Le sirolimus est un traitement des malformations vasculaires de bas débit (MVBD). L’étude SIROLO visait à évaluer les effets à long terme et la prise en charge en vraie vie du sirolimus oral pour les MVBD en examinant les données de 15 centres français.

Background: Cerebral venous thrombosis (CVT) is a rare cause of stroke, with 10–15% of patients experiencing dependence or death. The role of endovascular therapy (EVT) in the management of CVT remains controversial and practice patterns are not well-known. Methods: We distributed a comprehensive 53-question survey to neurologists, neuro-interventionalists, neurosurgeons and other relevant clinicians globally from May 2023 to October 2023. The survey asked about practice patterns and perspectives on EVT for CVT and assessed opinions regarding future clinical trials. Results: The overall response rate was 31% (863 respondents from 2744 invited participants) across 61 countries. A majority (74%) supported use of EVT for certain CVT cases. Key considerations for EVT included worsening level of consciousness (86%) and other clinical deficits (76%). Mechanical thrombectomy with aspiration (22%) and stent retriever (19%) were the most utilized techniques, with regional variations. Post-procedurally, low molecular weight heparin was the predominant anticoagulant administered (40%), although North American respondents favored unfractionated heparin. Most respondents supported future trials of EVT (90%). Conclusions: Our survey reveals significant heterogeneity in approaches to EVT for CVT, highlighting the necessity for adequately powered clinical trials to guide standard-of-care practices.

Sur la décennie précédant la crise COVID-19 étaient décrites une stabilisation ou diminution d'incidence chez les plus âgés et une augmentation chez les plus jeunes. L'objectif était d'actualiser les données épidémiologiques hospitalières des AVC et accidents ischémiques transitoires (AIT) au sein d'une région, le Centre-Val de Loire (CVL).

Cerebral amyloid angiopathy (CAA) is a frequent age-related small vessel disease (SVD) with cardinal magnetic resonance imaging (MRI) signatures that are hemorrhagic in nature, and include the presence of strictly lobar (superficial) cerebral microbleeds and intracerebral hemorrhages as well as cortical superficial siderosis. When investigating a patient with suspected CAA in the context of intracranial hemorrhage (parenchymal or subarachnoid) or cognitive dysfunction, various MRI parameters influence the optimal detection and characterization (and prognostication) of this frequent SVD. The present report describes the influence of imaging techniques on the detection of the key hemorrhagic CAA imaging signatures in clinical practice, in research studies, and the imaging parameters that must be understood when encountering a CAA patient, as well as reviewing CAA literature.

The management of patients with unruptured intracranial aneurysms (UIAs) is a complex clinical challenge and constitutes an immense field of research. While a preponderant proportion of these aneurysms never rupture, the consequences of such an event are severe and represent an important healthcare problem. To date, however, the natural history of UIAs is not completely understood and there is no accurate means to discriminate the UIAs that will rupture from those that will not. Yet, a good understanding of the recent evidence and future perspectives is needed when advising a patient with IA to tailor any information to the given patient's level of risk and psychoaffective status. Thus, this review addresses the current concepts of epidemiology, risk factors, detection and management of UIAs.

Les anévrismes intracrâniens doivent être recherchés en cas d’hémorragie sous-arachnoïdienne et dans cette situation, l’angioscanner est la technique privilégiée. Dans le cadre de formes familiales ou de maladies du tissu élastique, un dépistage peut être organisé et l’angiographie par résonance magnétique est alors l’examen de première intention. Ces méthodes non invasives ont aujourd’hui supplanté l’angiographie conventionnelle qui est réservée à la phase préthérapeutique. Une bonne connaissance technique de ces méthodes d’imagerie, des artefacts et des images pièges permettent une détection fiable des anévrismes intracrâniens et la réalisation d’un compte rendu précis à transmettre aux cliniciens.

L’hydrocéphalie est un facteur pronostic indépendant et modifiable de mauvais devenir neurologique après une hémorragie intracérébrale (HIC). Chez l’enfant, les malformations artério-veneuses cérébrales (MAVc) sont la cause plus fréquente d’HIC. Dans une large cohorte prospective d’enfants avec des MAVc rompues, nous avons analysé les facteurs prédictifs d’hydrocéphalie nécessitante une dérivation ventriculaire externe (DVE) dans la phase aiguë ou une dérivation ventriculo-péritonéale (DVP).
