The incidence of stenosis of uretero-intestinal anastomoses is about 5–10%. The most relevant effect of this post-surgical complication is the progressive renal function deterioration. Infections, ureteric ischemia, perianastomotic urinary extravasation and apposition of two different types of mucosa are the main causes of this complication. Several options are available for the treatment of these strictures: surgery, ureteral stenting, balloon dilation, endoscopic electrocautery or laser incision. The Acucise® catheter system combines diatermic incision with balloon dilation.Material and methods 9 stenosis of uretero-intestinal anastomoses have been treated with Acusise® endoureterotomy in 7 patients. All the patients underwent to radical cistectomy with urinary diversion for oncological disease. The diagnosis has been obtained with US and/or CT examination. Residual renal function has been assessed with sequential renal scintigraphy. The procedure includes two parts: percutaneous pleacement of nephrostomic catheter with distal part inside the neo-bladder and, after 15 days, treatment with Acucise®. The incision was performed with 75W activation of the cutting wire during inflation of the balloon at the level of stenosis.Results Mean follow-up is about 5 months so we can't consider the long term effects of the treatment but our preliminary results are interesting, with an adequate patency of the anastomoses in 8/9 cases at CT scan. The procedure, performed under fluoroscopic control and local anaesthesia, resulted ease and safe with low procedural complications.Conclusions If long term results will be confirm, Acucise® could be suggested as first-line treatment in these situations; surgery should be indicated in case of minimally invasive procedure approach.

In this report we describe an Italian patient with chronic T cell leukemia whose proliferating cells were mature T lymphocytes with a helper phenotype (T helper phenotype chronic lymphocytic leukemia, or Thp-CLL). Unlike other reported cases of Thp-CLL, fresh leukemic cells from this patient were positive with the anti-Tac monoclonal antibody, which recognizes the receptor for interleukin-2 (IL-2). Thus, the phenotype of these cells was similar to that expressed by Japanese patients with adult T cell leukemia (ATL) (OKT3+, OKT4+, OKT8-, Tac+). However, the Italian patient had Thp-CLL, not ATL, since his cells, unlike ATL cells, lacked human T cell leukemia virus (HTLV-I)-related DNA sequences. The Tac receptor, which appears to be modulated in vitro by the anti-Tac antibody, was biologically inactive, since the patient's cells did not respond in vitro to IL-2. In addition, they also failed to demonstrate in vitro functional activities. The clinical course was aggressive, as usual, for both Thp-CLL and ATL. Taking advantage of the description of this case, some similarities and differences between Thp-CLL and ATL are discussed, focusing on the importance of screening of HTLV-I in the differential diagnosis.

The aim of our study was to evaluate the performance and efficacy of a new self-expanding stent (nitinol Strecker stent) in the transjugular intrahepatic portosystemic shunt (TIPS) procedure. We have successfully placed 64 nitinol Strecker stents in 48 patients. The average portosystemic gradient decreased from 22 to 11 mm Hg. Balloon dilatation was necessary in 12 of 35 angiographically controlled cases at 5 days (34%), because of incomplete stent expansion, small thrombi within the stent or obstruction. At 1–6 months stent malfunctions occurred in 8 of 23 patients who underwent control angiography (34%) and at 6–24 months in 6 of 7 patients (85%). Rebleeding occurred in 2 of 39 patients (follow-up > 1 month) (5%) and temporary crises of de novo encephalopathy were observed in 11 of 48 patients (23%). Refractory ascites completely resolved in 4 of 6 patients (66%) and improved in the remaining 2 cases. Compared with other self-expanding stents, nitinol Strecker stents seem to be equally effective in TIPS; no increase in complication rate was observed, either clinical or stent-related.

SummarySixty-three patients, suffering from osteoarthrosis, rheumatoid arthritis and allied disorders, were admitted to a multi-centre, open study using 500 mg. ibuprofen suppositories. The results indicate that this is an effective form of therapy, night pain being relieved in 75% of the patients. Proctoscopic examinations carried out during the study confirmed good local tolerance.
