Background. The assessment of physical activity is of concern in patients after total hip arthroplasty (THA). However, so far, no questionnaire has demonstrated adequate reproducibility and validity for assessing physical activity in these patients. Objective. The aim of this study was to evaluate the reproducibility and validity of the Physical Activity Scale for the Elderly (PASE) questionnaire in patients after THA. Design. This was a measurement study. Methods. Fifty patients who had undergone THA (25 women, 25 men), with an average age of 68 years, were evaluated. Of these patients, 25 were assessed between 2 and 7 months after surgery ([THA.sub.early]), and another 25 were assessed between 7 and 12 months after surgery ([THA.sub.late]). Reproducibility of the PASE questionnaire was evaluated by administering the questionnaire on 2 different occasions. Construct validity of the PASE questionnaire was assessed by comparing the physical activity level reported by patients with that objectively recorded by a body-mounted accelerometer. Reproducibility was investigated with intraclass correlation coefficients (ICC [2,1]) for reliability and standard errors of measurement (SEM) for agreement. Validity was investigated with Pearson correlation coefficients (r). Results. The ICC (2,1) for the PASE total score was .77 (95% confidence interval [95% CI] = .63, .86); the SEM was 23.0% (95% CI=19.2, 28.7). Validity correlation for the PASE total score was .38 (95% CI=.12, .60). No significant differences were found between [THA.sub.early] and [THA.sub.late] groups for reliability, agreement, and validity outcomes. Limitations. Reproducibility of the PASE questionnaire may have been underestimated because the physical activity of patients was compared between 2 consecutive but different weeks. Reliability and validity analyses were underpowered. Conclusions. Further study with a larger sample size is necessary to obtain precise reliability and validity estimates. To appraise the highest available evidence provided by randomized controlled trials (RCTs) on the effectiveness of hip arthroscopy versus physical therapy in patients with femoroacetabular impingement syndrome (FAIS). Four databases (Medline, Embase, Web of Science, and Scopus) were systematically searched until October 1, 2019. Eligible studies were RCTs in which patients with FAIS underwent hip arthroscopy or physical therapy. The study outcome was the International Hip Outcome Tool, 33 Items (iHOT‐33) score, a measure of hip pain, function, and quality of life, assessed at baseline and at the follow‐up closer to 12 months after randomization. The pooled mean difference in iHOT‐33 scores within and between the treatment arms was computed using a random effects model. The minimum clinically important difference in the iHOT‐33 scores was set at 10 points. Three RCTs evaluating iHOT‐33 scores between 6 and 8 months after the interventions were included. Significant increases in iHOT‐33 scores were observed from baseline to follow‐up for both hip arthroscopy (22.3 points [95% confidence interval (95% CI) 17.3–27.4]) and physical therapy (13.0 points [95% CI 9.5–16.4]). Hip arthroscopy demonstrated significantly higher iHOT‐33 scores at follow‐up compared with physical therapy (10.9 points [95% CI 4.7–17.0]). Both hip arthroscopy and physical therapy resulted in statistically and clinically significant short‐term improvements in hip pain, function, and quality of life in patients with FAIS. Hip arthroscopy was statistically superior to physical therapy in improving the outcome at follow‐up even if improvement may not be detected by patients.

Background: Deformities of the femoral head-neck junction are associated with limited hip internal rotation, which may lead to femoroacetabular impingement and consequently to hip osteoarthritis. This study compared inter- and intra-observer reproducibility of 3 different methods to quantify hip internal rotation.Methods: 2 investigators assessed hip internal rotation of 30 asymptomatic participants during 2 separate testing sessions. Internal rotation was assessed by rotating the 90°-flexed hip manually while in a supine position (manual), in an examination chair capable of applying a single load (EC1) and in a newly developed examination chair with 5 load levels (EC2). Inter- and intra-observer reproducibility was compared among methods using reliability (intra-class correlation coefficient, ICC) and measurement error (smallest detectable chance).Results: Inter-observer reliability was good for the manual assessment (ICC = 0.83) and excellent for the EC1 and EC2 methods (ICC ⩾ 0.95) with expected measurement errors of 15.9°, 7.1° and 6.8°-14.3°, respectively. Intra-observer reliability was excellent for each method (ICC ⩾ 0.96), although measurement error ranged from 7.6°-11.8° for EC2 and was slightly higher compared to the manual (7.8°) and EC1 (5.9°) methods.Conclusions: Reproducibility of EC2 hip internal rotation angle assessment is superior to that of the manual assessment at specific load levels but not to the EC1 method. Future assessment devices need to incorporate a means of precisely producing and quantifying the loads applied to the hip joint in order to improve measurement reproducibility.

Hip-related pain can significantly impact quality of life, function, work capacity, physical activity and family life. Standardised measurement methods of physical capacity of relevance to young and middle-aged active adults with hip-related pain are currently not established. The aim of this consensus paper was to provide recommendations for clinical practice and research on standardised measurement methods of physical capacity in young and middle-aged active adults with hip-related pain. Four areas of importance were identified: (1) clinical measures (range of motion, muscle strength, functional impairments), (2) laboratory-based measures (biomechanics and muscle function (muscle activity, size and adiposity)), (3) physical activity, and (4) return to sport/performance. The literature was reviewed, and a summary circulated to the working group to inform discussion at the consensus meeting. The working group developed clinical and research recommendations from the literature review, which were further discussed and modified within the working group at the consensus meeting. These recommendations were then presented to all 38 International Hip-related Pain Research Network (IHiPRN) participants for further discussion, refinement and consensus voting. Therefore, the recommendations voted on were based on a combination of current evidence and expert opinion. The consensus meeting voted on 13 recommendations, six of which were clinically orientated, and seven more research specific. We recommended that clinicians working with young and middle-aged active adults with hip-related pain assess strength using objective methods of measurement, and clinically assess performance of functional tasks, including walking and running. Physical activity should be quantified using both self-reported and objective measures, and patient expectations of recovery should be quantified prior to treatment. It was recommended that return to physical activity (including sport and occupation) be quantified, and sport-specific activities should be assessed prior to return to sport. The IHiPRN participants were uncertain regarding recommendations for range of motion assessment. Research recommendations were that the measurement properties of range of motion, strength and functional performance tests be investigated, reported and improved in both clinical and research settings. Reporting of movement-related parameters (biomechanics and muscle function) should be standardised and the relationship among movement-related parameters, symptoms, function, quality of life, and intra-articular and imaging findings should be investigated. Quantification of return to physical activity (including sport and occupational demands) is required in future research, and the return to sport continuum should be used. Future research is required to determine the best criteria for rehabilitation progression and return to physical activity following hip-related pain management.

There is no agreement on how to classify, define or diagnose hip-related pain—a common cause of hip and groin pain in young and middle-aged active adults. This complicates the work of clinicians and researchers. The International Hip-related Pain Research Network consensus group met in November 2018 in Zurich aiming to make recommendations on how to classify, define and diagnose hip disease in young and middle-aged active adults with hip-related pain as the main symptom. Prior to the meeting we performed a scoping review of electronic databases in June 2018 to determine the definition, epidemiology and diagnosis of hip conditions in young and middle-aged active adults presenting with hip-related pain. We developed and presented evidence-based statements for these to a panel of 37 experts for discussion and consensus agreement. Both non-musculoskeletal and serious hip pathological conditions (eg, tumours, infections, stress fractures, slipped capital femoral epiphysis), as well as competing musculoskeletal conditions (eg, lumbar spine) should be excluded when diagnosing hip-related pain in young and middle-aged active adults. The most common hip conditions in young and middle-aged active adults presenting with hip-related pain are: (1) femoroacetabular impingement (FAI) syndrome, (2) acetabular dysplasia and/or hip instability and (3) other conditions without a distinct osseous morphology (labral, chondral and/or ligamentum teres conditions), and that these terms are used in research and clinical practice. Clinical examination and diagnostic imaging have limited diagnostic utility; a comprehensive approach is therefore essential. A negative flexion–adduction–internal rotation test helps rule out hip-related pain although its clinical utility is limited. Anteroposterior pelvis and lateral femoral head–neck radiographs are the initial diagnostic imaging of choice—advanced imaging should be performed only when requiring additional detail of bony or soft-tissue morphology (eg, for definitive diagnosis, research setting or when planning surgery). We recommend clear, detailed and consistent methodology of bony morphology outcome measures (definition, measurement and statistical reporting) in research. Future research on conditions with hip-related pain as the main symptom should include high-quality prospective studies on aetiology and prognosis. The most common hip conditions in active adults presenting with hip-related pain are: (1) FAI syndrome, (2) acetabular dysplasia and/or hip instability and (3) other conditions without distinct osseous morphology including labral, chondral and/or ligamentum teres conditions. The last category should not be confused with the incidental imaging findings of labral, chondral and/or ligamentum teres pathology in asymptomatic people. Future research should refine our current recommendations by determining the clinical utility of clinical examination and diagnostic imaging in prospective studies.

Hip-related pain is a well-recognised complaint among active young and middle-aged active adults. People experiencing hip-related disorders commonly report pain and reduced functional capacity, including difficulties in executing activities of daily living. Patient-reported outcome measures (PROMs) are essential to accurately examine and compare the effects of different treatments on disability in those with hip pain. In November 2018, 38 researchers and clinicians working in the field of hip-related pain met in Zurich, Switzerland for the first International Hip-related Pain Research Network meeting. Prior to the meeting, evidence summaries were developed relating to four prioritised themes. This paper discusses the available evidence and consensus process from which recommendations were made regarding the appropriate use of PROMs to assess disability in young and middle-aged active adults with hip-related pain. Our process to gain consensus had five steps: (1) systematic review of systematic reviews; (2) preliminary discussion within the working group; (3) update of the more recent high-quality systematic review and examination of the psychometric properties of PROMs according to established guidelines; (4) formulation of the recommendations considering the limitations of the PROMs derived from the examination of their quality; and (5) voting and consensus. Out of 102 articles retrieved, 6 systematic reviews were selected and assessed for quality according to AMSTAR 2 (A MeaSurement Tool to Assess systematic Reviews). Two showed moderate quality. We then updated the most recent review. The updated literature search resulted in 10 additional studies that were included in the qualitative synthesis. The recommendations based on evidence summary and PROMs limitations were presented at the consensus meeting. The group makes the following recommendations: (1) the Hip and Groin Outcome Score (HAGOS) and the International Hip Outcome Tool (iHOT) instruments (long and reduced versions) are the most appropriate PROMs to use in young and middle-aged active adults with hip-related pain; (2) more research is needed into the utility of the HAGOS and the iHOT instruments in a non-surgical treatment context; and (3) generic quality of life measures such as the EuroQoL-5 Dimension Questionnaire and the Short Form Health Survey-36 may add value for researchers and clinicians in this field. We conclude that as none of the instruments shows acceptable quality across various psychometric properties, more methods studies are needed to further evaluate the validity of these PROMS—the HAGOS and iHOT—as well as the other (currently not recommended) PROMS.

To investigate the responsiveness to exercise therapy of patients with femoroacetabular impingement syndrome (FAIS), and to investigate differences in hip function, strength, and morphology between responders versus nonresponders. Patients with FAISunderwent 12 weeks of semi‐standardized and progressive exercise therapy. A good therapy outcome (responders) versus a poor therapy outcome (nonresponders) was determined at week 18 with the Global Treatment Outcome questionnaire for hip pain. Hip function was evaluated using the Hip Outcome Score (HOS) activities of daily living (ADL) and Sport at baseline, and at weeks 6, 12, and 18. Hip muscle strength and dynamic pelvic control were evaluated using dynamometry and video analysis, respectively, at baseline, week 12, and week 18. Hip morphology was evaluated with imaging at baseline. Thirty‐one patients (mean age 24 years) were included. Sixteen patients (52%) were responsive and 15 patients (48%) were not responsive to exercise therapy. Only responders improved HOS ADLand HOSSport by 10 points (95% confidence interval [95% CI] 7, 14; P< 0.001) and by 20 points (95% CI15, 25; P< 0.001), respectively, and hip abductor strength by 0.27 Nm/kg (95% CI0.18, 0.36; P< 0.001). The prevalence of patients showing good dynamic pelvic control only increased in responders (44%; P= 0.029). The prevalence of severe cam morphology was higher in nonresponders than in responders (40% versus 6%; P= 0.037). Half of patients with FAISbenefit from exercise therapy in the short term. Responsiveness to hip abductor strength and dynamic pelvic control improvements is associated with a good therapy outcome, whereas the presence of severe cam morphology is associated with a poor therapy outcome.

ObjectiveTo identify which exercise combinations are most effective as part of a lower extremity injury prevention programme for team-sport athletes.DesignUmbrella review.Data sourcesA comprehensive literature search was performed in PubMed, Scopus, Cochrane Library and PEDro databases. Studies published between January 2000 and March 2017 were included in this umbrella review.Study eligibility criteriaModerate to high-quality systematic reviews that investigated the effectiveness of a combination of two or more exercise components, that is, strength, agility, plyometrics, balance, stretching, technique, warm-up and functional activity, regarding injury incidence/rate of lower extremity injuries in team-sport athletes. The methodological quality of the included systematic reviews was independently assessed by two reviewers using the Assessing the Methodological Quality of Systematic Reviews measurement tool and the Grading of Recommendations Assessment, Development and Evaluation guidelines were used to assess the overall quality of evidence for particular outcomes.ResultsTwenty-four systematic reviews met the inclusion criteria. Multicomponent exercise interventions were effective in reducing the injury incidence/rate of lower extremity, knee, ACL and ankle injuries, but not groin injuries. Strength and balance exercise components were included in 10 of 11 effective injury prevention programmes for the lower extremity, knee, ACL and ankle injuries.Summary/conclusionLower extremity injury prevention programmes in team sports are effective in preventing lower extremity, knee, ACL and ankle injuries. Lower extremity muscle strength and balance exercises should be prioritised in lower extremity injury prevention programmes for team-sport athletes.

AimTo investigate pain, activities of daily living (ADL) function, sport function, quality of life and satisfaction at different time points after hip arthroscopy in patients with femoroacetabular impingement (FAI).DesignSystematic review with meta-analysis. Weighted mean differences between preoperative and postoperative outcomes were calculated and used for meta-analysis.Data sourcesEMBASE, MEDLINE, SportsDiscus, CINAHL, Cochrane Library, and PEDro.Eligibility criteria for selecting studiesStudies that evaluated hip pain, ADL function, sport function and quality of life before and after hip arthroscopy and postoperative satisfaction in patients with symptomatic FAI.ResultsTwenty-six studies (22 case series, 3 cohort studies, 1 randomised controlled trial (RCT)) were included in the systematic review and 19 in the meta-analysis. Clinically relevant pain and ADL function improvements were first reported between 3 and 6 months, and sport function improvements between 6 months and 1 year after surgery. It is not clear when quality of life improvements were first achieved. On average, residual mild pain and ADL and sport function scores lower than their healthy counterparts were reported by patients following surgery. Postoperative patient satisfaction ranged from 68% to 100%.ConclusionsOn average, patients reported earlier pain and ADL function improvements, and slower sport function improvements after hip arthroscopy for FAI. However, average scores from patients indicate residual mild hip pain and/or hip function lower than their healthy counterparts after surgery. Owing to the current low level of evidence, future RCTs and cohort studies should investigate the effectiveness of hip arthroscopy in patients with FAI.Trial registration numberCRD42015019649.

Purpose The aim of the study was to prospectively evaluate hip muscle strength in a series of patients with symptomatic FAI after hip arthroscopy.Methods Hip muscle strength of eight patients (age: 29 ± 10 years) was evaluated preoperatively and 2.5 years after hip arthroscopy, and was compared to eight matched controls. Maximal voluntary contraction (MVC) strength was measured for all hip muscle groups. At follow-up, we used the symptom-specific well-being outcome to assess the acceptability of the health state related to the hip.Results Patients showed MVC strength increases for all hip muscles (9-59%, P<.05). At follow-up, only hip flexor MVC strength was lower for patients than controls (-18%, P<.05). At follow-up, four patients (out of eight) were “neither satisfied nor dissatisfied” with the health state of their operated hip.Conclusions Patients with symptomatic FAI recovered their hip muscle strength to normal levels 2.5 years after hip arthroscopy, except for hip flexors. Although all patients showed good hip muscle strength at follow-up, half of them were not completely satisfied with their health state related to the hip.
