Background: Biopsychosocially informed education is associated with improved back pain beliefs and positive changes in health care practitioners’ practice behaviours. Objective: Assess the effect of this type of education for insurance workers who are important non-clinical stakeholders in the rehabilitation of injured workers. Methods: Insurance workers operating in the Western Australian workers’ compensation system underwent two, 1.5 hour sessions of biopsychosocially informed education focusing on understanding and identifying barriers to recovery of injured workers with musculoskeletal conditions. Back pain beliefs were assessed pre-education, immediately post-education and at three-month follow-up (n = 32). Self-reported and Injury Management Advisor-reported assessment of change in claims management behaviours were collected at the three-month follow-up. Results: There were positive changes in the Health Care Providers’ Pain and Impairment Relationship Scale (p = 0.009) and Back Beliefs Questionnaire (p = 0.049) immediately following the education that were sustained at three-month follow-up. Positive changes in claims management behaviours were supported by self-reported and Injury Management Advisor-reported data. Conclusion: This study provides preliminary support that a brief biopsychosocially informed education program can positively influence insurance workers’ beliefs regarding back pain, with concurrent positive changes in claims management behaviours. Further research is required to ascertain if these changes result in improved claims management outcomes.

Short-beaked common dolphins (Delphinus delphis) are typically considered highly mobile, offshore delphinids. This study assessed the residency of a small community of short-beaked common dolphins in the shallow, urbanized Port Phillip Bay, south-eastern Australia. The ability to identify common dolphins by their dorsal fin markings and coloration using photo-identification was also investigated. Systematic and non-systematic boat surveys were undertaken between 2007 and 2014. Results showed that 13 adult common dolphins and their offspring inhabit Port Phillip Bay, of which 10 adults exhibit residency to the bay. The majority of these adults are reproductively active females, suggesting that female philopatry may occur in the community. Systematic surveys conducted between 2012 and 2014 revealed that the dolphins were found in a median water depth of 16 m and median distance of 2.2 km from the coast. The shallow, urbanized habitat of this resident common dolphin community is atypical for this species. As a result, these common dolphins face threats usually associated with inshore bottlenose dolphin communities. We suggest that the Port Phillip Bay common dolphin community is considered and managed separate to those outside the embayment and offshore to ensure the community's long-term viability and residency in the bay.

Kinetochores, multi-subunit complexes that assemble at the interface with centromeres, bind spindle microtubules to ensure faithful delivery of chromosomes during cell division. The configuration and function of the kinetochore-centromere interface is poorly understood. We report that a protein at this interface, CENP-M, is structurally and evolutionarily related to small GTPases but is incapable of GTP-binding and conformational switching. We show that CENP-M is crucially required for the assembly and stability of a tetramer also comprising CENP-I, CENP-H, and CENP-K, the HIKM complex, which we extensively characterize through a combination of structural, biochemical, and cell biological approaches. A point mutant affecting the CENP-M/CENP-I interaction hampers kinetochore assembly and chromosome alignment and prevents kinetochore recruitment of the CENP-T/W complex, questioning a role of CENP-T/W as founder of an independent axis of kinetochore assembly. Our studies identify a single pathway having CENP-C as founder, and CENP-H/I/K/M and CENP-T/W as CENP-C-dependent followers.

Objective: The International Mission on Prognosis and Analysis of Clinical Trials and Corticoid Randomisation After Significant Head injury prognostic models predict outcome after traumatic brain injury but have not been compared in large datasets. The objective of this is study is to validate externally and compare the International Mission on Prognosis and Analysis of Clinical Trials and Corticoid Randomisation after Significant Head injury prognostic models for prediction of outcome after moderate or severe traumatic brain injury. Design: External validation study. Patients: We considered five new datasets with a total of 9,036 patients, comprising three randomized trials and two observational series, containing prospectively collected individual traumatic brain injury patient data. Measurements and Main Results: Outcomes were mortality and unfavorable outcome, based on the Glasgow Outcome Score at 6 months after injury. To assess performance, we studied the discrimination of the models (by area under the receiver operating characteristic curves), and calibration (by comparison of the mean observed to predicted outcomes and calibration slopes). The highest discrimination was found in the Trauma Audit and Research Network trauma registry (area under the receiver operating characteristic curves between 0.83 and 0.87), and the lowest discrimination in the Pharmos trial (area under the receiver operating characteristic curves between 0.65 and 0.71). Although differences in predictor effects between development and validation populations were found (calibration slopes varying between 0.58 and 1.53), the differences in discrimination were largely explained by differences in case mix in the validation studies. Calibration was good, the fraction of observed outcomes generally agreed well with the mean predicted outcome. Background. As the proportion of survivors from breast cancer increases it is possible that follow-up care could be delivered wholly by generalists to relieve over subscribed hospital clinics. However, guidelines seldom take into account the nature of interactions between patients and health care professionals involved in hospital-based follow-up. Methods. Consultations between four Specialist Breast Nurses (SBNs) and 21 consecutive women attending a hospital-based breast cancer follow-up clinic in Western Australia were audio recorded and subjected to a thematic analysis. Recording of consultations ceased with saturation of themes. We analysed the data with reference to theoretical frameworks which postulate that social support is a powerful factor in determining positive health outcomes. We also drew on theories focusing on biographical disruption, biographical reinforcement and biographical reinvention. Results. The majority of participants were Australian born, married women in their sixties. The mean duration of the consultations was 19 min (SD = 7.5, min = 8, max = 43.5). A core theme was the established relationship between the woman and her SBN. Overall, the SBNs played an important role in facilitating the transition of patients by supporting the woman in adjustment to a new self-image and bodily functioning. The SBN accompanies each woman through this phase in her life, while supporting a new narrative, promoting her ‘rebirth’ as someone with ideas, concerns and expectations that have altered significantly after the diagnosis of cancer. Five key themes emerged to demonstrate this supportive role: normalising; facilitating access to services; prevention; promoting self-esteem and promoting a proactive approach. Conclusions. Many women with breast cancer claimed a new perspective on what was now possible, acceptable or desirable in a host of life domains. All rights reserved.Background: Although clinicians in both primary and tertiary care settings are involved in the care of breast cancer patients following the active treatment phase, few studies report how patients interact with health care providers.Methods: Participants in this breast cancer follow-up study were recruited from a hospital based nurse-led follow-up clinic in Western Australia. Methods included audio taped, transcribed consultations with Specialist Breast Nurses (SBNs) and patient self-completed surveys.Results: Preliminary data suggest that SBNs play an important role in supporting women to deal with the impact of breast cancer in the years following active treatment. The data suggest that the process of adjustment to a diagnosis of cancer continues for many years after the treatment has ceased. In many cases the women require on-going support to recalibrate their response to normal physical changes that may or may not be a consequence of surgery, radiotherapy and chemotherapy.Conclusions: These preliminary data map the plethora of issues that influence cancer patients in the years following treatment. Women who were attending follow-up appointments for breast cancer experienced similar levels of enablement following SBN consultations as would be expected from consultations with general practitioners.

The U orebody at the Beverley mine in South Australia occurs in permeable sands within the confined, highly saline Beverley aquifer, about 125 m below surface, and the U is mined using in situ recovery (ISR) technology. The differences are discussed between ISR operations and more traditional U production methods such as agitation leaching and heap leaching, where the ore is removed from the deposit, stockpiled, crushed, milled and leached on the surface. Solids tailings from either the mill or the heap are finally deposited in a tailing storage facility. In ISR mining the ore remains in place underground and the groundwater in the ore-bearing aquifer is used as the mining fluid. This is achieved by drilling into the deposit and installing wells for extracting and re- injecting the groundwater which is fortified with chemicals. The fortified groundwater flows through the permeable sands containing the orebody, dissolving the U. The pregnant mining fluid is then collected and transferred to the recovery plant where the U is captured on an ion exchange resin. The barren mining fluid is re-fortified and recycled to the well fields. Once mining is completed in a well field, the surface infrastructure is relocated to a new well field, and the mined-out well field is rehabilitated to its original surface condition. The often-told tale of Zulu King Shaka and his diviners may tell us more about colonial religion and politics in the nineteenth century than it does about Zulu religion and politics. The tale unfolds in various nineteenth-century books, popular and contemporary histories, and in oral traditions. Writers have frequently portrayed it as a key point in King Shaka’s leadership, most especially E A Ritter in the 1950s, some 100 years after the earliest accounts. King Shaka is said to have secretly sprinkled blood in the doorway of his isigodlo (pl. izigodlo, the king’s private enclosure consisting of the huts of his women and children), then ‘innocently’ proclaimed it to be an evil omen, and called on his diviners to ‘smell out’, or reveal, the culprit/s. Most of the numerous references to the tale explain it as a test devised by Shaka to expose diviners as frauds. This chapter examines various accounts of the story, including similar tales relating to other chiefs, and highlights discrepancies between them. The main players are usually Shaka and the diviners, and they are wholly African. The chapter also adds other dimensions, notably the colonial role in manufacturing some of these accounts, and the significance of the relationship between African politics and religion.

Over the last 40 years the reputation of Theophilus Shepstone has grown and grown. In his own time, most white settlers in Natal denounced him as the man who had locked up the land and the African labour they coveted. Most, though not all, missionaries opposed his toleration of African customs such as polygyny, bride wealth and beer brewing. After his death, historians tended to endorse the opinion of the Colonial Office that having served as a gifted colonial administrator in Natal, he brought about his own downfall after being promoted beyond his competence as the first Administrator of the British Transvaal in South Africa following its annexation in 1877. Others took a more sinister view. C J Uys, an Afrikaner nationalist, blamed Shepstone for subjecting a pioneering Boer republic to imperialist oppression. The crusading daughters (Frances, Harriette and Agnes) of Bishop J W Colenso blamed him and his sons for extinguishing the Zulu monarchy and running an unjust system of African administration. Shepstone’s handling of the Langalibalele affair, and particularly the chief’s trial in 1874, angered Colenso and caused a breakdown in a long friendship between him, his family and Shepstone. In a letter to her brother Charles in 1874, Frances Colenso wrote, ‘as to Mr Shepstone, as soon as John found the line he was taking, he says in this case “it must be war to the knife between us”, and he has not been to his house since, though of course they salute in public’.1 With the publication of David Welsh’s Origins of Segregation in 1968 different views began to surface. Welsh suggested that Shepstone deserved whatever credit or opprobrium might be due for laying the foundations of segregation based on culture, which underpinned the development of the twentieth-century apartheidregime. The focus on women’s leadership and chiefly women in this chapter aims to explore some of the political, social, religious and economic dimensions that shaped leadership that has been largely neglected in the history of leadership in southern Africa. Exceptions include Hamilton (1985) and Hanretta’s (1998) work (see later). Hamilton challenges the view of women in the Zulu state as a homogenous group marked by universal subordination. For the most part, however, the Zulu past, in particular, has predominantly been written with an emphasis on masculine militarism that overrides and suppresses the political and militarised activities of certain women.

uNkulunkulu was a term taken up by certain missionaries in Natal as synonymous with the Christian God. Though the idea of uNkulunkulu is now well entrenched in African as well as missionary Christian theology, historically the concept of uNkulunkulu, as the High God of all, is inaccurate. This paper will argue that there was actually a multiplicity of oNkulunkulu (plural of uNkulunkulu) in the early nineteenth century - including females. uNkulunkulu was simply a generic name for particular significant Zulu ancestors' family, chiefly or 'national'. The development of the concept of uNkulunkulu, as the High God of all, obscures important aspects of the relationship that formerly obtained between chiefs and their departed ancestors. The attainment of Zulu political ascendancy, which has so often been viewed in purely secular terms, had a critical religious dimension.

The worldwide transformation of higher education (HE) towards the end of the 20th century has resulted in an increasingly globalised and competitive HE environment, leading to further diversification across the sector and an expansion in the provision of educational programmes across national boundaries. The use of new information and communication technologies in particular is driving a sharp increase in the delivery of trans-national education and makes the notion of ‘borderless’ education a reality. However, the delivery of online or ‘offshore’ educational programmes raises important questions about quality assurance both from the perspective of institutions providing these academic programmes nationally, as well as the students enrolled in these programme. Driving the push towards international quality assurance is both current and projected enrolment by students in universities outside their own countries. Their number was estimated to be 1.4 million in 1992, 1.8 million in 2000, and has been forecast to grow to 7.2 million students by 2025. The issue of how these programme should be monitored and who has responsibility for determining the quality of offshore providers is not clear in what has become an increasingly complex higher education environment. It would appear that the regulation of educational programmes delivered across national borders is lagging well behind its rapid expansion and there is an emerging need to determine how best to ensure the quality of these programmes. It is in this context that we explore some of the issues involved in assuring the quality of university programmes offered by Australian universities to students in African countries and discuss some institutional and national responses to the challenges posed by transnational education.

This paper undertakes an exploration of quality reviews, focusing on the tension between central organisation and ownership at the periphery. At Monash, area reviews form a major component of the evaluation moment in the quality management system of planning, acting, evaluating and improving at the organizational level and throughout all levels of the university. All areas, including support services, faculties and research centres are subject to review. Similarly, at the University of Natal in South Africa, cyclical reviews are required for all areas of university activity. The models of implementation, are, however, very different. The Centre for Higher Education Quality (CHEQ) is responsible for leading and supporting driving quality within Monash University, but devolves the responsibility for organising and conducting the reviews resides with to the area concerned. On the other hand, the Quality Promotion Unit at the University of Natal has a comparatively greater role in running the reviews concerned. In both instances the Units are busy reflecting on their processes on the basis of their experiences and feedback from the areas concerned. In this paper we examine and compare the approaches so that the resulting lessons from these experiences will be of use to other universities undertaking reviews in support services and faculties. We focus particularly on the issue of how much involvement is optimal and what sort of support is needed for areas conducting reviews.

Chemical warfare agents were demolished by US soldiers at Khamisiyah, Iraq, in March 1991. The authors investigated postwar morbidity for Gulf War veterans, contrasting those who may have been exposed to low gaseous levels of nerve agents and those unlikely to have been exposed. Cox regression modeling was performed for hospitalizations from all causes and hospitalizations from diagnoses within 15 categories during the period March 10, 1991, through December 31, 2000, for the duration of active duty status. After adjustment for all variables in the model, only two of 37 models suggested that personnel possibly exposed to subclinical doses of nerve agents might be at increased risk for hospitalization from circulatory' diseases; specifically cardiac dysrhythmias. Of the 724 hospitalizations for cardiac dysrhythmias, 203 were in the potentially exposed group, slightly higher than expected (risk ratio = 1.23, 95% confidence interval: 1.04,1.44). The increase was small in comparison with potential observational variability, but the findings are provocative and warrant further evaluation. Veterans possibly exposed to nerve agents released by the Khamisiyah demolition were not found to be at increased risk for hospitalizations from any other chronic diseases nearly 10 years after the Gulf War.

A small cross-cut sampler is then used as the final stage to cut sample into a bucket to produce a composite sample. The units provide better sampling accuracy and easier maintenance than traditional methods, resulting in better long-term performance. They are inexpensive to install, easy to maintain and have low long-term ownership costs. Se revisaron 181.270 fichas de los archivos del Instituto Hematológico de Occidente, Edo. Zulia, Venezuela, procedentes de los diferentes Distritos con el fin de realizar un estudio sobre infección chagásica en donantes de sangre por un período de 9 años (1980 -1988). El método serológico utilizado fue la prueba de Machado-Guerreiro, obteniéndose 2829 casos positivos (1.56%).Al evaluar los resultados (afios 1980 -1988), se aprecia que los mayores porcentajes de Infección Chagásica se presentaron en los Distritos Baralt (10.16%), Sucre (4.56%) y Bolívar II (4.01%) y el año más afectado fue 1980 (19.44%). Nuestro trabajo señala la importancia que reviste la determinación de anticuerpos anti Trypanosoma cruzi en donantes de sangre y en esta forma controlar la posibilidad de transmisión transfusional.Â

Iron has been found in all of the major tissues examined in the chiton Clavarizona hirtosa. By far the highest concentrations were found in the radula and its associated structures. 2. 2. The major iron binding protein in each tissue has been isolated and characterised using molecular exclusion chromatography, PAGE, isoelectric focusing and antigenic specificity, and in all cases was found to be ferritin. 3. 3. Little evidence of heterogeneity in ferritin type was found between the various tissues examined. However, ferritin isolated from the hemolymph displayed different characteristics from tissue ferritins both on PAGE and isoelectric focusing.

Three species of epiphytic Dischidia have been investigated in terms of their relationship to ants on trees. Two species, D. parvifolia and D. astephana, are associated with ants and trees in montane areas. A clear association has been found between ants of the genus Crematogaster and the tree Leptospermum flavesens. This relationship is complex and probably both organisms benefit from the association. The ants live in tunnels in the wood of the major branches and the trunk, and the entire tree is occupied by one ant colony. Trees occupied by ants are maintained by the ants substantially clear of epiphytes other than the two species of Dischidia. The potential benefits to the tree and to the ants of this association are noted. The roots of D. astephana and D. parvifolia penetrate into the cavities of these ant nests and presumably gain nutrients from waste in the ant nests. Both Dischidia species are effectively scavenging upon the waste material from the ant-tree association. The leathery dome-shaped leaves of D. astephana are not vital to the development of the scavenging habit as D. parvifolia has lens-shaped leaves, but may offer some advantage to D. astephana by the uptake of nutrients from waste deposited by the ants under the dome-shaped leaves by interception of stem flow and by uptake of gaseous waste. Ants do not nest under these leaves. Seeds of these species of Dischidia are taken by ants into the central woody area of the ant nest where they germinate. Both Leptospermum and Dischidia can be visualized as showing adaptations to a nutient-deficient tropical montane environment. These adaptations are discussed as is the need for reassessment in this genus of the term 'ant-plant', and the need for wider recognition of the 'ant-tree' relationship between Crematogaster and Leptospermum.
