Abstract

The Laser Interferometer Space Antenna (LISA) has the potential to reveal wonders about the fundamental theory of nature at play in the extreme gravity regime, where the gravitational interaction is both strong and dynamical. In this white paper, the Fundamental Physics Working Group of the LISA Consortium summarizes the current topics in fundamental physics where LISA observations of gravitational waves can be expected to provide key input. We provide the briefest of reviews to then delineate avenues for future research directions and to discuss connections between this working group, other working groups and the consortium work package teams. These connections must be developed for LISA to live up to its science potential in these areas.

Abstract

In this paper, which is of programmatic rather than quantitative nature, we aim to further delineate and sharpen the future potential of the LISA mission in the area of fundamental physics. Given the very broad range of topics that might be relevant to LISA, we present here a sample of what we view as particularly promising directions, based in part on the current research interests of the LISA scientific community in the area of fundamental physics. We organize these directions through a "science-first" approach that allows us to classify how LISA data can inform theoretical physics in a variety of areas. For each of these theoretical physics classes, we identify the sources that are currently expected to provide the principal contribution to our knowledge, and the areas that need further development. The classification presented here should not be thought of as cast in stone, but rather as a fluid framework that is amenable to change with the flow of new insights in theoretical physics.

3. Understanding the toolbox: A mixed methods study of attitudes, barriers and facilitators in parental intervention of children's cancer pain at home

Abstract

Children with cancer experience pain throughout their cancer trajectory. Pain has short- and long-term negative consequences for children physically and psychologically. Children with cancer spend more time at home with their families and less time in hospital. While this has benefits for quality of life, it shifts responsibility for pain management from healthcare professionals to parents. Little is known about parents' pain management abilities in this setting. This study aimed to understand how parents of children with cancer manage their child's pain at home. A convergent, parallel, mixed methods design including pain diaries, surveys and interviews was used. Participants were parents of children with cancer on active treatment recruited from one tertiary cancer centre. Each data collection method was analysed separately and then integrated. Parents frequently under-medicate their child's pain at home. Practical barriers including the analgesic context and children finding medications unpalatable led parents to prefer non-pharmacological interventions. Attitudinal and practical barriers result in parents having an "empty toolbox" of pharmacological interventions. Consequently non-pharmacological interventions are essential to parents managing their child's cancer pain at home.

Abstract

In this paper, which is of programmatic rather than quantitative nature, we aim to further delineate and sharpen the future potential of the LISA mission in the area of fundamental physics. Given the very broad range of topics that might be relevant to LISA,we present here a sample of what we view as particularly promising fundamental physics directions. We organize these directions through a “science-first” approach that allows us to classify how LISA data can inform theoretical physics in a variety of areas. For each of these theoretical physics classes, we identify the sources that are currently expected to provide the principal contribution to our knowledge, and the areas that need further development. The classification presented here should not be thought of as cast in stone, but rather as a fluid framework that is amenable to change with the flow of new insights in theoretical physics.

8. Pre-Existing Mental Disorder, Clinical Profile, Inpatient Services, And Costs in People Hospitalised Following Traumatic Spinal Injury: A Population Record-Linkage Study

Abstract

IntroductionRisk of traumatic injury is increased in individuals with mental illness, substance use disorder and dual diagnosis (mental disorders); these conditions will pre-exist among individuals hospitalised with acute traumatic spinal injury (TSI). Although early intervention can improve outcomes for people who experience mental disorders or TSI, the incidence, management, and cost of this often complex comorbid health profile is not sufficiently understood. Objectives and ApproachIn a whole-population cohort of patients hospitalised with acute TSI, we aimed to describe the prevalence of pre-existing mental disorders, and compare differences in injury epidemiology, costs and inpatient allied health service access. Record-linkage study of all hospitalised cases of TSI between June 2013 and June 2016 in New South Wales, Australia. TSI was defined by specific ICD-10-AM codes. Mental disorder status was considered as pre-existing where specific ICD-10-AM codes were recorded in incident admissions. Results13,489 individuals sustained acute TSI during this study. 13.11%, 6.06%, and 1.82% had pre-existing mental illness, substance use disorder, and dual diagnosis, respectively. Individuals with mental disorder were older (p<0.001), more likely to have had a fall or self-harmed (p<0.001), experienced almost twice the length of stay and inpatient complications, and increased injury severity compared to individuals without mental disorder (p<0.001). Conclusion / ImplicationsIndividuals hospitalised for TSI with pre-existing mental disorder have greater likelihood of increased injury severity and more complex, costly acute care admissions compared to individuals without mental disorder. Care pathway optimisation including prevention of hospital acquired complications for people with pre-existing mental disorders hospitalised for TSI is warranted.

9. Manifestation and parental assessment of children's cancer pain at home: An exploratory mixed-methods study

Abstract

AIMS AND OBJECTIVES: To describe pain manifestation in children with cancer at home and understand how parents assess this pain. BACKGROUND: Pain is experienced by children with cancer throughout their cancer journey. Short-term, and into survivorship, pain has negative physical and psychological consequences. Changes in treatment location mean children with cancer spend more time at home. Little is known about pain experienced by children at home or how parents assess this pain. DESIGN: A mixed-methods convergent parallel study was reported using STROBE. METHOD: Parents of children with cancer on active treatment were recruited from one tertiary cancer centre. Parental attitudes towards pain expression were assessed using surveys. Parents recorded their child's pain manifestation in pain diaries kept for one month. Interviews captured a deeper understanding of pain manifestation and how parents assess this pain at home. Integration occurred after each data collection method was analysed separately. RESULTS: Predominantly children were not in pain at home. However, most children experienced at least one episode of problematic pain over the pain diary period. Surveys showed parents held misconceptions regarding children's pain expression. Interviews diverge from surveys and suggest parents used a range of information sources to assess pain. CONCLUSION: Children with cancer may differ from one another in the manifestation of pain at home resulting in multiple pain trajectories. Parents of children with cancer are able to adequately assess their child's pain using information from multiple sources. RELEVANCE TO CLINICAL PRACTICE: It is not currently possible to predict which children will experience problematic pain at home, so all parents require pain management education prior to discharge. Abstract

Aim: To explore the differences in perceived patient safety culture in cancer nurses working in Estonia, Germany, the Netherlands, and the United Kingdom. Design: An exploratory cross‐sectional survey. Methods: In 2018, 393 cancer nurses completed the 12 dimensions of the Hospital Survey on Patient Safety Culture. Results: The mean score for the overall patient safety grade was 61.3. The highest rated dimension was “teamwork within units” while “staffing” was the lowest in all four coun‐ tries. Nurses in the Netherlands and in the United Kingdom, scored higher on “com‐ munication openness”, the “frequency of events reported”, and “non‐punitive response to errors”, than nurses from Estonia or Germany. We found statistically significant dif‐ ferences between the countries for the association between five of the 12 dimensions with the overall patient safety grade: overall perception of patient safety, communica‐ tion openness, staffing, handoffs and transitions and non‐punitive response to errors. Conclusion: Patient safety culture, as reported by cancer nurses, varies between European countries and contextual factors, such as recognition of the nursing role and education have an impact on it. Cancer nurses’ role in promoting patient safety is a key concern and requires better recognition on a European and global level. Impact: Cancer Nursing Societies in any country can use these data as an indication on how to improve patient care in their country. Recognition of cancer nursing as a distinct specialty in nursing will help to improve patient safety.

Abstract

Background: Advances in research and technology coupled with an increased cancer incidence and prevalence have resulted in significant expansion of cancer nurse role, in order to meet the growing demands and expectations of people affected by cancer (PABC). Cancer nurses are also tasked with delivering an increasing number of complex interventions as a result of ongoing clinical trials in cancer research. However much of this innovation is undocumented, and we have little insight about the nature of novel interventions currently being designed or delivered by cancer nurses. Objectives: To identify and synthesise the available evidence from clinical trials on interventions delivered or facilitated by cancer nurses. Data sources and review methods: A systematic review of randomised controlled trials (RCT), quasi-RCTs and controlled before and after studies (CBA) of cancer nursing interventions aimed at improving the experience and outcomes of PABC. Ten electronic databases (CENTRAL, MEDLINE, AMED, CINAHL, EMBASE, Epistemonikos, CDSR, DARE, HTA, WHO ICTRP) were searched between 01 January 2000 and 31 May 2016. No language restrictions were applied. Bibliographies of selected studies and relevant Cochrane reviews were also hand-searched. Interventions delivered by cancer nurses were classified according to the OMAHA System. Heat maps were used to highlight the volume of evidence available for different cancer groups, intervention types and stage of cancer care continuum. Results: The search identified 22,450 records; we screened 16,169 abstracts and considered 925 full papers, of which 214 studies (247,550 participants) were included in the evidence synthesis. The majority of studies were conducted in Europe (n = 79) and USA (n = 74). Interventions were delivered across the cancer continuum from prevention and risk reduction to survivorship, with the majority of interventions delivered during the treatment phase (n = 137). Abstract

Aim: To identify, appraise and synthesize the available evidence relating to the value and impact of cancer nursing on patient experience and outcomes. Background: There is a growing body of literature that recognizes the importance and contribution of cancer nurses, however, a comprehensive review examining how cancer nurses have an impact on care quality, patient outcomes and overall experience of cancer, as well as cost of services across the entire cancer spectrum is lacking. Design: A systematic review and meta-analysis using Cochrane methods. Methods: We will systematically search 10 electronic databases from 2000, with pre-determined search terms. No language restrictions will be applied. We will include all randomized and controlled before-and-after studies that compare cancer nursing interventions to a standard care or no intervention. Two reviewers will independently assess the eligibility of the studies and appraise methodological quality using the Cochrane Risk of Bias tool. Disagreements will be resolved by discussion and may involve a third reviewer if necessary. Data from included studies will be extracted in accordance with the Template for intervention Description and Replication reporting guidelines. Missing data will be actively sought from all trialists. Data will be synthesized in evidence tables and narrative to answer three key questions. If sufficient data are available, we will perform meta-analyses. Discussion: This review will allow us to systematically assess the impact of cancer nursing on patient care and experience. This evidence will be used to determine implications for clinical practice and used to inform future programme and policy decisions in Europe. © 2017 John Wiley & Sons Ltd.

16. Holographic thermal field theory on curved spacetimes

Abstract

The AdS/CFT correspondence relates certain strongly-coupled CFTs with large effective central charge ceff to semi-classical gravitational theories with AdS asymptotics. We describe recent progress in understanding gravity duals for CFTs on non-trivial spacetimes at finite temperature, both in and out of equilibrium. Such gravity methods provide powerful new tools to access the physics of these strongly-coupled theories, which often differs qualitatively from that found at weak coupling. Our discussion begins with basic aspects of AdS/CFT and progresses through thermal CFTs on the Einstein Static Universe and on periodically identified Minkowski spacetime. In the latter context we focus on states describing so-called plasma-balls, which become stable at large ceff. We then proceed to out-of-equilibrium situations associated with dynamical bulk black holes. In particular, the non-compact nature of these bulk black holes allows stationary solutions with non-Killing horizons that describe time-independent flows of CFT plasma. As final a topic we consider CFTs on black hole spacetimes. This discussion provides insight into how the CFT transports heat between general heat sources and sinks of finite size. In certain phases the coupling to small sources can be strongly suppressed, resulting in negligible heat transport despite the presence of a deconfined plasma with sizeable thermal conductivity. We also present a new result, explaining how this so-called droplet behaviour is related to confinement via a change of conformal frame. © 2014 IOP Publishing Ltd.

17. Numerical Kaehler-Einstein metric on the third del Pezzo

Abstract

The third del Pezzo surface admits a unique Kaehler-Einstein metric, which is not known in closed form. The manifold's toric structure reduces the Einstein equation to a single Monge-Ampere equation in two real dimensions. We numerically solve this nonlinear PDE using three different algorithms, and describe the resulting metric. The first two algorithms involve simulation of Ricci flow, in complex and symplectic coordinates respectively. The third algorithm involves turning the PDE into an optimization problem on a certain space of metrics, which are symplectic analogues of the "algebraic" metrics used in numerical work on Calabi-Yau manifolds. Our algorithms should be applicable to general toric manifolds. Using our metric, we compute various geometric quantities of interest, including Laplacian eigenvalues and a harmonic (1,1)-form. Abstract

We describe new numerical methods to solve the static axisymmetric vacuum Einstein equations in more than four dimensions. As an illustration, we study the compactified non-uniform black string phase connected to the uniform strings at the Gregory-Laflamme critical point. We compute solutions with a ratio of maximum to minimum horizon radius up to nine. For a fixed compactification radius, the mass of these solutions is larger than the mass of the classically unstable uniform strings. Abstract

Using recently developed numerical methods, we examine neutral compactified non-uniform black strings which connect to the Gregory-Laflamme critical point. By studying the geometry of the horizon we give evidence that this branch of solutions may connect to the black hole solutions, as conjectured by Kol. We find the geometry of the topology changing solution is likely to be nakedly singular at the point where the horizon radius is zero. 20. Strong Brane Gravity and the Radion at Low Energies

Abstract

For the 2-brane Randall-Sundrum model, we calculate the bulk geometry for strong gravity, in the low matter density regime, for slowly varying matter sources. This is relevant for astrophysical or cosmological applications. The warped compactification means the radion can not be written as a homogeneous mode in the orbifold coordinate, and we introduce it by extending the coordinate patch approach of the linear theory to the non-linear case. The negative tension brane is taken to be in vacuum. For conformally invariant matter on the positive tension brane, we solve the bulk geometry as a derivative expansion, formally summing the `Kaluza-Klein' contributions to all orders. For general matter we compute the Einstein equations to leading order, finding a scalar-tensor theory with $\omega(\Psi) \propto \Psi / (1 - \Psi)$, and geometrically interpret the radion. We comment that this radion scalar may become large in the context of strong gravity with low density matter. Equations of state allowing $(\rho - 3 P)$ to be negative, can exhibit behavior where the matter decreases the distance between the 2 branes, which we illustrate numerically for static star solutions using an incompressible fluid. For increasing stellar density, the branes become close before the upper mass limit, but after violation of the dominant energy condition. This raises the interesting question of whether astrophysically reasonable matter, and initial data, could cause branes to collide at low energy, such as in dynamical collapse., Comment: 24 pages, 3 figures

21. Relativistic Stars in Randall-Sundrum Gravity

Abstract

The non-linear behaviour of Randall-Sundrum gravity with one brane is examined. Due to the non-compact extra dimension, the perturbation spectrum has no mass gap, and the long wavelength effective theory is only understood perturbatively. The full 5-dimensional Einstein equations are solved numerically for static, spherically symmetric matter localized on the brane, yielding regular geometries in the bulk with axial symmetry. An elliptic relaxation method is used, allowing both the brane and asymptotic radiation boundary conditions to be simultaneously imposed. The same data that specifies stars in 4-dimensional gravity, uniquely constructs a 5-dimensional solution. The algorithm performs best for small stars (radius less than the AdS length) yielding highly non-linear solutions. An upper mass limit is observed for these small stars, and the geometry shows no global pathologies. The geometric perturbation is shown to remain localized near the brane at high densities, the confinement interestingly increasing for both small and large stars as the upper mass limit is approached. Furthermore, the static spatial sections are found to be approximately conformal to those of AdS. We show that the intrinsic geometry of large stars, with radius several times the AdS length, is described by 4-dimensional General Relativity far past the perturbative regime. This indicates that the non-linear long wavelength effective action remains local, even though the perturbation spectrum has no mass gap. Abstract

Background: Having access to specialist nurses is essential to enhance person-centred care in advanced renal cell carcinoma (RCC). Training opportunities for unrestricted education in advanced RCC care are still limited, adversely impacting patient access to specialist nursing workforce in Europe. RCC4Nurses was co-designed with clinical experts and patient representatives to fill this education gap. Methods: RCC4Nurses was piloted in English with registered nurses from across Europe. Parameters of feasibility, acceptability, fidelity and efficacy were evaluated, guided by the Kirkpatrick framework. The project aimed to involve 160 participants, requiring a minimum of 30 completers for feasibility evaluation. Participants completed questionnaires before and after attending RCC4Nurses, including a knowledge test, confidence in looking after patients, and questions on acceptability and impact. Results: From November 2023 to March 2024, 90 registered nurses enrolled. Participants were female (97%) with a postgraduate degree (62%), and with a median 11.7 years of work experience. Participants originated from 9 countries, predominantly the UK (47%). As of May 2024, forty participants (44.4%) had completed the programme with two months remaining until end of data collection. RCC4Nurses was evaluated positively, with 93% of participants agreeing that it met their learning expectations and helped in their work. Almost 95% would recommend it to colleagues. Nine in 10 participants agreed course duration was about right. Post-course evaluation showed that participants felt more prepared to look after people with advanced RCC (pre-course: mean 5.7/10; post-course: mean 7.95/10), also indicating commitment to apply what they learnt to their work (mean 8.79/10). Conclusions: RCC4nurses is feasible, acceptable and effective as a continuing professional development resource for European nurses to develop specialist knowledge in advanced RCC and enhance survivorship care. Background: Children with cancer experience pain throughout their cancer trajectory, which has short- and long-term negative consequences, both physically and psychologically. Treatment advances have increased ambulatory care, enabling children to spend more time at home. This leads to a shift in pain management responsibility from healthcare professionals who have experience in pain management, to parents, most of whom do not. Little is known about parents' pain management abilities at home. Aim: To understand how parents of children with cancer manage their child's cancer pain at home. Methods: Guided by the Theory of Planned Behaviour, a convergent, parallel mixed methods design was used for the primary purpose of complementarity. Participants were parents of children with cancer on active treatment, recruited from one tertiary cancer centre in the South of England. Convenience sampling was used to recruit participants to complete surveys and pain diaries. Purposive sampling was used for interviews. Surveys measured parents' attitudes toward pain medications and their misconceptions regarding pain assessment. Pain diaries gathered baseline data on pain intensity, cause, location, and parent interventions at home. Interviews enabled deeper understanding of children's pain manifestation, as well as parents' pain assessment and interventions. Surveys and pain diaries were analysed using descriptive and inferential statistics. Thematic analysis was used to analyse interview data. Each dataset was analysed separately and then integrated. Results: Integration of the datasets revealed that most of the time, children were not in pain at home. However, most children experienced episodes of clinically significant pain. Parents combine different types of information to effectively assess their child's pain at home. Parents frequently under-medicated their child's pain but used a variety of non-pharmacological interventions to manage their child's pain at home. Paracetamol, ibuprofen, codeine, and morphine administration all have undesirable consequences for children with cancer who frequently found pharmacological interventions to be unpalatable. This left parents with an "empty toolbox" of pain management interventions which they restocked with non-pharmacological interventions. Contribution to knowledge: Using mixed methods, this research took a holistic approach to investigating parents' management of children's cancer pain at home. This research suggests children with cancer have heterogeneous pain trajectories but due to the unpredictability of pain at home, it is important all parents of children with cancer are prepared for their pain management role. Parents' use different types of information to assess pain. This constitutes a bundled approach to pain assessment. This approach may be helpful in clinical and research contexts. This research is the first to outline practical barriers to pharmacological interventions and the subsequent importance of non-pharmacological interventions for parents managing their child's cancer pain at home.
