Formic acid (FA) is a promising hydrogen carrier. Recently, we developed a formally underligated iridium(III) complex [Ir(Cp*)(O,N)]+(Cp* = pentamethylcyclopentadienyl, C5Me5–; O,N = pyridylidene amine (PYE)-phenolate) 1aas a high performing catalyst for FA dehydrogenation. Here, we exploited the facile synthetic accessibility of the PYE-phenolate ligand to investigate the influence of the ligand on FA dehydrogenation. Comparison of the activity of 1awith related iridium complexes containing ligands derived either from hydroxy-quinoline or N-aryl-aminophenol (complexes 2and 3) indicate that neither a static imine nor a static anionic amide bonding of the nitrogen site induces catalytic activity, in contrast to the donor-flexible PYE nitrogen in 1a. Introduction of substituents on the phenolate donor (−OMe, −Me, −H, −Cl, and −NO2, complexes 4–7), and on the PYE heterocycle (−Me, −H, and −Cl, complexes 8–10), as well as comparison of the ortho-PYE with its meta-and para-isomers (complexes 11–13) indicate a narrow window of electronic and steric tolerance before catalytic activity decreases. Evaluation of electron density at the iridium center via cyclic voltammetry (CV) as well as chemical shift and half-life time of the iridium-hydride intermediate derived from complexes 4–7reveals a direct correlation between iridium electron density, hydride stability, and Hammett σpparameters, though not with the catalytic activity of these complexes. This work indicates therefore that catalytic activity is not governed by maximizing hydricity or iridium electron density but by optimizing these parameters.

BackgroundAneurysm location is a key element in predicting the rupture risk of an intracranial aneurysm. A common impression suggests that pure ophthalmic aneurysms are under-represented in ruptured intracranial aneurysms (RIAs). The purpose of this study was to specifically evaluate the risk of rupture of ophthalmic aneurysms compared with other aneurysm locations.MethodsThis multicenter study compared the frequency of ophthalmic aneurysms in a prospective cohort of RIAs admitted to 13 neuroradiology centers between January 2021 and March 2021, with a retrospective cohort of patients with unruptured intracranial aneurysms (UIAs) who underwent cerebral angiography at the same neuroradiology centers during the same time period.Results604 intracranial aneurysms were included in this study (355 UIAs and 249 RIAs; mean age 57 years (IQR 49–65); women 309/486, 64%). Mean aneurysm size was 6.0 mm (5.3 mm for UIAs, 7.0 mm for RIAs; P<0.0001). Aneurysm shape was irregular for 37% UIAs and 73% RIAs (P<0.0001). Ophthalmic aneurysms frequency was 14.9% of UIAs (second most common aneurysm location) and 1.2% of RIAs (second least common aneurysm location; OR 0.07 (95% CI 0.02 to 0.23), P<0.0001).ConclusionsOphthalmic aneurysms seem to have a low risk of rupture compared with other intracranial aneurysm locations. This calls for a re-evaluation of the benefit–risk balance when considering preventive treatment for ophthalmic aneurysms.

Parmi les critères de qualité l’on peut juger le plagiat potentiel et la conformité des déclarations de liens d’intérêts. Ces données n’ont pas été comparées pour OTSR aussi nous avons mené une étude rétrospective afin de mesurer : (1) le taux de plagiat potentiel, (2) pour les auteurs français, le taux de défaut de déclaration des liens d’intérêts, (3) le lien éventuel entre ces deux méconduites scientifiques.

Kinship for First Nations people is a fundamental, yet complex, element of one's culture, enabling both belonging and relationality, and extending beyond blood family and relations. Kinship is also recognized as important within out‐of‐home care (OOHC) systems, with kinship carebeing the predominant OOHC placement type in Australia (AIHW, 2021). However, when First Nations children and young people are removed by the state, and placed into OOHC, it is important to interrogate whether kinship placements enable cultural connection and continuity with First Nations ways of understanding Kinship. This article begins by contextualising current OOHC policy and practice in Australia where a westernised and homogenised concept of kinship care is touted as similar to First Nations notions of Kinship. Here, the Aboriginal and Torres Strait Islander Child Placement Principle (ATSICPP) has become oversimplified by equating kinship care with relative care, thus creating ambiguity in care and placements in OOHC, voiding First Nations traditions and ways of understanding Kinship while a young person is in care. This article then reports the findings of qualitative interviews with 37 First Nations and non‐Indigenous people concerning the use of the term ‘Kinship’ in OOHC in New South Wales (NSW), how it is operationalised and how it is understood. Wide‐ranging experiences by participants concerning Kinship within OOHC were evident, thus demonstrating the imperative for a broader understanding of Kinship. This understanding needs to be applied within OOHC in ways that are consistent with First Nations notions of Kinship, as kinship care placements on their own are not enough to keep children connected in culture. The findings from this study show that the term Kinship is applied problematically within OOHC in NSW by equating legal and policy definitions of kinship care with Aboriginal ways of understanding Kinship. Ensuring First Nations traditions, practices and notions of Kinship are applied for First Nations children and young people in OOHC contexts is paramount.

Introduction: Liposomal irinotecan promotes controlled sustained release of irinotecan (CPT-11), therefore, we hypothesize that the therapeutic index (quantitative measurement of the relative efficacy/safety ratio of a drug) will be higher for liposomal than non-liposomal irinotecan. Methods: We compared the therapeutic indexes of liposomal and non-liposomal irinotecan in mice bearing subcutaneous patient-derived xenograft (PDX) pancreatic tumors under dosing regimens approximating the clinical setting. Following preliminary drug sensitivity/antitumor activity analyses on three PDX tumor models, one model was selected for analyses of efficacy, biomarker, toxicology, pharmacokinetics in mice receiving liposomal irinotecan (2.5, 10, 50 mg/kg/week) or non-liposomal irinotecan (10, 25, 50 mg/kg/week). The maximum tolerated dose (MTD) for each treatment was 50 mg/kg/week. Results: Using the selected IM-PAN-001 model at the MTD (both treatments, 50 mg/kg/week), antitumor activity, phospho-histone gamma-H2AX protein staining in cancer cell nuclei, histological tumor regression, and plasma levels of CPT-11 and its active metabolite SN-38 after 24 h were greater with liposomal than non-liposomal irinotecan, but tumor SN-38 levels were similar. At the lowest doses assessed, antitumor activity, histological tumor regression, and jejunum and bone marrow toxicity were similar. Based on these findings, liposomal and non-liposomal irinotecan had therapeutic indexes of 20 and 5, respectively. Conclusion: This non-clinical study showed a fourfold broader therapeutic index with liposomal than non-liposomal irinotecan in mice bearing IM-PAN-001 PDX pancreatic tumors, even at optimal dosing for the two drugs. These findings support the clinical benefit observed with liposomal irinotecan in patients with pancreatic cancer.

L’usage de substances psychoactives par voie intraveineuse (IV) représente une préoccupation de santé publique associée à un surrisque d’overdoses et de transmission des maladies infectieuses. L’analyse des résidus de seringues usagées offre une source d’information plus précise sur la nature des substances injectées, comparativement aux déclarations des utilisateurs. L’objectif de cette étude a été le développement et la validation d’une méthode permettant de rechercher 340 substances psychoactives et produits de coupe (hors insuline) par LC-MS/MS dans des résidus de seringues usagées collectés à Paris par le réseau ESCAPE (European Syringe Collection and Analysis Project Enterprise).

Les dépôts extracellulaires de plaques séniles principalement composées de protéine Aβ et les lésions intracellulaires ou dégénérescences neurofibrillaires (DNF) constituées de protéines tau hyperphosphorylées sont caractéristiques de la maladie d’Alzheimer (MA). Les lésions de DNF semblent corrélées à la symptomatologie clinique et permettent de classer la maladie d’Alzheimer en stades neuropathologiques. Dans cette étude, nous avons voulu comparer l’activité métabolique et la densité de DNF par imagerie moléculaire dans une cohorte restreinte de sujets.

The best transportation strategy for patients with suspected large vessel occlusion (LVO) is unknown. Here, we evaluated a new regional strategy of direct transportation to a Comprehensive Stroke Center (CSC) for patients with suspected LVO and low probability of receiving intravenous thrombolysis (IVT) at the nearest Primary Stroke Center (PSC).

L’échographie permet de détecter efficacement les modifications de structure de la rate, notamment les modifications diffuses : multiples nodules hypoéchogènes de petite taille, parenchyme en nid d’abeille… En revanche, elle ne permet pas d’établir un diagnostic. Cet article décrit les cas de quatre chiens qui présentaient des modifications diffuses de la rate, assez comparables à l’échographie. Des cytoponctions échoguidées ont conduit à quatre diagnostics très différents : lymphome, mastocytome, sarcome histiocytaire et leishmaniose. Les cytoponctions échoguidées de la rate constituent une méthode diagnostique facile à mettre en œuvre et peu invasive, assez peu sensible mais très spécifique, avec un rapport bénéfices/risques très favorable.

Les lésions capsulo-méniscales du segment postérieur du ménisque médial sont observées dans 16 à 40 % des cas lors des reconstructions du ligament croisé antérieur. La technique classique de réparation est la suture au crochet par voie postéro-médiale, l’arthroscope étant positionné en vue intercondylienne par voie antéro-latérale. La visualisation de ces lésions et leur réparation est parfois compliquée, même avec un arthroscope à 70°. L’objectif de cette étude était d’établir la faisabilité des sutures des lésions capsulo-méniscales par une double voie postéro-médiale arthroscopique optique et instrumentale.

The formation of dense protein interfacial layers at a free air–water interface is known to result from both diffusion and advection. Furthermore, protein interactions in concentrated phases are strongly dependent on their overall positive or negative net charge, which is controlled by the solution pH. As a consequence, an interesting question is whether the presence of an advection flow of water toward the interface during protein adsorption produces different kinetics and interfacial structure of the adsorbed layer, depending on the net charge of the involved proteins and, possibly, on the sign of this charge. Here we test a combination of the following parameters using ovalbumin and lysozyme as model proteins: positive or negative net charge and the presence or absence of advection flow. The formation and the organization of the interfacial layers are studied by neutron reflectivity and null-ellipsometry measurements. We show that the combined effect of a positive charge of lysozyme and ovalbumin and the presence of advection flow does induce the formation of interfacial multilayers. Conversely, negatively charged ovalbumin forms monolayers, whether advection flow is present or not. We show that an advection/diffusion model cannot correctly describe the adsorption kinetics of multilayers, even in the hypothesis of a concentration-dependent diffusion coefficient as in colloidal filtration, for instance. Still, it is clear that advection is a necessary condition for making multilayers through a mechanism that remains to be determined, which paves the way for future research.

The mechanistic target of rapamycin (mTOR) pathway is hyperactivated in cancer and neurological disorders. Rapalogs and mTOR kinase inhibitors (TORKi) have recently been applied to alleviate epileptic seizures in tuberous sclerosis complex (TSC). Herein, we describe a pharmacophore exploration to identify a highly potent, selective, brain penetrant TORKi. An extensive investigation of the morpholine ring engaging the mTOR solvent exposed region led to the discovery of PQR626 (8). 8displayed excellent brain penetration and was well-tolerated in mice. In mice with a conditionally inactivated Tsc1gene in glia, 8significantly reduced the loss of Tsc1-induced mortality at 50 mg/kg p.o. twice a day. 8overcomes the metabolic liabilities of PQR620 (52), the first-in-class brain penetrant TORKi showing efficacy in a TSC mouse model. The improved stability in human hepatocytes, excellent brain penetration, and efficacy in Tsc1GFAPCKO mice qualify 8as a potential therapeutic candidate for the treatment of neurological disorders.

Le traitement chirurgical des lésions multiligamentaires (MLKIs) est actuellement préféré au traitement fonctionnel mais il persiste des interrogations concernant la meilleure stratégie chirurgicale. Le meilleur moment pour opérer, des greffes autologues ou hétérologues, une chirurgie en un temps ou en séquentielle. Le but de cette étude était d’évaluer les résultats laximétriques des reconstructions multiligamentaires par allogreffes en un temps. L’hypothèse était que l’utilisation de ces allogreffes congelées et non irradiées donne une laxité résiduelle faible et des résultats cliniques satisfaisants.

À partir des nouveaux critères diagnostiques de l’anarthrie primaire progressive proposés en 2019, cet article présente une mise au point sur ce syndrome neurodégénératif focal rare, probablement sous-estimé. Il doit être évoqué devant des troubles arthriques isolés associés à une apraxie bucco-faciale. Les fonctions cognitives et l’autonomie sont préservées pendant de nombreuses années, jusqu’à des stades tardifs d’évolution.

Les instances en charge des destinées de la Revue de Chirurgie Orthopédique et Traumlatologique(RCOT) et de sa version anglaise Orthopaedics and Traumatology: Surgery and Research(OTSR) ont souhaité évaluer la perception de ces revues auprès des lecteurs francophones, par l’intermédiaire d’une enquête de lectorat. L’objectif était d’évaluer trois grands thèmes : connaissance de RCOT-OTSRet modalités de lecture, contenu de la revue, investissement dans la revue.

A new, scalable, rapid, high yielding, and practical synthesis of 4-(difluoromethyl)pyridin-2-amine provides a key intermediate for the preparation of numerous protein kinase inhibitors and clinical candidates targeting phosphoinositide 3-kinase (PI3K) and the mechanistic target of rapamycin (mTOR) kinase. Starting from 2,2-difluoroacetic anhydride, an efficient five-step and two-pot procedure to prepare 4-(difluoromethyl)pyridin-2-amine (1) has been developed. Noteworthy aspects of this strategy include the avoidance of an amination process using a sealed vessel. Each step of the synthetic route has been optimized, and key intermediates have been isolated and characterized prior to the final two-pot procedure, which has been successfully applied for large-scale production.

The mechanistic target of rapamycin (mTOR) plays a pivotal role in growth and tumor progression and is an attractive target for cancer treatment. ATP-competitive mTOR kinase inhibitors (TORKi) have the potential to overcome limitations of rapamycin derivatives in a wide range of malignancies. Herein, we exploit a conformational restriction approach to explore a novel chemical space for the generation of TORKi. Structure–activity relationship (SAR) studies led to the identification of compound 12bwith a ∼450-fold selectivity for mTOR over class I PI3K isoforms. Pharmacokinetic studies in male Sprague Dawley rats highlighted a good exposure after oral dosing and a minimum brain penetration. CYP450 reactive phenotyping pointed out the high metabolic stability of 12b. These results identify the tricyclic pyrimido-pyrrolo-oxazine moiety as a novel scaffold for the development of highly selective mTOR inhibitors for cancer treatment.

The phosphoinositide 3-kinase (PI3K)/mechanistic target of rapamycin (mTOR) pathway is frequently overactivated in cancer, and drives cell growth, proliferation, survival, and metastasis. Here, we report a structure–activity relationship study, which led to the discovery of a drug-like adenosine 5′-triphosphate-site PI3K/mTOR kinase inhibitor: (S)-4-(difluoromethyl)-5-(4-(3-methylmorpholino)-6-morpholino-1,3,5-triazin-2-yl)pyridin-2-amine (PQR530, compound 6), which qualifies as a clinical candidate due to its potency and specificity for PI3K and mTOR kinases, and its pharmacokinetic properties, including brain penetration. Compound 6showed excellent selectivity over a wide panel of kinases and an excellent selectivity against unrelated receptor enzymes and ion channels. Moreover, compound 6prevented cell growth in a cancer cell line panel. The preclinical in vivo characterization of compound 6in an OVCAR-3 xenograft model demonstrated good oral bioavailability, excellent brain penetration, and efficacy. Initial toxicity studies in rats and dogs qualify 6for further development as a therapeutic agent in oncology.

Les recommandations de l’HAS de 2008 favorisant les traitements dits « conservateurs » dans la pathologie méniscale et l’absence de supériorité de la méniscectomie sur le traitement non opératoire dans la pathologie méniscale dégénérative modifient la gestion des thérapeutiques. L’impact de ces preuves sur la pratique clinique française est actuellement inconnu. L’objectif de cette étude était d’évaluer l’évolution au cours du temps du nombre d’acte et les variations géographiques françaises de l’alternative thérapeutique chirurgicale (méniscectomies versus suture méniscale) grâce aux données de l’Agence technique de l’information sur l’hospitalisation (ATIH).

Background: Impingement of an elongated styloid process (ESP) or calcified stylohyoid ligament on surrounding neck structures defines Eagle syndrome. The vascular variant, also called stylocarotid syndrome, results from impingement of vascular structures and remains poorly known among physicians.Research Design: We report our own experience and review the literature in order to clarify the diagnostic and therapeutic management. Patients with vascular events in relation to an ESP and hospitalized at our institution were extracted from our databank and retrospectively reviewed. We also performed a comprehensive review of the literature on Eagle syndrome using PubMed® and Google Scholar, analysing the presentation, management, and follow-up.Results: We report five cases of the vascular variant of Eagle syndrome: one carotid perforation, one focal arteriopathy, one with both acute and chronic dissection and two acute internal carotid dissection. Vascular compression, whether permanent or transient, is also reported in the literature. Management varies, although styloidectomy is deemed appropriate for symptomatic compression, while stenting is preferred in cases of perforation.Conclusions: A common definition of Eagle syndrome is required for better diagnosis and management. The choice of styloidectomy is understandable for compression but remains to be investigated in other cases.

Complication after carotid artery revascularization is mainly represented by stroke. Reversible cerebral vasoconstriction syndrome triggering by carotid artery revascularization is exceptional but it is an unrecognized aetiology of stroke. It could be associated with brain edema and henceforth, a posterior reversible encephalopathy syndrome can be confused with post-carotid artery revascularization cerebral hyperperfusion syndrome. We reported three cases about reversible cerebral vasoconstriction syndrome following revascularization therapy whose one had also a posterior reversible encephalopathy syndrome. This complication occurred within two weeks after carotid artery revascularization. All took nimodipine and all had a functional improvement at 3-months follow-up. Reversible cerebral vasoconstriction syndrome with posterior reversible encephalopathy syndrome after carotid artery revascularization may mimic cerebral hyperperfusion syndrome. After a comprehensive review, nineteen cases of reversible cerebral vasoconstriction syndrome post-carotid artery revascularization have been reported in the literature mainly after symptoms. It occurs mainly in women around sixty years of age. The onset is expected four days after revascularization.

Phosphoinositide 3-kinase (PI3K) is deregulated in a wide variety of human tumors and triggers activation of protein kinase B (PKB/Akt) and mammalian target of rapamycin (mTOR). Here we describe the preclinical characterization of compound 1(PQR309, bimiralisib), a potent 4,6-dimorpholino-1,3,5-triazine-based pan-class I PI3K inhibitor, which targets mTOR kinase in a balanced fashion at higher concentrations. No off-target interactions were detected for 1in a wide panel of protein kinase, enzyme, and receptor ligand assays. Moreover, 1did not bind tubulin, which was observed for the structurally related 4(BKM120, buparlisib). Compound 1is orally available, crosses the blood–brain barrier, and displayed favorable pharmacokinetic parameters in mice, rats, and dogs. Compound 1demonstrated efficiency in inhibiting proliferation in tumor cell lines and a rat xenograft model. This, together with the compound’s safety profile, identifies 1as a clinical candidate with a broad application range in oncology, including treatment of brain tumors or CNS metastasis. Compound 1is currently in phase II clinical trials for advanced solid tumors and refractory lymphoma.

Coordinatively unsaturated complexes are interesting from a fundamental level for their formally empty coordination site and, in particular, from a catalytic perspective as they provide opportunities for substrate binding and transformation. Here, we describe the synthesis of a novel underligated ruthenium complex [Ru(cym)(N,N′)]+, 3, featuring an amide-functionalized pyridylidene amide (PYA) as the N,N′-bidentate coordinating ligand. In contrast to previously investigated underligated complexes, complex 3offers potential for dynamic modifications, thanks to the flexible donor properties of the PYA ligand. Specifically, they allow both for stabilizing the formally underligated metal center in complex 3through nitrogen π-donation and for facilitating through π-acidic bonding properties the coordination of a further ligand L to the ruthenium center to yield the formal 18 e–complexes [Ru(cym)(N,N′)(L)]+(4: L = P(OMe)3; 5: L = PPh3; 6: L = N-methylimidazole; 7: L = pyridine) and neutral complex [RuCl(cym)(N,N′)] 8. Analysis by 1H NMR and UV–vis spectroscopies reveals an increasing Ru–L bond strength along the sequence pyridine <1-methylimidazole < PPh3< P(OMe)3with binding constants varying over 3 orders of magnitude with log(Keq) values between 2.8 and 5.7. The flexibility of the Ru(PYA) unit and the ensuing accessibility of saturated and unsaturated species with one and the same ligand are attractive from a fundamental point of view and also for catalytic applications, as catalytic transformations rely on the availability of transiently vacant coordination sites. Thus, while complex 3does not form stable adducts with O-donors such as ketones or alcohols, it transiently binds these species, as evidenced by the considerable catalytic activity in the transfer hydrogenation of ketones. Notably, and as one of only a few catalysts, complex 3is compatible with EtOH as a hydrogen source. Complex 3shows excellent performance in the transfer hydrogenation of pyridyl-containing substrates, in agreement with the poor coordination strength of this functional group to the ruthenium center in 3.

Les CAR-T cellssont des lymphocytes T génétiquement modifiés exprimant un récepteur chimérique antigénique (chimeric antigenreceptor[CAR]), apporté via des vecteurs de transfert génétique aux lymphocytes T du patient. Indiqués dans le traitement d’hémopathies malignes, on dénombre 6 spécialités à base de CAR-T cellsayant fait l’objet d’une évaluation par la Commission de la transparence (CT) de la Haute Autorité de santé (HAS) depuis 2018 correspondant à 11 avis de droit commun. La totalité de ces demandes de remboursement ont reçu un avis favorable avec un service médical rendu (SMR) important. Entre 2018 et 2021, malgré l’absence de données comparatives, la CT a reconnu un progrès en octroyant des ASMR de niveau III ou IV. À partir de 2021, la majorité des CAR-T cellsont obtenu des ASMR de niveau V, traduisant une forte incertitude sur l’efficacité relative de ces thérapies. Plus récemment la CT a pu évaluer des CAR-T cellsprésentant des données robustes de phase III qui ont permis de reconnaître des ASMR de niveau III à IV. Enfin, depuis la mise en place de la réforme de l’accès précoce au 1erjuillet 2021, la HAS a autorisé l’ensemble des produits évalués. L’évaluation des CAR-T cellsse poursuit en vie réelle, notamment grâce à la mise en place du registre français DESCAR-T. Il demeure essentiel de disposer d’essais contrôlés randomisés qui permettent de s’affranchir des biais de sélection et d’apprécier la réelle valeur ajoutée de ces thérapies.

Mechanistic target of rapamycin (mTOR) promotes cell proliferation, growth, and survival and is overactivated in many tumors and central nervous system disorders. PQR620 (3) is a novel, potent, selective, and brain penetrable inhibitor of mTORC1/2 kinase. PQR620 (3) showed excellent selectivity for mTOR over PI3K and protein kinases and efficiently prevented cancer cell growth in a 66 cancer cell line panel. In C57BL/6J and Sprague–Dawley mice, maximum concentration (Cmax) in plasma and brain was reached after 30 min, with a half-life (t1/2) > 5 h. In an ovarian carcinoma mouse xenograft model (OVCAR-3), daily dosing of PQR620 (3) inhibited tumor growth significantly. Moreover, PQR620 (3) attenuated epileptic seizures in a tuberous sclerosis complex (TSC) mouse model. In conclusion, PQR620 (3) inhibits mTOR kinase potently and selectively, shows antitumor effects in vitro and in vivo, and promises advantages in CNS indications due to its brain/plasma distribution ratio.

Le positionnement en rotation du composant fémoral est un facteur essentiel de succès d’une arthroplastie totale de genou. Or, la torsion épiphysaire distale du fémur (TEDF) est variable selon les individus. La mesure tomodensitométrique préopératoire de celle-ci, couplée à la chirurgie assistée par ordinateur permet d’améliorer significativement le positionnement de l’implant fémoral en rotation. Mais le scanner préopératoire a un coût et impose une irradiation. Ceux-ci sont doublés en cas d’arthroplastie bilatérale. Le but de notre travail était, dans le cadre d’une arthroplastie bilatérale d’évaluer la TEDF des deux genoux d’un même patient. L’hypothèse était que la TEDF était symétrique entre les 2 genoux d’un patient et ce quelle que soit la déformation frontale.

L’utilisation des plaques verrouillées dans les ostéotomies tibiales de valgisation (OTV) par ouverture médiale permet théoriquement un appui précoce sans adjonction de greffe osseuse ou de substitut. Cette technique expose à des risques de non-consolidation en cas de correction importante (>10°) mais leur taux et facteurs de risques ne sont pas connus. Cette étude rétrospective comparant les OTV avec corrections de moins de 10° et de plus de 10° a été menée afin de préciser : (1) les taux de complications selon l’importance de la correction (pseudarthroses, démontages), (2) les facteurs de risque associés à ces complications.

L’asthme est une pathologie respiratoire chronique et fréquente affectant aussi bien les enfants que les adultes. Dans sa forme sévère, l’asthme est caractérisé notamment par des exacerbations, le plus souvent dues à une infection par un rhinovirus. Selon plusieurs études épidémiologiques, le rôle du rhinovirus A (RVA) et C (RVC) dans les exacerbations est différent entre les enfants et les adultes. En effet, le RVA est majoritairement responsable des exacerbations de l’asthme chez l’adulte alors que le RVC est plus largement impliqué dans les exacerbations les plus sévères de l’enfant. Cette spécificité liée à l’âge suggère une modification physiopathologique de l’épithélium bronchique d’enfants par rapport à celui de l’adulte. Cependant, l’étude de l’infection épithéliale par le RVA et le RVC se heurte à des difficultés techniques, notamment au niveau de la production des particules de RVC infectieuses, mais aussi dans la visualisation de l’infection par le rhinovirus dans un modèle d’épithélium différencié en interface air–liquide. Ainsi, l’objectif de cette étude est de développer un modèle d’étude in vitro d’infection par le rhinovirus A et C dans un modèle d’épithélium permettant l’étude de la cinétique d’infection des épithéliums d’adultes et d’enfants.

La méniscectomie demeure l’une des interventions orthopédiques les plus fréquentes alors même que le concept de préservation méniscale est mis en avant depuis plusieurs décennies. Le taux de méniscectomies demeure trop élevé malgré des publications scientifiquement robustes démontrant l’intérêt de la réparation méniscale (ou du traitement non opératoire) dans les déchirures traumatiques ou du traitement non opératoire dans les lésions méniscales dégénératives. Il est vraiment temps de changer le paradigme de prise en charge des lésions méniscales en mettant en première ligne la préservation méniscale. Dans les déchirures traumatiques, et singulièrement les lésions verticales longitudinales en zone vascularisée, la réparation méniscale a un taux de succès élevé que ce soit en termes de délai de récupération, de résultats fonctionnels, de protection du cartilage. L’abstention peut se discuter devant des lésions asymptomatiques du ménisque latéral dans le cadre de la reconstruction du ligament croisé antérieur (LCA). Les lésions capsulo-synoviales postérieures (dans le cadre de la rupture du LCA), les lésions traumatiques des racines, les lésions radiaires sont également d’excellentes indications à réparation. Encore faut-il souligner que l’histoire naturelle de ces lésions n’est pas totalement claire, posant aussi la question du traitement non opératoire. La lésion méniscale dégénérative du sujet d’âge mûr ou plus âgé est une découverte très fréquente à l’IRM. Elle est intimement liée à un processus de vieillissement des tissus, et donc probablement au processus arthrosique. La méniscectomie a été longtemps considérée comme la réponse de choix. Les huit études randomisées récentes, sauf une, montrent la non supériorité du traitement arthroscopique sur le traitement non opératoire. De sorte que ce traitement non opératoire doit être le choix de première ligne, et la méniscectomie arthroscopique envisagée seulement en cas d’échec du traitement non opératoire. Une indication précoce peut être envisagée en cas de symptômes mécaniques « considérables ». Un cas particulier est représenté par la lésion en clivage horizontal du jeune athlète, qui requiert une réparation méniscale pour éviter une méniscectomie nécessairement très étendue chez le patient jeune et actif. Le message demeure plus que jamais : save the meniscus !

Les instabilités postéro-latérales représentent un challenge chirurgical. De nombreuses plasties existent mais peu reconstruisent anatomiquement les différentes structures postéro-latérales. Nous présentons une technique opératoire nommée la plastie postéro-latérale Versailles, qui reconstruit anatomiquement les trois structures stabilisatrices principales du plan postéro-latéral (le ligament collatéral latéral, le tendon poplité et le ligament fibulo-poplité) à l’aide d’une autogreffe aux ischio-jambiers ou d’une allogreffe tendineuse.

Les lésions biméniscale associées à une rupture du ligament croisé antérieur (LCA) sont fréquentes mais les lésions biméniscales réparées dans le cadre d’une ligamentoplastie du LCA sont rares. L’objectif de cette étude était d’évaluer les résultats des ligamentoplasties du LCA associées à une réparation biméniscale. L’hypothèse était que la présence d’une lésion biméniscale, même réparée, est un facteur péjoratif d’échec de la ligamentoplastie.

L’implantation d’un substitut méniscal en polyuréthane (Actifit®) dans le cadre d’un syndrome douloureux post-méniscectomie partielle permet une amélioration fonctionnelle et analgésique à court terme. Peu d’études existent à moyen terme. L’objectif de cette étude était l’évaluation longitudinale des résultats fonctionnels et IRM à 5 ans de recul minimum. L’hypothèse était la stabilité de ces résultats avec le temps au recul de l’étude.

La lésion méniscale dégénératice (LMD) est un processus à développement lent qui se présente sous forme d’un clivage horizontal chez le patient d’âge mûr ou plus âgé. Lorsque le genou est symptomatique, une méniscectomie partielle arthroscopique (MPA) est proposée depuis longtemps avec de nombreuses publications de séries faisant état d’une amélioration de la fonction. Depuis 2002, plusieurs études randomisées montrent l’absence de bénéfice de la MPA par rapport au traitement non opératoire, la « sham » chirurgie, ou la « sham » méniscectomie arthroscopique. Ces résultats ont été à l’origine de controverses dans la communauté médicale et posait la question de la prise en charge en pratique quotidienne. Pour faciliter le processus de décision, un consensus porté par l’European Society for Sports Surgery, Knee Surgery and Arthroscopy (ESSKA) est proposé. La LMD était définie comme une lésion survenant dans un contexte atraumatique chez un patient de plus de 35 ans. Les lésions congénitales, les lésions traumatiques, et les lésions dégénératives du sujet jeune (en particulier de l’athlète) étaient exclues. Le projet a suivi le processus du consensus formel, impliquant successivement un groupe pilote, un groupe d’évaluation, et un groupe de relecture. Au total, 84 chirurgiens et scientifiques venant de 22 pays européens ont été impliqués dans le processus. Vingt questions, leurs réponses et un algorithme décisionnel ont été proposés, fondés sur une revue extensive de la littérature et des avis d’experts. À chaque question-réponse était attribué un grade fondé sur le niveau scientifique de la littérature. La MPA ne doit pas être proposée en première ligne du traitement des LMD. La MPA doit être uniquement envisagée après un examen clinique et radiologique standardisé et lorsque la réponse au traitement non opératoire n’a pas été satisfaisante. L’IRM du genou n’est habituellement pas indiquée dans le bilan initial. En revanche, la radiographie standard est l’examen de référence initial pour détecter une arthrose du genou ou de rares pathologies, telles que tumeurs ou fractures. Cet article produit un cadre clair pour la prise en charge des LMD, avec l’objectif d’un équilibre entre données de la science et expertise clinique. En raison de biais et de faiblesses de la littérature, et du manque de définition de critères importants comme les symptômes mécaniques, cet article ne peut être considéré comme un algorithme exact : il résume « l’ESSKA Meniscus Project » (http://www.esska.org/education/projects) et est le premier consensus européen officiel sur ce sujet. Ce consensus peut être actualisé au fur et à mesure de l’émergence de publications de haute qualité scientifique.

Meniscectomy is one of the most popular orthopaedic procedures, but long-term results are not entirely satisfactory and the concept of meniscal preservation has therefore progressed over the years. However, the meniscectomy rate remains too high even though robust scientific publications indicate the value of meniscal repair or non-removal in traumatic tears and non-operative treatment rather than meniscectomy in degenerative meniscal lesionsIn traumatic tears, the first-line choice is repair or non-removal. Longitudinal vertical tears are a proper indication for repair, especially in the red-white or red-red zones. Success rate is high and cartilage preservation has been proven. Non-removal can be discussed for stable asymptomatic lateral meniscal tears in conjunction with anterior cruciate ligament (ACL) reconstruction. Extended indications are now recommended for some specific conditions: horizontal cleavage tears in young athletes, hidden posterior capsulo-meniscal tears in ACL injuries, radial tears and root tears.Degenerative meniscal lesions are very common findings which can be considered as an early stage of osteoarthritis in middle-aged patients. Recent randomised studies found that arthroscopic partial meniscectomy (APM) has no superiority over non-operative treatment. Thus, non-operative treatment should be the first-line choice and APM should be considered in case of failure: three months has been accepted as a threshold in the ESSKA Meniscus Consensus Project presented in 2016. Les réparations méniscales sous arthroscopie permettent de limiter l’arthrose radiologique à moyen terme. L’IRM n’est pas une bonne méthode d’évaluation de la cicatrisation méniscale, mais peut montrer des signes précurseurs d’arthrose tels que l’extrusion méniscale. Le but de cette étude était d’évaluer l’extrusion méniscale à long terme d’une réparation méniscale. L’hypothèse est qu’il n’existe pas d’extrusion méniscale, confirmant le rôle fonctionnel durable d’un ménisque suturé.

La mise en place d’un dispositif de travail permet d’apercevoir chez l’enfant autiste, un bourgeon primitif, le cartouche, dont les processus n’ont pu se terminer ou être unifiés. Ils apparaissent à travers une cinématique qui, dans de nombreux cas, peut être reconstruite et redynamisée afin que l’enfant ait accès au symbolique.
