Background. Breast cancer is the most common cancer in women worldwide. Approximately 80% of breast cancers are oestrogen receptor positive (ER+). Patients treated surgically are usually recommended adjuvant endocrine therapy (AET) for 5–10 years. AET significantly reduces recurrence, but up to 50% of women do not take it as prescribed. Objective. To co-design and develop an intervention to support AET adherence and improve health-related quality-of-life (QoL) in women with breast cancer. Methods. Design and development of the HT&Me intervention took a person-based approach and was guided by the Medical Research Council framework for complex interventions, based on evidence and underpinned by theory. Literature reviews, behavioural analysis, and extensive key stakeholder involvement informed ‘guiding principles’ and the intervention logic model. Using co-design principles, a prototype intervention was developed and refined. Results. The blended tailored HT&Me intervention supports women to self-manage their AET. It comprises initial and follow-up consultations with a trained nurse, supported with an animation video, a web-app and ongoing motivational ‘nudge’ messages. It addresses perceptual (e.g. doubts about necessity, treatment concerns) and practical (e.g. forgetting) barriers to adherence and provides information, support and behaviour change techniques to improve QoL. Iterative patient feedback maximised feasibility, acceptability, and likelihood of maintaining adherence; health professional feedback maximised likelihood of scalability. Conclusions. HT&Me has been systematically and rigorously developed to promote AET adherence and improve QoL, and is complemented with a logic model documenting hypothesized mechanisms of action. An ongoing feasibility trial will inform a future randomised control trial of effectiveness and cost-effectiveness.

This chapter is concerned with low power epidermally or conformally mounted flexible sensing tags that use passive backscattered communications links with the UHF Gen2 RFID protocol (865–928 MHz). Very low-profile polymer or fabric substrate skin mountable UHF RFID tag designs are introduced, with proposed fabrication by inkjet printing, and incorporation of sensing, while alternatively, integrated, flexible silicon NFC sensing systems at 13.56 MHz have been pioneered.

Significant efforts have been devoted in the last twenty years to developing compounds that can interfere with the aggregation pathways of proteins related to misfolding disorders, including Alzheimer’s and Parkinson’s diseases. However, no disease-modifying drug has become available for clinical use to date for these conditions. One of the main reasons for this failure is the incomplete knowledge of the molecular mechanisms underlying the process by which small molecules interact with protein aggregates and interfere with their aggregation pathways. Here, we leverage the single molecule morphological and chemical sensitivity of infrared nanospectroscopy to provide the first direct measurement of the structure and interaction between single Aβ42 oligomeric and fibrillar species and an aggregation inhibitor, bexarotene, which is able to prevent Aβ42 aggregation in vitro and reverses its neurotoxicity in cell and animal models of Alzheimer’s disease. Our results demonstrate that the carboxyl group of this compound interacts with Aβ42 aggregates through a single hydrogen bond. These results establish infrared nanospectroscopy as a powerful tool in structure-based drug discovery for protein misfolding diseases.

Objectives: The purpose of this study was to assess the reliability and validity of Swedish translations of the Beliefs about Medicines Questionnaire-Specific (BMQ-Specific) and Brief Illness Perception Questionnaire (B-IPQ) for use in adolescents with ADHD. Methods: Forward and backward translations of the BMQ-Specific and B-IPQ scales to Swedish were conducted and reviewed by adolescents with ADHD and professionals. The validity and reliability of both questionnaires were investigated in a cross-sectional study of 101 adolescents (13-17 years) on a long-term prescription of ADHD medication recruited from two child and adolescent psychiatric outpatient clinics in Sweden. Results: Regarding the BMQ-Specific, principal component analysis (PCA) loadings confirmed the previously defined components of Specific-Necessity and Specific-Concern. The PCA for B-IPQ revealed two components, the first one, B-IPQ Consequences, captured questions regarding perceptions of the implication of having ADHD (items 1, 2, 5, 6 and 8) and the second one, B-IPQ-Control, the perceptions of the capability to manage the ADHD disorder (items 3, 4 and 7). The Cronbach alpha coefficients for BMQ-Specific-Necessity scale was α = 0.80, for BMQ-Specific-Concern scale α = 0.75, B-IPQ Consequences α = 0.74 and for B-IPQ-Control α = 0.44. Conclusions: The present results prove the Swedish translation of BMQ-Specific and B-IPQ to be valid and reliable for utilization in adolescents with ADHD. The PCA confirmed the original components for BMQ-Specific and the recent findings of two main B-IPQ components describing emotional and cognitive implications versus the capability for self-care maintenance of ADHD.

This study describes the development of an RFID tag system and antenna for real-time ac current sensing of individual appliances in smart homes. The operating principle of the tag system is based on the tag antenna tuning via a tuning circuit. The auto-tuning chip is embedded to compensate the antenna matching and stores the impedance tuning in the form of a 5–bit sensor code. The tag wirelessly streams the 5–bit sensor code that represents the ac current drawn by the electrical load, to the dedicated RFID reader in the range of 3 m at 868 MHz. The tag device is an energy harvester and a cost-effective ac current sensing solution compared to commercial smart meters in smart power metering systems. The antenna is designed to fit around the housing of the current sensor.

Objective: This individual patient data (IPD) meta‐analysis aimed to evaluate the effects of psychosocial interventions (PSI) on quality of life (QoL), emotional function (EF), and social function (SF) in patients with cancer, and to study moderator effects of demographic, clinical, personal, and intervention‐related characteristics. Methods: Relevant studies were identified via literature searches in 4 databases. We pooled IPD from 22 (n = 4217) of 61 eligible randomized controlled trials. Linear mixed‐effect model analyses were used to study intervention effects on the post‐intervention values of QoL, EF, and SF (z‐scores), adjusting for baseline values, age, and cancer type. We studied moderator effects by testing interactions with the intervention for demographic, clinical, personal, and intervention‐related characteristics, and conducted subsequent stratified analyses for significant moderator variables.Results: PSI significantly improved QoL (β = 0.14,95%CI = 0.06;0.21), EF (β = 0.13,95%CI = 0.05;0.20), and SF (β = 0.10,95%CI = 0.03;0.18). Significant differences in effects of different types of PSI were found, with largest effects of psychotherapy. The effects of coping skills training were moderated by age, treatment type, and targeted interventions. Effects of psychotherapy on EF may be moderated by cancer type, but these analyses were based on 2 randomized controlled trials with small sample sizes of some cancer types. Conclusions: PSI significantly improved QoL, EF, and SF, with small overall effects. However, the effects differed by several demographic, clinical, personal, and intervention‐related characteristics. Our study highlights the beneficial effects of coping skills training in patients treated with chemotherapy, the importance of targeted interventions, and the need of developing interventions tailored to the specific needs of elderly patients.

In this thesis I examine the current state of relations in Cambodia between individuals, their families and elite forces, and the direction taken by traditional spirit beliefs since the introduction of Western capital and education. I argue that Cambodia has been in the control of elites at least since the days of the Khmer kings from the 7th century, and that the didactic elements of Theravada Buddhist literature have been a critically important controlling factor. In my novel and my exegesis I examine the concept of the ‘ideal woman’ at the centre of the Buddhist social model and discuss the radical changes visited upon young women since the introduction of internationalism. In my exegesis the sociological theories of Max Weber and George Herbert Mead are used to show how Cambodian peasant life has been sustained for centuries by the Symbolic Interaction of individuals with family and Theravada Buddhism; I argue that in the last twenty years there has been a shift towards Marxian Conflict Theory. In my novel the advent of capital and factories has been followed by unionism, which I dramatise through the struggle of industrial garment workers for wage improvements. A related romance between an Australian man and Cambodian woman links the reader to an examination of the modern Cambodian family and the state of traditional spirit beliefs. My exegesis places my novel in the context of modern literature written by outsiders about Cambodia, and my interviews with five Cambodian writers show the confines still placed by Government on writing by Cambodians. I look at the impact of new capital on the lives of young women in the garment factories and examine the various types of relationship between Western men and Cambodian women. Drawing on my own interviews with forty young Cambodians, I argue that the new generation of young people has retained family ties and many traditional beliefs despite new materialism, Western education and often physical separation from their fa

Tongue control with low profile, passive mouth tags is demonstrated as a human–device interface by communicating values of tongue-tag separation over a wireless link. Confusion matrices are provided to demonstrate user accuracy in targeting by tongue position. Accuracy is found to increase dramatically after short training sequences with errors falling close to 1% in magnitude with zero missed targets. The rate at which users are able to learn accurate targeting with high accuracy indicates that this is an intuitive device to operate. The significance of the work is that innovative very unobtrusive, wireless tags can be used to provide intuitive human–computer interfaces based on low cost and disposable mouth mounted technology. With the development of an appropriate reading system, control of assistive devices such as computer mice or wheelchairs could be possible for tetraplegics and others who retain fine motor control capability of their tongues. The tags contain no battery and are intended to fit directly on the hard palate, detecting tongue position in the mouth with no need for tongue piercings.

El Beliefs about Medicines Questionnaire (BMQ) se diseñó para evaluar las creencias sobre la medicación y consta de dos secciones, BMQ-General y BMQEspecífico. El objetivo de este estudio instrumental fue evaluar la estructura factorial y fiabilidad de la versión española del BMQ. En el estudio del BMQ-Específico participaron 156 pacientes, 97 diabéticos tratados con insulina y 59 pacientes con medicación antihipertensiva. Las propiedades del BMQ-General se analizaron con las muestras de pacientes crónicos y en un grupo de 256 jóvenes universitarios. Se obtuvo un patrón bifactorial para ambas escalas de la versión española del BMQ, al igual que la versión original. Se comprobó su validez para discriminar entre pacientes con distintas modalidades de tratamiento. Los pacientes tratados con insulina obtuvieron puntuaciones significativamente mayores en la escala Necesidad en comparación con los sujetos hipertensos. La versión española del BMQ es un instrumento válido y fiable para evaluar las creencias sobre la medicación, aunque es preciso realizar otros estudios con diferentes grupos de pacientes y tipos de tratamiento, así como estudiar las relaciones entre el BMQ y la adherencia al tratamiento., The Beliefs about Medicines Questionnaire was designed to assess the cognitive representation of medication. The BMQ consists of two sections: the BMQGeneral and the BMQ-Specific. The aim of this instrumental study was to assess the factor structure and reliability of the Spanish version of the BMQ. In order to analyse the properties of the BMQ-Specific, a sample of 156 patients, 97 diabetics with insulin treatment and 59 patients with hypertensive treatment were used. The BMQ-General was studied with the chronic patients and with 256 undergraduates. As in the original version, for each section in the BMQ a bifactorial pattern was found. Its validity to discriminate between patients with different types of treatment was proved. Patients with insulin treatment had significantly higher scores on Specific-Necessity Scale than hypertensive patients. The internal consistency of the BMQ Scales was adequate in the three samples. The Spanish version of the BMQ is a valid and reliable instrument for assessing beliefs about medicines but additional research is needed not only with other groups of patients and types of treatment but also to study the relationships between the BMQ and the treatment adherence.

The annealed Constantan foil materials currently used in bonded fatigue sensing devices have fatigue life and sensitivity limitations which place restraints upon practical application to long life service aircraft. These restraints have required the development of more compatible materials to meet present and future applications to fleet aircraft within the Air Force inventory. The effort was eventually directed toward the development of a vacuum deposited sensor. A selected composition of copper-nickel-zinc material was vacuum deposited in a long-range order arrangement so that when disordered by cyclic deformation a relatively large, permanent electrical resistance change was produced. The sensor performance obtained indicates a potential for diversified application to fleet aircraft.

The report describes a feasibility study for the development of a fatigue damage indicator. Various fatigue sensing elements were evaluated and tested under simulated aerodynamic and environmental conditions. A fatigue sensor was optimized and integrated with an electronic system to function as an aid to fatigue damage assessment of an aircraft structure. After determining the behavior characteristics of the optimized sensor on laboratory coupons, practical applications were made on C-130E wing panels. The data indicated that a structures degree of exposure to repeated load occurrences could be monitored and the accumulated strain necessary to produce a fatigue failure could be forecast. The electronic phase was designed as the final element of an electro-structural inspection system for determining the fatigue status of an airframe. The electronic hardware consists of a module for an automatic quality control checkout of all detecting elements and module for indicating the sensors change in resistance. From the results obtained, it can be concluded that the system shows excellent promise as a method for determining the degree of exposure of an aircraft to repeated load occurrences. For 'long life' transport aircraft, the system offers a much better means for determining the fatigue status of an airframe than simply stating the number of accumulated flight hours.

The development testing of the C-5A full-scale fatigue specimens presented a timely opportunity to continue the development of a system of fatigue sensing devices. The fatigue sensor selected for use was previously evaluated during an Air Force funded feasibility study and has been adapted to meet the application requirements on full-scale C-5A structure. The report constitutes the final report regarding this effort and applies specifically to an evaluation of the sensor on the C-5A laboratory fatigue test specimens. The techniques of sensor installation, location, data acquisition methods, and analysis of sensor response are herein evaluated in view of potential utilization on fleet aircraft.

The quality of assessment of non-adherence to treatment in hypertensive is poor. Within this review, we discuss the different methods used to assess adherence to blood-pressure-lowering medications in hypertension patients. Subjective reports such as physicians’ perceptions are inaccurate, and questionnaires completed by patients tend to overreport adherence and show a low diagnostic specificity. Indirect objective methods such as pharmacy database records can be useful, but they are limited by the robustness of the recorded data. Electronic medication monitoring devices are accurate but usually track adherence to only a single medication and can be expensive. Overall, the fundamental issue with indirect objective measures is that they do not fully confirm ingestion of antihypertensive medications. Detection of antihypertensive medications in body fluids using liquid chromatography–tandem mass spectrometry is currently, in our view, the most robust and clinically useful method to assess non-adherence to blood-pressure-lowering treatment. It is particularly helpful in patients presenting with resistant, refractory or uncontrolled hypertension despite the optimal therapy. We recommend using this diagnostic strategy to detect non-adherence alongside a no-blame approach tailoring support to address the perceptions (e.g. beliefs about the illness and treatment) and practicalities (e.g. capability and resources) influencing motivation and ability to adhere.

Background: Antibiotics are extensively prescribed in intensive care units (ICUs), yet little is known about how antibiotic-related decisions are made in this setting. We explored how beliefs, perceptions and contextual factors influenced ICU clinicians' antibiotic prescribing. Methods: We conducted 4 focus groups and 34 semistructured interviews with clinicians involved in antibiotic prescribing in four English ICUs. Focus groups explored factors influencing prescribing, whereas interviews examined decision-making processes using two clinical vignettes. Data were analysed using thematic analysis, applying the Necessity Concerns Framework. Results: Clinicians' antibiotic decisions were influenced by their judgement of the necessity for prescribing/not prescribing, relative to their concerns about potential adverse consequences. Antibiotic necessity perceptions were strongly influenced by beliefs that antibiotics would protect patients from deterioration and themselves from the ethical and legal consequences of undertreatment. Clinicians also reported concerns about prescribing antibiotics. These generally centred on antimicrobial resistance; however, protecting the individual patient was prioritised over these societal concerns. Few participants identified antibiotic toxicity concerns as a key influencer. Clinical uncertainty often complicated balancing antibiotic necessity against concerns. Decisions to start or continue antibiotics often represented € erring on the side of caution' as a protective response in uncertainty. This approach was reinforced by previous experiences of negative consequences ( € being burnt') which motivated prescribing € just in case' of an infection. Prescribing decisions were also context-dependent, exemplified by a lower perceived threshold to prescribe antibiotics out-of-hours, input from external team members and local prescribing norms. Background: The benefits of medication optimization are largely uncontroversial but difficult to achieve. Behavior change interventions aiming to optimize prescriber medication-related decisions, which do not forbid any option and that do not significantly change financial incentives, offer a promising way forward. These interventions are often referred to as nudges. Objective: The current systematic literature review characterizes published studies describing nudge interventions to optimize medication prescribing by the behavioral determinants they intend to influence and the techniques they apply. Methods: Four databases were searched (MEDLINE, Embase, PsychINFO, and CINAHL) to identify studies with nudge-type interventions aiming to optimize prescribing decisions. To describe the behavioral determinants that interventionists aimed to influence, data were extracted according to the Theoretical Domains Framework (TDF). To describe intervention techniques applied, data were extracted according to the Behavior Change Techniques (BCT) Taxonomy version 1 and MINDSPACE. Next, the recommended TDF-BCT mappings were used to appraise whether each intervention applied a sufficient array of techniques to influence all identified behavioral determinants. Results: The current review located 15 studies comprised of 20 interventions. Of the 20 interventions, 16 interventions (80%) were effective. The behavior change techniques most often applied involved prompts (n = 13). The MINDSPACE contextual influencer most often applied involved defaults (n = 10). According to the recommended TDF-BCT mappings, only two interventions applied a sufficient array of behavior change techniques to address the behavioral determinants the interventionists aimed to influence. Background Rapid molecular diagnostic tests to investigate the microbial aetiology of pneumonias may improve treatment and antimicrobial stewardship in intensive care units (ICUs). Clinicians’ endorsement and uptake of these tests is crucial to maximise engagement; however, adoption may be impeded if users harbour unaddressed concerns or if device usage is incompatible with local practice. Accordingly, we strove to identify ICU clinicians’ beliefs about molecular diagnostic tests for pneumonias before implementation at the point-of-care. Methods We conducted semi-structured interviews with 35 critical care doctors working in four ICUs in the United Kingdom. A clinical vignette depicting a fictitious patient with signs of pneumonia was used to explore clinicians’ beliefs about the importance of molecular diagnostics and their concerns. Data were analysed thematically. Results Clinicians’ beliefs about molecular tests could be grouped into two categories: perceived potential of molecular diagnostics to improve antibiotic prescribing (Molecular Diagnostic Necessity) and concerns about how the test results could be implemented into practice (Molecular Diagnostic Concerns). Molecular Diagnostic Necessity stemmed from beliefs that positive results would facilitate targeted antimicrobial therapy; that negative results would signal the absence of a pathogen, and consequently that having the molecular diagnostic results would bolster clinicians’ prescribing confidence. Molecular Diagnostic Concerns included unfamiliarity with the device’s capabilities, worry that it would detect non-pathogenic bacteria, uncertainty whether it would fail to detect pathogens, and discomfort with withholding antibiotics until receiving molecular test results. Conclusions Clinicians believed rapid molecular diagnostics for pneumonias were potentially important and were open to using them; however, they harboured concerns about the tests’ capabilities and integration into clinical practice. Within this review, we discuss the different methods used to assess adherence to blood-pressure-lowering medications in hypertension patients. Subjective reports such as physicians’ perceptions are inaccurate, and questionnaires completed by patients tend to overreport adherence and show a low diagnostic specificity. Indirect objective methods such as pharmacy database records can be useful, but they are limited by the robustness of the recorded data. Electronic medication monitoring devices are accurate but usually track adherence to only a single medication and can be expensive. Overall, the fundamental issue with indirect objective measures is that they do not fully confirm ingestion of antihypertensive medications. Detection of antihypertensive medications in body fluids using liquid chromatography–tandem mass spectrometry is currently, in our view, the most robust and clinically useful method to assess non-adherence to blood-pressure-lowering treatment. It is particularly helpful in patients presenting with resistant, refractory or uncontrolled hypertension despite the optimal therapy. We recommend using this diagnostic strategy to detect non-adherence alongside a no-blame approach tailoring support to address the perceptions (e.g. beliefs about the illness and treatment) and practicalities (e.g. capability and resources) influencing motivation and ability to adhere.

Conventional methodologies rely on power intensive Bluetooth or Wi-Fi technologies for live communications, but this paper outlines the use of ultra-low power battery assistive passive modes on UHF RFID chips to harness sub-2mW continuous streaming of Accelerometer data.
