Abstract: The Sea Aster mining bee Colletes halophilus Verhoeff, 1944 is known as an endemic of the western European coasts of the Atlantic and the North Sea. The species has specific habitat requirements and is restricted to coastal habitats with populations of the Sea Aster (Tripolium pannonicum), its preferred host plant. Due to the late summer activity of adults and habitat specialisation, this solitary bee species is easily overlooked. In 2019, C. halophilus was first found at the Baltic Sea coast of the island of Fyn (Denmark), 2020 on the island of Rügen (Mecklenburg-Vorpommern, Germany) and in 2021 in Sehlendorf (Schleswig-Holstein, Germany), demonstrating that the species is far more widespread and also occurring along the more continental Western Baltic Sea coast and Kattegat. Targeted sampling in 2022 revealed that C. halophilus is now present in most of the potentially suitable coastal habitats in Schleswig-Holstein and at least in some parts of Mecklenburg-Vorpommern. We here present data on the currently known distribution of C. halophilus along the Baltic Sea coast of Denmark and Germany and discuss hypotheses of a potential recent range extension.

2. Pain interventions for people with dementia : a quasi-experimental study

Abstract: Background: Due to the complexity of the provision of care for people with dementia, pain assessment and management is still considered to be lacking. An optimal way to support frontline staff in providing pain assessment and management for people with dementia living in nursing homes has not yet been identified. The success of supporting interventions seems dependent on contextual factors in the nursing homes. This study, therefore, analyzes the feasibility of a nurse-led training intervention, using repeated on-site case studies, in modifying pain intensity and frequency in people with dementia. Methods: Using a quasi-experimental design, we undertook a multi-center study of nurse-led training in pain management, with subsequent on-site case studies. Healthcare workers from 3 nursing homes assessed pain in 164 residents with dementia over 147 days. We used mixed-effect growth curve models with spline regression to analyze the data. Results: We found that on-site case studies support frontline staff with pain management and assessment. Repeated reflection in case studies led to significantly longer pain free intervals (from 4.7 at baseline to 37.1 days at second follow-up) and decreased frequency of pain events (OR 0.54 at first follow-up and 0.43 at second follow-up). However no trends regarding pain intensity could be found. Therefore, on-site case studies may be valuable for improving pain frequency and pain-free intervals over time. Conclusion: This feasibility study shows the potential of on-site support for frontline nursing home staff. On-site case studies may also affect health outcomes in people with dementia. However, the complexity of dementia care necessitates the management of a broader range of needs.

4. Integrated Palliative Care Outcome Scale for People with Dementia : easy language adaption and translation

Abstract: Background: In this article, we report the cultural adaption and translation of the Integrated Palliative Care Outcome Scale for People with Dementia (IPOS-Dem) into a Swiss-German easy language version for proxy assessment of people with dementia living in Swiss nursing homes. The Swiss-German easy language version of the IPOS-Dem was developed and culturally adapted in a six-phase process from the German IPOS-Dem using recommended guidelines.With nursing home staff and laypeople, the conceptual definition and relevance of IPOS-Dem items were established during phase I. Phase II encompassed the completion of forward translations. Independent native speakers blind to the original scale translated and back-translated the Swiss-German easy language version. The resulting IPOS-Dem version was then blindly back-translated in phase III. Experts reviewed all resulting translations in phase IV to produce a pre-final IPOS-Dem version. Finally, the phase V cognitive debriefing involved two focus groups assessing the pre-final IPOS-Dem version. Phase V included cognitive interviews with laypeople (n = 2), family members of those with dementia (n = 4) and staff from different care contexts (n = 12). Results: Using easy language specialists yielded a clinically relevant, comprehensive and understandable translation. In addition, face and content validity for the easy language version were established in the cognitive interviews. Conclusions: With an easy language IPOS-Dem, all frontline staff and family members can be empowered to communicate their observations after caring interactions. Enhanced clinical communication with easy language tools shows the potential for research and clinical applications. In addition, attentive use in scales of easy language communication may foster increased engagement with untrained laypeople in clinical and care research.

Abstract: Background: In Switzerland, there is a lack of adequate rehabilitation services, and effective coordination, that take into account the multifactorial health risks of older people. The literature shows that the hospitalisation rate in rehabilitation facilities has increased in recent years and that a gender bias exists. Additionally, there is little or no evidence available on the effect that a post‑acute care programme might have over an extended period on functioning, quality of life and the informal network of older people. Therefore, the aim of this trial is to evaluate the sustainability of post‑acute care within three nursing homes in Zurich, Canton of Zurich, Switzerland. Methods: The Prevention Admission into Nursing homes (PAN) study is a explorative, prospective, longitudinal pilot trial based on a convenience sample of three long‑term care facilities in the Swiss Canton of Zurich. The proposed pilot study will examine the effects of a post‑acute care programme on people aged ≥65 years with a post‑acute care potential ≥ three admitted to any of the three post‑acute care units (n = 260). Older people of all sexes admitted to one of the post‑acute care units and likely to be discharged to home within 8 weeks will be eligible for participation in the study. The primary endpoint is functionality based on the Barthel Index. The secondary endpoints are independency based on delirium, cognition, mobility, falling concerns, frailty, weight/height/body mass index, post‑acute care capability, quality of life, and lastly, the informal network. As part of process evaluation, a qualitative evaluation will be conducted based on constructive grounded theory to specifically analyse how the experience of informal caregivers (n = 30) can contribute to a successful daily life 6 months after discharge. Abstract: Large-scale biodiversity databases have great potential for quantifying long-term trends of species, but they also bring many methodological challenges. Spatial bias of species occurrence records is well recognized. Yet, the dynamic nature of this spatial bias – how spatial bias has changed over time – has been largely overlooked. We examined the spatial bias of species occurrence records within multiple biodiversity databases in Germany and tested whether spatial bias in relation to land cover or land use (urban and protected areas) has changed over time. We focused our analyses on urban and protected areas as these represent two well-known correlates of sampling bias in biodiversity datasets. We found that the proportion of annual records from urban areas has increased over time while the proportion of annual records within protected areas has not consistently changed. Using simulations, we examined the implications of this changing sampling bias for estimation of long-term trends of species' distributions. When assessing biodiversity change, our findings suggest that the effects of spatial bias depend on how it affects sampling of the underlying land-use change drivers affecting species. Oversampling of regions undergoing the greatest degree of change, for instance near human settlements, might lead to overestimation of the trends of specialist species. For robust estimation of the long-term trends in species' distributions, analyses using species occurrence records may need to consider not only spatial bias, but also changes in the spatial bias through time.

Abstract: Aims: We aim to evaluate the effectiveness of the Integrated Palliative Care Outcome Scale for people with dementia-based case studies to improve the caring quality for people with dementia in nursing homes by frontline staff and family members. Background: Swiss nursing homes mostly care for people with dementia. This population is at high risk of receiving little to no palliation for their complex needs. The majority of Swiss frontline healthcare staff do not systematically report on the needs of their residents. Additionally, family members do not routinely participate in assessment processes. Design We will conduct a stepped-wedge cluster randomized trial of repeated assessment using the Integrated Palliative Care Outcome Scale for people with dementia (IPOS-Dem) and subsequent case studies. Clusters will consist of Swiss nursing homes randomly assigned to one of three sequential intervention time points. Methods: The study population will consist of people with dementia living in nursing homes with and without specialized dementia care facilities. Over 16 months, staff working at the frontline and family members will assess the needs and concerns of people with dementia using IPOS-Dem. Depending on sequence allocation, facilitated case studies will start after 3, 6 or 9 months. The primary outcome will be caring quality measured by QUALIDEM. The secondary outcome will be symptoms and concerns, as indicated by the IPOS-Dem sum-score. The Zürich Ethics Committee approved the study in 2019 (2019-01847). Impact: The results of this study will contribute to improving the effectiveness of person-centred care for people with dementia. Collaboration between healthcare staff and family members will be systematically developed and built upon thorough assessment using the IPOS-Dem and related case studies. Abstract: Recent astronomical data have provided the primordial deuterium abundance with percent precision. As a result, Big Bang nucleosynthesis may provide a constraint on the universal baryon to photon ratio that is as precise as, but independent from, analyses of the cosmic microwave background. However, such a constraint requires that the nuclear reaction rates governing the production and destruction of primordial deuterium are sufficiently well known. Here, a new measurement of the $^2$H($p,\gamma$)$^3$He cross section is reported. This nuclear reaction dominates the error on the predicted Big Bang deuterium abundance. A proton beam of 400-1650keV beam energy was incident on solid titanium deuteride targets, and the emitted $\gamma$-rays were detected in two high-purity germanium detectors at angles of 55$^\circ$ and 90$^\circ$, respectively. The deuterium content of the targets has been obtained in situ by the $^2$H($^3$He,$p$)$^4$He reaction and offline using the Elastic Recoil Detection method. The astrophysical S-factor has been determined at center of mass energies between 265 and 1094 keV, addressing the uppermost part of the relevant energy range for Big Bang nucleosynthesis and complementary to ongoing work at lower energies. The new data support a higher S-factor at Big Bang temperatures than previously assumed, reducing the predicted deuterium abundance., Comment: 13 pages, 9 figures, 4 tables; to be published in Phys. The majority of Swiss frontline healthcare staff do not systematically report on the needs of their residents. Additionally, family members do not routinely participate in assessment processes. Design We will conduct a stepped-wedge cluster randomized trial of repeated assessment using the Integrated Palliative Care Outcome Scale for people with dementia (IPOS-Dem) and subsequent case studies. Clusters will consist of Swiss nursing homes randomly assigned to one of three sequential intervention time points. Methods: The study population will consist of people with dementia living in nursing homes with and without specialized dementia care facilities. Over 16 months, staff working at the frontline and family members will assess the needs and concerns of people with dementia using IPOS-Dem. Depending on sequence allocation, facilitated case studies will start after 3, 6 or 9 months. The primary outcome will be caring quality measured by QUALIDEM. The secondary outcome will be symptoms and concerns, as indicated by the IPOS-Dem sum-score. The Zürich Ethics Committee approved the study in 2019 (2019-01847). Impact: The results of this study will contribute to improving the effectiveness of person-centred care for people with dementia. Collaboration between healthcare staff and family members will be systematically developed and built upon thorough assessment using the IPOS-Dem and related case studies. Abstract: Background: In all, 39 % of people living in Swiss nursing homes suffer from dementia. Detailed data about type and course of symptoms displayed by these patients in their terminal phase are lacking. Methods: This descriptive, retrospective study analysed 65nursing documents from deceased people with dementia in four nursing homes in the canton of Zurich, Switzerland. Results: Difficulties with mobility (81 %), pain (71 %) and sleep disturbance (63 %) were the most frequent of the 10identified symptoms. Towards the end of life, difficulties with mobility, sleep disturbance, agitation and other neuropsychiatric symptoms, such as episodes of depression, decreased (decrescent pattern), while pain, feeding problems, breathing abnormalities, apathy and anxiety increased (crescent pattern). Courses of pain were documented in 17 % of the nursing records. In addition, 76 % of the residents had been visited on a daily basis by next of kin in their last 7days, compared with only one third of residents previously. Furthermore, daily communication between healthcare professionals and next of kin tripled during this period. Conclusion: The documented prevalence of a high and increasing level of pain towards the end of life, combined with the lack of documented courses of pain, shows potential for improvement in pain relief and pain identification for patients with dementia in their terminal phase. The increasing number of visits by next of kin and the increasingly intensive contact between healthcare professionals and next of kin in the last 7days are a strong indicator that the end of life can be predicted relatively well by the involved participants and appropriate reactions follow.

Abstract: Pain after surgical interventions is a common issue. Less than 50 % of patients who underwent surgery report an adequate pain management. In a regional Swiss hospital the differences in pain scores, satisfaction and base medication between patients who underwent surgery electively and patients who underwent surgery in the case of an emergency were investigated. Influences on the pain scores of patients with non-pharmacological and pharmacological interventions were also investigated. 157 Patients from multiple surgical fields were surveyed for this study. No significant differences were found in the group comparisons. The mean pain score during rest on the NRS was 2.66 ± 2.22 (0-8). With 10.9 % of the participants the daily maximum dose was exceeded (Paracetamol and Metamizol-Sodium).

We then use interview results to explore potential solutions, applying modern information technology such as digital medical assistants. Lastly, those potential solutions are condensed in an activity scenario that can be used for further design science research activities.

To effective physician–patient communication, such as lack of time [10, 15], limited communication training [16], growing administrative duties [15], and low recall by patients of physicians’ information and recommendations [20]. The aim of this study is to explore the short- and long-term potential for improving the physician–patient consultation and thus adherence to treatment with the help of information technology. The physician–patient consultation as well as its preparation and follow-up are examined. We intend to identify starting points for the design of technological solutions that can be investigated and validated in further research with the help of Design Science Research (DSR) [17, 26]. We use scenarios to make problems and their potential solutions visible and tangible [30].

Abstract: Background: The frequency of late-onset Huntington's disease (>59 years) is assumed to be low and the clinical course milder. However, previous literature on late-onset disease is scarce and inconclusive. Objective: Our aim is to study clinical characteristics of late-onset compared to common-onset HD patients in a large cohort of HD patients from the Registry database. Methods: Participants with late- and common-onset (30–50 years)were compared for first clinical symptoms, disease progression, CAG repeat size and family history. Participants with a missing CAG repeat size, a repeat size of ≤35 or a UHDRS motor score of ≤5 were excluded. Results: Of 6007 eligible participants, 687 had late-onset (11.4%) and 3216 (53.5%) common-onset HD. Late-onset (n = 577) had significantly more gait and balance problems as first symptom compared to common-onset (n = 2408) (P <.001). Overall motor and cognitive performance (P <.001) were worse, however only disease motor progression was slower (coefficient, −0.58; SE 0.16; P <.001) compared to the common-onset group. Repeat size was significantly lower in the late-onset (n = 40.8; SD 1.6) compared to common-onset (n = 44.4; SD 2.8) (P <.001). Fewer late-onset patients (n = 451) had a positive family history compared to common-onset (n = 2940) (P <.001). Conclusions: Late-onset patients present more frequently with gait and balance problems as first symptom, and disease progression is not milder compared to common-onset HD patients apart from motor progression. The family history is likely to be negative, which might make diagnosing HD more difficult in this population. However, the balance and gait problems might be helpful in diagnosing HD in elderly patients.

35. Betreuungsindex in Pflegeheimen : Entwicklung und Validierung eines neuen Instruments zur Beurteilung von Betreuungsqualität in Pflegeheimen

Abstract: Erworben im Rahmen der Schweizer Nationallizenzen (http://www.nationallizenzen.ch), Hintergrund: Derzeit gibt es in der Literatur kein geeignetes Instrument zur Beurteilung der Betreuungsqualität in Pflegeheimen. Die Autoren haben deshalb ein neues Instrument, den Betreuungsindex in Pflegeheimen (Nursing Home Care Index, NCI), konzipiert und getestet. Material und Methode: Betreuungsqualität wird in der Literatur in 8 Dimensionen definiert. Es wurde ein Fragebogen mit 42 Fragen aus 12 validierten Messinstrumenten entwickelt. Der Originalfragebogen wurde mit 320 Mitarbeitern aus 15 Alteneinrichtungen getestet. Die Daten wurden mithilfe einer Faktorenanalyse und Cronbachs α untersucht und auf 3 Faktoren und 16 Fragen reduziert. Das revidierte Instrument wurde in einer weiteren Studie mit 136 Mitarbeitern auf seine Anwendbarkeit überprüft. Ergebnisse: Als Ergebnis der Faktorenanalyse konnten 16 Items mit einer 3-Faktoren-Struktur, d. h. soziale Teilhabe, emotionales Wohlbefinden und Selbstbestimmung, identifiziert werden. Diese 3 Faktoren erklären 51,2% der Gesamtvarianz. Die Reliabilität der Gesamtskala beträgt 0,87, die der 3 Subskalen Selbstbestimmung 0,86, emotionales Wohlbefinden 0,71 und soziale Beziehungen bzw. Teilhabe 0,78. Die Gesamtpunktezahl des NCI erlaubt eine Kategorisierung der Betreuungsqualität in den 3 Abstufungen gut, ausreichend und dringender Verbesserungsbedarf. Schlussfolgerungen: Der NCI hat eine doppelte Funktion. Einerseits dient er Mitarbeitern und dem Heimmanagement als internes Qualitätssicherungsinstrument. Andererseits könnte es zukünftig Angehörigen von potenziellen Heimbewohnern die Möglichkeit bieten, die Betreuungsqualität verschiedener Pflegeheime zu vergleichen. Abstract: Erworben im Rahmen der Schweizer Nationallizenzen (http://www.nationallizenzen.ch), Background: In many countries, people over 85 years of age are relocated involuntarily or unplanned to a nursing home. In Switzerland, 43% of elderly over 85 years are admitted to nursing homes after hospital discharge. This percentage is higher than in the USA with 32.5% or in Germany with only 19%. Despite those more frequent Swiss admissions, no research has been conducted exploring how unplanned admissions to nursing homes affect the adaptation. Therefore, the aim of this study was to gain an in-depth understanding into unplanned admissions to nursing homes and to explore its impact on adaptation. Methods: The study used a qualitative interview design based on Meleis’ transition model. Secondary data analysis was guided by Mayring's qualitative content analysis. Face-to-face interviews with elderly over 77 years (n = 31) were conducted from a convenience sample in Switzerland between January and March 2013. Results: The following four patterns of adaptation emerged from the analysis: “being cut-off,” “being restricted,” “being cared for,” and “moving on.” The patterns evaluate the relocation into nursing homes and provide an opportunity to appraise the stages of adaption. Conclusions: This study presents a model of analysis to evaluate patterns of adaptation following an unplanned admission to a nursing home after hospital discharge.

Abstract: Der Eintritt ins Pflegeheim wird als eines der herausforderndsten Lebensereignisse bezeichnet. Pflegende sind erste Ansprechpersonen wenn es darum geht, diesen Prozess zu begleiten. Allerdings fehlen ihnen bislang Erkenntnisse darüber, wie sich ein Heimeintritt und der damit verbundene Veränderungsprozess aus der Sicht Betroffener gestaltet. Dies erschwert die Planung gezielter Interventionen und Unterstützungsangebote. Ziel der qualitativ-deskriptiven Studie war es daher den Entscheidungsprozess zum Heimeintritt aus der Sicht von Betroffenen darzustellen. Hierzu erfolgte eine Sekundäranalyse 62 qualitativer Einzelinterviews mit älteren Personen nach Heimeintritt. Die Daten wurden mittels Inhaltsanalyse analysiert. Es kristallisierten sich drei aufeinanderfolgende Phasen des Heimeintritt heraus: die oft mehrjährige Planung, die Entscheidungsphase (von der ersten Anmeldung bis zur endgültigen Entscheidung) und die Transition. Mitbestimmung und Entscheidungsfreiräume scheinen in allen Phasen wichtig zu sein, sowohl vor als auch während und nach dem tatsächlichen Heimeintritt. Auch umfasst die Transition mitunter Jahre und belastet Betroffene. Pflegerische Interventionen könnten dazu beitragen, dass das Lebensereignis Heimeintritt positiv oder weniger einschneidend erlebt wird. Admission to a nursing home is considered to be one of the most challenging life events for older people. Since nurses are primary contact persons, they could provide support in the relocation process.
