This article presents an evaluation of the effectiveness of an applied mindfulness intervention for stress reduction delivered to Paralympic leaders prior to the Paralympic Games. OBJECTIVE: This study investigated the long-term outcomes of internet-delivered cognitive behavior therapy (ICBT) for children with anxiety disorders, and potential pre-treatment predictors of treatment outcome. METHOD: The sample included eighty-four children (8-12 years old) with anxiety disorders, from both a treatment group and a waitlist control (after participants had crossed over to treatment) of a previous randomized controlled study. Participants were assessed at post-treatment and three- and twelve-months after treatment using a semi-structured interview and parent ratings. Pre-treatment data were used to investigate predictors of treatment outcome at three-month follow-up. RESULTS: Intention-to-treat analysis showed that treatment gains were maintained at twelve-month follow-up, including clinician rated severity of the principal anxiety disorder, parent rated anxiety symptoms and global functioning, with mainly large effect sizes (Cohen's d = 0.63-2.35). Completer analyses showed that suspected autism spectrum disorder was associated with less change in symptom severity. No other pre-treatment measures significantly predicted treatment outcome. CONCLUSION: This study suggests that internet-delivered CBT can have long-term beneficial effects for children with anxiety disorders. Predictors of treatment outcome need to be evaluated further. The primary outcome measure was the Clinician Severity Rating (CSR) and secondary measures included child-and parent-reported anxiety. Assessments were made at pre-treatment, post-treatment and at three-month follow-up. Results: At post-treatment, there were significant reductions on CSR in the treatment group, with a large between-group effect size (Cohens d = 1.66). Twenty per cent of children in the treatment group no longer met criteria for their principal diagnosis at post-treatment and at follow-up this number had increased to 50%. Parent-reported child anxiety was significantly lower in the treatment group than in the waitlist group at post-treatment, with a small between-group effect size (Cohens d = 0.45). There were no significant differences between the groups regarding child-ratings of anxiety at post-treatment. Improvements were maintained at three-month follow-up, although this should be interpreted cautiously due to missing data. Among clinicians, it is common practice to include parents in treatment, and it has been taken for granted that parents' involvement in their children's treatment is beneficial for therapy outcome, although research on this issue is far from clear. A meta-analysis was carried out in order to investigate whether parent involvement potentiates the outcome for children with anxiety disorders when treated with cognitive-behavior therapy. Sixteen studies, which directly compared parent-involved treatments with child-only treatments, were included in the meta-analysis. The results showed a small, nonsignificant effect size of -0.10 in favor of the child-only treatments. There was no indication of publication bias in the analysis. Background Research has shown that computerized cognitive behavior therapy (cCBT), including internet-delivered CBT), can be effective. However, less is known about clinicians' attitudes towards this mode of treatment delivery. The aim of this study was to explore the attitudes of clinicians within child- and adolescent psychiatry towards cCBT and to explore if attitudes differed depending on rurality or theoretical orientation. Methods A random sample of Swedish child- and adolescent mental health services was selected for a survey study (N = 15). A total number of 156 surveys were collected. Results Results showed a generally low knowledge of cCBT. A majority of clinicians were positive to cCBT as a prevention program (73%), and as treatment for mild to moderate problems (75%). More caution was reported with regard to more severe mental health problems. Treatment orientation, but not rurality, had a significant effect on several ratings. Thematic analyses showed concerns regarding for example lack of human support and the restricted usefulness of cCBT regarding age and complexity of symptoms. Perceived advantages were for example increased availability and possibility of an alternative way of communication. Conclusions This study adds to the limited literature on attitudes towards cCBT. The emerging picture is of a mainly positive attitude and prerequisites for dissemination are promising.

Aims: An open trial was conducted to evaluate Internet-delivered CBT for children with specific phobia. Method: Children (N = 30) aged 8-12, and their parents, with a principal diagnosis of specific phobia were recruited through media advertisement. Participants received six weeks of Internet-delivered CBT with therapist support. The treatment was aimed for the parents and the children, with the first part being only for the parents. The primary outcome measure was the Clinician Severity Rating (CSR), and secondary measures included clinician-rated global functioning and child- and parent-reported anxiety and quality of life. All assessments were made at pretreatment, posttreatment, and three-month follow-up. Results: At posttreatment, there were significant reductions on the CSR, with a large within-group effect size (Cohen's d = 1.0) and 35% of children no longer meeting criteria for specific phobia. Aims: An open trial was conducted to evaluate Internet-delivered CBT for children with specific phobia. Method: Children (N=30) aged 8-12, and their parents, with a principal diagnosis of specific phobia were recruited through media advertisement. Participants received six weeks of Internet-delivered CBT with therapist support. The treatment was aimed for the parents and the children, with the first part being only for the parents. The primary outcome measure was the Clinician Severity Rating (CSR), and secondary measures included clinician-rated global functioning and child- and parent-reported anxiety and quality of life. All assessments were made at pretreatment, posttreatment, and three-month follow-up. Results: At posttreatment, there were significant reductions on the CSR, with a large within-group effect size (Cohen's d=1.0) and 35% of children no longer meeting criteria for specific phobia. Psychological treatments for Panic disorder with agoraphobia (PDA) currently havingempirical support are treatments including cognitive-behavioral interventions, e.g. cognitive therapy and exposure in-vivo. However, our knowledge about the additional efficacy when combining interventions is still limited. In study I, the contribution of cognitive therapy in the treatment of PDA-patients was investigated, by examining the relative efficacy of exposure in-vivo alone and exposure in combination with cognitive therapy. The addition of cognitive therapy was not found to enhance the treatment outcome effects for PDA-patients. Cognitive therapy is based on the cognitive models, which assume that panic attacks are caused by catastrophic misinterpretations(catastrophic cognitions) of bodily sensations. In study II, the cognitive reports of PDApatients when highly anxious were examined and the findings only partly supported the cognitive models for PDA. In study III, the congruence of the cognitive reports obtained with different measures for PDA-patients were examined. Poor congruence was found, and a higher frequency of cognitions in line with cognitive models was reported retrospectively than prospectively. In study IV, the memory bias for psychopathological relevant information in PDA-patients was examined. PDA-patients were found to have a memory bias towards faces rated as safe. The findings from study I-III in the present dissertation are in line with previous research, and consequently, the addition of cognitive therapy and the assumptions from the cognitive models have to be questioned.
