© The Author(s) 2019. Introduction. Health utilities are widely used in health care. The distributions of utilities differ between countries; some countries more often report worse than dead health states, while mild states are valued more or less the same. We hypothesize that cultural values explain these country-related utility differences. Research Question. What is the effect of sociodemographic background, methodological factors, and cultural values on differences in health utilities? Methods and Analyses. Time tradeoff data from 28 EQ-5D valuation studies were analyzed, together with their sociodemographic variables. The dependent variable was (Formula presented.), the utility difference between mild and severe states. Country-specific cultural variables were taken from the World Values Survey. Multilevel models were used to analyze the effect of sociodemographic background, methodology (3L v. 5L), and cultural values on (Formula presented.). Intraclass correlation (ICC) for country variation was used to assess the impact of the predicting variables on the variation between countries. Results. Substantial variation in (Formula presented.) was found between countries. Adding cultural values did not reduce ICCs for country variation. Sociodemographic background variables were only weakly associated with (Formula presented.) and did not affect the ICC. (Formula presented.) was 0.118 smaller for EQ-5D-5L studies. Discussion. (Formula presented.) varies between countries. These differences were not explained by national cultural values. In conclusion, despite correction for various variables, utility differences between countries remain substantial and unexplained. This justifies the use of country-specific value sets for instruments such as the EQ-5D.

3. Ubiquitin-specific protease 5 is required for the efficient repair of DNA double-strand breaks

Abstract

During the DNA damage response (DDR), ubiquitination plays an important role in the recruitment and regulation of repair proteins. However, little is known about elimination of the ubiquitination signal after repair is completed. Here we show that the ubiquitin-specific protease 5 (USP5), a deubiquitinating enzyme, is involved in the elimination of the ubiquitin signal from damaged sites and is required for efficient DNA double-strand break (DSB) repair. Depletion of USP5 sensitizes cells to DNA damaging agents, produces DSBs, causes delayed disappearance of γH2AX foci after Bleocin treatment, and influences DSB repair efficiency in the homologous recombination pathway but not in the non-homologous end joining pathway. USP5 co-localizes to DSBs induced by laser micro-irradiation in a RAD18-dependent manner. Importantly, polyubiquitin chains at sites of DNA damage remained for longer periods in USP5-depleted cells. Our results show that disassembly of polyubiquitin chains by USP5 at sites of damage is important for efficient DSB repair. © 2014 Nakajima et al.

Abstract

Strong shaping, favourable for stability and improved energy confinement, together with a significant expansion of the central region of improved confinement in negative central magnetic shear target plasmas, increased the maximum fusion power produced in DIII-D by a factor of 3. Using deuterium plasmas, the highest fusion power gain, the ratio of fusion power to input power, Q, was 0.0015, corresponding to an equivalent Q of 0.32 in a deuterium-tritium plasma, which is similar to values achieved in tokamaks of larger size and magnetic field. A simple transformation relating Q to the stability parameters is presented.

Abstract

Recent results from the DIII-D tokamak have provided significant contributions to the understanding of many of the elements of tokamak physics and the application of this understanding to the design of next generation devices including ITER and CIT. The limitations of magnetohydrodynamic stability on the values of plasma beta (the ratio of kinetic pressure to the containing pressure of the magnetic field) that can be attained has been experimentally demonstrated and found to be described by existing theory. Values of beta (10.7%) well in excess of those required for proposed devices (ITER and CIT) have been demonstrated. Regimes of confinement (H-mode) have been established that scale favorably to proposed next generation devices, and experiments demonstrating the dependence of the energy confinement on plasma size have been completed. Understanding of confinement is rapidly developing especially in the areas of bulk transport and the role of turbulence in the plasma edge. Key experimental results in areas of plasma transport and edge plasma phenomena are in agreement with theories based on short wavelength turbulence. Control of the divertor heat loads and impurity influx has been demonstrated, and new progress has been made in the understanding of plasma edge phenomena. Experiments with ion Bernstein wave heating have not found regimes in which these waves can produce effective central ion heating. Electron cyclotron current drive experiments have demonstrated 70 kA of driven current in 400 kA discharges.

Values of beta (10.7%) well in excess of those required for proposed devices (ITER and CIT) have been demonstrated. Regimes of confinement (H-mode) have been established that scale favorably to proposed next generation devices, and experiments demonstrating the dependence of the energy confinement on plasma size have been completed. Understanding of confinement is rapidly developing especially in the areas of bulk transport and the role of turbulence in the plasma edge. Key experimental results in areas of plasma transport and edge plasma phenomena are in agreement with theories based on short wavelength turbulence. Control of the divertor heat loads and impurity influx has been demonstrated, and new progress has been made in the understanding of plasma edge phenomena. Experiments with ion Bernstein wave heating have not found regimes in which these waves can produce effective central ion heating. Electron cyclotron current drive experiments have demonstrated 70 kA of driven current in 400 kA discharges.

Abstract

The authors' data indicate that the L-mode to H-mode transition in the DIII-D tokamak is associated with the sudden reduction in anomalous, fluctuation-connected transport across the outer midplane of the plasma. In addition to the reduction in edge density and magnetic fluctuations observed at the transition, the edge radial electric field becomes more negative after the transition. They have determined the scaling of the H-mode power threshold with various plasma parameters; the roughly linear increase with plasma density and toroidal field are particularly significant. Control of the ELM frequency and duration by adjusting neutral beam input power has allowed us to produce H-mode plasmas with constant impurity levels and durations up to 5 s. Energy confinement time in ohmic H-mode plasmas and in deuterium H-mode plasmas with deuterium beam injection can exceed saturated ohmic confinement times by at least a factor of two. Energy confinement times above 0.3 s have been achieved in these beam-heated plasmas with plasma currents in the range of 2.0 to 2.5 MA. Local transport studies have shown that electron and ion thermal diffusivities and angular momentum diffusivity are comparable in magnitude and all decrease with increasing plasma current.
