Abstract: BACKGROUND Among patients with resectable early-stage non-small-cell lung cancer (NSCLC), a perioperative approach that includes both neoadjuvant and adjuvant immune checkpoint inhibition may provide benefit beyond either approach alone. METHODS We conducted a randomized, double-blind, phase 3 trial to evaluate perioperative pembrolizumab in patients with early-stage NSCLC. Participants with resectable stage II, IIIA, or IIIB (N2 stage) NSCLC were assigned in a 1:1 ratio to receive neoadjuvant pembrolizumab (200 mg) or placebo once every 3 weeks, each of which was given with cisplatin-based chemotherapy for 4 cycles, followed by surgery and adjuvant pembrolizumab (200 mg) or placebo once every 3 weeks for up to 13 cycles. The dual primary end points were event-free survival (the time from randomization to the first occurrence of local progression that precluded the planned surgery, unresectable tumor, progression or recurrence, or death) and overall survival. Secondary end points included major pathological response, pathological complete response, and safety. RESULTS A total of 397 participants were assigned to the pembrolizumab group, and 400 to the placebo group. At the prespecified first interim analysis, the median follow-up was 25.2 months. Event-free survival at 24 months was 62.4% in the pembrolizumab group and 40.6% in the placebo group (hazard ratio for progression, recurrence, or death, 0.58; 95% confidence interval [CI], 0.46 to 0.72; P<0.001). The estimated 24-month overall survival was 80.9% in the pembrolizumab group and 77.6% in the placebo group (P = 0.02, which did not meet the significance criterion). A major pathological response occurred in 30.2% of the participants in the pembrolizumab group and in 11.0% of those in the placebo group (difference, 19.2 percentage points; 95% CI, 13.9 to 24.7; P<0.0001; threshold, P = 0.0001), and a pathological complete response occurred in 18.1% and 4.0%, respectively (difference, 14.2 percentage points

Abstract: OBJECTIVE: The soluble guanylate cyclase stimulator riociguat is approved for the treatment of adult patients with pulmonary arterial hypertension (PAH) and inoperable or persistent/recurrent chronic thromboembolic pulmonary hypertension (CTEPH) following Phase 3 randomized trials. The EXPosurE Registry RiociguaT in patients with pulmonary hypertension (EXPERT) study was designed to monitor the long-term safety of riociguat in clinical practice. METHODS: EXPERT was an international, multicenter, prospective, uncontrolled, non-interventional cohort study of patients treated with riociguat. Patients were followed for at least 1 year and up to 4 years from enrollment or until 30 days after stopping riociguat treatment. Primary safety outcomes were adverse events (AEs) and serious adverse events (SAEs) coded using Medical Dictionary for Regulatory Activities preferred terms and System Organ Classes version 21.0, collected during routine clinic visits and collated via case report forms. RESULTS: In total, 956 patients with CTEPH were included in the analysis. The most common AEs in these patients were peripheral edema/edema (11.7%), dizziness (7.5%), right ventricular (RV)/cardiac failure (7.7%), and pneumonia (5.0%). The most common SAEs were RV/cardiac failure (7.4%), pneumonia (4.1%), dyspnea (3.6%), and syncope (2.5%). Exposure-adjusted rates of hemoptysis/pulmonary hemorrhage and hypotension were low and comparable to those in the long-term extension study of riociguat (Chronic Thromboembolic Pulmonary Hypertension Soluble Guanylate Cyclase-Stimulator Trial [CHEST-2]). CONCLUSION: Data from EXPERT show that in patients with CTEPH, the safety of riociguat in routine practice was consistent with the known safety profile of the drug, and no new safety concerns were identified.

Abstract: Objective: The soluble guanylate cyclase stimulator riociguat is approved for the treatment of adult patients with pulmonary arterial hypertension (PAH) and inoperable or persistent/recurrent chronic thromboembolic pulmonary hypertension (CTEPH) following Phase 3 randomized trials. The EXPosurE Registry RiociguaT in patients with pulmonary hypertension (EXPERT) study was designed to monitor the long-term safety of riociguat in clinical practice. Methods: EXPERT was an international, multicenter, prospective, uncontrolled, non-interventional cohort study of patients treated with riociguat. Patients were followed for at least 1 year and up to 4 years from enrollment or until 30 days after stopping riociguat treatment. Primary safety outcomes were adverse events (AEs) and serious adverse events (SAEs) coded using Medical Dictionary for Regulatory Activities preferred terms and System Organ Classes version 21.0, collected during routine clinic visits and collated via case report forms. Results: In total, 956 patients with CTEPH were included in the analysis. The most common AEs in these patients were peripheral edema/edema (11.7%), dizziness (7.5%), right ventricular (RV)/cardiac failure (7.7%), and pneumonia (5.0%). The most common SAEs were RV/cardiac failure (7.4%), pneumonia (4.1%), dyspnea (3.6%), and syncope (2.5%). Exposure-adjusted rates of hemoptysis/pulmonary hemorrhage and hypotension were low and comparable to those in the long-term extension study of riociguat (Chronic Thromboembolic Pulmonary Hypertension Soluble Guanylate Cyclase–Stimulator Trial [CHEST-2]). Conclusion: Data from EXPERT show that in patients with CTEPH, the safety of riociguat in routine practice was consistent with the known safety profile of the drug, and no new safety concerns were identified.

13. Tobacco-smoking induced GPR15-expressing T cells in blood do not indicate pulmonary damage

Abstract: Background Recently, it was shown that chronic tobacco smoking evokes specific cellular and molecular changes in white blood cells by an excess of G protein-coupled receptor 15 (GPR15)-expressing T cells as well as a hypomethylation at DNA CpG site cg05575921 in granulocytes. In the present study, we aimed to clarify the general usefulness of these two biomarkers as putative signs of non-cancerous change in homeostasis of the lungs. Methods In a clinical cohort consisting of 42 patients with chronic obstructive pulmonary disease (COPD), interstitial lung disease (ILD) and pneumonia and a control cohort of 123 volunteers, the content of GPR15-expressing blood cells as well as the degree of methylation at cg05575921 were analysed by flow-cytometry and pyrosequencing, respectively. Smoking behaviour was estimated by questionnaire and cotinine level in plasma. Results Never-smoking patients could be distinguished from former and current smokers by both the proportion of GPR15-expressing T cells as well as cg05575921 methylation in granulocytes, with 100% and 97% specificity and 100% sensitivity, respectively. However, both parameters were not affected by lung diseases. The degrees of both parameters were not changed neither in non-smoking nor smoking patients, compared to appropriate control cohorts of volunteers. Conclusions The degree of GPR15-expressing cells among T cells as well as the methylation at cg05575921 in granulocytes in blood are both rather signs of tobacco-smoking induced systemic inflammation because they don’t indicate specifically non-cancerous pathological changes in the lungs.

Abstract: Lung cancer is a frequently occurring disease, particularly in the elderly; however, within the last 10 years the pharmaceutical treatment of lung cancer has been significantly improved. Due to a better understanding of the pathophysiological events and the identification of molecular subgroups of lung tumors, new therapeutic drugs have been developed that significantly prolong survival of patients with the respective molecular pattern. In particular immunotherapeutic agents, such as programmed death-ligand 1 (PD-L1) and programmed death 1 (PD1) antibodies have shown promising clinical results in a subgroup of lung cancer patients. Due to the high incidence of both lung cancer and rheumatic diseases they often occur together, which necessitates an interdisciplinary management. The success of improved therapy of lung cancer has led to a greater focus on the treatment of comorbidities; however, interventions into the immune system by immune checkpoint inhibitors can lead to new challenges when an autoimmune disease is simultaneously present. The possibility of an effective screening for lung cancer in the future also presents the prospect of an improvement in mortality, which raises the question of the optimal monitoring of patients with rheumatoid arthritis (RA) under immunosuppressive therapy. The aim of this review is to discuss the interaction between lung cancer and RA with respect to the currently available data.

Abstract: We have measured the $\pi^+\to {\rm e}^+\nu\gamma$ branching ratio over a wide region of phase space, based on a total of 65,460 events acquired using the PIBETA detector. Minimum-$\chi^2$ fits to the measured $(E_{e^+},E_\gamma)$ energy distributions result in the weak form factor value of $F_A=0.0119(1)$ with a fixed value of $F_V=0.0259$. An unconstrained fit yields $F_V=0.0258(17)$ and $F_A=0.0117(17)$. In addition, we have measured $a=0.10(6)$ for the dependence of $F_V$ on $q^2$, the ${\rm e}^{+}\nu$ pair invariant mass squared, parametrized as $F_V(q^2)=F_V(0)(1+a\cdot q^2)$. The branching ratio for the kinematic region $E_\gamma > 10 $MeV and $\theta_{{\rm e^+}\gamma} > 40^\circ $ is measured to be $B^{\rm exp}=73.86(54) \times 10^{-8}$. Earlier deviations we reported in the high-$E_\gamma$/low-$E_{{\rm e}^+}$ kinematic region are resolved, and we find full compatibility with CVC and standard $V$$-$$A$ calculations without a tensor term. We also derive new values for the pion polarizability, $\alpha_E = \rm 2.78(10) \times 10^{-4} fm^3$, and neutral pion lifetime, $\tau_{\pi 0} = (8.5 \pm 1.1) \times 10^{-17} $s., Comment: 4 pages, 2 PDF figures

Abstract: We have measured the $\pi^+\to {\rm e}^+\nu\gamma$ branching ratio over a wide region of phase space, based on a total of 65,460 events acquired using the PIBETA detector. Minimum-$\chi^2$ fits to the measured $(E_{e^+},E_\gamma)$ energy distributions result in the weak form factor value of $F_A=0.0119(1)$ with a fixed value of $F_V=0.0259$. An unconstrained fit yields $F_V=0.0258(17)$ and $F_A=0.0117(17)$. In addition, we have measured $a=0.10(6)$ for the dependence of $F_V$ on $q^2$, the ${\rm e}^{+}\nu$ pair invariant mass squared, parametrized as $F_V(q^2)=F_V(0)(1+a\cdot q^2)$. The branching ratio for the kinematic region $E_\gamma > 10 $MeV and $\theta_{{\rm e^+}\gamma} > 40^\circ $ is measured to be $B^{\rm exp}=73.86(54) \times 10^{-8}$. Earlier deviations we reported in the high-$E_\gamma$/low-$E_{{\rm e}^+}$ kinematic region are resolved, and we find full compatibility with CVC and standard $V$$-$$A$ calculations without a tensor term. We also derive new values for the pion polarizability, $\alpha_E = \rm 2.78(10) \times 10^{-4} fm^3$, and neutral pion lifetime, $\tau_{\pi 0} = (8.5 \pm 1.1) \times 10^{-17} $s., Comment: 4 pages, 2 PDF figures

Abstract: Using a large acceptance calorimeter and a stopped pion beam we have made a precise measurement of the rare Pi+ -> Pi0 e+ Nu,(pi_beta) decay branching ratio. We have evaluated the branching ratio by normalizing the number of observed pi_beta decays to the number of observed Pi+ -> e+ Nu, (pi_{e2}) decays. We find the value of Gamma(Pi+ -> Pi0 e+ Nu)/Gamma(total) = [1.036 +/- 0.004(stat.) +/- 0.004(syst.) +/- 0.003(pi_{e2})] x 10^{-8}$, where the first uncertainty is statistical, the second systematic, and the third is the pi_{e2} branching ratio uncertainty. Based on Dalitz distributions of 42,209 events we have evaluated absolute Pi -> e nu gamma branching ratios in the three regions. Abstract: We describe the design, construction and performance of the PIBETA detector built for the precise measurement of the branching ratio of pion beta decay, pi+ -> pi0 e+ nu, at the Paul Scherrer Institute. The central part of the detector is a 240-module spherical pure CsI calorimeter covering 3*pi sr solid angle. The calorimeter is supplemented with an active collimator/beam degrader system, an active segmented plastic target, a pair of low-mass cylindrical wire chambers and a 20-element cylindrical plastic scintillator hodoscope. The whole detector system is housed inside a temperature-controlled lead brick enclosure which in turn is lined with cosmic muon plastic veto counters. Commissioning and calibration data were taken during two three-month beam periods in 1999/2000 with pi+ stopping rates between 1.3*E3 pi+/s and 1.3*E6 pi+/s. We examine the timing, energy and angular detector resolution for photons, positrons and protons in the energy range of 5-150 MeV, as well as the response of the detector to cosmic muons. Abstract: We have measured scintillation properties of pure CsI crystals used in the shower calorimeter built for a precise determination of the pi+ -> pi0 e+ nu decay rate at the Paul Scherrer Institute (PSI). All 240 individual crystals painted with a special wavelength-shifting solution were examined in a custom-build detection apparatus (RASTA=radioactive source tomography apparatus) that uses a 137Cs radioactive gamma source, cosmic muons and a light emitting diode as complementary probes of the scintillator light response. We have extracted the total light output, axial light collection nonuniformities and timing responses of the individual CsI crystals. These results predict improved performance of the 3 pi sr PIBETA calorimeter due to the painted lateral surfaces of 240 CsI crystals. The wavelength-shifting paint treatment did not affect appreciably the total light output and timing resolution of our crystal sample. The predicted energy resolution for positrons and photons in the energy range of 10-100 MeV was nevertheless improved due to the more favorable axial light collection probability variation. Abstract: We have measured scintillation properties of pure CsI crystals used in the shower calorimeter built for a precise determination of the pi+ -> pi0 e+ nu decay rate at the Paul Scherrer Institute (PSI). All 240 individual crystals painted with a special wavelength-shifting solution were examined in a custom-build detection apparatus (RASTA=radioactive source tomography apparatus) that uses a 137Cs radioactive gamma source, cosmic muons and a light emitting diode as complementary probes of the scintillator light response. We have extracted the total light output, axial light collection nonuniformities and timing responses of the individual CsI crystals. These results predict improved performance of the 3 pi sr PIBETA calorimeter due to the painted lateral surfaces of 240 CsI crystals. The wavelength-shifting paint treatment did not affect appreciably the total light output and timing resolution of our crystal sample. The predicted energy resolution for positrons and photons in the energy range of 10-100 MeV was nevertheless improved due to the more favorable axial light collection probability variation. Abstract: A new upper limit for the probability of spontaneous muonium to antimuonium conversion was established at ${\rm P_{M\bar{M}}} \leq 8.2 \cdot 10^{-11}$ (90%C.L.) in 0.1~T magnetic field, which implies consequences for speculative extensions to the standard model. Coupling parameters in R-parity violating supersymmetry and the mass of a flavour diagonal bileptonic gauge boson can be significantly restricted. A Z$_8$ model with radiative mass generation through heavy lepton seed and the minimal version of 331-GUT models are ruled out.
