In view of this escalating prevalence, the World Health Organization (WHO) has launched the Global Diabetes Compact (GDC) campaign to improve the diagnosis and management of people with diabetes. To this end, a set of diabetes coverage targets, focusing on 80% of people living with diabetes, to be achieved by the year 2030, were defined at the 75th World Health Assembly for the first time in May this year. These targets aim to achieve not just glycemic control, but also blood pressure and lipid management at all levels of the health care system. India has the second largest number of people with diabetes in the globe. In this article, we have proposed the concept of “glycemic guardianship”, which means activities carried out by the health care team and the health care system in partnership with the patient to ensure optimal care of diabetes. Recognizing and acknowledging our role as “glycemic guardians” of the nation will automatically pave the way to realize the targets set by the GDC.

Pandemics are for the most part disease outbreaks that become widespread as a result of the spread of human-to-human infection. Beyond the debilitating, sometimes fatal, consequences for those directly affected, pandemics have a range of negative social, economic and political consequences. These tend to be greater where the pandemic is a novel pathogen, has a high mortality and/or hospitalization rate and is easily spread. According to Lee Jong-wook, former Director-General of the World Health Organization (WHO), pandemics do not respect international borders. Therefore, they have the potential to weaken many societies, political systems and economies simultaneously.

The new International Health Regulations adopted by the World Health Organization's annual assembly on May 23, 2005, will increase security against global epidemics of deadly diseases by improving national and international capacity for preventing and responding to disease outbreaks. The new regulations include comprehensive assessment, reporting, and response standards mandatory for each country and to be implemented within a specific timeframe; operational mechanisms; increased collaboration between countries' health offices and with the WHO; and a better coordinated international reporting and response system. The regulations stipulate the increased roles of countries and WHO in identifying, preventing, and responding to public health emergencies of international concern. WHO should be quickly informed of any outbreak of four diseases--SARS, bird flu, smallpox and polio--as well as any outbreaks of "potential international public health concern" from known or unknown causes or sources. The new regulations will formally come into force two years after approved by the Assembly. The new WHO regulations should be distributed to relevant military commands so they can determine the implications for their operations and potentials for collaboration with WHO country offices and the Global Outbreak Alert and Response Network (GOARN). Such reviews should also include the new matrix developed by WHO for helping countries identify whether new health incidents are of international concern.
