection chamber (TPC) is a strong candidate for the central tracker of the international linear collider (ILC) experiment and we have been conducting a series of cosmic ray experiments under a magnetic field up to 4 T, using a small prototype TPC with a replaceable readout device: multi-wire proportional chamber (MWPC) or gas electron multiplier (GEM). We first confirmed that the MWPC readout could not be a fall-back option of the ILC-TPC under a strong axial magnetic field of 4 T since its spatial resolution suffered severely from the so called E x B effect in the vicinity of the wire planes. The GEM readout, on the other hand, was found to be virtually free from the E x B effect as had been expected and gave the resolution determined by the transverse diffusion of the drift electrons (diffusion limited). Furthermore, GEMs allow a wider choice of gas mixtures than MWPCs. Among the gases we tried so far a mixture of Ar-CF4-isobutane, in which MWPCs could be prone to discharges, seems promising as the operating gas of the ILC-TPC because of its small diffusion constant especially under a strong magnetic field. We report the measured drift properties of this mixture including the diffusion constant as a function of the electric field and compare them with the predictions of Magboltz. They multiplied much faster than health-care-associated MRSA and were resistant to fewer nonbeta-lactam antibiotics. They seem to have been derived from more diverse S. aureus populations than health-care-associated MRSA strains.
