Abstract

Objective: Management of follow-up protocols after endovascular aortic repair (EVAR), vary significantly between centres and is not standardized according to the sac regression. By designing an international expert-based Delphi consensus, the study aimed to create recommendations on follow-up after EVAR according to sac evolution. Methods: Eight facilitators created appropriate statements regarding the study topic that were voted, using a 4-point Likert scale, by a selected panel of international experts using a three-round modified Delphi consensus process. Based on the experts' responses, only those statements reaching a Grade A (full agreement ≥75%) or B (overall agreement ≥80% and full disagreement <5%) were included in the final document. Results: One-hundred and seventy-four participants were included in the final analysis, and each voted the initial 29 statements related to the definition of sac regression (Q1-Q9), EVAR follow-up (Q10-Q14), and the assessment and role of sac regression during follow-up (Q15-Q29). At the end of the process, 2 statements (6.9%) were rejected, 9 statements (31%) received a grade B consensus strength, and 18 (62.1%) reached a grade A consensus strength. Out of twenty-seven final statements, fifteen statements (55.6%) were classified as grade I, while twelve (44.4%) were classified as grade II. Experts agreed that sac regression should be considered an important indicator of EVAR success and always be assessed during follow-up after EVAR. Conclusions: Based on the elevated strength and high consistency of this international expert-based Delphi consensus, most of the statements might guide current clinical management of follow-up after EVAR according to the sac regression. Future studies are needed to clarify debated issues.

Abstract

Objective: The aim of this study was to investigate the early outcomes of a novel off the shelf pre-loaded inner branched thoraco-abdominal endograft (E-nside) in the treatment of aortic pathologies.Methods: Data from a physician initiated national multicentre registry on patients treated with the E-nside endograft, were prospectively collected and analysed. Pre-operative clinical and anatomical characteristics, procedural data, and early outcomes (90 days) were recorded in a dedicated electronic data capture system. The primary endpoint was technical success. Secondary endpoints were early mortality (90 days), procedural metrics, target vessel patency, endoleak rate, and major adverse events (MAEs) at 90 days.Results: In total, 116 patients from 31 Italian centres were included. Mean + standard deviation (SD) patient age was 73 + 8 years and 76 (65.5%) were male. Aortic pathologies included degenerative aneurysm in 98 (84.5%), post-dissection aneurysm in five (4.3%), pseudoaneurysm in six (5.2%), penetrating aortic ulcer or intramural haematoma in four (3.4%), and subacute dissection in three (2.6%). Mean + SD aneurysm diameter was 66 + 17 mm; aneurysm extent was Crawford I -III in 55 (50.4%), IV in 21 (19.2%), pararenal in 29 (26.7%), and juxtarenal in four (3.7%). The procedure setting was urgent in 25 (21.5%) patients. Median procedural time was 240 minutes (interquartile range [IQR] 195, 303), with a median contrast volume of 175 mL (IQR 120, 235). The endograft's technical success rate was 98.2% and the 90 day mortality rate was 5.2% (n = 6; 2.1% for elective repair and 16% for urgent repair). The 90-days cumulative MAE rate was 24.1% (n = 28). At 90 days, there were 10 (2.3%) target vessel related events (nine occlusions and one type IC endoleak) and one type 1A endoleak requiring re-intervention.Conclusion: In this real life, non-sponsored registry, the E-nside endograft was used for the treatment of a broad spectrum of aortic pathologies, including urgent c

Abstract

Objective: Management of antithrombotic therapy in patients undergoing elective fenestrated branched endovascular aortic repair (F-BEVAR) is not standardised, nor are there any recommendations from current guidelines. By designing an international expert based Delphi consensus, the study aimed to create recommendations on the pre-, intra-, and postoperative management of antithrombotic therapy in patients scheduled for elective F-BEVAR in high volume centres.Methods: Eight facilitators created appropriate statements regarding the study topic that were voted on, using a four point Likert scale, by a selected panel of international experts using a three round modified Delphi consensus process. Based on the experts' responses, only those statements reaching Grade A (full agreement >= 75%) or B (overall agreement >= 80% and full disagreement < 5%) were included in the final document. The round answers' consistency was graded using Cohen's k, the intraclass correlation coefficient, and, in case of double re-submission, the Fleiss k.Results: Sixty-seven experts were included in the final analysis and voted the initial 43 statements related to pre- (n = 15), intra- (n = 10), and post-operative (n = 18) management of antithrombotic drugs. At the end of the process, six statements (13%) were rejected, 20 statements (44%) received a Grade B consensus, and 18 statements (40%) reached a Grade A consensus. Most statements (27; 71%) exhibited very high or high consistency grades, and 11 (29%) a fair or poor grading. The intra-operative statements mostly concentrated on threshold for and monitoring of proper heparinisation. The pre- and post-operative statements mainly focused on indications for dual antiplatelet therapy and its management, considering the possible need for cerebrospinal fluid drainage.Conclusion: Based on the elevated strength and high consistency of this international expert based Delphi consensus, most of the statements might guide current clinical man

Abstract

In meteor physics the luminous efficiency ${\tau}$ is used to convert the meteor's magnitude to the corresponding meteoroid's mass. However, lack of sufficiently accurate verification methods or adequate laboratory tests leave this parameter to be controversially discussed. In this work meteor/fireball data obtained by the Fireball Recovery and InterPlanetary Observation Network (FRIPON) was used to calculate the masses of the pre-atmospheric meteoroids which could in turn be compared to the meteor brightnesses to assess their luminous efficiencies. For that, deceleration-based formulas for the mass computation were used. We have found ${\tau}$-values, as well as the shape change coefficients, of 294 fireballs with determined masses in the range of $10^{-6}$ kg - $100$ kg. The derived ${\tau}$-values have a median of ${\tau}_{median}$ = 2.17 %. Most of them are on the order of 0.1 % - 10 %. We present how our values were obtained, compare them with data reported in the literature, and discuss several methods. A dependence of ${\tau}$ on the pre-atmospheric velocity of the meteor, $v_e$, is noticeable with a relation of ${\tau}=0.0023 \cdot v_e^{2.3}$. The higher luminous efficiency of fast meteors could be explained by the higher energy released. Fast meteoroids produce additional emission lines that radiate more efficiently in specific wavelengths due to the appearance of the so-called second component of higher temperature. Furthermore, a dependence of ${\tau}$ on the initial meteoroid mass, $M_e$, was found, with negative linear behaviour in log-log space: ${\tau}=0.48 \cdot M_e^{-0.47}$. This implies that the radiation of smaller meteoroids is more efficient.

Abstract

BACKGROUND: This paper reports the 4-year safety and effectiveness of the INCRAFT (R) AA Stent-Graft System (Cordis Corp., Milpitas, CA, USA), an ultra-low-profile device for the treatment of abdominal aortic aneurysms. METHODS: The INNOVATION Trial is the prospective, first-in-human, multicenter trial to evaluate the safety and effectiveness of the INCRAFT (R) System. Patients underwent annual clinical and computed tomography angiography examination as part of the study protocol. The INCRAFT (R) AAA Stent-Graft System is a customizable tri-modular design, with an ultra-low profile (14-Fr) delivery system. Patient were treated under approved protocol, the prescribed clinical and imaging follow-up at annually through 5 years. Results analyzed and adjudicated by a clinical events committee, independent core laboratory, and a data safety and monitoring board. This manuscript reports results through 4 years of follow-up. RESULTS: A total of 60 patients were enrolled in the trial, all of whom were successfully treated. Follow-up rates at 1 and 4 years were 93% (56/60) and 85% (51/60), respectively. All-cause mortality at 4 years was 17.6% and no death was AA-, device-, or procedure-related. The secondary reintervention rate at 1 year was 4.6%, primarily the result of stent thrombosis. In total, 10 patients required 13 post-procedure interventions within 4-years of follow-up (2 to repair a type I endoleak, 4 to repair a type II endoleak, 1 for stent thrombosis, 1 for renal stenosis, 1 for aneurysm enlargement, 2 for limb migration and 2 for prosthesis stenosis or occlusion). There were 4 cases (10%) of aneurysm enlargement reported at the 4 year follow-up. At 4 years, 38 out of 39 patients were free from type I and II endoleaks. There were no proximal type I or type II endoleaks at 4-year follow-up. Core laboratory evaluation of the postoperative imaging studies indicated absence of endograft migration while a single fracture was demonstrated without any clinical sequelae

7. Cavezzo—The double face of a meteorite: Mineralogy, petrography, and geochemistry of a very unusual chondrite

Abstract

The Cavezzo meteorite, which fell on January 1, 2020, is the first meteorite detected and recovered by the Italian PRISMA Fireball Network. Two specimens, weighing 3.12 g (specimen 1) and 52.19 g (specimen 2), were collected 3 days after the bolide was observed, thanks to an effective media campaign that encouraged the involvement of local people. The two specimens of this meteorite have not only completely different lithological characteristics but also a different geochemistry and oxygen isotopic composition as well. Specimen 1 is anomalous both for the textural–structural features, varying seamlessly from chondritic to “achondritic,” and a very unusual modal mineralogy—such as the relatively high amount of olivine (63.1 vol%), plagioclase (18.2 vol%), high-Ca pyroxene (10.3 vol%), and chlorapatite (2.1 vol%); and the unusually low content of low-Ca pyroxene (5.8 vol%), metal (0.1 vol%), and troilite (much lesser than 0.1 vol%)—although the compositional values for olivine (Fa 24.24 mol%) and low-Ca pyroxene (Fs 20.41 mol%) appear to be similar to those of the L chondrite group. Conversely, in specimen 2, not only the texture and the crystal chemistry but also the modal mineralogy (low-Ca pyroxene much more abundant than high-Ca pyroxene and occurrence of metal and sulfides) look like those of an ordinary L chondrite. The differences between the two specimens are also confirmed by geochemistry. The oxygen isotope composition of specimen 1 plots at the boundary between the H and L groups (δ17O‰ 3.250; δ18O‰ 4.736; Δ17O‰ 0.788) whereas specimen 2 plots at the boundary of the L and LL fields (δ17O‰ 3.737; δ18O‰ 4.957; Δ17O‰ 1.159). The bulk chemistry shows a different content of many minor and trace elements (including rare earth elements), such as a strong depletion of siderophile and chalcophile elements in specimen 1. Abstract

Among the many ungrouped meteorites, Acfer 370, NWA 7135, and El Médano 301—probably along with the chondritic inclusion in Cumberland Falls and ALHA 78113—represent a homogeneous grouplet of strongly reduced forsterite‐rich chondrites characterized by common textural, chemical, mineralogical, and isotopic features. All of these meteorites are much more reduced than OCs, with a low iron content in olivine and low‐Ca pyroxene. In particular, Acfer 370 is a type 4 chondrite that has olivine and low‐Ca pyroxene compositional ranges of Fa 5.2–5.8 and Fs 9.4–33.4, respectively. The dominant phase is low‐Ca pyroxene (36.3 vol%), followed by Fe‐Ni metal (16.3 vol%) and olivine (15.5 vol%); nevertheless, considering the Fe‐oxyhydroxide (due to terrestrial weathering), the original metal content was around 29.6 vol%. Finally, the mean oxygen isotopic composition Δ17O = +0.68‰ along with the occurrence of a silica phase, troilite, Ni‐rich phosphides, chromite, and oldhamite confirms that these ungrouped meteorites have been affected by strong reduction and are different from any other group recognized so far.
