With the exponential growth of virtualization and cloud computing over the last decade, many companies in the telecommunications sector have started their journey towards cloud migration by exchanging a lot of specialized hardware for virtualized solutions. With more and more applications running in a cloud environment, it became essential to run these applications on heterogeneous systems with shared underlying hardware and software resources. However, running these applications in a heterogeneous cloud environment often leads to unpredictable and non-deterministic performance, as all the applications compete for the shared resources to improve their individual performance. This becomes a problem when the interference from the co-hosted applications starts affecting the performance of the critical applications running on the same server. Ericsson is therefore investigating a solution to dynamically manage the low-level hardware and software resources to get deterministic performance on applications deployed using Kubernetes. In this thesis, the Intent Driven Orchestration (IDO) model developed by Intel is used as the baseline model. This model was then extended by adding another tool to the setup called Container Runtime Interface-Resource Manager (CRI-RM), which is used to manipulate low-level software and hardware resources managed by a Kubernetes cluster at runtime. The results achieved in this thesis suggest that it is possible to get deterministic performance for an application deployed using Kubernetes, by identifying and isolating the CPU cores in the cluster on which the application is running.

Indoleamine 2,3-dioxygenase 2 (IDO2) is a tryptophan-catabolizing enzyme and a homolog of IDO1 with a distinct expression pattern compared with IDO1. In dendritic cells (DCs), IDO activity and the resulting changes in tryptophan level regulate T-cell differentiation and promote immune tolerance. Recent studies indicate that IDO2 exerts an additional, non-enzymatic function and pro-inflammatory activity, which may play an important role in diseases such as autoimmunity and cancer. Here, we investigated the impact of aryl hydrocarbon receptor (AhR) activation by endogenous compounds and environmental pollutants on the expression of IDO2. Treatment with AhR ligands induced IDO2 in MCF-7 wildtype cells but not in CRISPR-cas9 AhR-knockout MCF-7 cells. Promoter analysis with IDO2 reporter constructs revealed that the AhR-dependent induction of IDO2 involves a short-tandem repeat containing four core sequences of a xenobiotic response element (XRE) upstream of the start site of the human ido2 gene. The analysis of breast cancer datasets revealed that IDO2 expression increased in breast cancer compared with normal samples. Our findings suggest that the AhR-mediated expression of IDO2 in breast cancer could contribute to a pro-tumorigenic microenvironment in breast cancer.

The identification and characterization of tumor antigens are central objectives in developing anti-cancer immunotherapy. Traditionally, tumor-associated antigens (TAAs) are considered relatively restricted to tumor cells (i.e., overexpressed proteins in tumor cells), whereas tumor-specific antigens (TSAs) are considered unique to tumor cells. Recent studies have focused on identifying patient-specific neoantigens, which might be highly immunogenic because they are not expressed in normal tissues. The opposite strategy has emerged with the discovery of anti-regulatory T cells (anti-Tregs) that recognize and attack many cell types in the tumor microenvironment, such as regulatory immune cells, in addition to tumor cells. The term proposed in this review is “tumor microenvironment antigens” (TMAs) to describe the antigens that draw this attack. As therapeutic targets, TMAs offer several advantages that differentiate them from more traditional tumor antigens. Targeting TMAs leads not only to a direct attack on tumor cells but also to modulation of the tumor microenvironment, rendering it immunocompetent and tumor-hostile. Of note, in contrast to TAAs and TSAs, TMAs also are expressed in non-transformed cells with consistent human leukocyte antigen (HLA) expression. Inflammation often induces HLA expression in malignant cells, so that targeting TMAs could additionally affect tumors with no or very low levels of surface HLA expression. This review defines the characteristics, differences, and advantages of TMAs compared with traditional tumor antigens and discusses the use of these antigens in immune modulatory vaccines as an attractive approach to immunotherapy. Different TMAs are expressed by different cells and could be combined in anti-cancer immunotherapies to attack tumor cells directly and modulate local immune cells to create a tumor-hostile microenvironment and inhibit tumor angiogenesis. Dysregulation of tryptophan metabolism has been linked to colorectal tumorigenesis; however, epidemiological studies investigating tryptophan metabolites in relation to colorectal cancer risk are limited. We studied associations of plasma tryptophan, serotonin and kynurenine with colon cancer risk in two studies with cancer patients and controls, and in one prospective cohort: ColoCare Study (110 patients/153 controls), the Colorectal Cancer Study of Austria (CORSA; 46 patients/390 controls) and the European Prospective Investigation into Cancer and Nutrition (EPIC; 456 matched case-control pairs). Logistic regression was used to estimate odds ratios (ORs) and 95% confidence intervals (CIs) for colon cancer risk. Tryptophan was inversely associated with colon cancer risk in ColoCare (OR per 1-SD = 0.44; 95% CI, 0.31-0.64) and EPIC (OR per 1-SD = 0.86; 95% CI, 0.74-0.99). Comparing detectable vs nondetectable levels, serotonin was positively associated with colon cancer in CORSA (OR = 6.39; 95% CI, 3.61-11.3) and EPIC (OR = 2.03; 95% CI, 1.20-3.40). Kynurenine was inversely associated with colon cancer in ColoCare (OR per 1-SD = 0.74; 95% CI, 0.55-0.98), positively associated in CORSA (OR per 1-SD = 1.79; 95% CI, 1.27-2.52), while no association was observed in EPIC. The kynurenine-to-tryptophan ratio was positively associated with colon cancer in ColoCare (OR per 1-SD = 1.38; 95% CI, 1.03-1.84) and CORSA (OR per 1-SD = 1.44; 95% CI, 1.06-1.96), but not in EPIC. These results suggest that higher plasma tryptophan may be associated with lower colon cancer risk, while increased serotonin may be associated with a higher risk of colon cancer. The kynurenine-to-tryptophan ratio may also reflect altered tryptophan catabolism during colon cancer development.

Tryptophan 2, 3-dioxygenase (TDO2) is a heme-containing enzyme constitutively expressed at high concentrations in the liver and responsible for L-tryptophan (L-Trp) homeostasis. TDO2 catalyses the cleavage of L-Trp’s pyrrole ring to produce N’-formylkynurenine; the first step of the kynurenine pathway. The expression of TDO2 in cancer cells leads to the depletion of L-Trp and an increase of KP catabolites, overall resulting in the suppression of the anti-tumour immune response. The present work describes the development of new (1H-indol-3-yl)-heterocyclic derivatives as TDO2 inhibitors through rational design, inspired from existing inhibitors. Evaluation of these compounds provided new insights regarding the structural modulations that can be accommodated inside the active site of TDO2 and their effect on inhibitory potency. Amino acids blood levels are remarkably constant, in spite of fasting periods and large variations in diet supply. Systemic homeostasis is particularly important for tryptophan, given its key role in many cellular processes, including immunity and neuronal activity. In humans, tryptophanemia is maintained around 60µM. Here, we showed that tryptophan homeostasis is largely controlled by the post-translational stability of tryptophan 2,3-dioxygenase (TDO), the hepatic enzyme catalyzing the first step of tryptophan catabolism along the kynurenine pathway. TDO, located at the strategic crossroad of the portal and systemic circulation, degrades more than 90% of food-derived tryptophan. High tryptophan levels stabilize the active tetrameric conformation of TDO through binding non-catalytic exosites, promoting rapid blood tryptophan catabolism. In low tryptophan, the lack of tryptophan binding in the exosites destabilizes the tetramer into inactive monomers and dimers, and unmasks a degron. This triggers the polyubiquitination of TDO by SKP1-CUL1-F-box complexes, resulting in proteasome-mediated degradation of TDO and rapid interruption of tryptophan catabolism. Artificial stabilization of tetrameric TDO with the non-metabolizable analog alpha-methyl-tryptophan in vivo stably reduces blood tryptophan levels. Our results uncover a mechanism allowing a rapid adaptation of tryptophan catabolism to ensure quick degradation of excess tryptophan while preventing further catabolism below physiological levels. This ensures a tight control of tryptophanemia, as required for both neurological and immune homeostasis., La concentration plasmatique des acides aminés est remarquablement constante en dépit des variations de nos apports alimentaires. L’homéostasie systémique des acides aminés est particulièrement importante pour le tryptophane, en raison de son rôle clef dans de nombreux processus biologiques, incluant l’immunité et l’activité neuronale. Chez l’homme, la tryptophanémie est en permanence maintenue à 60µM. Nos découvertes révèlent que l’homéostasie du tryptophane est majoritairement contrôlée par la stabilité post-traductionnelle de la tryptophane 2,3-dioxygenase (TDO), l’enzyme hépatique catalysant la première réaction du catabolisme du tryptophane dans la voie métabolique de la kynurénine. Localisée au carrefour de l’artère hépatique et de la veine porte, la protéine TDO occupe une position stratégique pour filtrer le sang et dégrade plus de 90% du tryptophane d’origine alimentaire. Lorsque la tryptophanémie augmente, le tryptophane active la protéine TDO en stabilisant sa conformation tétramérique au moyen de quatre exosites non catalytiques, promouvant ainsi le catabolisme du tryptophane plasmatique. Une fois la tryptophanémie faible, le tryptophane libère les exosites et provoque la déstabilisation du tétramère en dimères et en monomères inactifs, libérant ainsi un motif de dégradation ou dégron reconnu par des ubiquitines ligases SKP1-CUL1-F-box. Elles déclenchent alors la polyubiquitination de TDO, laquelle conduit à la dégradation de l’enzyme par le protéasome et à l’arrêt du catabolisme du tryptophane. In vivo, la stabilisation de la protéine TDO hépatique avec un analogue non métabolisable, l’alpha-méthyl-tryptophane, réduit de manière permanente la concentration plasmatique du tryptophane. Background The metabolism of tryptophan (Trp) along the kynurenine pathway has been shown to carry strong immunoregulatory properties. Several experimental studies indicate that this pathway is a major regulator of vascular inflammation and influences atherogenesis. Knowledge of the role of this pathway in human atherosclerosis remains incomplete. Objectives In this study, we performed a multiplatform analysis of tissue samples,in vitroandin vivofunctional assays to elucidate the potential role of the kynurenine pathway in human atherosclerosis. Methods and results Comparison of transcriptomic data from carotid plaques and control arteries revealed an upregulation of enzymes within the quinolinic branch of the kynurenine pathway in the disease state, whilst the branch leading to the formation of kynurenic acid (KynA) was downregulated. Further analyses indicated that local inflammatory responses are closely tied to the deviation of the kynurenine pathway in the vascular wall. Analysis of cerebrovascular symptomatic and asymptomatic carotid stenosis data showed that the downregulation of KynA branch enzymes and reduced KynA production were associated with an increased probability of patients to undergo surgery due to an unstable disease.In vitro, we showed that KynA-mediated signalling through aryl hydrocarbon receptor (AhR) is a major regulator of human macrophage activation. Using a mouse model of peritoneal inflammation, we showed that KynA inhibits leukocyte recruitment. Conclusions We have found that a deviation in the kynurenine pathway is associated with an increased probability of developing symptomatic unstable atherosclerotic disease. Our study suggests that KynA-mediated signalling through AhR is an important mechanism involved in the regulation of vascular inflammation.

BACKGROUND: Interferon-gamma (IFN-γ) represents a potent inducer for keratinocyte inflammatory and immune activation in vitro. Since tryptophan (trp) conversion to kynurenine (kyn) is involved in inflammation, the topical kyn/trp ratio may serve as a biomarker of skin inflammation. However, the trp metabolism in keratinocytes exposed to IFN-γ is not yet fully understood. OBJECTIVE: The aim of this study was to establish a human epidermis model in order to quantify cytokine and kyn/trp secretion from IFN-γ stimulated cells and tissues. Moreover, to compare the cell response of 2D-cultured keratinocytes and the 3D epidermis model. METHODS: Polycarbonate filters were used on which primary keratinocytes could attach and stratify in order to form the typical layers of reconstructed human epidermis (RHE). After IFN-γ treatment, secretion of kyn/trp was measured by high performance liquid chromatography. Gene and protein expression of indoleamine 2,3-dioxygenase 1 (IDO) was analyzed with real-time PCR and immunohistochemistry. The secretion of cytokines was quantified with ELISA. RESULTS: Trp catabolism to kyn was significantly increased (P < 0.01) in the 2D culture in response to IFN-γ treatment. Before kyn secretion, IDO was strongly upregulated (P < 0.001). IFN-γ treatment also increased the secretion of IL-6 and IL-8 from the keratinocytes. In the RHE, IDO was upregulated by IFN-γ, and kyn secretion could be detected. Interestingly, IDO expression was only present in the basal cells of the RHE. CONCLUSION: Our results suggest that IFN-γ acts as an inducer of trp degradation preferentially in undifferentiated keratinocytes, indicated by the IDO expression in the basal layer of the RHE.

The obligate intracellular protozoan parasite Toxoplasma gondii can infect any nucleated cell from a warm-blooded host. However, its interaction with host macrophages plays a critical role in shaping the immune response during infection. Therefore, assessing Toxoplasma-macrophage interactions at a cellular level is important. In this chapter, we describe assays that can be used to characterize Toxoplasma-macrophage interactions. These assays can also be used to evaluate other host-pathogen interactions. We describe multiplex approaches for measuring arginase activity, indoleamine 2,3 dioxygenase activity, cell death, and parasite growth during Toxoplasma-macrophage interactions. These assays can be used to compare how different Toxoplasma strains differ in their interaction with macrophages, and we describe how to properly assess Toxoplasma strain differences in Toxoplasma-macrophage interactions.

Immune checkpoint inhibitors have improved the prognosis of advanced melanoma. Although anti–programmed death ligand‐1 (PD‐L1) is a well‐studied biomarker for response to anti–programmed death‐1 PD‐1 therapy in melanoma, its clinical relevance remains unclear. It has been established that the high expression of indoleamine 2, 3‐dioxygenase (IDO) is correlated to a response to anti–CTLA‐4 treatment in melanoma. However, it is still unknown whether the IDO expression is associated with response to anti–PD‐1 therapy in advanced melanoma. In addition, acral and mucosal melanomas, which comprise a great proportion of all melanomas in Asians, are genetically different subtypes from cutaneous melanomas; however, they have not been independently analyzed due to their low frequency in Western countries. To evaluate the association of IDO and PD‐L1 expression with response to anti–PD‐1 antibody in acral and mucosal melanoma patients, we analyzed 32 Japanese patients with acral and mucosal melanomas treated with anti–PD‐1 antibody from the perspective of IDO and PD‐L1 expression levels by immunohistochemistry (IHC). Multivariate Cox regression models showed that the low expression of IDO in tumors was associated with poor progression‐free survival (HR = 0.33, 95% CI = 0.13‐0.81, P = 0.016), whereas PD‐L1 expression on tumors was not associated with progression‐free survival. Significantly lower expression of IDO in tumors was found in non–responders compared to responders. Assessment of the IDO expression could be useful for the identification of suitable candidates for anti–PD‐1 therapy among acral and mucosal melanomas patients. Further validation study is needed to estimate the clinical utility of our findings.

Titanium is the mean biocompatible metal found in dental titanium alloys (Ti-6Al-4V). The safety of certain dental biomaterial amalgams has been questioned in patients. The levels of several systemic cytokines (interleukin (IL)-1 beta, IL-4: pg/mL) and chemokines (monocyte chemoattractant protein-1 (MCP-1), soluble fractalkine (CX3CL1: pg/mL) were determined using ELISA and compared between these study groups. The study included 30 controls without dental materials (cont), 57 patients with long-term titanium dental implants plus amalgams (A + I group) as well as 55 patients with long-term dental amalgam alone (A group). All patients (except controls) have had dental titanium implants (Ti-6Al-4V) and/or amalgams for at least 10 years (average: 15 years). We evaluated whether systemic levels of cytokines/chemokines, kyn/L-trp ratio and aromatic amino acid levels (HPLC: mM/L, Phe, L-Trp, His, Treo) could be altered in patients with long-term dental titanium and/or amalgams. These systemic markers were evaluated in 142 patients. The A + I group had higher L-Kynurenine/L-Tryptophan ratios than patients with long-term dental amalgam fillings alone (A). In addition, levels of IL-1 Beta cytokine, CX3CL1 and MCP-1 chemokines were higher in the A + I group than in the A group (A). One of the immunosuppressive pathways acting in the tumor microenvironment relies on the degradation of tryptophan (Trp) into kynurenine (Kyn), orchestrated by two distinct enzymes: indoleamine 2,3-dioxygenase (IDO1) and tryptophan 2,3 dioxygenase (TDO). Local tryptophan depletion combined with kynurenine production strongly impair cellular immunity and make IDO1 and TDO endowing immunosuppressive properties. Like IDO1, TDO is considered as a relevant drug target to improve the efficacy of cancer immunotherapy. However, its role in various immunotherapy settings has not been fully characterized. Here we report that the small molecule inhibitor of TDO (PF06845102/EOS200809) modulates kynurenine and tryptophan concentration in the plasma, liver and tumor and increases the ability of checkpoint inhibitor anti-CTLA4 to induce rejection of CT26 tumors expressing TDO. To better characterize TDO as a therapeutic target, we also used TDO-KO mice. These mice have drastically increased levels of tryptophan in the blood, because they cannot degrade dietary tryptophan. In the MC38 colon carcinoma mouse model, we found that anti-tumor activity of immune checkpoint inhibitors anti-CTLA4 or anti-PD1 was higher in TDO-KO compared to the wild-type mice. Because these MC38 tumors did not express TDO, we related this improved immune response to the high systemic levels of tryptophan in TDO-KO mice. In agreement with this hypothesis, the effect of anti-PD1 was abolished in TDO-KO mice fed with a tryptophan-low diet that normalized their blood tryptophan level. Moreover, treatment of mice with an IDO1 inhibitor improved the efficacy of anti-PD1 in wild-type but not in TDO-KO mice. Since MC38 tumors express IDO1, its immunosuppressive activity limits the efficacy of anti-PD1 treatment in wild-type mice but appears to be overcome in TDO-KO mice due to the high levels of tryptophan. The T-win® technology is an innovative investigational approach designed to activate the body’s endogenous anti-regulatory T cells (anti-Tregs) to target regulatory as well as malignant cells. Anti-Tregs are naturally occurring T cells that can directly react against regulatory immune cells because they recognize proteins that these targets express, including indoleamine 2,3-dioxygenase (IDO), tryptophan 2,6-dioxygenase, arginase, and programmed death ligand 1 (PD-L1). The T-win® technology is characterized by therapeutic vaccination with long peptide epitopes derived from these antigens and therefore offers a novel way to target genetically stable cells with regular human leukocyte antigen expression in the tumor microenvironment. The T-win® technology thus also represents a novel way to attract pro-inflammatory cells to the tumor microenvironment where they can directly affect immune inhibitory pathways, potentially altering tolerance to tumor antigens. The modification of an immune regulatory environment into a pro-inflammatory milieu potentiates effective anti-tumor T cell responses. Many regulatory immune cells may be reverted into effector cells given the right stimulus. Because T-win® technology is based on the immune-modulatory function of the vaccines, the vaccines activate both CD4 and CD8 anti-Tregs. Of importance, in clinical trials, vaccinations against IDO or PD-L1 to potentiate anti-Tregs have so far proved to be safe, with minimal toxicity.

No standard treatment has been established for metastatic uveal melanoma (mUM). Immunotherapy is commonly used for this disease even though UM has not been included in phase III clinical trials with checkpoint inhibitors. Unfortunately, only a minority of patients obtain a clinical benefit with immunotherapy. The immunological features of mUM were reviewed in order to understand if immunotherapy could still play a role for this disease.

Background: Long-term follow-up on a clinical trial of 15 stage III-IV NSCLC patients treated with an Indoleamine 2, 3-Dioxygenase (IDO) peptide vaccine (NCT01219348). Methods: Fifteen HLA-A2-positive patients with stable stage III-IV NSCLC after standard chemotherapy were treated with subcutaneous vaccinations (100 μg IDO5 peptide, sequence ALLEIASCL, formulated in 900 μl Montanide) biweekly for 2.5 months and thereafter monthly until progression or up to 5 years. Here we report long-term clinical follow-up, toxicity and immunity. Results: Three of 15 patients are still alive corresponding to a 6-year overall survival of 20 %. Two patients continued monthly vaccinations for 5 years (56 vaccines). One of the two patients developed a partial response (PR) of target lesions in the liver 15 months after the first vaccine and has remained in PR ever since. The other patient had a solitary distant metastasis in a lymph node in retroperitoneum at baseline which normalized during treatment. All following evaluation scans during the treatment have been tumor free. The vaccine was well tolerated for all 5 years with no long-term toxicities registered. The third long-term surviving patient discontinued vaccinations after 11 months due to disease progression. Flow cytometry analyses of PBMCs from the two long-term responders demonstrated stable CD8+ and CD4+ T-cell populations during treatment. In addition, presence of IDO-specific T-cells was detected by IFN-γ Elispot in both patients at several time points during treatment. Conclusion: IDO peptide vaccination was well tolerated for administration up to 5years. Two of 15 patients are long-term responders with ongoing clinical response 6 years after 1st vaccination.

Atherosclerosis is a chronic inflammatory disease characterized by a complex interplay between innate and adaptive immunity. Dendritic cells (DCs) play a key role in T-cell activation and regulation by promoting a tolerogenic environment through the expression of the immunosuppressive enzyme indoleamine 2,3-dioxygenase (IDO), an intracellular enzyme involved in tryptophan catabolism. IDO expression and activity was analyzed in monocytes derived DCs (MDDCs) from non-ST segment elevation myocardial infarction (NSTEMI) patients, stable angina (SA) patients and healthy controls (HC) by real-time quantitative polymerase chain reaction (RT-qPCR) before and after in vitro maturation with lipopolysaccharide (LPS). The amount of tryptophan catabolite; kynurenine; was evaluated in the culture supernatants of mature-MDDCs by ELISA assay. Autologous mixed lymphocyte reaction (MLR) between mature-MDDCs and naà ̄ve T-cells was carried out to study the differentiation towards T-helper 1 (Th1) and induced regulatory T-cells (iTreg). Analysis of IDO mRNA transcripts in mature-MDDCs revealed a significant reduction in cells isolated from NSTEMI (625.0 ± 128.2; mean ± SEM) as compared with those from SA (958.5 ± 218.3; p = 0.041) and from HC (1183.6 ± 231.6; p = 0.034). Furthermore; the concentration of kynurenine was lower in NSTEMI patients (2.78 ± 0.2) and SA (2.98 ± 0.25) as compared with HC (5.1 ± 0.69 ng/mL; p = 0.002 and p = 0.016; respectively). When IDO-competent mature-MDDCs were co-cultured with allogeneic naà ̄ve T-cells, the ratio between the percentage of generated Th1 and iTreg was higher in NSTEMI (4.4 ± 2.9) than in SA (1.8 ± 0.6; p = 0.056) and HC (0.9 ± 0.3; p = 0.008). In NSTEMI, the tolerogenic mechanism of the immune response related to IDO production by activated MDDCs is altered, supporting their role in T-cell dysregulation.

Indoleamine 2,3-dioxygenase (IDO) is an important immunoregulatory heme enzyme that controls T-cell responses during diverse disease conditions by catalyzing tryptophan degradation along the kynurenine pathway. IDO is expressed within inflammatory or hypoxic tissues, characterized by oxidative stress or low oxygen tension, respectively. These studies investigated how reduction and oxidation reactions control IDO under oxidative or hypoxic conditions. The oxidant hydrogen peroxide (H2O2) potently inhibited dioxygenase activity by activating IDO’s peroxidase function, leading to compound I-mediated protein oxidation, an opening of the distal active-site pocket and non-productive substrate binding, changes inhibited by tryptophan. Protection by tryptophan involved its compound I-dependent oxidation into oxindolylalanine and kynurenine and binding to IDO compound II, retarding peroxidase turnover. Physiological peroxidase substrates ascorbate and nitrite also inhibited dioxygenase inactivation, despite the latter promoting IDO tyrosine nitration. Pathological oxidants, peroxynitrite and hypochlorite, inhibited IDO dioxygenase activity with reduced efficacy compared to H2O2, reflecting their propensity to directly oxidize IDO protein and inability to form compound I. Stopped flow spectroscopy revealed H2O2 and peroxynitrite react with ferric-IDO with rate-constants of 8.1x10^3 and 5.1x10^4 M^-1 s^-1, respectively. The reaction of peroxidase substrates with IDO compounds I and II revealed unique kinetic profiles for each substrate, with an overall order of reactivity: tyrosine >ascorbate >urate >nitrite. Ascorbate accelerated IDO compound II formation upon reaction of ferric-IDO with H2O2 but not peroxynitrite, indicating compound I formation is specific for H2O2. Under hypoxia, IDO efficiently reduced nitrite to nitric oxide (NO) in a nitrite-, enzyme concentration- and pHdependent manner via its nitrite reductase activity (rate-constant: 5.1 M^-1 s^-1). With the recent advances in immunotherapeutic approaches to cancer there is a pressing need to develop new immune specific biomarkers. In the current thesis we examine the role of several immune inhibitory molecules, tumor associated antigens and tumor infiltrating lymphocytes, as possible biomarkers in cancers of the gastrointestinal tract (hepatocellular carcinoma, pancreatic cancer, ampullary cancer and colon cancer). These molecules, and cell types, are frequently present in the tumor microenvironment or in the circulation of cancer patients. We show that panels of immune specific biomarkers can have powerful prognostic potential in several cancers types.

Novel systemic treatments for hepatocellular carcinoma (HCC) are strongly needed. Immunotherapy is a promising strategy that can induce specific antitumor immune responses. Understanding the mechanisms of immune resistance by HCC is crucial for development of suitable immunotherapeutics. We used immunohistochemistry on tissue-microarrays to examine the co-expression of the immune inhibiting molecules PD-L1, Galectin-9, HVEM and IDO, as well as tumor CD8+ lymphocyte infiltration in HCC, in two independent cohorts of patients. We found that at least some expression in tumor cells was seen in 97% of cases for HVEM, 83% for PD-L1, 79% for Gal-9 and 66% for IDO. In the discovery cohort (n = 94), we found that lack of, or low, tumor expression of PD-L1 (p < 0.001), Galectin-9 (p < 0.001) and HVEM (p < 0.001), and low CD8+TIL count (p = 0.016), were associated with poor HCC-specific survival. PD-L1, Galectin-9 and CD8+TIL count were predictive of HCC-specific survival independent of baseline clinicopathologic characteristics and the combination of these markers was a powerful predictor of HCC-specific survival (HR 0.29; p <0.001). These results were confirmed in the validation cohort (n = 60). We show that low expression levels of PD-L1 and Gal-9 in combination with low CD8+TIL count predict extremely poor HCC-specific survival and it requires a change in two of these parameters to significantly improve prognosis. In conclusion, intra-tumoral expression of these immune inhibiting molecules was observed in the majority of HCC patients. Low expression of PD-L1 and Galectin-9 and low CD8+TIL count are associated with poor HCC-specific survival. Combining immune biomarkers leads to superior predictors of HCC mortality.

Our initial understanding of immune-regulatory cells was based on the discovery of suppressor cells that assure peripheral T-cell tolerance and promote immune homeostasis. Research has particularly focused on the importance of regulatory T cells (Tregs) for immune modulation, e.g. directing host responses to tumours or inhibiting autoimmunity development. However, recent studies report the discovery of self-reactive pro-inflammatory T cells—termed anti-regulatory T cells (anti-Tregs)—that target immune-suppressive cells. Thus, regulatory cells can now be defined as both cells that suppress immune reactions as well as effector cells that counteract the effects of suppressor cells and support immune reactions. Self-reactive anti-Tregs have been described that specifically recognize human leukocyte antigen-restricted epitopes derived from proteins that are normally expressed by regulatory immune cells, including indoleamine 2,3-dioxygenase (IDO), tryptophan 2,6-dioxygenase (TDO), programmed death-ligand 1 (PD-L1), and forkhead box P3 (Foxp3). These proteins are highly expressed in professional antigen-presenting cells under various physiological conditions, such as inflammation and stress. Therefore, self-reactive T cells that recognize such targets may be activated due to the strong activation signal given by their cognate targets. The current review describes the existing knowledge regarding these self-reactive anti-Tregs, providing examples of antigen-specific anti-Tregs and discussing their possible roles in immune homeostasis and their potential future clinical applications.

The discovery that immunotherapy is a clinically-relevant approach for the treatment of malignant tumors is revolutionizing patient care. In adults diagnosed with glioblastoma (GBM), an aggressive and incurable primary brain tumor, autologous HSPPC-96 vaccination provides a significant increase in overall survival. However, all GBM patients eventually succumb to their disease, providing rationale for discovering new methods that proactively identify individuals that will respond, optimally. Of the immunosuppressive mediators that contribute to the inhibition of productive tumor immunity, indoleamine 2,3 dioxygenase 1 (IDO1), a rate-limiting enzyme that catabolizes tryptophan (Trp) into kynurenine (Kyn), has been demonstrated to be expressed at elevated levels in patients with malignant glioma. Recently, our group determined that a correlation exists between peripheral blood Trp and Kyn levels in GBM patients and the association with overall survival after HSPPC-96 treatment. Our findings indicate that the Kyn/Trp ratio may be a useful benchmark for identifying GBM patients with a higher likelihood to survive longer after vaccination. The relevance to future clinical trials, the limitations of brain tumor models to address these findings and the role of IDO1 versus tryptophan dioxygenase (TDO) in the maintenance of peripheral Trp and Kyn levels, is discussed.

In a recent issue of Immunity, Mark Davis and colleagues describe that thymic selection does not eliminate but prunes self-reactive T cell clones. Self-reactive T cells are a natural part of the T-cell repertoire and may be important in the fight against pathogens in addition to being important immune regulators.

The mammalian gut bacterial microbiome is increasingly recognized as a vital immune organ. Numerous studies in murine models implicate the gut bacterial microbiome not only as being required for the induction and maintenance of various important cellular mediators of immunity, but also as being capable of initiating and/or exacerbating states of chronic inflammation in the gut and the periphery. HIV infection and subsequent disease progression is driven by chronic inflammation, and is concomitantly characterized by a disruption within many mucosal cellular compartments involved in regulating gut microbial community composition, including Paneth cells, macrophages, dendritic cells, and Th17 cells. Previous work from our laboratory has shown a link between Th17 cell depletion and activity of the immunomodulatory enzyme indoleamine 2,3-dioxygenase (IDO1), which produces molecules known as kynurenines that can directly inhibit differentiation of Th17 cells in vitro and in vivo. However, a putative interaction between the gut microbiome and the constellation of immune disruptions in HIV, including chronic inflammation and mucosal immune disruption (perhaps involving the IDO1 pathway), had not yet been described. We hypothesized that HIV infection is associated with a shift in gut microbiome composition, and that this shift could potentiate further mucosal immune disruptions and the chronic inflammation that drives HIV disease progression. We profiled the gut mucosal-adherent microbiome of 32 human subjects at varying stages of HIV disease. We found that the microbiomes of untreated, infected individuals was significantly different from those that were uninfected, and the extent to which this microbiome differed was proportionally related to markers of chronic inflammation and mucosal immune disruption (IDO1 activity). Chorioamnionitis, an infection/inflammation of the fetomaternal membranes, is frequently associated with preterm delivery. The mechanisms of inflammation in chorioamnionitis are poorly understood. We hypothesized that neutrophils recruited to the decidua would be the major producers of proinflammatory cytokines. We injected intra-amniotic (IA) interleukin 1beta (IL-1beta) at ∼80% gestation in rhesus macaque monkeys, Macaca mulatta, delivered the fetuses surgically 24 h or 72 h after IA injections, and investigated the role of immune cells in the chorion-amnion decidua. IA IL-1beta induced a robust infiltration of neutrophils and significant increases of proinflammatory cytokines in the chorioamnion decidua at 24 h after exposure, with a subsequent decrease at 72 h. Neutrophils in the decidua were the major source of tumor necrosis factor alpha (TNFalpha) and IL-8. Interestingly, IA IL-1beta also induced a significant increase in anti-inflammatory indoleamine 2,3-dioxygenase (IDO) expression in the decidua neutrophils. The frequency of regulatory T cells (Tregs) and FOXP3 mRNA expression in the decidua did not change after IA IL-1beta injection. Collectively, our data demonstrate that in this model of sterile chorioamnionitis, the decidua neutrophils cause the inflammation in the gestational tissues but may also act as regulators to dampen the inflammation. These results help to understand the contribution of neutrophils to the pathogenesis of chorioamnionitis-induced preterm labor.

Toxoplasma gondii, an intracellular protozoan parasite, is a major cause of opportunistic infectious disease affecting the brain and has been linked to an increased incidence of schizophrenia. In murine hosts, infection with T. gondii stimulates tryptophan degradation along the kynurenine pathway (KP), which contains several neuroactive metabolites, including 3-hydroxykynurenine (3-HK), quinolinic acid (QUIN) and kynurenic acid (KYNA). As these endogenous compounds may provide a mechanistic connection between T. gondii and the pathophysiology of schizophrenia, we measured KP metabolites in both the brain and periphery of T. gondii-treated C57BL/6 mice 8 and 28 days post-infection. Infected mice showed early decreases in the levels of tryptophan in the brain and serum, but not in the liver. These reductions were associated with elevated levels of kynurenine, KYNA, 3-HK and QUIN in the brain. In quantitative terms, the most significant increases in these KP metabolites were observed in the brain at 28 days post-infection. Notably, the anti-parasitic drugs pyrimethamine and sulfadiazine, a standard treatment of toxoplasmosis, significantly reduced 3-HK and KYNA levels in the brain of infected mice when applied between 28 and 56 days post-infection. In summary, T. gondii infection, probably by activating microglia and astrocytes, enhances the production of KP metabolites in the brain. However, during the first two months after infection, the KP changes in these mice do not reliably duplicate abnormalities seen in the brain of individuals with schizophrenia.

In Europa stellt die Multiple Sklerose (MS) die häufigste neuroimmunologische Erkrankung des zentralen Nervensystems (ZNS) dar. Relevante Daten zum Krankheitsverlauf der MS wurden durch Untersuchungen im Tiermodell der experimentellen autoimmunen Enzephalomyelits (EAE) gewonnen. Nach gegenwärtigem Kenntnisstand wandern autoreaktive T-Zellen vom T-Helfer-1 (Th-1) Typ in das ZNS ein und lösen eine gegen Bestandteile der Markscheide gerichtete Entzündung aus. Diese Zellen exprimieren große Mengen des für sie typischen Zytokin Interferon-gamma (IFN-gamma), was in zahlreichen Zelltypen die Expression des Tryptophan degradierenden Enzyms Indolamin 2,3-Dioxygenase (IDO) induziert. Seit 1998 ist bekannt, dass die Inhibition der IDO zu einer durch T-Zellen vermittelten Abstoßung allogener Feten führt. Diese immunregulatorische Funktion von IDO konnte auf den schnellen Abbau der essentiellen Aminosäure Tryptophan und der im folgenden Abbauweg synthetisierten T-Zell-toxischen Metabolite zurückgeführt werden. Since 1998 it is known that inhibition of IDO induces the T cell-mediated rejection of allogeneic concepti in mice. The mechanism of this immunregulatory function of IDO was shown to be due to the degradation of the essential amino acid tryptophan and the subsequent synthesis of T cell toxic metabolites. Since the IDO-inducing cytokine IFN-gamma is also expressed within inflamed CNS tissue during MS and EAE, the present work investigated the role of IDO in immunregulation of the CNS during autoimmune neuroinflammation in the EAE model. A significant increase in relative IDO activity within the CNS during the acute and remission phases of EAE was identified by HPLC analysis. Systemic inhibition of IDO activity by the specific IDO inhibitor 1-methyl-tryptophan reduced the remission and exacerbated the progression of the disease in comparison to control animals.
