Carotid web is a rare intraluminal parietal protrusion in the carotid artery. This vascular anomaly mostly occurs at the bifurcation causing turbulent flow, and is responsible of thrombi formation associated with embolic ischemic events. We report the case of a 35-year-old woman, with no medical history, who presented a recurrent middle cerebral artery (MCA) occlusion within twelve hours caused by a carotid bulb web. Although considered as a rare entity, carotid web is associated with the risk of recurrent strokes in the MCA territory and especially in the absence of the typically recognized risk factors. The key imaging is the CT angiography that shows the web, the cerebral artery occlusion, and the outcome appreciation. Therapeutic strategy associates antithrombotic treatment and operative management with stenting of the carotid web or endarterectomy.

Most anatomic features of the internal carotid artery (ICA) are described as a straight course to the skull base free of branches. In some cases, the excessive elongation of the internal carotid artery in a confined space results in a curvature showing a “C” or “S” shape, or in an abnormal vascular shape made of a single or double vessel loop. These anatomic variants are called dolichoarteriopathies of the internal carotid artery. The correlation between dolichoarteriopathy of the ICA and stroke is still questionable, however it is believed that it can be associated with cerebral ischemia with a clinical symptomatology that accompanies ischemic stroke. We report a case of a 41-year-old patient, with a history of hypertension, who was admitted for right hemiparesis with Broca's aphasia. The rest of the clinical examination was normal. Radiological investigations confirmed an acute left sylvian ischemic stroke with an abrupt occlusion of the posterior trunk of the left M2 segment on the CT angiogram, an excessive elongation of the ICA on both sides, describing a shape of coils or loops. Etiologic workup for ischemic stroke was negative.

Le but de notre travail est de faire une revue générale sur les différents aspects clinico-radiologiques et la prise en charge de la thrombose veineuse cérébrale (TVC) au sein de notre structure hospitalière et les comparer à ceux décrits dans la littérature.

La survenue d’un faux anévrisme d’une artériole jéjunale, surtout à distance de la pancréatite aiguë est une complication rare dans les suites d’une pancréatite aiguë, elle se manifeste par une hémorragie digestive, lorsque ce faux anévrisme se fissure au niveau de la lumière digestive. Cette complication est extrêmement rare pouvant engager le pronostic vital. Le diagnostic peut se faire par un angioscanner abdominal ; la radiologie permet également la prise en charge thérapeutique en réalisant une embolisation sélective. Nous rapportons l’observation d’un patient âgé de 77 ans ayant présenté deux ans auparavant une pancréatite aiguë, compliquée d’un faux kyste du pancréas de l’arrière cavité des épiploons, surinfecté et spontanément fistulisé dans la lumière jéjunale ; il était admis aux urgences pour des mélénas de grande abondance sur rupture d’un faux anévrisme d’une artériole jéjunale certifié sur une angio-TDM abdominale. Le patient a bénéficié d’une embolisation par colle biologique du faux anévrisme avec bonne réponse radiologique et clinique. La survenue d’accidents hémorragiques suite à la rupture d’un faux anévrisme dans la lumière intestinale comme complication à distance d’une pancréatite reste exceptionnelle. Ce cas clinique rapporte une complication rare d’hémorragie digestive sur faux anévrisme jéjunal compliquant une pancréatite à la phase aiguë.

L'objectif de notre cas exposé est de faire une revue de littérature du principe diagnostique et de la prise en charge de la fistule aortocave, complication rare des anévrismes de l’aorte, souvent fatale.

Introduction: Large vessel vasculopathy, such as carotid stenosis, has been shown to be a side effect of radiotherapy (RT) and has received increasing attention especially in recent decades with the improvement of RT technology. These injuries can lead to a higher risk of cerebrovascular events such as ischemic stroke. The management of these lesions may be performed with surgical repair but also with endovascular technique.

Cerebral venous thrombosis (CVT) is a rare but serious complication of ulcerative colitis (UC) with a high morbidity and mortality rate. CVT is not usually readily recognized and treatment may be delayed, impacting on the prognosis. Here we are reporting 2 cases of CVT occurring during a relapse of ulcerative colitis (UC) with review of literature. The first patient was a 27-year-old woman with chronic UC who presented with headaches during a relapse of chronic UC. She was found to have cerebral venous sinus thrombosis. The second one was a 45-year-old man who presented with seizures and focal neurological symptom during a relapse of chronic UC. He was found to have cerebral venous sinus thrombosis complicated by left temporal infarction. Both of them were treated with therapeutic anticoagulation with good improvement. The presence of neurological signs in a patient with UC mostly during exacerbation period, should alert health professionals about the possibility of a CVT.
