Background: The influence of smoking on the risk of periprosthetic joint infection (PJI) remains unclear. The objective was to explore the impact of smoking on PJI after primary total knee (TKA) and hip (THA) arthroplasty. Hypothesis: Current smoking patients should have an increased risk of PIJ compared with nonsmoking patients. Patients and methods: A prospective registry-based observational cohort study was performed. A total of 4591 patients who underwent primary TKA (3076 patients) or THA (1515) were included. According to the smoking status at the time of arthroplasty, patients were classified as nonsmokers (3031 patients), ex-smokers (688), and smokers (872). Multivariate analysis included smoking status, age, gender, education level, body mass index, American Society of Anesthesiologists class, diagnosis (osteoarthritis, rheumatism), diabetes, chronic obstructive pulmonary disease, perioperative blood transfusion, site of arthroplasty (knee, hip), length of operation, and length of stay. Results: There were PJI after 59 (1.9%) TKA and 27 (1.8%) THA (p = 0.840). There were PJI in 47 (1.6%) nonsmokers, 12 (1.7%) ex-smokers, and 17 (1.9%) smokers (p = 0.413). There were wound complications (delayed wound healing and superficial wound infection) in 34 (0.7%) nonsmokers, 9 (1.3%) in ex-smokers, and 17 (1.9%) in smokers (p = 0.045). In multivariate analysis, only the female gender was a significant predictor of PJI (OR 1.3, 95% CI 1.1–2.4 [p = 0.039]). Specifically, the categories of ex-smokers (OR 0.8, 95% CI 0.2–1.7 [p = 0.241]) and smokers (OR 1.1, 95% CI 0.6–1.5 [p = 0.052]) were not significant predictors. The 4-year arthroplasty survival with PJI as the endpoint was 99.1% (95% CI: 99.0–99.7) for nonsmokers, 99.0% (95% CI: 98.8–99.2) for ex-smokers, and 98.7% (95% CI: 98.2–99.0) for smokers was not significantly different between smoking status groups (p = 0.318). Discussion: Smoking was not identified as a significant predictor for PJI following primary TKA

Background: A hip fracture usually presents significant blood loss in the perioperative period, with a transfusion rate of 20-60%. In order to reduce the complications associated with this procedure, the administration of Tranexamic Acid (TXA) has been implemented in the treatment of perioperative anemia. The objectives were to evaluate the effectiveness and safety of a single dose of intravenous Tranexamic Acid (TXA) in reducing perioperative blood loss and the requirement for transfusion in elderly patients undergoing hip fracture surgery within one postoperative year. Methods: A double-blind randomized controlled trial was conducted on 129 patients with hip fractures. After randomization, at the start of the surgery, 65 patients received a single dose of 1 gram of intravenous TXA (TXA group), and 64 received a placebo (placebo group). The primary effectiveness outcomes were the total blood loss and transfusion rate. The primary safety outcome was the rate of thromboembolic events. Data on surgical or medical infection, readmission and death were also collected. Results: The TXA group had a significant decrease in blood loss (p = 0.006) and requirement for transfusion (p < 0.001) compared with the placebo group. Likewise, there were no thromboembolic events in the TXA group and seven in the placebo group (p = 0.006). Mortality within 1-year postoperatively was not significantly different (p = 0.115). Conclusion: Using a single dose of intravenous TXA at the start of the surgery significantly reduces blood loss and the requirement for transfusion without increasing the risk of thromboembolic events or mortality within 1-year postoperatively in patients with hip fracture undergoing surgery. It includes 287 globally distributed coring sites, with metadata, isotopic and chronostratigraphic information, and age models. A quality check was performed for all data and age models, and sites with at least millennial resolution were preferred. Deep water mass structure as well as differences between the early deglaciation and LGM are captured by the data, even though its coverage is still sparse in many regions. We find high correlations among time series calculated with different age models at sites that allow such analysis. The database provides a useful dynamical approach to map physical and biogeochemical changes of the ocean throughout the last deglaciation.

Background: A European response plan to burn mass casualty incidents has been jointly developed by the European Commission and the European Burn Association. Upon request for assistance by an affected country, the plan outlines a mechanism for coordinated international assistance, aiming to alleviate the burden of care in the affected country and to offer adequate specialized care to all patients who can benefit from it. To that aim, Burn Assessment Teams are deployed to assess and triage patients. Their transportation priority recommendations are used to distribute outnumbering burn casualties to foreign burn centers. Following an appropriate medical evacuation, these casualties receive specialized care in those facilities. Methods: The European Burns Association's disaster committee developed medical-organizational guidelines to support this European plan. The experts identified fields of interest, defined questions to be addressed, performed relevant literature searches, and added their expertise in burn disaster preparedness and response. Due to the lack of high-level evidence in the available literature, recommendations and specially designed implementation tools were provided from expert opinion. The European Burns Association officially endorsed the draft recommendations in 2019, and the final full text was approved by the EBA executive committee in 2022. Recommendations: The resulting 46 recommendations address four fields. Field 1 underlines the need for national preparedness plans and the necessary core items within such plans, including coordination and integration with an international response. Field 2 describes Burn Assessment Teams' roles, composition, training requirements, and reporting goals. Field 3 addresses the goals of specialized in-hospital triage, appropriate severity criteria, and their effects on priorities and triage. Methods: The European Burns Association drafted medical guidelines for burn mass casualty incidents based on a literature review and an in-depth analysis of the Romanian incident. An online questionnaire surveyed European burn centers and EU States for burn mass casualty preparedness. Results: The Romanian burn mass casualty in 2015 highlighted the lack of a burn-specific mechanism, leading to the late onset of international transfers. In Europe, 71% of respondents had existing mass casualty response plans, though only 35% reported having a burn-specific plan. A burns response plan for burn mass casualties was developed and adopted as a Commission staff working document in preparation for further implementation. The plan builds on the existing Union Civil Protection Mechanism framework and the standards of the WHO Emergency Medical Teams initiative to provide 1) burn assessment teams for specialized in-hospital triage of patients, 2) specialized burn care across European burn centers, and 3) medevac capacities from participating states. The auxin-inducible degron (AID) system has emerged as a powerful tool to conditionally deplete proteins in a range of organisms and cell types. Here, we describe a toolkit to augment the use of the AID system in Caenorhabditis elegans. We have generated a set of single-copy, tissue-specific (germline, intestine, neuron, muscle, pharynx, hypodermis, seam cell, anchor cell) and pan-somatic TIR1-expressing strains carrying a co-expressed blue fluorescent reporter to enable use of both red and green channels in experiments. These transgenes are inserted into commonly used, well-characterized genetic loci. We confirmed that our TIR1-expressing strains produce the expected depletion phenotype for several nuclear and cytoplasmic AID-tagged endogenous substrates. We have also constructed a set of plasmids for constructing repair templates to generate fluorescent protein::AID fusions through CRISPR/Cas9-mediated genome editing. These plasmids are compatible with commonly used genome editing approaches in the C. elegans community (Gibson or SapTrap assembly of plasmid repair templates or PCR-derived linear repair templates). Together these reagents will complement existing TIR1 strains and facilitate rapid and high-throughput fluorescent protein::AID tagging of genes. This battery of new TIR1-expressing strains and modular, efficient cloning vectors serves as a platform for straightforward assembly of CRISPR/Cas9 repair templates for conditional protein depletion.

In sexually reproducing metazoans, spermatogenesis is the process by which uncommitted germ cells give rise to haploid sperm. Work in model systems has revealed mechanisms controlling commitment to the sperm fate, but how this fate is subsequently executed remains less clear. While studying the well-established role of the conserved nuclear hormone receptor transcription factor, NHR-23/NR1F1, in regulating C. elegans molting, we discovered that NHR-23/NR1F1 is also constitutively expressed in developing primary spermatocytes and is a critical regulator of spermatogenesis. In this novel role, NHR-23/NR1F1 functions downstream of the canonical sex-determination pathway. Degron-mediated depletion of NHR-23/NR1F1 within hermaphrodite or male germlines causes sterility due to an absence of functional sperm, as depleted animals produce arrested primary spermatocytes rather than haploid sperm. These spermatocytes arrest in prometaphase I and fail to either progress to anaphase or attempt spermatid-residual body partitioning. They make sperm-specific membranous organelles but fail to assemble their major sperm protein into fibrous bodies. NHR-23/NR1F1 appears to function independently of the known SPE-44 gene regulatory network, revealing the existence of an NHR-23/NR1F1-mediated module that regulates the spermatogenesis program.

Introduction: Treatment of the distal radius fracture in elderly remains controversial. The objective was to assess the outcomes of volar locking plate for displaced complete intra-articular distal radius fractures in elderly as compared to younger patients. Hypothesis: The outcomes in elderly patients would be comparable with those in younger patients with a low rate of complications. Material and methods: Non-randomised prospective comparative study between 70 patients older than 65 years and 46 younger patients with AO type-C fractures. The main outcome was Disabilities Arm, Shoulder and Hand (DASH) score. Secondary variables were Patient-Rated Wrist Evaluation (PRWE) score, range of motion, Visual Analogue Scale (VAS) for pain, and grip strength. Radiological measurements were also performed. Results: The mean follow-up was 30.9 (range, 24–53) months. There were no significant differences in mean DASH, PRWE, VAS-pain, wrist motion or radiological parameters at final follow-up. Multivariate analysis showed that the functional outcomes were significantly influenced by baseline ulnar positivity greater than 3 mm at baseline but not by age. Discussion: The study hypothesis was confirmed. Surgical treatment with volar locking plate for displaced complete intra-articular fractures of the distal radius in elderly patients represents a safe and effective treatment alternative with similar early complication rate than in younger. To identify environmental causes for past changes in vegetation in subtropical East Asia, we present carbon isotope compositions of plant-wax n-alkanes and provide estimates of the C4-plant contribution across the past four glacial terminations and interglacials, based on cores recovered from the northern South China Sea. Our results show a comparable C4-plant contribution between the Last Glacial Maximum (LGM) and the Holocene. An increase of the C4-plant contribution by 15–20% is found for Terminations IV, II and I relative to subsequent interglacial peaks, coeval with an expansion of Cyperaceae and Poaceae. In contrast, Termination V reveals a lower C4-plant contribution than Marine Isotope Stage (MIS) 11c. The data exhibit a long-term trend, with a stepwise increase of the C4-plant contribution across interglacials MIS 11c, 9e, 7e and 1. We suggest that no substantial changes in humidity levels over glacial-interglacial cycles occurred facilitating a similar C3/C4-plant ratio for the LGM and the Holocene. Instead, deglacial sea-level rises caused an extensive development of floodplains and wetlands on the exposed continental shelf, providing habitats for the spread of C4 sedges and grasses. The progressive subsidence of Chinese coastal areas and the broadening of the continental shelf over the late Quaternary explains the nearly absence of C4 plant occurrence during Termination V and a gradual increase of the C4-plant contribution across interglacial peaks. Taken together, changes in coastal environments should be considered when interpreting marine-based vegetation reconstructions from subtropical Asia.

INTRODUCTION Bromelain-based Enzymatic Debridement has been introduced as an additional concept to the burn surgeon's armamentarium and is best indicated for mid-to deep dermal burns with mixed patterns. Increasing evidence has been published focusing on special regions and settings as well as on limitations of Enzymatic Debridement to improve patient care. To better guide Enzymatic Debridement in view of the increasing experience, there is a need to update the formerly published consensus guidelines with user-orientated recommendations, which were last produced in 2017. METHODS A multi-professional expert panel of plastic surgeons and burn care specialists from twelve European centers was convened, to assist in developing current recommendations for best practices with use of Enzymatic Debridement. Consensus statements were based on peer-reviewed publications and clinical relevance, and topics for re-evaluation and refinement were derived from the formerly published European guidelines. For consensus agreement, the methodology employed was an agreement algorithm based on a modification of the Willy and Stellar method. For this study on Enzymatic Debridement, consensus was considered when there was at least 80 % agreement to each statement. RESULTS The updated consensus guidelines from 2019 refer to the clinical experience and practice patterns of 1232 summarized patient cases treated by the panelists with ED in Europe (2017: 500 cases), reflecting the impact of the published recommendations. We compared outcomes in elderly patients with intra-articular distal radius fractures treated by closed reduction and plaster immobilization or open reduction and internal fixation with a volar plate. Ninety-seven patients older than 60 years were randomly allocated to conservative (47 patients) or surgical (50 patients) treatment. Over a 2-year period, we assessed patient-rated wrist evaluation score, DASH (disability arm, shoulder and hand) questionnaire, pain, wrist range of motion, grip strength, and radiological parameters. The functional outcomes and quality of life were significantly better after volar plating fixation compared with conservative treatment. We found that restoration of the articular surface, radial inclination, and ulnar variance affected the outcomes, but the articular step-off did not. Twenty-five per cent of the patients with conservative treatment had secondary loss of reduction. We conclude that surgical plating leads to better outcomes than conservative treatment for elderly patients with intra-articular distal radius fractures.

Methods: Prospective cohort of 66 consecutive patients with mean age of 68 (range 60–81) years and AO type C fractures treated with volar locking plate. Radiographic measurements were performed pre and post-operatively at each follow-up. Clinical assessment was made by Patient-Rated Wrist Evaluation scores, Disabilities of the Arm, Shoulder and Hand score, range of motion, and grip strength. Results: The mean post-operative follow-up was 31 (range, 24–47) months. The most difficult parameters to restore intra-operatively were volar tilt and radial height, while radial inclination and ulnar variance were restored in a high rate of patients. There was significant loss of reduction within four post-operative months in volar tilt (p = 0.001) and radial height (p = 0.029). Mean radiographic parameters had no changes from four months to final evaluation. At final follow-up, 50.1% of the patients had all radiographic parameters restored in comparison with the noninvolved wrist, but 83.3% had all radiographic parameters within a functional range (p = 0.001). No significant relationship between radiographic and functional outcomes was found (p = 0.474). Conclusions: This study found that volar locking plate fixation for displaced intra-articular distal radius fractures in elderly patients was an effective procedure to obtain success functional outcomes. Although reduction loss in volar tilt and radial height occurred within four first months, volar plate was able to maintain fracture stabilization with radiographic parameters within functional range over the time in most patients of these elderly patients.

Methods: This is a retrospective comparative study between 93 patients with an interval between stages of 6-8 months (6-month group), 112 of 12-14 months (1-year group), and 108 of 24-26 months (2-year group). Outcome variables were Knee Society scores, Western Ontario and McMaster Universities, Short Form, and patient satisfaction. Results: Overall, the mean follow-up for the first TKA was 8.2 (range, 7-10) years, and for the second TKA, 6.7 (range, 5-10) years. At last follow-up, functional and patient-related outcomes were similar for both knees, regardless of the interval. However, mental score and patient satisfaction were significantly better for the second than for the first TKA in the 2-year group. Age did not correlate significantly with the functional scores but was significantly correlated with the mental score. Conclusion: The performing staged bilateral TKA with a wide interval between surgeries provided equivalent functional outcomes and quality of life for both knees. Postoperative outcomes were not affected by the length of the time interval between procedures or age. Our results can help the surgeon to inform the patients reliably about what they can expect in the delay of a second knee replaced. Thus, patients could make an informed decision.

Here more than 1700 C-13 observations of the benthic foraminifera genus Cibicides from late Holocene sediments (C-13(Cibnat)) are compiled and compared with newly updated estimates of the natural (preindustrial) water column C-13 of dissolved inorganic carbon (C-13(DICnat)) as part of the international Ocean Circulation and Carbon Cycling (OC3) project. Using selection criteria based on the spatial distance between samples, we find high correlation between C-13(Cibnat) and C-13(DICnat), confirming earlier work. Regression analyses indicate significant carbonate ion (-2.60.4)x10(-3)/(molkg(-1)) [CO32-] and pressure (-4.91.7)x10(-5)m(-1) (depth) effects, which we use to propose a new global calibration for predicting C-13(DICnat) from C-13(Cibnat). This calibration is shown to remove some systematic regional biases and decrease errors compared with the one-to-one relationship (C-13(DICnat)=C-13(Cibnat)). However, these effects and the error reductions are relatively small, which suggests that most conclusions from previous studies using a one-to-one relationship remain robust. The remaining standard error of the regression is generally sigma 0.25 parts per thousand, with larger values found in the southeast Atlantic and Antarctic (sigma 0.4 parts per thousand) and for species other than Cibicides wuellerstorfi. A paleoproductivity record from this area may therefore resolve temporal changes in the westerly dynamics. Accordingly, we produced a coccolith Sr/Ca-based paleoproductivity record from core MD96-2080 (Agulhas Bank slope) during the penultimate glacial-interglacial cycle. Deriving the productivity signal from Sr/Ca requires a correction for a temperature effect, here constrained using Mg/Ca sea surface temperatures from the foraminifer Globigerina bulloides from core MD96-2080. Phases of depressed productivity coincided with periods of stratification in the same core, indicated by high relative abundances of the coccolithophore Florisphaera profunda and with low relative abundances of the upwelling indicator G. bulloides in the nearby Cape Basin. These observations collectively suggest that productivity was regulated by upwelling throughout this region. We infer that, as in the present, periods of low productivity result from a more northerly position of the westerlies, potentially accompanied by subtropical front displacements, and blockage of upwelling promoting easterlies. Productivity minima also coincide with periods of increased ice-rafted detritus (IRD) deposition on the Agulhas Plateau, which also indicates extreme northward positions of the westerlies. The influence of the westerlies appears to be obliquity conditioned, as productivity minima (and IRD maxima) occur during low-obliquity intervals. A paleoproductivity record from this area may therefore resolve temporal changes in the westerly dynamics. Accordingly, we produced a coccolith Sr/Ca-based paleoproductivity record from core MD96-2080 (Agulhas Bank slope) during the penultimate glacial-interglacial cycle. Deriving the productivity signal from Sr/Ca requires a correction for a temperature effect, here constrained using Mg/Ca sea surface temperatures from the foraminifer Globigerina bulloides from core MD96-2080. Phases of depressed productivity coincided with periods of stratification in the same core, indicated by high relative abundances of the coccolithophore Florisphaera profunda and with low relative abundances of the upwelling indicator G. bulloides in the nearby Cape Basin. These observations collectively suggest that productivity was regulated by upwelling throughout this region. We infer that, as in the present, periods of low productivity result from a more northerly position of the westerlies, potentially accompanied by subtropical front displacements, and blockage of upwelling promoting easterlies. Productivity minima also coincide with periods of increased ice-rafted detritus (IRD) deposition on the Agulhas Plateau, which also indicates extreme northward positions of the westerlies. The influence of the westerlies appears to be obliquity conditioned, as productivity minima (and IRD maxima) occur during low-obliquity intervals. The dynamic connection between productivity and the westerlies is supported by coeval salinity changes in the South Indian Gyre that likewise respond sensitively to a poleward contraction of the westerlies.

The AAIW past variability is poorly understood mainly due to the lack of sedimentary archives at intermediate water depths. We present records of benthic stable isotopes from sediments retrieved with the seafloor drill rig MARUM-MeBo at 956m water depth off northern Chile (GeoB15016, 27 degrees 29.48S, 71 degrees 07.58W) that extend back to 970ka. The sediments at this site are presently deposited at the boundary between AAIW and Pacific Deep Water (PDW). For previous peak interglacials, our results reveal similar benthic C-13 values at site GeoB15016 and of a newly generated stack of benthic C-13 from various deep Pacific cores representing the average PDW. This suggests, unlike today, the absence of AAIW at the site and the presence of nearly pure PDW. In contrast, more positive C-13 values at site GeoB15016 compared to the stack imply a considerable AAIW contribution during cold phases of interglacials and especially during glacials. Besides, we used three short sediment cores to reconstruct benthic C-13 values from the AAIW core during the last glacial and found a C-13 signature similar to today's. Assuming that this was the case also for the past 970 kyr, we demonstrate that sea level changes and latitudinal migrations of the AAIW formation site can only account for about 50% of the full range of past C-13 increases at site GeoB15016 during cold periods. Other processes that could explain the remaining of the positive C-13 anomalies are increases in glacial AAIW production and/or deeper convection of the AAIW with respect to preceding interglacials.
