La transplantation pulmonaire (TxP) constitue le traitement ultime de l’insuffisance respiratoire terminale quelle qu’en soit l’origine et voit son activité en permanente expansion. Le travail minutieux des centres de TxP portant sur la sélection des donneurs et des receveurs d’un greffon pulmonaire ainsi que de l’ensemble des efforts fournis pour relever les défis de la prise en charge chirurgicale, péri-opératoire et la gestion des complications médicales de la TxP à plus long terme ont permis une augmentation du nombre de procédures et l’amélioration du pronostic post-TxP. Les membres du groupe de travail de la Société de pneumologie de langue française (SPLF) ont réalisé une mise à jour des données connues de la littérature et analysé les facteurs de risque qui limitent les chances de survie après la TxP. L’objectif de ce travail a été de guider les pneumologues dans leur prise en charge de l’insuffisance respiratoire terminale, de les aider à identifier les patients potentiellement éligibles à la TxP et de déterminer selon quels délais les adresser à centre de TxP. Les objectifs de la TxP demeurent l’allongement de la vie et l’amélioration de la qualité de vie des malades. Les propositions faites dans ce document portent sur une ressource limitée et restent guidées par des principes éthiques décrits plus loin.

Bronchiolitis obliterans syndrome is the main limitation for long‐term survival after lung transplantation. Some specific B cell populations are associated with long‐term graft acceptance. We aimed to monitor the B cell profile during early development of bronchiolitis obliterans syndrome after lung transplantation. The B cell longitudinal profile was analyzed in peripheral blood mononuclear cells from patients with bronchiolitis obliterans syndrome and patients who remained stable over 3 years of follow‐up. CD24hiCD38hitransitional B cells were increased in stable patients only, and reached a peak 24 months after transplantation, whereas they remained unchanged in patients who developed a bronchiolitis obliterans syndrome. These CD24hiCD38hitransitional B cells specifically secrete IL‐10 and express CD9. Thus, patients with a total CD9+B cell frequency below 6.6% displayed significantly higher incidence of bronchiolitis obliterans syndrome (AUC = 0.836, PPV = 0.75, NPV = 1). These data are the first to associate IL‐10‐secreting CD24hiCD38hitransitional B cells expressing CD9 with better allograft outcome in lung transplant recipients. CD9‐expressing B cells appear as a contributor to a favorable environment essential for the maintenance of long‐term stable graft function and as a new predictive biomarker of bronchiolitis obliterans syndrome–free survival. In lung transplant patients with bronchiolitis obliterans syndrome and patients who remained stable over 3 years of follow‐up, IL‐10–secreting CD24hiCD38hi transitional B cells expressing CD9 are associated with better allograft outcome, suggesting CD9‐expressing B cells as a new predictive biomarker of bronchiolitis obliterans syndrome–free survival.

Chronic lung allograft dysfunction (CLAD) is the major limitation of long‐term survival after lung transplantation. CLADmanifests as bronchiolitis obliterans syndrome (BOS) or restrictive allograft syndrome (RAS). Alloimmune reactions and epithelial‐to‐mesenchymal transition have been suggested in BOS. However, little is known regarding the role of allogenicity in epithelial cell differentiation. Primary human bronchial epithelial cells (BECs) were treated with activated T cells in the presence or absence of transforming growth factor (TGF)‐β. The expression of epithelial and mesenchymal markers was investigated. The secretion of inflammatory cytokines and matrix metalloproteinase (MMP)‐9 was measured in culture supernatants and in plasma from lung transplant recipients (LTRs): 49 stable, 29 with BOS,and 16 with RAS. We demonstrated that C‐C motif chemokine 2 secreted by T cells supports TGF‐β–induced MMP‐9 production by BECsafter binding to C‐C chemokine receptor type 2. Longitudinal investigation in LTRsrevealed a rise in plasma MMP‐9 before CLADonset. Multivariate analysis showed that plasma MMP‐9 was independently associated with BOS(odds ratio [OR] =6.19, p = 0.002) or RAS(OR= 3.9, p = 0.024) and predicted the occurrence of CLAD12 months before the functional diagnosis. Thus, immune cells support airway remodeling through the production of MMP‐9. Plasma MMP‐9 is a potential predictive biomarker of CLAD. The authors investigate the production of matrix metalloproteinase‐9 by primary bronchial epithelial cells after interaction with activated T cells and show that plasma matrix metalloproteinase‐9 can serve as a predictor of chronic lung allograft dysfunction 12 months before clinical diagnosis.

Information about the prevalence and nature of liver disorders in adults with alpha1-antitrypsin deficiency is scarce. At our center, systematic liver biopsy screening is part of the evaluation before lung transplantation (LT) in the emphysema patients with the PiZZ phenotype. Our aim was to report our experience with this prospective screening. Clinical, liver function, and imaging parameters as well as liver histology data were analyzed for 23 consecutive adult patients with PiZZ severe emphysema referred to our center for consideration of LT from 2006 to 2014. Overall 20 (87%) featured chronic liver disease characterized by a chronic inflammation and/or a significant portal fibrosis on histology. Two of the 23 patients (8.7%) had septal fibrosis according to the Metavir and Ishak scores and met our definition of severe chronic liver disease. They were both clinically asymptomatic with normal liver function tests. On abdominal ultrasonography, the liver appeared normal in one patient and with abnormal contours in the other. Our data indicate that in adults with PiZZ-related emphysema being evaluated for LT, most patients had some histologic involvement. The prevalence of severe liver dysfunction is <10%.

Le turn-overdu personnel infirmier est important dans les hôpitaux et les sous-effectifs chroniques rendent compliquée la formation continue du personnel. Celle-ci est fondamentale quand elle touche des secteurs spécialisés nécessitant des connaissances adaptées. C’est le cas dans notre hôpital où sont réalisées 50 transplantations pulmonaires par an et le suivi de 400 patients transplantés.

Human leukocyte antigen G (HLA-G) expression is thought to be associated with a tolerance state following solid organ transplantation. In a lung transplant (LTx) recipient cohort, we assessed (1) the role of HLA-G expression as a predictor of graft acceptance, and (2) the relationship between (i) graft and peripheral HLA-G expression, (ii) HLA-G expression and humoral immunity and (iii) HLA-G expression and lung microenvironment. We prospectively enrolled 63 LTx recipients (median follow-up 3.26 years [min: 0.44–max: 5.03]). At 3 and 12 months post-LTx, we analyzed graft HLA-G expression by immunohistochemistry, plasma soluble HLA-G (sHLA-G) level by enzyme–linked immunosorbent assay, bronchoalveolar lavage fluid (BALF) levels of cytokines involved in chronic lung allograft dysfunction (CLAD) and anti-HLA antibodies (Abs) in serum. In a time-dependent Cox model, lung HLA-G expression had a protective effect on CLAD occurrence (hazard ratio: 0.13 [0.03–0.58]; p = 0.008). The same results were found when computing 3-month and 1-year conditional freedom from CLAD (p = 0.03 and 0.04, respectively [log-rank test]). Presence of anti-HLA Abs was inversely associated with graft HLA-G expression (p = 0.02). Increased BALF level of transforming growth factor-β was associated with high plasma sHLA-G level (p = 0.02). In conclusion, early graft HLA-G expression in LTx recipients with a stable condition was associated with graft acceptance in the long term.

La survie après transplantation pulmonaire (TxP) reste limitée par la survenue d’une dysfunction chronique pulmonaire (CLAD), qui touche >50 % des greffés. La forme obstructive de CLAD, bronchiolite oblitérante (BO)/syndrome de BO (SBO), est le phénotype le plus fréquent de CLAD (80% des cas). Les lésions de BO semblent dues à des agressions répétées de l’épithélium bronchique avec développement d’une cicatrice fibreuse. Il n’existe actuellement pas de traitement démontré pour stabiliser le SBO. Le rôle crucial de la dysfonction de réparation fibrotique est démontré dans la BO via le remodellage bronchique, l’augmentation de matrice extra-cellulaire, la transition épithélio-mésenchymateuse, l’augmentation des facteurs de croissance PDGF, VEGF, FGF, IGF- Le TKI Nintedanib, démontré efficace dans la fibrose pulmonaire idiopathique, et qui cible ces facteurs de croissance, est une molécule candidate. Un essai thérapeutique testant le nintedanib dans la BOS post-TxP a débuté en France fin 2019.

Veno-occlusive disease (VOD) of the liver is mainly described after chemo-irradiation conditioning regimens during haematopoietic stem cell transplantation (SCT) and has been sporadically reported after kidney and liver transplantation. In the latter cases, it is commonly attributed to azathioprine andor tacrolimus. One case of tacrolimus-induced hepatic VOD developing after lung transplantation (LT) has been recently reported. Here we describe another case of VOD occurring after LT, but in which the causative role was played by azathioprine.

La vaccination contre la Covid a été recommandée chez les patients transplantés d’organes solides en début d’année 2021, selon un schéma comprenant 3 doses chez les patients n’ayant pas présenté d’infection par la Covid 19 et selon un schéma comprenant 2 injections chez les patients ayant été infectés par la Covid.

La transplantation (TxP) est le seul traitement curatif de la fibrose pulmonaire idiopathique (FPI). Des traitements antifibrosants (pirfenidone et nintedanib), qui ralentissent l’évolution de la maladie, ont été développés récemment. Ils pourraient augmenter le risque de complications post-opératoires lors de la TxP des patients traités. L’objectif de cette étude était d’évaluer l’association entre la prise d’un traitement antifibrosant et les complications après une TxP.

BackgroundPneumocystis carinii pneumonia (PCP) is an uncommon severe complication in immunocompromised patients. There is only one marketed treatment to prevent PCP: trimethoprime-sulfamethoxazole (TMP-SMZ). Due to side-effects (mainly neutropaenia), TMP-SMZ is sometimes replaced by off-label drugs. Notably, atovaquone has been studied in several conditions, but its effectiveness has never been assessed in lung transplantation.PurposeOur study intended to compare the effectiveness of TMP-SMZ and atovaquone for preventing PCP in lung-transplant recipients.Material and methodsThis single-centre, retrospective study included deceased and living patients, who received a lung transplant from 1 January 2007 to 31 August 2016, and PCP prophylaxis for more than 1 year. Inclusion in the groups was based on treatment at the time of PCP or death or, failing that, on 31 August 2017. Initially all patients were treated with TMP-SMZ (daily dosage regarding renal function and toxoplasmosis status). This treatment could be continued (group 1) or, if side-effects appeared, switched to atovaquone 750 mg or 1,500 mg daily (group 2 and 3, respectively, according to the prescriber’s choice). The reasons for prescribing atovaquone were assessed.ResultsTwo hundred and ten patients were included in the study. Death rate was 23% in group 1 (n=160), 37% in group 2 (n=41) and 20% in group 3 (n=9), but group 2 patients were transplanted in an earlier period than group 3 (2007 to 2014 vs 2013 to 2016). No patient directly died from PCP. Before switching to atovaquone, patients were treated with TMP-SMZ for 0.5 years (0.0; 4.2) (group 2) and 0.4 years (0.1; 1.4) (group 3). The main reason for stopping TMP-SMZ was haematological toxicity (63% of cases). Only 1 patient (group 2), taking the treatment once a week developed PCP.ConclusionThis is the first study evaluating atovaquone’s effectiveness in lung-transplant recipients. It seems to be effective, considering that the unique case of PCP was due to poor compliance. These retrospective results have to be confirmed. Because of it high cost and gastrointestinal effects that may affect treatment adhesion, atovaquone should be saved only for patients with TMP-SMZ intolerance.No conflict of interest

L’obésité et la dénutrition des patients en attente de greffe, définis par l’indice de masse corporelle (IMC), sont actuellement considérés comme des contre-indications relatives à la transplantation pulmonaire. L’albuminémie est un autre marqueur de dénutrition. L’objectif de l’étude est d’évaluer l’impact de l’état nutritionnel sur les résultats de la transplantation pulmonaire dans notre centre.

Un sommeil de mauvaise qualité est souvent rapporté par les patients en attente ou déjà greffés pulmonaires (LTX), lié aux traitement et comorbidités. Néanmoins les troubles du sommeil restent sous-diagnostiqués, y compris les syndromes d’apnées du sommeil. Le but de ce travail a été de d’évaluer de façon prospective le sommeil et les troubles respiratoires du sommeil d’une cohorte française monocentrique de candidats LTX.

Les infections respiratoires basses d’origine virale sont fréquentes chez les patients transplantés pulmonaires et cardiopulmonaires. Leurs prises en charge diagnostique et thérapeutique sont mal codifiées. L’objectif de cette étude était d’évaluer les pratiques diagnostique et thérapeutique des centres de transplantation français.

En cas de déficit en α antitrypsine (AAT) par mutation ZZ, l’accumulation d’AAT anormale dans le reticulum endoplasmique des hépatocytes peut être à l’origine d’une maladie hépatique parfois sévère. Peu de données sont disponibles sur la prévalence et la sévérité de la maladie chez l’adulte. Dans notre centre où une ponction biopsie hépatique (PBH) est réalisée de façon systématique en sus du bilan hépatique standard chez tous les patients présentant un emphysème par déficit en AAT de type ZZ, évalués pour une transplantation pulmonaire (TP), les données cliniques, biologiques, échographiques et histologiques de 23 patients adultes PiZZ ont été analysées.

La transplantation pulmonaire consiste à implanter un greffon pulmonaire d’un donneur dans la cage thoracique d’un receveur. On dispose de peu d’informations concernant la taille optimale du greffon à implanter chez un receveur donné. Ceci est particulièrement vrai chez les patients porteurs de fibrose pulmonaire idiopathique (FPI), cas de figure dans lequel le volume de la cage thoracique est très bas. L’objectif de cette étude est d’étudier l’impact de l’inadéquation de taille entre donneurs et receveurs sur les complications à court- et moyen terme, les résultats fonctionnels respiratoires et la survie chez les patients greffés pour FPI. De janvier 2007 à janvier 2014, 94 transplantations pulmonaires pour FPI ont été réalisées. Suivant les recommandations actuelles, nous avons utilisé le rapport CPT théorique du donneur/CPT théorique du receveur comme marqueur de congruence de taille. Parmi les 94 patients greffés dans notre centre, 48 ont bénéficié d’un greffon surdimensionné (ratio>1) et 46 d’un greffon sous dimensionné (ratio<1). Nous avons comparé ces deux groupes en termes de survie, de complications postopératoires et de résultats fonctionnels. Le ratio de CPT théorique entre donneur et receveur est compris entre 0,63 et 1,68. Les patients ayant bénéficié d’un grand poumon (ratio>1) ont un taux de complications, un taux de survie et des résultats fonctionnels (VEMS, capacité vitale) similaires aux patients de l’autre groupe. En conclusion, l’acceptation d’un greffon de grand volume en transplantation pulmonaire pour FPI permettrait d’élargir l’accès à la greffe de ces patients.

L’expression de HLA-G (HUMAN LEUKOCYTE ANTIGen G) semble être associée à un état de tolérance après greffe d’organe solide. Nous avons évalué dans une cohorte de transplantés pulmonaires (TxP), (1) le rôle de l’expression de HLA-G comme marqueur prédictif d’acceptation du greffon, et (2) l’association entre (i) l’expression de HLA-G au sein du greffon et périphérique, (ii) l’expression de HLA-G et l’immunité humorale, et (iii) l’expression de HLA-G et le microenvironement pulmonaire. Soixante-trois patients TxP ont été inclus prospectivement (suivi médian 3,26années [min : 0,44–max : 5,03]). À 3 et 12mois post-Tx, nous avons analysé l’expression de HLA-G au sein du greffon (immuno-histochimie), le dosage de la forme soluble de HLA-G (sHLA-G) plasmatique par ELISA, et le profil cytokinique dans le lavage bronchoalvéolaire (LBA), ainsi que les anticorps anti-HLA dans le sérum. Dans un modèle de Cox temps-dépendant, l’expression pulmonaire de HLA-G avait un effet protecteur sur la survenue ultérieure d’une dysfonction chronique du greffon (CLAD) (HR :0,13 [0,03–0,58] ; p=0,008). Les mêmes résultats étaient observés dans un modèle de survie sans CLAD, conditionnelle à une survie des patients à 3 et 12 mois post-Tx (p=0,03 and 0,04, respectivement [log-rank test]). La présence d’AC anti-HLA était inversement associée à l’expression de HLA-G dans le greffon (p=0,02). Une augmentation du TGF- β (LBA) était associée à des dosages élevés de sHLA-G (plasma) (p=0,02). En conclusion, l’expression précoce de HLA-G chez les patients greffés pulmonaires en état stable était associée à une acceptation du greffon à long terme.
